Abstract:Different forms of ventricular arrhythmias have been linked to mutations in the cardiac ryanodine receptor (RyR)2, but the molecular basis for this phenotypic heterogeneity is unknown. We have recently demonstrated that an enhanced sensitivity to luminal Ca 2+ and an increased propensity for spontaneous Ca 2+ release or store-overload-induced Ca 2+ release (SOICR) are common defects of RyR2 mutations associated with catecholaminergic polymorphic or bidirectional ventricular tachycardia. Here, we investigated the properties of a unique RyR2 mutation associated with catecholaminergic idiopathic ventricular fibrillation, A4860G. Single-channel analyses revealed that, unlike all other disease-linked RyR2 mutations characterized previously, the A4860G mutation diminished the response of RyR2 to activation by luminal Ca 2+ , but had little effect on the sensitivity of the channel to activation by cytosolic Ca 2+ . This specific impact of the A4860G mutation indicates that the luminal Ca 2+ activation of RyR2 is distinct from its cytosolic Ca 2+ activation. Stable, inducible HEK293 cells expressing the A4860G mutant showed caffeine-induced Ca 2+ release but exhibited no SOICR. Importantly, HL-1 cardiac cells transfected with the A4860G mutant displayed attenuated SOICR activity compared with cells transfected with RyR2 WT. These observations provide the first evidence that a loss of luminal Ca 2+ activation and SOICR activity can cause ventricular fibrillation and sudden death. These findings also indicate that although suppressing enhanced SOICR is a promising antiarrhythmic strategy, its oversuppression can also lead to arrhythmias.
Simulation of the Effect of Rogue Ryanodine Receptors on a Calcium Wave in Ventricular Myocytes with Heart Failure.

Abstract:Calcium homeostasis is considered to be one of the most important factors for the contraction and relaxation of the heart muscle. However, under some pathological conditions, such as heart failure (HF), calcium homeostasis is disordered, and spontaneous waves may occur. In this study, we developed a mathematical model of formation and propagation of a calcium wave based upon a governing system of diffusion-reaction equations presented by Izu et al (2001 Biophys. J. 80 103-20) and integrated non-clustered or 'rogue' ryanodine receptors (rogue RyRs) into a two-dimensional (2D) model of ventricular myocytes isolated from failing hearts in which sarcoplasmic reticulum (SR) Ca(2+) pools are partially unloaded. The model was then used to simulate the effect of rogue RyRs on initiation and propagation of the calcium wave in ventricular myocytes with HF. Our simulation results show that rogue RyRs can amplify the diastolic SR Ca(2+) leak in the form of Ca(2+) quarks, increase the probability of occurrence of spontaneous Ca(2+) waves even with smaller SR Ca(2+) stores, accelerate Ca(2+) wave propagation, and hence lead to delayed afterdepolarizations (DADs) and cardiac arrhythmia in the diseased heart. This investigation suggests that incorporating rogue RyRs in the Ca(2+) wave model under HF conditions provides a new view of Ca(2+) dynamics that could not be mimicked by adjusting traditional parameters involved in Ca(2+) release units and other ion channels, and contributes to understanding the underlying mechanism of HF.

Abstract:Heart failure (HF) is associated with susceptibility to sudden cardiac death. However, the underlying mechanism of electrical instability and mechanical dysfunction associated with HF remains poorly understood. In this study, a new canine ventricular cell model based on the Hund-Rudy dynamic (HRd) model and recently published experimental data was developed to investigate the electrical changes and calcium handling dysfunction in HF. Simulation results suggest that: 1) acute Ca2+/calmodulin-dependent protein kinase II (CaMKII) over-expression (CaMKII-OE) affects the action potential (AP) profile, while AP prolongation is mainly caused by the down-regulation of K+ currents; 2) enhanced late Na+ current (I-NaL) alone cannot adequately lead to [Na+] elevation in HF; 3) enhanced sarcoplasmic reticulum (SR) leak current (I-leak) causes disturbed Ca2+ handling and there is little contribution from Na+/Ca2+ exchanger (NCX); 4) at high SR Ca2+ load, a steeper fractional SR Ca2+ release is observed in HF than that in control, causing altemans to occur more easily; and 5) I-leak block restores the contraction and relaxation function, but cannot eliminate altemans. By inhibiting CaMKII, altemans is eliminated, but contractility is not improved. Partial CaMKII inhibition in combination with I-leak block could augment mechanical function and depress altemans, suggesting a new possible therapeutic target for HF treatment. Modeling the Mechanism of [na+]i Elevation in Heart Failure by a Canine Ventricular Cell Model

