Abstract:Polyetheretherketone (PEEK) is a high-performance thermoplastic polymer with an elastic modulus close to that of the jawbone. PEEK has the potential to become a new dental implant material for special patients due to its radiolucency, chemical stability, color similarity to teeth, and low allergy rate. However, the aromatic main chain and lack of surface charge and chemical functional groups make PEEK hydrophobic and biologically inert, which hinders subsequent protein adsorption and osteoblast adhesion and differentiation. This will be detrimental to the deposition and mineralization of apatite on the surface of PEEK and limit its clinical application. Researchers have explored different modification methods to effectively improve the biomechanical, antibacterial, immunomodulatory, angiogenic, antioxidative, osteogenic and anti-osteoclastogenic, and soft tissue adhesion properties. This review comprehensively summarizes the latest research progress in material property advantages, three-dimensional printing synthesis, and functional modification of PEEK in the fields of implant dentistry and provides solutions for existing difficulties. We confirm the broad prospects of PEEK as a dental implant material to promote the clinical conversion of PEEK-based dental implants.
Enhanced bioactivity of titanium-coated polyetheretherketone implants created by a high-temperature 3D printing process

Abstract:Polyetheretherketone (PEEK), one of the potential alternatives to metallic materials for implants, necessarily involves high temperature process conditions to be three-dimensionally (3D) printed. We developed a 3D printing setup equipped with thermally stabilized modules of the printing nozzle and building chamber, by which the PEEK implants could be successfully manufactured. Under optimized printing conditions, the maximal mechanical strength of the 3D printed sample attained over 80% of the original bulk property of PEEK. To enhance the interfacial biocompatibility, the as-printed implants were postprocessed with titanium (Ti) sputtering. The Ti-coated surfaces were evaluated through characterization studies of x-ray diffraction spectra, microscopic topographies, and wetting properties. For the in vitro tests, preosteoblasts were cultured on the developed PEEK-Ti structures and evaluated in terms of cell adhesion, proliferation, and osteogenic differentiation. In addition, the bone regeneration capability of the PEEK-Ti implants was confirmed by animal experiments using a rabbit tibia defect model for a period of 12 weeks. In the overall in vitro and in vivo tests, we confirmed the superior bioactivities of the Ti-modified and 3D printed interface by comparisons between the samples of machined and printed samples with or without Ti coating. Taken together, the comprehensive manufacturing approaches that involve 3D printing and biocompatible postprocessing are expected to have universal applicability in a wide range of bone tissue engineering.

Abstract:Three-dimensional printing (3DP) technology is suitable for manufacturing personalized orthopedic implants for reconstruction surgery. Compared with traditional titanium, polyether-ether-ketone (PEEK) is the ideal material for 3DP orthopedic implants due to its various advantages, including thermoplasticity, thermal stability, high chemical stability, and radiolucency suitable elastic modulus. However, it is challenging to develop a well-designed method and manufacturing technique to meet the clinical needs because it requires elaborate details and interplays with clinical work. Furthermore, establishing surgical standards for new implants requires many clinical cases and an accumulation of surgical experience. Thus, there are few case reports on using 3DP PEEK implants in clinical practice. Herein, we formed a team with a lot of engineers, scientists, and doctors and conducted a series of studies on the 3DP PEEK implants for chest wall reconstruction. First, the thoracic surgeons sort out the specific types of chest wall defects. Then, the engineers designed the shape of the implant and performed finite element analysis for every implant. To meet the clinical needs and mechanical requirements of implants, we developed a new fused deposition modeling technology to make personalized PEEK implants. Overall, the thoracic surgeons have used 114 personalized 3DP PEEK implants to reconstruct the chest wall defect and further established the surgical standards of the implants as part of the Chinese clinical guidelines. The surface modification technique and composite process are developed to overcome the new clinical problems of implant-related complications after surgery. Finally, the major challenges and possible solutions to translating 3DP PEEK implants into a mature and prevalent clinical product are discussed in the paper.

