Abstract:BACKGROUND:Recombinant activated factor VIIa (rFVIIa) is a prohemostatic agent initially approved for use in hemophilia patients and has also been used for a diverse range of off-label indications in the context of massive uncontrolled blood loss; however, no convincing evidence exists regarding the optimal dose of rFVIIa to treat uncontrolled bleeding in surgical patients. AIM:To evaluate the effects and safety of a very low dose of rFⅦa in patients with uncontrolled perioperative bleeding in the surgical intensive care unit (ICU). METHODS:55 patients from Beijing Hospital, who received rFⅦa between July 2004 and November 2018 for uncontrolled perioperative bleeding were included. The controls were matched for age, sex, severity, and operation type. The baseline demographics, survival, changes in bleeding and transfusion, coagulation parameters and complications were analyzed. RESULTS:A low dose of rFⅦa (2.0∼3.6 mg, with a median dose of 39.02 μg/kg) appears to be effective in controlling massive hemorrhage (with an effective rate of 74.55%), and can reduce volume of red blood cell transfusion, improve coagulation status, while has a relatively low risk of thromboembolic complications (3.6%). CONCLUSION:In patients with uncontrolled perioperative bleeding, a low dose of rFⅦa could be used when traditional methods are ineffective.
Use of recombinant activated factor VII for the treatment of perioperative bleeding in noncardiac surgery patients without hemophilia: A systematic review and meta-analysis of randomized controlled trials

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Purpose</h3>To evaluate the efficacy and safety of perioperative use of recombinant activated factor VII (rFVIIa) in noncardiac patients.<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Materials and methods</h3>We searched electronic databases for randomized controlled trials (RCTs) that involved the use of rFVIIa through December 13, 2019 in noncardiac patients without hemophilia. Two investigators extracted the related data and assessed the quality of the included trials.<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3>Eleven RCTs examining 993 perioperative patients were ultimately included. The use of rFVIIa did not decrease all-cause mortality (RR:0.90; 95% CI:0.50,1.64; I2 = 0.0%; P = 0.738), shorten the length of ICU (SMD:-0.15; 95% CI:-0.47,0.17; I2 = 0.0%; P = 0.346) or hospital (SMD:0.42; 95% CI:-0.05,0.89; I2 = 0.0%; P = 0.078) stay, or increase incidence of the thromboembolic events (RR:1.30; 95% CI:0.70,2.41; I2 = 0.0%; P = 0.403) among perioperative patients. However, individual RCT analyses showed that the use of rFVIIa could reduce the volume of blood loss (including prostatic cancer, severe acute pancreatitis (SAP), and spinal disease) and the transfusion of RBCs (including prostatic cancer, SAP, and spinal disease) and FFP (SAP) in a subset of perioperative patients. Publication bias was not present.<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3>For perioperative hemorrhagic patients, rFVIIa-based hemostatic therapy showed no effect on mortality, ICU or hospital LOS, or the rate of thromboembolic events, although it appears to decrease blood loss and reduce the need for blood product transfusion in a subset of patients.

