Abstract:The formation of a traumatic arteriovenous fistula (AVF) between the extracranial middle meningeal artery (MMA) and the pterygoid plexus (PP) is very rare, and understanding of this condition is limited. This paper reports the case of an 8-year-old who suffered minor injuries after a high fall four months prior to admission and showed good recovery after one month. However, the child gradually developed exophthalmos of the left eye and conjunctival redness one month prior to admission. Auscultation revealed an intracranial murmur near the left side of the face, in the temporal region. A digital subtraction angiography (DSA) showed rupture of the left extracranial MMA and an AVF between the MMA and the PP. The blood drained toward the cavernous sinus, resulting in retrograde blood flow into the ophthalmic vein and the cortical vein. The diagnosis was an AVF between the MMA and the PP, and a combination of coils and Onyx liquid embolic agent was employed to perform AVF embolization. Follow-up six months later indicated no recurrence of the AVF, and the patient showed good recovery with a normal-appearing left eye. The AVF in this case drained toward the cavernous sinus, and symptoms of increased intracranial venous system pressure were apparent, similar to those produced by fistulas between the internal carotid artery and the cavernous sinus. This condition is very rare, and the use of coils in combination with Onyx for AVF embolization is novel, warranting the reporting of the current case.

Abstract:Objective: The aim of this study was to summarize the clinical features of congenital preauricular fistula (CPF) in pediatric patients to improve the levels of diagnosis and treatment, reduce the rates of missed diagnosis and recurrence, and shorten the total diagnosis and treatment time.Methods: A total of 353 patients with CPF who were admitted to the Department of Otolaryngology in The Children's Hospital, Zhejiang University School of Medicine between January 2019 and December 2021 was enrolled in this retrospective observational study. Follow ups for 12-42 months were performed to investigate the classification, surgical methods, and postoperative conditions of CPF and to compare the recurrence rate, complication rate, and total diagnosis and treatment time between the active infection CPF group (AICPFG) and infection-controlled/non-infected CPF group (IC/NICPFG).Results: In 316 cases (89.5%) out of the 353 patients, the natural fistula orifice was located in front of the crus helicis; in 33 cases (9.4%), the natura fistula orifice was located at the crus helicis; and in 4 cases (1.1%), the natura fistula orifice was located in the external acoustic meatus. The AICPFG had 52 cases (14.7%), including 1 case (0.28%) of recurrence and 2 cases (0.56%) of infection at the incision site. The IC/NICPFG had 301 cases (85.3%), including 4 cases (1.13%) of recurrence, 6 cases (1.7%) of infection at the incision site, and 1 case (0.28%) of scar formation at the incision site. There were no significant differences in the recurrence rates and postoperative complications between the AICPFG and IC/NICPFG (p > 0.05). There was a statistically significant difference in the total diagnosis and treatment time between the AICPFG and IC/NICPFG (p < 0.05). Conclusion: A reasonable classification of CPF, use of appropriate surgical methods, and belonging to the AICPFG do not increase the recurrence and complication rates of children but shorten the total treatment course, relieve patients' suffering, reduce treatment costs, and achieve a better clinical prognosis.

Abstract:Objectives/Hypothesis: This study aimed to investigate the diagnosis and management of a distinct developmental deformity syndrome characterized by congenital cheek fistula, ectopic accessory parotid gland, and preauricular appendage. Study Design: Retrospective study. Methods: We analyzed the medical records, radiologic and histopathologic findings, and follow-up data for seven patients (four males) with a congenital cheek salivary fistula. Computed tomography, fistulography, and sialography had been performed for diagnosis. Surgical treatment effect was evaluated. Results: The mean age of the patients was 8.1 years (range, 2-16 years). The distinctive clinical feature was a congenital skin orifice lateral to the commissure with saliva discharge during eating. The cheek fistulae were accompanied by ipsilateral preauricular appendages in all seven patients. The skin orifice connected to an ectopic gland anterior to the masseter and inferior to Stensen's duct. Parotid sialography demonstrated an intact Stensen's duct in all cases. Hypoplasia of the ipsilateral mandible could be observed in five cases. Excision of the ectopic gland, skin orifice, and fistula was performed in five cases resulting in optimal treatment outcomes with no recurrent or adverse events. Conclusions: A congenital saliva-discharging fistula with an ectopic accessory parotid gland, ipsilateral preauricular appendage, and mandibular hypoplasia constitutes a rare developmental syndrome. Surgical excision can effectively treat congenital cheek salivary fistula.

Abstract:A carotid jugular fistula is a direct communication between the carotid artery and the jugular vein. Both congenital and spontaneous internal carotid-jugular fistulas are extremely rare. We describe the first case of successful endovascular treatment for a congenital internal carotid-jugular fistula. We report a 64-year-old woman who presented with a pulsatile mass swelling over the left cervical region and right hemiparesis after cough. Digital subtraction angiography confirmed the diagnosis of left high-flow internal carotid-jugular fistula. The fistula was successfully treated by fractional stent-assisted embolization.

