Abstract:The aim of this study was to evaluate the outcomes of temporomandibular joint (TMJ) arthroscopic and open disc repositioning procedures in the management of anterior disc displacement (ADD). All consecutive patients treated with arthroscopic (group I) or open (group II) disc repositioning between April 2014 and August 2018 were included prospectively. The patients were assessed clinically (1, 3, 6, 12, 24, and 36 months postoperative) and with magnetic resonance imaging (MRI). The statistical analysis was performed using IBM SPSS Statistics v.22.0; P < 0.05 was considered significant. A total of 177 patients (227 joints) were included: 104 patients (130 joints) in group I and 73 patients (97 joints) in group II. There were statistically significant improvements in pain score, clicking, quality of life, diet, and maximum inter-incisal opening when comparing pre-and postoperative clinical parameters within the two groups (P < 0.05 at all time-points for all clinical parameters); however, improvements occurred earlier in group I (at 1 month) than in group II (6 months). Postoperative MRI revealed an overall success rate of 98.1% in group I and 97.3% in group II. New bone formation was found in 70.2% in group I and 30.1% in group II. Arthroscopy may be a better choice for ADD patients, with the advantages of faster clinical improvement and recovery, minimal invasiveness, and better condylar remodelling.

Abstract:PURPOSE:The purpose of this study was to introduce a new arthroscopic disc suturing for treating internal derangement of the temporomandibular joint.METHODS:Arthroscopic disc repositioning and suturing surgery including anterior release and disc repositioning was performed on patients who visited the TMJ clinic at the Ninth People's Hospital,Shanghai JiaoTong University School of Medicine,between August 2004 and March 2007 with TMJ ID stages Ⅱ-Ⅴ.The patients' ID stages were based on the criteria of Wilkes & Bronstein's classification.Consecutive MRIs were used to evaluate ID before operation and after operation for all 639 patients.The disc position was judged according to our success criteria which included three different sagittal planes(lateral,central and medial).Operation efficiency of those patients,whose disc of TMJ was affirmed to be in normal position in all 3 planes,were evaluated to be excellent.Those patients,whose discs were in normal position in 2 planes,were evaluated to be good.The others were evaluated to be poor.Cases evaluated as "excellent" and "good" were used for calculation of success case(if the disc is displaced in only one or two planes,we thought it was success,when the whole disc was in normal position).Arthroscopic surgery or open surgery was carried out once more for the joints evaluated as poor.RESULTS:Postoperative consecutive MRIs for all 764 joints confirmed that 95.42%(729/764) joints was excellent,3.14%(24/764) was good,and only 1.44%(11/764) was poor.CONCLUSION:TMJ arthroscopic suturing is an effective,minimally invasive surgery in repositioning the TMJ disc,but long-term follow-up is necessary for its stability.

Abstract:Purpose: To evaluate the efficiency of an arthroscopic suturing technique for stabilizing anteriorly displaced discs in patients with internal derangement of the temporomandibular joint (TMJ) by magnetic resonance (MR) imaging.Patients and Methods: Six hundred thirty-nine patients (764 joints) diagnosed as having stages II to V of internal derangement were treated with arthroscopic disc repositioning and suturing from August 2004 to March 2007. Consecutive MR images were used to evaluate internal derangement before and approximately 1 to 7 days after the operation for all 639 patients. Abstract:Disc displacement is a common disorder affecting the temporomandibular joint. According to previous publications, the displaced disc can be categorized into pure anterior displacement and rotational displacement (anteromedial and anterolateral). However, the technique of arthroscopy treatment has only been reported for patients with pure anterior disc displacement. In this study, an arthroscopic discopexy for rotational anterior disc displacement was developed and its effectiveness evaluated over 24 months of follow-up. A total of 532 patients (749 joints) with rotational anterior disc displacement, admitted to Shanghai Ninth People's Hospital between January 2011 and December 2015, were included. The success rate was based on clinical parameters (visual analogue scale (VAS) for pain, maximum inter-incisal opening (MIO), and complications) and radiographic data. The clinical and radiographic data were collected preoperatively and at 1, 6, 12, and 24months postoperative. The VAS score decreased to 0.73±1.43 following surgery (P<0.001). A significant improvement in MIO (34.73±6.28mm) was also detected (P<0.001). Magnetic resonance imaging showed discs repositioned in both sagittal and coronal images for 714 of the 749 joints, giving a success rate of 95.3%. This study reports an effective and predictable technique of arthroscopic discopexy for rotational anterior disc displacement.

