Abstract:OBJECTIVE:To investigate the CT findings of patients with different clinical types of coronavirus disease 2019 (COVID-19).METHODS:A total of 67 patients diagnosed as COVID-19 by nucleic acid testing were collected and divided into 4 groups according to the clinical stages based on Diagnosis and treatment of novel coronavirus pneumonia (trial version 6). The CT imaging characteristics were analyzed among patients with different clinical types.RESULTS:Among 67 patients, 3(4.5%) were mild, 35 (52.2%) were moderate, 22 (32.8%) were severe, and 7(10.4%) were critical ill. No significant abnormality in chest CT imaging in mild patients. The 35 cases of moderate type included 3 (8.6%) single lesions, the 22 cases of severe cases included 1 (4.5%) single lesion and the rest cases were with multiple lesions. CT images of moderate patients were mainly manifested by solid plaque shadow and halo sign (18/35, 51.4%); while fibrous strip shadow with ground glass shadow was more frequent in severe cases (7/22, 31.8%). Consolidation shadow as the main lesion was observed in 7 cases, and all of them were severe or critical ill patients.CONCLUSIONS:CT images of patients with different clinical types of COVID-19 have characteristic manifestations, and solid shadow may predict severe and critical illness.

Abstract:ObjectiveTo investigate the clinical characteristics and CT features of novel coronavirus pneumonia (COVID-19).MethodsThe clinical and CT data of 13 cases of COVID-19 diagnosed by nucleic acid test in our hospital were retrospectively analyzed. The CT features focused on the density, number, distribution, location and morphology of the lesions, as well as pleural thickening, pleural effusion, mediastinal lymph node enlargement or other accompanying signs.ResultsThere were 9 males and 4 females, aged 31-67 years, with an average age of 49±12 years. Common clinical symptoms included fever (8/13), dry cough or cough (3/13), accompanied by diarrhea, nausea and vomiting in 1 case (1/13). Lymphocyte count in peripheral blood decreased in 2 cases (2/13) and CRP increased in 5 cases (5/13). 10 cases (10/12) have multiple lesions, 11 cases (11/12) involve the dorsal and posterior basal segment of the lower lobe bilateral lung. Peripheral distribution (located in subpleural or subpleural area of interlobular fissure) was dominant in 12 cases (12/12), and distribution along the bronchial tree in 10 cases (10/12), There were 10 cases (10/12) with mass and patchy morphology, 9 cases (9/12) with grid shadow. The density was mainly ground-glass opacity lesions (11/12 cases), thickening of the adjacent bronchial bundle was observed in 9 cases (9/12), Pleural thickening or traction deformation (including interlobular fissure distortion) was observed in 8 cases (8/12).ConclusionThe clinical features of COVID-19 are fever and increased CRP. The imaging features is peripheral GGO, and HRCT of the chest can detect the pulmonary changes of COVID-19 patients at an early stage.

Abstract:Objective: To explore the imaging characteristics of patients with novel coronavirus pneumonia (COVID-19) combined with different underlying diseases. Materials and methods: COVID-19 was diagnosed in 153 patients at Beijing Shijitan Hospital, Capital Medical University, from November 16, 2022 to December 16, 2022, and data were retrospectively collected. All patients underwent chest CT scan from 1 to 14 days after onset and were divided into two groups based on the presence or absence of underlying diseases. Forty-three patients had underlying diseases, and 110 patients had none. We compared the differences between the two groups. Result: The comparison between the two groups showed statistically significant differences in age, cough, bilateral lung distribution, diffuse distribution, honeycomb-like changes in the lungs, patchy distribution, large patchy distribution, band distribution, crazy-paving sign, air bronchogram sign, traction bronchiectasis, and pleural effusion. Conclusion: Fever and cough are the most common clinical symptoms in patients with COVID-19. Chest CT showed multiple lesions in both lungs. The most common types of lesions were thickening of bronchovascular bundle and GGO. Patients with underlying diseases had more honeycomb-like changes, crazy-paving sign, air bronchogram sign, traction bronchiectasis, and pleural effusion than those without underlying diseases. Chest thin-slice CT scan provides a key reference for the early detection and diagnosis of the disease.

