Abstract:Objective To investigate etiology,risk factors,treatment strategy and prognosis of endogenous endophthalmitis.Methods This study included 11 eyes of 11 patients diagnosed with endogenous endophthalmitis and treated at the First Affiliated Hospital,:Zhejiang University from 2007 to 2010 were retrospectively reviewed.Features studied included patients' systemic predisposing factors,culture results of vitreous,treatments and prognosis.Results For the 11 cases of endogenous endophthalmitis,there were 8 patients who had risk factors,including liver abscess in 3 cases (27.3％),1 case (9.1％) after liver transplantation,1 case (9.1％) after renal transplantation (9.1％),1 case (9.1％) was pneumonia,1 case (9.1％) was diabetic patient (9.1％),1 case (9.1％) was fungal skin infection.All the patients underwent vitreous bacterial,fungal culture and susceptibility testing,there were 9 (81.8％) positive cases for vitreous culture,in which 6 cases (66.7％) for bacterium and 3 cases (33.3％) for fungus.Nine patients received vitrectomy and systemic therapy,2 patients received intraocular injection and systemic therapy.Finally,there were 7 (78％) eyes with improved visual acuity,2 (18％) eyes were atrophy,and 2 (18％) eyes eviscerated or enucleated.Conclusions Most of the patients with endogenous endophthalmitis have systemic predisposing factors,early diagnosis,and effective anti-inflammatory and anti-infection treatment,line the vitreous injection and / or vitrectomy surgery in a timely manner,can save most of the patient's vision.

Abstract:Purpose: To summarize the characteristics of endogenous fungal endophthalmitis (EFE) after genitourinary procedures. Methods: Medical records of patients diagnosed with EFE after genitourinary procedures from a single center during a 6-year period were reviewed. Results: Nineteen eyes of 15 patients were included. The interval time between procedure to symptom was 3.6 +/- 3.6 weeks. As the initial treatment, 9/19 eyes underwent primary vitrectomy and 10/19 eyes underwent intravitreal antifungal injection . Candida albicans was the pathogen in 15 of 19 eyes. Systemic treatment with itraconazole was used in all patients. LogMAR best corrected visual acuity improved from 2.2 +/- 0.9 to 0.9 +/- 1.2 after treatment (p = 0.002) in 15 eyes that were followed-up for an average of 4.9 +/- 2.1 years. Conclusion: Genitourinary procedure is a predisposing factor for EFE. Candida albicans is the predominant pathogen. Normative systemic and local antifungal treatments improved the final visual outcome.
Fungal Endophthalmitis: Clinical Characteristics, Pathogens, and Factors Affecting Visual Outcome

Abstract:Aims: The aims of this study are to investigate the etiology, microbiological spectrum, and risk factors associated with visual outcomes of fungal endophthalmitis (FE) in a tertiary eye specialty hospital in Shanghai, China. Methods: This was a retrospective, single-center case series. The clinical characteristics, etiology, microbiological spectrum, and management, as well as the visual outcomes, were analyzed. Logistic regression was used to analyze the factors related to visual outcomes. Results: This study involved 102 eyes of 92 patients with FE, including 63 males (66.3%). The mean age was 44.4 ± 19.8 years. The most common etiology of FE was trauma (56.5%). The predominant fungal species isolated were Aspergillus spp. (31/93, 33.3%). Pars plana vitrectomy (PPV) and intravitreal antifungal agents was performed initially in 86 (84.3%) and 83 (81.4%) eyes, respectively. Only 35 (34.3%) eyes achieved final best corrected visual acuity (BCVA) of 20/400 or better. Ten (9.8%) eyes had a final BCVA of light perception or worse, and five (4.9%) had to be enucleated. The factors determining better visual outcomes included initial visual acuity better than finger-counting (FC) (odds ratio (OR) 5.811, p = 0.036), the absence of corneal infiltrate (OR 10.131, p = 0.002), and Candida species infection (OR 6.325, p = 0.011). Conclusions: Early diagnosis of FE and a timely vitrectomy, combined with an intravitreal injection of an antifungal drug, can mitigate the devastating results of intraocular fungal infection. Not being infected by Aspergillus spp., an initial BCVA that was no worse than FC, and the absence of corneal involvement were related to better visual prognosis.

