Abstract:In this paper, we present an enhanced fuzzy k -nearest neighbor (FKNN) classifier based computer aided diagnostic (CAD) system for thyroid disease. The neighborhood size k and the fuzzy strength parameter m in FKNN classifier are adaptively specified by the particle swarm optimization (PSO) approach. The adaptive control parameters including time-varying acceleration coefficients (TVAC) and time-varying inertia weight (TVIW) are employed to efficiently control the local and global search ability of PSO algorithm. In addition, we have validated the effectiveness of the principle component analysis (PCA) in constructing a more discriminative subspace for classification. The effectiveness of the resultant CAD system, termed as PCA-PSO-FKNN, has been rigorously evaluated against the thyroid disease dataset, which is commonly used among researchers who use machine learning methods for thyroid disease diagnosis. Compared to the existing methods in previous studies, the proposed system has achieved the highest classification accuracy reported so far via 10-fold cross-validation (CV) analysis, with the mean accuracy of 98.82% and with the maximum accuracy of 99.09%. Promisingly, the proposed CAD system might serve as a new candidate of powerful tools for diagnosing thyroid disease with excellent performance.

Abstract:In this paper, we present a novel hybrid diagnosis system named LFDA-EKELM, which integrates local fisher discriminant analysis (LFDA) and kernelized extreme learning machine method for thyroid disease diagnosis. The proposed method comprises of three stages. Focusing on dimension reduction, the first stage employs LFDA as a feature extraction tool to construct more discriminative subspace for classification, the system switches from feature extraction to model construction. And then, the obtained feature subsets are fed into designed kernelized ELM (KELM) classifier to train an optimal predictor model whose parameters are adaptively specified by improving artificial bee colony (IABC) approach. Here, the proposed IABC method introduces an improved solution search equation to enhance the exploitation of searching for solutions, and provides a new framework to make the global converge rapidly. Finally, the enhanced-KELM (EKELM) model is applied to perform the thyroid disease diagnosis tasks using the most discriminative feature subset and the optimal parameters. The effectiveness of the proposed system is evaluated on the thyroid disease dataset in terms of classification accuracy. Experimental results demonstrate that LFDA-EKELM outperforms the baseline methods.

Abstract:In this paper, we present an effective and efficient computer aided diagnosis (CAD) system based on principle component analysis (PCA) and extreme learning machine (ELM) to assist the task of thyroid disease diagnosis. The CAD system is comprised of three stages. Focusing on dimension reduction, the first stage applies PCA to construct the most discriminative new feature set. After then, the system switches to the second stage whose target is model construction. ELM classifier is explored to train an optimal predictive model whose parameters are optimized. As we known, the number of hidden neurons has an important role in the performance of ELM, so we propose an experimental method to hunt for the optimal value. Finally, the obtained optimal ELM model proceeds to perform the thyroid disease diagnosis tasks using the most discriminative new feature set and the optimal parameters. The effectiveness of the resultant CAD system (PCA-ELM) has been rigorously estimated on a thyroid disease dataset which is taken from UCI machine learning repository. We compare it with other related methods in terms of their classification accuracy. Experimental results demonstrate that PCA-ELM outperforms other ones reported so far by 10-fold cross-validation method, with the mean accuracy of 97.73% and with the maximum accuracy of 98.1%. Besides, PCA-ELM performs much faster than support vector machines (SVM) based CAD system. Consequently, the proposed method PCA-ELM can be considered as a new powerful tools for diagnosing thyroid disease with excellent performance and less time.