Abstract:Objectives: We report two cases of congenital salivary fistula of an accessory parotid gland, and we discuss its occurrence in Goldenhar syndrome. Methods: Two teenagers complained of a congenital cheek fistula with constant salivary discharge. Computed tomography fistulography and sialography were performed. The diagnosis of Goldenhar syndrome was established based on clinical and imaging findings. Previously reported cases are reviewed and the clinical and radiological features summarised. Results: In these two patients, a salivary fistula of an accessory parotid gland was demonstrated on computed tomography fistulography, and did not communicate with Stensen's duct. Deformity of Stensen's duct and hypoplasia of the ipsilateral mandibular ramus were present. Tragal appendices have frequently been reported in such cases. Conclusion: A congenital cheek salivary fistula of an accessory parotid gland should be considered indicative of Goldenhar syndrome.

Abstract:RATIONALE:A 35-year old Chinese female was admitted to hospital with refractory pericardial effusions 10 days post mitral valve replacement via median sternotomy. We performed an exploratory resternotomy and found lymphatic leakage on the surface of the diaphragm which was continuously emitting a light yellow fluid.PATIENT CONCERNS:The patient complained of no obvious discomfort except for the concern of massive pericardial effusion drainage.DIAGNOSES:Exploratory resternotomy and biochemical testing lead to a supradiaphragmatic lymphatic fistula being diagnosed as the cause of the refractory pericardial effusion.INTERVENTIONS:The fistula was closed with a continuous suture and no other fistulas were found after a thorough exploration.OUTCOMES:The patient was discharged home on postoperative day 5 and recovery was uneventful.LESSONS:In this case a timely exploratory resternotomy proved effective in seeking the cause of and treating pericardial effusion following cardiac surgery.

Abstract:To further document the clinical features of accessory maxilla with three additional cases report. Clinical and radiological features of three cases of accessory maxilla were presented. Related literature was summarized for comparison. Ectopic parotid gland, facial cleft and accessory maxilla are three concomitant malformations in this condition. The tooth-bearing accessory maxillary duplication derives from the abnormal growth of the zygoma or zygomatic arch. Facial cleft, parotid ectopia and tooth-bearing accessory maxilla may constitute a rare congenital syndrome.

Abstract:Rationale:Intraductal papillomas of the accessory parotid glands are extremely rare benign tumors that are most commonly derived from minor salivary glands and are easily misdiagnosed as other diseases. Studying these lesions by pathology and immunohistochemistry can raise awareness of the disease, reduce the rate of misdiagnosis, and provide more precise treatments.Patient concerns:A 35-year-old man first presented to our hospital with a 6-month history of a painless mass on his left parotid gland.Diagnoses:The patient was diagnosed with intraductal papilloma of the accessory parotid gland by pathology and immunohistochemistry.Interventions:The mass was completely resected.Outcomes:After 2 years of postoperative follow-up, the patient recovered well without recurrence.Lessons:Intraductal papilloma of the accessory parotid gland is very rare, and can easily be misdiagnosed as sialadenoma papilliferum, inverted ductal papilloma, or papillary cystadenoma, among others. It is necessary to analyze its pathology and immunohistochemistry in comparison with other diseases. Early excision and long-term follow-up are necessary to provide optimal treatment and to better understand the pathological processes of intraductal papilloma.
Congenital mature cystic teratoma of the parotid gland in an infant: Report of a unique case

Abstract:Introduction and importance: Teratoma is a germ cell tumor originating from pluripotent germ cells and embryonal cells that commonly occurs in the gonads with only 15 % of it arising in extragonadal sites. In infants and children, teratomas of the head and neck are uncommon that comprise 0.47 %-6 % of all teratomas, and their occurrence in parotid gland is extremely rare. It is considered a diagnostic pitfall preoperatively, and their definite diagnosis can only be made upon surgery followed by histopathological examination. Case presentation: We present a unique case of parotid gland teratoma in a 9-month-old girl who was brought to the hospital by her parents with right side parotid region swelling since birth. The ultrasonographic findings were suggestive of cystic hygroma. Upon surgery, the mass was completely excised with a part of parotid gland. The diagnosis of mature teratoma was made based on the histopathologic examination. No tumor recurrence was noted during the 4-month postoperative follow-up. Clinical discussion: Teratoma of the parotid gland is an extremely rare entity that may mimic diverse benign and malignant tumors of the salivary gland. Patients often present to the health care facility with a parotid gland swelling leading to defacement. Complete surgical resection of the tumor is considered the best treatment approach with careful preservation of facial nerve. Conclusion: Due to the scarcity of information available regarding the behavior and clinical management of parotid gland teratoma in the literature, a good follow-up of patient is required to exclude potential recurrency and neurological deficit.
A rare entity in salivary gland pathology: A case report of an epidermoid cyst in the parotid gland