Abstract:Purpose: The purpose of this study was to evaluate the predisposing risk factors and treatment outcomes of endogenous fungal endophthalmitis (EFE). Methods: Records of 25 patients (29 eyes), diagnosed with EFE and treated at Shanghai Eye, Ear, Nose Throat Hospital from January 2002 to December 2006, were retrospectively reviewed. Four patients had bilateral disease. Final visual acuity and recurrence of EFE were evaluated. Results: Of these 25 patients diagnosed with EFE, 20 patients (22 eyes) were treated by pars plana vitrectomy. Of the remaining 5 patients (7 eyes), 3 cases (4 eyes) were cured by antifungal drugs through systemic treatment and intravitreal injection, but another 2 cases (3 eyes) were only given intravitreal injection because of severe general conditions. Of the 22 eyes that underwent pars plana vitrectomy, 16 (73%) eyes gained visual acuity of counting fingers or better. Recurrent EFE was happened in 2 eyes. Vitrectomy was repeated in 1 eye, another was subsequently enucleated due to phthisis. Vitreous culture-proven or smear-proven EFE occurred in 27 or 2 eyes, respectively. Candida albicans occurred in 17 of 29 eyes (59%), and other causative organisms were yeast fungus (3 eyes), Aspergillus niger (3 eyes), Actinomyces (2 eyes),Aspergillus flavus (1 eye), and Fusarium (1 eye). Conclusion: Candida albicans were the most common causative organisms in EFE. The most common predisposing risk factors include recent major operation and intravenous administration in rural settings. Most patients with EFE will gain useful vision (counting fingers) after pars plana vitrectomy.

Abstract:Objective To evaluate the clinical features.risk factors and treatment outcomes ofendogenous candida albieans endophthalmitis.Methods The clinical data of 11 patients(18 eyes)withvitreous specimen culture-proven endogenous candida endophthalmitis were retrospective reviewed,including risk factors,clinical features and therapeutic methods and outcomes.Results There were 4males and 7 females patients,aged from 19 to 72 years with a mean age of(41.61±9.76)years.Sevenpatients had bilateral endophthalmitis.They had histories of induced abortion(2 patients),intravenoustransfusion(3 patients),colon cancer surgery(1 patient),chemotherapy after surgery of malignantlymphoma of colon(1 patient),renal transplantation(1 patient),acute necrotic pancreatitis surgery(1patient)and diabetes(1 patient).One patient has no special medical history.All patients had no historyof ocular trauma or intraocular surgery.The major complaints included blurred vision,metamorphopsiaand floaters.It taken an average of(15.23±8.70)days(3-38 days)for patients to go to the hospital aftergetting those symptoms.The main clinical manifestations included pre-or sub-retinal white exudates andvitreous inflammations.In 18 eyes,11 received vitreous surgery,and the other 7 were treated byintravitreal administration of anti-fungal drugs.Ten patients also underwent systemic anti-fungal therapy.The candida endophthalmitis was cured for 10/11 patients and most of them with increased visual acuity.Conclusions Endogenous candida albicans endophthalmitis iS characterized by pre-or sub-retinal whiteexudates and vitreous inflammations.Non-standard intravenous transfusion,induced abortion andmalignancy are its major risk factors.Pars plana vitrectomy or intravitreal delivery of anti-fungal drugs cancure this disease.
The Clinical Features of 19 Endogenous Endophthalmitis Cases