Abstract:Background The blood saving efficacy of TXA in cardiac surgery has been proved in several studies, but TXA dosing regimens were varied in those studies. Therefore, we performed this study to investigate if there is a dose dependent in-vivo effect of TXA on fibrinolysis parameters by measurement of fibrinolysis markers in adults undergoing cardiac surgery with CPB. Methods A double-blind, randomized, controlled prospective trial was conducted from February 11, 2017 to May 05, 2017. Thirty patients undergoing cardiac valve surgery were identified and randomly divided into a placebo group, low-dose group and high-dose group by 1: 1: 1. Fibrinolysis parameters were measured by plasma levels of D-Dimers, plasminogen activator inhibitor-1 (PAI-1), thrombin activatable fibrinolysis inhibitor (TAFI), plasmin-antiplasmin complex (PAP), tissue plasminogen activator (tPA) and thrombomodulin (TM). Those proteins were measured at five different sample times: preoperatively before the TXA injection (T 1 ), 5 min after the TXA bolus (T 2 ), 5 min after the initiation of CPB (T 3 ), 5 min before the end of CPB (T 4 ) and 5 min after the protamine administration (T 5 ). A Thrombelastography (TEG) and standard coagulation test were also performed. Results Compared with the control group, the level of the D-Dimers decreased in the low-dose and high-dose groups when the patients arrived at the ICU and on the first postoperative morning. Over time, the concentrations of PAI-1, TAFI, and TM, but not PAP and tPA, showed significant differences between the three groups ( P < 0.05). Compared with the placebo group, the plasma concentrations of PAI-1 and TAFI decreased significantly at the T3 and T4 ( P < 0.05); TAFI concentrations also decreased at the T5 in low-dose group ( P < 0.05). Compared with the low-dose group, the concentration of TM increased significantly at the T4 in high-dose group. Conclusions The in-vivo effect of low dose TXA is equivalent to high dose TXA on fibrinolysis parameters in adults with a low bleeding risk undergoing valvular cardiac surgery with cardiopulmonary bypass, and a low dose TXA regimen might be equivalent to high dose TXA for those patients. Abstract:Objective:To investigate the efficacy and safety of recombinant activated factor Ⅶ(rFⅦa)on uncontrolled perioperative hemorrhage in elderly patients in surgical intensive care unit(SICU).Methods:Clinical data of 27 elderly patients with uncontrolled perioperative hemorrhage treated successfully with rFⅦa in surgery-ICU of our hospital from May 2004 to December 2018 were retrospectively analyzed.And the application method and experience were summarized.Results:Of the 27 patients, 16 cases were cured, 11 cases died, and the total fatality rate was 40.74%(11/27). The bleeding stopped in 23 cases, four cases died of uncontrolled peri-operative hemorrhage, and the hemorrhage-caused fatality rate was 14.81%(4/27). Seven patients died of primary diseases after the control of bleeding, with a fatality rate of 25.93%(7/27 ). The thrombin time(TT), prothrombin time(PT), activated partial thromboplastin time(APTT)and international normalized ratio(INR)were reduced and the prothrombin activity(AT)was increased after versus before the application of rFⅦa( Z=-2.197, -3.180, -2.271, -2.803 and 2.756, P=0.028, 0.001, 0.023, 0.005 and 0.006, respectively). Conclusions:rFⅦa has a better effect on uncontrolled peri-operative hemorrhage in elderly patients, when traditional treatments are ineffective.

Abstract:OBJECTIVE:To find a kind of quick and effective haemostasis to decrease the mortality of severe bleeding.METHODS:18 severe bleeding patients with different cause received recombinant activated factor VIIa (rFVIIa) were analyzed retrospectively.RESULTS:Of total 18 cases with severe bleeding, 13 cases cured, 3 cases were effective, 2 cases ineffective. The total clinical effective rate is 88.89%. After using rFVIIa, the PT, APTT and fibrinogen level of 6 DIC patients returned to normal within 12 hours; 13 patients whose the amount of bleeding can be evaluated stopped bleeding quickly. The fastest onset time was 10 min.CONCLUSION:rFVIIa can stanch severe bleeding for a variety of reasons rapidly and effectively, including coagulopathy, thrombocytopenia, and obstetric hemorrhage. Application of rFVIIa may decrease mortality, when conventional treatment is not valid.

