Abstract:Popliteal artery entrapment syndrome (PAES) is a rare cause of lower extremity arterial ischemia, especially in elder patients. At present, the role of the endovascular technique for PAES is still mainly auxiliary, there is no successful case treated successfully by endovascular treatment without open surgery, and also the long-term follow-up after endovascular treatment are lacking. We successfully treated a 51-year-old man with pale, cold right foot by endovascular therapy only, including percutaneous transluminal angioplasty and catheter directed thrombolysis. After endovascular treatment, the ABI of the right leg increased from 0.41 to 0.85, and the patient return to his life without claudication. Computed tomography angiography demonstrated the diagnosis of PAES. The 3-year follow-up showed a good clinical outcome, suggesting that for certain type of PAES, endovascular treatment alone may achieve good results. However, this result still requires confirmation in a large-sample study.

Abstract:Infrapopliteal pseudoaneurysms can occur after trauma. In recent years, endovascular repair has been increasingly used to treat infrapopliteal pseudoaneurysms. Nevertheless, there has been no covered stent specifically for small peripheral vessels. Here, we report a case of a 65-year-old male with a pseudoaneurysm of the anterior tibial artery which was managed with a physician-modified covered stent. 6 months after the procedure, the mass had completely disappeared and the vascular anatomy of the calf is well preserved.

Abstract:Popliteal artery injury is common in the lower extremity trauma. For the features of sudden and occultness, the diagnosis and treatment of popliteal artery injury are sometimes difficult. A disaster would befall as an unreasonable management was decided. The surgeon should rebuild limb blood supply on the premise of maintaining stable vital signs. Six hours after trauma are the prime time to rebuild the blood supply. Surgeon should pay attention to the prevention and treatment of systemic and local complications after revascularization.

Abstract:Objective To investigate the surgical treatment for patients with popliteal artery aneurysm(PAA).Methods The clinical data of 9 patients with popliteal artery aneurysm admitted to our department from 2004 to 2010 were retrospectively analyzed.Results Nine patients(6 males and 3 females) with average age was 53.8 years.Of the patients,2 cases had bilateral PAAs(one of which coexisted with bilateral iliac artery aneurysms),4 cases had left and 3 cases had right PAAs.One patient had asymptomatic PAA and 8 cases had symptomatic PAAs that presented with pulsating mass in 2 cases,a mass with local pain in 3 cases,and chronic and acute lower extremity ischemia in one and two cases,respectively.Seven patients underwent surgical repair and the other 2 patients received non-surgical treatment(one of which underwent surgery of bilateral iliac artery aneurysms initially).Aneurysm resection and eFTFE graft or autologous saphenous vein interposition was performed in the operated patients.Follow-up of 0.5 month to 6 years was available in 8 patients.Symptoms of the affected limbs were obviously ameliorated,and pulsating masses and limb pain were disappearred after operation.The dorsalis pedis artery could be palpated in all cases.Conclusions Atherosclerosis is the main cause of PAA.Early elective surgical repair is suggested for symptomatic PAA or asymptomatic PAA with diameter larger than 2 cm,and is especially indicated for thrombosed PAAs.There is no significant difference between eFTFE and autologous saphenous vein graft in short-term and long-term patency.
[Endovascular Treatment of Popliteal Artery Aneurysm in a Clinical Analysis of 9 Cases].

Abstract:OBJECTIVE:To evaluate the efficacy of endovascular repair of popliteal artery aneurysm (PAA) and to summarize the clinical experience and lessons.METHODS:A clinical data of 9 PAA cases (males 8, females 1, age range 55-84) undergoing endovascular repair from October 2006 to December 2014 at Zhongshan Hospital Fudan University were retrospectively analyzed. CT angiography was made in all of the 9 cases preoperatively, and 10 PAA were included in the analysis. Of the 9 cases, 6 cases were symptomatic. The mean diameter of PAA was 3.9 cm (range, 2.1-8.2).RESULTS:The technique successful rate of endovascular repair was 10/10. The preoperative complications were 2/10, including one case suffered hematoma in the arterial puncture site and 1 case with distal artery embolization. The mean follow-up was 33 months (range, 3-87). During the follow-up, 2 cases received the secondary intervention and drug thrombolysis because of the stent distal thrombosis. The stent-graft primary patency at 1 year was 78.8% (95% CI: 48%-91%). One case suffered stent fracture, and one case died of brainstem hemorrhage 15 months post endovascular repair of PAA.CONCLUSIONS:Endovascular treatment of PAA is safe and effective in perioperative and midterm follow-up. However, individual endovascular strategy and postoperative complications in the long-term follow-up should be payed more attention.
Diagnosis and Surgical Management of Popliteal Venous Aneurysms: Report of Two Cases