Abstract:Objective To investigate the etiological features and visual outcomes of endogenous endophthalmitis (EE).Methods A retrospective consecutive case series of 19 patients (23 eyes) presenting with presumed EE from 2009 to 2016 in the Ophthalmology Department of Zhongshan Hospital and Huashan Hospital,Fudan University were included.Patients' medical history (main risk factors),microbial culture results,treatment regimen and visual outcomes were evaluated.Results In total,19 patients (23 eyes) were identified,including 7 female (8 eyes) and 12 male (15 eyes).Mean age was 54.0 ±17.9,ranging from 24 to 85.Most patients had severe underlying diseases,mainly diabetic mellitus (9/19) and liver abscess (8/19).Twenty vitreous samples from 17 cases were cultured,and 12 eyes from 10 cases were positive with a positive rate of 12/20.Fifteen cases had blood or body fluids cultures and 10 were positive,with the positivity rate of 10/15.Candida albicans (5/6) was the most common pathogen isolated in fungal endophthalmitis,and Klebsiella pneumonia (7/13) was the most common pathogen in bacterial endophthalmitis.Patients mainly received vitrectomy,intravitreal injection,and systemic/local antibiotic therapy.Only five eyes had improved visual acuities,and 6 eyes were enucleated or eviscerated.Among the 10 eyes from 8 cases who were treated within one week after onset,5 eyes from 3 cases (5/10) had improved visual acuities;one eye from one case had no visual improvement;four eyes from four cases had no light perception or enucleated.No improved visual acuity occurred in patients who were treated after one week.Conclusions Although the incidence is low,EE has poor visual outcomes.Timely diagnosis and treatment may be helpful to maximize the recovery of vision.
Optical Coherence Tomography Features of Retinal Lesions in Chinese Patients with Endogenous Candida Endophthalmitis.

Abstract:BACKGROUND:To evaluate the optical coherence tomography (OCT) features of retinal lesions in Chinese patients with endogenous Candida endophthalmitis (ECE).METHODS:We performed a retrospective review of patients diagnosed with ECE at one medical center. The medical records of the patients including predisposing risk factors, treatment and visual acuity were reviewed. And we focused on the analysis of OCT images of retinal lesions before and after treatment.RESULTS:A total of 16 Chinese patients (22 eyes) were included in this study. The most frequent predisposing risk factors were intravenous use of corticosteroids or antibiotics, lithotripsy for urinary calculi, and diabetes. After treatment, visual acuity was improved in 13 (59.1%) of the 22 eyes, and remained the same in the other 9 (40.9%) eyes. Pre-treatment OCT images obtained at presentation were available for 17 of the 22 eyes. Four types of the OCT manifestations of retinal lesions were identified: type 1 (subretinal macular lesions), type 2 (lesions are located in the inner retinal layer), type 3 (lesions involve the full-thickness retina and accompanied with macular edema), type 4 (sub-inner limiting membrane lesions). Pre-treatment OCT imaging of the 17 eyes revealed five as type 1, four as type 2, six as type 3, and two as type 4. After treatment, OCT images revealed epiretinal membrane and subretinal fibrosis as the most common post-treatment complications of ECE. Epiretinal membrane was detected in 2/4 type 2 lesions, in 4/6 type 3 lesions, and in 1/2 type 4 lesions, while subretinal fibrosis was mainly seen in type 1 lesions (4/5). Among the types, visual prognosis was best in eyes with type 2 lesions.CONCLUSIONS:In this case series, the OCT manifestations of retinal lesions in ECE could be classified into four types. The post-treatment OCT manifestations were different in four types of lesions. We preliminarily found that the OCT morphology of retinal lesions was associated with the visual prognosis of ECE.
Report of endophthalmitis caused by Paradictyoarthrinium diffractum after plant trauma: a case involving left enucleation

Abstract:During outdoor work in April 2022, a 48-year-old man was stabbed by a tree branch and underwent intraocular foreign body extraction and repair of scleral wound with sutures and amniotic membrane graft at a local hospital. Steroid therapy with prednisone was prescribed after a diagnosis of uveitis. Vitrectomy was performed in June 2023; a fungal culture was positive, and ITS sequencing identified the organism as Paradictyoarthrinium diffractum . Empiric antifungal therapy did not have an effect, and, because of deterioration of the condition, the left eye was enucleated in October 2023. P. diffractum is a mangrove host-specific saprophytic fungus that has not been reported in humans.

Endogenous Fungal Endophthalmitis following COVID-19 Hospitalization: Evidence from a Year-Long Observational Study

