Abstract:OBJECTIVE:To retrospectively review the safety and efficacy of ultrasound (US)-guided percutaneous microwave ablation (MWA) of bilateral renal cell carcinomas (BRCCs).PATIENTS AND METHODS:In this study, eight patients of BRCCs with 20 lesions seen from November 2012 to October 2018 were enrolled retrospectively. The patients were treated with US -guided percutaneous MWA and were followed up with contrast-enhanced US and computed tomography or magnetic resonance imaging.RESULTS:Technical success was achieved 20 tumors. The 18 lesions were performed to ablate 90%; 2 lesions were repeated because of detecting residual ablated tumor. Except six patients were noted fever, no severe complications occurred. The median follow-up time was 24 months (range 7-59 months). Among the eight patients, there was no local tumor recurrence in 6 of 8 completely ablated tumor lesions. One patient was retreated by MWA 6 months after ablation, and another was retreated after 19 months due to local tumor recurrence. No patients have obvious change of renal function and lost their life.CONCLUSION:US-guided percutaneous MWA is a beneficial treatment for BRCC.

Abstract:Objective To evaluate the safety and efficacy of ultrasound-guided percutaneous microwave ablation of renal neoplasms.Methods Twenty-two patients with pathologically proven renal neoplasms(18 renal cell carcinoma,3 renal angiomyolipoma,1 oncocytoma)with diameters of 1.0 to 3.8 cm were treated by microwave ablation.Cooled-shaft needle antenna was percutaneously inserted into the tumors under ultrasound guidance.One antenna was used for tumors less than 2 cm,two antennas were used for tumors larger than 2 cm.One thermal couple was placed adjacent to the tumor monitoring temperature in real-time during ablation.Immediate treatment efficacy was assessed by contrast-enhanced ultrasound within 3 days after ablation.Short-term efficacy was assessed by contrast-enhanced CT and/or contrast-enhanced ultrasound at 1,3,6 months and every 6 months thereafter.Results Twenty tumors were completely ablated in a single session,2 tumors were completely ablated in 2 sessions.No complications occurred.No residual tumor or recurrence was observed during follow-up.Conclusions Ultrasound-guided pereutaneous microwave ablation appears to be a safe and effective technique for the management of renal neoplasms in selected patients.
Efficacy of Contrast-Enhanced Ultrasound-Guided Percutaneous Microwave Ablation Treatment of Renal Carcinomas

Abstract:ObjectiveTo assess the safety and efficacy of contrast-enhanced ultrasound (CEUS)-guided percutaneous microwave ablation (MWA) treatment of renal carcinomas.MethodsFifteen patients with 17 renal tumors were retrospectively analyze. The patients’ ages were in the range of 46-80 years (65.2±9.4 years), the tumor diameters were in the range of 1.1-3.9 cm (2.3±0.7 cm) and biopsy pathological results were renal cell carcinoma. All of the tumors were inconspicuous on conventional ultrasound, while conspicuous on contrast enhanced-computed tomography (CT) or magnetic resonance imaging (MRI). Seventeen tumors were ablated under the guidance of CEUS.ResultsAll tumors were successfully visualized on CEUS and MWA was performed under CEUS guidance. The durations of energy application for each tumor were in the range of 5-11 min (mean± SD 7.3±2.2 min). The mean session of CEUS-guided MWA for each tumor was 1.1±0.3. All patients were followed up for 3-49 months (median, 17 months) to observe the therapeutic effects and complications. There was no local tumor progress and major complication. ConclusionCEUS-guided PMWA is a safety, efficient and feasible therapy for patients with RCC which is inconspicuous on conventional US.

