Deaths were identified from state vital statistic records, the National Death Index, family reports, and the postal system. We documented 2025 deaths during 6 years of follow-up (2004-2010). The multivariable-adjusted hazard ratios of total mortality were 1.25 (95% confidence interval [CI], 1.04-1.50) for difficulty initiating sleep, 1.09 (95% CI, 0.97-1.24) for difficulty maintaining sleep, 1.04 (95% CI, 0.88-1.22) for early-morning awakenings, and 1.24 (95% CI, 1.05-1.46) for nonrestorative sleep, comparing men with those symptoms most of the time with men without those symptoms, after adjusting for age, lifestyle factors, and presence of common chronic conditions. Men with difficulty initiating sleep and nonrestorative sleep most of the time had a 55% (hazard ratio, 1.55; 95% CI, 1.19-2.04; P-trend=0.01) and 32% (hazard ratio, 1.32; 95% CI, 1.02-1.72; P-trend=0.002) increased risk of cardiovascular disease mortality, respectively, relative to men without those symptoms. CONCLUSION:Some insomnia symptoms, especially difficulty initiating asleep and nonrestorative sleep, are associated with a modestly higher risk of mortality.

Abstract:Background: To explore the relationship between sleep disturbances and falls in an elderly Chinese population. Methods: Data from 1726 individuals aged 70-87 years from the Rugao Longevity and Ageing Study were used. The Pittsburgh Sleep Quality Index (PSQI) was used to assess sleep variables. Outcomes were falls >= 1 time per year and falls >= 2 times per year. Results: A total of 22.7% of the participants experienced >= 1 fall, and 9.8% experienced >= 2 falls per year. Poor sleep quality was associated with >= 1 fall (OR 1.08, 95% CI 1.05-1.12; OR 1.27, 95% CI 1.14-1.41) and >= 2 falls (OR 1.08, 95% CI 1.03-1.14; OR 1.28, 95% CI 1.10-1.48), with an increase per PSQI score and SD PSQI score, respectively. In addition, sleep quality, sleep latency, sleep efficiency, and sleep disturbance subcomponents were associated with an increased risk of >= 1 fall with ORs of 1.44 (95% CI, 1.21-1.72), 1.23 (95% CI, 1.09-1.40), 1.12 (95% CI, 1.01-1.23) and 1.70 (95% CI, 1.35-2.14), respectively, and were associated with an increased risk of >= 2 falls with ORs 1.54 (95% CI, 1.22-1.96), 1.21(95% CI, 1.02-1.44), 1.17 (95% CI 1.02-1.33), and 1.78 (95% CI, 1.31-2.44), respectively. Further, participants slept <= 5 h per night had an increased risk of >= 1 fall (OR 2.34; 95% CI, 1.59-3.46) and >= 2 falls (OR 2.19; 95% CI, 1.30-3.69). Conclusions: Poor sleep quality and several subcomponent sleep symptoms were consistently associated with increased risk of falls >= 1 time and >= 2 times in Chinese elderly. The identification of sleep disturbances may help identify high-risk Chinese elders who may benefit from fall prevention education.

