Abstract:This paper reports a novel remotely actuated manipulator for access to prostate tissue under magnetic resonance imaging guidance (APT-MRI) device, designed for use in a standard high-field MRI scanner. The device provides three-dimensional MRI guided needle placement with millimeter accuracy under physician control. Procedures enabled by this device include MRI guided needle biopsy, fiducial marker placements, and therapy delivery. Its compact size allows for use in both standard cylindrical and open configuration MRI scanners. Preliminary in vivo canine experiments and first clinical trials are reported.

Abstract:Percutaneous needle-based interventions such as transperineal prostate brachytherapy require the accurate placement of multiple needles to treat cancerous lesions within the target organ. To guide needle placement, magnetic resonance imaging (MRI) offers excellent visualization of the target lesion without the need for ionizing radiation. To date, multi-needle insertion relies on a grid template, which limits the ability to steer individual needles. This work describes an MR-compatible robot designed for the sequential insertion of multiple non-parallel needles under MR guidance. The 6-DOF system is designed with an articulated arm to extend the reach of the robot. This strategy presents a novel approach enabling the robot to maneuver around existing needles while minimizing the footprint of the robot. Forward kinematics as well as optimization-based inverse kinematics are presented. The impact of the robot on image quality was tested for four sequences (T1w-TSE, T2w-TSE, THRIVE and EPI) on a 3T Philips Achieva system. Quantification of the signal-to-noise ratio showed a 46% signal loss in a gelatin phantom when the system was powered on but no further adverse effects when the robot was moving. Joint level testing showed a maximum error of 2.10 ± 0.72°s for revolute joints and 0.31 ± 0.60 mm for prismatic joints. The theoretical workspace spans the proposed clinical target surface of 10 x 10 cm. Lastly, the feasibility of multi-needle insertion was demonstrated with four needles inserted under real-time MR-guidance with no visible loss in image quality.

Abstract:In this paper the performance of a high-precision parallel robot manipulator with bistable actuation is experimentally evaluated. The manipulator is for performing prostate cancer biopsy and treatment within the bore of a Magnetic Resonance Imaging (MRI) system. The analysis and simulations have shown that this bistable manipulator is able to perform well with dielectric elastomer actuators that have been shown to be compatible with the high magnetic fields of an MRI. In this work an experimental prototype system was developed and tested. The results show that it provides the precise needle placement required by the medical task.

Abstract:Early prostate cancer detection and treatment are of major importance to reduce mortality rate. magnetic resonance (MR) imaging provides images of the prostate where an early stage lesion can be visualized. The use of robotic systems for MR-guided interventions in the prostate allows us to improve the clinical outcomes of procedures such as biopsy and brachytherapy. This work presents a novel MR-conditional robot for prostate interventions. The minimally invasive robotics in an magnetic resonance imaging environment (MIRIAM) robot has 9 degrees-of-freedom (DoF) used to steer and fire a biopsy needle. The needle guide is positioned against the perineum by a 5 DoF parallel robot driven by piezoelectric motors. A 4 DoF needle driver inserts, rotates and fires the needle during the procedure. Piezoelectric motors are used to insert and rotate the needle, while pneumatic actuation is used to fire the needle. The MR-conditional design of the robot and the needle insertion controller are presented. MR compatibility tests using T2 imaging protocol are performed showing a SNR reduction of 25% when the robot is operational within the MR scanner. Experiments inserting a biopsy needle toward a physical target resulted in an average targeting error of 1.84[Formula: see text]mm. Our study presents a novel MR-conditional robot and demonstrated the ability to perform MR-guided needle-based interventions in soft-tissue phantoms. Moreover, the image distortion analysis indicates that no visible image deterioration is induced by the robot.

Abstract:Magnetic resonance imaging (MRI) provides high-quality soft-tissue images of anatomical structures and radiation free imaging. The research community has focused on establishing new workflows, developing new technology, and creating robotic devices to change an MRI room from a solely diagnostic room to an interventional suite, where diagnosis and intervention can both be done in the same room. Closed bore MRI scanners provide limited access for interventional procedures using intraoperative imaging. MRI robots could improve access and procedure accuracy. Different research groups have focused on different technology aspects and anatomical structures. This paper presents the results of a systematic search of MRI robots for needle-based interventions. We report the most recent advances in the field, present relevant technologies, and discuss possible future advances. This survey shows that robotic-assisted MRI-guided prostate biopsy has received the most interest from the research community to date. Multiple successful clinical experiments have been reported in recent years that show great promise. However, in general the field of MRI robotic systems is still in the early stage. The continued development of these systems, along with partnerships with commercial vendors to bring this technology to market, is encouraged to create new and improved treatment opportunities for future patients.