Abstract:Imaging-guided percutaneous microwave ablation (MWA) with high thermal efficiency comprises rapid, successful management of small renal cell carcinomas (RCCs) in selected patients. Ultrasound Committee of Chinese Medical Association, Interventional Oncology Committee of Chinese Research Hospital Association developed evidence-based guidelines for MWA of RCCs after systematically reviewing the 1969-2019 literature. Systematic reviews, meta-analyses, randomized controlled trials, cohort, and case-control studies reporting MWA of RCCs were included and levels of evidence assessed. Altogether, 146 articles were identified, of which 35 reported percutaneous MWA for T1a RCCs and 5 articles for T1b RCCs. Guidelines were established based on indications, techniques, safety, and effectiveness of MWA for RCCs, with the goal of standardizing imaging-guided percutaneous MWA treatment of RCCs. Key points Microwave ablation is recommended for managing small renal cell carcinoma in selected patients. Imaging protocols are tailored based on the procedural plan, guidance, and evaluation. Patient's selection evaluation, updated technique information, clinical efficacy, and complications are recommended to standardize management. A joint task force (multidisciplinary team) summarized the key elements of the standardized report.

Abstract:We evaluated the feasibility, safety and efficacy of ultrasound guided percutaneous microwave ablation for small renal cell cancers.A total of 12 patients with a pathologically proven renal cell cancer 1.3 to 3.8 cm in diameter were treated with microwave ablation. A cooled shaft needle antenna was percutaneously inserted into the tumor under ultrasound guidance. One antenna was used for tumors 2 cm or smaller and antennae were used for tumors larger than 2 cm. One thermocouple was placed about 0.5 cm away from the tumor to monitor temperature in real time during ablation. Microwaves were emitted at 50 W for 500 seconds and prolonged as necessary to attain temperatures sufficient to ensure tumor kill. Immediate treatment efficacy was assessed by contrast enhanced ultrasound 1 day after ablation. Short-term efficacy was assessed by contrast enhanced computerized tomography and/or contrast enhanced ultrasound at 1, 3 and 6 months, and every 6 months thereafter.All tumors were completely ablated at a single session and no complications occurred. No residual tumor or recurrence was observed at a median followup of 11 months (range 4 to 20). The ablation zone was well defined on contrast enhanced imaging and it gradually shrank with time.Ultrasound guided percutaneous microwave ablation appears to be a safe and effective technique for small renal cell cancer in select patients.

Abstract:Objective To evaluate low-mechanical-index contrast-enhanced ultrasound (CEUS) in assessing the clinical efficacy and feasibility of percutaneous microwave ablation (MWA) for the treatment of renal cell carcinoma (RCC). Methods From March 2006 to July 2013, a total of 105 RCC patients with 109 lesions, mean size being (3.1 ± 1.3) cm, were treated with ultrasound-guided percutaneous MWA. CEUS results obtained on the third day after MWA were compared with the contrast-enhanced CT/MRI and biopsy findings in the corresponding period. Follow-up examinations with CEUS and CT/MRI were carried out 1, 3 and 6 months after the treatment and once every 6 months thereafter to evaluate the short-term and the long-term therapeutic effectiveness. Results Three days after MWA, CEUS showed that 94 of 109 lesions (86.2%) were completely ablated, and residual tumor tissue was seen in 15 of 109 lesions (13.8%). Among the 15 lesions with residual tumor tissue, 13 (86.7%) were proved by contrast-enhanced CT/MRI and/or biopsy performed in the corresponding period. The sensitivity, specificity, accuracy, positive predictive value (PPV) and negative predictive value (NPV) of CEUS for the evaluation of the short-term MWA effectiveness were 100%, 97.9%, 98.2%, 86.7% and 100%, respectively. Patients were followed up for 3 - 90 months with a median period of 24.6 months. During the follow - up period, CEUS detected recurrence in 7 patients, and 6 of them were consistent with CT/MRI results. The sensitivity, specificity, accuracy, positive and negative predictive value of CEUS for the evaluation of the long-term MWA effectiveness were 85.7%, 99.0%, 98.2%, 85.7% and 99.0%, respectively. Conclusion For the evaluation of the effectiveness of percutaneous microwave ablation in treating renal cell carcinoma, CEUS is a sensitive, reliable and convenient technique.
Long-term Effects of Ultrasound-Guided Microwave Ablation in the Treatment of Small Renal Cell Carcinoma