Abstract:Abstract Introduction Subjective sleep duration often differs from objective measurements. Our prior research indicates that overestimating sleep duration may be a risk factor for all-cause mortality in older men. Conversely, those with insomnia, which is an another mortality risk factor, typically underestimate their sleep duration. This study investigates how both underestimation and overestimation of sleep duration impact all-cause mortality in community-dwelling older men (aged ≥ 65) comparing with and without insomnia symptoms. Methods We conducted a secondary analysis using data from the Osteoporotic Fractures in Men Sleep Study, involving 2,200 participants who underwent in-home polysomnography and completed surveys assessing their subjective sleep duration, without using sleep-affecting medication (such as antidepressants, hypnotics or benzodiazepines). Insomnia was defined by a Pittsburgh Sleep Quality Index (PSQI) score over 5 (N = 816). We examined the relationship between subjective-objective sleep duration discrepancy index (SODI) and mortality using Cox regression. SODI, the ratio of subjective to objective sleep duration, was analyzed categorically, comparing its lowest and highest quartiles to the interquartile range within each group. The study was approved by the Ethics Committee of the National Center of Neurology and Psychiatry. Results During follow-up periods (mean: 10.6 years for insomniacs, 11.2 years for non-insomniacs), 504 (61.8%) insomniacs and 774 (55.9%) non-insomniacs died. In the insomnia group, both the lowest (adjusted Hazard Ratio [HR], 1.31; 95% Confidence Interval [CI], 1.02–1.68) and the highest (adjusted HR, 1.31; 95% CI, 1.03–1.66) SODI quartiles were associated with increased mortality compared to the interquartile range. Conversely, in the non-insomnia group, only the highest SODI quartile showed a significant increase in mortality (adjusted HR, 1.30; 95% CI, 1.07–1.59), with the lowest quartile showing no significant mortality association (adjusted HR, 0.94; 95% CI, 0.78–1.14). Conclusion Results demonstrate the significant role of subjective-objective sleep duration discrepancy in long-term health outcomes in both the insomnia and the non-insomnia group. Further physiological studies may elucidate the mechanisms of subjective-objective sleep duration discrepancy on mortality. Abstract:BACKGROUND:Physical activity and sleep are established modifiable lifestyle factors, but the optimal time of the day of these behaviours for health is unknown. This study examined the independent and joint associations of diurnal patterns of physical activity and sleep with all-cause mortality. METHODS:This prospective cohort study included 6,673 participants who have attended the accelerometer assessment in the 2011-2014 National Health and Nutrition Examination Surveys (NHANES). Diurnal patterns of accelerometer-measured physical activity and sleep were identified using K-means clustering analysis. All-cause mortality was ascertained from the accelerometer measurement to December 31, 2019 (median follow-up 6.8 years). Survey-weighted Cox proportional hazard models were performed to estimate the independent and joint associations of diurnal patterns of physical activity and sleep with all-cause mortality. RESULTS:Diurnal patterns identified were: early-morning (32.4%), midday (42.5%), and late-afternoon (25.1%) for physical activity; and irregular sleep (37.4%), morning lark (33.6%), and night owl (29.0%) for sleep. After adjusting for volume of physical activity, sleep duration and other potential covariates, the early-morning physical activity pattern (hazard ratio 1.36, 95% confidence interval 1.13-1.64) and irregular sleep pattern (1.42, 1.01-1.99) were independently associated with higher risk of all-cause mortality, compared with midday physical activity and morning lark sleep patterns, respectively. In addition, participants with the combined pattern of early-morning physical activity and irregular sleep had higher risk of all-cause mortality compared to those with midday physical activity combined with a morning lark sleep pattern (1.92, 1.33-2.78). Several sociodemographic differences were observed in the strength of these associations. CONCLUSIONS:Wearable activity-rest monitoring data showed that peak physical activity in the early morning and irregular sleep diurnal patterns are associated with increased mortality risk, and the combination of these patterns further exaggerated the risk. Public health program should acknowledge that the diurnal patterns of physical activity and sleep, in addition to their duration and frequency, may play a crucial role in lifestyle-based health promotion and management strategies.

Abstract:Study Objectives: Cohort studies about the sleep duration on the risk of death among Chinese older adults are still lacking. The aim of this study was to examine whether extremely long or short sleep duration was associated with mortality in Chinese adults aged 65 years or older. Methods: We included participants aged 65 years or older in 2011 at baseline in 23 provinces from the Chinese Longitudinal Healthy Longevity Survey who were followed up in 2014/2018 in China. Sleep duration was categorized as short sleep duration (< 7 hours) and long sleep duration (> 8 hours). We used the Cox proportional hazards model and restricted cubic spline analysis to explore the association between sleep duration and mortality. Results: Among 9578 participants, short sleep duration was associated with an 11% higher risk of death (adjusted hazard ratio [aHR]: 1.11; 95% confidence interval [CI]: 1.02-1.20) and long sleep duration was associated with a 24% higher risk of death (aHR: 1.24; 95% CI: 1.15-1.34), after adjustment for all covariates. There was a U-shaped association between sleep duration and all-cause mortality (nonlinear, P< .0001). Stratified analyses showed that the risk was higher among older people who smoked and with a higher level of education both for short and long sleepers than for those who never smoked and were illiterate (P value for interaction < .05). Conclusions: There was a U-shaped association between sleep duration and all-cause mortality in Chinese older adults, especially in more educated individuals and smokers.
Associations of Insomnia Symptoms and Trajectories with Incident Cardiovascular Disease: A Population-Based Cohort Study

