Abstract:Objective:To summarize nursing experience of patients treated with sellar pituitary tumor resection by endoscopic endonasal transsphenoidal approach.Methods:Twelve patients with pituitary tumors who underwent sellar pituitary tumor resection by endoscopic endonasal transsphenoidal approach were treated with subsidiary nursing measures before and during the operation.Results:Procedures were all smooth successful on the 12 patients,no serious complications were observed and all patients recovered well.Conclusion:Preoperative psychological care,skilled technique during the operation and postoperative instrument maintenance work can help to improve the success rate of the procedure.

Perioperative Nursing of Pituitary Tumor Surgery by Single Nostril Transsphenoidal Approach in 47 Cases

Abstract:Objective:To summarize the perioperative nursing of pituitary tumor surgery by single nostril transsphenoidal approach and discuss corresponding nursing measures.Method:Did a good job in mental nursing and preoperative preparation,took correct posture after operation,and strengthened the observation and nursing on consciousness,pupil,vital sign and complications.Result:In 47 cases,all the cases recovered well excepted 5 cases complicated with cerebrospinal rhinorrhea.Conclusion:Good preoperative nursing,the observation of conditions after operation and early discovery of complications are the key to the success of pituitary tumor surgery.

Abstract:Objective: The endoscopic transsphenoidal pituitary surgery has gained popularity and has shown excellent results with a more comfortable postoperative course. However, the quality of the early postoperative course is not well-established in endoscopic transsphenoidal pituitary surgery. We hypothesized that the quality of the early postoperative course would be improved when an enhanced recovery after surgery (ERAS) protocol and minimally invasive endoscopic transsphenoidal pituitary surgery is implemented. Methods: We implemented a perioperative management ERAS protocol for endoscopic transsphenoidal pituitary surgery by an experienced surgeon (Yuehui Ma) in our department from January 2018. From then the endoscopic transsphenoidal pituitary surgery was implemented with a minimally invasive technique, such as bony sella reconstruction and partial nasal packing. We compared the results of 78 endoscopic transsphenoidal pituitary surgery cases during the initiation of the ERAS protocol and minimally invasive technique implementation: 37 cases in the control group and 41 cases in the ERAS group. Outcomes assessed included the effectiveness and security of surgery, postoperative hospital length of stay (LOS), and postoperative status on postoperative day 1 (POD1). Results: Postoperative status on POD1, such as nasal ventilation, out of bed, headache score, and liquid supplement, had significant improvement (P < 0.05). The median postoperative LOS decreased from 8 days in the control group to 3 days in the ERAS group (P < 0.05). The ERAS group had better economic benefit with fewer hospital charges (P < 0.05). There was no difference in the early postoperative diabetes insipidus and 30-day readmission for epistaxis, hyponatremia, or other complications between the two groups. Conclusion: The quality of the early postoperative course was improved when a neurosurgical ERAS protocol and minimally invasive endoscopic transsphenoidal pituitary surgery with partially nasal packing were implemented. Endoscopic transsphenoidal pituitary day surgery could be recommended in some classes of patients though further evaluation in large case studies is warranted.

Abstract:OBJECTIVE To explore the risk factors of postoperative neurosurgical patients with intracranial infec‐tion ,so as to provide the basis for clinical minimizing and avoiding the risk of intracranial infection .METHODS Totally 344 cases of hospitalized patients with craniotomy in our hospital from Mar .2011 to Jul .2015 were chosen as study objects .According to the results of postoperative infection ,the independent risk factors of intracranial in‐fection were analyzed by logistic regression analysis .SPSS 19 .0 was explored for statistical analysis .RESULTS Totally 344 patients were checked and 32 of them were found to be infected with the infection rate of 9 .30% .Ex‐ternal ventricular drainage ,ASA score ,cerebrospinal fluid leakage and operation time were proved to be related risk factors causing infections (P<0 .05) .Multi‐factor non‐condition logistic regression analysis showed that ex‐ternal ventricular drainage ,ASA score ,cerebrospinal fluid leakage and operation time were independent risk fac‐tors causing intracranial infections after operation in the department of neurosurgery (P<0 .05) .CONCLUSION The factors that lead to postoperative intracranial infection in the department of neurosurgery are various .Special attention must be paid on external drainage ,ASA score ,cerebrospinal fluid leakage and operation time to improve the prognosis of patients ,develop targeted preventive measures ,improve the treatment effect ,and reduce postop‐erative infection rate .

