Abstract:Background: Immunologic dysfunction due to coronavirus disease 2019 (COVID-19) is closely related to clinical prognosis, and the inflammatory response of pregnant women may affect the directional differentiation and function of fetal immune cells. Objective: We sought to analyze the immune status of newborns from mothers with COVID-19 in the third trimester. Methods: Along with collecting the clinical data from 51 newborns and their respective mothers, we recorded the immunophenotypes and cytokine and immunoglobulin levels of the newborns. Results: None of the 51 newborns showed fever or respiratory distress during hospitalization. Detection of severe acute respiratory syndrome coronavirus 2 nucleic acid in pharyngeal swabs was negative. Except for the low level of CD16-CD56 cells, the count and proportion of lymphocytes, CD3, CD4, CD8, and CD19 were all in the normal range. Moreover, the serum IgG and IgM levels were within the normal range, whereas IL-6 showed increased levels. There was no correlation between maternal COVID-19 duration and the lymphocyte subsets or cytokine levels (IFN-gamma, IL-2, IL-4, IL-6, IL-10, and TNF-alpha). There was a positive correlation between IL-6 and IL-10 levels and CD16-CD56 cells. One (1.96%) infant with an extremely elevated IL-6 concentration developed necrotizing enterocolitis in the third week after birth, and the remaining 50 infants did not show abnormal symptoms through the end of the follow-up period. Conclusions: COVID-19 in the third trimester did not significantly affect the cellular and humoral immunity of the fetus, and there was no evidence that the differentiation of lymphocyte subsets was seriously unbalanced.

Abstract:We report the detection and decline over time of severe acute respiratory syndrome coronavirus 2 antibodies in infants born to women with coronavirus disease. Among 11 infants tested at birth, all had detectable IgG and 5 had detectable IgM. IgG titers with positive IgM declined more slowly than those without.

Abstract:Owa and Owa inquired about the serologic tests of the 3 neonates and the treatment of neonates with COVID-19.The serologic tests for SARS-CoV-2 were not available until the end of February 2020.Currently, only 6 infected neonates have been reported in China.Four mildly infected neonates improved with primary care.Of 2 critical cases in China, 1 was the third patient in our article, and another neonate with congenital heart disease (6.9-mm atrial septal defect) presented with pneumonia and cardiac insufficiency.The neonate improved rapidly with antibiotics, inotropic therapy, and fluid management.None of the infected neonates received antiviral therapy.Verd et al raised the issue of whether mothers with COVID-19 should continue breastfeeding.Viral nucleic acid for SARS-CoV-2 has not been detected in breast milk thus far.However, the neonates born to an affected mother are at great risk of COVID-19 owing to close contact and should be separated from mothers and fed by formula initially.As our protocol suggested, the isolated mothers should keep pumping to maintain breast milk. 3 Considering the virus may be excreted into the milk during the incubation period, donor milk and breast milk can be considered for use after being screened for SARS-CoV-2. 3 Mimouni et al questioned the criteria used to diagnose COVID-19 pneumonia.In our study, 3 neonates with COVID-19 had not been reported in other cohorts before publication.Patients 1 and 2 were born February 3, 2020, and February 5, 2020, respectively.Patient 1 was admitted to the neonatal intensive care unit owing to lethargy and fever (37.6 °C) at day 3 of life (February 5, 2020).His procalcitonin level (PCT) was 0.090 ng/mL (normal range, ≤0.05 ng/mL).Patient 2 experienced lethargy, vomiting, and fever (37.7 °C), and laboratory test results showed leukocytosis, lymphocytopenia, and an elevated creatine kinase myocardial band.Although no respiratory symptoms were observed and chest images showed infiltrates, the 2 neonates were diagnosed as having symptomatic neonatal COVID-19 based on the exposure history and positive SARS-CoV-2 test result. 3,4 Currently, less than 10 confirmed neonatal cases have been reported.Therefore, the full clinical picture of neonatal COVID-19 remains uncertain.Hijikata et al, Abu Raya et al, and Vecchio pointed out that the serial serologic tests were helpful to diagnose congenital COVID-19.We do agree that viral or serologic test results cannot confirm the intrauterine SARS-CoV-2 infection.However, at the early phase of the COVID-19 epidemic in China, the testing methods for SARS-CoV-2 were limited.Initially, real-time fluorescence PCR was only available for SARS-CoV-2 using nasopharyngeal and oropharyngeal specimens.At present, both serological testing and RT-PCR can be performed; yet, no more neonates with early-onset COVID-19 have been identified with the control of the COVID-19 epidemic in China.We are concerned about the neurologic outcomes of neonates with COVID-19 or born to infected mothers.The study from Zhu et al 5 has shown that 2 newborns presented with elevated PCT levels and interleukin 6 with 72 hours of life (interleukin 6, 274.6 pg/mL and PCT, 9.2 ng/mL in newborn 3; and PCT, 20.74 pg/mL in newborn 4); 1 died of multiple organ failure.These elevated inflammatory markers may be associated with the maternal COVID-19.Therefore, the infants with COVID-19 or born to affected mothers have been followed up in our clinic.