Abstract:Accurate diagnosis of thyroid nodules using ultrasonography is a valuable but tough task even for experienced radiologists, considering both benign and malignant nodules have heterogeneous appearances. Computer-aided diagnosis (CAD) methods could potentially provide objective suggestions to assist radiologists. However, the performance of existing learning-based approaches is still limited, for direct application of general learning models often ignores critical domain knowledge related to the specific nodule diagnosis. In this study, we propose a novel deep-learning-based CAD system, guided by task-specific prior knowledge, for automated nodule detection and classification in ultrasound images. Our proposed CAD system consists of two stages. First, a multi-scale region-based detection network is designed to learn pyramidal features for detecting nodules at different feature scales. The region proposals are constrained by the prior knowledge about size and shape distributions of real nodules. Then, a multi-branch classification network is proposed to integrate multi-view diagnosis-oriented features, in which each network branch captures and enhances one specific group of characteristics that were generally used by radiologists. We evaluated and compared our method with the state-of-the-art CAD methods and experienced radiologists on two datasets, i.e. Dataset I and Dataset II. The detection and diagnostic accuracy on Dataset I were 97.5% and 97.1%, respectively. Besides, our CAD system also achieved better performance than experienced radiologists on Dataset II, with improvements of accuracy for 8%. The experimental results demonstrate that our proposed method is effective in the discrimination of thyroid nodules.

Abstract:Fine-needle aspiration cytology (FNAC) is regarded as one of the most important preoperative diagnostic tests for thyroid nodules. However, the traditional diagnostic process of FNAC is time-consuming, and its accuracy is highly related to the experience of the cytopathologist. Computer-aided diagnostic (CAD) systems are rapidly evolving to provide objective diagnostic recommendations. So far, most studies have used fixed-size patches and usually hand-select patches for model training. In this study, we develop a CAD system to address these challenges. In order to be consistent with the diagnostic working mode of cytopathologists, the system is mainly composed of two task modules: the detecting module that is responsible for detecting the regions of interest (ROIs) from the whole slide image of the FNAC, and the classification module that identifies ROIs having positive lesions. The system can then output the top-k ROIs with the highest positive probabilities for the cytopathologists to review. In order to obtain the overall good performance of the system, we compared different object detection and classification models, and used a combination of the YOLOV4 and EfficientNet networks in our system.
Evaluation of a Deep Learning-Based Computer-Aided Diagnosis System for Distinguishing Benign from Malignant Thyroid Nodules in Ultrasound Images.

Abstract:PURPOSE:Computer-aided diagnosis (CAD) systems assist in solving subjective diagnosis problems that typically rely on personal experience. A CAD system has been developed to differentiate malignant thyroid nodules from benign thyroid nodules in ultrasound images based on deep learning methods. The diagnostic performance was compared between the CAD system and the experienced attending radiologists.METHODS:The ultrasound image dataset for training the CAD system included 651 malignant nodules and 386 benign nodules while the database for testing included 422 malignant nodules and 128 benign nodules. All the nodules were confirmed by pathology results. In the proposed CAD system, a support vector machine (SVM) is used for classification and fused features which combined the deep features extracted by a convolutional neural network (CNN) with the hand-crafted features such as the histogram of oriented gradient (HOG), local binary patterns (LBP), and scale invariant feature transform (SIFT) were obtained. The optimal feature subset was formed by selecting these fused features based on the maximum class separation distance and used as the training sample for the SVM.RESULTS:The accuracy, sensitivity, and specificity of the CAD system were 92.5%, 96.4%, and 83.1%, respectively, which were higher than those of the experienced attending radiologists. The areas under the ROC curves of the CAD system and the attending radiologists were 0.881 and 0.819, respectively.CONCLUSIONS:The CAD system for thyroid nodules exhibited a better diagnostic performance than experienced attending radiologists. The CAD system could be a reliable supplementary tool to diagnose thyroid nodules using ultrasonography. Macroscopic features in ultrasound images, such as the margins and shape of thyroid nodules, could influence the diagnostic efficiency of the CAD system.