Abstract:Background There is limited understanding regarding associations between insomnia symptoms, particularly the trajectories of insomnia symptoms, and cardiovascular disease (CVD). We aimed to investigate the associations of insomnia symptoms and trajectories with the risk of incident CVD. Methods This study used data from the Health and Retirement Study. Insomnia symptoms included non-restorative sleep, difficulty initiating sleep, early morning awakening, and difficulty maintaining sleep, classified on a scale ranging from 0 to 8. We also identified four distinct trajectories of insomnia symptoms: low, decreasing, increasing, and high insomnia symptoms. Examined outcomes included incident heart disease, stroke, and the combination of the two referred as CVD in the present study. Cox proportional hazard models were used to calculate the hazard ratio (HR) and 95% confidence interval (95% CI) after adjusting for potential confounders. Results A total of 12 102 participants aged 50 years or over without CVD at baseline were included. During a median follow-up of 10.2 years, 3 962 first CVD events occurred (3 372 heart disease and 1 200 stroke). Participants experiencing one (HR, 1.16 [95% CI, 1.05-1.27]), two (HR, 1.16 [ 95% CI, 1.05-1.28]), or three to four (HR, 1.26 [95% CI, 1.15-1.38]) insomnia symptoms had a higher risk of incident CVD compared to those not experiencing any insomnia symptoms. After a median follow-up of 8.4 years after the visit 2, 2 375 first CVD events occurred (1 981 heart disease and 705 stroke). Using the trajectory with low insomnia symptoms as the reference, increasing insomnia symptoms (HR, 1.28 [95% CI, 1.10-1.50]) and high insomnia symptoms (HR, 1.32 [95% CI, 1.15-1.50]) were associated with an increased risk of incident CVD. Conclusions Higher insomnia symptoms and increasing insomnia symptoms over time are associated with a higher risk of CVD in the community. Public health awareness and screening for insomnia symptoms in the middle-aged and elderly population should be encouraged to reduce CVD. Abstract:Summary Existing evidence linking insomnia to all‐cause mortality in older individuals remains inconclusive. We conducted a retrospective study of a large cohort of veterans aged 65–80 years old identified from the Corporate Data Warehouse, a large data repository derived from the Veterans Health Administration integrated medical records. Veterans' enrollees with and without International Classification of Diseases, Ninth and Tenth Revision, codes corresponding to insomnia diagnosis between 1 January 2010 and 30 March 2019 were assessed for eligibility. The primary outcome was all‐cause mortality. A total of 36,269 veterans, 9584 with insomnia and 26,685 without insomnia, were included in the analysis. Baseline mean (SD) age was 72.6 (4.2) years. During a mean follow‐up of 6.0 (2.9) years of the propensity score matched sample, the mortality rate was 34.8 [95% confidence interval: 33.2–36.6] deaths per 1000 person‐years among patients with insomnia compared with 27.8 [95% confidence interval: 26.6–29.1] among patients without insomnia. In a Cox proportional hazards model, insomnia was significantly associated with higher mortality (hazard ratio: 1.39; [95% confidence interval: 1.27–1.52]). Patients with insomnia also had a higher risk of non‐fatal cardiovascular events (hazard ratio: 1.21; [95% confidence interval: 1.06–1.37]). Secondary stratified analyses by sex, race, ethnicity and hypertension showed no evidence of effect modification. A higher risk of mortality (hazard ratio: 1.51; [95% confidence interval: 1.33–1.71]) was observed when depression was present compared with absent (hazard ratio: 1.26; [95% confidence interval: 1.12–1.44]; p = 0.02). In this cohort study, insomnia was associated with increased risk‐adjusted mortality and non‐fatal cardiovascular events in older individuals.

