Laparoscopic Versus Open Complex Hepatectomy for Hepatocellular Carcinoma: a Propensity Score Matching Study

Abstract:Introduction: Laparoscopic hepatectomy (LH) for large tumor and tumor located in posterolateral segments and laparoscopic major hepatectomy are regarded as complex resection. Whether LH can be feasible to complex resection for hepatocellular carcinoma (HCC) remains controversial. Method: All patients with HCC performed LH (n=648) and open hepatectomy (n=1506) from January 2012 to June 2018 were reviewed retrospectively. Among these patients, complex resection was retrieved in each subgroup and a 1:1 matching was conducted respectively. Result: After the matching, 630 patients in each group were included in the final analysis. Of these, 143 cases were large tumor, 211 cases had tumor located in posterolateral segments, and 70 cases performed major hepatectomy, and each case could find a matching case in the open group. Patients in the laparoscopic group experienced longer operative time (245 vs. 219 min, P<0.001), but less blood loss (215 vs. 295 ml, P<0.001) and lower incidence of severe complications (2.5% vs. 9.0%, P<0.001). The 1-, 3-, and 5-year overall survival rates were similar between the laparoscopic and open group (93.1% vs. 88.1%, 77.4% vs. 75.6%, and 67.4% vs. 67.1%, P=0.141). In each subgroup analysis for complex hepatectomy, the operative time in the laparoscopic group was always longer, but intraoperative blood loss was similar to their open counterparts. However, patients in the laparoscopic group experienced more implementation of Pringle maneuver and longer time of inflow clamping, but lower postoperative complications than that in the open group. Subgroup survival analysis showed no significant difference in long-term survival with regarding the two approaches of complex hepatectomy for HCC (both P value >0.05) Conclusion: Complex hepatectomy in a laparoscopic approach is feasible to some selective patients. Patients performed complex LH experienced longer operative time and similar intraoperative blood loss, but lower postoperative complications and similar long-term survival when compared with an open approach.