Abstract:Introduction Anterior disc displacement is one of the most frequent temporomandibular joint (TMJ) disorders, which often results in clicking, joint pain, a limited range of motion and masticatory difficulties.Methodology Four subjects (with a total number of six joints) who had significant pain and dysfunction that did not respond to non-surgical therapy, went into arthroscopic disc repositioning using wire fixation for 3 weeks then it was removed, and the patient continued on occlusal splints.Results After six months postoperatively, there was a significant increase in the MIO (Maximum incisal opening) and an insignificant decrease in the pain perception in the VAS (visual analogue scale) Conclusion The study concluded that arthroscopic temporomandibular joint disc repositioning is an effective minimally invasive procedure for the treatment of some temporomandibular joint internal derangements refractory to conventional conservative therapy in regards to clinical outcomes

Abstract:Objective: This study aimed to explain the change of disc position at different over-corrected place after arthroscopic disc repositioning and suturing surgery. Study design: Patients treated with temporomandibular joint arthroscopic disc repositioning and suturing surgery were reviewed. All patients underwent magnetic resonance imaging (MRI) before, immediately after surgery and at 2-year follow-up. The position of disc was checked and the change was compared. Results: 133 patients were included in the final analysis with 203 TMJ sides. The incidence rate for anterior movement of the disc after surgery was 33.0 %. Disc repositioned between 12 and 1 o'clock showed smaller movement rate of 22.6 %, while higher movement rate of 53.6 % was seen when repositioned between 2 and 3 o'clock. Conclusions: After arthroscopic disc repositioning and suturing surgery for ADD patients, the repositioned disc showed tendency to move forward. 12 to 1 o'clock was optional disc site for repositioning while excessive over-correction was not recommended.

Abstract:BackgroundInternal derangement is the clinical and pathological condition of disc displacement of the temporomandibular joint. Management of these cases involve conservative and surgical treatment options. Minimally invasive surgical procedures namely arthrocentesis and arthroscopy are promising techniques in the management of internal derangement. However patient selection algorithms, indications for minimally invasive procedures and details of the techniques should be further studied for safe and cost effective management of these cases.This manuscript aims to retrospectively analyze the significance of a stepped surgical treatment approach (arthrocentesis under local anaesthesia as the first line of treatment, followed by arthroscopic lysis and lavage under general anaesthesia in unresolving cases) of internal derangement with or without osteoarthritis.MethodsThis is a retrospective cohort study. Case notes of 1414 patients that were managed with a standard protocol were reviewed. Appropriate inclusion and exclusion criteria were set. Thirty-three patients were eligible for inclusion. Parameters recorded were pain-free inter-incisal opening, spontaneous pain, pain on function, difficulty on chewing, and perceived disability on jaw movements. Pre-operative and post-operative (at the end of the follow up period) pain free maximum interincisal opening values were compared with paired t test and the subjective parameters were evaluated with Chisquare analysis. Treatment outcome and success rate according to American Association of Oral and Maxillofacial Surgeons were descriptively shown.ResultsInterincisal opening values increased, and the number of patients with severe or medium rated subjective parameters were reduced at discharge. These improvements were found to be statistically significant. Clinical (Wilkes) staging of internal derangement pre-operatively and at discharge remained either unchanged or was lower. Treatment outcome and success according to American Association of Oral and Maxillofacial Surgeons criteria was 94%.ConclusionThe stepped approach for the management of symptomatic internal derangement with or without osteoarthritis is a successful treatment strategy with favourable therapeutic outcomes.

Abstract:Disc repositioning is a common procedure for patients with anterior disc displacement (ADD). The purpose of this retrospective record-based study was to evaluate changes in the widths of joint spaces and condylar position changes in patients with unilateral ADD following arthroscopic disc repositioning, with the healthy sides as self-control, using magnetic resonance images (MRI).Widths of anterior, superior, and posterior joint spaces (AS, SS, and PS) were measured. The condylar position was described as anterior, centric or posterior, expressed as . Paired-t test and Chi-square test were used to analyze the data. Fifty-four records conformed to the inclusion criteria (mean age of 21.02 years). Widths of SS and PS increased significantly after surgery ( P < 0.001) on the operative sides, while joint spaces of healthy sides and AS of operative sides had no significant changes. Dominant location of condyles of operative sides changed from a posterior position to an anterior position, while healthy sides were mostly centric condylar position no matter preoperatively or postoperatively. Therefore, the results of this study indicate that unilateral arthroscopic disc repositioning significantly increases the posterior and superior spaces of the affected joints, without affecting spaces of the healthy sides.