Abstract:Background - Up to now, the risk factors related to intracranial infections after transsphenoidal pituitary odenomectomy remain controversial. Purpose - To analyze the risk factors related to intracranial infections after transsphenoidal pituitary adenomectomy under an endoscope, and to provide evidence for preventing and controlling the occurrence and development of infections. Methods - A total of 370 patients receiving endoscopic transsphenoidal pituitary adenomectomy in our hospital from January 2014 to October 2017 were selected. The risk factors related to postoperative intracranial infections were analyzed. The hospitalization lengths and expenditures of patients with and without intracranial infections were compared. Results - Of the 370 patients, 18 underwent postoperative intracranial infections, with the infection rate of 4.86%. Intraoperative blood loss >120 mL, cerebrospinal leakage, diabetes, preoperative use of hormones, macroadenoma as well as surgical time >4 h all significantly increased the infection rate (P<0.05). Preoperative use of antibacterial agents prevented intracranial infection. Compared with patients without intracranial infections, the infected ones had significantly prolonged hospitalization length and increased expenditure (P<0.05). Discussion - It is of great clinical significance to analyze the risk factors related to intracranial infection after endoscopic transsphenoidal pituitary adenomectomy, aiming to prevent and to control the onset and progression of infection. Conclusion - Intracranial infections after endoscopic transsphenoidal pituitary adenomectomy were affeded by many risk factors, also influencing the prognosis of patients and the economic burden.
Analysis of risk factors and preventive strategies for intracranial infection after neuroendoscopic transnasal pituitary adenoma resection

Abstract:Objective: To analyse the risk factors for intracranial infection after neuroendoscopic transnasal pituitary adenoma resection (NTPAR) to provide a reference for the prevention and treatment of postoperative intracranial infection. Methods: The clinical data of 387 patients who underwent NTPAR in the Department of Neurosurgery of the First People's Hospital of Yichang from March 2013 to March 2021 were retrospectively analysed. The patients were divided into an infected group and a noninfected group according to the occurrence of intracranial infection. The detailed clinical data of the two groups were collected. Univariate and multivariate logistic regression was used to analyse the risk factors for intracranial infection after NTPAR. Results: Among the 387 surgical patients, 32 patients (8.27%) were in the intracranially infected group and 355 patients (91.73%) were in the noninfected group. The results of the univariate analysis suggested that age > 45 years, tumour size > 1 cm, operation time > 240 min, blood loss > 400 ml, Kelly Grade of cerebrospinal fluid (CSF) leakage > Grade 2, postoperative CSF leakage, lumbar cistern drainage and blood transfusion were the influencing factors for postoperative intracranial infection, while the results of multivariate logistic regression analysis implied that intraoperative CSF leakage (Kelly Grade > 2) and postoperative CSF leakage were independent influencing factors for intracranial infection after NTPAR, and perioperative use of antibiotics was an independent protective factor for postoperative intracranial infection. Conclusions: There are a variety of risk factors for intracranial infection after NTPAR, which indicates that it is necessary to develop different repair strategies for CSF leakage according to the Kelly Grade, timely treatment of postoperative CSF leakage and perioperative use of antibiotics. These measures have been shown to effectively reduce the probability of intracranial infection after NTPAR.

The Incidence of Radiologic Evidence of Sinusitis Following Endoscopic Pituitary Surgery: A Multi-Center Study

