Abstract:Abstract Depression is very common after stroke, causing multiple sequelae. We aimed to explore the efficacy of escitalopram for poststroke depression (PSD). PubMed, Embase, Scopus, Cochrane Central Register of Controlled Trials, Clinical trials. gov, Wan fang Data (Chinese), VIP (Chinese) and CNKI (Chinese) were retrieved from inception to May 2021. We recruited Randomized Controlled Trials (RCTs) which met the inclusion criteria in our study. The depression rating scores, the incidence of PSD, adverse events as well as functional outcomes were analyzed. 11 studies and 1374 participants were recruited in our work. The results were depicted: the reduction of depression rating scores was significant in the escitalopram groups and the standard mean difference (SMD) was − 1.25 ( P < 0.001), 95% confidence interval (95% CI), − 1.82 to − 0.68; the risk ratio (RR) of the incidence of PSD was 0.52 (95% CI, 0.29 to 0.91; P = 0.007 < 0.05), which was significantly lower in the escitalopram groups; Escitalopram is safe for stroke patients; there was improvement of the motor function. However, in sensitivity analyses, the conclusions of the motor function and the incidence of drowsiness were altered. The study suggests that escitalopram has a potentially effective role compared with control groups and demonstrates escitalopram is safe. However, the results of the motor function and the incidence of drowsiness should be considered carefully and remain to be discussed in the future.

Abstract:Post-stroke depression (PSD) is a common complication after stroke. PSD is associated with emotional disorders and psychological dependence, which are potential risk factors for stroke recurrence and suicidality. This study aimed to perform an umbrella review of therapies for PSD through a comprehensive literature search. A systematic search was conducted in the PubMed and Web of Science by two independent authors. We examined the Hamilton Depression Scale (HAMD), Activities of daily living (ADL), Neurologic function as efficacy endpoints, and the incidence of adverse events as safety profiles. Seventeen eligible studies, including 267 clinical trials were included in this study. The results showed that High-Frequency Repetitive Transcranial Magnetic Stimulation (HfrTMS), Acupuncture/EA+conventional treatment, Escitalopram, Modified Sini San, Moxibustion, Xiaoyao Formula, Paroxetine, Chinese herbal medicine, Exercise, Citalopram, and Cognitive behavioral therapy are beneficial for improving the depression symptoms of patients with PSD. HfrTMS and Sertraline may have an impact on slowing the scores of activities of daily living or neurologic function. In addition, Acupuncture/EA+conventional, Escitalopram, Citalopram, Sertraline, and Fluoxetine showed no serious adverse events in PSD patients. Our study demonstrated that 11 treatment methods can effectively improve the condition of PSD patients.

Abstract:Background and hypothesis: Post-stroke emotional symptoms are common and adversely influence the patients’ outcome. We aim to evaluate the efficacy of escitalopram in patients with acute stroke for the prevention of various emotional disorders, and for improvement of neurologic, cognitive function and patient quality of life. Method: In this multicenter, placebo-controlled, double-blind trial, 405 stroke patients (within 21 days after onset) were randomized either to the escitalopram (n = 217) (5 mg -10mg) or placebo (n = 188) for 3 months. Depressive symptoms (Montgomery-Asberg Depression Rating Scale [MADRS] and mini-international neuropsychiatric interview [MINI]), emotional incontinence (Kim’s criteria), anger proneness (Modified Spielberger trait), cognition (MOCA test), Modified Rankin scale (MRS), and quality of life (Stroke Specific QOL) were assessed initially, and at 3 and 6-months. Results: Baseline characteristics were similar between the groups. Proportions of patients with MADRS≥16, the primary outcome, were not different at 3-month (placebo vs. escitalopram group, 13.8% vs. 12.0%, p=0.579). However, new development of depression determined by MINI was lower in the escitalopram group (0 vs. 4 patients, P = 0.046). For the secondary outcomes, anger proneness score was lower in the escitalopram group (21.5 vs 20.1 at 3-month, P = 0.011). There were no significant differences in other items at 3-months: emotional incontinence (% of patients, 8.5% vs. 10.2%, p=0.565), MOCA score (21.1±6.8 vs. 20.9±7.2, p=0.704), MRS (3.1±1.2 vs. 3.0±1.3, p=0.822), and patient quality of life (165.0±38.8 vs. 166.7±39.1, p=0.667). When chronological changes of MADRS were analyzed after categorizing patients into normal (0-6), mild (7-15), and moderate/severe (≥16) depression groups, escitalopram significantly improved MADRS of patients with mild depression (10.30 to 7.23 vs. 10.91 to 5.06, p = 0.036). Conclusion: Escitalopram was not effective in the prevention of post-stroke depressive symptoms, and did not improve cognitive function, neurologic severity, and patient quality of life. However, it prevented development of depression defined by MINI, anger-proneness and seems to improve depressive symptoms in patients with mild depression.