Abstract:Background: Severe acute pancreatitis is a life-threatening disease. Patients with peripancreatic necrotic infection often require surgical removal of necrotic infected tissue and a wide debridement will cause blood loss and worsen the condition. Aim: To assess whether treatment with NovoSeven, a recombinant activated FVII (rFVIIa), could improve coagulation function and therefore reduce blood loss, blood transfusion and all-cause mortality during necrosectomy in patients with infected necrosis secondary to severe acute pancreatitis. Materials and Methods: Severe acute pancreatitis patients admitted to Nanjing Jinling Hospital for necrosectomy were enrolled and randomized to receive either standard treatment or standard treatment plus an intravenous infusion of rFVIIa (40 mu g per kilogram of body weight per hour) before operation. The prospectively defined primary end points were perioperative coagulation parameters (prothrombin time, activated partial thromboplastin time), blood transfusion unit and blood loss. The secondary end points were operation time, ICU stay and all-cause mortality at 28 days after the operation. Results: A total of 64 patients were enrolled (31 in the rFVIIa group and 33 in the control group). Treatment with rFVIIa was associated with a reduction in operation time, red blood cell and fresh froze plasma transfusion, blood loss and prothrombin time compared to the control group (p < 0.05 for all). Activated partial thromboplastin time and mortality were similar between the two groups (P > 0.05). Conclusion: Treatment with rFVIIa significantly improved the extrinsic coagulation function in patients with severe acute pancreatitis and was associated with decreased risk of bleeding. However, rFVIIa did not improve intrinsic coagulation or reduce over-cause mortality. (C) 2014 Elsevier Ltd. All rights reserved.

Abstract:Recombinant activated factor VII (rFVIIa) is a novel therapeutic agent for life-threatening massive gastrointestinal bleeding. We report a case of massive gastrointestinal bleeding in a 78-year-old female patient with respiratory and renal failure. After failure of management of the bleeding with routine pharmacotherapy, we gave the patient rFVIIa injection at the dose of 20 µg/kg and the bleeding was rapidly controlled. Adverse side effects of the drug were not observed in this patient.

Abstract:Introduction: Acquired haemophilia A (AHA) is an autoimmune disease that potentially leads to severe bleeding and has a high rate of mortality. This collaborative study aimed to assess the efficacy of the co-administration of FVIII and low-dose rFVIIa in patients with AHA.Materials and Methods: This study retrospectively compared the combined FVIII/low-dose rFVIIa therapy (initial dose range of 25-55 mu g/Kg) with the combined FVIII/PCC therapy and low-dose rFVIIa monotherapy. Adverse drug reactions and recurrent bleeding episodes were also monitored. Crude comparisons and the exact conditional logistic regression were performed to compare the outcomes between three treatment groups.'Results: First bleeding episodes of 56 consecutive patients from 5 centres were analyzed, and 37 bleeding episodes (66.1%) were determined to be severe. Specifically, the rate of bleeding control was significantly higher with the FVIII/low-dose rFVIIa therapy compared to that of the low-dose rFVIIa alone therapy or the FVIII/PCC therapy (58.3% vs. 41.7% vs. 95.0%, respectively). Analyzing of total 236 bleeding episodes showed a clear positive association between the early initiation of haemostatic treatment and efficacy. No therapy-related adverse events in which thrombosis predominated were reported.Conclusions: The combination of FVIII and low-dose rFVIIa offers an ideal haemostatic cover and may be promoted as a feasible and safe therapy protocol for patients with AHA. Abstract:Background: SCT800 (Omfiloctocog alfa) is the first domestic recombinant factor VIII approved in China based on pivotal study NCT03815318. The extension study (NCT03947567) is ongoing to evaluate the long term safety and efficacy in previously treated patients (≥12 years) with severe hemophilia A. Aims: To report the interim data for the extension study. Methods: Patients who completed the main study enrolled in the open‐label extension study and continued receiving 25-50 IU/kg SCT800 every other day or three times weekly. The primary endpoint was development of inhibitors; the key secondary endpoints included median annualized bleeding rate (ABR). Results: As of 24 Feb 2022, no inhibitors were observed in total 69 enrolled patients with a median exposure of 332 days (range 79-432 days). Adverse reactions occurred in 4.3% of the patients and SAEs were reported in 4 patients (5.8%). Median ABR (IQR) for all bleeds was 2.00 (4.10) in the pivotal study and 0.48 (1.46) during the extension (Table). Of the 178 reported bleeds, hemostatic efficacy was rated as “Excellent” or “Good” for 88.8% of the events. Improved joint function was observed in 67 patients with a mean total HJHS scores decreased by 3.4. Image:Summary/Conclusion: The interim data demonstrate favorable long-term safety of SCT800 and the maintenance of low ABR in patients with severe hemophilia A.
Inverse Relationship of Bleeding Risk with Clot Burden During Pulmonary Embolism Treatment with LMW Heparin.