Abstract:Purpose: To report CT features of coronavirus disease-2019 (COVID-19) in patients with various disease severity. Methods: The CT manifestations and clinical data of 73 patients with COVID-19 were retrospectively collected in 6 hospitals from Jan 21 to Feb 3, 2020. We analyzed the initial and follow-up CT features of patients with disease severity, according to the Guidelines for the Diagnosis and Treatment of New Coronavirus Pneumonia. Results: Six patients (8%) were diagnosed as mild type pneumonia; these patients had no obvious abnormal CT findings or manifested mild changes of lung infection. All 43 patients (59 %) with common type presented unique or multiple ground-glass opacities (GGO) in the periphery of the lungs, with or without interlobular septal thickening. In the 21 patients (29 %) with severe type, extensive GGO and pulmonary consolidation were found in 16 cases (16/21, 76 %) and 5 cases (24 %), respectively. An extensive "white lung", with atelectasis and pleural effusion were found in critical type patients (3, 4%). On the resolutive phase of the disease, CT abnormalities showed complete resolution, or demonstrated residual linear opacities. Conclusions: Different CT features are seen according to disease severity, which can help COVID-19 stratification.

Abstract:The novel coronavirus 2019 (2019 nCoV), appeared in Wuhan in December 2019, can cause a novel coronavirus pneumonia (Corona Virus Disease 2019, COVID-19). COVID-19 is highly infectious and easy to infect people. The epidemic has gradually spread to all parts of the country. In order to provide a basis for clinical diagnosis, this study retrospectively analyzed the imaging characteristics, evolution and related imaging manifestations of COVID-19 patients in different stages of the disease. The results suggest that the imaging findings of 48 COVID-19 patients from Hengyang, Hunan Province are comparable in different stages of the disease. Chest CT showed no pneumonia in one mild patient. Chest CT findings of moderate type (n=38) and severe type (n=9) had comparable characteristics. The main manifestations were ground-glass opacity (GGO) (18/38, 47.37%; 1/9, 11.11%), and GGO with consolidation (16/38, 42.11%; 5/9, 55.56%), which respectively presented in bilateral lungs (34/38, 89.47%; 9/9, 100.00%), and multi-lobe distribution (involving 5 lobes) (17/38, 44.74%; 8/9, 88.89%). After treatment, 28 patients were isolated for 14 days and returned to the hospital for re-examination; among them, the pulmonary lesion was completely absorbed in 15 moderate patients, while 13 patients mainly manifested as GGO. The CT imaging findings of patients with COVID-19 can detect the lesions early, observe the scope of the lesions, evaluate the severity of the lesions, and assist the clinician in completing rapid isolation, diagnosis and treatment. At the same time, it can help to understand the performance of COVID-19 in different stages and dynamically detect changes in the patient's condition.

They are essential for clinical classification, treatment selection, monitoring disease progression, assessing risk factors, and predicting patient prognosis. This article aims to enhance the understanding and diagnostic capabilities of clinical and imaging physicians by reviewing the epidemiology, pathological changes, clinical manifestations, and CT features of COVID-19.

Abstract:In this retrospective case series, chest CT scans of 21 symptomatic patients from China infected with the 2019 Novel Coronavirus (2019-nCoV) were reviewed with emphasis on identifying and characterizing the most common findings. Typical CT findings included bilateral pulmonary parenchymal ground-glass and consolidative pulmonary opacities, sometimes with a rounded morphology and a peripheral lung distribution. Notably, lung cavitation, discrete pulmonary nodules, pleural effusions, and lymphadenopathy were absent. Follow-up imaging in a subset of patients during the study time window often demonstrated mild or moderate progression of disease as manifested by increasing extent and density of lung opacities.

