Abstract:Introduction and Aims: Choline-metabolizing genetic variation may interact with choline intake on fetal programming and pregnancy outcome. This case-control study aims to explore the association of maternal choline consumption and phosphatidylethanolamine N-methyltransferase (PEMT) gene polymorphism rs7946 with preterm birth risk. Methods: 145 Han Chinese women with preterm delivery and 157 Han Chinese women with term delivery were recruited in Shanghai. Dietary choline intake during pregnancy was assessed using a validated food frequency questionnaire. Additionally, DNA samples were genotyped for PEMT rs7946 (G5465A) with plasma homocysteine (Hcy) levels measured. Results: Compared with the lowest quartile of choline intake, women within the highest consumption quartile had adjusted odds ratio (aOR) for preterm birth of 0.48 (95% confidence interval, CI [0.24, 0.95]). There was a significant interaction between maternal choline intake and PEMT rs7946 (p for interaction = 0.04), where the AA genotype carriers who consumed the energy-adjusted choline <255.01 mg/day had aOR for preterm birth of 3.75 (95% CI [1.24, 11.35]), compared to those with GG genotype and choline intake >255.01 mg/day during pregnancy. Additionally, the greatest elevated plasma Hcy was found in the cases with AA genotype and choline consumption <255.01 mg/day (p < 0.001). Conclusion: The AA genotype of PEMT rs7946 may be associated with increased preterm birth in these Han Chinese women with low choline intake during pregnancy.

Abstract:Background Choline is an essential nutrient for fetal and neonatal development. Common single nucleotide polymorphisms (SNPs) may change requirements of choline supply in total parenteral nutrition (TPN) for preterms. Objectives This study was aimed to investigate whether total choline intake during pregnancy and/or SNPs influence choline metabolism and the growth in preterms receiving TPN therapy. Methods A total of 90‐paired Chinese parturients with premature delivery and their preterm infants were enrolled at Xinhua Hospital Affiliated to Shanghai Jiaotong University School of Medicine. The inclusion criteria for preterms were Asian singleton fetus with no other diseases, gestational age < 37 weeks, 1600g ≤ birth weight ≤ 2100g and administration of TPN for ≥7 days. 90‐paired healthy Chinese parturients and their full‐term newborns were also enrolled as controls. Dietary choline intake during pregnancy was assessed by food frequency questionnaires and 24 h dietary recalls. Plasma choline levels in peripheral venous blood from all parturients and preterms before or after TPN were measured with SNPs of PEMT rs12325817(G‐744C) and CHDH rs12676 (G432T) genotyped in all parturients and neonates. X 2 test, a linear regression model and a multivariable logistic regression were performed for statistical analysis. Results Dietary choline intakes of women with preterm delivery were lower than those in controls during pregnancy ( P < 0.05); Plasma choline levels in peripheral vein from preterm parturients were also lower than those in control parturients ( P < 0.05); Among the parturients with choline intake within lowest quartile level during pregnancy, PEMT ‐774 C > G was found to be related to preterm risk (OR 2.51, 95% CI 1.56 – 3.79, P < 0.05). Plasma choline levels in peripheral vein of preterms after TPN for 7 days were decreased relative to those before TPN ( P < 0.05); Compared with PEMT‐744GG and GC genotypes, the maternal variant CC genotype enhanced the risk of lower plasma choline concentrations in peripheral vein of their preterms after TPN for 7 days ( P < 0.05). Moreover, the CHDH 432T allele was associated with a higher risk of reduced plasma choline levels in preterms after TPN for 7 days (OR 2.63, 95% CI 1.32–5.29, P < 0.05), more days to regain the birth weight (OR 1.43, 95% CI, 1.28–2.69, P < 0.05), and lower weight gain during hospital stay (OR 1.78, 95% CI, 1.16–3.39, P < 0.05), compared to the G allele. Conclusions Inadequate intake of choline during pregnancy and PEMT‐744CC genotype in gestational Chinese Asian women could enhance the risk of premature births. TPN may promote the decrement of choline in the Chinese preterms. The PEMT‐744CC genotype in the parturients with premature delivery and the CHDH 432T allele may further increase susceptibility of choline decline and inhibit the growth in preterms on TPN feeding. These preterms may need more choline for growth and development after birth. Maternal Choline Intake Modulates Maternal and Fetal Biomarkers of Choline Metabolism in Humans