Abstract:Numerous studies have found the phenomenon of [Na+]i elevation in cardiac myocytes in heart failure (HF), which may influence cardiac action potential (AP) and intracellular Ca2+ handling to induce cardiac arrhythmias. As one of the most important regulators in the cardiac myocytes, Ca2+/Calmodulin-dependent protein kinase II (CaMKII), which could alter Na+ channel gating, was found to be over-expressed in HF. It may contribute to [Na+]i elevation in HF. For the purpose of this study, we developed a new canine epicardial cell model based on the framework of published Hund-Rudy dynamic (HRd) model. The effects of dynamic CaMKII regulation on fast Na+ current (INa) and late Na+ current (INaL) were incorporated into our model according to the recent experimental data. Simulation results suggested that the regulation role of CaMKII on INaL could elevate [Na+]i. The effect of COE on INa had a function of lowering [Na+]i in fact. However, it could not offset the trend of Na+ up-regulation by enhanced INaL. The magnitude of [Na+]i elevation was smaller than experimentally measured values. We speculate that other [Na+]i elevation mechanisms such as lowered Na+/K+ pump (INaK) or increased Na+/H+ exchange may happen in HF.
Alterations of Calcium Channels and Cell Excitability in Intracardiac and Stellate Ganglion Neurons from Chronic Heart Failure Rats

Abstract:Chronic heart failure (CHF) is characterized by increased cardiac sympathetic and decreased cardiac parasympathetic nerve activity. This autonomic imbalance increases the risk of arrhythmias and sudden death in CHF patients. As the final pathways for the autonomic control of the cardiac function, sympathetic postganglionic neurons from the stellate ganglia (SG) release norepinephrine and parasympathetic postganglionic neurons of the intracardiac ganglia (ICG) release acetylcholine upon neuronal excitation. In the present study, we measured the protein expression and current density of Ca2+ channels in the SG and ICG neurons and investigated the role of Ca2+ channels in neuronal excitability and also recorded the cardiac sympathetic nerve activity in sham and coronary artery ligation‐induced CHF rats. Immunofluorescent data showed that L, P/Q, N, and R types of Ca2+ channels were expressed in the SG and ICG neurons, but only protein expression of N‐type Ca2+ channels was decreased in the ICG neurons from CHF rats. Using whole‐cell patch clamp technique, we found that CHF significantly enhanced the Ca2+ currents and cell excitability in SG neurons and reduced the Ca2+ currents and cell excitability in ICG neurons. N‐type Ca2+ channel blocker (1 μM ω‐Conotoxin GVIA) lowered the Ca2+ currents and cell excitability to the same level in SG and ICG neurons from sham and CHF rats. In addition, the cardiac sympathetic nerve activity was augmented in CHF rats. Our results indicate that CHF caused changes of Ca2+ channels and cell excitability in SG and ICG neurons may contribute to the cardiac autonomic imbalance in CHF.
Tonic Nmda Receptor-Mediated Inward Current In Pre-Sympathetic Pvn Neurons Is Enhanced By Heart Failure.