Abstract:Objective To evaluate the long-term efficacy of microwave ablation in the treatment of small renal cell carcinoma (RCC). Methods We retrospectively analyzed 140 cases of small cell renal carcinoma (151 lesions with a mean diameter of 2.8± 0.8 cm) treated between April, 2006 and October, 2015 with ultrasound-guided microwave ablation with cooled-shaft needle antenna. One microwave ablation antenna was used for tumors less than 2 cm in diameter and 2 antennas were used for larger tumors. The patients received enhanced ultrasound and CT/MRI examinations at 1, 3, and 6 months after the operation and every 6 months thereafter. The overall survival, disease-free survival, and local tumor progression rate of the patients were evaluated. Results The response rate of treatment (complete ablation at one month on enhanced images) was 100% in these patients. The local tumor progression rates at 1, 3, and 5 years were 0.9%, 2.0%, and 7.1%, respectively, and the 1-, 3-, and 5-year distant metastasis rates were 1.6%, 2.5%, and 7.9%, respectively. The overall survival rates of the patients at 1, 3, and 5 years were 98.4%, 94.8%, 89.5%, respectively, with disease-free survival rates of 98.4%, 93.0%, and 83.1%, respectively. No major complications occurred in these cases, and multivariate analysis showed that the tumor number (P=0.015) and tumor growth patterns (P=0.049) were independent risk factors that adversely affected the long-term outcome after surgery. Conclusion Our data show that microwave ablation is a safe and effective modality for treatment of renal cell carcinoma.
Percutaneous Microwave Ablation of Renal Cell Carcinoma is Safe in Patients with a Solitary Kidney

Abstract:Objectives: To present the clinical effect of microwave ablation (MWA) on renal cell carcinoma (RCC) of the patients with renal dysfunction, mainly focussing on the extent of renal tumour control and damage to the residual kidney function.Methods and materials: From 2006 to 2014, 19 tumours of 18 patients with renal dysfunction underwent percutaneous ultrasound-guided MWA in our institution. The tumour diameters range from 1.9 to 5.0cm. The serum creatinine and urea levels of each patient pre-MWA, oneday after MWA and the most recent occasion on record at our institution were collected. After MWA all the patients were followed up using contrast enhanced ultrasound (CEUS) and computed tomography (CT) or magnetic resonance imaging (MRI) at the first 1, 3 and 6 months and every six months thereafter. Patients were available for clinical and laboratory evaluations at a median follow-up time of 24.9 months (range from 3.5 to 85.9 months). The technical success, survival rates and complications were accessed.Results: Complete ablation was achieved in 19/19 (100%) lesions after 1 or 2 MWA sessions; 2/18(11.1%) patients died of other diseases. No severe complications occurred during MWA. After MWA no significant elevation of renal function was observed either in patients of CKD stage 1-3 or in patients of CKD stage 4-5.Conclusions: MWA is an effective and relatively safe treatment option for patients with renal tumour who also suffered from renal dysfunction. The complication rate is low, and excellent tumour control can be achieved with acceptable loss of residual renal function.
Microwave Treatment of Renal Cell Carcinoma Adjacent to Renal Sinus

