Abstract:This article poses the question: Is elder abuse and neglect a social problem, showing that it is. Elder abuse, though, is still the most hidden form of mistreatment and a key to governmental responses to an ageing population. It is an important facet as a family violence problem, an intergenerational concern, as well as a health, justice and human rights issue. Because the phenomenon of elder abuse and neglect is so complex and multi-dimensional, it has to be addressed by multi-professional and inter-disciplinary approaches. Raising awareness is a fundamental prevention strategy and an important step in causing changes in attitudes and behaviors. This has been accomplished by INPEA and the article was developed from the lecture given by the author on receiving the International Rosalie Wolf Award from INPEA. The discussion focuses on elder abuse as a product of global ageing, stemming from population ageing, which is consistent with an increased prevalence of abuse of all vulnerable groups, older people among them. It is pointed out that baseline and trend data on the nature and prevalence of senior abuse are crucial to policy responses and the development of appropriate programs and services. Difficulties in assessing the scope of the phenomenon, though, are due to: problems in definitions and methodology, which create difficulties in comparing data from various countries; lack of social and familial awareness; isolation of some elders, especially migrants; elder abuse as a 'hidden issue' that usually occurs in the privacy of the home and is viewed as a family affair; limited access to institutional settings. Difficulties also exist in constructing a unifying research framework in order to study the phenomenon due to a lack of comparison groups, a lack of representative national surveys and difficulties in measurement. There is currently, however, an increase in prevalence and incidence studies from both sides of the Atlantic and especially from Europe. But while prevalence studies provide base-data on numbers, little is known about key conceptual issues for policy, practice and the understanding of different forms of abuse and neglect. Theoretical under-development hampers the collection of systematic cumulative knowledge which is based on universally agreed upon and standardized tools, and reduces the ability to discover unifying themes and their relationship to local idiosyncrasies existing in the field. Additionally, there has been no attempt to develop theoretical knowledge grounded in data from the study of elder abuse itself. The following vehicles for action are, thus, suggested: Developing international, national and regional research agendas and data bases; developing policy and legislation; developing services and interventions and developing educational programs.

Evidence suggests that 1 out of 10 older adults experiences some form of elder abuse, and only a fraction of cases are actually reported to social services agencies. At the same time, elder abuse is independently associated with significant morbidity and premature mortality. Despite these findings, there is a great paucity in research, practice, and policy dealing with this pervasive issue. In this paper, I review the epidemiology of elder abuse as well as key practical issues in dealing with the cases of elder abuse. Through my experiences as a Congressional Policy Fellow/National Health and Aging Policy Fellow, I highlight key previsions on 2 major federal legislations dealing with the issues of elder abuse: Older Americans Act (OAA) and Elder Justice Act (EJA). Lastly, I highlight major research gaps and future policy relevant research directions to advance the field of elder abuse. Interdisciplinary and community-based efforts are needed to devise effective strategies to detect, treat, and prevent elder abuse in our increasingly diverse aging populations. Collective advocacy and policy advances are needed to create a national infrastructure to protect the vulnerable older adults.

Abstract:This article is based on the lecture for the 2014 American Geriatrics Society Outstanding Scientific Achievement for Clinical Investigation Award. Elder abuse is a global public health and human rights problem. Evidence suggests that elder abuse is prevalent, predictable, costly, and sometimes fatal. This review will highlight the global epidemiology of elder abuse in terms of its prevalence, risk factors, and consequences in community populations. The global literature in PubMed, MEDLINE, PsycINFO, BIOSIS, Science Direct, and Cochrane Central was searched. Search terms included elder abuse, elder mistreatment, elder maltreatment, prevalence, incidence, risk factors, protective factors, outcomes, and consequences. Studies that existed only as abstracts, case series, or case reports or recruited individuals younger than 60; qualitative studies; and non-English publications were excluded. Tables and figures were created to highlight the findings: the most-detailed analyses to date of the prevalence of elder abuse according to continent, risk and protective factors, graphic presentation of odds ratios and confidence intervals for major risk factors, consequences, and practical suggestions for health professionals in addressing elder abuse. Elder abuse is common in community-dwelling older adults, especially minority older adults. This review identifies important knowledge gaps, such as a lack of consistency in definitions of elder abuse; insufficient research with regard to screening; and etiological, intervention, and prevention research. Concerted efforts from researchers, community organizations, healthcare and legal professionals, social service providers, and policy-makers should be promoted to address the global problem of elder abuse.

