Abstract:PLAG1 (pleomorphic adenoma gene 1) is a proto-oncogene whose overexpression is a crucial oncogenic event in salivary gland pleomorphic adenomas (PA), and in carcinoma ex-PA. The aim of the present study is to evaluate the sensitivity and the specificity of PLAG1 as a marker in the differential diagnosis of salivary gland benign and malignant tumors. We examined 101 cases, including 36 PAs, 8 myoepitheliomas, 3 basal cell adenomas, and 1 canalicular adenoma among benign tumors; 16 mucoepidermoid carcinomas, 10 adenoid cystic carcinomas, 8 acinic cell carcinomas, 8 polymorphous low-grade adenocarcinomas, 7 salivary duct carcinoma, and 4 epithelial-myoepithelial carcinoma among malignant tumors. PLAG1 was diffusely positive in 94.4% of PAs and in all myoepitheliomas, although with a lower staining intensity. Among malignant tumors, 2 (25%) polymorphous low-grade adenocarcinomas and 1 salivary duct carcinoma ex-PA were positive. In conclusion, PLAG1 is a marker with good specificity for PA and could be a useful diagnostic adjunct in the diagnosis of salivary gland tumors. In particular, this marker is negative in the most common salivary carcinomas, including adenoid cystic carcinoma, mucoepidermoid carcinoma, and acinic cell carcinoma. However, some mimickers of PA, like polymorphous low-grade adenocarcinoma, may show occasional positivity for PLAG1, thus limiting its diagnostic use. In addition, carcinoma ex-PA shows consistent positivity, and therefore should be considered as a diagnostic possibility in case of a malignant tumor with PLAG1 expression.

Abstract:Representative images of a fine‐needle aspiration showing a myoepithelial‐rich pleomorphic adenoma with a novel PLAG1 inversion of chromosome 8 and LRP1B, PBRM1 and TCF3 mutations. The diagnosis of pleomorphic adenoma may be challenging due to the predominance of one of the three components. We present the case of a 64‐year‐old male with a remote history of a submandibular gland excision and recent regrowth of his mass in the surgical bed, which was consistent with a myoepithelial‐rich pleomorphic adenoma with a novel inversion of the chromosome 8 and LRP1B, PBRM1, and TCF3 mutations. The cytomorphologic and immunophenotypic pitfalls of this tumor and the differential diagnoses are reviewed in this article. Pleomorphic adenoma (PA) is the most common salivary gland neoplasm worldwide (50%–70%). Most cases of PA are straightforward diagnoses. However, the diagnosis may be challenging due to the morphological diversity of PAs, which in many cases is caused by the predominance of one of the three components or the presence of metaplastic epithelium. We present the case of a 64‐year‐old male with a history of prostate cancer and right submandibular gland excision 16 years ago with an unknown previous diagnosis and recent regrowth of his mass in the surgical bed. The tumour showed a predominant spindle cell morphology, cytokeratin‐positivity, variable expression of myoepithelial markers in the cytology and surgical pathology specimens, and novel inversion of the chromosome 8, and LRP1B, PBRM1 and TCF3 mutations.

Pleomorphic adenoma: detection of PLAG1 rearrangement-positive tumor components using whole-slide fluorescence in situ hybridization