Abstract:Background: In 1998 choline Adequate Intakes of 425 and 450 mg/d were established for nonpregnant and pregnant women, respectively. However, to our knowledge, no dose-response studies have been conducted to evaluate the effects of pregnancy or maternal choline intake on biomarkers of choline metabolism.Objective: We sought to quantify the effects of pregnancy and maternal choline intake on maternal and fetal indicators of choline metabolism.Design: Healthy pregnant (n = 26; 27 wk gestation) and nonpregnant (n = 21) women were randomly assigned to receive 480 or 930 mg choline/d for 12 wk. Fasting blood samples and placental tissue and umbilical cord venous blood were collected and analyzed for choline and its metabolites.Results: Regardless of the choline intake, pregnant women had higher circulating concentrations of choline (30%; P < 0.001) but lower concentrations of betaine, dimethylglycine, sarcosine, and methionine (13-55%; P < 0.001). Obligatory losses of urinary choline and betaine in pregnant women were similar to 2-4 times as high (P <= 0.02) as those in nonpregnant women. A higher choline intake yielded higher concentrations of choline, betaine, dimethylglycine, and sarcosine (12 46%; P <= 0.08) in both pregnant and nonpregnant women without affecting urinary choline excretion. The higher maternal choline intake also led to a doubling of dimethylglycine in cord plasma (P = 0.002).Conclusion: These data suggest that an increment of 25 mg choline/d to meet the demands of pregnancy is insufficient and show that a higher maternal choline intake increases the use of choline as a methyl donor in both maternal and fetal compartments. Abstract:The importance of ensuring adequate choline intakes during pregnancy is increasingly recognized. Choline is critical for a number of physiological processes during the prenatal period with roles in membrane biosynthesis and tissue expansion, neurotransmission and brain development, and methyl group donation and gene expression. Studies in animals and humans have shown that supplementing the maternal diet with additional choline improves several pregnancy outcomes and protects against certain neural and metabolic insults. Most pregnant women in the U.S. are not achieving choline intake recommendations of 450 mg/day and would likely benefit from boosting their choline intakes through dietary and/or supplemental approaches.

Abstract:Objective To investigate the association between choline intake during pregnancy and choline metabolism in parturients and infants,and to analyze the difference in plasma choline level between premature and healthy full-term infants at birth.Methods Neonates who were born in the Department of Obstetrics of Xinhua Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from September 2014 to May 2016 were enrolled.Demographic and clinical information of these neonates and their mothers were recorded.Dietary choline intake during pregnancy was determined on the second day after delivery using food frequency questionnaires.Free choline levels in plasma of all parturients and neonates were measured.Then,data were collected and analyzed by independent samples t-test,trend Chi-square test,Chi-square test and linear regression methods.Results (1) A total of 134 parturients and their neonates were enrolled in this study including 80 full-term infants and 54 premature infants.Compared with the full term group,the parturients in the premature group had higher pregestational BMI (22.7±3.2 vs 20.8±2.3,t=-3.720) and lesseducation background (trendx2=4.020) (both P＜0.05).(2) The food frequency questionnaires showed that 30.0％(24/80) of pregnant women in the full-term group reached adequate intake (AI) of choline during pregnancy,while only 11.1％ (6/54) in the premature group did.(3) Plasma choline level in maternal peripheral blood was positively associated with maternal dietary choline intake during pregnancy (r=0.882,r2=0.778,P＜0.001).Moreover,plasma choline level in the umbilical artery was closely related to maternal plasma choline level (r=0.965,r2=0.931,P＜0.001).(4) The dietary choline intake during pregnancy,maternal plasma choline level and plasma choline level in umbilical artery in premature infant group were all lower than those of full-term group [(305.24± 102.17) vs (386.16± 121.36) rmg/d,t=4.026;(5.23±0.79) vs (7.18± 1.51) μmol/L,t=9.783;(20.68±3.52) vs (27.96±3.94) μmol/L,t=10.930;all P＜0.01].Conclusions Insufficient dietary intake of choline during pregnancy is common.The dietary choline intake during pregnancy in parturients of preterm delivery group is lower than that in full-term group.Dietary choline intake during pregnancy is related to plasma choline level in umbilical artery.Therefore,dietary guidance should be emphasized for pregnant and lactating women to increase the intake of foods rich in choline.It is also important to provide premature infants with choline in time to meets the needs of their development.
Pregnancy and Choline Intake Alter the Metabolic Use of Orally Consumed Choline in Women Consuming Deuterium Labeled Choline