Abstract:Background Poststroke depression is closely related to increased mortality in stroke patients. Compared with antidepressants, electroacupuncture (EA) treatment for poststroke depression (PSD) has relatively more stable effectiveness and can reduce side effects. This trial is designed to provide solid evidence for the efficacy and safety of EA treatment for patients with PSD. Methods/design This ongoing study is a single-blind, single-center, parallel group, randomized controlled trial. Sixty-two participants will be recruited from Shanghai Shuguang Hospital and randomized into either the EA group or the sham EA group. Baihui, Sishencong, Ganshu, Sanyinjiao, and Taichong are selected as the treatment acupoints in both groups. The EA group will receive the traditional EA treatment with de-qi sensation, and the sham EA group will receive sham EA treatment without needle penetration and electrostimulation. Participants will receive treatment 3 times per week for a total of 12 sessions over 4 weeks. The primary outcome is Hamilton Rating Scale for Depression score, and the secondary outcomes are scores on the Zung Self-Rating Depression Scale, National Institutes of Health Stroke Scale, Barthel Index of Activities of Daily Living, and Depression Scale of traditional Chinese medicine. All of the outcome measures will be assessed at baseline, 2 weeks after EA treatment onset, 4 weeks after treatment onset, and at 8-week follow-up. Safety assessments will be done at each visit. Discussion The results of this trial will demonstrate the efficacy and safety of EA treatment for PSD with credible and important clinical evidence, thus supporting EA treatment as an ideal choice for PSD treatment. Abstract:INTRODUCTION: Post-stroke depression (PSD) is a common and serious complication after stroke, occurring in nearly one third of stroke survivors, and affecting mortality rate, functional outcome, rehabilitation results and quality of life. However, in the common clinical practice only a minority of patients are properly treated. A relatively small number of scientific reports are available on clinical usefulness and safety of antidepressants (ADs) in PSD. Areas covered: This report provides an updated review about pharmacological state of art of PSD, including efficacy and safety of different drugs and their role on prevention, treatment and functional outcome. Expert opinion: Even if currently an antidepressant treatment can improve depressive symptoms, neither the optimal drug nor the optimal lengths of treatment, have been identified. Serotonergic drugs are preferable because of their better safety profile, but in the recent years there has been an important debate on possible association between selective serotonin reuptake inhibitor use and increased mortality. Another issue is the potential role of ADs for improving functional recovery. Newer ADs have interesting properties, in particular vortioxetine, due to its properties of enhancing cognitive functions, but further research is needed to clarify its/their role in treatment of PSD.

Clinical Research of the Elderly Patients with Post-Stroke Depression

Abstract:Objective:To investigate the clinical feature and effect with the therapy of citalopram in elderly patients with post-stroke depression(PSD).Methods:230 elderly patients above 65 with first acute stroke were evaluated 15 days after stroke by activity of daily living(ADL),hamilton depressive scale(HAMD)and mini-mental state examination(MMSE).The patients with PSD were treated with citalopram.Follow-up was 3 months,and the patients were compared with the control group.Results:90 patients with PSD(39.1%)were followed up.PSD had no correlation with the type of stroke.There was a tendency of correlation between lobe of brain,left hemisphere and PSD.The comparation of ADL and MMSE score between PSD group and no PSD group had significant difference.After the therapy of citalopram,ADL and MMSE score improved.Conclusion:The percentage of PSD in elderly stroke patients was 39.5%.PSD can affect the recognition.Citalopram has good effect in treating PSD.