Abstract:Background: Little is known about the neonatal immunological response to a maternal SARS-CoV-2 infection present during childbirth. Here we analyze a cohort of 75 neonates from SARS-CoV-2-infected mothers. Methods: The SARS-CoV-2 infection status was laboratory assessed by real-time reverse transcription polymerase chain reaction on nasopharyngeal swabs (NPS) in both mothers during childbirth and neonates within 24 hours of life. Immunophenotypes of peripheral blood mononucleated cells and SARS-CoV-2 antispike IgA, IgM and IgG of the newborns were recorded. Ten (13.3%) of 75 neonates had positive NPS for SARS-CoV-2; 17 of 75 (23%) were SARS-CoV-2-IgG seropositive, of which one with positive NPS. All the newborns resulted seronegative for SARS-CoV-2 IgA and IgM and were asymptomatic. Our cohort of newborns was divided into groups according to IgG seropositivity (IgG+/−) and NPS results (NPS+/−). Results: The count and proportion of lymphocyte subsets (evaluated measuring CD3, CD4, CD8 and CD19 markers) and of natural killer cells (evaluated by measuring the CD3−/CD16+/CD56+ subset) were all in the normal range, with no statistical differences among groups. We found a significant expansion of the T cell (CD3+) subset in the IgG+ group interpreted as the result of immune effects triggered by trained immunity in these newborns, but a decrease in CD4+ T cells for NPS+ neonates. It is therefore difficult to conclude that the decrease in CD4 can certainly be caused by an infection. Conclusions: A maternal SARS-CoV-2 infection resulted in an expansive effect of CD3+ T cells in IgG+ newborns; nonetheless, it seems not to affect structural and functional development of the newborn immune system.

Neonatal Outcome in 29 Pregnant Women with COVID-19: A Retrospective Study in Wuhan, China