Abstract:A 12‐week choline intervention study was conducted to investigate the effects of pregnancy and choline intake on the fate of orally consumed choline. Healthy non‐pregnant (n = 21) and pregnant women (n=27, 27wk gestation) were randomized to controlled choline intakes of either 450 (choline AI) or 900 mg/day; 350 mg/day dietary choline and either 100 or 550 mg/d supplemental choline chloride. During the last six weeks of the study, ~20% of the total choline intake was provided as deuterium labeled choline (d9‐choline). The labeling of choline and its metabolites was examined at the end of the study (wk 12) in plasma and urine. Pregnancy affected the use of dietary choline with higher concentrations of plasma d9‐choline (P = 0.008), lower concentrations of plasma d6‐dimethylglycine (d6‐DMG) (P < 0.001), and higher urinary d9‐choline excretion (P = 0.001) in pregnant women. A higher choline intake (900 vs 450 mg/d) yielded higher plasma concentrations of d9‐choline (P = 0.003), d6‐DMG (P = 0.02), and d‐9 betaine (especially in pregnant women; P= 0.002 for choline x pregnancy) as well as greater urinary d6‐DMG excretion (P = 0.006). These data show that (i) pregnancy alters the use of orally consumed choline and (ii) a choline intake level exceeding the choline AI increases the availability of labile methyl groups for one‐carbon metabolic reactions especially in pregnant women.Supported by grants from the USDA, ENC, and NCBA.

Abstract:This study sought to systemically elucidate the effects of pregnancy and maternal choline intake on genomic markers. Healthy third trimester pregnant (n=26, wk 27 gestation) and non‐pregnant (n=21) women were randomized to controlled choline intakes of 480 or 930 mg/day for 12‐wk. Leukocyte samples were acquired at wk 0 and 12. Pregnant women had higher DNA damage compared to non‐pregnant women (P < 0.001), suggesting insufficient defense against oxidative stress. Genome‐wide transcript profiling revealed the up‐regulation of genes related to immune response, consistent with an activated host defense system towards the end of pregnancy. Pregnant women responded to the higher choline intake with increased (P < 0.05) histone 3 lysine 4 di‐methylation (H3K4me2), a marker of transcription activation. In sum, pregnancy altered markers of genomic stability and modulated the expression of immune‐related genes in leukocytes. Choline and Betaine Intakes During Pregnancy in Relation to Risk of Gestational Diabetes Mellitus among Chinese Women

Abstract:Previous animal studies found beneficial effects of choline and betaine on maternal glucose metabolism during pregnancy, but few human studies explored the association between choline or betaine intake and incident gestational diabetes mellitus (GDM). We aimed to explore the correlation of dietary choline or betaine intake with GDM risk among Chinese pregnant women. A total of 168 pregnant women with GDM cases and 375 healthy controls were enrolled at the Seventh People's Hospital in Shanghai during their GDM screening at 24-28 gestational weeks. A validated semi-quantitative FFQ was used to estimate choline and betaine consumption through face-to-face interviews. An unconditional logistic regression model was adopted to examine OR and 95 % CI. Compared with the controls, those women with GDM incidence were likely to have higher pre-pregnancy BMI, be older, have more parities and have higher plasma TAG and lower plasma HDL-cholesterol. No significant correlation was observed between the consumption of choline or betaine and incident GDM (adjusted OR (95 % CI), 077 (041, 143) for choline; 080 (042, 152) for betaine). However, there was a significant interaction between betaine intake and parity on the risk of GDM (Pfor interaction = 001). Among those women with no parity history, there was a significantly inverse correlation between betaine intake and GDM risk (adjusted OR (95 % CI), 025 (006, 081)). These findings indicated that higher dietary betaine intake during pregnancy might be considered a protective factor for GDM among Chinese women with no parity history.

