Abstract:OBJECTIVES:Remifentanil preconditioning attenuates myocardial ischemia reperfusion injury, but the underlying mechanism is incompletely understood. The Janus activated kinase-2 (JAK2)/signal transducers and activators of transcription-3 (STAT3) and phosphatidylinositol 3-kinase (PI3K)/Akt signaling pathways are critical in both ischemic and pharmacologic preconditioning cardioprotection, which involve the inactivation of glycogen synthase kinase-3β. We hypothesized that remifentanil preconditioning confers cardioprotection via the JAK2/STAT3 and/or PI3K/Akt activation-mediated glycogen synthase kinase-3β inhibition. DESIGN:Pharmacologic intervention. SETTING:Research laboratory. SUBJECTS:Male Sprague-Dawley rats. INTERVENTIONS:In vivo and in vitro treatments. MEASUREMENTS AND MAIN RESULTS:Male Sprague-Dawley rats (n = 6 per group) were sham operated or subjected to myocardial ischemia reperfusion injury. The JAK2 inhibitor AG490 (3 mg/kg), the PI3K inhibitor wortmannin (15 μg/kg), or the glycogen synthase kinase-3β inhibitor SB216763 (600 μg/kg) were given before inducing in vivo myocardial ischemia reperfusion injury achieved by occluding coronary artery for 30 minutes followed by 120 minutes of reperfusion in the absence or presence of remifentanil preconditioning (6 μg/kg/min). Also, isolated rat hearts were Langendorff perfused and subjected to 30 minutes of global ischemia and 120 minutes of reperfusion without or with remifentanil preconditioning (100 ng/mL) in the presence or absence of AG490 and/or SB216763. Isolated rat cardiomyocytes and H9C2 cells were subjected to hypoxia/reoxygenation alone or in combination with AG490 (100 μM), wortmannin (100 nM), or SB216763 (3 μM) without or with remifentanil preconditioning (2.5 μM). Remifentanil preconditioning reduced postischemic myocardial infarction and hemodynamic dysfunction induced by myocardial ischemia reperfusion injury concomitant with increased phosphorylation of STAT3 at tyr-705 (p-STAT3) and glycogen synthase kinase-3β but not Akt. AG490 but not wortmannin cancelled remifentanil preconditioning cardioprotection, and SB216763 restored it despite the presence of AG490. In Langendorff-perfused hearts, AG490-mediated cancellation of remifentanil preconditioning cardioprotection in attenuating postischemic myocardial infarction and creatinine kinase-MB release was reverted by concomitant administration of SB216763. Remifentanil preconditioning also attenuated posthypoxic cardiomyocyte injury and increased p-STAT3 and glycogen synthase kinase-3β in isolated primary cardiomyocytes and H9C2 cells. STAT3 gene knockdown with specific synthetic RNA cancelled remifentanil preconditioning cardioprotection, whereas glycogen synthase kinase-3β gene knockdown, which per se did not affect STAT3 under hypoxia/reoxygenation condition, preserved remifentanil preconditioning cardioprotection regardless of STAT3 abrogation. CONCLUSIONS:Remifentanil preconditioning confers cardioprotection primarily via activation of JAK2/STAT3 signaling that can function independent of PI3K/Akt activation. Glycogen synthase kinase-3β is a critical downstream effector of remifentanil preconditioning cardioprotection.