Abstract:Three-dimensional (3D) printing, medical imaging, and implant design have all advanced significantly in recent years, and these developments may change how modern craniomaxillofacial surgeons use patient data to create tailored treatments. Polyether-ether-ketone (PEEK) is often seen as an attractive option over metal biomaterials in medical uses, but a solid PEEK implant often leads to poor osseointegration and clinical failure. Therefore, the objective of this study is to demonstrate the quantitative assessment of a custom porous PEEK implant for cranial reconstruction and to evaluate its fitting accuracy. The research proposes an efficient process for designing, fabricating, simulating, and inspecting a customized porous PEEK implant. In this study, a CT scan is utilized in conjunction with a mirrored reconstruction technique to produce a skull implant. In order to foster cell proliferation, the implant is modified into a porous structure. The implant's strength and stability are examined using finite element analysis. Fused filament fabrication (FFF) is utilized to fabricate the porous PEEK implants, and 3D scanning is used to test its fitting accuracy. The results of the biomechanical analysis indicate that the highest stress observed was approximately 61.92 MPa, which is comparatively low when compared with the yield strength and tensile strength of the material. The implant fitting analysis demonstrates that the implant's variance from the normal skull is less than 0.4436 mm, which is rather low given the delicate anatomy of the area. The results of the study demonstrate the implant's endurance while also increasing the patient's cosmetic value.

Abstract:Fused deposition modeling (FDM) is a rapidly growing three-dimensional (3D) printing technology and has great potential in medicine. Polyether-ether-ketone (PEEK) is a biocompatible high-performance polymer, which is suitable to be used as an orthopedic/dental implant material. However, the mechanical properties and biocompatibility of FDM-printed PEEK and its composites are still not clear. In this study, FDM-printed pure PEEK and carbon fiber reinforced PEEK (CFR-PEEK) composite were successfully fabricated by FDM and characterized by mechanical tests. Moreover, the sample surfaces were modified with polishing and sandblasting methods to analyze the influence of surface roughness and topography on general biocompatibility (cytotoxicity) and cell adhesion. The results indicated that the printed CFR-PEEK samples had significantly higher general mechanical strengths than the printed pure PEEK (even though there was no statistical difference in compressive strength). Both PEEK and CFR-PEEK materials showed good biocompatibility with and without surface modification. Cell densities on the “as-printed” PEEK and the CFR-PEEK sample surfaces were significantly higher than on the corresponding polished and sandblasted samples. Therefore, the FDM-printed CFR-PEEK composite with proper mechanical strengths has potential as a biomaterial for bone grafting and tissue engineering applications.

Abstract:This work addresses the topic of extrusion-based additive manufacturing (filament-based material extrusion) of patient-specific biofunctional maxillofacial implants. The technical approach was chosen to overcome the shortcomings of medically established fabrication processes such as a limited availability of materials or long manufacturing times. The goal of the work was a successful fabrication of basic implants for defect reconstruction. The underlying vision is the implants’ clinic-internal and operation-accompanying application. Following a literature search, a material selection was conducted. Digitally prepared three-dimensional (3D) models dealing with two representative mandible bone defects were printed based on the material selection. An ex-vivo model of the implant environment evaluated dimensional and fitting traits of the implants. Glycol-modified PET (PETG) and thermoplastic polyurethane (TPU) were finally selected. These plastics had high cell acceptance, good mechanical properties, and optimal printability. The subsequent fabrication process yielded two different implant strategies: the standard implant made of PETG with a build-up rate of approximately 10 g/h, and the biofunctional performance implant with a TPU shell and a PETG core with a build-up rate of approximately 4 g/h. The standard implant is meant to be intraoperatively applied, as the print time is below three hours even for larger skull defects. Standard implants proved to be well fitting, mechanically stable and cleanly printed. In addition, the hybrid implant showed particularly cell-friendly behavior due to the chemical constitution of the TPU shell and great impact stability because of the crack-absorbing TPU/PETG combination. This biofunctional constellation could be used in specific reconstructive patient cases and is suitable for pre-operative manufacturing based on radiological image scans of the defect. In summary, filament-based material extrusion has been identified as a suitable manufacturing method for personalized implants in the maxillofacial area. A further clinical and mechanical study is recommended.