Abstract:Pleomorphic adenoma (PA) consists of heterogeneous histological architecture mixed with epithelioid and mesenchymal forms. Various types of epithelial or myoepithelial malignancies arise from PA, but sarcomas are extremely rare. A human androgen receptor gene (HUMARA) clonality assay has suggested that PA is clonal in nature. However, clonality of various tumor components of PA would be difficult to determine with this assay. In addition, the results obtained should be carefully interpreted. PLAG1 rearrangements are considered a good molecular marker for neoplasticity in PA. We aimed to clarify the neoplasticity of the various tumor components present in PA using whole-slide fluorescence in situ hybridization (FISH). Five PA cases positive for PLAG1 rearrangements were examined. Using an immunohistochemistry panel, cell components in PA were classified into eight cell types. To precisely localize PLAG1 rearrangement-positive cell components at the cellular level, sequential retrieval of whole-slide imaging (WSI) data of HE histology and FISH for PLAG1 rearrangement was carried out. PLAG1 rearrangements were detected in ductal cells, myoepithelial spindle cells, myoepithelial oncocytic cells, myoepithelial plasmacytoid cells, and mesenchymal chondroid cells, but not in mesenchymal lipid cells, mesenchymal fibrous cells, or vascular endothelial cells. Immunohistochemical PLAG1 expression was restricted to cell components harboring PLAG1 rearrangements.The results of the present study indicate that ductal and myoepithelial, chondroid cells are neoplastic but lipid, fibrous, and endothelial cells are not. PLAG1 immunohistochemistry is useful in discriminating neoplastic from non-neoplastic cell components. These findings may be important for elucidating tumorigenesis and the process of malignant transformation in PA.
Clinicopathological study of 74 palatal pleomorphic adenomas.

Abstract:Pleomorphic adenoma (PA) is the most common salivary gland tumor and the palate is the most common intraoral site for PA. This study aimed to present the clinicopathological features of a series of 74 palatal PAs.Seventy-four palatal PAs were collected from 1993 to 2009. The clinical and histopathological features of these 74 PAs were reviewed and analyzed.The 74 palatal PA patients showed a marked female predilection (49:25, ∼2:1) and were nearly evenly distributed from the 3(rd) to 8(th) decades of life with a mean age of 47 years. All palatal PAs were treated by wide surgical excision and recurrence was noted in one PA. Histopathologically, there were 47 classic and 27 cellular PAs. Of the 74 PAs, 12 were completely encapsulated, 40 partially encapsulated, and 22 nonencapsulated. The duct-like structures and myxoid stroma were more or less found in every palatal PA. Plasmacytoid myoepithelial cell, clear cell, squamous epithelial nest, keratin pearl, hyalinized stroma, osteoid area, and chondroid area were found in 50, 19, 29, 19, 49, eight, and six PAs, respectively.We conclude that the palatal PA patients show a prominent female predilection (2:1) and are nearly evenly distributed from 20 years to 79 years of age. Plasmacytoid myoepithelial cell is the most characteristic type of tumor cell in PAs. Wide surgical excision is treatment of choice for PAs. Although ∼84% of palatal PAs are partially or nonencapsulated, recurrence of the lesion is rarely encountered after total surgical removal of the tumors.
PLAG1 immunohistochemistry is a sensitive marker for pleomorphic adenoma: a comparative study with PLAG1 genetic abnormalities

Abstract:Aims: Pleomorphic adenoma gene 1 (PLAG1) gene rearrangement is the most common genetic abnormality in pleomorphic adenoma (PA), resulting in overexpression of PLAG1 protein. PA and carcinoma ex pleomorphic adenoma (CA ex-PA) can mimic various benign and malignant salivary gland tumours. The aims of this study are to evaluate the sensitivity and specificity of PLAG1 immunohistochemistry (IHC) in the differential diagnosis of PA and CA ex-PA and to compare the PLAG1 immunohistochemical results to PLAG1 gene abnormalities as detected by fluorescence in-situ hybridisation (FISH). Methods and results: PLAG1 immunostaining was performed on 83 salivary gland tumours, including 23 PA, 15 CA ex-PA and 45 other salivary gland tumours. In addition, PLAG1 FISH was performed in 44 cases for the presence of gene rearrangements/amplifications. The results showed high sensitivity of PLAG1 IHC in 96% of PA; however, discordant results between PLAG1 FISH abnormalities and IHC were noted in 15 of 44 cases (34%). Seven PA, four de-novo myoepithelial carcinomas and one basal cell adenocarcinoma had negative FISH results, but were positive for IHC; while three salivary duct carcinomas (SDC) ex-PA were positive for FISH but negative for IHC. PLAG1 IHC can differentiate CA ex-PA from de-novo SDC (P = 0.02), but not from de-novo myoepithelial carcinoma. PLAG1 IHC is a sensitive marker for PA. This could be due to PLAG1 gene abnormalities beyond FISH resolution. Conclusions: A negative PLAG1 IHC might be helpful in excluding a PA diagnosis. Interestingly, in the context of CA ex-PA, FISH is more sensitive than IHC in detecting PLAG1 abnormalities.