Abstract:The nutritional status of the mother-to-be has a key impact on the proper development of the fetus. Although all nutrients are important for the developing baby, recent research indicates the importance of adequate choline intake during the periconceptional period, pregnancy, and lactation. Choline plays a key role in the biosynthesis of cell membranes, supporting liver function, neurotransmission, brain development, and DNA and histone methylation. Choline participates in the formation of a child's nervous system, supports its cognitive development, and reduces the risk of neural tube defects. The human body is incapable of producing sufficient choline to meet its needs; therefore, it must be obtained from the diet. Current data indicate that most women in their reproductive years do not achieve the recommended daily intake of choline. The presented narrative review indicates the importance of educating mothers-to-be and thereby increasing their awareness of the effects of choline on maternal and child health, which can lead to a more aware and healthy pregnancy and proper child development.

A Higher Maternal Choline Intake Favorably Alters Placental Gene Expression of Biological Pathways Related to Disease Risk

Abstract:A 12-week randomized controlled feeding study was conducted in third trimester pregnant women (gestational week 27, n=26) to investigate the effects of varied choline intake on placental gene expression. The participants consumed 480mg/d (approximating the choline Adequate Intake, AI) or 930mg/d choline during the study. RNA was extracted from placental tissue obtained at delivery and genome wide expression arrays were performed. The higher choline intake altered the expression of 208 genes (fold change>1.7, adjusted P value < 0.01) with down-regulation (P<0.01) of genes involved in stress response (e.g. heat shock protein 90, cAMP response element-binding protein, stress-associated endoplasmic reticulum protein 1) and inflammation (e.g. interleukin-2 receptor alpha, macrophage scavenger receptor, lysozyme). Notably, the placental anti-angiogenic factor soluble fms like tyrosine kinase 1 (sFLT-1) expression was also diminished (45%, P=0.004) and the rise in maternal circulating sFLT-1, a biomarker for preeclampsia, was attenuated (38%, P=0.011) in the 930mg/d group. Collectively, these data suggest that a choline intake higher than the current AI favorably alters placental gene expression of several biological pathways related to disease risk. Choline in pregnant women: a systematic review and meta-analysis

