ATTENUATION OF LOW AMBIENT TEMPERATURE-INDUCED ELEVATION OF BLOOD PRESSURE BY SPECIAL HYPERTENSIVE MEDICINES

Abstract:Objective: It is well documented that elevated blood pressure is associated with low ambient temperature, which is related with increase of sympathetic nervous system and renin-angiotensin system activities. In the current study, we aim to determine whether hypertension medicine, such as angiotensin converting enzyme inhibitor(ACEI), beta-blocking agent is effective in the prevention of cold-induced elevation of blood pressure and to shed light on underlying mechanism.Design and method: A stratified multistage random sampling method was used in The China Hypertension Survey study from 2012 to 2016, aimed to obtain nationally representative sample of the general Chinese population aged greater than 15 years in 31 provinces of mainland China, finally, a total of 466,670 was eligible and enrolled into our analysis. generalized linear model was applied to explore the estimates of ambient temperature on blood pressure after adjustment for potential confounders. Results: Among 46513 individuals taking hypertensive drugs, those taking beta-blocking agent, and medicine decreasing renin-angiotensin system activities were observed to mitigate the adverse effects of cold weather on blood pressure. Individuals taking beta-blocking agent was showed to have lower effect of cold weather on blood pressure, with 3.4 mmHg increase in SBP with per 10°C decrease in ambient temperature, while, those not taking beta-blocking agent had a average 5.7mmHg increment in SBP with per 10°C decrease in ambient temperature. Furthermore, individuals taking ACEI or angiotensin receptor blocker(ARBs) could completely counteract the adverse effect of cold weather on blood pressure. Hypertensive patients taking ACEI and ARB were found to had a average 1.4 and 1.5 mmHg increase in SBP and DBP, respectively, which did not have statistical significance. Conclusions: Hypertensive medicines such as ACEI, ARB and beta-blocking agent attenuated adverse effect of blood sympathetic nervous system and renin-angiotensin system activities. With the widely awareness of extreme weather on human health, it may provide a novel strategy for low temperature-induced blood pressure, aimed to decrease the prevalence of hypertension in cold weather.