Abstract:The opioid receptor agonist remifentanil (REM) has been shown to attenuate myocardial ischemia reperfusion injury, but the underlying mechanism is unclear. We hypothesized that REM confers cardioprotection by activating the JAK/STAT and PI3K/Akt pathways, two important survival signal pathways for rescuing ischemic myocardium. After anesthesia, male Sprague-Dawley rats were subjected to 30 min of coronary artery occlusion followed by 2 hours of reperfusion. Rats (n=5–6/group) were treated with vehicle, REM preconditioning (6 μg/kg, 3 consecutive 5 min intravenous REM interspersed with 5 min infusion-free periods), or the GSK3β kinase inhibitor SB216763 (SB, 600 μg/kg) immediately prior to inducing ischemia in the absence or concomitant presence of the putative JAK2 inhibitor AG490 (3 mg/kg) or the PI3K inhibitor wortmannin (15 μg/kg). Myocardial infarct size (IS) was significantly reduced by either REM (25±10%) or SB (18±8%) compared to vehicle control (46±13%) (P<0.05). AG490 but not wortmannin abolished the IS sparing effect of REM, although AG490 or wortmannin alone did not significantly affect IS. REM enhanced myocardial phosphorylation of STAT3 and GSK3β compared with that of vehicle-treated rats (P<0.05), and AG490 abolished these effects of REM. By contrast, REM reduced Akt phosphorylation. Abstract:Recent studies have demonstrated that volatile anesthetic postconditioning confers myocardial protection against ischemia-reperfusion injury through activation of the reperfusion injury salvage kinase (RISK) pathway. As RISK has been shown to be impaired by ventricular hypertrophy, we investigate whether anesthetic-induced cardiac protection was maintained in rat hearts with ventricular hypertrophy. Transverse aortic constriction operation was performed on male Sprague-Dawley rats to induce left ventricular (LV) hypertrophy, then sham-operated or hypertrophied rat hearts were subjected to 40min of global ischemia and 2 h of reperfusion. The isolated hearts received 3% sevoflurane for 10min or six cycles of 10-s ischemia/10-s reperfusion after reperfusion. The hemodynamics, infarct size, PTEN (phosphatase and tensin homolog deleted on chromosome ten), phosphorylated Akt, phosphorylated extracellular regulated protein kinase (ERK) 1/2, and phosphorylated glycogen synthase kinase 3" (GSK3") were determined. We found the myocardial expression of PTEN, phosphorylated Akt, ERK1/2, and phosphorylated GSK3" did not significantly differ between sham-operated and transverse aortic constrictionYcontrol groups. Both sevoflurane and ischemic postconditioning significantly improved LV hemodynamics, reduced infarct size, and increased the phosphorylation of Akt, ERK1/2, and their downstream target of GSK3" in the shamoperated rat hearts. In contrast, neither sevoflurane nor ischemic postconditioning improved LV hemodynamic, reduced infarct size, and increased the phosphorylated Akt, ERK1/2, and GSK3" in hypertrophied myocardium. All the results above indicate that ventricular hypertrophy abrogated sevoflurane-induced cardioprotection against ischemia-reperfusion injury by alteration of RISK/GSK3" signals.

Abstract:Objective:To investigate the protective effect of remifentanil preconditioning on liver ischemia and reperfusion injury in rats.Methods:Forty-eight healthy adult SD rats,weighing 200-300 g,were divided into 4 groups:ischemic reperfusion group(I/R),ischemic preconditioning group(IPC),sham operation group(Sham),and remifentanil preconditioning group(RPC).The RPC group was further subdivided into 3 subgroups according to the dose of remifentanil:0.2 μg·kg-1·min-1(RPC1 group),1 μg·kg-1·min-1(RPC2 group),and 10 μg·kg-1·min-1(RPC3 group).All rats except for those in the sham group were subjected to ischemia for 45 min and followed by reperfusion for 2 hours.Serum levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST)and liver homogenate levels of MDA,SOD were determined.H-E staining was used to observe the hepatic histopathological changes and TUNEL staining was used to examine hepatocyte apoptosis.Results:In I/R and RPC1 group,serum ALT and AST were significantly increased;hepatic homogenate MDA content was increased and SOD content was decreased,accompanied by aggravated pathological injury;TUNEL staining showed large amount of apoptotic cells.Compared with I/R and RPC1 groups,serum ALT and AST levels in IPC,RPC2,and RPC3 groups were significantly decreased after liver ischemia and reperfusion,accompanied by decreased homogenate MDA level,increased SOD level,and improved pathological injury.TUNEL staining showed much less apoptotic hepatocytes in IPC,RPC2,and RPC3 groups compared with I/R and RPC1 groups.No obvious changes in histopathology or in other parameters were observed in the sham group.Conclusion:Pre-treatment with remifentanil,like ischemic precondition,can protect liver from ischemia and reperfusion injury.

