Abstract:OBJECTIVE:The aim of this study was to evaluate strain distribution in peri-implant bone, stress in the abutments and denture stability of mandibular overdentures anchored by different numbers of implants under different loading conditions, through three-dimensional finite element analysis (3D FEA).METHODS:Four 3D finite element models of mandibular overdentures were established, using between one and four Straumann implants with Locator attachments. Three types of load were applied to the overdenture in each model: 100N vertical and inclined loads on the left first molar and a 100N vertical load on the lower incisors. The biomechanical behaviours of peri-implant bone, implants, abutments and overdentures were recorded.RESULTS:Under vertical load on the lower incisors, the single-implant overdenture rotated over the implant from side to side, and no obvious increase of strain was found in peri-implant bone. Under the same loading conditions, the two-implant-retained overdenture showed more apparent rotation around the fulcrum line passing through the two implants, and the maximum equivalent stress in the abutments was higher than in the other models. In the three-implant-supported overdenture, no strain concentration was found in cortical bone around the middle implant under three loading conditions.CONCLUSIONS AND CLINICAL SIGNIFICANCE:Single-implant-retained mandibular overdentures do not show damaging strain concentration in the bone around the only implant and may be a cost-effective treatment option for edentulous patients. A third implant can be placed between the original two when patients rehabilitated by two-implant overdentures report constant and obvious denture rotation around the fulcrum line.
Mandibular overdentures retained by 1 or 2 implants: a 5-year randomized clinical trial on implant stability and peri-implant outcomes

Abstract:Aim: This is a report of the 5-year results of a two-group parallel randomized clinical trial comparing longitudinal implant stability, and clinical and radiographic peri-implant outcomes of mandibular overdentures retained by one (1-IOD group) or two (2-IOD group) implants. Methods: All participants received 4.1 mm diameter tissue-level implants (Straumann® Standard Plus - SLActive®, Institut Straumann AG), installed in the mandible midline (1-IOD; n = 23) or the lateral incisor-canine area bilaterally (2-IOD; n = 24), and loaded after 3 weeks. Implant Stability Quotient (ISQ) was measured using a resonance frequency device (Osstell® Mentor, Integration Diagnostics) at implant placement, after three weeks (loading), and at the 6-month, 1-, 3-, and 5-year follow-ups. Marginal bone loss and clinical implant outcomes (plaque, calculus, suppuration and bleeding) were assessed periodically up to 5 years after loading. Results: Only minor changes in marginal bone level were observed after 5 years (mean = 0.37; SD = 0.44 mm), and satisfactory and stable peri-implant parameters were observed throughout the 5-year follow-up. No significant differences between groups were found. Overall, the mean primary implant stability was considered high (> 70) for the two groups (1-IOD = 78.1 ± 4.5; 2-IOD = 78.0 ± 5.8). No noticeable changes were observed between implant insertion and loading. A marked increase was observed from insertion to the 6-month follow-up - the mean difference for the 1-IOD group was + 5.5 ± 5.5 (Effect size = 1.00), while for the 2-IOD group, the mean difference was + 6.0 ± 5.6 (Effect size = 1.08). No relevant changes were observed throughout the follow-up periods up to 5 years. Linear mixed-effect model regression showed no influence of the bone-related variables (p > 0.05) and the number of implants (p = 0.087), and a significant effect of the time variable (p < 0.001). Conclusion: Satisfactory peri-implant outcomes and stable secondary stability suggest good clinical performance and successful long-term osseointegration of the implants for single and two-implant mandibular overdentures. Using a single implant to retain a mandibular overdenture does not seem to result in detrimental implant loading over the five years of overdenture use. Clinical relevance: This study corroborates the use of a single implant to retain a mandibular denture.