Abstract:Pleomorphic adenoma (PA) is a benign salivary gland tumor characterized by genetic heterogeneity and diversity of morphological aspects. PA has epithelial, myoepithelial and stromal components. PA with extensive squamous metaplasia can mimic malignancies presenting a diagnostic challenge. A 35-year-old female patient complained of painless swelling in the left palate with 5 years of evolution. After the incisional biopsy, microscopic analysis revealed a benign neoplasm of glandular origin consisting of epithelial and myoepithelial cells arranged in nests, cords and ductal structures.

Abstract:BACKGROUND: Pleomorphic adenoma (PA) is a commonly occurring benign tumor originating in the salivary glands.OBJECTIVE: The aim was to carry out a systematic literature of reports on pleomorphic adenoma from 2000 to 2018 to determine patient's age spread, gender, anatomical location, capsular invasion, histopathology, treatment and patient outcome.MATERIALS AND METHODS: A PubMed search was conducted with the following key words: adenoma, pleomorphic adenoma, and mixed salivary tumor.RESULTS: Twenty-two articles in English were read in full after fulfilling the eligibility criteria. The mean age of PA occurrence was 44.14 years with a definite female predilection (M:F ratio = 13:8). It most commonly occurred in the facial region (42.85%), and surgical approach is the preferred intervention.CONCLUSION: Pleomorphic adenomas are benign salivary gland neoplasms that can grow into extensive sizes if left untreated and hence need to be diagnosed early. Complete excision of the tumor is the definitive treatment, as enucleation can result in recurrence. Abstract:Background: Pleomorphic adenomas (PAs) of the salivary glands are rare in children and adolescents. We reviewed the clinical manifestations, pathologic features, treatment, and prognosis of salivary gland PA in 90 children and adolescents.Methods: Clinical data including age, sex, location, symptoms, the period of evolution of the symptoms, pathologic type, and surgical treatment were evaluated.Results: The median age was 16.5 years old. There was a 1.43: 1 female-to-male ratio. The parotid gland was the most commonly affected major gland (56.7%), and the palate, the most common site of minor salivary gland involvement. Ultrasonography and computed tomography were performed in most cases for diagnosis. Superficial parotidectomy or total parotidectomy with facial nerve dissection and preservation was the most common surgical procedure. Submandibular triangle dissection was applied to all submandibular PAs. Classic-type PA was the most common histologic subtype (43.3%).Conclusions: Salivary gland PAs in children and adolescents have different characteristics compared with their adult counterparts in regard to histologic subtype and location. Some pleomorphic adenomas of the breast share PLAG1 rearrangements with the analogous tumour of the salivary glands

Abstract:Aims Pleomorphic adenoma (PA) of the breast, and especially its malignant transformation, is extremely rare and represents a diagnostic pitfall. Molecular alterations in this entity have not been investigated. We aimed to examine the clinicopathological features of our breast PAs and perform molecular analysis. Methods and results Seven cases of breast PA, including two cases of carcinoma ex PA, were analysed. PLAG1 and HMGA2 gene rearrangements were assayed by fluorescence in-situ hybridisation (FISH) and RNA sequencing (RNA-Seq), respectively. Reverse transcription-polymerase chain reaction (RT-PCR) and Sanger sequencing were used to verify RNA sequencing results. All seven cases of breast PA occurred in women. The histological features were similar to the analogous tumour in salivary glands, including a dual epithelial-myoepithelial component and negativity of oestrogen receptor (ER), progesterone receptor (PR) and human epidermal growth factor receptor 2 (HER2) by immunohistochemistry. Of the two cases with carcinoma ex PA, one demonstrated minimal invasion and one was extensively invasive. PLAG1 rearrangements were identified in two cases (28.6%), but no rearrangements of HMG2A were found. A novel fusion product in PAs, TRPS1-PLAG1, was identified in one case. No patients had recurrence or metastasis with a follow-up period of 6-158 months. Conclusions Breast PA is rare, but it is an important differential diagnosis of breast pathology with the potential to develop carcinoma ex PA. We report a novel TRPS1-PLAG1 fusion gene in breast PA.
Pleomorphic Adenoma - A Case Report on an Abnormal Mass