Abstract:Study Objectives: Previous studies have highlighted the importance of sleep patterns for human health. This study aimed to investigate the association of sleep timing with all -cause and cardiovascular disease mortality. Methods: Participants were screened from two cohort studies: the Sleep Heart Health Study (SHHS; n = 4,824) and the Osteoporotic Fractures in Men Study (n = 2,658). Sleep timing, including bedtime and wake-up time, was obtained from sleep habit questionnaires at baseline. The sleep midpoint was defined as the halfway point between the bedtime and wake-up time. Restricted cubic splines and Cox proportional hazards regression analyses were used to examine the association between sleep timing and mortality. Results: We observed a U-shaped association between bedtime and all -cause mortality in both the SHHS and Osteoporotic Fractures in Men Study groups. Specifically, bedtime at 11:00 PM and waking up at 7:00 AM was the nadir for all -cause and cardiovascular disease mortality risks. Individuals with late bedtime (> 12:00 AM) had an increased risk of all -cause mortality in SHHS (hazard ratio 1.53, 95% confidence interval 1.28-1.84) and Osteoporotic Fractures in Men Study (hazard ratio 1.27, 95% confidence interval 1.01-1.58). In the SHHS, late wake-up time (> 8:00 AM) was associated with increased all -cause mortality (hazard 1.39, 95% confidence interval 1.13-1.72). No significant association was found between wake-up time and cardiovascular disease mortality. Delaying sleep midpoint (> 4:00 AM) was also significantly associated with all -cause mortality in the SHHS and Osteoporotic Fractures in Men Study. Conclusions: Sleep timing is associated with all -cause and cardiovascular disease mortality. Our findings highlight the importance of appropriate sleep timing reducing mortality risk.
Insomnia and daytime sleepiness predict 20-year mortality in older male adults: data from a population-based study

Abstract:Data regarding the possible relationship of insomnia and EDS with mortality are inconclusive. The aim of this study was to investigate the association between these sleep complaints and the risk of long-term (20 years) all-cause mortality in older adults. Between April 2000 and March 2001, 750 subjects aged 65 years and older, who resided in the seventh district of Udine, were recruited. Data on sociodemographic characteristics, past medical history, and pharmacological treatment were collected. Dementia was diagnosed using a comprehensive neurological and neuroradiological assessment. Older adults were interviewed by neuropsychologists trained in sleep disturbances in order to assess the presence of sleep complaints. Vital status was followed over 20 years until March 2020. Older male adults affected by insomnia and EDS were significantly more likely to die over the follow-up period. Indeed, males reporting poor sleep and daytime somnolence had a 60% and 48% higher chance of dying than subjects who were not affected by these sleep complaints, respectively. The HR was attenuated after adjusting for confounding variables among insomniacs, whereas that of somnolent men strengthened. Differently from men, insomnia and EDS did not have any impact on mortality in older women. In conclusion, older male adults affected by insomnia and EDS had a significant increased risk of mortality, which is independent of cancer, depression, dementia, cardiovascular diseases, and sleeping pill use.
Insomnia and Multimorbidity in the Community Elderly in China