I will also highlight major research gaps and future policy relevant research directions for the field of elder abuse.

Abstract:The history of policy related to elder abuse spans more than 50 years and reflects an evolving understanding by policymakers and the general public that rather than being a private sphere, family matter problem, elder abuse is a public sphere problem. In fact, it is an issue of national import, touching such sectors as social services, healthcare, law enforcement, and banking. As a social justice issue, societal recognition of and action concerning elder abuse has been both protracted and circuitous. Although elder abuse dates back to the beginning of human history, the problem has gained traction relatively recently. Given the attention paid to child abuse and intimate partner violence against women, the historical lack of political will to take action to prevent and punish elder abuse is surprising (Teaster et al. 2010).

Abstract:Elder abuse is increasing recognized as a global health concern.At the foundation of this public health issue, elder abuse incidence, risk factors and its clinical implications remain poorly understood, particularly in minority aging populations.Further population-based epidemiological studies using standard elder abuse measurements with longitudinal study design is required to advance the current literature.To fill in the knowledge void, the purpose of this symposium is to improve our understanding of elder abuse epidemiology on incidence, psychological and behavioral risk factors, and consequences using cross-sectional and longitudinal data from the Population-based Study of Chinese Elderly in Chicago (PINE).Currently in its third wave of data collection, the PINE study gathers detailed information about elder abuse extent, subtypes, frequency, intensity, and self-perceived seriousness of the program.Based on the PINE study data, in Session 1, we will investigate the two-year incidence of elder abuse and its subtypes in a representative, population-based sample of U.S. Chinese older adults.Session 2 will examine the association between specific cognitive domains and risk of subtypes of elder abuse.Session 3 will explore psychological and behavioral health burdens as risk factors for elder abuse.Session 4 will discuss the consequences of elder abuse and health services utilization.In the last session, we will investigate the association between elder self-neglect and suicidal ideation.In summary, this symposium aims to increase the practical and clinical relevance of elder abuse epidemiology in various community, research, healthcare, and policy settings
ELDER ABUSE AND NEGLECT IN URBAN AND RURAL AREAS IN CHINA: PREVENTION STRATEGIES

Abstract:Abstract With acceleration of the ageing population globally, more and more governments are concerned about the potential increase in elder abuse and neglect (EA/N). Recently the National Office for Ageing and the provincial offices for ageing conducted a survey of 224,352 Chinese over the age of 60 years using household interviews to assess economy, health, service, social participation, culture, rights protection, livable environment, etc. Author’s analysis of this data shows that 54% of the elderly people interviewed experienced physical and mental abuse or intimidation, and 6.95% of them felt that their legal rights were violated. Data also supports that the occurrence of EA/N was significantly correlated to self-care ability, economic status, and urban and rural regions of the elders. The researcher will discuss the practice and policy implications for the prevention of EA/N.

Abstract:Recognition of elder abuse and neglect among health care professionals has been a relatively recent phenomenon. Each year, millions of elderly persons suffer as the result of abuse and neglect. Their quality of life is severely jeopardized in the form of worsened functional status and progressive dependency, poorly rated self-health, feelings of helplessness, and from the vicious cycle of social isolation, stress and further psychologic decline. Other medical implications of abuse and neglect include higher health systems use in the form of frequent ER visits, higher hospitalization, and higher nursing home placement; most importantly, it is an independent predictor for higher mortality. Physicians are well situated in detecting and reporting suspected cases and taking care of the frail elders who are victims of abuse and neglect, but there are barriers on the individual level, and there is a broader need for system change. Through education, training, and reinforcement, there are strategies to get health care professionals more involved and provide effective management protocols and guidelines for us to advocate for our patients in the current epidemic of elder abuse and neglect.