Abstract:Sympathetic nerve activity (SNA) is increased in heart failure (HF). In rats with HF, activation and blockade of NMDA receptors in the hypothalamic PVN is reported to evoke exaggerated increases and decreases of renal SNA. Such actions could involve enhanced NMDA receptor activation, increased neuronal excitability, or both. To test these possibilities, whole‐cell recordings were made from RVLM‐projecting PVN neurons in brain slices from rats with and without HF. Graded current injections induced graded increases in firing frequency in both groups, but responses to the maximally effective current of 200 pA were significantly greater in the HF group (43 ± 6 Hz, n=5) compared to controls (20 ± 4 Hz, n=6) (P<0.05). In slices perfused with Mg++‐ and TTX‐free aCSF containing picrotoxin (100 μM), blockade of NMDA receptors with AP5 (50 μM) (Vhold: −80 mV) induced a significantly greater (P<0.01) outward shift of holding current in HF (41 ± 5 pA, n=3) compared to control (10 ± 1 pA, n=3) neurons. The functional impact of this was assessed by microinjecting AP5 (3.0 nmol in 50 nl) into the PVN of anesthetized rats. AP5 was confirmed to significantly (P<0.01) reduce RSNA by 25 ± 5 %, LSNA by 27 ± 9 % and MAP by 20 ± 6 mmHg in HF rats (n=3), but not controls (n=6). We conclude that enhanced excitability and tonic NMDA receptor‐mediated inward current among PVN‐RVLM neurons may contribute to enhanced PVN control of SNA in HF. Support: HL071645 (GMT)

Abstract:Chronic heart failure (CHF) is characterized by decreased cardiac parasympathetic and increased cardiac sympathetic nerve activity. This autonomic imbalance increases the risk of arrhythmias and sudden death in patients with CHF. We hypothesized that the molecular and cellular alterations of cardiac postganglionic parasympathetic (CPP) neurons located in the intracardiac ganglia and sympathetic (CPS) neurons located in the stellate ganglia (SG) possibly link to the cardiac autonomic imbalance in CHF. Rat CHF was induced by left coronary artery ligation. Single-cell real-time PCR and immunofluorescent data showed that L (Ca(v)1.2 and Ca(v)1.3), P/Q (Ca(v)2.1), N (Ca(v)2.2), and R (Ca(v)2.3) types of Ca2+ channels were expressed in CPP and CPS neurons, but CHF decreased the mRNA and protein expression of only the N-type Ca2+ channels in CPP neurons, and it did not affect mRNA and protein expression of all Ca2+ channel subtypes in the CPS neurons. Patch-clamp recording confirmed that CHF reduced N-type Ca2+ currents and cell excitability in the CPP neurons and enhanced N-type Ca2+ currents and cell excitability in the CPS neurons. N-type Ca2+ channel blocker (1 μM ω-conotoxin GVIA) lowered Ca2+ currents and cell excitability in the CPP and CPS neurons from sham-operated and CHF rats. These results suggest that CHF reduces the N-type Ca2+ channel currents and cell excitability in the CPP neurons and enhances the N-type Ca2+ currents and cell excitability in the CPS neurons, which may contribute to the cardiac autonomic imbalance in CHF.
Inhibition of N-type calcium channels in cardiac sympathetic neurons attenuates ventricular arrhythmogenesis in heart failure