Abstract:The aim of this work is to develop a remotely controlled manipulator to perform minimally invasive diagnostic and therapeutic interventions in the abdominal and thoracic cavities, with real-time magnetic resonance imaging (MRI) guidance inside clinical cylindrical MR scanners. The manipulator is composed of a three degree of freedom Cartesian motion system, which resides outside the gantry of the scanner, and serves as the holder and global positioner of a three degree of freedom arm which extends inside the gantry of the scanner At its distal end, the arm's end-effector can carry an interventional tool such as a biopsy needle, which can be advanced to a desired depth by means of a seventh degree of freedom. These seven degrees of freedom, provided by the entire assembly, offer extended manipulability to the device and a wide envelope of operation to the user, who can select a trajectory suitable for the procedure. The device is constructed of nonmagnetic and nonconductive fiberglass, and carbon fiber composite materials, to minimize artifacts and distortion on the MR images as well as eliminate effects on its operation from the high magnetic field and the fast switching magnetic field gradients used in MR imaging. A user interface was developed for man-in-the-loop control of the device using real-time MR images. The user interface fuses all sensor signals (MR and manipulator information) in a visualization, planning, and control command environment. Path planning is performed with graphical tools for setting the trajectory of insertion of the interventional tool using multislice and/or three dimensional MR images which are refreshed in real time. The device control is performed with an embedded computer which runs real-time control software. The manipulator compatibility with the MR environment and image-guided operation was tested on a 1.5 T MR scanner.
Development of a Novel Robot for Transperineal Needle Based Interventions: Focal Therapy, Brachytherapy and Prostate Biopsies

Abstract:PURPOSE: We report what is to our knowledge the initial experience with a new 3-dimensional ultrasound robotic system for prostate brachytherapy assistance, focal therapy and prostate biopsies. Its ability to track prostate motion intraoperatively allows it to manage motions and guide needles to predefined targets.MATERIALS AND METHODS: A robotic system was created for transrectal ultrasound guided needle implantation combined with intraoperative prostate tracking. Experiments were done on 90 targets embedded in a total of 9 mobile, deformable, synthetic prostate phantoms. Experiments involved trying to insert glass beads as close as possible to targets in multimodal anthropomorphic imaging phantoms. Results were measured by segmenting the inserted beads in computerized tomography volumes of the phantoms.RESULTS: The robot reached the chosen targets in phantoms with a median accuracy of 2.73 mm and a median prostate motion of 5.46 mm. Accuracy was better at the apex than at the base (2.28 vs 3.83 mm, p <0.001), and similar for horizontal and angled needle inclinations (2.7 vs 2.82 mm, p = 0.18).CONCLUSIONS: To our knowledge this robot for prostate focal therapy, brachytherapy and targeted prostate biopsies is the first system to use intraoperative prostate motion tracking to guide needles into the prostate. Preliminary experiments show its ability to reach targets despite prostate motion.

Abstract:Effective radiation therapy aims to maximize the radiation dose delivered to the tumor, while minimizing damage to the surrounding healthy tissues, which can be a challenging task when the tissue–tumor space is small. To eliminate the damage to healthy tissue, it is now possible to inject biocompatible hydrogels between cancerous targets and surrounding tissues to create a spacer pocket. Conventional methods have limitations in poor target visualization and device tracking. In this article, we leverage our MR-tracking technique to develop a novel injection needle for hydrogel spacer deployment. Herein, we present the working principle and fabrication method, followed by benchtop validation in an agar phantom, and magnetic resonance imaging (MRI)-guided validation in tissue-mimic prostate phantom and sexually mature female swine. Animal trials indicated that the spacer pockets in the rectovaginal septum can be accurately visualized on T2-weighted MRI. The experimental results showed that the vaginal–rectal spacing was successfully increased by 12 $ pm {m{ }}$2 mm anterior–posterior.