Abstract:Purpose This paper aims to explore the trends in academic research on elder abuse from 1990 to 2023 using bibliometric analysis. It seeks to identify research trends, hotspots and gaps and proposes future research directions. Design/methodology/approach Using bibliometric analysis method, this study analyzes 2,404 documents related to elder abuse from the Scopus database. Visual analysis is conducted using VOSviewer software to reveal research trends, thematic clusters and their interrelationships. Findings The study shows a rising concern for elder abuse, especially in nursing homes, domestic settings and among dementia patients. However, research on prevention and intervention measures is lacking, despite increasing international collaboration. Yet, deeper exploration of cross-cultural and regional differences remains limited. Practical implications This study reveals that improving care conditions for nursing home residents and dementia patients requires increased funding, professional training for caregivers, the strengthening of regulations and the establishment of clear guidelines for reporting abuse. Additionally, promoting international cooperation, sharing best practices, raising public awareness and supporting ongoing research are essential measures to ensure the safety and dignity of older adults. Originality/value To the best of the authors' knowledge, this study represents the first systematic review of elder abuse research using bibliometric analysis, providing researchers and policymakers with a comprehensive knowledge framework of the field's development trends and research hotspots.
Elder Abuse and Dementia: A Review of the Research and Health Policy

Abstract:Older adults with dementia may be at high risk for abuse, but the topic has not been well studied. We conducted a literature review to examine the relationships between elder abuse and dementia. We found that psychological abuse was the most common form of abuse among older adults, with estimates of its prevalence ranging from 27.9 percent to 62.3 percent. Physical abuse was estimated to affect 3.5-23.1 percent of older adults with dementia. We also found that many older adults experienced multiple forms of abuse simultaneously, and the risk of mortality from abuse and self-neglect may be higher in older adults with greater levels of cognitive impairment. We summarize programs and policies related to the abuse of older adults with dementia, including adult protective services, mandatory elder abuse reporting, and the Long-Term Care Ombudsman Program. We also summarize aspects of the National Alzheimer's Project Act, the Older Americans Act, and the Elder Justice Act. In spite of a recent increase in research and policy developments on elder abuse, challenges such as insufficient funding, limited knowledge about elder abuse, a lack of funding for the implementation of federal and state programs relevant to elder abuse and dementia, and a lack of dementia-specific training for front-line health care staff persist. Stronger programs targeting the well-being of older adults with dementia are needed.
Culture Diversity and Elder Abuse: Implications for Research, Education, and Policy