Abstract:Purpose: To evaluate the efficacy and safety of ultrasound (US)-guided percutaneous microwave ablation (MWA) for renal cell carcinoma (RCC) adjacent to renal sinus.Materials and methods: This retrospective study included 41 patients who underwent US-guided percutaneous MWA of 41 RCCs adjacent to the renal sinus from April 2006 to December 2015. Contrast-enhanced images of US and computed tomography (CT) or magnetic resonance (MR) imaging were performed at pre-ablation and 1 day, 1 month, 3 months, and every 6 months after ablation. Initial ablation success (IAS), disease-free survival (DFS), RCC-related survival (RRS), and overall survival (OS) were recorded at the follow-up visits.Results: IAS was achieved in 92.7% (38/41) of the study subjects. The IAS significantly differed between patients with RCCs <4 cm (100%, 29/29) and RCCs >4 cm (75%, 9/12, p = 0.021). During the median follow-up of 37.6 (range, 3.0-97.3) months, the estimated 1-, 3-, and 5-year DFS of patients with an initial tumor of <= 4 cm were 100%, 89.7%, and 81.5%, respectively. The 1-, 3-, and 5-year RRS were 100%, 93.3%, and 93.3%, respectively. The 1-, 3-, and 5-year OS were 97.1%, 87.8%, and 83.6%, respectively. The multivariate analysis using the Cox proportional hazard model revealed no independent predictor of recurrence among all the variables. There were no MWA-related deaths among the study subjects. One patient developed a retroperitoneal abscess after ablation.Conclusion: US-guided percutaneous MWA appears to be a promising method for RCCs adjacent to renal sinus, especially for tumors <= 4 cm. Abstract:PURPOSE:To retrospectively review intermediate-term (median, 20.1 months) clinical outcomes after microwave ablation (MWA) of renal cell carcinoma (RCC).MATERIALS AND METHODS:This retrospective study was approved by the institutional review board. The results from 46 patients with 49 RCC nodules (diameter, 0.6-7.7 cm; mean, 3.0 cm ± 1.5 [standard deviation]) treated with ultrasonography (US)-guided percutaneous MWA with cooled-shaft needle antenna from April 2006 to December 2010 were reviewed. One antenna was used for tumors smaller than 2 cm; two, for tumors 2 cm or larger. The patients were followed up with contrast material-enhanced US and computed tomography or magnetic resonance imaging at 1, 3, and 6 months and every 6 months thereafter. The effect of changes in key parameters (including overall survival, disease-free survival, and local tumor progression rate) was statistically analyzed by using the log-rank test.RESULTS:Technical effectiveness (complete ablation at follow-up enhanced imaging 1 month after MWA) was achieved in 48 of 49 (98.0%) tumors, and the metastasis-free rate was 100% (46 of 46). The 1-, 2-, and 3-year local tumor progression rates were 4.6%, 7.7%, and 7.7%, respectively. The cancer-specific survival rate was 100% (46 of 46), and 1-, 2-, and 3-year overall survival rates were 100%, 100%, and 97.8%, respectively. The 1-, 2-, and 3-year disease-free survival rates were 95.4%, 92.3%, and 92.3%, respectively. No major complications occurred. Multivariate analysis showed that tumor number (P = .046), tumor growth patterns (P = .003), and ablation time (P = .04) were independent unfavorable prognostic factors.CONCLUSION:In the intermediate term, US-guided percutaneous MWA appears to be a safe and effective technique for the management of RCC, especially small RCC, in selected patients.
Role of Contrast-Enhanced Ultrasound in Evaluating the Efficiency of Ultrasound Guided Percutaneous Microwave Ablation in Patients with Renal Cell Carcinoma