Abstract:Background: Endoscopic endonasal skull base surgery has become a viable alternative to open procedures for the surgical treatment of benign and malignant lesions in the sinonasal and skull base regions. As in sinus surgery, skull base surgery may cause crusting and posterior rhinorrhea, particularly when a nasoseptal flap is required for skull base reconstruction. Post-operative radiological sinonasal findings have been reported previously with no clear correlation with intraoperative decision-making. As in open surgery, endoscopic surgery is not standardized and there is variability in the intervention to assist with exposure and skull base repair. These modifications, including middle turbinate resection, nasoseptal flap, fat graft, and maxillary antrostomy have the potential for nasal morbidity. The aim of this study was to evaluate whether specific interventions during surgery or specific patient and tumor characteristics harbor a more significant risk of causing nasal morbidity post-operatively, as demonstrated by post-operative imaging. Methods: A retrospective analysis of all patients who underwent endoscopic endonasal skull base surgery for pituitary lesions at two major referral centers was performed. Data on demographic, clinical, and pathological features were collected, and pre- and post-operative imaging studies (computed tomography (CT) and magnetic resonance imaging (MRI)) were reviewed and scored according to the Lund-Mackay (LM) scoring system. Results: The study included 183 patients. Radiographic evidence of sinusitis was observed in 30 patients (LM score > 4) in post-operative imaging studies. Patients who underwent middle turbinectomy or nasoseptal flap were found to have significantly higher LM scores on follow-up imaging. A nasoseptal flap was found to be associated with an average increase in LM score of 1.67 points and middle turbinectomy with an average increase of 2.21 points. There was no correlation between tumor size and findings that were compatible with sinusitis on post-operative imaging. Conclusions: The findings of the present study suggest that endoscopic endonasal skull base surgery is associated with radiological evidence of sinusitis. Nasoseptal flap reconstruction and middle turbinectomy were strongly associated with radiographic sinusitis and should be judiciously performed during surgery. A clinical correlation is needed for further recommendations.

Abstract:Objective To analyze preventive measures and therapeutic methods of postoperative complications of endoscopic transsphenoidal pituitary adenoma resection.Methods A retrospective analysis was conducted on 175 cases of endoscopic transsphenoidal resections of pituitary adenoma between January 2014 and June 2016 at Neurosurgery Department of the First Affiliated Hospital of Zhengzhou University.A follow-up was performed on all cases at 3 to 24 months post surgery to observe postoperative complications.Results Among the 175 cases of endoscopic transsphenoidal resection of pituitary adenoma,gross total resection was achieved in 152 patients (86.8 ％),subtotal resection in 18 patients (10.3 ％),and partial resection in 5 patients (2.9 ％).Complications included 2 (1.1％) cases of transient postoperative visual worsening due to compression of the optic nerve by hematoma in 1 case and tamponade in the other,20 (11.4％) cases of postoperative pituitary dysfunction,39 (22.3 ％) cases of transient diabetes insipidus,7 (4.0 ％) cases of cerebrospinal fluid rhinorrhea,3 (1.7％) cases of intracranial infection,2 (1.1％) cases of permanent loss of smell and 1 (0.6％) case of sphenoid sinus cyst.Intraoperative and postoperative cerebrospinal fluid leakage occurred in 71 cases and 6 cases (8.5％),respectively.Among the 104 patients without cerebrospinal fluid leakage during surgery,1 case (1.0％)developed cerebrospinal fluid leakage post operation,and the difference was statistically significant (P =0.037).Out of 9 cases of tumor recurrence undergoing secondary surgery,3 cases developed postoperative cerebrospinal fluid leakage.Among the 166 cases undergoing one operation,the cerebrospinal fluid leakage occurred in 4 (2.4％) cases,and the difference was statistically significant (P =0.000).There was no death in this series.Conclusion Endoscopic transsphenoidal resection of pituitary adenoma seems to have relatively low incidence of complications and their majority could be prevented.
Risk Factors for Isolated Sphenoid Sinusitis after Endoscopic Endonasal Transsphenoidal Pituitary Surgery