Abstract:This report of four clinical cases aims to illustrate the use of a lateral implant as a solution for implant overdentures in the mandible in different clinical situations. The first two cases describe the clinical situations of patients wearing two‐implant mandibular overdentures until the failure of one of the implants, one due to implant loss and the other due to a fracture of an abutment screw, and how the conditions were managed. The third case illustrates the placement of a single implant to retain an overdenture, where a midline implant, as originally planned, was not feasible due to anatomic reasons. The final case describes the use of a lateral implant to support and retain a single‐implant mandibular overdenture. The four cases demonstrate that a single lateral implant can be utilized as sole retention in cases of a failing contra‐lateral implant and as an alternative to a single implant in the midline.

Abstract:Aim: The aim of this within-subject prospective clinical study was to investigate the scope of single implant mandibular overdenture by assessing its masticatory efficiency, clinical performance, and patient satisfaction compared to conventional complete dentures. Settings and Design: Prospective In Vivo Study. Material and Methods: This prospective in vivo study was conducted in the Department of Prosthodontics and Crown and Bridge, Christian Dental College, Ludhiana. A total of 12 completely edentulous patients received a single implant in the mandibular anterior midline region. After the healing period, the conventional maxillary and mandibular dentures were fabricated. 15 days post insertion of the conventional dentures, patients were evaluated for masticatory efficiency, clinical performance including retention and stability, and patient satisfaction. To evaluate the masticatory efficiency blue raspberry and original pink “Hubba Bubba tape gum” were used as a test food. Colorimetric analysis was done to assess variance of hue. To assess clinical performance, retention, and stability of the mandibular denture was recorded using a digital force gauge and was tabulated as per CU-modified Kapur's criteria. OHIP-14 index was used to assess patient satisfaction. After evaluation of the parameters of conventional dentures, the denture was converted into an implant-retained mandibular denture by chairside conversion with locator attachments. 15 days post-implant loading, parameters of the implant retained mandibular overdenture were assessed again followed by statistical analysis. Statistical Analysis Used: The masticatory efficiency was assessed using a paired t -test. The patient satisfaction was sequentially assessed with Wilcoxon signed rank test and thereafter paired t -test was used to compare between conventional complete denture and overdenture. Clinical performance was assessed using the Wilcoxon signed rank test. Results: The masticatory efficiency of single implant mandibular overdenture was higher than that of conventional complete dentures. The clinical performance of the overdenture was higher than that of the conventional denture. Stability being a time-dependent parameter might need longer follow-ups for further conclusions. Patient satisfaction with single implant retained overdenture was significantly higher than conventional complete dentures. Conclusion: Single implant mandibular overdenture has improved masticatory efficiency, clinical performance, and patient satisfaction compared to conventional complete dentures.
[Preliminary Clinical Study of Microimplants in the Restoration of Edentulous Jaws].

Abstract:OBJECTIVE:To investigate if magnetic- attached overdenture supported by microimplants of 2.5 mm in diameter was able to function properly.METHODS:Fourteen microimplants were placed in 4 patients. Implants were exposed 3 months later and magnetic abutments were connected directly. X-ray was taken during the 3 year follow-up to examine marginal bone level. Peri-implant soft tissue was also checked. Denture retention, chewing stability and overall satisfaction with the microimplants retained overdenture were evaluated.RESULTS:All of the 14 microimplants achieved initial primary stability without infection or any other postoperative complications. No implant was lost during the follow-up period. Only one magnetic abutment fractured one year after restoration and was replaced. X-ray showed highly stable marginal bone level around the microimplants. The patients were satisfied with the restoration.CONCLUSIONS:The microimplants retained overdenture leads to a significant improvement of function despite of the small dimensions of these implants. In particular for elder patients with severe residual ridge resorption, this treatment represents a less invasive alternative compared to conventional implant systems.