Abstract:Context: Choline is a critical nutrient. Inadequate choline intake during pregnancy increases the risk of adverse maternal and offspring health. Objective: A systematic review and meta-analysis were conducted to examine the current recommendations for choline intake by pregnant women, estimate the overall prevalence of pregnant women with adequate choline intake, and explore associations between maternal choline level and adverse pregnancy outcomes (APOs). Methods: Choline recommendations for pregnant women were assessed from eight nutrient guidelines of the United States, United Kingdom, Canada, Australia, Asia, International Federation of Gynecology and Obstetrics, and World Health Organization. Data on the prevalence of pregnant women with adequate choline intake and the association between maternal choline level and APOs were collected from 5 databases up to May 2023. Meta-analyses with random effects and subgroup analyses were performed for the pooled estimate of prevalence and association. Results: Five recent nutrition guidelines from the United States (United States Department of Agriculture), United States (Food and Drug Administration), Canada, Australia, and the International Federation of Gynecology and Obstetrics have emphasized the importance of adequate choline intake for pregnant women. Of 27 publications, 19 articles explored the prevalence and 8 articles explored the association. Meta-analysis of 12 prevalence studies revealed a concerning 11.24% (95% confidence interval, 6.34-17.26) prevalence of pregnant women with adequate choline intake recommendations. A meta-analysis of 6 studies indicated a significant association between high maternal choline levels and a reduced risk of developing APOs, with an odds ratio of 0.51 (95% confidence interval, 0.40-0.65). Conclusion: The existing guidelines highlight the importance of choline in supporting maternal health and fetal development during pregnancy. Furthermore, a high maternal choline level was likely to be associated with a lower risk of APOs. However, 88.76% of pregnant women do not achieve the optimal choline intake. Therefore, specific policies and actions may be necessary to improve choline intake in pregnant women's care and support the well-being of pregnant women. Abstract:BACKGROUND:Although biomarkers of choline metabolism are altered by pregnancy, little is known about the influence of human pregnancy on the dynamics of choline-related metabolic processes.OBJECTIVE:This study used stable isotope methodology to examine the effects of pregnancy on choline partitioning and the metabolic activity of choline-related pathways.DESIGN:Healthy third-trimester pregnant (n = 26; initially week 27 of gestation) and nonpregnant (n = 21) women consumed 22% of their total choline intake (480 or 930 mg/d) as methyl-d9-choline for the final 6 wk of a 12-wk feeding study.RESULTS:Plasma d9-betaine:d9-phosphatidylcholine (PC) was lower (P ≤ 0.04) in pregnant than in nonpregnant women, suggesting greater partitioning of choline into the cytidine diphosphate-choline (CDP-choline) PC biosynthetic pathway relative to betaine synthesis during pregnancy. Pregnant women also used more choline-derived methyl groups for PC synthesis via phosphatidylethanolamine N-methyltransferase (PEMT) as indicated by comparable increases in PEMT-PC enrichment in pregnant and nonpregnant women despite unequal (pregnant > nonpregnant; P < 0.001) PC pool sizes. Pregnancy enhanced the hydrolysis of PEMT-PC to free choline as shown by greater (P < 0.001) plasma d3-choline:d3-PC. Notably, d3-PC enrichment increased (P ≤ 0.011) incrementally from maternal to placental to fetal compartments, signifying the selective transfer of PEMT-PC to the fetus.CONCLUSIONS:The enhanced use of choline for PC production via both the CDP-choline and PEMT pathways shows the substantial demand for choline during late pregnancy. Selective partitioning of PEMT-PC to the fetal compartment may imply a unique requirement of PEMT-PC by the developing fetus.

Abstract:This study explored the role of choline in placental vascular function. In a 12‐week controlled feeding study among third trimester pregnant women, the higher choline intake group (930 vs. 480 mg/d) had lower (P=0.05) placental and circulating levels of soluble fms‐like tyrosine kinase‐1 (sFLT1), an anti‐angiogenic factor that predicts preeclampsia risk. The effect of choline on placental cell function was further investigated using immortalized HTR‐8/SVneo trophoblasts cultured in different choline concentrations (28, 13 and 8 μM) for 96 hours. The decrease in choline concentrations led to a graded increase (P<0.01) in the gene expression and secretion of sFLT1, expression of the proinflammatory markers [e.g. interleukin 1 beta (IL1B)], apoptosis and oxidative stress. Cellular phosphatidylcholine dramatically decreased (50%, P<0.01) in the lower choline group, whereas the protein kinase C (PKC) isoforms δ and ε, activated by breakdown products of phosphatidylcholine, increased (P<0.01). The addition of a PKC inhibitor partially rescued the increased expression of sFLT1 and apoptosis. Choline and Betaine Alter Fetal Brain Expression of Genes Related to Docosahexaenoic Acid Availability in A Mouse Model of Gestational Diabetes

Abstract:Gestational diabetes mellitus (GDM) is associated with lower cord blood docosahexaenoic acid (DHA) concentrations, which can potentially delay brain development of affected infants. Phosphatidylcholine synthesized via the de novo pathway is enriched with DHA, thereby providing an available source of DHA. Using a mouse model of GDM, we assessed whether supplementation of dams with choline or its oxidative derivative, betaine, modified the expression of genes related to DHA availability in fetal brains. C57BL/6J female mice were fed a high‐fat diet (60% kcal from fat) six weeks before and throughout gestation to induce GDM‐like symptoms. The animals received 25 mM choline chloride, 1% betaine, or control drinking water until embryonic day 17.5. Fetal brain samples were collected for gene expression analysis. Betaine supplementation in the high‐fat fed dams increased the expression of fatty acid binding protein 7 (Fabp7), which transports DHA, and Fabp5, which transports saturated fatty acids in fetal brains. Choline supplementation did not alter the expression of Fabp genes. However, choline upregulated expression of phospholipase A2 (Pla2g6), which releases DHA from the middle position of the glycerol backbone of phospholipids such as phosphatidylcholine, enhancing availability of free DHA in the brain. Expression of arachidonate 15‐lipoxygenase, cyclooxygenase‐2, and brain‐derived neurotrophic factor were not altered by choline or betaine supplementation. Our results support a role for maternal choline or betaine supplementation in modifying DHA availability in fetal brain by altering gene expression in the offspring of GDM dams.
The distribution of dietary choline intake and serum choline levels in Australian women during pregnancy and associated early life factors