Abstract:Background As of June 1, 2020, coronavirus disease 2019 (COVID-19) has caused more than 6,000,000 infected persons and 360,000 deaths globally. Previous studies revealed pregnant women with COVID-19 had similar clinical manifestations to nonpregnant women. However, little is known about the outcome of neonates born to infected women. Methods and findings In this retrospective study, we studied 29 pregnant women with COVID-19 infection delivered in 2 designated general hospitals in Wuhan, China between January 30 and March 10, 2020, and 30 neonates (1 set of twins). Maternal demographic characteristics, delivery course, symptoms, and laboratory tests from hospital records were extracted. Neonates were hospitalized if they had symptoms (5 cases) or their guardians agreed to a hospitalized quarantine (13 cases), whereas symptom-free neonates also could be discharged after birth and followed up through telephone (12 cases). For hospitalized neonates, laboratory test results and chest X-ray or computed tomography (CT) were extracted from hospital records. The presence of antibody of SARS-CoV-2 was assessed in the serum of 4 neonates. Among 29 pregnant COVID-19-infected women (13 confirmed and 16 clinical diagnosed), the majority had higher education (56.6%), half were employed (51.7%), and their mean age was 29 years. Fourteen women experienced mild symptoms including fever (8), cough (9), shortness of breath (3), diarrhea (2), vomiting (1), and 15 were symptom-free. Eleven of 29 women had pregnancy complications, and 27 elected to have a cesarean section delivery. Of 30 neonates, 18 were admitted to Wuhan Children’s Hospital for quarantine and care, whereas the other 12 neonates discharged after birth without any symptoms and had normal follow-up. Five hospitalized neonates were diagnosed as COVID-19 infection (2 confirmed and 3 suspected). In addition, 12 of 13 other hospitalized neonates presented with radiological features for pneumonia through X-ray or CT screening, 1 with occasional cough and the others without associated symptoms. SARS-CoV-2 specific serum immunoglobulin M (IgM) and immunoglobulin G (IgG) were measured in 4 neonates and 2 were positive. The limited sample size limited statistical comparison between groups. Conclusions In this study, we observed COVID-19 or radiological features of pneumonia in some, but not all, neonates born to women with COVID-19 infection. These findings suggest that intrauterine or intrapartum transmission is possible and warrants clinical caution and further investigation. Abstract:BACKGROUND:Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) is causing an outbreak of pneumonia in Wuhan, Hubei Province, China, and other international areas.OBJECTIVE:Here, we report the clinical characteristics of the newborns delivered by SARS-CoV-2 infected pregnant women.METHODS:We prospectively collected and analyzed the clinical features, laboratory data and outcomes of 7 newborns delivered by SARS-CoV-2 infected pregnant women in Zhongnan Hospital of Wuhan University during January 20 to January 29, 2020.RESULTS:4 of the 7 newborns were late preterm with gestational age between 36 weeks and 37 weeks, and the other 3 were full-term infants. The average birth weight was 2096 ± 660 g. All newborns were born without asphyxia. 2 premature infants performed mild grunting after birth, but relieved rapidly with non-invasive continuous positive airway pressure (nCPAP) ventilation. 3 cases had chest X-ray, 1 was normal and 2 who were supported by nCPAP presented mild neonatal respiratory distress syndrome (NRDS). Samples of pharyngeal swab in 6 cases, amniotic fluid and umbilical cord blood in 4 cases were tested by qRT-PCR, and there was no positive result of SARS-CoV-2 nucleic acid in all cases.CONCLUSIONS:The current data show that the infection of SARS-CoV-2 in late pregnant women does not cause adverse outcomes in their newborns, however, it is necessary to separate newborns from mothers immediately to avoid the potential threats.
Breastfed 13 Month-Old Infant of a Mother with COVID-19 Pneumonia: a Case Report

Abstract:Background In China, mothers with confirmed or suspected COVID-19 pneumonia are recommended to stop breastfeeding. However, the evidence to support this guidance is lacking. There have been relatively few cases reported about direct breastfeeding an infant by a mother with SARS-CoV-2 pneumonia. Therefore, it is necessary to assess the safety of breastfeeding and the possible protective effects of breast milk on infants. Case presentation This report analyzes the case of a mother who continued breastfeeding her 13 month-old child when both were diagnosed with confirmed COVID-19 pneumonia. We describe the clinical presentation, diagnosis, treatment, and outcome. The presence of SARS-CoV-2 nucleic acid was determined in maternal serum, breast milk, nasopharyngeal (NP) swabs and feces, and in infant serum, NP swabs and feces. IgM and IgG antibodies against SARS-CoV-2 were assessed in maternal serum and breast milk and in infant serum. SARS-CoV-2 nucleic acid was not detected in the breast milk, and antibodies against SARS-CoV-2 were detected in the mother's serum and milk. Conclusions The present case further confirms that the possibility of mother-to-child transmission about SARS-CoV-2 via breast milk alone was very small, and breast milk is safe for direct feeding of infants.