Abstract:The positive impact of implant interventions on dental patient-reported outcomes is an essential parameter of treatment effectiveness. This study assessed the 2-year changes in patient satisfaction and oral health-related quality of life (OHRQoL) of edentulous patients treated with a four mini implant mandibular overdenture (IOD) METHODS: The study was planned as a 2 × 2 factorial randomized clinical trial that tested two surgical approaches (flapped or flapless) and two loading protocols (immediate and delayed) using a titanium-zirconium mini implant (Straumann Mini Implant System®) and a PEEK retentive system (Straumann® Optiloc® Retentive System). Outcome measures (OHIP-Edent scores and the McGill Denture Satisfaction questionnaire) were assessed before treatment and at the 3-, 6-, 12-, and 24-month follow-ups. The Friedman test and multiple regression using Generalized Estimating Equations (GEE) were used for data analysis, considering the per-protocol (PP) and intention-to-treat (ITT) approaches RESULTS: Seventy-four patients were randomized to the study groups. No implant failure occurred during the study period. Marked improvement in all post-treatment periods compared to baseline were observed for the two outcomes. No significant effect of patient's gender, age, and surgical protocol on the study outcomes. The effect of treatment provision was significant for the two outcomes in the PP and ITT approaches (p < 0.001). A barely significant positive effect of the immediate loading was observed for OHIP-Edent in the PP approach (p = 0.020) CONCLUSION: IOD treatment significantly improved patient-reported outcomes measures, with sustained benefits over the two years of overdenture use, and can be considered a promising treatment option in for the edentulous mandible.
Stress distribution of single-implant-retained overdenture reinforced with a framework: A finite element analysis study

Abstract:Statement of problem: Studies have demonstrated the effectiveness of a single-implant-retained mandibular overdenture for elderly patients with edentulism. However, due to the high concentration of stress around the housing portion of the single implant, this prosthesis tends to fracture at the anterior region more than the 2-implant-retained mandibular overdenture. Purpose: The purpose of this finite-element analysis study was to evaluate the stress distribution in a single-implant-retained mandibular overdenture reinforced with a cobalt-chromium framework, to minimize the incidence of denture base fracture. Material and methods: Two 3-dimensional finite element models of mandibular overdentures supported by a single implant with a stud attachment were designed in SolidWorks 2013 software. The only difference between the models was the presence or absence of a cobalt-chromium framework at the denture base between canines. Subsequently, the models were imported into the mathematical analysis software ANSYS Workbench v15.0. A mesh was generated with an element size of 0.7 mm and submitted to convergence analysis before mechanical simulation. All materials were considered to be homogeneous, isotropic, and linearly elastic. A 100-N load was applied to the incisal edge of the central mandibular incisors at a 30-degree angle. Maximum principal stress was calculated for the overdenture, von Mises stress was calculated for the attachment and implant, and minimum principal stress was calculated for cortical and cancellous bone. Results: In both models, peak stress on the overdenture was localized at the anterior intaglio surface region around the implant. However, the presence of the framework reduced the stress by almost 62% compared with the overdenture without a framework (8.7 MPa and 22.8 MPa, respectively). Both models exhibited similar stress values in the attachment, implant, and bone. Conclusions: A metal framework reinforcement for a single-implant-retained mandibular overdenture concentrates less stress through the anterior area of the prosthesis and could minimize the incidence of fracture.
Single-implant overdentures retained by the Novaloc attachment system: study protocol for a mixed-methods randomized cross-over trial