Abstract:Background. The aim of the study was to evaluate the efficiency and feasibility of contrast-enhanced ultrasound (CEUS) with Sonovue in assessing of renal cell carcinomas (RCCs) following ultrasound (US)-guided percutaneous microwave ablation (MWA).Patinets and methods. Seventy-nine patients (60 males and 19 females) with 83 lesions (mean size 3.2 +/- 1.6 cm) were treated by US-guided percutaneous MWA. The CEUS results of the third day after the ablation were compared with the synchronous contrast-enhanced computed tomography (CT)/magnetic resonance imaging (MRI) results and biopsy pathological results. The follow-up was performed by CEUS and CT/MRI after 1, 3, 6 months and every 6 months subsequently. The combination of clinical follow-up results and CT/MRI imaging findings was the reference standard of CEUS results for evaluating the therapeutic effect. The identification of residual or recurrence tumour was assessed by two blinded radiologists.Results. On the third day after MWA, CEUS showed 68 of 83 lesions (68/83, 81.9%) successfully ablated and 15 of 83 (18.1%) with residual tumours. Among residual tumours, 13 (86.7%) were confirmed by contrast-enhanced CT/MRI findings and biopsy results. The sensitivity, specificity, accuracy, positive and negative predictive value of CEUS evaluating the short-term MWA effectiveness were 100%, 97.1%, 97.6%, 86.7% and 100%, respectively. During the six years follow-up (median 26 months), the CEUS showed recurrence in 7 patients, and six of them achieved consistent results on CEUS and CT/MRI imaging. The sensitivity, specificity, accuracy, positive and negative predictive value for CEUS evaluating long-term MWA effectiveness were 85.7%, 98.7%, 97.6%, 85.7% and 98.7%, respectively.Conclusions. The post-procedural CEUS demonstrated as an effective and feasible method in evaluating a therapeutic effect of RCCs following MWA.
Contrast-Enhanced Ultrasound-Guided Microwave Ablation for Hepatocellular Carcinoma Inconspicuous on Conventional Ultrasound

Abstract:Purpose: This study was designed to determine the safety, effectiveness and feasibility of contrast enhanced ultrasound (CEUS)-guided percutaneous microwave ablation (MWA) of renal cell carcinoma (RCC) that is inconspicuous on conventional ultrasound (US).Materials and methods: A total of 32 RCC nodules in 29 patients (23 men and 6 women) were treated with CEUS-guided percutaneous MWA between January 2010 and September 2014. The median maximum diameter of the nodules was 2.4 cm (interquartile range: 1.8-2.9 cm). The US contrast agent was SonoVue, a second-generation contrast agent. CEUS was applied before the needle was inserted into the tumour, and percutaneous MWA was performed under CEUS-guidance.Results: In total 31 tumours were successfully visualised via CEUS using 1-2 (1.0-2.0 mL) contrast agent injections, and percutaneous MWA was performed under CEUS-guidance. The technical success rate of CEUS-guided percutaneous MWA of RCC was 96.9% (31/32). The mean number of sessions of CEUS-guided percutaneous MWA for each tumour was 1.2 +/- 0.4. The mean duration of energy application for each tumour was 7.3 +/- 2.7 min. All patients were followed up for 3-71 months (median 17 months) to observe the therapeutic effects and complications. The therapeutic effects were assessed at follow-up with computed tomography (CT) or magnetic resonance imaging (MRI) and CEUS. There was no local tumour progression and the technique effectiveness rate was 100% (31/31). The complications rate was 6.5% and the major complications rate was 3.2%. We observed one case of pleural effusion and one case of renal subcapsular haemorrhage after the percutaneous MWA procedures.Conclusion: CEUS-guided percutaneous MWA is a safe, efficient and feasible therapy for patients with RCCs inconspicuous on conventional US.

Abstract:To retrospectively review intermediate-term clinical outcomes of microwave ablation (MWA) compared to those after open radial nephrectomy (ORN) in T1a renal cell carcinoma (RCC) patients. A total of 163 patients with T1a RCC measuring ≤4 cm were included from April 2006 to March 2012. 65 patients underwent MWA and 98 patients ORN. The RCC-related survival (97.1% at 5 years) after MWA was comparable to those following ORN (97.8% at 5 years, p = 0.78). There was one local tumor progress at 32 months after MWA and none occurred after ORN. The major complication rates was comparable (p = 0.81) between two techniques (2.5% in MWA vs 3.1% in ORN). MWA group needed less operative time (p < 0.0001), estimated blood loss (p = 0.000), postoperative hospitalization (p < 0.0001). The multivariate analysis showed age (p = 0.014), tumor type (p = 0.003), postoperative urea nitrogen (P=0.042), co-morbid disease (p = 0.005) and treatment modality (p = 0.000) may become predictors related to the survival rate. In the intermediate term, ultrasound-guided percutaneous MWA and ORN provide comparable results in oncologic outcomes. MWA appears to be a safe and effective technique for the management of T1a RCC patients with less invasion.