Abstract:BackgroundRecent studies have demonstrated that volatile anesthetic preconditioning confers myocardial protection against ischemia–reperfusion (IR) injury through activation of the reperfusion injury salvage kinase (RISK) pathway. As RISK has been shown to be impaired in hypercholesterolemia, we investigate whether anesthetic-induced cardiac protection was maintained in hypercholesterolemic rats.MethodsNormocholesteolemic or hypercholesterolemic rat hearts were subjected to 30min of ischemia and 2h of reperfusion. Animals received 2.4% sevoflurane during three 5min periods with and without PI3K antagonist wortmannin (10μg/kg, Wort) or the ERK inhibitor PD 98059 (1mg/kg, PD). The infarct size, apoptosis, p-Akt, p-ERK1/2, p-GSK3β were determined.ResultsTwo hundred and six rats were analyzed in the study. In the healthy rats, sevoflurane significantly reduced infarct size by 42%, a phenomenon completely reversed by wortmannin and PD98059 and increased the phosphorylation of Akt, ERK1/2 and their downstream target of GSK3β. In the hypercholesterolemic rats, sevoflurane failed to reduce infarct size and increase the phosphorylated Akt, ERK1/2 and GSK3β. In contrast, GSK inhibitor SB216763 conferred cardioprotection against IR injury in healthy and hypercholesterolemic hearts.ConclusionsHyperchoesterolemia abrogated sevoflurane-induced cardioprotection against IR injury by alteration of upstream signaling of GSK3β and acute GSK inhibition may provide a novel therapeutic strategy to protect hypercholesterolemic hearts against IR injury.
Remifentanil protects against myocardial ischemia/reperfusion injury via miR-205-mediated regulation of PINK1

Abstract:Myocardial ischemia/reperfusion (I/R) injury could lead to severe cardiovascular ischemic disease, including myocardial infarction and contractile dysfunction. Remifentanil demonstrated protective effect on myocardial I/R injury. The underlying pathophysiological mechanism was then investigated in this study. In the current study, primary cardiomyocytes were isolated from rats, and then preconditioned with remifentanil. Rats, tail vein injected with miR-205 antagomir, were subjected to infusion of remifentanil, and then subjected to regional ischemia followed by reperfusion. The results demonstrated that cell viability of hypoxia/reoxygenation-induced cardiomyocytes was increased post remifentanil, while the apoptosis was decreased accompanied with reduced cleaved caspase-3 expression. Hypoxia/reoxygenation treatment increased miR-205 and decreased PINK1 (PTEN induced putative kinase 1) expression. However, preconditioning with remifentanil reduced miR-205 and enhanced PINK1. Moreover, over-expression of miR-205 decreased PINK1 expression and counteracted the effects of remifentanil-induced increase of cell viability and decrease of cell apoptosis in hypoxia/reoxygenation-induced cardiomyocytes. Injection with miR-205 antagomir improved remifentanil-induced decrease of infarct size and LDH (lactic acid dehydrogenase) activity in rat model with I/R injury. In conclusion, miR-205 might participate in the protective effect of remifentanil in rat myocardial I/R injury via regulation of PINK1, providing a potential target for amelioration of cardiovascular ischemic disease.
Remifentanil preconditioning induces protection against focal cerebral ischemia-reperfusion injury in rats

Abstract:Objective To investigate whether remifentanil preconditioning induced protection against ischemia-reperfusion injury in rat brain.Methods Twenty-three male SD rats were randomly assigned to control (n=10) and remifentanil (n=13) groups.The control group rats received saline infusion intravenously in the regime (3×5min infusion with 5min interval). The rats in remifentanil preconditioning group received remifentanil infusion intravenously at the rate of 0.6μg·kg -1 ·min -1 in the same regime with those in control group. At 30min after pretreatments,all rats were subjected to the right middle cerebral artery occlusion ( MCAO ) for 120min and 24h reperfusion. The neurological deficit scores ( NDS ) were evaluated at 24h after reperfusion, then all animals were killed. Infarct volume, as a percentage of volume at normal cerebral hemisphere, was determined by 2,3,5-triph-enyltetrazolium(TTC) staining. Results The neurological deficit scores( NDS )in the remifentanil group at 24h after reperfusion were significantly lower than those in the control group(P<0.05 ). The infarct volume in control group was significantly larger than that in remifentanil group (P<0.01) at 24h after reperfusion.Conclusion Remifentanil preconditioning can protect rat brain against ischemia-reperfusion injury.