Abstract:Aims: Cardiac sympathetic overactivation is an important trigger of ventricular arrhythmias in patients with chronic heart failure (CHF). Our previous study demonstrated that N-type calcium (Cav2.2) currents in cardiac sympathetic post-ganglionic (CSP) neurons were increased in CHF. This study investigated the contribution of Cav2.2 channels in cardiac sympathetic overactivation and ventricular arrhythmogenesis in CHF. Methods and results: Rat CHF was induced by surgical ligation of the left coronary artery. Lentiviral Cav2.2-α shRNA or scrambled shRNA was transfected in vivo into stellate ganglia (SG) in CHF rats. Final experiments were performed at 14 weeks after coronary artery ligation. Real-time polymerase chain reaction and western blot data showed that in vivo transfection of Cav2.2-α shRNA reduced the expression of Cav2.2-α mRNA and protein in the SG in CHF rats. Cav2.2-α shRNA also reduced Cav2.2 currents and cell excitability of CSP neurons and attenuated cardiac sympathetic nerve activities (CSNA) in CHF rats. The power spectral analysis of heart rate variability (HRV) further revealed that transfection of Cav2.2-α shRNA in the SG normalized CHF-caused cardiac sympathetic overactivation in conscious rats. Twenty-four-hour continuous telemetry electrocardiogram recording revealed that this Cav2.2-α shRNA not only decreased incidence and duration of ventricular tachycardia/ventricular fibrillation but also improved CHF-induced heterogeneity of ventricular electrical activity in conscious CHF rats. Cav2.2-α shRNA also decreased susceptibility to ventricular arrhythmias in anaesthetized CHF rats. However, Cav2.2-α shRNA failed to improve CHF-induced cardiac contractile dysfunction. Scrambled shRNA did not affect Cav2.2 currents and cell excitability of CSP neurons, CSNA, HRV, and ventricular arrhythmogenesis in CHF rats. Conclusions: Overactivation of Cav2.2 channels in CSP neurons contributes to cardiac sympathetic hyperactivation and ventricular arrhythmogenesis in CHF. This suggests that discovering purely selective and potent small-molecule Cav2.2 channel blockers could be a potential therapeutic strategy to decrease fatal ventricular arrhythmias in CHF.
Cardiac Piezo1 Exacerbates Lethal Ventricular Arrhythmogenesis by Linking Mechanical Stress with Ca2+ Handling after Myocardial Infarction

Abstract:Ventricular arrhythmogenesis is a key cause of sudden cardiac death following myocardial infarction (MI). Accumulating data show that ischemia, sympathetic activation, and inflammation contribute to arrhythmogenesis. However, the role and mechanisms of abnormal mechanical stress in ventricular arrhythmia following MI remain undefined. We aimed to examine the impact of increased mechanical stress and identify the role of the key sensor Piezo1 in ventricular arrhythmogenesis in MI. Concomitant with increased ventricular pressure, Piezo1, as a newly recognized mechano-sensitive cation channel, was the most up-regulated mechanosensor in the myocardium of patients with advanced heart failure. Piezo1 was mainly located at the intercalated discs and T-tubules of cardiomyocytes, which are responsible for intracellular calcium homeostasis and intercellular communication. Cardiomyocyte-conditional Piezo1 knockout mice (Piezo1Cko) exhibited preserved cardiac function after MI. Piezo1Cko mice also displayed a dramatically decreased mortality in response to the programmed electrical stimulation after MI with a markedly reduced incidence of ventricular tachycardia. In contrast, activation of Piezo1 in mouse myocardium increased the electrical instability as indicated by prolonged QT interval and sagging ST segment. Mechanistically, Piezo1 impaired intracellular calcium cycling dynamics by mediating the intracellular Ca2+ overload and increasing the activation of Ca2+-modulated signaling, CaMKII, and calpain, which led to the enhancement of phosphorylation of RyR2 and further increment of Ca2+ leaking, finally provoking cardiac arrhythmias. Furthermore, in human induced pluripotent stem cell-derived cardiomyocytes (hiPSC-CMs), Piezo1 activation remarkably triggered cellular arrhythmogenic remodeling by significantly shortening the duration of the action potential, inducing early afterdepolarization, and enhancing triggered activity.This study uncovered a proarrhythmic role of Piezo1 during cardiac remodeling, which is achieved by regulating Ca2+ handling, implying a promising therapeutic target in sudden cardiac death and heart failure.
Stellate Ganglia and Cardiac Sympathetic Overactivation in Heart Failure