Abstract:The International Journal of Psychiatry in Medicine, Ahead of Print. ObjectiveThe prevalence of post-stroke anxiety (PSA) is reported to be 20%–25%. There is insufficient evidence on the efficacy of antidepressants for treating anxiety in such patients. This Prospective Randomized Open Blinded Endpoint (PROBE) study was designed to assess the safety and efficacy of fluoxetine in PSA.MethodsIn this single-center pilot study conducted in India, post-stroke patients (between 1 to 6 months) were randomized to fluoxetine (intervention group: 20 mg/day for 12 weeks) or standard medical care (control group). The primary outcome was improvement in the Hamilton Anxiety Rating Scale (HAM-A) at 12 weeks. The secondary outcomes were anxiety remission (>50% improvement in HAM-A), modified Rankin Scale (mRS), Barthel Index (BI), quality of life (SF-36), and Hamilton Depression Rating Scale (HAM-D). A linear regression analysis was done for determinants of HAM-A to account for baseline differences in the intervention and control groups.ResultsA total of 60 patients were randomized: (30: intervention, 30: control). The overall prevalence of post-stroke anxiety among participants in the study was 50.8%, and 31.5% experienced both anxiety and depression. The average HAM-A score at baseline was 11, and average follow-up score at study conclusion was 4. There was similar improvement in the HAM-A score at 12 weeks post-randomization in the intervention and control groups [fluoxetine: −8.0 (95% CI = −11.0 to −4.0); control: −7.0 (95% CI = −9.5 to −4.0); P = 0.91]. Likewise, there was no significant difference between intervention and control groups at 12 weeks post-randomization on the mRS, BI, SF-36, or HAM-D. There were no serious adverse events in either group during the study.ConclusionFluoxetine and standard medical care had comparable improvement in HAM-A in post-stroke patients with mild anxiety at 12 weeks. Further study of the pharmacological treatment of post-stroke patients with more severe anxiety is needed.Clinical trial registrationCTRI/2018/12/016568.

Escitalopram and sertraline in treatment of depression with anxiety

Abstract:Objective:To evaluate the efficacy and safety of escitalopram and sertraline in the treatment of depression with anxiety.Method:76 of patients with depression with anxiety were randomly assigned to receive escitalopram(n=38) or sertraline(n=38) for up to 6 weeks in this randomized,parallel-group study.The rapeutic efficacy was assessed with Hamilton depression scale(HAMD-17) and Hamilton anxiety scale(HAMA).Safety profile was assessed with treatment emergent symptom scale(TESS).Results:The mean values of HAMD-17 and HAMA total scores improved significantly for both groups from the baseline to the end of week 6th(P<0.01).At the end of the first week,the total scores of HAMD and HAMA in the group of escitalopram were significant lower than that of group of sertraline(t=-2.839,-2.862;P<0.01),but there were no statistically significant differences between two groups at the end of week 2,4 and 6th(P>0.05).The incidences of side effects were 39.5% in escitalopram group and 42.1% in sertraline group(P>0.05).Conclusion:The therapeutic effect and side effect on depression with anxiety are not different between escitalopram and sertraline.Escitalopram has earlier response than sertraline.
Efficacy and safety of fluoxetine for the treatment of post-stroke depression:a meta-analysis

Abstract:Although depression may affect patients' recovery and even their survival rate after stroke,it is often overlooked or inadequately managed.And data regarding the treatment efficacy and safety of fluoxetine in this setting are inconsistent.We aimed to systematically assess those two indices in patients with post-stroke depression (PSD).Through a systematic literature search in 10 biomedical databases,244 articles were first identified,through which we collected and evaluated a total of 600 patients identified from 11 RCTs.The meta-analysis with Revman software indicated that fluoxetine was more effective than placebo,the combined results of 9 RCTs showed that fluoxetine decreased the depression rating scale scores significantly compared with placebo,and the pooled weighted mean difference (WMD) was 0.44 (95% CI-0.03 to 0.92).Moreover,time-dependent effects were also observed.No consistent evidence was found for its positive effects on the recovery of neurological impairments and improvements in activities of daily living (ADL).Six studies reported rate of adverse effects in both fluoxetine groups and control groups and showed no significant difference between them (OR=0.03,95% CI-0.00 to 0.07).This meta-analysis suggested that fluoxetine may be effective and safe for patients with PSD and it may have time-dependent effects.
Electroacupuncture versus Escitalopram for mild to moderate Post-Stroke Depression: A randomized non-inferiority trial