Abstract:Background: Maternal dietary choline has a central role in foetal brain development and may be associated with later cognitive function. However, many countries are reporting lower than recommended intake of choline during pregnancy. Methods: Dietary choline was estimated using food frequency questionnaires in pregnant women participating in population-derived birth cohort, the Barwon Infant Study (BIS). Dietary choline is reported as the sum of all choline-containing moieties. Serum total choline-containing compounds (choline-c), phosphatidylcholine and sphingomyelin were measured using nuclear magnetic resonance metabolomics in the third trimester. The main form of analysis was multivariable linear regression. Results: The mean daily dietary choline during pregnancy was 372 (standard deviation (SD) 104) mg/day. A total of 236 women (23%) had adequate choline intake (440 mg/day) based on the Australian and New Zealand guidelines, and 27 women (2.6%) took supplemental choline ([Formula: see text] 50 mg/dose) daily during pregnancy. The mean serum choline-c in pregnant women was 3.27 (SD 0.44) mmol/l. Ingested choline and serum choline-c were not correlated (R2) = - 0.005, p = 0.880. Maternal age, maternal weight gain in pregnancy, and a pregnancy with more than one infant were associated with higher serum choline-c, whereas gestational diabetes and environmental tobacco smoke during preconception and pregnancy were associated with lower serum choline-c. Nutrients or dietary patterns were not associated with variation in serum choline-c. Conclusion: In this cohort, approximately one-quarter of women met daily choline recommendations during pregnancy. Future studies are needed to understand the potential impact of low dietary choline intake during pregnancy on infant cognition and metabolic intermediaries.

Abstract:This study investigated the effects of dietary choline levels during gestation on reproductive performance of sows. In addition, the plasma metabolome and gut microbiota of sows was studied. A total of 260 multiparous sows were allocated to five dietary treatment groups with increasing choline concentrations (1,050, 1,450, 1,850, 2,250, and 2,650 mg/kg) in a randomized complete block design. The sows were fed experimental diets from breeding until farrowing and a common lactating diet during lactation. The results showed that the backfat (BF) gain of sows during gestation, individual birth weight for total piglets born, piglets born alive, average piglet weight at weaning increased linearly (P < 0.05), whereas the within-litter birth weight variation coefficient (CV) of piglets born alive and suckling piglet mortality decreased linearly (P < 0.05) as dietary choline level increased. A quadratic effect of dietary choline level was observed for the average daily feed intake (ADFI) of sows during lactation (P < 0.05). ADFI was maximized when the dietary choline concentration reached 1,910 mg/kg. Plasma H2O2 concentration at day 30 of gestation in the 1,050 mg/kg group was greater than that in the 1,850 and 2,650 mg/kg groups (P < 0.05). Plasma metabolomics identified 46 metabolites among the three groups. Specifically, plasma concentrations of trimethylamine-N-oxide (TMAO), dopamine, and L-proline increased while 1-methylhistidine concentration decreased as dietary choline levels increased. In addition, bacterial observed species and richness (Chao 1 and ACE) at day 110 of gestation decreased as dietary choline levels increased (P < 0.05). For the gut microbiota composition, the enhanced dietary choline level decreased the abundance of phylum Proteobacteria (P < 0.05) and increased the abundance of phylum Actinobacteria (P < 0.05) at day 30 of gestation. Compared with the 1,050 mg/kg group, the abundance of genus Terrisporobacter was less in the 1,850 mg/kg group, and genera Bacillus and Cellulomonas were greater in the 2,650 mg/kg group. In summary, increasing dietary choline levels improved the birth weight, uniformity of neonatal piglets and litter performance during lactation. This may be associated with better antioxidant capability, metabolic status, and gut microbiota of sows during gestation.