Abstract:IntroductionClinically relevant bleeding occurs three times as frequently as recurrent venous thromboembolism in the modern early treatment of pulmonary embolism (PE) with fixed-dose, unmonitored anticoagulants. Unfractionated heparin (UFH) is monitored and adjusted to assure efficacy and minimize bleeding risk, but low molecular weight heparin (LMWH) is not. PE requires more anticoagulant than isolated deep venous thrombosis. Speculating that PE with low clot burden could lead to excess bleeding with unadjusted LMWH treatment but not with UFH, we compared PE patients receiving either UFH or LMWH with high and low clot burden for clinically significant bleeding in an observational study.Materials and MethodsPatients with acute PE at multiple Chinese teaching hospitals had been randomized to UFH or LMWH for initial treatment. These treatment cohorts had baseline measurement of pulmonary artery obstruction (PAO) score, which was prospectively separated into quartiles, lowest to highest PAO. All patients were followed for bleeding episodes, which were subsequently analyzed by quartile of PAO.ResultsTwo hundred seventy-four patients divided between the two groups had similar efficacy and safety outcomes (12 clinically significant bleeds in the UFH group vs 15 in the LMWH group). LMWH recipients with the smallest clot burdens (lowest PAO quartiles) had highest bleeding rates (Cochran-Armitage trend test, P trend=0.048), but there was no such trend for UFH recipients.ConclusionsFor UFH, excess anticoagulant pro-hemorrhagic potential is down-adjusted via activated partial thromboplastin time monitoring, but for LMWH it is not. For PE patients at high bleeding risk, UFH may be safer if the clot burden is small.
Severe Congenital Factor VII Deficiency with Normal Perioperative Coagulation Profile Based on ROTEM Analysis in a Hepatectomy

Abstract:Objective: Rare disease Background: Factor VII (FVII) deficiency is the most common autosomal-recessive bleeding disorder. FVII activity level (FVII: C) of 10-20% is often used as the threshold for administering activated recombinant FVII (rFVIIa) for patients undergoing major surgery. However, rFVIIa is expensive and carries the risk of a thromboembolic event, and thus should only be administered when truly indicated. Case Report: A 22-year-old woman with 8% FVII: C underwent a hepatectomy. Although there were no clinical signs of bleeding, peri-operative administration of rFVIIa was recommended by the hematologist (first dose at surgical incision, then 4 h later, then every 12 h until 48 h postoperatively). Intraoperatively, serials of ROTEM analysis were performed to evaluate the effect of rFVIIa administration. No significant effect of rFVIIa was seen on NATEM. Surgery was unremarkable, without any significant blood loss. The patient developed radial artery thrombosis 24 h postoperatively, the arterial line was removed, and rFVIIa was discontinued (PT: 14.6, FVII: C 36%). On POD 3, INR was elevated (3.15, FVII: C 3%). To correct INR, the patient was transfused 8 units of FFP, despite any signs of clinical bleeding. However, INR and FVII: C did not correct and the patient was discharged on POD 7 in a stable condition. Conclusions: Even with FVII: C of 8%, the ROTEM analysis revealed a normal coagulation status. The administration of rFVIIa did not improve the already normal baseline coagulation profile, but rather potentially led to an accelerated coagulation or hypercoagulable state and may have led to the radial artery thrombosis. We endorse the use of viscoelastic testing for hemostasis assessment and factor replacement in congenital FVII deficiency.
Experience of recombinant activated factor VII (NovoSeven) in the operating theatre and intensive care unit for the management of intracranial bleeding in nonhaemophilic patients