Abstract:Purpose: To retrospectively review the complications of ultrasound (US)-guided percutaneous microwave ablation (MWA) of renal cell carcinoma.Patients and methods: In this study, 101 patients with 105 tumors seen from April 2006 to Feb 2014 were enrolled retrospectively. The patients were treated with US-guided percutaneous MWA and were followed up with contrast-enhanced US and computed tomography or magnetic resonance imaging at 1, 3, and 6 months and every 6 months thereafter.Results: Technical success was achieved in 99 of 105 tumors (94.3%). The median follow-up time was 25 (range 1.13-93.23) months. Among the 105 tumors, 26 complications in 24.8% of patients and 23 minor complications (Clavien-Dindo Grades I and II) in 21.9% of patients were noted, accounting for 88.5% of all complications. All the minor complications were cured. Three major complications (Clavien-Dindo Grade $ III) occurred in 2.9% of the patients, accounting for 11.5% of all complications: hydrothorax in two patients and bowel injury in one. The two patients who had hydrothorax post-MWA had a history of cirrhosis and were treated with catheter drainage. The bowel injury was treated surgically. In all patients, the changes in serum creatinine and urea nitrogen levels from before to after the procedure were small.Conclusion: US-guided percutaneous MWA is a beneficial treatment for renal cell carcinoma in selected patients; however, if the renal tumor is close to the bowel, or the patient has serious comorbidities or has undergone abdominal surgery, the procedure must be performed more carefully.
US-guided Percutaneous Microwave Ablation Versus Open Radical Nephrectomy for Small Renal Cell Carcinoma: Intermediate-Term Results.

Abstract:PURPOSE:To review intermediate-term clinical outcomes of microwave ablation (MWA) compared with open radial nephrectomy (ORN) in small renal cell carcinoma (RCC) patients and to identify prognostic factors associated with two techniques.MATERIALS AND METHODS:This retrospective study was institutional review board-approved. A total of 163 patients (127 men and 36 women) with small RCC (≤4 cm) were included from April 2006 to March 2012. Sixty-five patients underwent MWA and 98 patients underwent ORN. Survival, recurrence, and renal function changes were compared between the two groups. Effect of changes in key parameters (ie, overall survival, RCC-related survival, and metastasis-free survival) was statistically analyzed with the log-rank test.RESULTS:Although overall survival after MWA was lower than that after ORN (P = .002), RCC-related survival was comparable to ORN (P = .78). Estimated 5-year overall survival rates were 67.3% after MWA and 97.8% after ORN; for RCC-related survival, estimated 5-year rates were 97.1% after MWA and 97.8% after ORN. There was one local tumor recurrence 32 months after MWA and none after ORN. Major complication rates were comparable (P = .81) between the two techniques (MWA, 2.5% vs ORN, 3.1%). The MWA group had less surgical time (P < .001), estimated blood loss (P < .001), and postoperative hospitalization (P < .001). Multivariate analysis showed age (P = .014), tumor type (P = .003), postoperative urea nitrogen (P = .042), comorbid disease (P = .005), and treatment modality (P < .001) may become survival rate predictors.CONCLUSION:In intermediate term, ultrasonographically guided percutaneous MWA and ORN provide comparable results in oncologic outcomes. MWA appears to be a safe and effective technique for management of small RCC in patients with little loss of renal function.
Percutaneous Microwave Ablation Versus Laparoscopic Partial Nephrectomy for Ct1a Renal Cell Carcinoma: A Propensity-matched Cohort Study of 1955 Patients.

