Abstract:OBJECTIVES:To identify the FXI gene mutations in two Chinese pedigrees of congenital factor XI deficiency.METHODS:The peripheral blood samples were collected from the probands and their family members and the plasma FXI:C and FXI:Ag were determined. All the exons and exon-intron boundries of FXI gene were amplified with PCR and sequenced thereafter.RESULTS:A nonsense mutation Trp228stop and two missense mutations Glu323Lys and Leu172Pro were disclosed in the two pedigrees. All mutations existed in a heterozygous state.CONCLUSION:The FXI gene mutations Trp228stop, Glu323Lys and Leu172Pro attribute to the pathogenesis of the congenital factor XI deficiency in Chinese. The Leu172Pro is identified for the first time.
Compound Heterozygous Nonsense Mutations Leading to Hereditary Deficiency of Coagulation Factor XI in a Chinese Pedigree

Abstract:The objective is to identify the gene mutations responsible for the deficiency of factor XI (FXI) in a Chinese pedigree. The FXI activity was tested with clotting assay. The F11 gene was amplified by PCR with direct sequencing. ClustalX-2.1-win and three online bioinformatics softwares were used to study the conservatism and harm of the mutation. The proband was a 70-year-old male and had a prolonged APTT of 67.8s. The corrected APTT was 28.3s and he had reduced FXI: activity of 0.8%. His son and daughter had normal APTT of 26s and 27.3s and FXI:C of 72% and 75%, respectively. DNA sequencing analysis showed the proband carried a compound heterozygous g.841C>T and g.1556G>A mutation , resulting in Ser281Stop and Trp519Stop, respectively, which caused premature termination of transcription in the F11. His son had the heterozygous g.841C>T mutation of F11 and his daughter had the heterozygous g.1556G>A mutation of F11. The three bioinformatics softwares indicated that the mutation had affected the function of the protein. The two nonsense mutations had been reported previously in different patient, but this is the first time to be reported in the proband, which was responsible for the decrease of FXI: activity.

Abstract:Objective:To explore the genetic mutation, clinical characteristics, diagnosis and treatment of patients with hereditary coagulation factor FⅪ deficiency.Methods:On August 10 and October 30, 2020, two patients with hereditary FⅪ deficiency who were admitted at Department of Hematology, West China Hospital of Sichuan University were selected as research subjects. Patients 1 and 2 were numbered according to visiting time. Diagnosis was made according to coagulation function test and results of gene sequencing F11 gene.Clinical observation or symptomatic treatment were performed according to clinical bleeding symptom and severity. Novel mutations were confirmed by retrieving Human Gene Mutation database (HGMD), Single Nucleotide Polymorphism Database (dbSNP) and PubMed database. Pathogenicity of the novel missense mutation was predicted by computational software of Mutation Taster, Polyphen-2, and PROVEAN. Follow-up of patients was conducted until February 28, 2021. Clinical manifestations and process of diagnosis and treatment of two patients were analyzed retrospectively. China National Knowledge Infrastructure database, Wanfang Data Knowledge Service Platform, PubMed database were searched for the same and similar case reports as F11 genetic mutation in this study.Retrieval time was from database inception to February 28, 2021. Types of genetic mutations and clinical manifestations related to patients in this study were summarized.This study meeted the requirements of the World Medical Association Declaration of Helsinki revised in 2013. Informed consent of patients was obtained from pateints. Results:① Results of medical history: patient 1, a 24 years old man, was admitted due to " hematoma after soft tissue trauma in left thigh and preoperative examination revealed coagulation dysfunction" and did not complain of significant discomfort.Patient 2, a 32 years old woman, was admitted due to " pre-pregnancy physical examination found prolonged activated partial thromboplastin time (APTT)" and complained of occasional ecchymosis.② Results of related laboratory examination: APTT, prothrombin time (PT) and FⅪ activity (FⅪ∶C) of patient 1 were 97.9 s, 11.9 s and 0.6%, while patient 2 were 95.4 s, 12.5 s, and 1.2%, respectively. ③ Results of DNA sequencing: patient 1 had a compound heterozygous missense mutation of F11 gene at c. 149G>T(p.Cys50Phe) and c. 1204C>T (p.Gln402Ter), while patient 2 had a homozygous deletion of F11 gene at c. 1058delA (p.Asn353ThrfsTer18). ④ F11 gene missense mutation at c. 149G>T(p.Cys50Phe) and deletion c. 1058delA(p.Asn353ThrfsTer18) were two novel mutations and both were pathogenic.⑤Results of treatment and follow-up: two patients received clinical observation. At end of follow-up, general condition of the patients was good. ⑥ Results of literatures review: among 10 patients reported in 6 relevant literatures, there were 1, 3, 6 patients caused by heterozygous nonsense mutation of c. 1204C>T(p.Gln402Ter), nonsense mutation of c. 1202C>T(p.Trp401Ter) adjacent to c. 1204C>T (p.Gln402Ter) and missense mutations near the position of c. 149G>T(p.Cys50Phe), respectively. Conclusions:Two novel mutations of hereditary FⅪ deficiency namely a missense mutation at c. 149G>T(p.Cys50Phe) and a deletion at c. 1058delA(p.Asn353ThrfsTer18) of F11 gene were reported in this study. Both of the two mutations related to decrease of FⅪ∶C.
Clinical manifestations and mutation spectrum of 57 subjects with congenital factor XI deficiency in China