It explores the various PEEK-based 3D printing applications in the dental, orthopedic, spinal, cranial, cardiovascular, and emerging implant domains, supported by real-world case studies and success stories. The review also discusses difficulties, developments, and sustainability issues related to PEEK-based 3D printing. Abstract:<p>Poly-ether-ether-ketone (PEEK) possesses excellent biocompatibility and similar elastic modulus as bones but yet suffers from poor osseointegration. In order to balance PEEK's mechanical and osseointegration properties, a novel surface porous PEEK (SP-PEEK) is successfully fabricated by fused deposition modelling three-dimensional printing (FDM 3DP) and characterized by mechanical and osteogenesis in vitro tests. Moreover, the effects of pore diameter and pore layer number on the mechanical behaviors of SP-PEEK are investigated by theoretical model and numerical simulation. Comparison among experimental, theoretical and simulation results show good agreement. As pore diameter decreases, the equivalent strength and modulus become more sensitive to the decrease of pore layer number. In addition, the SP-PEEK exhibits the mechanical properties within the range of human trabecular bone and cortical bone, and thus can be tailored to mimic human bone by adjusting the pore diameter and pore layer number, which is benefit to mitigate stress shielding. The effects of pore diameter on the cell proliferation and osteogenic differentiation of SP-PEEK are tested by the co-culture of osteoblast precursor cells (MC3T3-E1) and SP-PEEK round discs. Results showcase that porous surface improves the osteogenesis in vitro, and the SP-PEEK group that the pore diameter is 0.6 mm exhibits optimal-performance osteogenesis in vitro.</p>

Abstract:Large bone defects caused by tumors and traumas are difficult clinical problems. For its treatment, autogenous bone transplantation is the "gold standard". However, there are some shortcomings of this treatment, such as limited supply and extra trauma from bone removal. In recent years, orthopedic implants and bone tissue engineering materials have developed rapidly, adding impetus to the solution of this problem. At present, the main orthopedic implants include medical metal materials, medical polymer materials, bone cement, ceramic materials, artificial bone materials, etc. Among which medical polymer materials have become the research hotspot and achieved outstanding results in recent years, especially the in-depth study of polyether-ether-ketone (PEEK) materials has great promising to solve this medical problem. PEEK materials have the advantages of non-toxicity, high-temperature resistance, corrosion resistance, abrasion resistance, high strength, high toughness, X-ray radiolucency, and excellent sterilization performance. PEEK materials have been successfully applied in clinical practice and have achieved excellent clinical efficacy and wide recognition. This review overviews the research progress of the performance requirements, material development, and material surface modification of PEEK as an orthopedic implant, and discusses prospects for the advance of medical PEEK materials.

Abstract:Currently, dental implants primarily rely on the use of titanium and titanium alloys. However, the extensive utilization of these materials in clinical practice has unveiled various problems including stress shielding, corrosion, allergic reactions, cytotoxicity, and image artifacts. As a result, polyetheretherketone (PEEK) has emerged as a notable alternative due to its favorable mechanical properties, corrosion resistance, wear resistance, biocompatibility, radiation penetrability and MRI compatibility. Meanwhile, the advancement and extensive application of 3D printing technology has expanded the range of medical applications for PEEK, including artificial spines, skulls, ribs, shinbones, hip joints, and temporomandibular joints. In this review, we aim to assess the advantages and disadvantages of PEEK as a dental implant material, summarize the measures taken to address its shortcomings and their effects, and provide insight into the future potential of PEEK in dental implant applications, with the goal of offering guidance and reference for future research endeavors.
Optimization and manufacture of polyetheretherketone patient specific cranial implants by material extrusion - A clinical perspective

Abstract:Polyetheretherketone (PEEK) is a high performing thermoplastic that has established itself as a 'gold-standard' material for cranial reconstruction. Traditionally, milled PEEK patient specific cranial implants (PSCIs) exhibit uniform levels of smoothness (excusing suture/drainage holes) to the touch (<1 μm) and homogenous coloration throughout. They also demonstrate predictable and repeatable levels of mechanical performance, as they are machined from isotropic material blocks. The combination of such factors inspires confidence from the surgeon and in turn, approval for implantation. However, manufacturing lead-times and affiliated costs to fabricate a PSCI are high. To simplify their production and reduce expenditure, hospitals are exploring the production of in-house PEEK PSCIs by material extrusion-based additive manufacturing. From a geometrical and morphological perspective, such implants have been produced with good-to-satisfactory clinical results. However, lack of clinical adoption persists. To determine the reasoning behind this, it was necessary to assess the benefits and limitations of current printed PEEK PSCIs in order to establish the status quo. Afterwards, a review on individual PEEK printing variables was performed in order to identify a combination of parameters that could enhance the aesthetics and performance of the PSCIs to that of milled implants/cranial bone. The findings from this review could be used as a baseline to help standardize the production of PEEK PSCIs by material extrusion in the hospital.