Abstract:Background Percutaneous microwave ablation (MWA) and laparoscopic partial nephrectomy (LPN) are two modalities indicated for early-stage renal cell carcinoma (RCC) with low extent of invasion. Purpose To compare the long-term results of percutaneous MWA and LPN in the treatment of cT1a RCC. Materials and Methods This retrospective study included 1955 patients with cT1a RCC treated with percutaneous MWA or LPN between April 2006 and November 2017. Propensity score matching was used. Oncologic outcomes were analyzed by using the Fine-and-Gray competing risk models. Results A total of 185 patients underwent percutaneous MWA (mean age, 63.2 years ± 15.2 [standard deviation]) and 1770 underwent LPN (mean age, 50.9 years ± 13.2). During the follow-up (median, 40.6 months), after propensity score matching, no difference was observed between local tumor progression (3.2% vs 0.5%, P = .10), cancer-specific survival (2.2% vs 3.8%, P = .24), and distant metastases (4.3% vs 4.3%, P = .76). Patients who underwent percutaneous MWA had worse overall survival (hazard ratio, 2.4; 95% confidence interval: 1.0, 5.7; P = .049 vs LPN) and disease-free survival (82.9% vs 91.4%, P = .003). Percutaneous MWA led to smaller drop in estimated glomerular filtration rate at discharge (6.2% vs 16.4%, P < .001), smaller estimated blood loss (4.5 mL ± 1.3 vs 54.2 mL ± 69.2), lower cost ($3150 ± 2970 vs $6045 ± 1860 U.S. dollars), shorter operative time (0.5 minute ± 0.1 vs 1.8 minutes ± 0.6), and shorter postoperative hospitalization time (5.1 days ± 2.6 vs 6.9 days ± 2.8) (all P < .001 vs LPN). There were fewer cases of fever in the percutaneous MWA group (16.2% vs 73.0%, P < .001). Conclusion There were no significant differences regarding oncologic outcomes and complications between percutaneous microwave ablation and laparoscopic partial nephrectomy for patients with cT1a renal cell carcinoma. Percutaneous microwave ablation led to smaller renal function change and lower blood loss. For patients who cannot be subjected to the risks of more invasive laparoscopic partial nephrectomy, percutaneous microwave ablation could be an alternative less invasive treatment option. © RSNA, 2020 Online supplemental material is available for this article.

Abstract:Objectives: The treatment of synchronous bilateral renal cell carcinoma is challenging. Radiofrequency ablation has been increasingly applied in the management of renal tumors. Herein, we report our experience of radiofrequency ablation on 12 patients with synchronous bilateral renal cell carcinoma.Methods: From March 2006 to September 2010, 12 patients with bilateral synchronous sporadic renal cell carcinoma (29 lesions overall) were identified from our kidney database. The mean age was 62.3 years (range 35-81). The mean tumor diameter was 4.5 cm (range 0.9-9.0). Three patients received unilateral radiofrequency ablation and contralateral radical nephrectomy, whereas nine patients received bilateral radiofrequency ablation. The oncological and functional outcomes were analyzed. Contrast-enhanced computed tomography examinations were carried out at day 7, and at 3 and 6 months after the procedure, and every 6 months thereafter.Results: The mean follow-up period was 33 months (range 10-64). The local tumor control rate was 93.1% (27/29). Cancer-specific survival and the overall survival rates were 100%. No death or renal failure after the procedure was found. In patients who underwent bilateral radiofrequency ablation, the latest mean glomerular filtration rate had not significantly declined compared with preoperative levels (93.7 +/- 13.0 mL/min/1.73 m(2) vs 96.9 +/- 13.3 mL/min/1.73 m(2), respectively; P > 0.05).Conclusion: Radiofrequency ablation shows encouraging outcomes in the treatment of bilateral renal cell carcinoma. It can provide adequate local tumor control and cancer-specific survival compared with nephron-sparing surgery while not affecting the renal function.