Abstract:Purpose: To investigate the relationships among docosahexaenoic acid (DHA) intake, nutrient intake, and maternal characteristics on pregnancy outcomes in a phase III randomised clinical trial designed to determine the effect of a DHA dose of 1000 mg/day compared to 200 mg/day on early preterm birth (<34 weeks gestation). Methods: A secondary aim of the phase III randomised trial was to explore the relationships among pregnancy outcomes (maternal red blood cell phospholipid (RBC-PL) DHA at delivery, preterm birth, gestational age at delivery, labor type, birth anthropometric measures, low birth weight, gestational diabetes, pre-eclampsia, and admission to a neonatal intensive care unit) in participants (n = 1100). We used Bayesian multiple imputation and linear and logistic regression models to conduct an analysis of five general classes of predictor variables collected during the trial: a) DHA intake, b) nutrient intake from food and supplements, c) environmental exposure to tobacco and alcohol, d) maternal demographics, and e) maternal medical history. Results: DHA supplementation lowered the risk of preterm birth and NICU admission, and increased gestation and birth weight as observed in the primary analysis. Higher maternal RBC-PL-DHA at delivery was associated with DHA supplementation and formal education of a bachelor's degree or higher. DHA supplementation and maternal age were associated with a higher risk of gestational diabetes. Total vitamin A intake was associated with longer gestation, while fructose and intake of the long chain omega-6 fatty acid, arachidonic acid, were associated with shorter gestation. Risk of preterm birth was associated with a history of low birth weight, preterm birth, pre-eclampsia, and NICU admission. Conclusion: Bayesian models provide a comprehensive approach to relationships among DHA intake, nutrient intake, maternal characteristics, and pregnancy outcomes. We observed previously unreported relationships between gestation duration and fructose, vitamin A, and arachidonic acid that could be the basis for future research. Abstract:The modulatory role of choline on the fetal epigenome and the impact of in utero choline supply on fetal programming and health are of great interest. Studies in animals and/or humans suggest that maternal choline supplementation during pregnancy benefits important physiologic systems such as offspring cognitive function, response to stress, and cerebral inhibition. Because alterations in offspring phenotype frequently coincide with epigenetic modifications and changes in gene expression, maternal choline supplementation may be a nutritional strategy to improve lifelong health of the child. Future studies are warranted to elucidate further the effect of choline on the fetal epigenome and to determine the level of maternal choline intake required for optimal offspring physiologic function.
Maternal One-Carbon Nutrient Intake and Risk of Being Overweight or Obese in Their Offspring-A Transgenerational Prospective Cohort Study

Abstract:We aimed to investigate the associations between maternal intake of folate, vitamin B12, B6, B2, methionine, choline, phosphatidylcholine and betaine during the period surrounding pregnancy and offspring weight outcomes from birth to early adulthood. These associations were examined among 2454 mother-child pairs from the Nurses' Health Study II and Growing Up Today Study. Maternal energy-adjusted nutrient intakes were derived from food frequency questionnaires. Birth weight, body size at age 5 and repeated BMI measurements were considered. Overweight/obesity was defined according to the International Obesity Task Force (<18 years) and World Health Organization guidelines (18+ years). Among other estimands, we report relative risks (RRs) for offspring ever being overweight with corresponding 95% confidence intervals across quintiles of dietary factors, with the lowest quintile as the reference. In multivariate-adjusted models, higher maternal intakes of phosphatidylcholine were associated with a higher risk of offspring ever being overweight (RRQ5vsQ1 = 1.16 [1.01-1.33] p-trend: 0.003). The association was stronger among offspring born to mothers with high red meat intake (high red meat RRQ5vsQ1 = 1.50 [1.14-1.98], p-trend: 0.001; low red meat RRQ5vsQ1 = 1.05 [0.87-1.27], p-trend: 0.46; p-interaction = 0.13). Future studies confirming the association between a higher maternal phosphatidylcholine intake during pregnancy and offspring risk of being overweight or obese are needed.