Abstract:Objective: To explore the efficacy of electroacupuncture in treating post-stroke depression (PSD) by modulating the inflammatory response pathway. Methods: One hundred and fifty participants with mild or moderate PSD were randomly divided into 75 cases each in the electroacupuncture group (EA group) and escitalopram group (ESC group). In the EA group, 30 sessions of electroacupuncture were performed on the Baihui (GV 20), Yintang (GV 29), and the ipsilateral Taichong (LR 3) and Hegu (LI 4), simultaneous oral placebo for 40 days. The ESC group received oral escitalopram oxalate tablets 10mg to 20mg for 40 days, plus 30 sessions of sham electroacupuncture. The effectiveness of the treatment was evaluated by the Hamilton Depression Scale (HAMD-17), Self-Depression Scale (SDS), Modified Barthel Index Score (MBI), and the serum levels of IL-1β, IL-6, IL-10, TNF-α, and INF-γ. Results: There was no statistically significant difference in the baseline data, HAMD-17, SDS, MBI scores, and serum IL-1β, IL-6, IL-10, TNF-α, and INF-γ levels between the two groups of participants before the intervention ( P > 0.05). After treatment, HAMD-17 and SDS scores continued to decrease and MBI scores continued to increase in both groups. The differences were statistically significant at the 6th week and baseline, the 10th week and baseline, and the 10th week and the 6th week (all P < 0.001). The differences in HAMD-17, SDS, and MBI scores between the two groups at the 6th week were not statistically significant ( P =0.110, 0.115, 0.516, respectively); HAMD-17 scores and SDS scores in the EA group were lower than those in the ESC group at the 10th week, and the differences were statistically significant ( P =0.002,0.026, respectively). In the 6th week, the serum levels of pro-inflammatory factors such as IL-1β, IL-6, TNF-α, and INF-γ were significantly lower in both groups compared with the baseline, while the level of anti-inflammatory factor IL-10 was significantly higher. The difference between the pre-and post-intervention intra-group comparisons was statistically significant ( P < 0.001), and the difference between the inter-group comparisons was not statistically significant ( P > 0.05). No serious adverse events occurred throughout the trial. Both therapies could safely and effectively improve HAMD-17, SDS, and MBI scores and modulate neuroinflammatory responses in PSD participants. After the treatment was stopped, some parameters were better in the EA group than the ESC group in a short time. Conclusion: Electroacupuncture is an effective, alternative to escitalopram for the treatment of mild-to-moderate PSD. Abstract:Objective To evaluate the efficacy and safety of escitalopram in the treatment of senile depression.Methods A total of 90 patients with depression were randomly divided into escitalopram and sertraline and venlafaxine groups,30 cases in each group,respectively.The duration of the treatment was 8 weeks with escitalopram and sertraline and venlafaxine.The efficacy of drugs were evaluated by Hamilton Depression Rating Scale(HAMD) before and after treatment in 1,2,4,6 and 8 weeks.The adverse drug reactions(ADR) were evaluated by treatment emergent symptom(TESS).Results The score of HAMD were decreased significantly at the time 1,2,4,6,8 weeks after treatment in the three groups,in order to sort: escitalopram > venlafaxine > sertraline(P < 0.01).The ADR in escitalopram and sertraline groups were less than the venlafaxine group.But compared the ADR in the escitalopram and sertraline groups at the time 1,6 weeks after treatment,the difference was not significantly.The confidant of ADR in escitalopram was more less than in venlafaxine(P ﹤ 0.01).Conclusion The curative effect of escitalopram in the treatment of senile depression had a good compliance,advantages of quick response,fewer adverse reactions.
Escitalopram Vs Citalopram: a Parallel Controlled Study for Depression