Characteristics and Risks of COVID-19 in Pregnant Women & Neonates: A Retrospective, Age-matched Comparison Study—Wuhan Experience in the Management of Peripartum Health

Abstract:ABSTRACT Background: Coronavirus disease 2019 (COVID-19) has since spread globally causing the ongoing pandemic. However, there are few studies on the symptoms and outcomes of pregnant women with COVID-19 and their newborns. Limited data are available comparing pregnant women with and without COVID-19 pneumonia. Our study aimed to find symptoms unique in pregnancy to help the early detection of COVID-19 in pregnant women and to evaluate its impact on perinatal women and their newborns. Methods: Clinical data were reviewed and collected for 11 pregnant women with laboratory-confirmed COVID-19 (pregnant confirmed cohort) who were consecutively admitted to Zhongnan Hospital of Wuhan University and Maternal and Child Health Hospital of Hubei Province, from January 26 to February 26, 2020. They were subsequently matched by age with 11 pregnant women suspected of COVID-19 (pregnant suspected group) who had typical changes in chest computed tomography (CT) scans but were repeatedly negative for reverse-transcriptase-polymerase-chain-reaction (RT-PCR), with 11 pregnant women who were not infected (pregnant non-infected group), and with 44 non-pregnant women with confirmed COVID-19 (Non-pregnant confirmed group). Results: The significant symptoms in pregnant confirmed women were mild fever and fatigue. Both the confirmed and suspected women had a fever (45.5% vs. 72.7%, P > 0.05) which were barely distinguishable. The chest CT scans of confirmed women showed unspecific manifestations, such as bilateral pleural effusions and slightly increased densities. Eight of eleven (72.7%) confirmed women did not feel anything unusual until abnormalities were found on chest CT scans on the admission screening test. All three groups had elevated white blood cell count, neutrophil count, lactate dehydrogenase, and reduced total protein. Compared to the normal group, women in the confirmed group had significantly reduced hemoglobin, lymphocyte, albumin, and increased lactate dehydrogenase ( P < 0.05). The infection did not increase the risk of premature delivery, premature rupture of the membrane, or comorbidities in pregnancy. The neonates of the three groups of mothers had similar symptoms, except for those of the confirmed group had increased heart rate (130–143 bpm) and mean arterial pressure (93–112 mmHg) than those born to the normal group. The infection of the mother did lead to more complications in their neonates. And there was no evidence of vertical transmission during late pregnancy. Conclusions: Pregnant women were often asymptomatic and had incidental abnormal findings on a chest CT scan on admission. Screening of CT scans in disease prevalent areas is thus of crucial importance. Even after the laboratory confirmation, the manifestation of the CT scan could be atypical, which alerted the necessity of protection for healthcare workers. COVID-19 did not increase the risk of complications in pregnant women and their neonates.
Clinical Analysis of Neonates Born to Mothers with or without COVID-19: A Retrospective Analysis of 48 Cases from Two Neonatal Intensive Care Units in Hubei Province

Abstract:Objective The perinatal consequences of neonates born to severe acute respiratory syndrome-associated coronavirus-2 (SARS-CoV-2) infected mothers are uncertain. This study aimed to compare the differences in clinical manifestation, laboratory results, and outcomes of neonates born to mothers with or without coronavirus disease 2019 (COVID-19). Study Design A total of 48 neonates were admitted to Tongji Hospital and HuangShi Maternal and Child Healthcare Hospital from January 17 to March 4, 2020. The neonates were divided into three groups according to the mothers' conditions: neonates born to mothers with confirmed COVID-19, neonates born to mothers with clinically diagnosed COVID-19, and neonates born to mothers without COVID-19. The clinical data of mothers and infants in the three groups were collected, compared, and analyzed. Results The deliveries occurred in a negative pressure isolation room, and the neonates were separated from their mothers immediately after birth for further observation and treatment. None of the neonates showed any signs of fever, cough, dyspnea, or diarrhea. SARS-CoV-2 reverse transcriptase-polymerase chain reaction of the throat swab and feces samples from the neonates in all three groups was negative. No differences were detected in the whole blood cell, lymphocytes, platelet, and liver and renal function among the three groups. All mothers and their infants showed satisfactory outcomes, including a 28-week preterm infant. Conclusion The clinical manifestations, radiological, and biochemical results did not show any difference between the three groups. No evidence of vertical transmission was found in this study whether the pregnant women developed coronavirus infection in the third (14 cases) or second trimester (1 case).