Abstract:Rationale: Pleomorphic adenoma (PA) is the most prevalent salivary gland tumour, accounting for 60%-80% of all benign salivary gland tumours, and frequently affects the parotid gland. Their epithelial and connective tissue origins can explain the 'pleomorphic' nature of tumours. This tumour is most common in women between 30 and 50 years. Patient concerns: A 45-year-old female came with the chief complaint of a slow-growing, painless swelling on the left side of her face that had been present for 25 years. Diagnosis: PA of the parotid gland was diagnosed using computed tomography and fine-needle aspiration cytology. PA is a slowly progressing, asymptomatic swelling that rarely exceeds 6 cm in its greatest dimension; in our case, it was 13 cm × 10 cm and weighed 5 kg. Treatment: The ideal management is the surgical removal of the tumour mass, which was done under general anaesthesia. Early diagnosis and treatment planning was critical. Complete removal of the tumour without remnants is crucial to prevent a recurrence. Total parotidectomy was done in this case. Outcomes: There was no evidence of recurrence after comprehensive local surgical resection of the tumour during the follow-up period. Take-away lessons: This case report is significant as it sheds light on the successful treatment approach for a large pleomorphic adenoma. It can provide valuable insights into the management of similar cases, potentially leading to improved patient outcomes and guiding future treatment strategies.

Abstract:Pleomorphic adenoma is the most common histological form of salivary gland tumors such as large salivary gland, parotid gland, and submandibular gland, [ 1 ] while primary lung tumors presenting as pleomorphic adenomas (PA) are extremely rare lesions and have traditionally been considered to be lesions caused by bronchial gland epithelial cells. [ 2 ] We report 7 patients with primary pleomorphic adenoma, which occurred mainly in the lower respiratory tract. Most of the patients reported here had mucoepidermoid and adenoid cystic changes. The clinical features, pathology, immunohistochemistry, imaging, treatment, and prognosis of these patients were mainly analyzed, and the relevant features recorded in the previous literature were compared and analyzed.

Abstract:Salivary gland tumors are morphologically and clinically heterogeneous, making diagnosis and management difficult for oral pathologists and surgeons. Pleomorphic adenoma (PA), a mixed tumor, is a most common salivary gland tumor of benign origin. [ 1 ] Histologically, this tumor is characterized as epithelial and mesenchymal origin, predominantly affecting major salivary glands. Approximately 75% of this tumor is reported in the parotid gland, followed by the submandibular gland (15–17%) and the minor nasal and sublingual gland (10–12%). [ 2 ] This neoplasm is rarely reported in the minor glands of the larynx, lips, floor of the mouth, and trachea (5–7%). [ 3 ] The palatal mucosa is the most common intraoral site, followed by the upper lip and buccal mucosa. Rarely, PAs have been reported in soft tissues, lymph nodes, lacrimal glands, upper and lower limbs, breast, axilla, ear, and mediastinum. [ 4 ] PAs can occur synchronously and asynchronously with other oral lesions and tumors. The neoplasm typically presents with mild symptoms, thus rendering it difficult to make an initial clinical diagnosis during the prehospital stage. Although PA may appear benign, its biological behavior and prognosis indicate a low-grade malignancy, necessitating immediate treatment wherever diagnosed.

Abstract:Objective To expiore the PLAG1expression and analyze the relation between the expression and ER and P53 in pleomorphic adenoma.Methods The expression of PLAG1,ER and P53 in pleomorphic adenoma was detected by using immunohistochemical method.Results In 45 cases of pleomorphic adenoma,the PLAG1,ER and P53 expression was positive,PLAG1 showing a strongly positive expression.The expression level of PLAG1 in recurrence group was lower than that in no recurrence group.The expression of PLAG1,ER and P53 was present in pleomorphic adenoma.Conclusion There was PLAG1,ER and P53 expression in pleomorphic adenoma,with a close correlation among the three.It is suggested that the expression of PLAG1 ER and P53 should be a valuable marker in predicting the biological behavior of pleomorphic adenoma.