Abstract:ABSTRACT Background In‐bore MRI prostate biopsy offers improved visualisation and detection of significant prostate cancer; however, it is not widely practiced in cancer diagnosis due to its associated costs. Methods This work introduces the first prototype towards a 7‐degrees‐of‐freedom (DOF) MRI‐conditional piezoelectrically actuated robotic system for transperineal prostate biopsy. The robot enables needle insertions in the desired trajectories. Kinematic and static models of the active needle as well as automated control of the robot are presented. Results It is shown that the controller can force the needle to realize the reference sine and triangular bending angles with an accuracy of 1.78 and 1.88°, respectively, in air. The trajectory tracking capability of the system in free space is shown with an RMS error of 0.86 mm and a standard deviation of 0.36 mm. Conclusions The robot's capability to steer the needle towards target inside a phantom and extract a sample was evaluated.

Abstract:In contemporary brachytherapy procedures, needle placement at the desired target is challenging due to a variety of reasons. A robot-assisted brachytherapy system can improve the needle placement and seed delivery resulting in enhanced patient care. In this paper we present a 16 DOF (degrees-of-freedom) robotic system (9DOF positioning module and 7 DOF surgery module) developed and fabricated for prostate brachytherapy. Techniques to reduce needle deflection and target movement have been incorporated after verifying with extensive experiments. Provisions for needle motion and force feedback have been included into the system for improving the robot control and seed delivery. Preliminary experimental results reveal that the prototype system is quite accurate (sub-millimeter) in placing brachytherapy needles.

Abstract:Needle manipulation with the guidance of magnetic resonance imaging (MRI) plays a key role in minimally invasive procedures such as biopsy and ablation. However, the confined bore and strong magnetic field of the MR environment pose great challenges in developing a robotic system that fulfills the needle manipulation function. This paper presents the design and analysis of a soft needle manipulator (SoNIM) that can achieve needle manipulation in the MR environment. This pneumatically actuated manipulator consists of two bending actuators and one elongation actuator that are completely made of non-magnetic materials. These soft pneumatic actuators can generate flexible movements while maintaining a compact design, ensuring that the SoNIM is accommodated within the MRI bore. The kinematic modeling and closed-loop control of the SoNIM are investigated to achieve the position control of the needle tip. Experiments showed that the SoNIM was capable of directing the needle tip to reach the targets with a satisfactory accuracy of 2.9 ± 0.98 mm. Furthermore, the functionality and MRI compatibility of the SoNIM were validated in the clinical setting, demonstrating the capability of the SoNIM to perform needle manipulation in the MRI bore with negligible degradation to the image quality. With excellent MRI compatibility, compact design, and flexible movements, the SoNIM provides a promising solution for manipulating surgical needles in MRI-guided minimally invasive surgeries.

Abstract:The MR-compatible robot for needle insertion has attracted increasingly attentions due to high precision in minimally invasive surgery. A robotic system for prostate brachytherapy has been designed and is actuated by five ultrasonic motors to realize needle orientation and insertion. The application of cable transmission leads to a compact and dexterous mechanism, because all the motors are placed in the base and the moving part of the mechanism becomes as light and small as possible. What’s more, placing the motors far away from the center of the scanner will eliminate artifacts and distortion of the MR images caused by the motors. The displacement analysis is carried out and the reachable workspace is obtained. Jacobian matrix is deduced in velocity mapping model for dimensional synthesis in further study. Configuration singularities are analyzed on the basis of Jacobian matrix.

Abstract:Needle-based percutaneous prostate interventions include biopsy and brachytherapy and the former is the gold standard for the diagnosis of prostate cancer and the latter is often used in the treatment of prostate cancer. This paper introduces a novel robotic assistant system for prostate intervention and the system architecture and workflow are described, which is significant for the design of similar systems. In order to offer higher precision and better real-time performance, a Ultrasound (US)/Magnetic Resonance Imaging (MRI) fusion method is proposed to guide the procedures in this study. Moreover, image registration is a key step and a hot issue in image fusion, especially in multimodal image fusion. In this work, we adopt a novel registration method based on active demons and optic flow for prostate image fusion. To verify the availability of the system, we evaluate our approach of the US/MRI image fusion by using data acquired from six patients, and root mean square error (RMSE) for anatomical landmarks is 3.15mm. In order to verify the accuracy and validity of the system developed in this paper, a system experimental platform was built and used for bionic tissue puncture of prostate under the guidance of MR and Transrectal Ultrasound (TRUS) fusion images. The experimental results show that the deviations of the final actual needle points of the three target points on the bionic tissue model measured in the laboratory environment are less than 2.5mm.