Abstract:Background & aimCoronavirus disease 2019 (COVID‐19) vaccine generates functional antibodies in maternal circulation that are detectable in infants, while the information is restricted to the usage of COVID‐19 vaccine during pregnancy. In this study, we aimed to evaluate the effect of maternal COVID‐19 vaccines before pregnancy.MethodInfants were included from mothers with no inactivated COVID‐19 vaccine, 1‐dose, 2‐dose and 3‐dose before pregnancy, and SARS‐CoV‐2 IgG antibodies were tested. Comparative analysis was done between the groups.ResultsA total of 130 infants were enrolled in the study. Significantly higher levels of SARS‐CoV‐2 IgG antibodies in infants born to mothers with 3‐dose COVID‐19 vaccine before pregnancy compared with 1‐dose and 2‐dose groups (p<0.0001). The levels of antibodies decreased significantly with age in infants born to mothers with the 3‐dose COVID‐19 vaccine before pregnancy (r=‐0.338, p=0.035), and it was still higher than that 2‐dose COVID‐19 vaccine group.ConclusionThe maternal SARS‐CoV‐2 antibodies produced from the inactivated COVID‐19 vaccine before pregnancy can be transferred to newborns via the placenta. Abstract:Background Coronavirus disease 2019 (COVID-19) caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is an emerging disease. The consequences of SARS-CoV-2 exposure in infants remain unknown. Therefore, this study aims to investigate whether neonates born to mothers with COVID-19 have adverse brain development. Methods This multicenter observational study was conducted at two designated maternal and children’s hospitals in Hubei Province, mainland China from February 1, 2020 to May 15, 2020. Neonates born to mothers with COVID-19 were enrolled. Brain magnetic resonance imaging (MRI) findings, and volumes of grey and white matters, and physical growth parameters were observed at 44 weeks corrected gestational age. Results Of 72 neonates born to mothers with COVID-19, 8 (11%) were diagnosed with COVID-19, 8 (11%) were critically ill, and no deaths were reported. Among the eight neonates that underwent brain MRI at corrected gestational age of 44 weeks, five neonates were diagnosed with COVID-19. Among these five neonates, three presented abnormal MRI findings including abnormal signal in white matter and delayed myelination in newborn 2, delayed myelination and brain dysplasia in newborn 3, and abnormal signal in the bilateral periventricular in newborn 5. The other three neonates without COVID-19 presented no significantly changes of brain MRI findings and the volumes of grey matter and white matter compared to those of healthy newborns at the equivalent age ( P > 0.05). Physical growth parameters for weight, length, and head circumference at gestational age of 44 weeks were all above the 3rd percentile for all neonates. Conclusions Some of the neonates born to mothers with COVID-19 had abnormal brain MRI findings but these neonates did not appear to have poor physical growth. These findings may provide the information on the follow-up schedule on the neonates exposed to SARS-CoV-2, but further study is required to evaluate the association between the abnormal MRI findings and the exposure to SARS-CoV-2.

Abstract:At present, there are still ambiguous reports about the perinatal infection of infants born to mothers infected with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The dynamic characteristics of infantile serum antibodies born to mother with SARS-CoV-2 has not been well described. In this study, we analyzed the seroconversion of 27 newborns born to 26 pregnant women infected with SARS-CoV-2. The SARS-CoV-2 IgG positive rate of parturient was 80.8%, and half of their infants obtained maternal IgG. IgG transfer rates were 18.8% and 81.8% in those infants whose mother infected less and more than 2 weeks before delivery. In the first two months of life, the IgG level of infants dropped sharply to one tenth of that at birth. These results suggest that maternal SARS-CoV-2 IgG provides limited protection for infants.
Breastfeeding in Mothers with COVID-19: Insights from Laboratory Tests and Follow-Up from Early Outbreak of the Pandemic in China