Abstract:Objective: Intracranial haemorrhage (ICH) is associated with high morbidity and mortality. Our aim was to explore the use of recombinant activated factor VII (rFVIIa NovoSeven Novo Nordisk, A/S, Bagsvaerd, Denmark) for the management of ICH in the operating theater and intensive care unit. Patients and methods: We reviewed all the records of nonhaemophilic patients entered into the haemostasis.com database who received rFVIIa for ICH. Results: Sixteen suitable patients were identified (mean age: 23.3 years; range: 1-58 years). The total dose of rFVIIa administered ranged from 31 to 270 microg/kg. Indications were stabilization of ICH (n=6), control of peri- or post-operative haemorrhage associated with neurosurgical procedures (n=8), or correction of coagulopathy prior to neurosurgical intervention (n=2). The majority (13/16 [81.25%]) required one dose of rFVIIa. A clinical effect (stabilization of bleed, reduction of peri- or post-operative haemorrhage, or prevention of excessive blood loss during neurosurgery) was seen in 14/16 (87.5%) patients. Some improvement in coagulation status was noted. No thromboembolic events were reported. One patient experienced massive elevation of D-dimer levels-an effect possibly due to rFVIIa. Two patients suffered adverse events unrelated to rFVIIa. Six deaths occurred, all attributable to underlying brain injury. Conclusion: This observational study suggests that rFVIIa is of value for the management of ICH in nonhaemophilic patients secondary to a range of aetiologies. These findings justify further investigation.

Abstract:Congenital factor VII deficiency (FVIID), caused by mutations in the F7 gene, is a rare autosomal recessive blood disorder with an estimated prevalence of 1:500,000. [ 1–3 ] FVIID can cause spontaneous bleeding, including menorrhagia, ecchymosis, epistaxis, gum bleeding, hematuria, gastrointestinal bleeding, hemarthrosis, and intracranial hemorrhage. [ 4 ] Unlike hemophilia, factor VII activity (FVII:C) does not serve as a predictor of hemorrhage severity. [ 5 ] The primary therapeutic option is replacement therapy (RT), which includes recombinant activated factor VII (rFVIIa), fresh frozen plasma (FFP), plasma-derived factor VII, and prothrombin complex concentrates. [ 6 ] Furthermore, 40% to 70% of patients remain asymptomatic, [ 4 , 7 , 8 ] for whom surgery is considered a major threat. [ 9 ] Most of them are occasionally diagnosed by a prolonged prothrombin time (PT) and a decreased FVII:C in preoperative coagulation tests. [ 10 ] Despite the absence of previous bleeding episodes, the inherent hemostatic defect is consistently a predisposition to excessive bleeding. In patients with coagulation factor deficiency, the prevention of surgical bleeding should be meticulously considered. [ 11 ] However, effective guidelines for the perioperative coagulation management of patients with FVIID have yet to be established, and consensus on the optimal strategy for rFVIIa infusion remains elusive. [ 12 ] Currently, the perioperative management of patients with FVIID are individualized, considering factors such as hemorrhage history, family history, FVII:C baseline, and the specific type of surgery. [ 10 ]

The Efficacy of Recombinant FVIII Low-Dose Prophylaxis in Chinese Pediatric Patients With Severe Hemophilia A: A Retrospective Analysis From the ReCARE Study