Abstract:In this paper we present a semi-automated device designed and developed to deliver radio-active seeds for treating prostate cancer. In the brachytherapy procedure a slander needle is inserted through the perineum and passed through different types of tissues. Thus, the needle experiences significant amount of force which may cause it to buckle and bend. In our design, we have considered the buckling force and insertion force on needle by collecting in-vivo data from real patient and performing in-vitro experiments. Techniques to reduce force and organ/tissue deformation have been implemented into this new design. To track the axial force on the needle for detecting pubic arch interference and to improve robotic control, we have incorporated three force sensors. Rigidity and factor of safety of the device has been analyzed using finite element method which was very useful for iterative design process.
Vivo motion and force measurement of surgical needle intervention during prostate brachytherapy

Abstract:In this paper, we present needle insertion forces and motion trajectories measured during actual brachytherapy needle insertion while implanting radioactive seeds in the prostate glands of 20 different patients. The needle motion was captured using ultrasound images and a 6 degree-of-freedom electromagnetic-based position sensor. Needle velocity was computed from the position information and the corresponding time stamps. From in vivo data we found the maximum needle insertion forces to be about 15.6 and 8.9 N for 17 gauge (1.47 mm) and 18 gauge (1.27 mm) needles, respectively. Part of this difference in insertion forces is due to the needle size difference (17G and 18G) and the other part is due to the difference in tissue properties that are specific to the individual patient. Some transverse forces were observed, which are attributed to several factors such as tissue heterogeneity, organ movement, human factors in surgery, and the interaction between the template and the needle. However, theses insertion forces are significantly responsible for needle deviation from the desired trajectory and target movement. Therefore, a proper selection of needle and modulated velocity (translational and rotational) may reduce the tissue deformation and target movement by reducing insertion forces and thereby improve the seed delivery accuracy. The knowledge gleaned from this study promises to be useful for not only designing mechanical/robotic systems but also developing a predictive deformation model of the prostate and real-time adaptive controlling of the needle.

Abstract:Traditional hand-held ultrasound probe has some limitations in prostate biopsy. Improving the localization and accuracy of ultrasound probe will increase the detection rate of prostate cancer while biopsy techniques remain unchanged. This paper designs a manipulator for transrectal ultrasound probe, which assists doctors in performing prostate biopsy and improves the efficiency and accuracy of biopsy procedure. The ultrasound probe manipulator includes a position adjustment module that can lock four joints at the same time. It reduces operating time and improves the stability of the mechanism. We use the attitude adjustment module designed by double parallelogram RCM mechanism, the ultrasound probe can realize centering and prevent its radial motion. The self-weight balance design helps doctors operate ultrasound probe without weight. Using MATLAB to analyze the manipulator, the results show that the workspace of the mechanism can meet the biopsy requirements. And simulate the centering effect of the ultrasound probe when the attitude is adjusted at different feeding distances, the results show that the ultrasound probe is centering stability. Finally, the centering and joint interlocking tests of the physical prototype are completed. In this paper, a 7-DOF manipulator for transrectal ultrasound probe is designed. The mechanism is analyzed for kinematics, workspace analysis, simulation of centering effects, development of a physical prototype and related experimental research. The results show that the surgical demand workspace is located inside the reachable workspace of the mechanism and the joint locking of the manipulator is reliable.
In-bore biopsies of the prostate assisted by a remote-controlled manipulator at 1.5 T

Abstract:Purpose: To evaluate the technical and clinical utility of a fully MRI-compatible, pneumatically driven remote-controlled manipulator (RCM) for targeted biopsies of the prostate at 1.5 T. Materials and methods: The data of the first 22 patients that were biopsied under robotic assistance were analyzed. Interventional planning relied on T2-weighted (T2w) turbo spin-echo (TSE) images (axial and sagittal) with a high-b-value diffusion-weighted acquisition added in selected cases. Alignment of the needle guide was controlled with a short balanced SSFP sequence in two oblique planes along the MR-visible sheath. Signals were acquired with a combination of elements from a 30-channel body and a 32-channel spine coil. Biopsy samples were taken with a fully automatic 18-G biopsy gun with a length of 150 or 175 mm. Results: Mean age was 66.6 years and average PSA level was 11.5 ng/ml. Fourteen out of 22 patients (63%) had received prior biopsies under transrectal ultrasound guidance. Diagnostic MRI reports (before biopsy) involved 17 cases with a single suspicious finding (four PI-RADS 3, one PI-RADS 3-4, eight PI-RADS 4 and nine PI-RADS 5 cases). The median effective procedure time was 33.9 (range 25.0-55.9) min for 16 cases with one CSR and 63.4 (52.7-81.8) min for 5 cases with two CSRs. The biopsy with three CSRs took 74.0 min. Histopathologic examination revealed prostate cancer in 14 of 22 cases. Conclusion: MR-targeted, transrectal biopsy of the prostate could be reliably performed with a robotic manipulator at a field strength of 1.5 T. Balanced SSFP imaging is considered a viable option for fast procedural control. Follow-up work needs to evaluate to what extent in-bore adjustments and workflow enhancements will contribute to shorter procedure times or higher patient comfort.