Abstract:Background: Overdentures retained by a single implant in the midline have arisen as a minimal implant treatment for edentulous mandibles. The success of this treatment depends on the performance of a single stud attachment that is susceptible to wear-related retention loss. Recently developed biomaterials used in attachments may result in better performance of the overdentures, offering minimal retention loss and greater patient satisfaction. These biomaterials include resistant polymeric matrixes and amorphous diamond-like carbon applied on metallic components. The objective of this explanatory mixed-methods study is to compare Novaloc, a novel attachment system with such characteristics, to a traditional alternative for single implants in the mandible of edentate elderly patients. Methods/design: We will carry out a randomized cross-over clinical trial comparing Novaloc attachments to Locators for single-implant mandibular overdentures in edentate elderly individuals. Participants will be followed for three months with each attachment type; patient-based, clinical, and economic outcomes will be gathered. A sample of 26 participants is estimated to be required to detect clinically relevant differences in terms of the primary outcome (patient ratings of general satisfaction). Participants will choose which attachment they wish to keep, then be interviewed about their experiences and preferences with a single implant prosthesis and with the two attachments. Data from the quantitative and qualitative assessments will be integrated through a mixed-methods explanatory strategy. A last quantitative assessment will take place after 12 months with the preferred attachment; this latter assessment will enable measurement of the attachments' long-term wear and maintenance requirements. Discussion: Our results will lead to evidence-based recommendations regarding these systems, guiding providers and patients when making decisions on which attachment systems and implant numbers will be most appropriate for individual cases. The recommendation of a specific attachment for elderly edentulous patients may combine positive outcomes from patient perspectives with low cost, good maintenance, and minimal invasiveness. Maxillary Four Implant‐retained Overdentures Via Locator® Attachment: Intermediate‐term Results from a Retrospective Study

Abstract:Background: Maxillary overdentures have been utilized to restore oral function in scenarios where limited bone structure represents a drawback for financial issues and for cases where oral hygiene is uncertain.Purpose: The aim of this study was to evaluate the intermediate-term clinical outcome of four locator attachment retaining maxillary overdentures and to test their reliability.Materials and Methods: A retrospective study was conducted from January 2007 to October 2013. After the placement of four maxillary dental implants and a healing period of approximately 3 months, all the implants were restored with locator-retained overdentures with partial palatal coverage. Subjects and implant characteristics, implant position (anterior vs posterior maxilla), bone quality, and opposing dentition were recorded. Peri-implant clinical parameters and marginal bone loss (MBL) were evaluated after delivery of the final prosthesis and annually thereafter. The performance of locator components and the prosthetic restoration were also recorded at follow-up.Results: A total of 104 implants were placed in 26 subjects (11 women, 15 men; mean age, 64 years; age range, 55-76 years). One implant failed before loading. One subject dropped out of the study during a mean follow-up period of 46 months (range: 7-73 months), achieving an overall survival rate of 95.2%. Clinical parameters revealed healthy tissues around most of the implants, with low scores of plaque and bleeding indices. At the last follow-up evaluation, the mean MBL was 1.7 +/- 1.1 mm (0.4-2.6 mm). Regression model analysis indicated that MBL for implants in the posterior maxilla was significantly higher than that for implants in the anterior maxilla (p = .0487). Overall, 26 technical and prosthetic complications were reported.Conclusion: Within the limitations of this study, maxillary four implant-retained overdentures via Locator attachment seem to be a predictable alternative for oral rehabilitation.
Implant-Supported Milled-Bar Overdenture:Laboratory Technique and Preliminary Clinical Research

Abstract:Objective:To evaluate the feasibility of the laboratory technique of implant supported milled-bar overdenture. Method: 28 cases with edentulous jaws underwent milled-bar overdenture restoration. 168 implants were placed. 12 cases were male and 16 were female. The age range was between 22 and 71. The follow-up time was from 12 months to 39 months. Clinical examination and x ray film was conducted. Results: No implant was lost during the loading time. All prostheses were stable. All the 28 patients expressed satisfaction with regard to the implant-supported milled-bar prostheses in terms of sufficient function and improved facial esthetics. Peri-implant soft tissue inflammation was observed in five cases due to bad oral hygiene. 2-4 mm bone resorption around 8 implants was observed in one case. The overdenture was remade in one case with magnet abutment after treatment of periimplantitis. The peri-implant bone level was stable around 160 implants . The resin flange was broken after loading one year in two cases and another two after loading four years and repaired without any further problems. Conclusion:The implant-supported milled-bar overdenture could provide an excellent result to the edentulous patients. But the laboratory technique is highly demanding.
Mandibular overdenture retained by four one-piece titanium-zirconium mini implants: A 2-year follow-up on prosthodontic outcomes