Abstract:Background: Opioid preconditioning against ischemia reperfusion injury has been well studied in myocardial and neuronal tissues. The objective of this study was to determine whether remifentanil could attenuate hepatic injury and to investigate the mechanisms. Methods: A rat model of hepatic ischemia reperfusion injury and a hepatocyte hypoxia reoxygenation (HR) injury model were used, respectively, in two series of experiments. Remifentanil was administered before ischemia or hypoxia and the experiments were repeated with previous administration of naloxone, L-arginine and N-omega-nitro-L-arginine methyl ester, a nonselective opioid receptor antagonist, a nitric oxide donor, and nitric oxide synthase (NOS) inhibitor, respectively. Serum aminotransferase, cytokines, and hepatic lipid peroxidation were measured. Histopathology examination and apoptotic cell detection were assessed. For the in vitro study, cell viability, intracellular nitric oxide, apoptosis, and NOS expression were evaluated. Results: Remifentanil and L-arginine pretreatment reduced concentrations of serum aminotransferases and cytokines, decreased the concentrations of hepatic malondialdehyde and myeloperoxidase activity, and increased superoxide dismutase, nitric oxide, and inducible NOS expression in vivo. Decreased histologic damage and apoptosis were also seen in these two groups. These changes were prevented by previous administration of N-omega-nitro-L-arginine methyl ester but not naloxone. There was an increase in inducible NOS protein expression but not endogenous NOS in remifentanil and L-arginine pretreated groups compared with control, naloxone, and N-omega-nitro-L-arginine methyl ester groups. Conclusion: Pretreatment with remifentanil can attenuate liver injury both in vivo and in vitro. Inducible NOS but not opioid receptors partly mediate this effect by exhausting reactive oxygen species and attenuating the inflammatory response.
Remifentanil preconditioning promotes liver regeneration via upregulation of β-arrestin 2/ERK/cyclin D1 pathway

Abstract:<p>Remifentanil is a potent, short-acting opioid analgesic drug that can protect tissues from ischemia and reperfusion injury though anti-inflammatory effects. However, the utility of remifentanil in liver regeneration after hepatectomy is not known. Using a 70% hepatectomy mouse model (PHx), we found that preconditioning animals with 4 μg/kg remifentanil enhanced liver regeneration through supporting hepatocyte proliferation but not through anti-inflammatory effects. These effects were also phenocopied in vitro where 40 mM remifentanil promoted the proliferation of primary mouse hepatocyte cultures. We further identified that remifentanil treatment increased the expression of β-arrestin 2 in vivo and in vitro. Demonstrating specificity, remifentanil preconditioning failed to promote liver regeneration in liver-specific β-arrestin 2 knockout (CKO) mice subjected to PHx. While remifentanil increased the expression of activated (phosphorylated)-ERK and cyclin D1 in PHx livers, their levels were not significantly changed in remifentanil-treated CKO mice nor in WT mice pretreated with the ERK inhibitor U0126. Our findings suggest that remifentanil promotes liver regeneration via upregulation of a β-arrestin 2/ERK/cyclin D1 axis, with implications for improving regeneration process after hepatectomy.</p>