Abstract:This article focuses on the topic of the structural design of surgical radioactive surgery robot for prostate cancer. To improve the weight-to-payload ratio of surgery robot end-effector, the energy consumption and stability of robot joint drive and reducing the displacement and deformation of needle insertion in soft tissue. This article discusses the new static torque balancing method and multi-needle insertion soft tissue stabilization mechanisms that may be used in previously articulated seed implantation robots. Compared with the existing balancing system schemes, we adopt the idea of mutual conversion of gravitational potential energy and elastic potential energy and establish a static balancing model. With preloaded displacement parameter of the spring α, the variable gravity torque balance of robot arm can be achieved. Torque and equivalent gravity balancing distribution with the spring balance system and the quantitative evaluation experiment were performed, and experiment results provide evidence that these spring balance devices can basically compensate the gravity torque of the robot arm. In addition, we used nonlinear spring–damper model to establish multi-needles insertion soft tissue force model. Then, a variable multi-needle insertion soft tissue stabilization device is designed with six working modes. The innovative design of this device is the use of the first four needles that are introduced simultaneously on either side of the midline. Initially completed displacement simulation of different numbers of needle insertion prostate tissue, experiment results indicate that multi-needle puncture mechanism could reduce prostate displacement in the y- or z-direction. By this method, the prostate may be fixed, thus this mechanism maybe reduces rotation of the prostate and enabling subsequent needles to be inserted accurately.

Abstract:This paper discusses several new mechanisms that may be used in prostate cancer seed implant robotics. Aiming at the limitation of human body structure space, and in order to improve robotic kinematic dexterity and execution efficiency. We have developed an articulated seed implantation robot with three-dimensional transrectal ultrasound navigation. It is noteworthy that the organization will produce displacement, deformation and needle tip deflection and other issues while needle inserting prostate. In order to improve positioning accuracy of robot to control the puncture needle, a piezoelectric vibration and rotation needle insertion device is designed and developed. Based on the evaluation experiment of vibration and rotation needle insertion, a high accuracy mixed needle insertion strategy is proposed and the corresponding control software is designed. Finally, the insertion force evaluation experiment is completed by mixed needle insertion strategy, and the experimental results validate the effectiveness of this method.

Abstract:Prostate cancer is the second most frequently diagnosed cancer in the world, and also the sixth leading cause of cancer death in males. Many types of treatment, including active surveillance, surgery, radiation therapy, high-intensity focused ultrasound (HIFU), chemotherapy, cryosurgery, hormonal therapy, and sometimes combination of treatments, are used in prostate cancer therapy. High intensity focused ultrasound (HIFU) technology, which focus ultrasound waves to prostate to locally heat and destroy the target tissue, has been shown to be effective in prostate cancer treatment. In the past few years, HIFU therapy has taken benefit from the development of new technological advances, such as new probe design, new therapeutical planning, imaging guided techniques, which lead to more precise spatial treatment. The current trend in medicine is to perform focal therapy by only treating the tumor area within the organ. So, in the near future, the new HIFU device must be able to locate the tumor area within the prostate and then to focus the ultrasound waves to this specific area.Our Thesis is part of the ANR TecSan MULTIP project (Matrice de transducteurs Ultrasonores pour La Thérapie et l'Imagerie de la Prostate) which aimed at using up-to-date piezoelectric technologies to design dual-mode ultrasonic probes for cancer-foci treatment and monitoring and also to propose image processing methods permitting the design of the ultrasonic imaging/therapy process.