Abstract:Purpose: To describe cases of endogenous fungal endophthalmitis (EFE) post-recovery from or hospitalization for coronavirus disease 2019 (COVID-19). Methods: This prospective audit involved patients with suspected endophthalmitis referred to a tertiary eye care center over a one-year period. Comprehensive ocular examinations, laboratory studies, and imaging were performed. Confirmed cases of EFE with a recent history of COVID-19 hospitalization±intensive care unit admission were identified, documented, managed, followed up, and described. Results: Seven eyes of six patients were reported; 5/6 were male, and the mean age was 55. The mean duration of hospitalization for COVID-19 was approximately 28 days (14-45); the mean time from discharge to onset of visual symptoms was 22 days (0-35). All patients had underlying conditions (5/6 hypertension; 3/6 diabetes mellitus; 2/6 asthma) and had received dexamethasone and remdesivir during their COVID-related hospitalization. All presented with decreased vision, and 4/6 complained of floaters. Baseline visual acuity ranged from light perception (LP) to counting fingers (CF). The fundus was not visible in 3 out of 7 eyes; the other 4 had "creamy-white fluffy lesions" at the posterior pole as well as significant vitritis. Vitreous taps were positive for Candida species in six and Aspergillus species in one eye. Anti-fungal treatment included intravenous amphotericin B followed by oral voriconazole and intravitreal amphotericin B. Three eyes underwent vitrectomy; the systemic health of two patients precluded surgery. One patient (with aspergillosis) died; the others were followed for 7-10 months - the final visual outcome improved from CF to 20/200-20/50 in 4 eyes and worsened (hand motion to LP) or did not change (LP), in two others. Conclusion: Ophthalmologists should maintain a high index of clinical suspicion for EFE in cases with visual symptoms and a history of recent COVID-19 hospitalization and/or systemic corticosteroid use - even without other well-known risk factors.

Abstract:Purpose To evaluate factors that inform systemic antifungal choices in patients with endogenous fungal endophthalmitis (EFE). Design Single institution retrospective case series. Methods Charts of EFE patients from 2010 to 2023 were reviewed. Patients treated systemically for EFE with a minimum of 14 days of follow-up were included. Outcome measures included time to improvement in vitritis or chorioretinitis, systemic therapy modification, and need for surgical intervention. Results Twenty eyes of 16 patients were included. Candida species were most common (43.8%), followed by culture negative EFE (37.5%) and Aspergillus species (18.8%). Ninety percent of eyes had vitritis and/or macula-involving chorioretinitis. Most Candida infections (60%) or culture negative EFE (75%) were treated initially with oral antifungals. Patients with a history of immune compromise, positive fungal culture or positive Fungitell assay were more likely to be treated with early intravenous (IV) antifungal therapy. Two cases required systemic antifungal therapy modification due to worsening chorioretinitis: in one case due to voriconazole-resistant Aspergillosis that demonstrated chorioretinal lesion growth despite intravitreal amphotericin B injections and systemic voriconazole, and in a second case due to worsening chorioretinitis from C. dubliniensis infection that regressed upon switch from oral to IV fluconazole. Conclusion Initial systemic treatment decisions in patients with EFE were driven by systemic culture positivity, systemic symptoms, or comorbidities. Intravitreal antifungal therapy may be insufficient to arrest progression of chorioretinal lesions in some cases. Larger studies are needed to determine whether visible end-organ damage in the form of chorioretinitis may be useful for guiding systemic therapy changes.

Vitrectomy Combined with Intravitreal Antifungal Therapy for Posttraumatic Fungal Endophthalmitis in Eastern China

Abstract:Background To evaluate the effect and prognostic factors of vitrectomy combined with intravitreal antifungal therapy for posttraumatic fungal endophthalmitis in Eastern China. Methods We retrospectively reviewed the medical records of patients who developed fungal endophthalmitis after penetrating ocular trauma at an ophthalmic center in Eastern China. All patients underwent vitrectomy and intravitreal injection of antifungal drugs. Results Thirty-five patients (35 eyes) were included. Twelve eyes suffered plant trauma, 17 eyes metal trauma, and 6 eyes other trauma. The culture results for all 35 eyes showed filamentous fungi, including Aspergillus in 26 eyes (74.3%). Twenty-three eyes underwent vitrectomy once and 12 eyes were treated twice. Four eyes were iridectomized because of a fungal lesion behind the iris. Fungal endophthalmitis was effectively controlled in 33 eyes (94.3%), whereas 2 eyes were ultimately enucleated. Visual acuity was significantly better after treatment than before treatment (P = 0.0006). According to the preoperative vision, the affected eyes were divided into two groups: group 1A (light perception) and group 1B (better than light perception). The final visual acuity in group 1B was significantly better than that in group 1A (P = 0.0289). Conclusions Vitrectomy combined with intravitreal antifungal therapy is an effective treatment for posttraumatic fungal endophthalmitis. Preoperative visual acuity is a significant factor affecting the prognosis of visual acuity.
Delayed Bacterial Endotheliitis and Endophthalmitis 11 Years after Cataract Surgery