Abstract:Objective: The objective of this study was to explore methods for the diagnosis and treatment of popliteal venous aneurysms. Methods: We retrospectively analyzed the diagnostic and treatment processes used for 2 patients with popliteal venous aneurysms. The main symptoms in these 2 patients were pain and local swelling; pulmonary embolism (PE) was not found in these patients. Both the patients were treated using tangential aneurysmectomy with lateral venorrhaphy, followed by warfarin anticoagulation therapy for at least 1 year. Results: Both the patients recovered well with good popliteal vein shapes after surgery, as shown by angiography. No thrombosis-related complications were found until 1 year and 3 months after surgical intervention. Conclusion: Surgical resection should be performed as soon as possible after the diagnosis of popliteal venous aneurysm. Patients should be administered postoperative anticoagulation therapy to prevent thrombosis that may result from the potential risk of PE. Abstract:Journal of Endovascular Therapy, Ahead of Print. Purpose:Traumatic popliteal pseudoaneurysms may occur in accidents or as an iatrogenic complication of a total knee arthroplasty. Complications often arise in open repair because of distorted anatomy. Up to 22% of the patients may suffer above-knee amputation. Endovascular treatment has proven to be an effective solution. However, the long-term performance of stents at the hinge point of the popliteal artery is questionable. We present a hybrid technique that was used successfully in 2 cases.Technique:Our approach takes advantage of both open and endovascular techniques. At first, we apply a stent graft at the side of the injury to cover the arterial trauma and stop blood leakage to the aneurysm sac. This allows for a safer dissection and open repair. We clamp the artery proximally and distally, open the sac, extract the stent graft, and extend to a longitudinal arteriotomy. We then reconstruct the entire area with a standard in lay end-to-end anastomosis using a vein graft. This hybrid technique may reduce the risk of uncontrollable bleeding and allow for a safer nerve decompression.Conclusions:Repair of such injuries is technically demanding. A hybrid approach may reduce the risk of complications and offer excellent long-term outcomes.Clinical ImpactThe hybrid approach to the treatment of traumatic popliteal pseudoaneurysms combines the advantages of both open and endovascular approaches. It may be possible to make an acute operation just as safe as an elective operation by implementing the proposed strategy. The procedure can be performed by surgeons of all levels, and patients may benefit from a safer surgical dissection with fewer complications and blood loss. This smart combination of standard techniques may prove invaluable in a hostile surgical environment where limb loss is likely.

Surgical Treatment of Post-Traumatic Pseudoaneurysms and Arteriovenous Fistulas

Abstract:OBJECTIVE: To report the experience in the diagn osis and treatment of post-traumatic pseudoaneurysms and arteriovenous fistulas. METHODS: A series of 30 patients (11 women and 19 men) with pos ttraumatic pseudoaneurysms were reviewed retrospectively. Among them 7 patients (5 women and 2 men) were associated with arteriovenous fistula. RESULTS: The causes included sharp penetrating trauma (18 cases ), blunt trauma (6 cases) and iatrogenic arterial injury (6 cases). The main cli nical manifestations consisted of local pulsatile mass (26 cases), vascular brui ts (19 cases), thrill (13 cases), ischemia of distal limb (9 cases), neuropathy (5 cases) and pseudoaneurysm rupture (2 cases). All patients underwent surgery. The operations included: ligation of the vessels (12 cases), surgical resection and primary suture repair of the vascular defect or anastomosis (11 cases), vasc ular reconstruction with autogenous saphenous vein (3 cases) and synthetic vascu lar graft (4 cases). CONCLUSIONS: Because of the imminent clinical course, early op eration is usually indicated. The operative treatment is effective and safe for most of the patients with post-traumatic pseudoaneurysms and arteriovenous fist ulas.
Incidence, risk factors, treatment and prognosis of popliteal artery embolization in the superficial femoral artery interventions.