Abstract:Study objectives: To examine the comorbidity between insomnia and medical conditions. Methods: This cross-sectional study was conducted in community adults, aged ≥ 60 years, who resided in one of four major cities in northern China. Sociodemographic and clinical data were collected simultaneously. A total of 3,176 elderly adults (1,292 male, mean ± standard deviation age 70.2 ± 6.8 years; 1,884 female, 68.8 ± 6.7 years) were interviewed. Results: The prevalence of specific medical conditions in both people with insomnia and people without insomnia was detected. Significantly higher proportions of arrhythmia, hypertension, cerebral hemorrhage, migraine, and hyperlipidemia were observed in people with insomnia than in people without insomnia. Moreover, a significantly higher proportion of insomnia was seen in elderly people with arrhythmia and migraine. We also found that elderly people with insomnia who took sleep medications reported a higher prevalence of coexisting arrhythmia, hypertension, and migraine, even after adjusting for age, sex, and depression. Conclusions: Our results indicate associations between insomnia and medical conditions in the community elderly in China. People who complained of insomnia had poorer physical health conditions. Sleep medication may not be a covariate that influences the comorbidity of some specific physical conditions. Abstract:Objectives: To evaluate associations of sleep duration with total mortality and disease-specific mortality in a Chinese population. Design: Prospective study conducted from 1996 (for women)/2002 (for men) to 2010. Setting: A population-based cohort study in Shanghai, China. Intervention: None. Measurements and Results: A total of 113,138 participants (68,548 women and 44,590 men) of the Shanghai Women's and Men's Health Studies, aged 44-79 y and 40-75 y (women and men, respectively) at sleep duration assessment, were included in the study. In-person interviews were conducted to collect information on sleep duration, socioeconomic status, living conditions, history of chronic disease, participation in regular exercise, and family history of disease. The cohort has been followed using a combination of biannual in-person interviews and record linkages with Shanghai's population-based death registry. Survival status of participants on December 31, 2010 was included as the study outcome. Relative risks were calculated using a Cox proportional model stratified by sex and comorbidity score. There were 4,277 deaths (2,356 among women; 1,921 among men) during a median follow-up time of 7.12 y for women and 6.07 y for men. Among both women and men, sleep duration showed a J-shaped association with total mortality. Hazard ratios (95% confidence intervals) were 1.15 (1.01-1.32), 1.06 (0.94-1.20), 1.17 (1.04-1.32), 1.36 (1.13-1.64), and 2.11 (1.77-2.52) for women and 1.06 (0.90-1.25), 1.07 (0.94-1.23), 1.13 (1.00-1.28), 1.34 (1.10-1.62), and 1.55 (1.29-1.86) for men who slept 4-5, 6, 8, 9, and >= 10 h per day, respectively, compared with those who slept 7 h per day. Associations for disease-specific mortality, including cardiovascular disease, stroke, diabetes, and cancer, also generally followed the same J-shaped pattern. The sleep duration-mortality association was more evident among participants with comorbidities, but varied little by sex. Conclusion: In our study population of Chinese adults, shorter and longer sleep durations were independently associated with increased risk of mortality. But longer sleep duration had a higher mortality risk of cardiovascular disease and diabetes than short sleep.

Abstract:Background Sleep, cognition, and mortality may be interdependent. Objective We explored paths between sleep, cognition and mortality and potential interactions. Methods The study examines the relationship among sleep, global mental status, and mortality risk using data from 1364 participants from the Healthy Aging in Neighborhood of Diversity across the Life Span (HANDLS) study. We used Cox proportional hazards models and four-way decomposition models to analyze sleep patterns and global mental status. Results After a median time at risk of 8.2 years, 172 deaths occurred, with rate of 16 per 1000 person-years. A 1-unit increase in the Pittsburgh Sleep Quality Index (PSQI) global score was linked to a 7% increase in mortality risk in the reduced model, but this effect was attenuated in the full model. In both reduced and fully adjusted models, the PSQI global score and sleep quality domains interacted with global mental status, with poor sleep generally associated with mortality risk in the group with better global mental status at first-visit. In four-way decomposition models, total effects (TE) of PSQI scores on mortality risk were positive and statistically significant, while being mostly controlled direct effects. However, among women, the inverse TE of global mental status on mortality risk was partially mediated by PSQI sleep latency and the PSQI global. Conclusions Poor global mental status is associated with greater mortality risk at better sleep quality levels and vice versa. Further longitudinal studies with multiple sleep and cognitive performance repeats are needed to corroborate these findings.