Abstract:Infective endophthalmitis is an uncommon complication following intraocular surgery. Chronic endophthalmitis may present some time after intraocular surgery, making the diagnosis challenging. Cutibacterium acnes is a well-recognised causative agent of these chronic infections. Practitioners should be aware of the conditions required to culture this slow-growing organism. We report a case of delayed low-grade endophthalmitis presenting 11 years after cataract surgery. Cutibacterium acnes and Staphylococcus warneri were cultured from Descemet's membrane biopsy following three failed previous attempts at microbiological studies. Clinical features of the infection included discrete white granules on the iris, endothelium, and within the capsular bag of the patient's right eye. The patient presented with no signs of systemic infection and the left eye was normal on examination. Bullous keratopathy, secondary to endothelial dysfunction was a feature of this infection. This retrospective case report illustrates the prolonged periods for which Cutibacterium acnes can remain latent before causing clinical signs. While uncommon, endothelial involvement may occur and clinicians should consider low-grade infective endophthalmitis in cases with corneal oedema.

Abstract:Purpose: To evaluate the causative organisms of and predisposing medical conditions in endogenous endophthalmitis and review visual acuity after pars plana vitrectomy. Methods: Records of 23 patients (32 eyes) who were diagnosed with endogenous endophthalmitis and treated at Shanghai Eye, Ear, Nose & Throat-Hospital from January 2000 to December 2003 were retrospectively reviewed. Final visual acuity was followed up. Results: Of these 23 patients, 19 (86%) had endogenous endophthalmitis confirmed with a positive smear or culture; 12 cases (63%) were due to fungi, 6 (32%) were due to bacteria, and 1 (5%) was a mixed infection (fungus and bacteria). Culture specimens from four patients, which were obtained by vitrectomy, were all positive, while their initial vitreous needle biopsy specimens were negative. Of the 20 eyes that underwent pars plana vitrectomy, 17 (85%) had anatomical success, and 16 (80%) gained visual acuity of counting fingers or better; of these eyes, 8 (40%) had visual acuity of 20/200 or better. Conclusion: Fungi, especially Candida albicans, were the most common causative organisms. The most common predisposing medical conditions were recent tumor surgery and intravenous administration in rural settings. Most patients with endogenous endophthalmitis who undergo pars plana vitrectomy will have useful vision (counting fingers). Vitreous specimens for culture that were obtained by vitrectomy were more sensitive in detecting the causative organism.
Endogenous Fungal Endophthalmitis in a Patient Admitted to Intensive Care and Treated with Systemic Steroid for COVID-19

Abstract:A 61-year-old woman presented to our clinic with complaints of decreased visual acuity, pain, and redness in her left eye. Best corrected visual acuity (BCVA) was 20/20 in the right eye and counting fingers at 3 meters in the left eye. On slit-lamp examination, 1+ cells were detected in the anterior chamber. Fundus examination revealed 1+ haze in the vitreous and multiple creamy-whitish lesions in the retina and vitreous. Her history included a diagnosis of coronavirus disease 2019 (COVID-19) one month earlier, for which she was hospitalized in the intensive care unit for 20 days and received systemic corticosteroid treatment. Vitreous culture yielded Candida albicans. The patient's nasopharyngeal swab sample was positive for COVID-19 by reverse transcription polymerase chain reaction test. BCVA was improved to 20/40 after amphotericin therapy (via intravitreal injection and intravenous routes), and the vitritis and chorioretinitis lesion regressed after 2 weeks of treatment. Two weeks later, intravenous amphotericin was discontinued and oral fluconazole treatment was started at a dose of 400 mg/day. At 3-month follow-up, her BCVA was 20/25 and no inflammatory reaction was observed in the anterior chamber and vitreous.