Abstract:Objective To evaluate the efficacy and safety of escitalopram in treatment of depression.MethodsA randomized,double-blind,active drug controlled clinical trial was conducted.One hundred twenty-six patients met with CCMD-3 criteria of depression received 6 weeks treatment with escitalopram(1-2 weeks with oral 10 mg,3-6 weeks with flexible dosages 10-20 mg,n=63) or citalopram(1-2 weeks with oral 20mg,3-6 weeks with flexible dosages 20-40 mg,n=63).The primary clinical efficacy was evaluated with the change in total score of 17-items Hamilton Depression Rating Scale(HAMD-17),HAMA,GCI,and the safety with the reports of adverse events,the result of laboratory or electrocardiography examination,etc.ResultsTotal of 106 patients completed the study,52 in escitalopram group and 54 in citalopram group,with total response rates(82.7% vs 88.9%,P=0.484)and remission rates(36.5% vs 27.8%,P=0.36).There were no significant differences between the two groups(P0.05) in total response rates,remission rates and the score of the HAMA,GCI at all time points.The incidence of adverse events and drug-related adverse event were well tolerated,with no significant difference in the prevalence between the two groups(17.5% vs 30.2%,P=0.07).ConclusionEscitalopram has similar therapeutic effect to citalopram,and rapid onset,better tolerability.
Prophylactic Effects of Duloxetine on Post-Stroke Depression Symptoms: an Open Single-Blind Trial

Abstract:Post-stroke depression (PSD) is a common yet severe sequela of stroke, and is often accompanied with somatic symptoms. Duloxetine, a new serotonin-norepinephrine reuptake inhibitor, may help to prevent depression after stroke. 95 ischemic stroke patients without depression were randomly divided into two groups: duloxetine group (n = 47) and control group (n = 48). Patients in the control group received routine ischemic stroke therapy, whereas patients in the duloxetine group received duloxetine (dose range 30-90 mg) for 12 weeks in addition to routine therapy. Follow-up observations lasted for 24 weeks. The Hamilton Depression Scale was used to measure depression, and the National Institute of Stroke Scale, Mini-Mental State Examination, Activities of Daily Living Scale (Chinese version) and Short Form 36 Health Survey Questionnaire were used to assess neurological function, cognitive function, rehabilitation from stroke and quality of life. Results showed that in general, duloxetine spared ischemic stroke patients from both minor and major depression by 16%. In addition, duloxetine helped patients to rehabilitate more rapidly from stroke, and was associated with better cognitive function and quality of life. In conclusion, the prophylactic use of duloxetine not only decreased the incidence of PSD, but also promoted rehabilitation, cognitive function and quality of life.

Abstract:Purpose: To determine the effects of combined use of cognitive behavioral therapy (CBT) and paroxetine hydrochloride tablets in patients with post-stroke depression (PSD), and its effect on scores on Hamilton Rating Scale for Depression (HAMD) and Stroke Specific Quality of Life Scale (SS-QOL). Methods: Clinical data for 96 patients with PSD who were treated in Dongying Traditional Chinese Hospital, Dongying City, China from June 2018 to June 2019 were retrospectively analyzed. Patients who met the inclusion criteria were divided into treatment group (TG, n = 48) and reference group (RG, n = 48) based on odd and even hospitalization numbers. Both groups received conventional treatment, but RG patients were in addition given clopidogrel, while TG received CBT in combination with paroxetine hydrochloride tablets. Clinical indices were evaluated in both groups before and after treatment. Moreover, therapeutic effects in the two different treatment methods on PSD, as well as on Hamilton Rating Scale for Depression (HAMD) and Stroke Specific Quality of Life Scale (SS-QOL) scores were analyzed. Results: After treatment, TG had lower HAMD score (p < 0.001), lower scores on modified Rankin scale, and few incidences of adverse reactions at 3, 7, 15 and 30 days of treatment (p < 0.05), but higher total clinical effectiveness and mean SS-QOL score (p < 0.05), when compared with RG. Conclusion: Combined use of CBT and oral paroxetine hydrochloride tablets may be a promising strategy for treating depression and enhancing the quality of life of PSD patients, as it greatly improves neurological deficit and prognosis. However, further clinical trials should be carried out prior to introducing it in clinical practice.

Effect of transcranial magnetic stimulation in combination with citalopram on patients with post-stroke depression