Abstract:OBJECTIVE:Percutaneous transluminal angioplasty and stenting (PTA + stent) has gained acceptance as a primary treatment modality for the superficial femoral artery (SFA) diseases. Popliteal artery embolization (PAE) is a severe complication in SFA interventions. The purpose of this study was to evaluate the incidence, risk factors, treatment and prognosis of PAE in primary SFA PTA + stent. METHODS:Chronic SFA arteriosclerosis cases that underwent primary PTA + stent were reviewed from a retrospectively maintained database. Runoff vessels were evaluated in all cases before and after the interventions for PAE detection. The primary patency, secondary patency and limb salvage rates were calculated using Kaplan-Meier analysis and compared using log-rank analysis. Cox multivariate regression was performed to evaluate predictors of patency and limb salvage rates. RESULTS:There were 436 lesions treated in 388 patients with 10 PAE events (2.3%) in total. PAE rate was significantly higher in Transatlantic Inter-Society Consensus (TASC) C/D group compared with TASC A/B group (OR = 8.91, P = .002), in chronic total occlusion (CTO) lesions compared with stenotic lesions (P<.0001), and in group with history of cerebral ischemic stroke (OR = 6.11, P = .007). PAE rates were not significantly affected by age, sex, smoking, hypertension, diabetes, hyperlipidemia and runoff status. The binary logistic regression showed that only the TASC C/D was an independent predictor of PAE (P = .031). The 12-month and 24-month primary patency, secondary patency and limb salvage rates in PAE group showed no significant differences comparing with non-PAE group. CONCLUSIONS:PAE is a rare event in primary SFA PTA + stent. TASC C/D lesion, CTO and cerebral ischemic stroke history are risk factors for PAE. PAE is typically reversible by comprehensive techniques. If the popliteal flow is restored in time, PAE has no significant effect on long-term patency and limb salvage rates.

Abstract:Aim To evaluate the clinical value of dual source computed tomography (DSCT) angiography in the diagnosis and treatment for popliteal artery entrapment syndrome (PAES). Materials and Methods 8 patients with PAES were retrospectively reviewed. 64-slice dual source CT angiography was performed based on the following protocol: 100mL of Iopamidol (370mgI/mL) was injected at a rate of 3.5mL/s and arterial phase images were obtained by using bolus tracking. Axial DSCT images and reconstructed images including multi-planar reconstruction (MPR), maximum intensity projection (MIP), volume rendering (VR) were collected and analysed. All patients underwent Doppler colour ultrasound examinations and surgeries. Results The popliteal artery and the neighbouring muscular structures were clearly shown on the axial images revealing the cause of the arterial entrapment. Furthermore, the site and length of the segmental occlusion and collateral developments were well demonstrated on reconstructed images. Characterisation and classification based on DSCT angiography were confirmed by surgeries. PAES was accurately diagnosed by DSCT angiography in all enrolled patients. In contrary, only 5 PAES cases were accurately diagnosed by ultrasound examination. Conclusion DSCT angiography is a noninvasive and valuable tool in the diagnosis of PAES and plays an important role in the determination of treatment plans.
Endovascular treatment of arterial injuries：a clinical analysis of 8 cases

Abstract:Objective: To explore endovascular treatment of arterial injuries.Methods: Clinical records of 8 patients with arterial injuries who underwent endovascular treatment between September 2012 and December 2015 at the First Affiliated Hospital of Wenzhou Medical University were retrospectively analyzed.Results:Eight patients presented with 8 arterial injuries. The causes included impact injuries (n=4), drug injection (n=1), iatrogenic injuries (n=3). Among them, 2 cases of aortic dissection, 2 cases of carotid arterial injury, 1 case of subclavian arterial injury and 1 case of femoral profound artery pseudoaneurysm were treated with stent grafting. During follow up ranged from 4 to 36 months, all stent grafts were patent without any migration or endoleak. One case of superior gluteal artery pseudoaneurysm and 1 case of posterior tibial artery pseudoaneurysm were treated with arterial embolization. During follow up from 3 to 15 months, false aneurysm sac shrank without visible blood lfow.Conclusion: Endovascular treatment plays an irreplaceable role in the treatment of arterial injuries.