Abstract:Pleomorphic adenomas (PA) are the most common type of salivary gland tumors. These slow-growing benign tumors most commonly involve the parotid gland, but can sometimes occur at atypical sites such as the submandibular or minor salivary glands. We describe an atypical case of pleomorphic adenoma with multicentric involvement of the parotid, the submandibular gland, and the parapharyngeal space in a 35-year-old male which mimicked a slow-flow malformation on magnetic resonance imaging (MRI). Diagnosis was confirmed on fine needle aspiration cytology, and conservative approach was opted for the patient in view of perioperative risks. This case highlights the uncommon multicentricity and atypical presentation of PA, challenging the initial differential diagnosis based on MRI features. It also underscores the importance of considering atypical presentations and utilizing accurate diagnostic tools like cytology for managing complex salivary gland tumors.
Malignant Pleomorphic Adenoma of Salivary Gland:A Clinicopathologic Study of 161 Cases

Abstract:Purpose To analyze the clinicopathologic features of salivary malignant pleomorphic adenoma. Methods The clinical and pathological characteristics of 161 salivary gland tumors histologically diagnosed as MPA were analyzed. Results Of the 161 MPA cases, 152 were primary tumors, 7 were recurrent tumors, and 2 were cervical lymph node metastatic tumors. The ratio of male to female of 152 patients with primary MPA was 1.7 ∶1(95 to 57), and the mean age was 59 years(from 27 to 92). 85 tumors were located in parotid gland, 39 in palate, 21 in submandibular gland, 3 in buccal, 2 in lip, and one in retromolar region and maxillary respectively. The subtype of the malignant component was classified as myoepithelial carcinoma in 62 tumors, adenocarcinoma not other specified in 59, adenoid cystic carcinoma in 8, mucoepidermoid carcinoma in 7, salivary duct carcinoma in 3, adenosquamous carcinoma in 3, epithelial-myoepithelial carcinoma in 3, carcinosarcoma in 3, undifferentiated carcinoma in 2, acinic cell carcinoma in 1, and mucinous adenocarcinoma in 1. Histologically the malignant components of 45 tumors were high-grade, 69 intermediate-grade, and 38 low-grade. The malignant components made up less than 50% of the mass in 21 patients, and greater than 50% in 131. 106 tumors were invasive carcinoma, 10 minimally invasive, and 33 non-invasive. The invasive carcinoma and the high-grade tumor had correlation with cervical lymph node metastasis(P0.01). Conclusions MPA is more common in elder male, frequently involves the parotid gland and usually presentes as a painless mass, which grew slowly and recently occurs rapid enlargement. Histologically, benign pleomorphic adenoma components as well as various types of malignant components can be found in most of MPA. The majority of tumors are invasive carcinoma. The malignant components are usually much more than the benign components, and myoepithelial carcinoma and adenocarcinoma not other specified are the most common malignant subtypes. It is more possible for the invasive carcinoma and the high-grade tumor to occur cervical lymph node metastasis.

Abstract:Pleomorphic adenoma (PA) is a common benign salivary gland tumor. This abstract presents a clinical case of a 49-year-old white female with a PA in a minor salivary gland located on the palate, which had been evolving for two months. The patient sought evaluation by a Stomatology specialist. Clinically, the lesion manifested as a unilateral, well-defined, irregularly shaped, infiltrative, and painful mass. The management of this case is detailed, leading to the definitive diagnosis of PA, with the aid of complementary clinical examinations including panoramic radiography and computed tomography.