Abstract:Recent advancements in medical imaging, virtual surgical planning (VSP), and three-dimensional (3D) printing have potentially changed how today’s craniomaxillofacial surgeons use patient information for customized treatments. Over the years, polyetheretherketone (PEEK) has emerged as the biomaterial of choice to reconstruct craniofacial defects. With advancements in additive manufacturing (AM) systems, prospects for the point-of-care (POC) 3D printing of PEEK patient-specific implants (PSIs) have emerged. Consequently, investigating the clinical reliability of POC-manufactured PEEK implants has become a necessary endeavor. Therefore, this paper aims to provide a quantitative assessment of POC-manufactured, 3D-printed PEEK PSIs for cranial reconstruction through characterization of the geometrical, morphological, and biomechanical aspects of the in-hospital 3D-printed PEEK cranial implants. The study results revealed that the printed customized cranial implants had high dimensional accuracy and repeatability, displaying clinically acceptable morphologic similarity concerning fit and contours continuity. From a biomechanical standpoint, it was noticed that the tested implants had variable peak load values with discrete fracture patterns and failed at a mean (SD) peak load of 798.38 ± 211.45 N. In conclusion, the results of this preclinical study are in line with cranial implant expectations; however, specific attributes have scope for further improvements.

Biomechanics and Modeling in Mechanobiology

Abstract:A tumour resection normally involves a large tissue resection and bone replacement. Polyether ether ketone (PEEK) has become a suitable candidate for use in various prostheses owing to its lightness in weight, modulus close to that of natural bone, and good biocompatibility, among other factors. This study proposes a new design method for a rib prosthesis using the centroid trajectory of the natural replaced rib, where the strength can be adjusted by monitoring the cross-sectional area, shape, and properties. A custom-designed rib prosthesis was manufactured using fused deposition modelling (FDM) manufacturing technology, and the mechanical behaviour was found to be close to that of a natural rib. A finite element analysis of the designed rib was carried out under similar loading conditions to those used in mechanical testing. The results indicate that the centroid trajectory derived from a natural rib diaphysis can provide reliable guidance for the design of a rib prosthesis. Such methodology not only offers considerable design freedom in terms of shape and required strength, but also benefits the quality of the surface finishing for samples manufactured using the FDM technique. FDM-printed PEEK rib prostheses have been successfully implanted, and good clinical performances have been achieved.Graphical abstract

Abstract:Polyetheretherketone (PEEK) constitutes a preferred alternative material for orthopedic implants owing to its good mechanical properties and biocompatibility. However, the poor osseointegration property of PEEK implants has limited their clinical applications. To address this issue, in this study, we investigated the mechanical and biological properties of fully porous PEEK scaffolds with different pore sizes both in vitro and in vivo. PEEK scaffolds with designed pore sizes of 300, 450, and 600 μm and a porosity of 60% were manufactured via fused deposition modeling (FDM) to explore the optimum pore size. Smooth solid PEEK cylinders (PEEK-S) were used as the reference material. The mechanical, cytocompatibility, proliferative, and osteogenic properties of PEEK scaffolds were characterized in comparison to those of PEEK-S. In vivo dynamic contrast-enhanced magnetic resonance imaging, microcomputed tomography, and histological observation were performed after 4 and 12 weeks of implantation to evaluate the microvascular perfusion and bone formation afforded by the various PEEK implants using a New Zealand white rabbit model with distal femoral condyle defects. Results of in vitro testing supported the good biocompatibility of the porous PEEK scaffolds manufactured via FDM. In particular, the PEEK-450 scaffolds were most beneficial for cell adhesion, proliferation, and osteogenic differentiation. Results of in vivo analysis further indicated that PEEK-450 scaffolds exhibited preferential potential for bone ingrowth and vascular perfusion. Together, our findings support that porous PEEK implants designed with a suitable pore size and fabricated via three-dimensional printing constitute promising alternative biomaterials for bone grafting and tissue engineering applications with marked potential for clinical applications.This article has not yet been cited by other publications.