Abstract:Objectives: To describe the characteristics of computed tomography (CT) in patients with 2019 novel coronavirus (COVID-19) pneumonia and their changes during disease progression. Patients and Methods: A total of 96 chest CT scans of 61 pneumonia patients associated with COVID-19 were reviewed to identify CT features associated with the time of symptom onset and the evolution of disease. Results: The initial CTs of 61 patients were obtained during 1 to 11 days after the onset. The main CT pattern of initial CT obtained during 1 - 3 days after the symptom onset was single (7/23, 35%) or multiple ground-glass opacity (GGO, 8/23, 35%). At 4 - 7 days after the symptom onset, the main imaging features were crazy paving GGO mixed with partial consolidation pattern (15/32, 47%). At 8 - 11 days after the symptom onset, the CT images showed consolidation pattern (3/6, 50%). A total of 35 follow up CTs were collected. The mean interval time between each follow up CT was 3 ± 2 days. The CT patterns also changed with the evolution of the disease: the features of GGO manifested at the early stage (1 - 3d). The crazy paving GGO pattern, consolidation pattern and mixed with partial consolidation pattern were found 4 to 14 days after the onset. In the absorption stage (15 - 24d), both density and extent of lesions were reduced. Conclusion: The CT imaging features are associated with the time of symptom onset and evolution of disease. Understanding the imaging characteristics of each stage is very helpful for understanding the development of disease.
Characteristics of Clinical and Imaging Features of Severe Coronavirus Disease 2019

Abstract:Objective: To retrospectively investigate the clinical manifestations and imaging features of heavy novel coronavirus pneumonia (NCP). Materials and methods: Fifty-two patients with positive chest computed tomography (CT) manifestations and a confirmed diagnosis of COVID-19 in the Infection Unit from November 2022 to January 2023 were included in our study. All patients underwent chest thin-section CT examination and had more complete clinical data from 1 to 14 days after onset. Patients were divided into two groups based on the time interval between onset and CT examination (<7 and ≥7 days), and the differences in CT performance characteristics between the two groups were compared. Results: Among the 52 patients with severe COVID-19, the age range was approximately 53 to 97 years, with a median age of 80 years in 4 men and 18 women. Thirty-three patients (63.5%) with underlying diseases were combined, including 2 (3.8%) with pulmonary disease, 6 (11.5%) with diabetes mellitus, 18 (34.6%) with hypertension, 16 (30.8%) with coronary heart disease, and 7 (13.5%) with cerebrovascular disease. The main symptoms of patients included fever in 44 (84.6%), cough in 43 (82.7%), myalgia in 2 (3.8%), sore throat in 19 (36.5%), chest tightness in 9 (17.3%), diarrhea in 2 (3.8%), and poor appetite in 3 (5.8%). The differences in lesion involvement and lesion size were statistically significant between patients grouped at the time interval between onset and CT examination. Conclusions: Severe COVID-19 is more common in older adult males, mostly combined with underlying disease. CT features include more airway involvement in the short term, and bloodway involvement is more common in those with longer onset.

