Abstract:Background: High prevalence and mortality of sepsis have been widely reported in pediatric intensive care units (PICUs) around the world. Till now, there’s a lack of national epidemiological data in Mainland China. We carried out this national epidemiological study to reveal the prevalence, therapeutic interventions, clinical outcomes and other characteristics of pediatric sepsis in PICUs of China.Methods: In this cross-sectional study, a point prevalence was conducted on 12 days from December 2018 to November 2019 (from 9 AM of the third Wednesday of every month to the next 9 AM of Thursday) in 53 hospitals from 6 regions of China. Eligible for inclusion were children aged older than 28 days and younger than 18 years with sepsis as defined by the 2005 International Pediatric Sepsis Consensus (excluding previously-diagnosed sepsis patients). The patients were excluded if they did not meet the inclusion criteria, died within 1 hour of admitted to PICU or underwent extracorporeal membrane operation or blood purification 5 days before the enrollment time. General information (source of admission, primary site of infection, diagnosis, chest X-ray findings), therapeutic strategies (antibiotics, vasoactive drug, fluid resuscitation, mechanical ventilation, extracorporeal membrane oxygenation and adjuvant therapies) and prognostic factors (complications, risk factors, social factors and clinical outcomes) were collected.Findings: Totally 10307 individuals were screened, among them, 580 patients were diagnosed as sepsis, severe sepsis or sepsis shock. The overall prevalence of sepsis was 5.6% (95% confidence interval 5.2%-6.1%). 70% patients were rescued with fluid resuscitation, and 45.5% septic children were administered with corticosteroids. Dopamine (61.2%) was the most common initial vasoactive drug. The mortality in mild sepsis, severe sepsis and septic shock was 8.3%, 16.5% and 23.9%, respectively. The school age children own the highest mortality (14.6%) compared to the infancy, toddler’s age, preschool age and adolescence. The chemotherapy in non-survival group (n=11, 16.2%) was much higher than survival group (n=34, 8.3%)(p=0.040). The most common site of infection was respiratory tract (52.9%) followed by abdominal cavity (13.8%). A total of 683 specimens were collected from all enrolled patients in which 79.2% of these specimens were tested positive for an isolated infectious organism. Catheter specimen culture showed the highest positive detection rate (94.6%, 35/37), Gram-negative bacteria (38%,203/541) and gram-positive bacteria (25%,136/541) were detected with certain difference in all positive specimen cultures. The most common isolated bacteria were Staphylococcus aureus (8.1%) followed by Klebsiella species (6.8%), Streptococcus pneumonia (6.7%), and Escherichia coli (5.9%). 99.1% of the patients were treated with antibacterial, 7.5% with antivirals, and 16.6% with antifungal agents. Cephalosporins (26.3%) and carbapenems (25.1%) were the most commonly used antibiotics. 68.8% of children had presented at least one complication. multiple organ failure was the most common complication (n=M97, 33.9%).Interpretation: Sepsis in children is a major public health problem in China. This study firstly described the epidemiological characteristics of pediatric sepsis through prospective and cross-sectional method in China.