Abstract:Objective: The outbreak of Coronavirus Disease 2019 (COVID-19) caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) threatens a surging number of community groups within society, including women actively breastfeeding. Breastfeeding involves intimate behaviors, a major transmission route of SARS-CoV-2, and is integral to the close mother-baby relationship highly correlated with maternal psychological status. Materials and Methods: Twenty-three pregnant women and puerperae with either confirmed or suspected diagnoses of COVID-19 were enrolled in the study. The clinical characteristics and outcomes of the mothers and neonates were recorded. The presence of SARS-CoV-2, IgG, and IgM in breast milk, maternal blood, and infant blood, together with feeding patterns, was assessed within 1 month after delivery. Feeding patterns and maternal psychological status were also recorded in the second follow-up. Results: No positive detection of SARS-CoV-2 was found in neonates. All breast milk samples were negative for the detection of SARS-CoV-2. The presence of IgM for SARS-CoV-2 in breast milk was correlated with IgM presence in the maternal blood. The results of IgG detection for SARS-CoV-2 were negative in all breast milk samples. All infants were in a healthy condition in two follow-ups, and antibody tests for SARS-CoV-2 were negative. The rate of breast milk feeding increased during two follow-ups. All mothers receiving a second follow-up experienced negative psychological factors and status. Conclusions: Our findings support the feasibility of breastfeeding in women infected with SARS-CoV-2. The additional negative psychological status of mothers due to COVID-19 should also be considered during the puerperium period.
Maternal Preconception COVID-19 Vaccination and Its Protective Effect on Infants after a Breakthrough Infection during Pregnancy

Abstract:Background and aims: The transplacental vertical transfer of maternal antibodies was determined to be a crucial factor in conferring protective immunity to infants following delivery, and this study aimed to evaluate the protective effect of maternal preconception COVID-19 vaccination on infants. Methods: A prospective cohort study was conducted at the National Clinical Medical Research Center for Child Health and Diseases in Chongqing, China, spanning from July 2022 to April 2023. The study included infants from mothers with a preconception COVID-19 vaccination and (or) a SARS-CoV-2 infection during pregnancy. Titers of SARS-CoV-2 immunoglobulin G (IgG) and cross-neutralizing activity against SARS-CoV-2 variants were detected. Results: In this cohort study comprising 158 infants, it was observed that infants born to mothers who experienced a pregnancy-related breakthrough infection following a preconception vaccination had the highest titers of SARS-CoV-2 IgG and cross-neutralizing antibody activity against different variants compared to those with either of these factors alone. The transplacental vertical transmission of anti-SARS-CoV-2 antibodies decreased significantly with increasing age, from 3.16 ODs at birth to 2.29 ODs at two months, and persisted for approximately four months after birth. The predominant subclass of passively transmitted antibodies via the placenta was found to be IgG1, and a positive correlation was observed between the titers of SARS-CoV-2 IgG and IgG1 (R = 0.59, p < 0.001; Slope: 0.49 ± 0.070, p < 0.001). Conclusions: Maternal preconception COVID-19 vaccination represents a promising immunological strategy for conferring postnatal protection to infants, especially during the period of heightened risk of exposure to SARS-CoV-2 infection. It is imperative to underscore the significance of vaccination for women who are preparing to become pregnant or are pregnant, and concerted efforts must be made to promote vaccination among eligible women.

Abstract:Abstract Background Our study aimed to find symptoms unique in pregnancy and to help the early diagnosis in pregnant women and to evaluate the impact of COVID-19 in postpartum women and their newborns. Methods Clinical data were reviewed and collected for 11 pregnant women with laboratory-confirmed COVID-19 who were consecutively admitted to Zhongnan Hospital of Wuhan University and Women and Children’s Hospital of Hubei Province, from Jan 26 to Feb 26, 2020. Results All the confirmed women didn’t have any exposure history and their early symptoms were mildly elevated temperate and fatigue. The chest CT scans of confirmed women can be atypical manifestations, such as bilateral pleural effusions and slightly increased densities. Eight of eleven confirmed women did not feel anything unusual until abnormalities were found on chest CT scans on admission screening test. All three groups had elevated white blood cell count, neutrophil count and lactate dehydrogenase, and reduced total protein.The infection did not increase the risk for premature delivery, premature rupture of membrane, or comorbidities in pregnancy. Conclusions Pregnant women were often asymptomatic and accidentally detected abnormalities on chest CT scan on admission which emphasize the importance of CT scan in prevalent areas of the COVID-19. Even after the laboratory confirmation, the manifestation of the CT scan could be atypical, which alerted the necessity of protection for healthcare workers. The COVID-19 did not increase the risk of complications in pregnant women and their neonates. Trial registration: This case series was approved by the institutional ethics board of Zhongnan Hospital of Wuhan University (No. 2020020) and Women and Children’s Hospital of Hubei Province(NO. LW035).