Abstract:Objective To discuss related faetorsand treatment of endophthalmitis secondary tophacoemulsification. Methods 6 cases suffered from infective endophthalmitis after cataract surgery from June 2007 to May 2008 were analyzed retrospectively. Results 5 cases were performed vitrectomy with exception of 1 case. All specimens found no fungal mycelium. Culture： 2 cases of Staphylococci, 2 cases of Enterocoecus faeealis, 1 case of pneumo- nococcus, 1 case of none. Follow-up vision： 4 cases of increase, 1 case of no change, 1 case of decline. Conclusion Early detec- tion, early diagnosis and appropriate clinical intervention in time appear to be particularly critical to endophthalmitis secondary to phaeoemulsificatiou.\r\n\r\nKey words: \r\ncataract extraction ; complications

Abstract:A systematic search of the China National Knowledge Infrastructure database for articles on endophthalmitis after cataract surgery between 1995 and 2009 identified 57 articles that included 403 eyes. The incidence of endophthalmitis after cataract surgery was 0.06% (140/233 115). Treatments included intraocular antibiotic injection in 54.1% of eyes and immediate vitrectomy in 40.2%. Of 356 intraocular samples, 214 (60.1%) were culture positive; 187 (87.3%) bacterial and 27 (12.7%) fungal. The presenting visual acuity was <5/200 in 195 (75.3%) of 259 eyes and the final visual acuity, ≥20/40 in 63 eyes (16.8%). During these 15 years, the use of vitrectomy increased significantly (P<.05). The proportion of eyes with a final visual acuity of ≥20/400 also increased significantly (P<.05) during each 5-year period. Although these represent favorable changes in the treatment and prognosis of endophthalmitis, there are differences between these outcomes and those in developed countries. A more unified therapeutic standard is needed to further improve the prognosis of postoperative endophthalmitis in China.
Aspergillus flavus endogenous endophthalmitis in an immunocompetent man: treatment outcomes and swept-source optical coherence tomography findings

Abstract:A non-diabetic man in his 40s presented with a 4-day history of sudden, painless, rapidly progressive severe vision loss in his right eye. Anterior segment was unremarkable. A dense, solitary vitreous abscess with feathery margins obscured the macula. Retinal haemorrhages, exudates and retinal vasculitis were noted. There was no history of ocular surgery, trauma, intravenous drug abuse or immunocompromise. He received intravenous fluids during a spinal procedure 5 days prior. Prompt diagnostic and therapeutic pars-plana vitrectomy was done and Aspergillus flavus was cultured. Postoperatively, he was treated with oral Voriconazole and four intravitreal Voriconazole injections, till the chorioretinitis lesions had completely healed and was objectively confirmed on follow-up swept-source optical coherence tomography (OCT). He achieved a best-corrected visual acuity of 6/36. A high index of suspicion, early vitrectomy and OCT-based treatment were key to favourable outcomes in this case. Intravenous fluid bottles must be checked for contamination prior to administration.

Abstract:Enterococcus faecalis is known to cause severe acute endophthalmitis and often leads to poor visual outcomes in most ophthalmic infections. This retrospective study is to report the clinical settings, antimicrobial susceptibility patterns, and visual outcome of E. faecalis endophthalmitis at a tertiary referral institution in Taoyuan, Taiwan. E. faecalis endophthalmitis was diagnosed in 37 eyes of 37 patients. Post-cataract surgery was the most common cause (n = 27, 73%), followed by bleb-associated (n = 3, 8%), endogenous (n = 2, 5%), corneal ulcer-related (n = 2, 5%), post-vitrectomy (n = 1, 3%), post-pterygium excision (n = 1, 3%), and trauma (n = 1, 3%). Visual acuities upon presentation ranged from counting fingers to no light perception. Pars plana vitrectomy with intravitreal antibiotics were performed in 23 eyes (76%) as primary or secondary treatment. All isolates (37/37, 100%) were sensitive to vancomycin, penicillin, ampicillin, and teicoplanin. Six of 22 eyes (27%) were resistant to high-level gentamicin (minimum inhibitory concentration > 500 mg/L). Final visual acuities were better than 20/400 in 11 eyes (30%), 5/200 to hand motions in 4 eyes (11%), and light perception to no light perception in 22 eyes (59%). Three eyes were treated with evisceration. Compared with non-cataract subgroups, the post-cataract subgroup showed a significant difference of better visual prognosis (p = 0.016).