Abstract:Objective This 2-year prospective study reports the incidence of prosthetic complications and maintenance events after treatment with mandibular overdenture retained by four mini implants opposed to a maxillary denture. Methods Implant intervention included flapless or flapped surgery combined with immediate or delayed loading, as part of a randomized clinical trial. Four one-piece titanium-zirconium mini-implants (Straumann Mini Implant System) were inserted, and the retentive PEEK elements (Optiloc) were incorporated into the overdenture using chairside procedures. Prosthodontic complications and maintenance events were recorded over a 2-year follow-up, and the final outcome was defined according to standardized criteria. Data analysis included descriptive statistics, incidence and incidence density rates, and Kaplan-Meier survival. Results 73 out of 74 patients (64.9 % female), mean age of 64 (SD=8.2) years, completed the study follow-up (one withdrew after 9 months). Implant survival was 100 %. A total of 163 prosthodontic events occurred in 53 patients (72.6 %), and 20 patients had no clinical complaints or maintenance needs. The most common procedures were adjustment/repair of the overdenture base (47.0 %), replacement of retentive inserts (19.8 %), and laboratory relining (12.9 %). A high prosthodontic success rate was achieved (93.2 %), and all patients presented serviceable overdentures and continuous use after the resolution of prosthodontic complications. The incidences of matrix replacement and laboratory relines were low in the first year, while denture base adjustments were common within the first year, especially in the first 6 months. Conclusion The mini implant system showed high prosthodontic success rates. Minor adjustments/repairs during the initial follow-up were common. Relines and matrix replacements tend to occur after one year of overdenture use, and matrix replacements may occur as a consequence of the need for relining.

Evaluation of the Therapeutic Efficiency of Mandibular Anterior Implant-Supported Fixed Bridges with Cantilevers

Abstract:Background Dental implant technology has developed rapidly in recent years. However, the use of implant-supported fixed bridges with cantilevers has been controversial. The purpose of this study was to evaluate the clinical results of the mandibular anterior implant-supported fixed bridges with a cantilever.Method Thirty-three patients (15 males, 18 females; mean age, 42.6 years; range 20-54 years) with two missing anterior mandibular teeth had single implant-supported fixed bridges with a cantilever. Clinical examination was recorded and radiographs were taken. The mean duration of follow-up was 30 months (15-44 months).Results All implants survived. Loosening or fracture of the prosthesis was not observed. All patients were satisfied with the treatment. The mean bone resorption values after 12, 24, and 36 months of implant loading were 0.94, 1.18 and 1.35 mm respectively. The changes of gingival papilla height ranged from 0 to 0.5 mm. There was significant difference between 1-year and 2 or 3 years restoration groups regarding the average gingival height changes (P <0.05).Conclusion After careful and precise selection of patients, restoration with a single implant-supported fixed bridge with a cantilever can be recommended if two anterior mandibular teeth are missing.
Immediate loading of four freestanding implants retaining a mandibular overdenture: 3-5 years follow-up study

Abstract:Objective To evaluate the clinical results of immediate loading of four freestanding implants retaining a mandibular overdenture.Methods Ten patients were treated with implants retaining a mandibular overdenture.All 40 implants had osseointegmtion.The treatment method was based on immediate loading of four interforaminal implants without the use of a bar retainer.The denture was placed on and retained by prefabricated conical crowns that were inserted into the existing denture base by direct intraoral polymerization immediately after surgery and supported by the corresponding conical primary implant abutments.Clinical examination and radiographs were conducted.Changes in the marginal bone level around the implants were evaluated with radiograph.The mean follow-up time was 60 months(range 24-60 months).Results The cumulative implant survival rate was 100％.All implants presented healthy peri-implant soft tissue conditions showing low value of clinical parameters.Average marginal bone level change after 5 years of function was (0.52 ± 0.49) mm.The patients were satisfied with the outcomes.Conclusions The preliminary clinical results of this clinical study suggest that immediate loading of four unsplinted implants by conical crowns to support overdenture for edentulous mandible is predictable.
Repercussion of Conventional Complete Mandibular Denture Versus Single Implant Over-Denture on Retention and Biting Force