Abstract:Objectives: To evaluate existing evidence of prospective cohort studies on associations between insomnia and multiple health outcomes. Study design: An umbrella review of meta-analyses of prospective cohort studies. Methods: A systematic search was undertaken in Pubmed, Embase, Cochrane, and Web of Science from inception to October 2021 to find meta-analyses of prospective cohort studies investigating the association of insomnia with any health outcome. The summary relative risk (SRR) for each meta-analysis was recalculated with random-effects model. The methodological quality and the quality of evidence were assessed by the A Measurement Tool to Assess Systematic Reviews and Grading of Recommendations, Assessment, Development and Evaluation, respectively. Results: A total of 25 published meta-analyses of prospective cohort studies, reporting 63 SRRs for 29 unique outcomes were included. Insomnia was mainly related to cardiovascular outcomes and mental disorders. The former comprised atrial fibrillation (SRR: 1.30, 95% confidence interval: 1.26 to 1.35), cardiovascular diseases (1.45, 1.29 to 1.64), coronary heart disease (1.28, 1.10 to 1.50), myocardial infarction (1.42, 1.17 to 1.72), and stroke (1.55, 1.39 to 1.72). The latter involved alcohol abuse (1.35, 1.08 to 1.67), all mental disorders (2.16, 1.70 to 3.97), anxiety (3.23, 1.52 to 6.85), depression (2.31, 1.90 to 2.81), suicidal ideation (2.26, 1.79 to 2.86), suicidal attempt (1.99, 1.31 to 3.02), and suicidal death (1.72, 1.42 to 2.08). Besides, insomnia enhanced the risk of Alzheimer's disease (1.51, 1.06 to 2.14) and hyperlipidemia (1.64, 1.53 to 1.76). Conclusion: Insomnia exhibits considerable adverse outcomes, primarily comprises cardiovascular outcomes and mental disorders, but further studies with robustly designed trials are needed to draw firmer conclusions.

Abstract:Limited information exists on age and racial disparities in sleep duration and mortality in the United States (US) population. This study compared the association between mortality and sleep duration within distinct races and age groups in the US. This study used data on 26,915 US citizens (≥ 18 years) from the 2004 wave of the National Health Interview Survey, linked to the National Death Index prospective mortality through 2019. Cox proportional hazard models were used to obtain hazard ratios (HRs) and 95% confidence intervals (CIs) for mortality by sleep duration, race (Whites, Black/African Americans, and Others (AIAN, Asian, and Native Hawaiian or other Pacific Islander)), and age (< 40, 40–60, and ≥ 60 years), while controlling for covariates such as sex, education, smoking status, disease history, and other vital factors. Race and age significantly modified the sleep duration-mortality relationship. Compared to other races, White participants exhibited higher mortality risks at all hours except at 5–6 h [HR: 0.993, 95% CI: 0.923–1.069]. Likewise, sleep duration associated mortality risks varied by age. Those at greater risk included < 40 years sleeping for 1–4 h [HR: 2.461, 95% CI: 1.446–4.187], 40–< 60 years sleeping for less than 7 h and more than 8 h, and ≥ 60 years sleeping for 9 h [HR: 1.309, 95% CI: 1.162–1.475] and ≥ 10 h [HR: 1.662, 95% CI: 1.486–1.858]. Age and race were significant effect modifiers in the sleep duration-mortality relationship. Thus, it is important to consider these factors when evaluating mortality risks associated with sleep patterns.