Abstract:Objective: This study explores the efficacy of recombinant factor VIII (rFVIII) low-dose prophylaxis in Chinese pediatric patients with severe hemophilia A from the Retrospective Study in Chinese Pediatric Hemophilia A Patients With rFVIII Contained Regular Prophylaxis (ReCARE) population. Methods: This is additional analysis of the multicenter, retrospective ReCARE study, in which the annual bleeding rate (ABR), annual joint bleeding rate (AJBR), and safety of > 12-week, low dose (10-30 IU/kg/wk) rFVIII prophylaxis divided into primary, secondary, and tertiary groups based on the joint status and joint bleeding history were analyzed. Results: A total of 57 patients (median age: 8.2 [0.4-17.3] years) from the ReCARE study receiving primary (n = 3), secondary (n = 21), and tertiary (n = 33) prophylaxes were included. Low-dose prophylaxis had significant bleeding reduction in all 3 groups compared to the baseline (S = 408.5, P < .001), with median ABR rates of -4.0 (-8.0 to -3.1), -4.0 (-24.0 to 8.0), and -13.9 (-110.6 to 20.6) in the primary, secondary, and tertiary groups, respectively, with a significant difference between the secondary and tertiary groups (P = .008). Median AJBR reduction rates were -2.3 (-6.3 to 8.4) and -14.9 (-61.5 to 19.1) in the secondary and tertiary groups, respectively. But there was no significant difference in AJBRs between the secondary and tertiary groups (P = .061), which was related to damaged joint status. Hence, longer prophylaxis was associated with better prevention of joint bleeding (P = .024). Conclusion: Despite significant ABR/AJBR reduction in all 3 groups, the efficacy of the primary prophylaxis was better than the secondary and tertiary prophylaxes.
Recombinant Factor VIIa for Variceal Bleeding in Patients with Advanced Cirrhosis: A Randomized, Controlled Trial

Abstract:A beneficial effect of recombinant activated factor VII (rFVIIa) in Child-Pugh class B and C patients with cirrhosis who have variceal bleeding has been suggested. This randomized controlled trial assessed the efficacy and safety of rFVIIa in patients with advanced cirrhosis and active variceal bleeding. At 31 hospitals in an emergency setting, 256 patients (Child-Pugh > 8; Child-Pugh B = 26%, C = 74%) were randomized equally to: placebo; 600 mu g/kg rFVIIa (200 + 4 x 100 mu g/kg); or 300 mu g/kg rFVIIa (200 + 100 mu g/kg). Dosing was intravenous at 0, 2, 8, 14, and 20 hours after endoscopy, in addition to standard vasoactive, prophylactic antibiotic, and endoscopic treatment. The primary composite endpoint consisted of failure to control 24-hour bleeding, or failure to prevent rebleeding or death at day 5. Secondary endpoints included adverse events and 42-day mortality. Baseline characteristics were comparable between groups. Administration of rFVIIa had no significant effect on the composite endpoint compared with placebo (P = 0.37). There was no significant difference in 5-day mortality between groups; however, 42-day mortality was significantly lower with 600 mu g/kg rFVIIa compared with placebo (odds ratio 0.31, 95% confidence interval = 0.13-0.74), and bleeding-related deaths were reduced from 12% (placebo) to 2% (600 mu g/kg). A marked heterogeneity in the failure rate in all treatment groups was observed across participating centers. Adverse events, including overall thromboembolic events, were comparable between groups. Conclusion: Treatment with rFVIIa had no significant effect on the primary composite endpoint compared with placebo. Therefore, decision on the use of this hemostatic agent in acute variceal bleeding should be carefully considered, because results of this study do not support the routine use of rFVIIa in this setting. Adverse events were comparable across groups.