Abstract:Congenital factor XI (FXI) deficiency is a rare bleeding disorder with unpredictable bleeding tendency. Few studies in a large cohort have been reported regarding associations between FXI activity (FXI:C) or genotypes and bleeding symptoms currently. This study characterized clinical manifestations and mutation spectrum of 57 subjects with FXI deficiency in China. Clinical data were collected and mutations were identified by direct sequencing and determined by mRNA analysis. The result revealed bleeding symptoms were only found in 12 patients (12/57, 21.1%) with severely reduced FXI:C, and prolonged bleeding post injury/surgery as well as easy bruising were the commonest bleeding manifestations presented in respective 5 cases (5/12, 41.7%). A total number of 37 mutations were identified including 19 missense mutations, 9 nonsense mutations, 6 splice site mutations and 3 small deletions. Among them, 4 missense mutations, 5 splice mutations, 3 small deletions and a nonsense mutation were newly detected. W228*, G400V, Q263* and c.1136-4delGTTG with a total frequency of 48.3% were the most four common mutations in Chinese patients. RT-PCR analysis was carried out and confirmed that both c.596-8T>A and c.1136-4delGTTG were pathogenic due to frameshift resulting in respective truncated proteins. Our findings suggested clinical manifestations had little to do with FXI:C or genotypes, which required further study. This study, the largest investigation of FXI deficiency in China revealed that the F11 mutation spectrum of Chinese population was distinct from those of other populations earlier established.

Abstract:Congenital factor XII (FXII) deficiency is a rare autosomal recessive disorder, characterized by a great variability in its clinical manifestations. In this study, we screened for mutations in the F12 gene of 4 unrelated patients with FXII coagulant activity <10% of that of normal human plasma. To investigate the molecular defects in these FXII-deficient patients, we performed FXII mutation screening. By sequencing all coding exons as well as flanking intronic regions of the F12 gene, 6 different mutations, including 3 missense mutations (Gly341Arg, Glu502Lys and Gly542 Ser), 1 insertion (7142insertC) and 2 deletions (5741-5742 delCA and 6753-6755delACA), were identified on the F12 gene. Three of them (Gly341Arg, 5741-5742delCA and 6753-6755delACA) are reported here for the first time. Computer-based algorithms predicted these missense mutations to be deleterious. This study has increased our knowledge of the mutational spectrum underlying FXII deficiency.

Abstract:OBJECTIVE:To identify gene defect in a Chinese pedigree of hereditary coagulation factor XI (FXI) deficiency.METHODS:The peripheral blood samples were collected from the proband and her family members. The plasma PT, APTT, FXI:C and FXI:Ag were assayed. The FXI gene exons and exon-intron boundaries of the proband were amplified by PCR and then sequenced directly. The mRNA of FXI in the peripheral blood was analyzed with RT-PCR.RESULTS:The proband and some of her family members had prolonged APTT. The plasma FXI:C and FXI:Ag of the proband, her brother and her parents were lower than 10% and 50% of the normal values, respectively. Nucleotide sequence analysis revealed that the proband and her brother had a homozygous mutation of IVS J-4delgttg in FXI gene. The mutation was inherited from her parents who were heterozygotes. The mutation was not found in 60 normal subjects. No FXI mRNA was detected in peripheral blood sample of the proband.CONCLUSION:The IVS J-4delgttg is a novel mutation causing FXI deficiency, which may interfere with mRNA splicing.
A Compound Heterozygosis of Two Novel Mutations Causes Factor X Deficiency in a Chinese Pedigree