Abstract:Objective:To assess the technical success,and effectiveness of ultrasound- guided in percutaneous radiofrequency ablation(RFA) for renal tumors.Methods:From March 2008 to June 2010,25 consecutive patients with renal tumors located in the dorsolateral zone of the kidney or middle and low pole of kidney underwent ultrasound -guided percutaneous RFA.Results:Therapeutic success was achieved in all patients.The mean time of the operation was 37 minutes(range,30-50 minutes).No serious complications(perirenal hemorrhage,perirenal effusion and adjacent organ injury) occurred in the all patients.Biopsy results revealed renal cell carcinoma(RCC) in 18 cases and renal hamartoma in 5 cases,2 case were nondiagnostic.1 patient confirmed residual tumor by review, once again percutaneous RFA.Conclusions:The technology of ultrasound-guided is the key for percutaneous RFA of renal tumors.The expert operation can significant improve the achievement of a complete ablation.

Abstract:Purpose: To evaluate the safety and efficacy of the clinical application of hydrodissection under ultrasound (US) guidance for assisting percutaneous microwave ablation (MWA) for the treatment of renal cell carcinomas (RCCs) adjacent to the intestinal tract.Materials and methods: From April 2014 to December 2016, clinical data from 24 patients with 25 RCCs were retrospectively analysed. Percutaneous MWA under the assistance of US-guided hydrodissection were performed to treat RCCs with a mean maximal diameter of 3.801.60cm because the distance between the index tumour and the adjacent intestinal tracts were less than 0.5cm on imaging. The separation success rate of the hydrodissection, technique efficacy rate of the MWA, local tumour progression (LTP), complications, and renal function including serum creatinine (Cr) and blood urea nitrogen (BUN) were assessed.Results: In total, 28 sessions of hydrodissection and MWA procedures were performed (one procedure in 22 patients and two procedures in 3 patients), and the separation success rate was 100% (28/28). The technique efficacy rate was 100% (25/25), and no LTP occurred. One patient exhibited a major complication (4.2%). Minor complications in 5 patients (20.8%) and side effects in 12 patients (50.0%) occurred. Compared with the pre-MWA levels, there were no significant differences in serum Cr and BUN 1-day post-MWA and at the last follow-up.Conclusions: US-guided hydrodissection assistance for percutaneous MWA could be a safe and effective alternative for selected patients with RCCs adjacent to the intestinal tracts and can achieve good local tumour control and renal function preservation.

Abstract:Objective To evaluate the clinical safety of percutaneous microwave ablation (MWA) treatment for renal carcinomas at T1a stage under ultrasound (US) guidance.Methods From September 2009 to December 2015,the clinical data of 112 patients at T1a stage with 119 renal carcinomas proven histopathologically were retrospectively analyzed.All the tumors were performed the procedure of US-guided percutaneous MWA at Chinese PLA General Hospital.The mean maximal diameter of the tumors was (2.82±0.79) cm (range from 0.9 to 4.0 cm) and the mean follow-up period was (25.3±17.2) months (range from 2 to 68 months).The clinical safeties were assessed by thepresentations of the complications and side effects following the procedure and patients' renal function before and after MWA.Results The major complications occurred in three patients (2.7％) after MWA,including colonic perforation,urinary fistula and pseudoaneurysm formation adjacent to the ablated zone.20 cases (17.9％) with minor complications and 18 ones (16.1％) with side effects were presented after procedures.MWA was safely and effectively completed for 6 patients on dialysis because of renal function failure before ablation.For the other 106 ones,the indexes of renal function such as serum creatinine and blood urea nitrogen at last follow-up were shown no statistical difference to the ones of pre-ablation (P ＞ 0.05).Conclusion US-guided percutaneous MWA would be a safe alternative for the treatment of patients with renal carcinomas at T1a stage with no significant influence to renal function after ablation.