Abstract:Elder abuse is a pervasive public health issue-a challenge that cries out for culturally appropriate training and education to enable practice and policy changes that will best protect our increasingly diverse aging population.Elder abuse is a substantial global public health issue. The World Health Organization has declared that elder abuse is a violation of one of a human being's most basic fundamental rights: the right to be safe and free of violence (World Health Organization, 2002). In the United States, an estimated 10 percent of elders experience abuse each year, and many of them experience it in multiple forms (Beach et al., 2010; Acierno et al., 2010). In addition, elder abuse is associated with increased risk of premature morbidity and mortality (Dong et al., 2009; Dong et al., 2011a; Dong, Simon, and Evans, 2011; Dong et al., 2011c). Despite the accessibility of Adult Protective Services (APS) and nursing home regulations in all fifty states, an overwhelming number of abused older adults pass through our healthcare system undetected and untreated.A major complexity in advancing the field of elder abuse is exemplified by the issues of cultural diversity surrounding elder abuse. In 2003, the National Research Council put forth a strong recommendation to urge the field to explore cultural issues related to elder abuse. In 2010, the National Academy of Sciences and the National Institute on Aging organized a state-ofscience meeting on research issues in elder mistreatment and financial fraud, including discussion of cultural diversity (The National Academies Committee on National Statistics, 2010). Despite these reports and the continued effort of multiple disciplines across academic, community, state, and federal organizations, there remain vast gaps in our understanding of cultural issues in elder abuse.In the United States, the aging population (ages 65 and older) represents approximately 40 million (12.9 percent) of the population; by 2030, there will be about 72.1 million older people, more than twice the number in 2000. In 2010, approximately 20 percent of people ages 65 and older were minorities; 8.4 percent were African American; 6.9 percent were of Hispanic origin; 3.5 percent were Asian or Pacific Islander; and 1 percent were American Indian or Native Alaskan (Administration on Aging, 2012).Recent studies have expanded our knowledge about elder abuse in African American, Latino, Korean, Indian, Native American, and Chinese populations. However, we need more studies to fill in the large gaps in our knowledge. We need quantitative and qualitative studies to better define the concept and cultural variations in the construct, definition, and understanding of elder abuse; and we need cultural explorations to better study the barriers to reporting elder abuse and help-seeking behaviors with respect to the specific sociocultural contexts. Studies also are needed to understand the prevalence, incidence, risk/protective factors, and consequences associated with incident cases of elder abuse and its subtypes in diverse populations. And we must have research to explore the issues of cultural norms and cultural expectations in relation to the perception, determinants, and impact of elder abuse in different racial and ethnic communities.A Call for Community-Based Participatory ResearchSignificant challenges exist in the preparation and conduct of aging research in minority communities, especially regarding culturally sensitive issues such as elder abuse. A communitybased participatory research (CBPR) approach could be a potential model for exploring the issues of elder abuse in minority communities.CBPR necessitates equal partnership between academic institutions with community organizations and key stakeholders to examine the relevant issues. Global Research Activity on Elder Abuse: A Bibliometric Analysis (1950–2017)

Abstract:Elder abuse is an emerging worldwide public health, human right, and social priority for governments and health policy makers. The aim of the current study was to provide an in-depth quantitative analysis of literature on elder abuse published in academic journals. A bibliometric method was implemented using Scopus database for the study period from 1950 to 2017. The search strategy utilized specific keywords to retrieve relevant documents. One thousand eight hundred seventy-two documents appeared in Scopus when using the search strategy. The annual number of publications showed a fluctuating pattern in the past four decades. Publications on elder abuse originated mainly from Northern America and Western Europe. International research collaboration on elder abuse was relatively low. The mean number of authors per document was 1.4. The <i>Journal of Elder Abuse and Neglect</i> published almost one third of the retrieved documents. The <i>Rush University</i> was the most active institution and Professor Dong, X.Q. was the most active author in this field. The most frequently encountered keywords were risk factors, prevalence, intervention, prevention, dementia, and nursing homes. Physical abuse was the most common type of elder abuse studied followed by psychological and financial abuse. Elder abuse is under-researched and of limited priority in most world regions. Governments need to take into consideration preventive policies of elder abuse based on research findings.

Strategies to build more effective interventions for elder abuse: a focus group study of nursing and social work professionals in Hong Kong