Abstract:Background: Mutations in the F10 -coding gene can cause factor X (FX) deficiency, leading to abnormal coagulation activity and severe tendency for hemorrhage. Therefore, identifying mutations in F10 is important for diagnosing congenital FX deficiency. Methods: We studied a 63-year-old male patient with FX deficiency and 10 of his family members. Clotting and immunological methods were used to determine activated partial thromboplastin time (aPTT), prothrombin time (PT), thrombin time (TT), fibrinogen levels, FX activity, and FX antigen levels. The platelet count was determined. A mixing study was performed to eliminate the presence of coagulation factor inhibitors and lupus anticoagulant. Mutations were searched using whole-exome sequencing and certified by Sanger sequencing. Results: Genetic analysis of the proband identified two single-base substitutions: c.1085G>A (p.Ser362Asn) and c.1152C>A (p.Tyr384Ter, termination codon, caused by the DNA sequence TAA). His FX activity and antigen levels were 1.7% and 408.53 pg/mL, respectively; aPTT and PT were 52.3 and 48.0 s, respectively. One brother had the same compound heterozygous mutations, and his FX activity and antigen levels were 1.3% and 465.47 pg/mL, respectively; his aPTT and PT were 65.2 and 54.5 s, respectively. His mother, another brother, and one sister were heterozygous for c.1085G>A (p.Ser362Asn), and his daughter and grandson (6 years old) were heterozygous for c.1152C>A (p.Tyr384Ter). Conclusion: The heterozygous variants p.Ser362Asn or p.Tyr384Ter indicate mild FX deficiency, but the compound heterozygous mutation of the two causes severe congenital FX deficiency and bleeding. Genetic analysis of these two mutations may help characterize the bleeding tendency and confirm congenital FX deficiency.

Abstract:The aim of this study was to elucidate the molecular defects of a Chinese family with hereditary coagulation factor XII (FXII) deficiency. The FXII activity (FXII:C) and FXII antigen (FXII:Ag) levels were measured by clotting assay and ELISA, respectively. To identify mutations, the F12 gene sequencing was carried out. ClustalX-2.1-win and four online bioinformatics tools were applied to study the conservatism and harm of the mutation. The proband's FXII:C and FXII:Ag were 3 and 4%, respectively. Sequencing analysis revealed compound heterozygous mutations, including the deletion mutation (c.130delG) resulting in p.E26Sfs∗50 and the missense mutation (c.1561G>A) resulting in p.E502K. Bioinformatics indicated that mutations probably disrupt the function of the FXII protein. The c.130delG heterozygous deletion variation and the c.1561G>A heterozygous missense variation were responsible for the reduction of FXII:C in this family, of which c.130delG was first reported in the world.

Abstract:Objective: To determine whether polymorphisms in the genes for coagulation factor. II, V, VII could predispose an individual to increase risk for coronary artery disease (CAD) and/or myocardial infarction (MI) in Chinese. Methods: We screened coagulation factor II(G20210A), V(G1691A), VII (R353Q and HVR4) genotype in 374 patients undergoing coronary angiography by polymerase chain reaction and restriction fragment length polymorphism (PCR-RFLP) assay. Results: The R353Q and HVR4 gemotype of the factor VII distribution was in accordance with Hardy-Weinberg equilibrium. The frequencies of FVII genotype or allele did not show statistically significant differences between CAD group and controls or between male and female. The frequencies of the Q allele and (RQ+QQ) genotype were significantly higher among the CAD patients without myocardial infarction (MI) history than among those with MI history (P<0.05). However, HVR4 polymorphism was not significantly different within groups. We only find one normal control of factorII (G20210A) mutation. No coagulation factor V(G1691A) mutation was found in the CAD patients and controls. Conclusion: The factor II(G20210A), V(G1691A) mutation is absent and may not be a major genetic factor for CAD and/or MI; the Q allele of the R353 Q polymorphism of the factor VII gene may be a protective genetic factor against myocardial infarction in Chinese.

Abstract:Objective To explore the relationship between the genotypes and phenotypes in a inherited FⅦ deficiency pedigree. Methods Factor VII coagulation (FⅦ:C) and Factor X coagulation (FX: C) were measured in a inherited FⅦ deficiency pedigree. FⅦ gene mutations were detected in the proband and her family members by PCR and DNA sequencing. The relation between the detective results and the clinical manifestation were analyzed. Results Homozygote of C→T transition at position 10827 in exon 8 resulting in Arg304Trp transition was identified in the proband and two siblings. High level of FX: C was also found in the proband and two siblings, but all had no symptoms of bleeding. Conclusion Not only the level of FⅦ:C but also the level of FX: C might be involved in the clinical manifestation in the patients with inherited FⅦ: C deficiency.