Abstract:Isolated popliteal artery occlusions are rare compared with femoropopliteal occlusive diseases. Although endovascular procedures have gained importance in treatment, conventional surgery remains the gold standard. In this study, we reviewed popliteal endarterectomy and patch plasty using a posterior approach. Fourteen patients who underwent surgery for isolated popliteal artery occlusions were retrospectively examined. Patients were assessed in terms of age, sex, and risk factors, such as accompanying diseases and smoking, surgical method and anesthesia, incision type, preoperative and postoperative pulse examination, ankle-brachial indices, patency, wound infection, postoperative complications, and the treatment applied. Twelve (85.7%) patients were male, and 2 (14.3%) were female. Limb ischemia was critical (ABI < 0.7) in 11 (78.5%) patients. The average duration of postoperative hospitalization was 8 ± 3.7 days on average, and the average length of follow-up was 17 ± 3.4 months. Thrombosis and complications requiring secondary intervention did not develop during the early postoperative period. While the patency rate in the first 6 months of follow-up was 100%, it was 92.8% in the 1st year and 85.7% in the 2nd year. Surgical treatment with the posterior approach in isolated popliteal artery lesions is preferred by vascular surgeons as a prioritized treatment method, with a sufficient recanalization rate and low perioperative morbidity and mortality rates. Furthermore, it is promising because it does not prevent below-knee femoropopliteal bypass, which is the subsequent stage of treatment. Moreover, the great saphenous vein was protected, and the acceptable early- and mid-term results were encouraging.

A Retrospective Comparative Study of Twelve-Month Clinical Outcomes for Drug-Coating Balloon Angioplasty and Stent Implantation in Treating Patients with Popliteal Obstructive Lesions

Abstract:PurposeDespite increased interest in treating superficial femoral and popliteal arteries with endovascular technology, there is little data regarding comparative results of different treatment modalities in the popliteal artery. It was hypothesized that to improve clinical outcomes in this segment, drug-coating balloon (DCB) angioplasty would not be inferior to stent implantation.MethodsThe records of consecutive Chinese patients with popliteal lesions who were treated with DCB or stenting between June 2016 and February 2017 were retrospectively reviewed. The preoperative demographic, lesion characteristics and postoperative outcomes were compared and statistically analyzed. All patients were divided into different subgroups according to the degree of calcium or degree of ischemia.ResultsOne hundred two consecutive patients with popliteal lesions were treated with DCB or stenting and 95 (93.1%) completed a follow-up. Critical limb ischemia was present in 70.5% and occlusions were present in 61.1% of patients. DCB angioplasty was performed on 43 patients and angioplasty with primary stenting was performed on 52 patients. There was no difference in twelve-month primary patency (79.1% vs 82.7%; P = 0.687), secondary patency (95.3% vs 94.2%; P = 0.808) and amputation-free survival (93.0% vs 94.2%; P = 0.825) between the DCB group and stent groups. In patients with severe calcium, the twelve-month primary patency (61.1% vs. 79.2%; P = 0.239) and amputation-free survival (83.3% vs 87.5%; P = 0.739) did not differ between the DCB and stent groups.ConclusionsThis study demonstrates that DCB treatment in Chinese patients with popliteal atherosclerotic occlusive lesions can be associated with similar twelve-month patency and amputation-free survival compared to stenting, even in patients with severe calcification.Level of EvidenceLevel 3a, Non-randomized follow-up study.