Sleep-disordered breathing and mortality: a prospective cohort study

Abstract:Background: Sleep-disordered breathing is a common condition associated with adverse health outcomes including hypertension and cardiovascular disease. The overall objective of this study was to determine whether sleep-disordered breathing and its sequelae of intermittent hypoxemia and recurrent arousals are associated with mortality in a community sample of adults aged 40 years or older. Methods and findings: We prospectively examined whether sleep-disordered breathing was associated with an increased risk of death from any cause in 6,441 men and women participating in the Sleep Heart Health Study. Sleep-disordered breathing was assessed with the apnea-hypopnea index (AHI) based on an in-home polysomnogram. Survival analysis and proportional hazards regression models were used to calculate hazard ratios for mortality after adjusting for age, sex, race, smoking status, body mass index, and prevalent medical conditions. The average follow-up period for the cohort was 8.2 y during which 1,047 participants (587 men and 460 women) died. Compared to those without sleep-disordered breathing (AHI: <5 events/h), the fully adjusted hazard ratios for all-cause mortality in those with mild (AHI: 5.0-14.9 events/h), moderate (AHI: 15.0-29.9 events/h), and severe (AHI: >or=30.0 events/h) sleep-disordered breathing were 0.93 (95% CI: 0.80-1.08), 1.17 (95% CI: 0.97-1.42), and 1.46 (95% CI: 1.14-1.86), respectively. Stratified analyses by sex and age showed that the increased risk of death associated with severe sleep-disordered breathing was statistically significant in men aged 40-70 y (hazard ratio: 2.09; 95% CI: 1.31-3.33). Measures of sleep-related intermittent hypoxemia, but not sleep fragmentation, were independently associated with all-cause mortality. Coronary artery disease-related mortality associated with sleep-disordered breathing showed a pattern of association similar to all-cause mortality. Conclusions: Sleep-disordered breathing is associated with all-cause mortality and specifically that due to coronary artery disease, particularly in men aged 40-70 y with severe sleep-disordered breathing. Abstract:Abstract Introduction Sleep-related symptoms have been associated with adverse health outcomes, including increased mortality risk. Studies that analyzed the correlation between sleep and mortality focused on the single effect of self-reported sleep duration or relied on nonstructured questionnaires about sleep. In order to evaluate the detrimental effects of sleep disorders, studies should be based on well-structured and validated questionnaires that inquire about different components of sleep, including sleep quality. In addition, taking into consideration the fluctuations in sleep duration over the study period is essential. We aimed to assess the association between sleep quality and all-cause mortality in community-dwelling adults living in rural Ecuador. Methods Individuals aged ≥40 years enrolled in the prospective population-based Three Villages Study cohort were included. Sleep quality was assessed by means of the Pittsburgh Sleep Quality Index (PSQI). Study participants were evaluated at baseline and at every annual door-to-door survey until they remained enrolled in the study. Mixed models Poisson regression for repeated PSQI determinations and multivariate Cox-proportional hazards models were fitted to estimate mortality risk according to sleep quality. Results Analysis included 1494 individuals (mean age: 56.6 ± 12.5 years; 56% women) followed for a median of 6.3 ± 3.3 years. At baseline, 978 (65%) individuals had good sleep quality and 516 (35%) had poor sleep quality. The effects of PSQI scores changing over time on mortality was confounded by the impact of the SARS-CoV-2 pandemic on both. One hundred ninety-five individuals (13%) died during the follow-up, resulting in a crude mortality rate of 1.58 per 100 person years (95% C.I.: 1.27-1.88) for individuals with good sleep quality, and 3.18 (95% C.I.: 2.53-3.82) for those with poor sleep quality at baseline. A multivariate Cox-proportional hazards model showed that individuals with poor sleep quality at baseline were 1.38 times (95% C.I.: 1.02-1.85) more likely to die compared to those with good sleep quality; in this model, increased age, poor physical activity, and high fasting glucose remained significant. Conclusion Poor sleep quality is associated with increase mortality risk among middle-aged and older adults. Support (if any)