Abstract:To explore the molecular mechanisms involved in a pedigree with inherited coagulation factor X (FX) deficiency.The activated partial thromboplastin time (APTT), prothrombin time (PT), FX activity (FX:C) and FX antigen (FX:Ag) test were adopted for phenotype diagnosis. All the 8 exons, intron/exon boundaries and the 5'untranslated regions (UTR) of the FX gene were amplified by polymerase chain reaction (PCR) from the genomic DNA extracted from the peripheral blood of the propositus. The PCR products were screened by direct sequencing. The mutation was confirmed by allele specific PCR (ASPCR).The phenotype of the propositus was identified as FX deficiency (type II). Two novel FX gene mutations were detected in the propositus: one was a donor site splice mutation in intron 1 (IVS1 + 1G-->A), another was a missense mutation 1185G-->A in exon 8 (Arg347His).The FX deficiency of the propositus is caused by double heterozygous mutations IVS1 + 1G-->A and Arg347His.

Abstract:FX is a vitamin K-dependent coagulation protease critically essential for the coagulation cascade. FXD (congenital deficiency of factor X) is a rare coagulation disorder that inherited as an autosomal recessive trait. Here we reported a patient with bleeding diathesis from infant. The proband with pseudotumor in cerebral articular and cavity were identified as encapsulated hematocele ultimately. FX sequence analysis revealed that the patient carried a novel homozygous missense mutation that resulted in the Val384Ala substitution. Further investigation of the novel mutation would deepen our understanding of the bleeding mechanism involved in FXD.
Severe Coagulation Factor X Deficiency Caused by Two De Novel Heterozygous Missense Mutations in Factor X Gene

Abstract:Objective:To identify gene mutations of a patient with coagulation factor Ⅹ (FⅩ) deficiency. Methods:The activated partial thromboplastin time(APTT), prothrombin time(PT), thrombin time, the activities of FⅡ,FV、FⅦ,FⅧ,FⅨ and FⅩ(FⅡ: C,FⅦ:C,FⅧ:C,FⅨ:C and FⅩ:C) were performed for phenotypic diagnosis. The genomic DNA was extracted from the peripheral blood of the proband and all the 8 exons and their flanks of F10 gene were amplified by polymerase chain reaction (PCR). The PCR products were screeed by direct sequencing and the mutations were further confirmed by restricted enzyme digestion to eliminate the possibility of polymorphism. Results:APTT,PT,TT,FⅡ:C,FⅤ:C,FⅦ:C,FⅧ:C,FⅨ:C and FⅩ:C of the proband were 100 s, 46.6s,13.9 s,60.0%,89%,59%,81.5%,63% and 2.4%, respectively. Compound heterozygous missense mutations in F10 gene were identified in the proband. One was G to A in exon 2, leading Gla(GAG) 26 to Lys(AAG) in the Gla domain; another was T to C in exon 8, resulting the substitution of Ser(UCC)425 by Pro(CCC) in the catalytic domain. Conclusion:These two de novel heterozy-gous missense mutations might be the molecular mechanisms of coagulation factor Ⅹ deficiency for the patient.