Abstract:Background & Objectives: Isoflurane postconditioning (IsoPostC) against myocardial ischemia reperfusion injury (IRI) is significantly decreased in diabetes. The underlying mechanism is unclear. Signal transducer and activator of transcription 3 (STAT3) is critical in ischemic postconditioning cardioprotection which is reduced in diabetic hearts as well as decreased expressions of cardiac Brahma-related gene 1 (Brg1) and nuclear factor-erythroid 2-related factor 2 (Nrf2). We hypothesized that Brg1/Nrf2 and STAT3 activation may play a critical role in IsoPostC-mediated cardioprotection in normal and diabetic hearts. Materials & Methods: Non-diabetic (C) and 8-week streptozotocin-induced type-1 diabetic (D) rat hearts were perfused ex vivo using Langendorff-perfused model. Rat hearts were subjected to 30 minutes global ischemia and 120 minutes of reperfusion with or without IsoPostC. IsoPostC was achieved by administrating emulsified isoflurane(2.0%, v/v) at the onset of reperfusion for 10 min. Left ventricular (LV) function was assessed throughout the experiment. Myocardial infarct size (IS) in relation to the area at risk (AAR) was used to measure the extent of myocardial injury. Cell injury was assessed by quantification of lactate dehydrogenase (LDH). Cardiac Nrf2, STAT3 and Brg1 protein expressions were measured using western blot. Results: (1)IsoPostC reduced myocardial IS in normal but not diabetic rats. (2) IsoPostC attenuated myocardial dysfunction in normal but not diabetic rats. (3) Brg1, Nrf2 and STAT3 protein expressions were enhanced in normal but decreased in diabetic rats. (4) The beneficial effects of IsoPostC were abolished by Brg1, Nrf2 or STAT3 gene knockdown.Conclusion: Our results indicate that IsoPostC confers cardioprotection through Brg1/Nrf2/STAT3 signaling, and impairment of this pathway may be responsible for the loss of sensitivity to IsoPostC cardioprotectio in type 1 diabetes. Abstract:Background Myocardial injury caused by myocardial ischemia (MI) is still a severe condition that can result in apoptosis, oxidative stress, and inflammation. Remifentanil is a selective, ultra-short-acting, µ-opioid receptor agonist opioid. It can improve sinusoidal heart rate patterns in the fetus, for bupivacaine-induced cardiotoxicity, and with lipopolysaccharide (LPS)-induced cardiomyocytes injuries. This study aimed to explore the cardioprotective effects of remifentanil in MI model rats. Methods Sprague Dawley (SD) rats were split into five groups at random, including a control group, Isop group, low-dose remifentanil treatment group (10 µg/kg), medium-dose remifentanil treatment group (20 µg/kg), and a high-dose remifentanil treatment group (40 µg/kg). The MI model was achieved by subcutaneously injecting rats with isoproterenol (85 mg/kg) for two consecutive days. With the expression of apoptotic molecules, myocardial systolic function index, inflammation, antioxidant enzymes, and the myocardial enzyme taken into account, the data was analyzed. Results After treatment with remifentanil, the left ventricular wall thickness (LVWT), left ventricular end-systolic volume (LVESV), left ventricular ejection fraction (LVEF), fraction shortening (FS), and heart rate (HR) were significantly increased in comparison with the Isop group. Creatine kinase-MB (CK-MB), Mb, and cTnl expressions were decreased. Meanwhile, the levels of cleaved caspase-3 and caspase-9 were decreased. Remarkably, the levels of reactive oxidative species (ROS), malondialdehyde (MDA), and lactate dehydrogenase (LDH) were observed to be repressed, while the levels of superoxide dismutase (SOD) was significantly increased. More importantly, the levels of tumor necrosis factor-α (TNF-α), interleukin (IL)-1β, IL-6, and interferon (IFN)-γ were decreased. Conclusions Remifentanil has significant potential as a therapeutic intervention strategy for ameliorating myocardial injury after MI and these findings provide the rationale for further clinical studies.

Abstract:Remifentanil is a potent, short-acting opioid analgesic drug that can protect tissues from ischemia and reperfusion injury though anti-inflammatory effects. However, the utility of remifentanil in liver regeneration after hepatectomy is not known. Using a 70% hepatectomy mouse model (PHx), we found that preconditioning animals with 4 mu g/kg remifentanil enhanced liver regeneration through supporting hepatocyte proliferation but not through anti-inflammatory effects. These effects were also phenocopied in vitro where 40 mM remifentanil promoted the proliferation of primary mouse hepatocyte cultures. We further identified that remifentanil treatment increased the expression of beta-promote liver regeneration in liver-specific beta-arrestin 2 knockout (CKO) mice subjected to PHx. While remifentanil increased the expression of activated (phosphorylated)-ERK and cyclin D1 in PHx livers, their levels were not signifi-cantly changed in remifentanil-treated CKO mice nor in WT mice pretreated with the ERK inhibitor U0126. Our findings suggest that remifentanil promotes liver regeneration via upregulation of a beta-arrestin 2/ERK/cyclin D1 axis, with implications for improving regeneration process after hepatectomy. Sevoflurane Postconditioning Reduces Myocardial Reperfusion Injury in Rat Isolated Hearts Via Activation of PI3K/Akt Signaling and Modulation of Bcl-2 Family Proteins.