Abstract:OBJECTIVE. Confronting the new coronavirus infection known as coronavirus disease 2019 (COVID-19) is challenging and requires excluding patients with suspected COVID-19 who actually have other diseases. The purpose of this study was to assess the clinical features and CT manifestations of COVID-19 by comparing patients with COVID-19 pneumonia with patients with non-COVID-19 pneumonia who presented at a fever observation department in Shanghai, China. MATERIALS AND METHODS. Patients were retrospectively enrolled in the study from January 19 through February 6, 2020. All patients underwent real-time reverse transcription-polymerase chain reaction (RT-PCR) testing. RESULTS. Eleven patients had RT-PCR test results that were positive for severe acute respiratory syndrome coronavirus 2, whereas 22 patients had negative results. No statistical difference in clinical features was observed (p > 0.05), with the exception of leukocyte and platelet counts (p < 0.05). The mean (± SD) interval between onset of symptoms and admission to the fever observation department was 4.40 ± 2.00 and 5.52 ± 4.00 days for patients with positive and negative RT-PCR test results, respectively. The frequency of opacifications in patients with positive results and patients with negative results, respectively, was as follows: ground-glass opacities (GGOs), 100.0% versus 90.9%; mixed GGO, 63.6% versus 72.7%; and consolidation, 54.5% versus 77.3%. In patients with positive RT-PCR results, GGOs were the most commonly observed opacification (seen in 100.0% of patients) and were predominantly located in the peripheral zone (100.0% of patients), compared with patients with negative results (31.8%) (p = 0.05). The median number of affected lung lobes and segments was higher in patients with positive RT-PCR results than in those with negative RT-PCR results (five vs 3.5 affected lobes and 15 vs nine affected segments; p < 0.05). Although the air bronchogram reticular pattern was more frequently seen in patients with positive results, centrilobular nodules were less frequently seen in patients with positive results. CONCLUSION. At the point during the COVID-19 outbreak when this study was performed, imaging patterns of multifocal, peripheral, pure GGO, mixed GGO, or consolidation with slight predominance in the lower lung and findings of more extensive GGO than consolidation on chest CT scans obtained during the first week of illness were considered findings highly suspicious of COVID-19.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Objective</h3><p>We systematically reviewed the CT imaging features of COVID‐19 in order to provide reference for clinical practice.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>Our article comprehensively searched PubMed, FMRS, EMbase, CNKI,WanFang databases and VIP databases to collect literatures about the CT imaging features of COVID‐19 from 1 January 2020 to 16 March 2020. Three reviewers independently screened literature, extracted data and assessed the risk of bias of included studies, and then, this meta‐analysis was performed by using Stata12.0 software.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>34 retrospective studies involving a total of 4 121 COVID‐19 patients were included. The results of meta‐analysis showed that most patients presented bilateral lung involvement (73.8%, 95%<i>CI</i>: 65.9%‐81.1%) or multilobar involvement (67.3%,95%<i>CI</i>: 54.8%‐78.7 %) and just a little patients showed normal CT findings (8.4%). We found that the most common changes in lesion density was ground‐glass opacities (68.1%,95%CI: 56.9%‐78.2%). Other changes in density included air bronchogram sign(44.7%), crazy‐paving pattern (35.6%) and consolidation (32.0%). Patchy (40.3%), spider web sign (39.5%), cord‐like (36.8%) and nodular (20.5%) were common lesion shapes in COVID‐19 patients. Pleural thickening (27.1%) was found in some patients. Lymphadenopathy(5.4%) and pleural effusion (5.3%) were rare. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>The lung lesions of patients with COVID‐19 were mostly bilateral lungs or multilobar involved. The most common chest CT findings were patchy and ground‐glass opacities. Some patients had air bronchogram, spider web sign and cord‐like. Abstract:<b>OBJECTIVE.</b> The purpose of this study was to investigate 62 subjects in Wuhan, China, with laboratory-confirmed coronavirus disease (COVID-19) pneumonia and describe the CT features of this epidemic disease. <b>MATERIALS AND METHODS.</b> A retrospective study of 62 consecutive patients with laboratory-confirmed COVID-19 pneumonia was performed. CT images and clinical data were reviewed. Two thoracic radiologists evaluated the distribution and CT signs of the lesions and also scored the extent of involvement of the CT signs. The Mann-Whitney <i>U</i> test was used to compare lesion distribution and CT scores. The chi-square test was used to compare the CT signs of early-phase versus advanced-phase COVID-19 pneumonia. <b>RESULTS.</b> A total of 62 patients (39 men and 23 women; mean [± SD] age, 52.8 ± 12.2 years; range, 30-77 years) with COVID-19 pneumonia were evaluated. Twenty-four of 30 patients who underwent routine blood tests (80.0%) had a decreased lymphocyte count. Of 27 patients who had their erythrocyte sedimentation rate and high-sensitivity C-reactive protein level assessed, 18 (66.7%) had an increased erythrocyte sedimentation rate, and all 27 (100.0%) had an elevated high-sensitivity C-reactive protein level. Multiple lesions were seen on the initial CT scan of 52 of 62 patients (83.9%). Forty-eight of 62 patients (77.4%) had predominantly peripheral distribution of lesions. The mean CT score for the upper zone (3.0 ± 3.4) was significantly lower than that for the middle (4.5 ± 3.8) and lower (4.5 ± 3.7) zones (<i>p</i> = 0.022 and <i>p</i> = 0.020, respectively), and there was no significant difference in the mean CT score of the middle and lower zones (<i>p</i> = 1.00). The mean CT score for the anterior area (4.4 ± 4.1) was significantly lower than that for the posterior area (7.7 ± 6.3) (<i>p</i> = 0.003). CT findings for the patients were as follows: 25 patients (40.3%) had ground-glass opacities (GGO), 21 (33.9%), consolidation; 39 (62.9%), GGO plus a reticular pattern; 34 (54.8%), vacuolar sign; 28 (45.2%), microvascular dilation sign; 35 (56.5%), fibrotic streaks; 21 (33.9%), a subpleural line; and 33 (53.2%), a subpleural transparent line. With regard to bronchial changes seen on CT, 45 patients (72.6%) had air bronchogram, and 11 (17.7%) had bronchus distortion. In terms of pleural changes, CT showed that 30 patients (48.4%) had pleural thickening, 35 (56.5%) had pleural retraction sign, and six (9.7%) had pleural effusion. Compared with early-phase disease (≤ 7 days after the onset of symptoms), advanced-phase disease (8-14 days after the onset of symptoms) was characterized by significantly increased frequencies of GGO plus a reticular pattern, vacuolar sign, fibrotic streaks, a subpleural line, a subpleural transparent line, air bronchogram, bronchus distortion, and pleural effusion; however, GGO significantly decreased in advanced-phase disease. <b>CONCLUSION.</b> CT examination of patients with COVID-19 pneumonia showed a mixed and diverse pattern with both lung parenchyma and the interstitium involved. Identification of GGO and a single lesion on the initial CT scan suggested early-phase disease. CT signs of aggravation and repair coexisted in advanced-phase disease. Lesions presented with a characteristic multifocal distribution in the middle and lower lung regions and in the posterior lung area. A decreased lymphocyte count and an increased high-sensitivity C-reactive protein level were the most common laboratory findings.
Epidemiological, Clinical Characteristics of Cases of SARS-CoV-2 Infection with Abnormal Imaging Findings