Abstract:(1) Background: Transsphenoidal pituitary surgery can be conducted via microscopic or endoscopic approaches, and there has been a growing preference for the latter in recent years. However, the occurrence of rare complications such as postoperative sinusitis remains inadequately documented in the existing literature. (2) Methods: To address this gap, we conducted a comprehensive retrospective analysis of medical records spanning from 2018 to 2023, focusing on patients who underwent transsphenoidal surgery for pituitary neuroendocrine tumors (formerly called pituitary adenoma). Our study encompassed detailed evaluations of pituitary function and MRI imaging pre- and postsurgery, supplemented by transnasal endoscopic follow-up assessments at the otolaryngology outpatient department. Risk factors for sinusitis were compared using univariate and multivariate logistic regression analyses. (3) Results: Out of the 203 patients included in our analysis, a subset of 17 individuals developed isolated sphenoid sinusitis within three months postoperation. Further scrutiny of the data revealed significant associations between certain factors and the occurrence of postoperative sphenoid sinusitis. Specifically, the classification of the primary tumor emerged as a notable risk factor, with patients exhibiting nonfunctioning pituitary neuroendocrine tumors with 3.71 times the odds of developing sinusitis compared to other tumor types. Additionally, postoperative cortisol levels demonstrated a significant inverse relationship, with lower cortisol levels correlating with an increased risk of sphenoid sinusitis postsurgery. (4) Conclusions: In conclusion, our findings underscore the importance of considering tumor classification and postoperative cortisol levels as potential predictors of postoperative sinusitis in patients undergoing transsphenoidal endoscopic pituitary surgery. These insights offer valuable guidance for clinicians in identifying at-risk individuals and implementing tailored preventive and management strategies to mitigate the occurrence and impact of sinusitis complications in this patient population.

Intracranial infection and acute obstructive hydrocephalus after lateral ventricle central neurocytoma resection: case report and review of the literature

Abstract:Background Central neurocytoma (CN) is a rare neurogenic intraventricular tumor that accounts for 0.25–0.5% of all intracranial tumors. Surgery is the cornerstone of therapy in most instances. Postoperative complications such as intracranial infection and hydrocephalus may be serious and even fatal. However, there are few reports on the management of complications after CN resection.Case presentation: The authors described a 41-year-old male patient, whose CN was near the interventricular foramen with a high Ki-67 index. Seven days following surgery, an intracranial infection struck the patient. On the seventeenth day, the patient experienced signs of obstructive hydrocephalus, including headache, unconsciousness, and gatism. One month after surgery, the patient recovered and was discharged with no sequela through high-grade antibiotics, lumbar cisterna drainage and external ventricular drainage.Conclusions For CN with elevated Ki-67 or growing in the lateral ventricle, particularly in the anterior region close to the interventricular foramen, obstructive hydrocephalus was more likely to be complicated after surgery. The temporary external ventricular drainage may be a substitute for VP shunt, avoiding maintenance risks and complications of VP shunt during long-term drainage. Besides, we also need to etiologically personalize the treatment approaches based on the various stages of patient for maximum clinical benefit.

Abstract:Objective: To investigate the clinical indication, key operating, points advantages & disadvantages of endoscopic neurosurgery. Methods: A total of 69 patients with intracranial disorders were treated by endoscopic neurosurgery, 6 suffering from pituitary adenoma, 21 from intracranial arachnoid cysts, 18 from hydrocephalous, 6 from septum pellucidum, 8 from intracranial epidermoid cyst, 9 from angioreticuloma and 1 from intracranial multiple metastatic tumor. Results: Clinical symptoms and signs were relieved in various degrees, and the clinical manifestation and CT and MR image were obviously improved in all patients after operation. Conclusion: Endoscopic neurosurgery is a safe and effective treatment methord for some patients with intracranial disorders. It is characteristic of less operation damage, light patient's reaction and faster rehabilitation for the patients following operation. Therefore, it is now the first choice for the minimally invasive neurosurgery.

Abstract:In this study, we analyze and discuss the treatments of postoperative nasal complications after endonasal transsphenoidal resection of pituitary neoplasms (PNs). We performed 129 endonasal transsphenoidal resections of PNs and analyzed and treated cases with nasal complications. After endonasal transsphenoidal resection of PNs, there were 26 cases of postoperative nasal complications (20.1%), including nasal hemorrhage (4.8%), cerebrospinal fluid rhinorrhea (6.9%), sphenoid sinusitis (2.3%), atrophic rhinitis (1.6%), olfactory disorder (1.6%), perforation of nasal septum (0.8%), and nasal adhesion (2.3%). All patients clinically recovered after therapy, which included treatment of the cavity through nasal endoscopy, intranasal corticosteroids, and nasal irrigation. We propose that regular nasal endoscopic review, specific nasal medications, and regular nasal irrigation can effectively clear nasal mucosal hyperemia-induced edema and nasal/nasoantral secretions, as well as promote regeneration of nasal mucosa, prevent nasal adhesion, maintain the sinus cavity drainage, and accelerate the recovery of the physiological function of the paranasal sinus. Timely treatment of patients with nasal complications after endonasal transsphenoidal resections of PNs could greatly relieve the clinical symptoms. Nasal cleaning is very beneficial to patients after surgery recovery.