Abstract:Sevoflurane postconditioning reduces myocardial infarct size. The objective of this study was to examine the role of the phosphatidylinositol-3-kinase (PI3K)/Akt pathway in anesthetic postconditioning and to determine whether PI3K/Akt signaling modulates the expression of pro- and antiapoptotic proteins in sevoflurane postconditioning. Isolated and perfused rat hearts were prepared first, and then randomly assigned to the following groups: Sham-operation (Sham), ischemia/reperfusion (Con), sevoflurane postconditioning (SPC), Sham plus 100 nmol/L wortmannin (Sham+Wort), Con+Wort, SPC+Wort, and Con+dimethylsulphoxide (DMSO). Sevoflurane postconditioning was induced by administration of sevoflurane (2.5%, v/v) for 10 min from the onset of reperfusion. Left ventricular developed pressure (LVDP), left ventricular end-diastolic pressure (LVEDP), maximum increase in rate of LVDP (+d P /d t ), maximum decrease in rate of LVDP (−d P /d t ), heart rate (HR), and coronary flow (CF) were measured at baseline, R30 min (30 min of reperfusion), R60 min, R90 min, and R120 min. Creatine kinase (CK) and lactate dehydrogenase (LDH) were measured after 5 min and 10 min reperfusion. Infarct size was determined by triphenyltetrazolium chloride staining at the end of reperfusion. Total Akt and phosphorylated Akt (phospho-Akt), Bax, Bcl-2, Bad, and phospho-Bad were determined by Western blot analysis. Analysis of variance (ANOVA) and Student-Newman-Keuls’ test were used to investigate the significance of differences between groups. The LVDP, ±d P /d t , and CF were higher and LVEDP was lower in the SPC group than in the Con group at all points of reperfusion ( P <0.05). The SPC group had significantly reduced CK and LDH release and decreased infarct size compared with the Con group [(22.9±8)% vs. (42.4±9.4)%, respectively; P <0.05]. The SPC group also had increased the expression of phospho-Akt, Bcl-2, and phospho-Bad, and decreased the expression of Bax. Wortmannin abolished the cardioprotection of sevoflurane postconditioning. Sevoflurane postconditioning may protect the isolated rat heart. Activation of PI3K and modulation of the expression of pro- and antiapoptotic proteins may play an important role in sevoflurane-induced myocardial protection.
Effects of Sevoflurane Postconditioning on the Expressions of AKT/GSK3β/mTOR During Ischemia-Reper-fusion in Rat Hearts

Abstract:Objective To investigate the role of AKT/GSK3β/mTOR signaling molecule in myocardial protection of sevoflurane postconditioning.Methods Thirty-nine male Sprague-Dawley rats,weighing 200-250 g,installed in vivo myocardial ischemia-reperfusion model by left anterior de-scending coronary occlusion for 30 min.Rat hearts were randomly divided into 3 groups (n = 13 ):sham control group (group Sham),purely ischemia-reperfusion group (group IR),sevoflurane post-conditioning group (group SPC).With the exception of group Sham,each group was subjected to oc-clusion for 30 min followed by 2 h reperfusion.Group SPC was subjected to sevoflurane postcondi-tioning:2.4% sevoflurane was inhaled for 1 5 min starting from the end of ischemia until 1 5 min after beginning of reperfusion,while 33% oxygen was inhaled in the other groups.At the end of 2 h reper-fusion,cardiac function was evaluated by two-dimensional echocardiography,myocardial infarction size was measured by using 1% 2,3,5-triphenyltetrazolium chloride triazole (TTC),myocardial ultra-structural alterations was detected by transmission electron microscopy (TEM),cardiomyocytes ap-optosis was examined by terminal deoxynucleotidyl nickend labeling (TUNEL),the expressions of p-AKT/t-AKT, p-GSK3β/t-GSK3β, p-mTOR/t-mTOR,Bcl-2 and Bax proteins was measured by Western blot.Results Compared with group Sham,cardiac function was deteriorated,myocardial in-farct size was increased,cardiomyocyte mitochondrial damage was increased,positive apoptotic car-diomyocyte was increased,the expression of Bcl-2 was down-regluated,and the expressions of p-AKT/t-AKT,p-GSK3β/t-GSK3β,p-mTOR/t-mTOR and Bax were up-regluated in group IR (P <0.05).Compared with group IR,cardiac function was improved,myocardial infarct size was de-creased,cardiomyocyte mitochondrial damage was decreased,positive apoptotic cardiomyocyte was decreased,the expression of Bax was down-regluated,and the expressions of p-AKT/t-AKT,p-GSK3β/t-GSK3β,p-mTOR/t-mTOR and Bcl-2 were up-regluated in group SPC (P < 0.05 ). Conclusion Sevoflurane postconditioning can mitigate ischemia-reperfusion injury to in vivo rat hearts,decreased cardiomyocyte mitochondrial damage,inhibited cardiomyocyte apoptosis,and its mechanism was related to the activation of AKT/GSK3β/mTOR signaling molecule.