Abstract:Heart failure (HF) is a major public health problem worldwide, especially coronary heart disease (myocardial infarction)-induced HF with reduced ejection fraction (HFrEF), which accounts for over 50% of all HF cases. An estimated 6 million American adults have HF. As a major feature of HF, cardiac sympathetic overactivation triggers arrhythmias and sudden cardiac death, which accounts for nearly 50-60% of mortality in HF patients. Regulation of cardiac sympathetic activation is highly integrated by the regulatory circuitry at multiple levels, including afferent, central, and efferent components of the sympathetic nervous system. Much evidence, from other investigators and us, has confirmed the afferent and central neural mechanisms causing sympathoexcitation in HF. The stellate ganglion is a peripheral sympathetic ganglion formed by the fusion of the 7th cervical and 1st thoracic sympathetic ganglion. As the efferent component of the sympathetic nervous system, cardiac postganglionic sympathetic neurons located in stellate ganglia provide local neural coordination independent of higher brain centers. Structural and functional impairments of cardiac postganglionic sympathetic neurons can be involved in cardiac sympathetic overactivation in HF because normally, many effects of the cardiac sympathetic nervous system on cardiac function are mediated via neurotransmitters (e.g., norepinephrine) released from cardiac postganglionic sympathetic neurons innervating the heart. This review provides an overview of cardiac sympathetic remodeling in stellate ganglia and potential mechanisms and the role of cardiac sympathetic remodeling in cardiac sympathetic overactivation and arrhythmias in HF. Targeting cardiac sympathetic remodeling in stellate ganglia could be a therapeutic strategy against malignant cardiac arrhythmias in HF.
Correlation of Ventricular Arrhythmogenesis with Neuronal Remodeling of Cardiac Postganglionic Parasympathetic Neurons in the Late Stage of Heart Failure after Myocardial Infarction.

Abstract:Introduction: Ventricular arrhythmia is a major cause of sudden cardiac death in patients with chronic heart failure (CHF). Our recent study demonstrates that N-type Ca2+ currents in intracardiac ganglionic neurons are reduced in the late stage of CHF rats. Rat intracardiac ganglia are divided into the atrioventricular ganglion (AVG) and sinoatrial ganglion. Only AVG nerve terminals innervate the ventricular myocardium. In this study, we tested the correlation of electrical remodeling in AVG neurons with ventricular arrhythmogenesis in CHF rats. Methods and Results: CHF was induced in male Sprague-Dawley rats by surgical ligation of the left coronary artery. The data from 24-h continuous radiotelemetry ECG recording in conscious rats showed that ventricular tachycardia/fibrillation (VT/VF) occurred in 3 and 14-week CHF rats but not 8-week CHF rats. Additionally, as an index for vagal control of ventricular function, changes of left ventricular systolic pressure (LVSP) and the maximum rate of left ventricular pressure rise (LV dP/dtmax) in response to vagal efferent nerve stimulation were blunted in 14-week CHF rats but not 3 or 8-week CHF rats. Results from whole-cell patch clamp recording demonstrated that N-type Ca2+ currents in AVG neurons began to decrease in 8-week CHF rats, and that there was also a significant decrease in 14-week CHF rats. Correlation analysis revealed that N-type Ca2+ currents in AVG neurons negatively correlated with the cumulative duration of VT/VF in 14-week CHF rats, whereas there was no correlation between N-type Ca2+ currents in AVG neurons and the cumulative duration of VT/VF in 3-week CHF. Conclusion: Malignant ventricular arrhythmias mainly occur in the early and late stages of CHF. Electrical remodeling of AVG neurons highly correlates with the occurrence of ventricular arrhythmias in the late stage of CHF.