Abstract:Irregular sleep and non-optimal sleep duration separately have been shown to be associated with increased disease and mortality risk. We used data from the prospective cohort Multi-Ethnic Study of Atherosclerosis sleep study (2010-2013) to investigate: do aging adults whose sleep is objectively high in regularity in timing and duration, and of sufficient duration tend to have increased survival compared with those whose sleep is lower in regularity and duration, in a diverse US sample? At baseline, sleep was measured by 7-day wrist actigraphy, concurrent with at-home polysomnography and questionnaires. Objective metrics of sleep regularity and duration from actigraphy were used for statistical clustering using sparse k-means clustering. Two sleep patterns were identified: "regular-optimal" (average duration: 7.0 ± 1.0 hr obtained regularly) and "irregular-insufficient" (duration: 5.8 ± 1.4 hr obtained with twice the irregularity). Using proportional hazard models with multivariate adjustment, we estimated all-cause mortality hazard ratios. Among 1759 participants followed for a median of 7.0 years (Q1-Q3, 6.4-7.4 years), 176 deaths were recorded. The "regular-optimal" group had a 39% lower mortality hazard than did the "irregular-insufficient" sleep group (hazard ratio [95% confidence interval]: 0.61 [0.45, 0.83]) after adjusting for socio-demographics, lifestyle, medical comorbidities and sleep disorders. In conclusion, a "regular-optimal" sleep pattern was significantly associated with a lower hazard of all-cause mortality. The regular-optimal phenotype maps behaviourally to regular bed and wake times, suggesting sleep benefits of adherence to recommended healthy sleep practices, with further potential benefits for longevity.

Abstract:Introduction: Insomnia symptoms may impact on various health outcomes. It is unclear how insomnia symptoms impact on health in Thailand. In a longitudinal study of ageing adults in Thailand, the goal of the research was to evaluate the relationships between insomnia symptoms and 20 health indicators. Methods: Prospective cohort data from the Health, Aging and Retirement in Thailand study, which included participants 45 years of age and older (N = 2863) from three successive waves in 2015, 2017, and 2020 were analyzed. Insomnia symptoms and health indicators were assessed by self-report. Generalized Estimating Equations analysis and logistic regression analysis were used to evaluate the longitudinal relationships between measures of insomnia symptoms and 20 health indicators. Results: In 2020, 11.7% of people reported having insomnia symptoms, compared to 15.6% in 2015 and 14.6% in 2017. Insomnia symptoms were positively correlated with three poor mental health indicators (depressive symptoms, quality of life and self-rated mental health), eight poor physical health indicators (hypertension, self-rated physical health status, cardiovascular disease, diabetes, osteoporosis, chronic lung disease, kidney disease, and physical injury), and two lifestyle indicators (physical inactivity and meal skipping) in logistic regression models. Conclusion: The study found that a number of indicators of physical and mental illness as well as lifestyle choices were linked to symptoms of insomnia. Improved detection and management of insomnia symptoms could lower indicators of physical and mental illness in Thailand.

Abstract:Background: Increased mortality has been reported in people with insomnia and in those with obstructive sleep apnoea (OSA). However, these conditions commonly co-occur and the combined effect of comorbid insomnia and sleep apnoea (COMISA) on mortality risk is unknown. This study used Sleep Heart Health Study (SHHS) data to assess associations between COMISA and all-cause mortality risk. Methods: Insomnia was defined as difficulties falling asleep, maintaining sleep and/or early morning awakenings from sleep ≥16 times per month, and daytime impairments. OSA was defined as an apnoea-hypopnoea index ≥15 events·h-1. COMISA was defined if both conditions were present. Multivariable adjusted Cox proportional hazards models were used to determine the association between COMISA and all-cause mortality (n=1210) over 15 years of follow-up. Results: 5236 participants were included. 2708 (52%) did not have insomnia/OSA (reference group), 170 (3%) had insomnia-alone, 2221 (42%) had OSA-alone and 137 (3%) had COMISA. COMISA participants had a higher prevalence of hypertension (OR 2.00, 95% CI 1.39-2.90) and cardiovascular disease (CVD) (OR 1.70, 95% CI 1.11-2.61) compared with the reference group. Insomnia-alone and OSA-alone were associated with higher risk of hypertension but not CVD compared with the reference group. Compared with the reference group, COMISA was associated with a 47% (hazard ratio 1.47, 95% CI 1.06-2.07) increased risk of mortality. The association between COMISA and mortality was consistent across multiple definitions of OSA and insomnia. Conclusions: COMISA was associated with higher rates of hypertension and CVD at baseline, and an increased risk of all-cause mortality compared with no insomnia/OSA.