Abstract:Aim: Aim of this study was the evaluate the retention and biting force of conventional complete denture and after placement of a single implant in the mandible for an implant retained over-denture Material and methods Eight completely edentulous patients were selected. A single implant was inserted at the symphysis of the mandible and left to osseointergrate. During the osseointegration period a conventional complete denture was fabricated and inserted. Retention and biting force was measured at insertion and after 3 months of service. After osseointegration attachment was connected, space was formed in the fitting surface of the existing mandibular denture and retention silicon was placed. Retention and biting force were measured at insertion and after 3 months follow-up. Data were collected and statistically analyzed. Results: The retention mean values for the complete denture was 2.420±0.360, however the retention mean values for the single implant over-denture was 6.395±0.289. F=3.80 with statically significance difference between the groups of P 0.01 Conclusion: Single implant overdenture improved the retention and the biting force when compared with the complete denture, this has improved the quality of life and happiness.

Abstract:OBJECTIVE:To investigate clinical feasibility and technical features of immediate loading with linear occlusion on 2-implant-supported overdenture and to evaluate short-term effect of the treatmentMETHODS:Six edentulous patients with severe residual ridge resorption were enrolled. Two interforaminal implants were inserted for each patient and then immediate impressions were taken. Implant-supported bar-retained overdentures were restored for the patients within 24 hours. Clinical and radiographic examinations were conducted post-operative 1 week and 1, 3, 6, and 12 months. Thereafter at every 6 months the stability of implants, tissue situations around implants, radiographs and satisfaction level of patients were examined for each patient.RESULTS:Six cases and 12 implants were followed from 9 to 30 months. No implant was loosened or dropped. Sulculus bleeding index was 0-1 and probing depth of sulcus was less than 2 mm. The gingival tissues around the implants were healthy. Radiographic examination showed that bone resorption was less than 1 mm in the first year. The alveolar bone around the implants hasn't show obvious resorption stable height. Patients were satisfied with the implants.CONCLUSIONS:The results of this study indicate that immediate loading using linear occlusion on implant-supported bar-retained overdenture is predictable in some cases and can achieve satisfaction in the short-term service. Further study is needed to assess the long-term results.
Complete Removable Denture Retained by a Symphyseal Single Implant: A Case Report

Abstract:Complete denture is a real challenge for any practitioner. The aim of implantology is to optimize prosthetic balance and ensure that prostheses are perfectly integrated from a bio-functional viewpoint, despite an unfavorable anatomophysiological context. Here, we present a case managed in our department concerning a mandibular complete removable denture retained by a medial symphyseal implant. A 61-year-old fully edentulous mandibular patient with a Kennedy Applegate class I edentulous maxilla in good general health consulted for the renewal of his mandibular complete denture, which was deemed unstable and non-retentive. The exo-oral examination was normal. The mandibular crest was heavily resorbed, especially posteriorly, and covered with slightly inflammatory fibromucosa. The patient refused bone augmentation surgery. We performed a piezographic mandibular prosthesis retained by a single standard implant at the level of the mandibular symphysis. The prosthesis was stable and retentive. Masticatory comfort and efficiency were satisfactory and the psychological integration of the prosthesis was improved. Several studies have shown that a single symphyseal implant is a therapeutic alternative that completes the therapeutic range in specific cases. It should be limited to the mandibular arch in elderly patients with reduced bone volume. Given the lack of randomized controlled trials, routine use of this new approach is not recommended, and further studies are required.