Abstract:Objectives To complete the task of automatic recognition and classification of thyroid nodules and solve the problem of high classification error rates when the samples are imbalanced. Methods An improved k‐nearest neighbor (KNN) algorithm is proposed and a method for automatic thyroid nodule classification based on the improved KNN algorithm is established. In the improved KNN algorithm, we consider not only the number of class labels for various classes of data in KNNs, but also the corresponding weights. And we use the Minkowski distance measure instead of the Euclidean distance measure. Results A total of 508 ultrasound images of thyroid nodules, including 415 benign nodules and 93 malignant nodules, were used in the paper. Experimental results show the improved KNN has 0.872549 accuracy, 0.867347 precision, 1 recall, and 0.928962 F1‐score. At the same time, we also considered the influence of different distance weights, the value of k, different distance measures on the classification results. Conclusions A comparison result shows that our method has a better performance than the traditional KNN and other classical machine learning methods.

Abstract:Objective: There has been a considerable amount of Computer-Aided Diagnosis (CAD) methods highlighted in the field of ultrasonic examination (USE) of thyroid nodules. However, few researches focused on the automatic risk classification, which was the basis for determining whether Fine Needle Aspiration (FNA) was needed. The aim of this work was to implement automatic risk level assessment of thyroid nodules. Approach: Firstly, 1862 cases of thyroid nodules with the results of USE and FNA were collected as the dataset. Then, an improved U-Net++ model was utilized for segmenting thyroid nodules in ultrasound images automatically. Finally, the segmentation result was imported into a multi-task convolutional neural network (MT-CNN), the design of which was based on the clinical guideline called KWAK TI-RADS. Apart from the category of benign and malignant, the MT-CNN also exported the classification result of four malignant features, Solid Component (SC), Hypoechogenicity or Marked Hypoechogenicity (HMH), Microlobulated or Irregular Margin (MIM), Microcalcification (MC), which were used for counting the risk level in KWAK TI-RADS. Main results: The performance of the improved U-Net++ was evaluated on our test set, including 302 cases. The Dice coefficient and Intersection over Union (IoU) reached 0.899, 0.816, respectively. The classification accuracy rates of SC, HMH, MIM, MC, were 94.5%, 92.8%, 86.1%, 88.9%, while the false positive (FP) rate was 6.0%, 5.6%, 10.6%, 12.9% separately. As for the category of benign and malignant, the precision and recall rates were 93.7% and 94.4%. Significance: The proposed CAD method showed favourable performance in the diagnosis of thyroid nodules. Compared with other methods, it could provide reports closer to clinical practice for doctors.

Abstract:Thyroid cancer is the most pervasive disease in the endocrine system and is getting extensive attention. The most prevalent method for an early check is ultrasound examination. Traditional research mainly concentrates on promoting the performance of processing a single ultrasound image using deep learning. However, the complex situation of patients and nodules often makes the model dissatisfactory in terms of accuracy and generalization. Imitating the diagnosis process in reality, a practical diagnosis-oriented computer-aided diagnosis (CAD) framework towards thyroid nodules is proposed, using collaborative deep learning and reinforcement learning. Under the framework, the deep learning model is trained collaboratively with multiparty data; afterward classification results are fused by a reinforcement learning agent to decide the final diagnosis result. Within the architecture, multiparty collaborative learning with privacy-preserving on large-scale medical data brings robustness and generalization, and diagnostic information is modeled as a Markov decision process (MDP) to get final precise diagnosis results. Moreover, the framework is scalable and capable of containing more diagnostic information and multiple sources to pursue a precise diagnosis. A practical dataset of two thousand thyroid ultrasound images is collected and labeled for collaborative training on classification tasks. The simulated experiments have shown the advancement of the framework in promising performance.