Abstract:OBJECTIVE Postoperative intracranial infection in neurosurgery department patients undergoing craniot‐omy is a severe complication in department of neurosurgery ,which may lead to serious sequelae and even endan‐gers the lives of the patients .As antibiotics are initially used for clinical prevention of postoperative infection ,the incidence of the postoperative intracranial infection is remarkably decreased in the patients undergoing craniotomy ;however ,with the abuse of antibiotics and increase of drug‐resistant strains ,the incidence of the postoperative in‐tracranial infection then shows an upward trend .This review briefly describes the clinical characteristics of the postoperative intracranial infection ,pathogenesis ,pathogenic bacteria ,risk factors ,diagnosis approaches ,treat‐ment methods ,and prognosis ;the recommendations for prevention ,diagnosis ,and treatment of the postoperative intracranial infection are put forward for the patients undergoing craniotomy .

Abstract:BACKGROUND The neuroendoscopic approach has the advantages of a clear operative field, convenient tumor removal, and less damage, and is the development direction of modern neurosurgery. At present, transnasal surgery for sphenoidal pituitary tumor is widely used. But it has been found in clinical practice that some patients with this type of surgery may experience post-operative nausea and vomiting and other discomforts. AIM To explore the effect of reserved gastric tube application in the neuroendoscopic endonasal resection of pituitary tumors. METHODS A total of 60 patients who underwent pituitary adenoma resection via the endoscopic endonasal approach were selected and randomly divided into the experimental and control groups, with 30 in each group. Experimental group: After anesthesia, a gastric tube was placed through the mouth under direct vision using a visual laryngoscope, and the fluid accumulated in the oropharynx was suctioned intermittently with low negative pressure throughout the whole process after nasal disinfection, during the operation, and when the patient recovered from anesthesia. Control group: Given the routine intraoperative care, no gastric tube was left. The number of cases of nausea/vomiting/aspiration within 24 h post-operation was counted and compared between the two groups; the scores of pharyngalgia after waking up, 6 h post-operation, and 24 h post-operation. The frequency of postoperative cerebrospinal fluid leakage and intracranial infection were compared. The hospitalization days of the two groups were statistically compared. RESULTS The times of postoperative nausea and vomiting in the experimental group were lower than that in the control group, and the difference in the incidence of nausea was statistically significant (P < 0.05). After the patient woke up, the scores of sore throat 6 h after the operation and 24 h after operation were lower than those in the control group, and the difference was statistically significant (P 0.05). The hospitalization days of the experimental group was lower than that of the control group, and the difference was statistically significant (P < 0.05). CONCLUSION Reserving a gastric tube in the endoscopic endonasal resection of pituitary tumors, combined with intraoperative and postoperative gastrointestinal decompression, can effectively reduce the incidence of nausea, reduce the number of vomiting and aspiration in patients, and reduce the complications of sore throat The incidence rate shortened the hospitalization days of the patients.

Abstract:Following the introduction of fully endoscopic techniques for the resection of pituitary tumors, there was a rapid expansion of the indications for endonasal endoscopic surgery to include extrasellar tumors of the skull base. These techniques offer significant advantages over traditional open surgical approaches to the skull base, including improved tumor resection, and better post-operative neurological outcomes. Following their introduction, however, the initial rate of post-operative CSF leak was unacceptably high. Post-operative CSF leak following skull base surgery is a major source of morbidity, and can lead to the development of life-threatening intracranial infection. The use of vascularized naso-septal flaps transformed the management of these patients, significantly reducing the rate of post-operative CSF leak and increasing the number of patients that could benefit from this less invasive treatment modality. Adequate repair of iatrogenic defects in the skull base is of crucial importance for patients with skull base tumors, as the development of a post-operative CSF leak, and the associated complications can significantly delay the administration of the adjunctive oncological therapies these patients require. In this review, we provide an overview of the latest evidence regarding skull base reconstruction following endoscopic skull base surgery, and describe the skull base repair technique in use at our institution.