Abstract:Cardiac sympathetic overactivation is linked to ventricular arrhythmogenesis and is associated with mortality in patients with chronic heart failure (CHF). However, the sites and mechanisms responsible for increased cardiac sympathetic activity are still poorly understood. Our previous studies showed that CHF-elevated proinflammatory cytokine levels and macrophage expansion in stellate ganglia are critical factors for cardiac sympathoexcitation. Here we tested if activation of satellite glial cells (SGCs) contributes to macrophage expansion in stellate ganglia from CHF rats. Rat CHF was induced by surgical ligation of the left anterior descending coronary artery. SGCs and postsynaptic sympathetic neurons were respectively identified by anti-S100B and anti-tyrosine hydroxylase (TH) antibodies with immunofluorescence staining. As a marker of SGC activation, glial fibrillary acidic protein (GFAP) was measured by immunofluorescence double staining and Western blot analysis. Sympathetic neurons in stellate ganglia from both sham and CHF rats were totally surrounded by S100B-positive SGCs. However, However, a few GFAP-postive SGCs covered sympathetic neurons in sham stellate ganglia, whereas more GFAP-positive SGCs surrounded sympathetic neurons in CHF stellate ganglia. Western blot data showed that GFAP was elevated in stellate ganglia from CHF rats, compared to sham rats. From these results, we conclude that activation of SGCs in stellate ganglia is higher in CHF rats than that in sham rats, which might contribute to the neuroinflammation and sympathoexcitation in stellate ganglia.

Abstract:Atrial fibrillation (AF) frequently complicates congestive heart failure (CHF). The current study examines whether patients with AF and CHF have alternation in atrial connexins (Cxs) and Na+–H+ exchanger isoform 1 (NHE1). Left atrium was obtained from 29 patients of patients undergoing mitral valve replacement. The connexin 40 (Cx40) protein was significantly higher in AF and sever HF, whereas connexin 43 (Cx43) protein were markedly increased in AF and mild HF but downregulated in the patients with AF and sever HF. NHE activity is significantly greater in AF and sever HF with highest level of NHE1 protein. These change may induce a direct effect on the progressive deterioration of cardiac function in the fibrillation and failure heart.
Reduced N-Type Ca2+ Channels In Atrioventricular Ganglion Neurons Are Involved In Ventricular Arrhythmogenesis

Abstract:Background-Attenuated cardiac vagal activity is associated with ventricular arrhythmogenesis and related mortality in patients with chronic heart failure. Our recent study has shown that expression of N-type Ca2+ channel alpha-subunits (Ca(v)2.2-alpha) and N-type Ca2+ currents are reduced in intracardiac ganglion neurons from rats with chronic heart failure. Rat intracardiac ganglia are divided into the atrioventricular ganglion (AVG) and sinoatrial ganglion. Ventricular myocardium receives projection of neuronal terminals only from the AVG. In this study we tested whether a decrease in N-type Ca2+ channels in AVG neurons contributes to ventricular arrhythmogenesis.Methods and Results-Lentiviral Ca(v)2.2-alpha shRNA (2 mu L, 2x10(7) pfu/mL) or scrambled shRNA was in vivo transfected into rat AVG neurons. Nontransfected sham rats served as controls. Using real-time single-cell polymerase chain reaction and reverse-phase protein array, we found that in vivo transfection of Ca(v)2.2-alpha shRNA decreased expression of Ca(v)2.2-alpha mRNA and protein in rat AVG neurons. Whole-cell patch-clamp data showed that Ca(v)2.2-alpha shRNA reduced N-type Ca2+ currents and cell excitability in AVG neurons. The data from telemetry electrocardiographic recording demonstrated that 83% (5 out of 6) of conscious rats with Ca(v)2.2-alpha shRNA transfection had premature ventricular contractions (P<0.05 versus 0% of nontransfected sham rats or scrambled shRNA-transfected rats). Additionally, an index of susceptibility to ventricular arrhythmias, inducibility of ventricular arrhythmias evoked by programmed electrical stimulation, was higher in rats with Ca(v)2.2-alpha shRNA transfection compared with nontransfected sham rats and scrambled shRNA-transfected rats.Conclusions-A decrease in N-type Ca2+ channels in AVG neurons attenuates vagal control of ventricular myocardium, thereby initiating ventricular arrhythmias.