Abstract:The thyroid, an endocrine gland that secretes hormones in the blood, circulates its products to all tissues of the body, where they control vital functions in every cell. Normal levels of thyroid hormone help the brain, heart, intestines, muscles and reproductive system function normally. Thyroid hormones control the metabolism of the body. Abnormalities of thyroid function are usually related to production of too little thyroid hormone (hypothyroidism) or production of too much thyroid hormone (hyperthyroidism). Therefore, the correct diagnosis of these diseases is very important topic. In this study, Linguistic Hedges Neural-Fuzzy Classifier with Selected Features (LHNFCSF) is presented for diagnosis of thyroid diseases. The performance evaluation of this system is estimated by using classification accuracy and k-fold cross-validation. The results indicated that the classification accuracy without feature selection was 98.6047% and 97.6744% during training and testing phases, respectively with RMSE of 0.02335. After applying feature selection algorithm, LHNFCSF achieved 100% for all cluster sizes during training phase. However, in the testing phase LHNFCSF achieved 88.3721% using one cluster for each class, 90.6977% using two clusters, 91.8605% using three clusters and 97.6744% using four clusters for each class and 12 fuzzy rules. The obtained classification accuracy was very promising with regard to the other classification applications in literature for this problem.

Abstract:Background: The evaluation of thyroid nodules with ultrasonography has created a large burden for radiologists. Artificial intelligence technology has been rapidly developed in recent years to reduce the cost of labor and improve the differentiation of thyroid malignancies. This study aimed to investigate the diagnostic performance of a novel computer-aided diagnosing system (CADs: S-detect) for the ultrasound (US) interpretation of thyroid nodule subtypes in a specialized thyroid center. Methods: Our study prospectively included 180 thyroid nodules that underwent ultrasound interpretation. The CADs and radiologist assessed all nodules. The ultrasonographic features of different subtypes were analyzed, and the diagnostic performances of the CADs and radiologist were compared. Results: There were seven subtypes of thyroid nodules, among which papillary thyroid cancer (PTC) accounted for 50.6% and follicular thyroid carcinoma (FTC) accounted for 2.2%. Among all thyroid nodules, the CADs presented a higher sensitivity and lower specificity than the radiologist (90.5% vs 81.1%; 41.2% vs 83.5%); the radiologist had a higher accuracy than the CADs (82.2% vs 67.2%) for diagnosing malignant thyroid nodules. The accuracy of the CADs was not as good as that of the radiologist in diagnosing PTCs (70.9% vs 82.1%). The CADs and radiologist presented accuracies of 43.8% and 60.9% in identifying FTCs, respectively. Conclusions: The ultrasound CADs presented a higher sensitivity for identifying malignant thyroid nodules than experienced radiologists. The CADs was not as good as experienced radiologists in a specialized thyroid center in identifying PTCs. Radiologists maintained a higher specificity than the CADs for FTC detection.

Abstract:Image-based computer-aided diagnosis (CAD) systems have been developed to assist doctors in the diagnosis of thyroid cancer using ultrasound thyroid images. However, the performance of these systems is strongly dependent on the selection of detection and classification methods. Although there are previous researches on this topic, there is still room for enhancement of the classification accuracy of the existing methods. To address this issue, we propose an artificial intelligence-based method for enhancing the performance of the thyroid nodule classification system. Thus, we extract image features from ultrasound thyroid images in two domains: spatial domain based on deep learning, and frequency domain based on Fast Fourier transform (FFT). Using the extracted features, we perform a cascade classifier scheme for classifying the input thyroid images into either benign (negative) or malign (positive) cases. Through expensive experiments using a public dataset, the thyroid digital image database (TDID) dataset, we show that our proposed method outperforms the state-of-the-art methods and produces up-to-date classification results for the thyroid nodule classification problem.