Abstract:Background: One in six older adults living in communities experience abuse and neglect. Elder abuse has serious consequences for individuals, families, and society, including mortality, physical and psychological morbidities, and increased care requirements. Timely and effective interventions for elder abuse should therefore be a priority. This study used a qualitative focus group approach to address the following questions: What are the essential elements of elder abuse interventions? What can be done to improve current interventions? Method: The 32 participants in this focus group study included social workers, medical social workers, and nurses from seven organizations who shared their knowledge and insights. All sessions were conducted online, audio-recorded, and transcribed verbatim. Three researchers with backgrounds in social work and psychology independently coded the transcripts and agreed on the themes emerging from the focus groups. Results: Based on the experiences of frontline helping professionals in Hong Kong, we highlighted the key factors for effective elder abuse intervention: 1) identification and assessment; 2) essential skills and attitudes; 3) elements of effective interventions; 4) collaborative efforts across disciplines and agencies; and 5) raising awareness among professionals and the public. Conclusions: Training can equip frontline professionals with the necessary skills to identify elder abuse cases and to assess the risk of abuse. Effective interventions should not only address clients' safety and need for tangible support but also respect their autonomy and privacy. A client-centered, strength-based approach that involves supportive peers and addresses the complex family relationships involved can be useful. Interventions should also involve cross-discipline and cross-agency collaboration.
Elder Abuse in Chinese Populations: A Global Review

Abstract:This review focuses on the epidemiology of elder abuse in the global Chinese population with respect to its prevalence, risk factors, and consequences, as well as the perceptions of elder abuse. Evidence revealed that elder abuse and its subtypes are common among the global Chinese population with prevalence ranging from 0.2% to 64%. Younger age, lower income levels, depression, cognitive impairment, and lack of social support were consistently associated with self-reported elder abuse. Caregiver burden was a constant risk factor for the proclivity to elder abuse by caregivers. The adverse health outcomes of elder abuse included suicidal ideation and psychological stress. Some primary research gaps exist: such as, lack of consistency in measurements and recall periods, insufficient studies on the causal relationships between potential risk factors and elder abuse, consequences of elder abuse, and possible interventions. In order to reduce the risk of elder abuse in the global Chinese population, collaboration is encouraged among researchers, health care professionals, social service providers, and policy makers.

Abstract:This is the study on abuse of the elderly has been and nursing intervention. Abuse of the elderly has been neglected by both professionals and the public.Although there has been a dramatic increase in the attention to this problem in the past decade, many service providers are still unable to deal with elder abuse in an adequate manner.In the past elder abuse has been viewed with child abuse and spouse abuse in the general category of family abuse. This situation can be shown in several ways, textbooks on family violence tend to include elder abuse as one of the categories. Theoretical models are often derived from general family viol ence theories, focusing od such factors as the psychopathology of abusers, or on situational issues, such as general family stress or the specific stress involved in care of dependent family members.Technique for identifying elder abuse are often borrowed from child abuse identification techniques.Furthermore, elder abuse is categorized as a criminal act, requiring that penalties be imposed on those re sponsible for the abuse.More recently, some specialists who deal with the problem are beginning to suggest that elder abuse is the wrong issue. These analysts point out that much of what is termed elder abuse is more accurately termed neglect and many indeed be unintentional on the past of the so-called abuser,who rnay be overwhelmed by the responsibilities of caring for a dependent aged person. Previous research has indicated that elder abuse actually constitutes a variety of behaviors, all of which threaten the health, comfort, and possibly the lives of elderly people. However, the nature of the threat and its effect on aged persons' health and comfort may take different forms. Research on elder abuse has focused on a broader range of behaviors than other family abuse, including six major categories of abuse and neglect : psychological or emotional neglect, psychological or emotional abuse, violation of personal rights, financial abuse, physical neglect, direct physical abuse.As Block and Sinnott i)oint out, strategies of intervention must be developed primarily to end the abuse, but secondarily to help in establishing the physical and mental well-being of the entire family.Further research is necessary to determine the needs of aged abuse victims and the most appropri ate ways to provide for them.The key element in the provision of high quality care to elder abuse victims is the establishment of a trusting and insightful relati이isiiip between the victim and the primary care provider. In large past, education of the public and of those providing health, social, and psychological intervention will facilitate this process, other care providers shculd work through this trusted professional. All components of the care must be recorded so that all professionals can follow the progress of the client and achieve continuity in care Nurses willing to accept this challenge may gain satisfaction from the knowledge that they are forerunners in the developmenr od methods of assistance to elderly abuse victims who have heretofore been neglect by other professionals in the health care area.