Abstract:Purpose: To investigate the epidemiological and clinical characteristics of COVID-19 patients with abnormal imaging findings. Methods: Patients confirmed with SARS-CoV-2 infection in Zhejiang province from January 17 to February 8 who had undergone CT or X-ray were enrolled. Epidemiological and clinical data were analyzed among those with abnormal or normal imaging findings. Results: Excluding 72 patients with normal images, 230 of 573 patients showed abnormalities affecting more than two lung lobes. The median radiographic score was 2.0, and there was a negative correlation between that score and the oxygenation index (ρ = −0.657, P < 0.001). Patients with abnormal images were older (46.65 ± 13.82), with a higher rate of coexisting condition (28.8%), a lower rate of exposure history, and longer time between onset and confirmation (5 days) than non-pneumonia patients (all P < 0.05). A higher rate of fever, cough, expectoration and headache, a lower level of lymphocytes, albumin, and serum sodium levels and a higher total bilirubin, creatine kinase, lactate dehydrogenase, and C-reactive protein levels and a lower oxygenation index were observed in pneumonia patients (all P < 0.05). Muscle ache, shortness of breath, nausea and vomiting, lower lymphocytes levels, and higher serum creatinine and radiographic score at admission were predictive factors for the severe/critical subtype. Conclusion: Patients with abnormal images have more obvious clinical manifestations and laboratory changes. Comparison of the Clinical and CT Features Between COVID-19 and H1N1 Influenza Pneumonia Patients in Zhejiang, China