Abstract:Background To assess the diagnostic efficacy of the computer-aided ultrasonic diagnosis system (CAD system) in differentiating benign and malignant thyroid nodules. Methods The images of 296 thyroid nodules were included in validation sets. The diagnostic efficacy of the CAD system was compared with that of junior physicians and senior physicians, as well as that of the combination diagnosis of the CAD system with junior physicians. The diagnostic efficacy of the CAD system for different sizes of thyroid nodules was compared. Results The diagnostic sensitivity and accuracy of the CAD system were higher than those of junior physicians (83.4% vs. 72.2%, 73.0% vs. 69.6%), but the diagnostic specificity of the CAD system was lower than that of junior physicians (62.1% vs. 66.9%). The diagnostic accuracy of the CAD system was lower than that of senior physicians (73.0% vs. 83.8%). However, the combination diagnosis of the CAD system with junior physicians had higher accuracy (81.8%) and AUC (0.842) than those of either the CAD system or junior physicians alone, and comparable diagnostic performance with those of senior physicians. The Kappa was 0.635 in the combination diagnosis of the CAD system with junior physicians, showing good consistency with the pathological results. The accuracy (76.4%) of the CAD system was the highest for nodules of 1-2 cm. Conclusion The CAD system can effectively assist physicians to identify malignant and benign thyroid nodules, reduce the overdiagnosis and overtreatment of thyroid nodules, avoid unnecessary invasive fine needle aspiration, and improve the diagnostic accuracy of junior physicians.

Abstract:ABSTRACT In this research, we present a refined image‐based computer‐aided diagnosis (CAD) system for thyroid cancer detection using ultrasound imagery. This system integrates a specialized convolutional neural network (CNN) architecture designed to address the unique aspects of thyroid image datasets. Additionally, it incorporates a novel statistical model that utilizes a two‐dimensional random coefficient autoregressive (2D‐RCA) method to precisely analyze the textural characteristics of thyroid images, thereby capturing essential texture‐related information. The classification framework relies on a composite feature vector that combines deep learning features from the CNN and handcrafted features from the 2D‐RCA model, processed through a support vector machine (SVM) algorithm. Our evaluation methodology is structured in three phases: initial assessment of the 2D‐RCA features, analysis of the CNN‐derived features, and a final evaluation of their combined effect on classification performance. Comparative analyses with well‐known networks such as VGG16, VGG19, ResNet50, and AlexNet highlight the superior performance of our approach. The outcomes indicate a significant enhancement in diagnostic accuracy, achieving a classification accuracy of 97.2%, a sensitivity of 84.42%, and a specificity of 95.23%. These results not only demonstrate a notable advancement in the classification of thyroid nodules but also establish a new standard in the efficiency of CAD systems.

Abstract:We evaluated the accuracy of human and computer-aided diagnosis (CAD) in a reading map diagnosis competition for detection of thyroid cancers via ultrasonography (US). The competition comprised 33 thyroid nodule images randomly chosen between 2015 and 2017. One hundred seventy-seven contestants including one operator using CAD participated in the competition. The competition was separated into an online part and a live part. We compared the average accuracy of contestants and CAD in the detection of thyroid cancers. The accuracy of contestants and the CAD system was 60.3% and 84.8%, respectively. The accuracy of the CAD system was higher than that of the contestants with different technical titles. The areas under the curve for CAD and contestants were 0.985 (0.881-1.00) and 0.659 (0.645-0.673) (Z = 7.55, p < 0.01). The CAD system had high accuracy in this thyroid nodule reading map competition, and may be an adjuvant tool for radiologists.