Abstract:Introduction and importance: Cystic lesions of the parotid gland are uncommon, comprising approximately 5 % of all salivary gland tumours. Among these, epidermoid cysts are particularly rare. These benign developmental cysts typically manifest in the skin of the scalp, face, and neck, but are rarely observed in the parotid gland. It is of paramount importance to be able to differentiate these lesions from malignancies, given the similarities in their clinical presentations. Case presentation: We present a case of a 12-year-old boy who experienced a gradual, painless swelling in the right parotid region over the course of a year. Clinical discussion: A physical examination revealed the presence of a 3 cm cystic mass in the parotid gland. Magnetic Imaging Resonance (MRI) revealed a well-defined, multilobulated cystic formation with characteristics consistent with a non-neoplastic cyst. The surgical procedure involved a superficial parotidectomy, which resulted in the successful removal of the cyst and no postoperative complications. Conclusion: Epidermoid cysts in the parotid gland are uncommon but can be effectively managed with precise diagnostic and surgical techniques. It is imperative that accurate preoperative imaging and histopathological examination are conducted in order to distinguish these benign cysts from malignant tumours, thereby ensuring the appropriate treatment and favourable outcomes.
Primary Aorto-Appendicular Fistula Accompanied by Infected Penetrating Aortic Ulcer - Case Report

Abstract:Primary aorto-appendicular fistulas are extremely rare in clinical practice, and there are only 3 such cases reported in the available literature. The aim of this study is to present the case of a 68-year-old female patient with primary aorto-appendicular fistula accompanied by infected penetrating aortic ulcer (PAU). At the time of her hospital admission, the patient did not demonstrate any typical symptoms. Multidetector computed tomography imaging of aorta was performed during an additional preoperative examination and the PAU of the infrarenal aortic segment was detected. Other pathological changes were not found. The intraoperative findings confirmed the presence of the PAU firmly attached to the appendix and surrounding signs of inflammation. Appendectomy and partial resection of the PAU were performed, along with aortic reconstruction, using a Dacron tube graft and right renal artery reattachment. During the postoperative period, there were no complications, and the patient was discharged on the 10th postoperative day. Control medical examination and color duplex ultrasonography were performed 18 months after the surgery and they provided satisfactory results.
Lingual dermoid cyst with congenital discharging sinus: a rare entity.

Abstract:Objective: There have been few reports of lingual dermoid cysts with a congenital discharging fistula. We report such a case, which mimicked lipoma on computed tomography, and we discuss relevant clinical and radiological features. Method: We present a case report and a review of the English language literature concerning lingual dermoid cysts either mimicking lipoma or with a congenital discharging sinus. Results: A 35-year-old man presented with a lingual mass associated with a congenital fistula in the midline of the tongue dorsum, which discharged abnormal secretions. Computed tomography images showed that the lesion was well defined and contained homogeneous fatty tissue giving computed tomography attenuation values of -120 Hounsfield units. Conclusion: Lingual dermoid cysts may develop with a midline discharging fistula, and may exhibit radiological similarity to lipomas on computed tomography.

Abstract:Background Coronary artery-to-pulmonary artery fistula is a rare disorder characterized by abnormal vascular communication between the coronary artery and pulmonary artery. While most patients remain asymptomatic, some might exhibit symptoms of myocardial ischemia, congestive heart failure, or even sudden cardiac death if coronary aneurysm, thrombosis, infective carditis, or other congenital cardiac defects coexist. Case presentation We present a 66-year-old male complaining of angina pectoris with a history of hypertension and active smoking. He was diagnosed with a coronary aneurysm based on coronary computed tomography angiography. We subsequently identified a coronary artery-to-pulmonary artery fistula with giant aneurysmal dilation on coronary angiography. Ultimately we conducted surgery ligation and aneurysmorrhaphy. During surgery, we discovered newly formed thrombus within the aneurysmal cavity. Histological analysis of the aneurysmal wall supported the diagnosis of the congenital disorder. Our patient was successfully discharged and remained asymptomatic at two months of follow-up. Conclusion We presented a rare and complex combination of congenital coronary artery-to pulmonary artery fistula, giant coronary aneurysmal dilatation, and thrombosis through multi-modality evaluations.
First branchial cleft cyst accompanied by external auditory canal atresia and middle ear malformation: A case report

Abstract:Background: We report a rare case of first branchial cleft anomaly (FBCA) accompanied by bony atresia of the external auditory canal, middle ear malformation, and location malformation of the facial nerve according to the intraoperative findings. Case summary: A 19-year-old male patient presented to our department with a mass behind the right earlobe and recurrent postauricular swelling and pain since childhood, he also had severe hearing loss in the right ear since birth. The patient underwent surgery including mass removal, mastoidectomy, and simultaneous meatoplasty and ossiculoplasty under microscopy. No facial palsy or recurrence was noted during postoperative follow-up. Conclusion: FBCAs are rare, and to our knowledge, this is the first report of FBCA accompanied by external auditory canal bony atresia, middle ear malformation, and location malformation of the facial nerve. An effective postauricular approach under microscopy facilitated complete lesion removal and simultaneous otologic reconstruction.