Abstract:Background: Amelioration of depression in patients with post-stroke depression (PSD) remains challenging. Objective: The primary vision was to explore the effect of transcranial magnetic stimulation (TMS) in combination with citalopram on patients with PSD. Methods: One hundred eligible patients who were diagnosed with PSD were recruited and randomly assigned to the control group ( n = 50) or the TMS group ( n = 50). The controls were given citalopram (10 mg/d for consecutive 8 weeks), while, in addition to citalopram, patients in the TMS group were also given TMS at 5 Hz once a workday for 8 weeks. The primary outcome was patient depression status as reflected by 17-item Hamilton Rating Scale for Depression (HAMD-17) score, and the secondary outcome was patient neuropsychological score determined by Mini-Mental State Examination (MMSE) and Wisconsin Card Sorting Test (WCST). Results: Patients treated with TMS in combination with citalopram had a drastic decrease in HAMD-17 score during treatment. Bigger changes in HAMD-17 score between baseline and 2 weeks as well as between baseline and 8 weeks in the TMS group were observed ( P < 0.01). Patients in both groups had increased MMSE scores after treatment. Data of WCST revealed patients with TMS treatment completed more categories ( P < 0.01) and had a lower RPP in comparison to patients in the control group ( P < 0.0001). Additionally, TMS in combination with citalopram strikingly improved patients' MMSE scores when compared with those taking citalopram alone. Last, there was no striking difference in side effects between the two groups ( P > 0.05). Conclusion: Our study found TMS in combination with citalopram is conducive to improving depression status and neuropsychological function, which holds great promise for treating PSD.

Abstract:Context: Post stroke depression (PSD) is an under diagnosed morbidity of stroke and can negatively affect the prognosis of the patient. Aims: We intended to study the prevalence of PSD and the commonly used anti-depressants and their outcome in patients with PSD. Settings and Design: A prospective observational study was conducted in the patients admitted to the stroke unit of a tertiary care centre. Methods and Materials: Diagnosis of post stroke depression was made by the Hamilton Depression Rating Scale (HDRS) during the two-week period after stroke or in the clinic follow up. A comparison of clinical outcome and adverse events of the two anti-depressants used, i.e. venlafaxine and fluoxetine were done by a follow up of up to 6 months. Statistical Analysis Used: Independent sample test was used for statistical purposes in the study Results: Out of the 326 stroke patients admitted in the department, 73 had PSD and 60 patients out of this were assigned into the study. Forty patients were males, and the mean age of the sample population was found to be 62.13 ± 11.14. Major risk factors identified were hypertension, diabetes mellitus, and dyslipidemia. Venlafaxine showed better outcome and less adverse events compared to fluoxetine. Major adverse events observed were hyponatremia, headache, insomnia, and anxiety. Conclusions: PSD in the early phase affects a substantial number of the stroke patients. Venlafaxine has got a better outcome and adverse event profile compared to fluoxetine in this group of patients. However, larger multicenter studies will provide more helpful data in this area.

Abstract:Stroke is a common cerebrovascular disease with a high disability and case fatality rate, ranking as the second leading cause of death worldwide. With the acceleration of aging, the incidence rate of stroke is rising. [ 1 ] Poststroke depression (PSD) is one of the most common complications after stroke. About one-third of patients will have PSD in different periods after stroke, mainly manifested as depression, anxiety, sleep disorders, loss of interest, memory loss, difficulty concentrating, and other symptoms. [ 2 ] PSD hinders patients' rehabilitation process and functional recovery, diminishes their quality of life, and increases their disability and mortality rates. [ 3 ] Currently, the primary treatment method for PSD are antidepressants, but they have a slow onset. Some patients may not experience immediate mood improvement during the early stage of drugs, resulting in decreased treatment compliance, recurrent illness, and the delay of the best period of rehabilitation treatment. [ 4 ] Studies have confirmed that rational emotive behavior therapy (REBT) can improve the mood and sleep of PSD patients, but the treatment is mainly based on group therapy, which is difficult to achieve individualized precise treatment, and the cost is high, which is not conducive to the later family rehabilitation. In this study, based on the first-line antidepressant escitalopram, the method of combining traditional Chinese and Western medicine was used to combine REBT and traditional Baduanjin training to observe its effect on sleep, mood and serum BDNF, 5-HT and IL-6 levels in PSD patients. In order to develop a more individualized, reasonable, safe, and effective treatment plan for PSD patients, and provide clinical basis for the short-term and long-term prognosis of PSD patients.