Abstract:Remifentanil, an ultra-short acting opiate, has been reported to protect against hepatic ischemia-reperfusion injury, which is a major cause of postoperative liver dysfunction. The objective of this study was to determine whether a central vagal pathway is involved in this protective procedure. Rat models of hepatic ischemia-reperfusion were used in the experimental procedures. The results revealed that intravenous pretreatment with remifentanil decreased serum aminotransferases and hepatic histologic damage; however, an intraperitoneal injection of μ-opioid receptor antagonist did not abolish the protection of remifentanil preconditioning. c-Fos immunofluorescence of the brain stem showed that dorsal motor nucleus of the vagus was activated after remifentanil preconditioning. Moreover, serum alanine aminotransferase, histopathologic damage, and apoptosis decreased in remifentanil preconditioning group compared to vagotomized animals with remifentanil preconditioning, and there was no statistical difference of TNF-α and IL-6 between NS/Va and RPC/Va groups. In addition, remifentanil microinjection into dorsal vagal complex decreased serum aminotransferases, inflammatory cytokines, and hepatic histologic injury and apoptosis, and these effects were also abolished by a peripheral hepatic vagotomy. In conclusion, remifentanil preconditioning conferred liver protection against ischemia-reperfusion injury, which was mediated by the central vagal pathway.

Abstract:Remifentanil protects against ischemia/reperfusion (I/R)-induced organ injury, although its underlying mechanism remains elusive. This study was designed to examine the protective effect of remifentanil preconditioning, if any, against hepatic I/R injury in rats and the underlying mechanism involved.Adult Sprague-Dawley rats were randomly divided into sham operation (S group), ischemia/reperfusion (I/R group), and remifentanil preconditioning (R group) groups. Rats in the I/R group were subjected to a partial (70%) hepatic ischemia for 45 min, followed by 1 h, 3 h, and 6 h of reperfusion. Rats in the R group received venous injection of remifentanil (2 μg/kg/min) from 30 min prior to hepatic ischemia to the end of ischemia. Hepatic morphology and apoptosis were examined. Markers of liver damage, oxidative stress, and inflammation were evaluated. Mitochondrial function was assessed using mitochondrial membrane potential and appearance of mitochondrial swelling.Compared with the S group, rats in the I/R group displayed a massive degenerative death in liver tissues and significantly enhanced cell apoptosis. Remifentanil preconditioning significantly reduced I/R-induced hepatocyte apoptosis. In addition, remifentanil protected against I/R-induced mitochondrial swelling and loss of membrane potential. Remifentanil preconditioning inhibited I/R-induced increases in tumor necrosis factor α, intercellular adhesion molecule 1, and nuclear factor κB p65 levels in liver tissues. Remifentanil preconditioning also inhibited the loss in superoxide dismutase and rise in malondialdehyde levels in liver tissues going through I/R injury.Our data revealed that remifentanil preconditioning may turn on multiple cellular pathways in hepatocytes to protect the liver from I/R injury by alleviating hepatic apoptosis.

Abstract:Aim: To investigate the role of extracellular signal-regulated kinases (ERKs) in sevoflurane post-conditioning induced cardioprotection in vitro . Methods: Isolated rat hearts were subjected to 30 min ischemia followed by 120 min reperfusion (I/R). Sevoflurane post-conditioning was carried out by administration of O 2 -enriched gas mixture with 3% sevoflurane (SEVO) for 15 min from the onset of reperfusion. Cardiac functions, myocardial infarct size, myocardial ATP and NAD + contents, mitochondrial ultrastructure, and anti-apototic and anti-oncosis protein levels were measured. Results: Sevoflurane post-conditioning significantly improved the heart function, decreased infarct size and mitochondria damage, and increased myocardial ATP and NAD + content in the I/R hearts. Furthermore, sevoflurane post-conditioning significantly increased the levels of p-ERK and p-p70S6K, decreased the levels of porimin, caspase-8, cleaved caspase-3, and cytosolic cytochrome c in the I/R hearts. Co-administration of the ERK1/2 inhibitor PD98059 (20 μmol/L) abolished the sevoflurane-induced protective effects against myocardial I/R. Conclusion: Sevoflurane post-conditioning protects isolated rat hearts against myocardial I/R injury and inhibits cell oncosis and apoptosis via activation of the ERK1/2 pathway.