Comparative Analysis of Fistula Development After Parotid Gland Surgery: Lateral Parotidectomy Versus Extracapsular Dissection Technique

Abstract:Purpose: The primary objective of this study was to explore the potential disparity in postoperative fistula occurrence rates between patients who undergo (partial) lateral parotidectomy and those who undergo the extracapsular dissection technique for the management of benign parotid gland tumours. Methods: A consecutive series of 363 patients treated with (partial) lateral parotidectomy and extracapsular dissection technique for benign parotid gland tumours at one tertiary centre between 2018 and 2022 were included. To evaluate the impact of the surgical technique and possible other risk factors (tumour location, tumour size, Body Mass Index, age, smoking, diabetes mellitus, arterial hypertension) for the development of fistulas, multivariate logistic regression analyses using backward lection were applied to estimate odds ratios (ORs) and 95%-confidence intervals (CIs). Results: In 363 patients, 21 patients (5.8%) developed a fistula. Patients who underwent (partial) lateral parotidectomy had three times higher chance of developing a fistula compared to patients who were operated using the extracapsular dissection technique (ORadjusted = 2.6, 4.1% vs. 12.5%, p = 0.044). In the multivariate analyses, no other risk factors for the development of fistulas were statistically significant in this cohort. The incidence of facial nerve paralysis was not significantly different between the extracapsular dissection and lateral parotidectomy group (5/73 = 6.8% vs. 11/290 = 3.8%, p = 0.333). Conclusion: Fistulas occur more often in patients treated by means of a (partial) lateral parotidectomy approach compared to patients treated using the extracapsular dissection technique. Therefore, surgeons should be vigilant about postoperative fistula risks in lateral parotidectomy and consider preventive measures.
Leiomyomatous Hamartoma of Incisive Papilla with High Frenal Attachment: A Case Report

Abstract:Aim: This case report aims to describe a rare congenital lesion of the incisive papilla with a high labial frenulum attachment, clinically mimicking congenital epulis but histopathologically diagnosed as an oral leiomyomatous hamartoma. Background: Oral leiomyomatous hamartoma is a very rare congenital lesion, mainly appearing on the median anterior maxilla/incisive papilla and tongue. Case description: This clinical paper is about a rare lesion in a 6-year-old female child whose parents reported to the department with the complaint of slow-growing soft tissue overgrowth between the front teeth of the upper jaw, present since birth. The soft tissue growth is now causing difficulty in biting food and is visible during smiling and speaking, causing an esthetically unpleasing appearance. The clinical examination also revealed a high labial frenulum attached to the lesion. The lesion was provisionally diagnosed as congenital epulis based on the clinical picture. However, after excisional biopsy and histologic evaluation with special stains, the lesion was finally diagnosed as leiomyomatous hamartoma. Conclusion: Surgical excision of the lesion followed by frenectomy was performed with no postoperative complications. Clinical significance: Owing to the rare occurrence and nature of mimicking congenital epulis, it is important for a dental practitioner to have knowledge about these types of lesions. The final diagnosis of such lesions can only be made after histopathological evaluation using special stains. This case report describes the clinical and histopathological features of a rare leiomyomatous hamartoma of the incisive papilla, along with high frenulum attachment and its management. Abstract:Cheek swelling can be attributed to several pathologies, including masseteric hypertrophy, diffuse inflammatory changes and neoplasia.We report an extremely rare case of bilateral cheek swelling as a result of ectopic parotid glands.This case is a young female patient with bilateral ectopic parotid glands superficial to the masseter muscle and the zygomatic arch, demonstrated by the enhanced computed tomography (CT).Medical history, clinical features, videography and management of this case are described.After two years of observation, no significant change in symptoms was observed on this patient.Besides, we conducted a case report and systematic review of cases of ectopic parotid gland.A literature search was performed using PubMed, Web of Science, and Ovid electronic database.A total of 144 papers were retrieved and only one paper was included in the systematic review.In conclusion, bilateral ectopic parotid gland is extremely rare and easily confused with other lumps in the area of head and neck.CT, magnetic resonance imaging (MRI), ultrasound imaging and parotid sialography allow for noninvasive diagnosis of ectopic parotid gland.If the parotid ectopic is highly suspected and the patient does not have obvious symptoms, conservative treatment and long-term observation follow-up are recommended.