CDK5 Promotes Ventricular Arrhythmogenesis Through Phosphorylation of N‐type Calcium Channels in Cardiac Sympathetic Postganglionic Neurons

Abstract:Cardiac sympathetic overactivation is an important trigger of fatal ventricular arrhythmias in patients with chronic heart failure (CHF). Our previous study found that increased N‐type calcium (Cav2.2) currents in cardiac sympathetic postganglionic (CSP) neurons contributes to cardiac sympathetic overactivation and ventricular arrhythmogenesis in CHF rats. Cyclin‐dependent kinase 5 (CDK5), a proline‐directed serine/threonine kinase, has been reported to play an important role in regulation of calcium influx and neurotransmitter release through phosphorylation of Cav2.2α subunit. Our current study aims to investigate whether CDK5 induces cardiac sympathetic overactivation and increases the susceptibility to ventricular arrhythmias via phosphorylation of Cav2.2α in CSP neurons. Lentiviral‐CDK5 cDNA (1×107 TU/ml, 2μl) was in vivo transfected into bilateral stellate ganglia (SG) in sham rats. Nontransfected sham rats served as controls. Final experiments were performed at 9 days after gene transfection. Western blot data demonstrated that gene transfection with lentiviral‐CDK5 cDNA into SGs significantly increased protein expression of CDK5 in CSP neurons in sham rats, compared with nontransfected sham rats. CDK5 cDNA transfection in SGs increased phosphorylation level of Cav 2.2α in CSP neurons from sham rat. The power spectral analysis of heart rate variability in conscious rats demonstrated that overexpression of CDK5 in CSP neurons increased low frequency power and ratio of low frequency to high frequency (index of cardiac sympathetic activation). Additionally, transfection of CDK5 cDNA into CSP neurons markedly increased cardiac sympathetic nerve activity in anesthetized sham rats (45.3±6.4% in sham+CDK5 group vs. 29.2±2.1% in sham group). ECG data obtained from 24‐hour continuous telemetry ECG recording revealed that overexpression of CDK5 in CSP neurons induced heterogeneity of ventricular electrical activities in conscious sham rats, as evidenced by CDK5 cDNA‐elongated ventricular arrhythmia‐related ECG markers including QT interval, QT dispersion, and T‐peak to T‐end interval. More importantly, transfection with CDK5 cDNA into CSP neurons significantly increased the susceptibility to ventricular arrhythmias (arrhythmia score was 1.7±0.9 in sham+CDK5 group vs. 0 in sham group) in anesthetized sham rats. Cellular level electromechanical modeling and simulation of heart failure

Abstract:Objective: To investigate the mutation of cellular mechanical properties of both fast and slow contracting myocytes in heart failure and the effects of heart failure on the mechanical and electrophysiological function of the heart. Methods: By integrating cellular action potential model based on experimental data of heart failure with modified Hunter-McCulloch-ter Keurs (HMT) mechanical heart cell model, an electromechanical cardiac cell model was constructed. Results: The simulation results show that the differences of the electrical responses between failing cells and normal cells can cause slowing relaxation of the Ca 2+ transient, and the difference of the Ca 2+-TnC concentrations between fast and slow myocytes in failing hearts was more reduced than that in nonfailing hearts. Conclusions: Heart failure results in a decrease of ventricular contracting force, which might diminish the role of mechanoelectric feedback (MEF), then induce an increase of transmural action potential duration (APD) gradients. These might cause arrhythmia in heart failure. These results are in good accordance with experimental findings reported in the literatures and might motivate further research on modeling and simulation of heart failure at the tissue and the whole organ levels.
Small-conductance Ca2+-activated K+ Channels Promote J-wave Syndrome and Phase 2 Reentry