Abstract:The three arthroscopic techniques were particularly method I(lysis and lavage),method II(disc repositioning and sclerotherapy plus method I),method III(traction,fixation plus method II) in order to make comparative study.The distributions between the 76 joints and three techniques were:14 joints in method I,31 in II,and,31 in III.The total follow-up was 30m(6-84m),and the total success rate was 73.7%(53/76).The mean follow-ups were particularly:28.6m in method I,38.4m in II.and,74.2%(23/31) in III,particularly.There were no statically significance between I and II.In disc displacement with reducing,there were no statically significance between method II(18/20) or III(14/15) and I(1/40),and no statically significance between II and III.In disc displacement without reducing,there were no statically significance between method I(6/10)and II(8/11) or III(9/16),or between II and III.Conclusions were that II or method III was effective on disc displacement with reducing,and that it don't make clear for any one of three methods to be more effective on disc displacement without reducing.

Abstract:The potential role of disc repositioning in preventing postsurgical recurrence of traumatogenic temporomandibular joint (TMJ) ankylosis was investigated. Seventeen cases of traumatogenic TMJ ankylosis underwent disc repositioning during arthroplasty. During surgery, the dislocated disc was carefully dissected outside the ankylotic TMJ and repositioned over the top of the condylar stump, and then sutured to the soft tissue of the zygomatic root. In the 22 ankylotic TMJs of the 17 patients, dislocated discs were found in front of the ankylotic TMJ, behind the ankylotic TMJ or between the ramus and fossa. At the last follow-up (longer than 1 year) examination, interincisal opening distances ranged from 24 to 43mm (mean 32.86mm). No recurrence and TMJ symptoms were found during the period of follow-up. Disc repositioning in the treatment of traumatogenic TMJ ankylosis proves to be a feasible and effective method of preventing recurrence of this condition.

Abstract:Background: Open joint procedures using bone anchors have shown clinical and radiograph good success, but post surgical disc position has not been documented with MRI imaging. We have designed a modified technique of using two bone anchors and 2 sutures to reposition the articular discs. This MRI study evaluates the post surgical success of this technique to reposition and stabilize the TMJ articular discs.Methods: Consecutive 81 patients with unilateral TMJ internal derangement (ID) (81 TMJs) were treated between December 1, 2003, and December 1, 2006, at the Department of Oral and Maxillofacial Surgery, Ninth Peoples Hospital, Shanghai, Jiao Tong University School of Medicine. All patients were subjected to magnetic resonance imaging before and one to seven days post surgery to determine disc position using the modified bone anchor technique.Results: Postoperative MRIs (one to seven days) confirm that 77 of 81 joints were identified as excellent results and one joint was considered good for an overall effective rate of 96.3% (78 of 81 joints). Only 3.7% (3 of 81) of the joints were designated as poor results requiring a second open surgery.Conclusions: This procedure has provided successful repositioning of the articular discs in unilateral TMJ ID at one to seven days post surgery.

Abstract:SummaryAnterior repositioning splint (ARS) therapy is considered one of the most effective therapies for treating disc displacement-related temporomandibular disorders (TMDs), which account for a large proportion of TMD cases. Owing to the wide application of this therapy, the exact mechanism of remission has increasingly drawn attention. Given that practitioners have different views on ARS therapy, its indications are broadened, and operating methods diverged. This review attempts to provide an overview of ARS therapy and helps practitioners establish indications and suitable operating methods. Representative views in the past 10 years were summarised, and conclusions were drawn as follows: The mechanism of ARS therapy is mainly attributed to internal derangement correction, improvement of stress distribution and recently reported joint remodeling. It has an evident effect in the short term, and the most prevalent operating methods are protruding the mandible to the edge-to-edge position and wearing the ARS for 24 hours daily for 3–6 months. However, long-term stability is not optimal, and thus indications should be selected carefully. Notably, most of the clinical studies in this field are case analyses with low-quality evidence. Well-designed RCTs are required to further validate relevant theories.

Prognostic Indicators of Arthroscopic Discopexy for Management of Temporomandibular Joint Closed Lock