Abstract:Objective To summarize our experience on the diagnosis and management of 8 patients with popliteal artery entrapment syndrome (PAES). Methods Clinical data of 8 PAES cases admitted from Jul 2002 to Jul 2010 were retrospectively analyzed. There were 7 males and 1 female with the mean age of (29 ± 14)years (ranging 16 -56 years). In 6 cases posterior "S" shaped incisions in the popliteal fossa were applied and anomalous anatomic structures were verified. Segmental stenosis and post-stenotic popliteal arterial aneurysm was identified in 2 cases, and partial resection of the aneurismal wall and arterioplasty including one with saphenous vein patch were applied; For 4 cases with short segmental occlusion of the popliteal artery, surgical treatment included thromboendarterectomy in 2 cases (with saphenous vein patch plasty in one case), saphenous vein interposition in 1 case, and saphenous vein bypass grafting in 1 case.Medial longitudinal incisions and saphenous vein bypass grafting were applied in 2 cases with long segmental occlusion in popliteal artery without exploration for anatomic anomalies. Results All patients recovered uneventfully without any notable complication. During the follow-up period ranging from 4 to 99 months [average (50± 37) months], no ischemic symptom reoccurred in 7 cases with patent arteries or grafts, and recurrent claudication occurred in 1 case with distal anastomostic stenosis. The stenosis was subsequently treated with balloon angioplasty and vein graft thrombsis occurred one month later. Medicine and exercise were recommended for the patient and now mild claudication still remains without affecting his normal life.Conclusions PAES is a disease of relatively low incidence resulting in lower extremity ischemia, which can be successfully cured with proper management.
Residual Popliteal Aneurysm Perfusion Leading to Embolic Complications: A Case Report

Abstract:Introduction: Surgical management of popliteal artery aneurysms has been described for half a century. Long term development of the excluded aneurysm sac in the popliteal segment however remains widely unknown, with only a few small series describing outcomes. Residual aneurysm perfusion has the potential to lead to serious complications. Report: A 63 year old man presents with skin and soft tissue necrosis of the right calf two years after proximal and distal aneurysm ligation and great saphenous vein bypass for a popliteal artery aneurysm. Computed tomography and magnetic resonance angiography show perfusion of the excluded aneurysm as well as extensive necrosis of the gastrocnemius muscle. Direct angiography of the aneurysm demonstrated retrograde aneurysm perfusion due to insufficient distal ligation with recurrent micro-embolisation to the calf via geniculate arteries. Coiling of the geniculate arteries was performed, plugging the connection to the tibiofibular trunk and embolisation of the aneurysm sac. After the intervention, no flow was seen in the aneurysm sac and the patient made full recovery. Discussion: Residual aneurysm sac perfusion can lead to complications long after successful aneurysm exclusion. Follow-up after surgery of popliteal aneurysms should include observation of the excluded aneurysm sac with control of residual blood flow. For persistent sac perfusion, aneurysm enlargement or symptoms, further treatment should be considered. Surgical aspects such as complete transection of the artery after aneurysm ligation or end to end anastomosis of the bypass may be considered, to prevent such complications.

Abstract:The effectiveness of endovascular treatment for popliteal arterial injury has not been well documented. This study was aimed to investigate the mid-term outcomes of endovascular repair of traumatic isolated popliteal arterial injury.Medical records of the patients who underwent endovascular repair for traumatic popliteal arterial injuries from January 2012 to February 2020 were reviewed retrospectively. Clinical data including patient demographics, Injury Severity Score (ISS), type of injury, classification of acute limb ischemia, concomitant extremity fracture, runoff vessel status, complications, time of endovascular procedure, time interval from injury to blood flow restoration, length of hospital stay, reintervention, and follow-up were collected and analyzed.Endovascular repair was performed in 46 patients with traumatic popliteal arterial injuries. The mean ISS was 15.8±6.2. The overall limb salvage rate was 89.1%. There were 10 penetrating and 36 blunt injuries (78.3%). Initial angiographic findings revealed: occlusion in 34 patients (73.9%), pseudoaneurysm in 2 (4.4%), active extravasation in 9 (19.5%), and arteriovenous fistulas in 1 (2.2%). Technical success was achieved in all 46 patients, via antegrade access in 24 patients (52.2%) and concurrent retrograde access in 22 (47.8%). The mean time interval from popliteal artery injury to blood flow restoration was 10.6±4.9 hours and mean operative time was of 54.9±10.0 minutes. The mean follow-up was 36.1 ±14.5 months. Primary patency rate was 75.3% at 12 months, 61.9% at 24 months and 55.7% at 48 months. Secondary patency rate was 92.2% at 12 and 24 months, 85.2% at 48 months. Cox multivariate analysis revealed single vessel runoff was independent risk factor for primary patency loss.Endovascular repair of isolated popliteal artery injury may be a safe and effective alternative treatment in select patients, with acceptable mid-term outcomes. Single vessel runoff was an independent risk factor for primary patency loss.