Abstract:OBJECTIVE: To explore the different clinical and CT features distinguishing COVID-19 from H1N1 influenza pneumonia. PATIENTS AND METHODS: We compared two independent cohorts of COVID-19 pneumonia (n=405) and H1N1 influenza pneumonia (n=78), retrospectively. All patients were confirmed by RT-PCR. Four hundred and five cases of COVID-19 pneumonia were confirmed in nine hospitals of Zhejiang province, China from January 21 to February 20, 2020. Seventy-eight cases of H1N1 influenza pneumonia were confirmed in our hospital from January 1, 2017 to February 29, 2020. Their clinical manifestations, laboratory test results, and CT imaging characteristics were compared. RESULTS: COVID-19 pneumonia patients showed less proportions of underlying diseases, fever and respiratory symptoms than those of H1N1 pneumonia patients (p<0.01). White blood cell count, neutrophilic granulocyte percentage, C-reactive protein, procalcitonin, D-Dimer, and lactate dehydrogenase in H1N1 pneumonia patients were higher than those of COVID-19 pneumonia patients (p<0.05). H1N1 pneumonia was often symmetrically located in the dorsal part of inferior lung lobes, while COVID-19 pneumonia was unusually showed as a peripheral but non-specific lobe distribution. Ground glass opacity was more common in COVID-19 pneumonia and consolidation lesions were more common in H1N1 pneumonia (p<0.01). COVID-19 pneumonia lesions showed a relatively clear margin compared with H1N1 pneumonia. Crazy-paving pattern, thickening vessels, reversed halo sign and early fibrotic lesions were more common in COVID-19 pneumonia than H1N1 pneumonia (p<0.05). Pleural effusion in COVID-19 pneumonia was significantly less common than H1N1 pneumonia (p<0.01). CONCLUSIONS: Compared with H1N1 pneumonia in Zhejiang, China, the clinical manifestations of COVID-19 pneumonia were more concealed with less underlying diseases and slighter respiratory symptoms. The more common CT manifestations of COVID-19 pneumonia included ground glass opacity with a relatively clear margin, crazy-paving pattern, thickening vessels, reversed halo sign, and early fibrotic lesions, while the less common CT manifestations of COVID-19 pneumonia included consolidation and pleural effusion.

Abstract:<span>Coronavirus disease 2019 (COVID-19) has rapidly swept across the world. This study aimed to explore the relationship between the chest CT findings and clinical characteristics of COVID-19 patients.Patients with COVID-19 confirmed by next-generation sequencing or RT-PCR who had undergone more than 4 serial chest CT procedures were retrospectively enrolled.This study included 361 patients – 192 men and 169 women. On initial chest CT, more lesions were identified as multiple bilateral lungs lesions and localised in the peripheral lung. The predominant patterns of abnormality were ground-glass opacities (GGO) (28.5%), consolidation (13.0%), nodule (23.0%), fibrous stripes (5.3%) and mixed (30.2%). Severe cases were more common in patients with a mixed pattern (21.1%) and less common in patients with nodules (2.4%). During follow-up CT, the mediumtotal severity score (TSS) in patients with nodules and fibrous strips was significantly lower than that in patients with mixed patterns in all three stages (<i>p</i> < .01).Chest CT plays an important role in diagnosing COVID-19. The CT features may vary by age. Different CT features are not only associated with clinical manifestation but also patient prognosis.The initial chest CT findings of COVID-19 could help us monitor and predict the outcome.Nodules were more common in non severe cases and had a favorable prognosis.The mixed pattern was more common in severe cases and usually had a relatively poor outcome.</span>

Abstract:Objective: To investigate the clinical value of thin-section chest computed tomography (CT) in the typing of coronavirus disease 2019 (COVID-19). Methods: A retrospective analysis was performed on 134 patients diagnosed with COVID-19 in our hospital’s Department of Infectious Diseases from December 20, 2022, to December 31, 2022. All patients underwent thin-section chest CT scan with complete clinical data. According to clinical classification, patients were divided into the non-severe and severe groups. Clinical data and imaging features of the two groups were compared and analyzed, and statistical analysis was conducted. Results: There was a statistically significant difference with respect to diabetes mellitus between the two groups, and the incidence of diabetes mellitus in the severe group (45.8%) was higher than that in the non-severe group (25.5%); There were no significant differences in sex, age, average course of disease, and clinical symptoms between the two groups; There were significant differences in the number of lesions, symmetrical distribution, predominant peripheral distribution, diffuse distribution, blurred edge, morphology of large flake and band, vascular bundle thickening, paving stone sign, arcade sign, and fried egg sign between the two groups, the number of lesions >10, diffuse distribution, morphology of large flake and band, vascular bundle thickening, paving stone sign, and arcade sign were more common in the severe group than in the non-severe group, while predominant peripheral distribution, blurred edge, and fried egg sign were more common in the non-severe group than in the severe group. Conclusions: Thin-section chest CT scan can identify the abnormal imaging manifestations of the lung in patients with COVID-19 and evaluate the number, distribution range, and morphological characteristics of the lesions. Combined background diseases, number, distribution characteristics, blurred edge, large flake and band morphology, vascular bundle thickening, paving stone sign, arcade sign, and fried egg sign can effectively indicate the classification of patients with COVID-19. This can provide imaging evidence for the diagnosis and treatment of COVID-19.
Coronavirus disease 2019: initial chest CT findings