Abstract:In order to optimize patient selection for temporomandibular joint (TMJ) arthroscopic discopexy to achieve favorable outcomes, prognostic indicators impacting the results are important to analyze. This longitudinal retrospective study aimed to analyze various prognostic factors impacting surgical outcomes following arthroscopic discopexy for management of TMJ closed lock using success criteria based on pain, maximal interincisal opening, diet, and quality of life. Furthermore, a quantitative MRI assessment was performed pre- and post-operatively. Multivariate analysis was used to evaluate various prognostic variables including gender, age, side, duration of illness, Wilkes staging, parafunctional habits, splint therapy and orthodontic treatment. A total of 147 patients (201 joints) were included. The outcome was categorized as excellent (n = 154/76.61%), good (n = 34/16.91%), or poor (n = 13/6.46%) with a success rate of 93.54%. Patients aged > 30 years old ( p = 0.048), longer duration of illness (12–24 months: p = 0.034) and (> 24 months: p = 0.022), and patients with Wilkes stage IV ( p = 0.002) were all significantly more likely to be in the poor outcome group. Finally, orthodontic treatment showed a significant association with excellent outcomes ( p = 0.015). Age, duration of illness, Wilkes staging, and orthodontic treatment are considered significant prognostic factors that can predict the outcomes following the arthroscopic discopexy for management of TMJ closed lock.

Diagnosis and treatment of acute traumatic temporomandibular joint disc displacement in 42 consecutive patients

Abstract:PURPOSE:To explore the diagnosis,treatment results and sequela of traumatic temporomand1bular joint disc displacement (ATDD) in 42 consecutive patients.METHODS:Patients diagnosed of ATDD and treated from 2010 to 2015 were recruited.Their clinical and radiologic characters were analyzed based on 3 stages:acute phase,subacute phase and chronic phase.Conservative and surgical treatment results were evaluated and compared with SPSS software package.RESULTS:Forty-two patients with 59 joints were included in the study.Among the acute and sub-acute phases patients (18 cases with 27 joints),18.5％ of the joints had condylar surface bone destruction,whereas 75％ in the chronic phase (24 cases with 32 joints).Twenty-two cases with 30 joints accepted conservative treatment;among them,90％ developed osteoarthritis after a mean follow-up period of 8.8 months.Thirty-one cases with 47 joints accepted surgical treatment after injury or conservative treatment.Among them,23 cases with 34 joints had disc reposition,95.5％ were effective (15 cases with 22 joints,mean follow-up period of 22 months).Eight cases with 13 end-stage joints accepted condylar resection and joint replacement.Their maximal incisor opening after surgery was significantly increased compared with that before surgery and conservative treatment (P＜0.01).CONCLUSIONS:ATDD is easy to lead to osteoarthritis and ankylosis,disc reposition in the acute and sub-acute phases can significantly reduce the occurrence of above sequel;however,in the chronic phase,joint replacement may be necessary in selected patients.

Abstract:Abstract: Disk perforation can result in degenerative changes within the joint structures. While discectomy has demonstrated enduring benefits, it has traditionally been described using an open approach, with the disadvantages inherent to this method. This study aims to present a series of patients who underwent arthroscopic discectomy technique and to report the outcomes. Methods: Patients diagnosed with internal disorders of the temporomandibular joint underwent arthroscopic arthroscopic discectomy technique. Surgical outcomes were assessed by changes in pain using a visual analog scale and the maximum incisal opening. Results: One hundred seventy-eight joints from 106 patients who underwent arthroscopic surgery were included. Discectomy was performed on 22 joints. Prior to surgery, patients reported an average visual analog scale pain score of 6.5, which decreased to an average of 0.5 at 6 months postsurgery (P<0.001). Before surgery, the average maximum incisal opening was 30 mm, which increased to 41 mm at 6 months postsurgery (P<0.001). Conclusions: The described technique represents an excellent alternative for managing patients with disk perforations.
Arthroscopic Disk Repositioning After Failed Open Disk Repositioning

Abstract:Purpose:Open disk repositioning has been long achieving excellent functional and stability outcomes. However, still remains some relapses for whom a second open surgery is often challenging. This study aimed to evaluate the effectiveness of arthroscopic disk reposition as an alternative surgery for unsuccessful cases of anterior disk displacement (ADD) after an initial open disk repositioning. Materials and Methods:This retrospective study included all patients who underwent secondary arthroscopy for disk repositioning of the relapsed ADD after an initial open surgery between January 2012 to June 2017. The redo arthroscopic disk repositioning and suturing procedure was the primary predictor input variable in this study. Outcome evaluation was based on both clinical (visual analog scale and maximal interincisal opening) and magnetic resonance imaging data. Results:Twenty-seven joints fulfilling the inclusion criteria were included. A significant improvement was detected at 24-month postoperatively compared with the baseline visual analog scale. The maximal interincisal opening showed a statistical improvement from 25.07 mm preoperatively to 38.44 mm at 24-month postoperatively. Twenty-six joints maintained a stable disk position with only 1 joint relapsed to ADD without reduction. Conclusion:Arthroscopic disk reposition and suturing technique is a reliable and effective repeat surgery after failed initial open disk repositioning for management of ADD.