Abstract:Objective To investigate the effects of recombinant activated factorⅦa ( rFⅦa) on patients with traumatic bleed-ing.Methods One hundred and thirty patients with traumatic massive hemorrhage treated between September 2012 and September 2015 were randomly divided into control and observation groups,65 in each group.The control group received conventional hemostatics to stop bleeding while the observe group received rFⅦa treatment in addition to conventional methods.The blood coagulation indicators including prothrombin time (PT),activated partial thromboplastin time (APTT) and plasma fibrinogen (FIB) were compared before and after 1 h,2 hs,4 hs,8 hs and 24 hs of treatment between the two groups.The blood transfusion volume,dosage of dopamine,inci-dence of multiple organ dysfunction syndrome and mortality were also compared between the two groups.Results In the observation group,APTT was shortened after 1 h of treatment,PT and APTT were decreased after 2 hs,4 hs,8 hs and 24 hs of treatment,and FIB was improved after 2 hs,4 hs and 8 hs of treatment when compared to the control group ( P<0.05) .Compared with the control group,vol-ume of blood transfusion and dosage of dopamine were decreased significantly(P<0.05).The incidence of multiple organ dysfunction syndrome and mortality in ICU were reduced in the observation group when compared to the control group.Conclusion Application of rFⅦa on patients with traumatic massive haemorrhage can strengthen the control of bleeding,reduce the hemorrhagic complication and improve the prognosis.

Abstract:Introduction Recombinant porcine factor VIII (rpFVIII; susoctocog alfa) is predicted to provide functional FVIII activity in patients with congenital haemophilia A with inhibitors (CHAWI). Aims To evaluate the efficacy and safety of rpFVIII in patients with CHAWI undergoing invasive procedures. Methods This phase 3, multicentre, single‐arm, open‐label study (NCT02895945) enrolled males aged 12–75 years with severe/moderately severe CHAWI who required surgical/invasive procedures. Patients received a loading dose of rpFVIII 1–2 h before surgery. The primary outcome was the proportion of all procedures with a 'good' or 'excellent' response (treatment success) on the global haemostatic efficacy assessment score. Results Of the eight dosed patients, five completed the study. Six of seven surgeries (85.7%; 95% confidence interval, 42.1–99.6) achieved treatment success; five were rated 'excellent', one was rated 'good'. Seven surgery‐related bleeding episodes occurred in three patients during the study, with none requiring additional surgical intervention. Overall, six of eight patients experienced 17 treatment‐emergent adverse events. Three patients developed de novo inhibitors to rpFVIII. Five patients reported anamnestic reactions, three to both human (h) FVIII (i.e., alloantibodies to exogenous FVIII detected with anti‐hFVIII assays) and rpFVIII, and two to hFVIII only. Four serious adverse events were considered related to rpFVIII (three anti‐rpFVIII antibody positive; one anamnestic reaction to hFVIII and rpFVIII). Conclusion Good haemostasis was achieved with rpFVIII during the immediate perioperative period. The study was terminated early because the study sponsor and health authorities determined that the risk of anamnestic reactions outweighs the benefits in this study population.

Abstract:[Objective]To explore the safety and value of middle and low dose of recombinant Ⅶa factor (rFⅦa)for the treatment of patients with hypertensive cerebral hemorrhage.[Methods]A prospective single-centre case control study was conducted.A total of 168 patients with hypertensive cerebral hemorrhage from Aug 2011 to Jan 2013 were admitted in our hospital.According to the recruitment criteria,86 patients were enrolled in this study including 27 patients in experiment group and 59 patients in control group.In the experi-ment group,patients were injected with rFⅦa 20 μg/kg within 2 hours after admission.Glasgow coma scale (GCS)and coagulation parameters of two groups at admission were recorded and compared.The amount of blood transfusion,length of stay in ICU,medical expense,mortality on day 30,and GOS on day 90 were fol-lowed up.[Results]Compared with before treatment,coagulation parameters in two groups after treatment were improved.Compared with control group,international normalized ratio(INR)was decreased(0.29±0.23 v.s 0.11 ± 0.39)and prothrombin time(PT)was shortened in the experiment group(3.26±2.53 v.s 1.85± 4.5)which were better than the control group,and there were significant differences(P <0.05).GOS score of patients was evaluated during 3 months of follow up.The prognosis of patients in the experiment group showed slight better,but there was no significant difference.[Conclusion]rFⅦa for the treatment of cerebral hemorrhage can improve the coagulation of patients,but its correlation with the prognosis needs further inves-tigation by large-sample clinical data.
Hemorrhagic features and the application of recombinant activated factor VII in liver transplantation