Abstract:Objectives: To systematically analyze CT findings during the early and progressive stages of natural course of coronavirus disease 2019 and also to explore possible changes in pulmonary parenchymal abnormalities during these two stages. Methods: We retrospectively reviewed the initial chest CT data of 62 confirmed coronavirus disease 2019 patients (34 men, 28 women; age range 20-91 years old) who did not receive any antiviral treatment between January 21 and February 4, 2020, in Chongqing, China. Patients were assigned to the early-stage group (onset of symptoms within 4 days) or progressive-stage group (onset of symptoms within 4-7 days) for analysis. CT characteristics and the distribution, size, and CT score of pulmonary parenchymal abnormalities were assessed. Results: In our study, the major characteristic of coronavirus disease 2019 was ground-glass opacity (61.3%), followed by ground-glass opacity with consolidation (35.5%), rounded opacities (25.8%), a crazy-paving pattern (25.8%), and an air bronchogram (22.6%). No patient presented cavitation, a reticular pattern, or bronchial wall thickening. The CT scores of the progressive-stage group were significantly greater than those of the early-stage group (p = 0.004). Conclusions: Multiple ground-glass opacities with consolidations in the periphery of the lungs were the primary CT characteristic of coronavirus disease 2019. CT score can be used to evaluate the severity of the disease. If these typical alterations are found, then the differential diagnosis of coronavirus disease 2019 must be considered. Key points: • Multiple GGOs with consolidations in the periphery of the lungs were the primary CT characteristic of COVID-19. • The halo sign may be a special CT feature in the early-stage COVID-19 patients. • Significantly increased CT score may indicate the aggravation of COVID-19 in the progressive stage.

Abstract:Abstract Background Since the first case of a coronavirus disease 2019 (COVID-19) infection pneumonia was detected in Wuhan, China, a series of confirmed cases of the COVID-19 were found in Southwest China. The aim of this study was to describe the imaging manifestations of hospitalized patients with confirmed COVID-19 infection in southwest China. Methods In this retrospective study, data were collected from 131 patients with confirmed coronavirus disease 2019 (COVID-19) from 3 Chinese hospitals. Their common clinical manifestations, as well as characteristics and evolvement features of chest CT images, were analyzed. Results A total of 100 (76%) patients had a history of close contact with people living in Wuhan, Hubei. The clinical manifestations of COVID-19 included cough, fever. Most of the lesions identified in chest CT images were multiple lesions of bilateral lungs, lesions were more localized in the peripheral lung, 109 (83%) patients had more than two lobes involved, 20 (15%) patients presented with patchy ground glass opacities, patchy ground glass opacities and consolidation of lesions co-existing in 61 (47%) cases. Complications such as pleural thickening, hydrothorax, pericardial effusion, and enlarged mediastinal lymph nodes were detected but only in rare cases. For the follow-up chest CT examinations (91 cases), We found 66 (73%) cases changed very quickly, with an average of 3.5 days, 25 cases (27%) presented absorbed lesions, progression was observed in 41 cases (46%), 25 (27%) cases showed no significant changes. Conclusion Chest CT plays an important role in diagnosing COVID-19. The imaging pattern of multifocal peripheral ground glass or mixed consolidation is highly suspicious of COVID-19, that can quickly change over a short period of time.