Abstract:Objective To investigate the protective effects of remifentanil postconditioning on isolated rabbit hearts and compare the effects of remifentanil postconditioning with remifentanil preconditioning.Methods The ischemia reperfusion model of isolated rabbit heart was set up,24 rabbits were randomly divided into 3 groups:model group,remifentanil preconditioning(RPC),remifentanil postconditioning (RPO).In model group,the hearts was induced by 30 min of global ischemia and 120 min of reperfusion;in RPC group,the hearts were subjected to preconditioning by 60 μg · L-1 remifentanil buffer before 30 min of global ischemia;in RPO group,the hearts were subjected to postconditioning by 60 pg · L-1 remifentanil buffer after 30 min of global ischemia.The heart rate (HR),left ventricular developed pressure(LVDP),maximum change rate of left ventricular pressure rise (+ dp/dtmax) and the maximum change rate of left ventricular pressure drop (-dp/dtmax),contents of lactate dehydrogenase(LDH) and creatine phosphokinase(CK) in the liquid of coronary were measured.The myocardial infarct size was caculated.Results At 30 min after reperfusion,LDVP in RPC group and RPO group were (39.95 + 8.18),(49.55 + 10.38) mmHg,significantly higher than the model group of LDVP (27.21 + 10.26) mmHg.At 90 min after reperfusion,LDH in RPC group and RPO group were (90.70 + 4.31),(88.75 + 7.14) U · L-1,significantly lower than the model group (109.54 + 11.38) U · L-1.Myocardial infarction area of RPC group and RPO group were (21.54 +5.28)％,(17.82 +6.39)％,the area of myocardial infarction was significantly lower than that of model group (59.49 +9.94)％.Comparison between RPC group,RPO group and model group,the differences were statistically significant (all P ＜ 0.05).Conclusion Remifentanil postconditioning can protect the rabbit heart from ischemia reperfusion injury by improving myocardial systolic-diastolic function and preserving the myocardial and decreasing infarct size.
[Protective Effect of Remifentanil Preconditioning Against Hepatic Ischemia-Reperfusion Injury in Rats: Role of P38 Mitogen-Activated Protein Kinases].

Abstract:OBJECTIVE:To assess the role of p38 mitogen-activated protein kinases (p38MAPK) in the protective effect of remifentanil preconditioning (RPC) on hepatic ischemia-reperfusion injury in rats.METHODS:Ninety-six male SD rats were randomly assigned into sham-operated group, ischemia-reperfusion group (I/R group), RPC group, and SB (an inhibitor of p38 MAPK) +RPC group. The rats were sacrificed at the end of reperfusion, and serum levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), tumor necrosis factor-α (TNF-α), and interleukin-1β (IL-1β) were measured. HE staining was used to observe the hepatic histopathological changes, and Western blotting was employed to examine p38MAPK and pp38MAPK protein expression. TUNEL staining was used to examine cell apoptosis in the liver tissues.RESULTS:Compared with sham-operated group, I/R group showed significantly increased serum levels of AST, ALT, TNF-α and IL-1β with obvious histopathological changes and cell apoptosis in the liver. RPC significantly decresed the elevated serum levels of AST, ALT, TNF-α and IL-1β and lessened hepatic histopathological changes, and caused reduced p38MAPK phosphorylation and hepatic cell apoptosis index. The protective effects of RPC were abolished by SB 203580 pretreatment.CONCLUSION:RPC attenuates the production of inflammatory factors by activating p38MAPK signal pathway to improve hepatic ischemia-reperfusion injury, and these effects can be blocked by SB203580, a p38MAPK inhibitor.
Remifentanil preconditioning protects against hypoxia-induced senescence and necroptosis in human cardiac myocytes in vitro

Abstract:Remifentanil and other opioids are suggested to be protective against ischemia-reperfusion injury in animal models and coronary artery bypass surgery patients, however the molecular basis of such protection is far from being understood. In the present study, we have used a model of human cardiomyocytes treated with the hypoxia-mimetic agent cobalt chloride to investigate remifentanil preconditioning-based adaptive responses and underlying mechanisms. Hypoxic conditions promoted oxidative and nitrosative stress, p21-mediated cellular senescence and the activation of necroptotic pathway that was accompanied by a 2.2-, 9.6- and 8.2-fold increase in phosphorylation status of mixed lineage kinase domain-like pseudokinase (MLKL) and release of pro-inflammatory cytokine IL-8 and cardiac troponin I, a marker of myocardial damage, respectively. Remifentanil preconditioning was able to lower hypoxia-mediated protein carbonylation and limit MLKL-based signaling and pro-inflammatory response to almost normoxic control levels, and decrease hypoxia-induced pro-senescent activity of about 21% compared to control hypoxic conditions. In summary, we have shown for the first time that remifentanil can protect human cardiomyocytes against hypoxia-induced cellular senescence and necroptosis that may have importance with respect to the use of remifentanil to diminish myocardial ischemia and reperfusion injury in patients undergoing cardiac surgery.