Abstract:Background: Hemorrhage, hemostasis, and blood coagulation, as well as the application of hemostasis measures, in the liver transplantation have been poorly understood. There have been no protocols regarding routine hemostasis. Objective: To investigate the hemorrhagic features in each phase and to observe the application efficacy of recombinant activated factor V II (rFV II a) during the liver transplantation. Design, Time and Setting: A retrospective case analysis, controlled observation experiment was performed at the Department of Common Surgery, Second Affiliated Hospital of Sun Yat-sen University between April 2001 and July 2006. Participants: Fifteen patients who received liver transplantation between April 2001 and March 2003 served as retrospective study subjects. An additional 28 patients who underwent liver transplantation between March 2003 and July 2006 were randomly and evenly divided into two groups: rFV II a and control. Methods: The hemorrhage rule of 15 patients who received liver transplantation in the protophase was retrospectively analyzed and the hemorrhagic feature in each time period was localized. A comparative observation was performed in the 28 patients in the anaphase. The rFV II a group received an intravenous injection of 70-80 μ g/kg rFV II a for 3-5 minutes. Simultaneously, the control group was given 50 mL physiological saline in parallel. Main Outcome Measures: Hemorrhage volume of 15 patients that received liver transplantation in each phase; prothrombin time, activated partial thromboplastin time, and total hemorrhage volume prior to and 30 minutes after rFV II a application in 14 patients who received liver transplantation. Results: Extensive errhysis was a primary cause of hemorrhage in the liver transplantation. Hemorrhage primarily occurred in the phase of diseased liver resection (i.e., pre-anhepatic phase). rFV II a could well improve various coagulation functional indices, i.e., thromboelastography indices (reaction time, coagulation time, α angle, and maximum amplitude) and routine blood indices (prothrombin time and activated partial thromboplastin time). Compared with the control group, hemorrhage volume was obviously decreased and transplantation time was significantly shorter in the rFV II a group. In addition, no thrombotic complications were found in the rFV II a group during the observation period. Conclusion: The pre-anhepatic phase is a primary hemorrhage phase during the liver transplantation. rFV II a can be successfully applied for liver transplantation.

Abstract:Abstract rFVIIa, a human recombinant activated coagulation factor VII, has been used worldwide for more than two decades for the treatment of bleeding episodes and prevention of bleeding in patients undergoing surgery/invasive procedures with congenital haemophilia A or B with inhibitors (CHwI A or B), acquired haemophilia (AH), congenital factor VII deficiency and Glanzmann thrombasthenia (GT), refractory to platelet transfusion. The approved dosage, administration and indication of rFVIIa in the US, Europe and Japan differ, depending on the needs of the patient population and regulatory practices. This review presents an overview of the current status and future prospects, including that from a Japanese perspective, of using rFVIIa in the treatment of approved indications. The efficacy and safety of rFVIIa in the approved indications has been demonstrated in several randomised and observational studies and data from registries. The overall incidence of thrombosis across all approved indications in a retrospective safety assessment of clinical trials and registries, prelicensure studies and postmarketing surveillance studies of rFVIIa use was 0.17%. Specifically, the risk of thrombotic events was 0.11% for CHwI, 1.77% for AH, 0.82% for congenital factor VII deficiency and 0.19% for GT. Emerging non-factor therapies such as emicizumab have changed the treatment landscape of haemophilia A, including preventing bleeding in patients with CHwI. However, rFVIIa will continue to play a significant role in the treatment of such patients, particularly during breakthrough bleeding or surgical procedures.