Iatrogenic popliteal artery pseudoaneurysm: A case report

Abstract:Background: Popliteal artery pseudoaneurysm is a state of vascular wall rupture in the popliteal artery. It is generally a rare situation and the most common etiologic factor is iatrogenic causes. Case presentation: This case report presents a successful diagnosis and management of a 31-year-old patient who was presented with a mass behind the knee, three months after femoropopliteal bypass for the treatment of a gunshot to the knee. An endovascular approach using coils was utilized for the patient, which led to complete remission for the patient. Discussion: The current patient had two primary therapeutic challenges: bleeding and hematoma growth, clot development causing blood flow restriction, and limb ischemia. Pseudoaneurysm also caused internal inflammation, which increased the risk of thrombosis and bypass graft damage during open surgery. Due to the risk of recurrence and graft damage, ultrasound-guided compression was not possible. Thus, endovascular therapy was preferred. Conclusion: The endovascular approach using coils is an option for the management of PAP. However, the endovascular approach should be considered carefully according to the patient's status.

Abstract:Objective To evaluate the incidence and risk factors of popliteal embolization among patients who received primary percutaneous transluminal angioplasty stenting (PTA + S) and its remedy.Methods 436 lesions in 388 superficial femoral artery (SFA) arteriosclerosis patients were recruited in this study from November 2008 to December 2012.All of them underwent PTA + S.An angiography was performed before and after the procedure to evaluate the existence of embolization.Then we calculated the incidence and analyzed the relationship between popliteal embolization and age,gender,hypertension,diabetes mellitus,coronary artery disease,cerebral ischemic stroke,lesion type and TASC Ⅱ classification.Results There were 10 popliteal embolization events (2.3％) in the 436 lesions.The rate of popliteal embolization was higher in Transatlantic Inter-Society Consensus (TASC) C/D group than that in TASC A/B group (P =0.002),and it was higher in chronic total occlusion (CTO) lesions than that in stenotic lesions (P ＜0.0001),and in those with a history of ischemic stroke (P =0.007) than those without.Age,sex,smoking,hypertension,diabetes,hyperlipidemia and runoff status didn't correlate significantly with the incidence of popliteal embolization.Conclusions Popliteal embolization is generally reversible by comprehensive remedy techniques.TASC C/D lesion,CTO and history of ischemic stroke are risk factors for popliteal embolization.

Abstract:OBJECTIVE:To evaluate the role of multidetector row spiral computed tomography in the diagnosis of popliteal artery entrapment syndrome (PAES).METHOD:Ten patients (13 limbs) were diagnosed as PAES through open surgery in Peking Union Medical College Hospital from January 2006 to December 2009, among whom three (30.0%) had both limbs involved.RESULTS:Eleven limbs (84.6%) had occlusions in popliteal artery at admission. While only 15.4% of the patients were diagnosed as PAES by positional stress test, Doppler ultrasound, and arterial angiography, the disease was confirmed in all 10 patients by spiral computed tomography. As shown by inter-operative exploration, the PAES types included type 1 (n=2, 15.4%), 2(n=5, 38.5%),3 (n=4, 30.8%), and 5 (n=2, 15.4%). Two limbs underwent popliteal artery release only, 8 limbs underwent politeal artery release, thromboendarterectomy, and angioplasty with a venous or prosthetic patch, and 3 patients underwent catheter-based thrombolysis and popliteal artery release subsequently. During the follow-up (mean: 22 months; range: 4-33 months), the primary patency rate was 92.3% (12/13), the total patency rate was 100% (13/13), and the limb salvage rate was 100%.CONCLUSION:Positional stress test, Doppler ultrasound, and arterial angiography play limited roles in the diagnosis of PAES with popliteal artery occlusion, while multidetector row spiral computed tomography is much helpful in diagnosing and typing PAES.