Abstract:Objectives: This study aims to investigate the efficacy of a computer-aided diagnosis (CAD) system in distinguishing between benign and malignant thyroid nodules in the context of Hashimoto's thyroiditis (HT) and to evaluate the role of the CAD system in reducing unnecessary biopsies of benign lesions. Methods: We included a total of 137 nodules from 137 consecutive patients (mean age, 43.5 ± 11.8 years) who were histopathologically diagnosed with HT. The two-dimensional ultrasound images and videos of all thyroid nodules were analyzed by the CAD system and two radiologists with different experiences according to ACR TI-RADS. The diagnostic cutoff values of ACR TI-RADS were divided into two categories (TR4 and TR5), and then the sensitivity, specificity, and area under the receiver operating characteristic curve (AUC) of the CAD system and the junior and senior radiologists were compared in both cases. Moreover, ACR TI-RADS classification was revised according to the results of the CAD system, and the efficacy of recommended fine-needle aspiration (FNA) was evaluated by comparing the unnecessary biopsy rate and the malignant rate of punctured nodules. Results: The accuracy, sensitivity, specificity, PPV, and NPV of the CAD system were 0.876, 0.905, 0.830, 0.894, and 0.846, respectively. With TR4 as the cutoff value, the AUCs of the CAD system and the junior and senior radiologists were 0.867, 0.628, and 0.722, respectively, and the CAD system had the highest AUC (P < 0.0001). With TR5 as the cutoff value, the AUCs of the CAD system and the junior and senior radiologists were 0.867, 0.654, and 0.812, respectively, and the CAD system had a higher AUC than the junior radiologist (P < 0.0001) but comparable to the senior radiologist (P = 0.0709). With the assistance of the CAD system, the number of TR4 nodules was decreased by both junior and senior radiologists, the malignant rate of punctured nodules increased by 30% and 22%, and the unnecessary biopsies of benign lesions were both reduced by nearly half. Conclusions: The CAD system based on deep learning can improve the diagnostic performance of radiologists in identifying benign and malignant thyroid nodules in the context of Hashimoto's thyroiditis and can play a role in FNA recommendations to reduce unnecessary biopsy rates.

Abstract:Thyroid nodules are exceedingly common, and most nodules are benign. The classification of thyroid nodules can greatly reduce the cost of surgery. Current research focuses on the classifications of benign and malignant tumors in the diagnosis of thyroid nodules, but there are few studies that are based on the combination of CAD and the expriences based inference of radiologists. Following the necessity of manual intervention in diagnoses that use a CAD system, we prospectively developed a new method for classifying thyroid nodules that integrate human inference and classifiers for ultrasound images.
A Clinical Assessment of an Ultrasound Computer-Aided Diagnosis System in Differentiating Thyroid Nodules with Radiologists of Different Diagnostic Experience

Abstract:IntroductionThis study aimed to assess the diagnostic performance and the added value to radiologists of different levels of a computer-aided diagnosis (CAD) system for the detection of thyroid cancers.Methods303 patients who underwent thyroidectomy from October 2018 to July 2019 were retrospectively reviewed. The diagnostic performance of the senior radiologist, the junior radiologist, and the CAD system were compared. The added value of the CAD system was assessed and subgroup analyses were performed according to the size of thyroid nodules.ResultsIn total, 186 malignant thyroid nodules, and 179 benign thyroid nodules were included; 168 were papillary thyroid carcinoma (PTC), 7 were medullary thyroid carcinoma (MTC), 11 were follicular carcinoma (FTC), 127 were follicular adenoma (FA) and 52 were nodular goiters. The CAD system showed a comparable specificity as the senior radiologist (86.0% vs. 86.0%, p > 0.99), but a lower sensitivity and a lower area under the receiver operating characteristic (AUROC) curve (sensitivity: 71.5% vs. 95.2%, p < 0.001; AUROC: 0.788 vs. 0.906, p < 0.001). The CAD system improved the diagnostic sensitivities of both the senior and the junior radiologists (97.8% vs. 95.2%, p = 0.063; 88.2% vs. 75.3%, p < 0.001).ConclusionThe use of the CAD system using artificial intelligence is a potential tool to distinguish malignant thyroid nodules and is preferable to serve as a second opinion for less experienced radiologists to improve their diagnosis performance.

Abstract:This paper proposes a computer-aided diagnosis system which can automatically detect thyroid nodules (TNs) and discriminate them as benign or malignant. The system firstly uses variational level set active contour with gradients and phase information to complete automatic extraction of the boundaries of thyroid nodules images. Then according to thyroid ultrasound images and clinical diagnostic criteria, a new feature extraction method based on the fusion of shape, gray and texture is explored. Due to the imbalance of thyroid sample classes, this paper introduces a weight factor to improve support vector machine, offering different classes of samples with different weights. Finally, thyroid nodules are classified and discriminated by the improved support vector machine. Experiments show that the efficiency of discrimination on benign and malignant thyroid nodules is improved.