Abstract:OBJECTIVE:To evaluate the clinical effects of implants supported magnet-retained overdenture.METHODS:From November 1999 to March 2005, 25 cases with edentulous jaws underwent implant-supported magnet-retained overdenture. Among them, 14 patients were male, 11 patients were female. The average age of the patients was 67.6 years. (Range 45 - 79 years). Ninety-five implants used included Komet (18), IMZ (11), Frialit-2 (12), Ankylos (10), Camlog (44). The fellow-up time was from 6 months to 70 months. Clinical examination and radiographs were conducted.RESULTS:No infections, nerve or sinus damage or other sequelae occurred. The overdentures were stable and functioned effectively. From November 1999 to March 2005, One Komet abutment was fracture and 1 Frialit-2 implant was lost because of overloading during follow-up. The remaining implants achieved successful osseointegration. Patients were satisfied with the treatment.CONCLUSIONS:Implant-supported magnet-retained overdenture was a predictable and reliable method, especially for old patients with edentulous jaws.
A 3-D Finite Element Analysis of a Single Implant Retained Overdenture Reinforced with Short Versus Long Frameworks

Abstract:Due to its simplicity and patient satisfaction, an implant-retained overdenture has become the most preferred treatment for edentulous patients. Due to the presence of an attachment system at mandibular anterior region, however, base fracture is the most common complication of overdenture. This study aimed to evaluate the stress distribution and deformation on a symphyseal single implant retained mandibular overdenture using a three-dimensional finite element. Zirconia versus acrylic overdenture was investigated. Acrylic overdentures reinforced with short (over inter-canine distance) or long (extending between first molars) zirconia, cobalt-chromium alloy or polyetherketoneketone framework were also investigated. A load of 100 N was applied to the incisal edge of mandibular central incisors at a 30º angle. Results showed that zirconia overdenture had lower von Mises stress and deformation in its components than acrylic. Reinforcement of an acrylic overdenture with cobalt chromium or zirconia short frameworks reduced von Mises stress and deformation on its components. Reinforcement of an acrylic overdenture with polyetherketoneketone framework did not show any significant reduction in von Mises stress and deformation. Therefore, it can be concluded that using zirconia overdenture or reinforcing an acrylic overdenture with cobalt chromium or zirconia framework could increase the longevity of the prosthesis.

Abstract:Objectives The study compares three minimally invasive approaches for the retention of implant supported mandibular complete dentures, particularly focusing on patient satisfaction. Background The McGill Consensus Statement recommends restoration of the edentulous mandible with an overdenture retained on two implants. Alternatively, less invasive treatment concepts with shorter treatment times have been developed for critical cases. Material and Methods Thirty‐nine patients (with a total of 78 implants) with advanced mandibular bone atrophy were randomly assigned to three groups: "single standard implant‐retained overdentures" (SSO) and "mini‐implant‐retained overdentures" (MO), which was further subdivided into "two mini‐implant‐retained overdentures" (TMO) and "four mini‐implant‐retained overdentures" (FMO). The technical and biological parameters and oral health‐related quality of life were evaluated over a 10‐year period. Data were analysed for group comparisons and longitudinal trend analysis. Results Sixteen patients (42%) dropped out during the study period. At the time of follow‐up, 98.4% of the implants were in situ. The first need for technical intervention occurred after 3.8 ± 1.1, 4.2 ± 0.9, and 4.6 ± 1.3 years in the TMO, SSO, and FMO groups, respectively. Attachment exchange (39%) was the most frequently performed intervention in all groups. Healthy peri‐implant and mucosal conditions were observed in 74% and 40% of patients after 1 and 10 years, respectively. The OHIP‐G14 score was 22.6 before implantation, 7.6 at 1 year (effect size [ES]: 1.1), and 5.4 at 10 years (ES: 2.3). Conclusion Irrespective of the minimal concept selected, complete mandibular dentures retained on implants improved the subjective perception of the quality of life. Application of these alternative minimal concepts may be practical in clinical practice.