Abstract:Introduction: Cerebro spinal fluid (CSF) leakage is common and might lead to severe postoperative complications after endoscopic transsphenoidal pituitary adenoma resection. However, the risk factors of postoperative CSF leakage are still controversial. This article presents a systematic review to explore the explicit risk factors of CSF leakage after endoscopic transsphenoidal pituitary adenomere section. Methods: PRISMA and AMSTAR guidelines were followed to assess the methodological quality of the systematic review. PubMed, Medline, Embase, Web of Science, Cochrane, Clinical Trails, CNKI, CBM, Wan Fang, and VIP databases were searched for all studies on postoperative CSF leak risk factors. The quality of the included studies was assessed by the Newcastle-Ottawa scale. Review Manager 5.4 software was used to calculate the pooled effect size of potential factors with statistical significance. Results: A total of 6775 patients with pituitary adenoma across 18 articles were included, containing 482 cases of postoperative CSF leakage (accounting for 7.11%). All of the articles had a quality score > 5, indicating good quality. Meta-analysis showed that an increased risk of CSF leak was found for higher levels of BMI (MD=1.91, 95% CI (0.86,2.96), bigger tumor size [OR=4.93, 95% CI (1.41,17.26)], greater tumor invasion (OR=3.01, 95% CI (1.71, 5.31), the harder texture of tumor [OR=2.65, 95% CI (1.95,3.62)], intraoperative cerebrospinal fluid leakage [OR=5.61, 95% CI (3.53,8.90)], multiple operations [OR=2.27, 95% CI (1.60,3.23)]. Conclusion: BMI, multiple operations, tumor size, tumor invasion, hard texture, and intraoperative cerebrospinal fluid leakage are the risk factors of postoperative CSF leakage. Clinical doctors should pay attention to these risk factors, and conduct strict skull base reconstruction and careful postoperative management.

A systematic review of effects and complications after transsphenoidal pituitary surgery: endoscopic versus microscopic approach

Abstract:Background: Microscopic transsphenoidal surgery (MTS) has been considered as the gold standard for transsphenoidal pituitary surgery, but nowadays endoscopic transsphenoidal surgery (ETS) has become popular due to its wide view and improved lighting.Material and methods: The electronic databases were systematically searched, and the meta-analyses of the eligible studies that evaluated endoscopic versus microscopic methods in patients with pituitary surgery were conducted with Review Manager 5.0. The primary outcomes included visual improvement, gross tumor removal (GTR), cerebrospinal fluid (CFS) leak, diabetes insipidus (DI), other complications, and length of hospital stay. The Egger's test was conducted to estimate possible publication bias.Results: In total, 13 articles eventually met the inclusion criteria. The meta-analyses suggested that the differences with regard to visual improvement, overall complication rate, GTR, CSF leak, diabetes insipidus (DI), meningitis, visual impairment, syndrome of inappropriate antidiuretic hormone secretion (SIADH), new onset hypopituitarism, and hypothyroidism between the endoscopic and microscopic groups were not statistically significant. The length of hospital stay was much longer with the microscopic approach when compared with the endoscopic method.Conclusion: The endoscopic and microscopic approaches show similar effects and complication rates. The endoscopic technique could be adopted as a reasonable alternative in pituitary surgery.

Abstract:Objective To exlpore the prevention and therapy methods of sphenoid sinusitis after transsphenoidal pituita-ry adenomas resection.Methods Totally 129 patients underwent transsphenoidal pituitary adenomas resection.The postoperative sphenoid sinusitis was observed and analyzed.Results A total of 6 cases (4.6%)complicated with sphenoid sinusitis in all 129 cases.All of them were cured after the corresponding specialized subject treatment under nasal endoscope,like regular surgical cavity cleanning,nasal steroid spray,and nasal irrigation.Conclusion The complication of sphenoid sinusitis after transsphenoidal pituitary adenomas resection should be timely disposal under na-sal endoscopy so as to effectively improve the patientsclinical symptoms.