Abstract:Purpose: Pleomorphic adenoma (PA) is the most common benign salivary gland tumor with salivary gland excision as the treatment of choice; however, recurrences after parotidectomy have been reported. The current study is aimed at evaluating podoplanin expression in PA as a myoepithelial (ME) cell marker and its role in estimating the prognosis and outcome of the tumors by correlating it with various clinical and histological parameters. Materials and Methods: A total of 10 paraffin-embedded specimens of histologically diagnosed PA with clinical records of the patients were retrieved and the slides were then stained using hematoxylin and eosin staining and were then immunohistochemically stained with podoplanin. Results: The study revealed the specificity of podoplanin as a differential marker for ME cells. Conclusion: Although the study revealed the specificity of podoplanin as a differential marker for ME cells, additional markers to overcome the limitations of podoplanin which will predict the biologic behavior (biologic behavior of the tumor) the tumor with respect to parameters such as age, gender and site of the tumor are required. Furthermore, a larger sample size is required to validate the findings in our data.

Abstract:Pleomorphic adenoma (PA) is the most common neoplasm of the salivary gland, presenting with a variety of histological features. In some cases, PA can undergo malignant transformation to carcinoma ex pleomorphic adenoma (CXPA). The transition from PA to CXPA is associated with complex molecular alterations, particularly involving the pleomorphic adenoma gene 1 (PLAG1) and high mobility group protein gene (HMGA2). This review investigates the molecular alterations of PLAG1 and HMGA2 in all domains in the malignant transformation of PA. Our analysis highlights that these markers are key alterations in the etiopathogenesis of PA and CXPA, with gene fusion and amplification being frequently reported mechanisms. Although the exact role of PLAG1 and HMGA2 in the oncogenic process remains unclear, further studies on the HMGA2 and PLAG1, are needed particularly in HMGA2-PLAG1-IGF2 which is proving to be a potential pathway for the development of clinically applicable therapies, especially for CXPA management.

Abstract:Salivary gland tumors account for approximately 2-3% of head and neck tumors. Among these, pleomorphic adenoma (PA) is the most common benign lesion. We present the case of a 60-year-old woman who visited a stomatology service with a submucosal nodule in the left buccal mucosa. The nodule had a normochromic smooth surface, was mobile, had a rubbery consistency, was painless to manipulation, and measured approximately 1 cm in its largest dimensions. The patient reported that the nodule had been present for 8 months, and no prior treatment had been administered.

Abstract:Introduction: Currently, the diagnosis of salivary gland tumors using imaging techniques is unreliable. Methods: In this monocentric retrospective study, we examined patients who received a 68Ga-DOTATOC PET/CT and subsequently underwent a salivary gland tumor resection between 1 January 2010 and 31 December 2021. PET/CT image assessment was compared with somatostatin receptor (SSTR) expression and histology. Results: Thirteen patients (five pleomorphic adenoma (PA) and eight other parotid lesions (OPL)) received a 68Ga-DOTATOC PET/CT. Imaging displayed strong focal tracer uptake in all PA except for one with strong tumor to background discrimination. PA revealed higher SUVmax, SUVmean, liver and blood pool quotients than those of Warthin tumors (WT) and of OPL. In comparison to the contralateral parotid, SUVmax (p = 0.02), SUVmean (p = 0.02), liver quotient (p = 0.03) and blood pool quotient (p = 0.03) were all significantly higher. In contrast, WT and OPL showed in relation to the contralateral parotid no significant differences of SUVmax (WT p = 0.79; OPL p = 0.11), SUVmean (WT p = 1.0; OPL p = 0.08), liver quotient (WT p = 0.5; OPL p = 0.08) and blood pool quotient (WT p = 0.8; OPL p = 0.19). Two PA and one granuloma were not available for examination. In the immunohistochemal analysis, all PA demonstrated the highest intensity of SSTR2 expression (grade 3). Furthermore, PA had a high percentage of cells expressing SSTR2 (20%, 80% and 55%). Conclusions: A strong tracer uptake in PA was shown in 68Ga-DOTATOC PET/CT. This may allow physicians to utilize radioligated somatostatin analogue PET CT/MR imaging to accurately diagnose PA. Additionally, it may be possible in the future to treat the PA with a noninvasive peptide receptor radionuclide therapy or with somatostatin analogues.