Abstract:The prevalence of factor XI (FXI) deficiency is 1 per 10 to 20,000 in the general population, much higher than that reported in most texts. The prevalence is higher in Ashkenazi Jews where it is about 1:20. Clinically, FXI deficiency presents as a mild bleeding disorder mostly associated with posttraumatic or postsurgical hemorrhages or unexplained minor bleeding. It is often discovered due to incidental finding of a prolonged activated partial thromboplastin time (aPTT) on routine laboratory screening. FXI deficiency is an autosomal recessive bleeding disorder with many causative F11 gene defects. Diagnosis is based on FXI activity, antigen levels, and molecular diagnostics. As FXI levels do not correlate with bleeding symptoms, identification of pathogenic genetic variants may be a more accurate predictor of bleeding risk and therefore aid in the clinical management of the patient. Two variants in the F11 gene account for most cases found in the Jewish and Arab populations. Patients with FXI deficiency can develop inhibitors to FXI although spontaneously acquired inhibitors are extremely rare. We will discuss laboratory and molecular assays used to diagnose FXI deficiency as well as interferences that can complicate diagnosis including new anticoagulants and acquired FXI inhibitors. Abstract:OBJECTIVE:To study the distribution of coagulation factor V(FV), VII(FVII) gene polymorphisms in Chinese Han population and the association of the polymorphisms with coronary heart disease(CHD). METHODS:Genotypes of FV FVII were typed in 234 CHD patients and 210 controls by polymerase chain reaction-restriction fragment length polymorphism. Selected coronary angiography was performed in 234 CHD patients. RESULTS:FVII allelic frequencies of R,Q and H7,H6 were 94.6%, 5.6%, 70.3%, 29.7% and 91.9%, 8.1%, 60.9%, 39.1% in CHD group and control group respectively. Genotype distribution was in accordance with Hardy-Weinberg equilibrium. There was no significant difference in frequencies of allele and genotype in R353Q or HVR4 polymorphisms between CHD group and control group. The distribution of allele and genotype in R353Q was of significant difference between non-myocardial infarction subgroup and myocardial infarction subgroup (chi2 = 4.711, P<0.05, OR=0.37,95% CI: 0.15-0.94). However, HVR4 polymorphism was not found to be of significant difference within two group (chi2 = 0.142, P>0.05). There was no FV Leiden mutation in all the CHD patients and normal controls. CONCLUSION:The Q allele of the R353Q polymorphism of the FVII gene may be a protective factor against myocardial infarction.
Global epidemiology of factor XI deficiency: A targeted review of the literature and foundation reports

Abstract:Introduction Hereditary factor XI (FXI) deficiency is a rare coagulation disorder that may result in excessive bleeding requiring intervention to restore haemostasis. Aim The aim of this review was to report the current knowledge of the worldwide incidence and prevalence of FXI deficiency. Methods A targeted PubMed search using terms related to FXI deficiency was conducted to identify studies published from April 2002 through April 2022. A manual search supplemented the electronic search. Studies were eligible for data abstraction if they reported population-based incidence proportions/rates or prevalence proportions for FXI deficiency. Results The electronic and manual searches returned 253 publications. After applying exclusion criteria, seven publications were included in the analysis, including a global report from the World Federation of Haemophilia (WFH). Six publications provided information on the prevalence of FXI deficiency that included 74 countries and regions. The estimated prevalence of FXI in the WFH report ranged from 0/100,000 in several countries to 55.85/100,000 individuals in the United Kingdom. Prevalence estimates in the PubMed findings ranged from .1 to 246.2/1,000,000 inhabitants with varying methods of case identification and time periods of analysis. One study estimated the incidence of FXI deficiency in Yecla, Spain at 2% of blood donors and .09% of hospital inpatients/outpatients with activated partial thromboplastin time (aPTT) tests. Conclusion FXI deficiency is rare across the world, but additional steps could be taken to improve incidence and prevalence estimation, for example, development of a consistent FXI deficiency definition and incorporating genetic testing into a clinical routine to better identify and characterise cases.

Abstract:Background and Objectives. Most secreted proteins, including coagulation factor X (FX), are synthesized with a signal peptide, which is necessary for targeting the nascent polypeptide into the endoplasmic reticulum. Characterization of naturally occurring mutations may provide insights into the functional roles of the amino acids in the signal peptide.Design and Methods. A 52-year old male patient with type I FX deficiency was studied. Mutations were searched for by FX gene (F10) sequencing. The wild-type and the mutant FX proteins were expressed in transfected cells and then immunological assays were performed. Pulse-chase experiments and cell-free expression studies were conducted to determine the cellular fate of the mutant FX molecules.Results. The patient we studied was homozygous for a substitution of arginine for serine at codon -30 in the signal sequence of F10. Immunoassays detected low FX antigen levels in both the conditioned media and lysates of the cells expressing the mutant protein. Pulse-chase analysis showed that only trace amounts of the mutant FX protein were detectable in the conditioned media, and that the mutant molecules did not accumulate inside the cells either. The results of cell-free expression studies showed that although the transcription and translation of the mutant construct were normal, no post-translational processing, such as N-linked glycosylation, occurred in the presence of microsomes.Interpretations and Conclusions. These findings suggest that substitution of a neutral polar amino acid, serine by arginine, in the hydrophobic core of FX signal peptide severely impairs the ability of the protein to enter the endoplasmic reticulum and results in FX deficiency.