Efficacy and safety of agomelatine versus SSRIs/SNRIs for post-stroke depression: a systematic review and meta-analysis of randomized controlled trials

Abstract:Agomelatine is effective in the treatment of depression, but its effect for post-stroke depression (PSD) remains unclear. This study was conducted to compare the efficacy and safety of agomelatine versus SSRIs/SNRIs in treating PSD. We systematically searched Embase, PubMed, Cochrane Library, WanFang Data, China National Knowledge Infrastructure, and Cqvip databases for double-blind randomized controlled studies comparing the efficacy and safety of agomelatine versus SSRIs/SNRIs for PSD until December 2022. The primary efficacy endpoint was the Hamilton Depression Rating Scale (HAMD) score, and the primary safety endpoint was the incidence of overall adverse reactions. Nine studies comprising 857 patients with PSD were included. After 6-12 weeks of treatment, the HAMD score ( P = 0.16) and the overall response rates ( P = 0.20) in the agomelatine group were comparable to that in the SSRIs/SNRIs group. Participants treated with agomelatine achieved higher Barthel Index scores compared with the SSRIs/SNRIs group ( P = 0.02). There was a significantly lower incidence of overall adverse reactions ( P = 0.008) and neurological adverse reactions ( P < 0.0001) in the agomelatine group. The efficacy of agomelatine for treating PSD is probably comparable to that of SSRIs/SNRIs, and it may improve stroke outcomes with better safety.
Effect of Sertraline on Cardiovascular Events and Restenosis after Coronary Stenting in Depression Patients with Acute Coronary Syndrome

Abstract:Objective To investigate the clinical effects of sertraline on cardiovascular events and restenosis after coronary stenting in depression patients with acute coronary syndrome.Methods Depression patients with acute coronary syndromes reffered for coronary stenting were randomly divided into two groups,in which one group received oral sertraline and base therapy(therapy group),and the other received base therapy only (control group). The cardiovascular events(angina pectoris, myocardia infarction, congestive heart failure and death) and stenting restenosis were carefully observed for 1 year. Results (1)Baseline patametres had no statistic difference between two groups(P0.05). (2)Cardiovascular events after coronary stenting were significantly lower in therapy group than control group (13.7.8%vs 30.2%), but restenosis had no statistic difference between therapy group and control group (11.8% vs 17.0%). Conclusion Sertraline can decrease cardiovascular events after coronary stenting in depression patients with acute coronary syndrome but no difference on stenting restenosis.

Abstract:Detailed data on post-stroke depression (PSD) in older adults are limited in spite of the high vulnerability of this population to stroke. In fact, PSD prevalence in older adults ranges from 16.0 to 43.9%; however, timing and instruments of evaluation often differ significantly across all available studies. The etiology, genetic and inflammatory factors, as well as structural brain alterations, are claimed as part of a multifaceted mechanism of action in PSD onset. Thus, the aim of this narrative review was to further elaborate on the prevalence, etiology, diagnosis, consequences and treatment of PSD in older adults. The consequences of PSD in older adults may be devastating, including a poor functional outcome after rehabilitation and lower medication adherence. In addition, lower quality of life and reduced social participation, higher risk of new stroke, rehospitalization, and mortality have been reported. In this scenario, treating PSD represents a crucial step to prevent these complications. Both pharmacological and non-pharmacological therapies are currently available. The pharmacological treatment utilizes antidepressant drugs, such as selective serotonin reuptake inhibitors (SSRIs), serotonin-norepinephrine reuptake inhibitors (SNRIs), monoamine oxidase inhibitors (MAOIs), tricyclic antidepressants (TAs) and new multimodal antidepressants (NMAs). Non-pharmacological therapies include psychological interventions and non-invasive brain stimulation techniques, while excluding drug administration. In the general population experiencing PSD, SSRIs (sertraline in particular) are the most prescribed, whereas the combination of antidepressants and psychotherapy is underused. Furthermore, about one-third of patients do not receive treatment for PSD. In regard to older adults with PSD, the possibility of more adverse effects or contraindications to antidepressant prescription due to comorbidities may limit the therapeutic window. Although drugs such as citalopram, escitalopram, sertraline, venlafaxine, and vortioxetine are usually well tolerated by older patients with PSD, the few randomized controlled trials (RCTs) specifically considering older adults with PSD have been conducted with fluoxetine, fluvoxamine, reboxetine, citalopram and nortriptyline, often with very small patient samples. Furthermore, data regarding the results of non-pharmacological therapies are scarce. High-quality RCTs recruiting large samples of older adults are needed in order to better manage PSD in this population. In addition, adequate screening and diagnosis instruments, with reliable timing of evaluation, should be applied.