These aspects have been investigated in the light of incredible mechanical and physicochemical properties of PEEK and its composites. 3D printing has provided the much needed assistance to develop and fabricate complex implants made up of PEEK based composites. The observations concluded that PEEK has shown promising characteristics to be a potential alternative for conventional implant materials. In dental applications, its light weight, biocompatibility and aesthetic pleasance has left patient healthy and satisfied. Similar satisfactory findings have also been observed when used in knee, hip, spine and other implants. PEEK can fulfill the long term demand of an ideal multipurpose material for biomedical applications. However, still there is a vast scope for future researches and works for the innovation and development of the techniques to make it more feasible and accessible, to ensure unhampered use of PEEK and PEEK based composites to benefit the society.</p>

Abstract:Polyetheretherketone (PEEK) is a lightweight, bioinert, high-performance thermoplastic that is beginning to see clinical use in orthopedic applications. PEEK outperforms conventional metallic counterparts in terms of reduced stress shielding and improved chemical resistance, making it highly suitable for implantable applications. However, despite its excellent mechanical properties, the elevated melting point (343 °C) presents significant challenges during manufacturing. Furthermore, PEEK requires surface modifications to enhance antibacterial, bioactive, and osseointegration properties suitable for in vivo applications. In this context, the present manuscript highlights current manufacturing challenges for implantable PEEK biomaterials and typical fiber reinforced PEEK composites. Emphasis is placed on reinforcements such as carbon fiber (CF), hydroxyapatite (HA) and titanium dioxide (TiO 2 ), along with multi-material PEEK composites and their applications. Opportunities are identified to address these challenges, contributing toward the development of synergetic, multi-functional PEEK biomaterials suitable for long-term implantable applications.

Abstract:With the advantage of remarkable mechanical processing ability without losing its strength , polyetheretherketone (PEEK) based medical devices have been drawing great attention in orthopedic treatments. However, its limited osseointegration capacity and susceptibility to bacterial colonization-induced infections pose a considerable risk of implant failure. Current methods of antimicrobial and osteogenic functionalization frequently exhibit conflicting outcomes. The antimicrobial functionalization of implants usually leads to cytotoxicity and histotoxicity, adversely affecting the connection with surrounding tissues, whereas the osteogenic functionalization may inadvertently promote bacterial adhesion and biofilm development. Therefore, it is highly demanded to endow the PEEK implants with antibacterial and osseointegration properties simultaneously. In this study, 3D-printed PEEK implants were surface modified with hyperbranched poly- l -lysine (HBPL) via physisorption (PEEK- a -HBPL) and chemical grafting (PEEK- g -HBPL), respectively, which were demonstrated by attenuated total reflection Fourier transform infrared spectroscopy, water contact angle, and X-ray photoelectron spectroscopy. Remarkably, both the PEEK- a -HBPL and PEEK- g -HBPL exhibited significantly improved antibacterial efficacy against S. aureus and E. coli , while their osteogenesis ability was not affected but even promoted in vitro . In an in vivo bone-infection and implantation model in rats, significantly reduced surrounding tissue inflammation and larger new bone formation were observed in both the PEEK- a -HBPL and PEEK- g -HBPL groups compared to the untreated PEEK group. This comprehensive study lays a foundation for the advancing of organic orthopedic implant materials, offering potential solutions to the pressing issues of osseointegration deficits and implant-related infections.

Abstract:Polyether ether ketone (PEEK) is a non-toxic polymer with elastic modulus close to human bone. Compared with metal implants, PEEK has advantages such as evasion of stress shielding effect, easy processing, and similar color as teeth, among others. Therefore, it is an excellent substitute material for titanium dental orthopedic implants. However, PEEK’s biological inertia limits its use as an implant. To change PEEK’s biological inertia and increase its binding ability with bone tissue as an implant, researchers have explored a number of modification methods to enhance PEEK’s biological activities such as cellular compatibility, osteogenic activity, and antibacterial activity. This review summarizes current biological activity modification methods for PEEK, including surface modification and blending modification, and analyzes the advantages and disadvantages of each modification method. We believe that modified PEEK will be a promising dental and orthopedic implant material.