Abstract:BACKGROUND Small-conductance Ca2+-activated potassium (SK) channels play complex roles in cardiac arrhythmogenesis. SK channels colocalize with L-type Ca2+ channels, yet how this colocalization affects cardiac arrhythmogenesis is unknown. OBJECTIVE The purpose of this study was to investigate the role of colocalization of SK channels with L-type Ca2+ channels in promoting J-wave syndrome and ventricular arrhythmias. METHODS We carried out computer simulations of single-cell and tissue models. SK channels in the model were assigned to preferentially sense Ca2+ in the bulk cytosol, subsarcolemmal space, or junctional cleft RESULTS When SK channels sense Ca2+ in the bulk cytosol, the SK current (ISK) rises and decays slowly during an action potential, the action potential duration (APD) decreases as the maximum conductance increases, no complex APD dynamics and phase 2 reentry can be induced by ISK. When SK channels sense Ca2+ in thesubsarcolemmal space or junctional cleft, ISK can rise and decay rapidly during an action potential in a spike-like pattern because of spiky Ca2+ transients in these compartments, which can cause spike-and-dome action potential morphology, APD alternans, J wave elevation, and phase 2 reentry. Our results can account for the experimental finding that activation of ISK induced J-wave syndrome and phase 2 reentry in rabbit hearts. CONCLUSION Colocalization of SK channels with L-type Ca2+ channels so that they preferentially sense Ca2+ in the subsarcolemmal or junctional space may result in a spiky ISK, which can functionally play a similar role of the transient outward K+ current in promoting J-wave syndrome and ventricular arrhythmias.
Mechanisms of Phase-3 Early Afterdepolarizations and Triggered Activities in Ventricular Myocyte Models.

Abstract:Early afterdepolarizations (EADs) are abnormal depolarizations during the repolarizing phase of the action potential, which are associated with cardiac arrhythmogenesis. EADs are classified into phase-2 and phase-3 EADs. Phase-2 EADs occur during phase 2 of the action potential, with takeoff potentials typically above -40 mV. Phase-3 EADs occur during phase 3 of the action potential, with takeoff potential between -70 and -50 mV. Since the amplitude of phase-3 EADs can be as large as that of a regular action potential, they are also called triggered activities (TAs). This also makes phase-3 EADs and TAs much more arrhythmogenic than phase-2 EADs since they can propagate easily in tissue. Although phase-2 EADs have been widely observed, phase-3 EADs and TAs have been rarely demonstrated in isolated ventricular myocytes. Here we carry out computer simulations of three widely used ventricular action potential models to investigate the mechanisms of phase-3 EADs and TAs. We show that when the T-type Ca2+ current (ICa,T) is absent (e.g., in normal ventricular myocytes), besides the requirement of increasing inward currents and reducing outward currents as for phase-2 EADs, the occurrence of phase-3 EADs and TAs requires a substantially large increase of the L-type Ca2+ current and the slow component of the delayed rectifier K+ current. The presence of ICa, T (e.g., in neonatal and failing ventricular myocytes) can greatly reduce the thresholds of these two currents for phase-3 EADs and TAs. This implies that ICa, T may play an important role in arrhythmogenesis in cardiac diseases.
Arrhythmogenesis and contractile dysfunction in heart failure: Roles of sodium-calcium exchange, inward rectifier potassium current, and residual beta-adrenergic responsiveness

Abstract:Ventricular arrhythmias and contractile dysfunction are the main causes of death in human heart failure (HF). In a rabbit HF model reproducing these same aspects of human HF, we demonstrate that a 2-fold functional upregulation of Na(+)-Ca(2+) exchange (NaCaX) unloads sarcoplasmic reticulum (SR) Ca(2+) stores, reducing Ca(2+) transients and contractile function. Whereas beta-adrenergic receptors (beta-ARs) are progressively downregulated in HF, residual beta-AR responsiveness at this critical HF stage allows SR Ca(2+) load to increase, causing spontaneous SR Ca(2+) release and transient inward current carried by NaCaX. A given Ca(2+) release produces greater arrhythmogenic inward current in HF (as a result of NaCaX upregulation), and approximately 50% less Ca(2+) release is required to trigger an action potential in HF. The inward rectifier potassium current (I(K1)) is reduced by 49% in HF, and this allows greater depolarization for a given NaCaX current. Partially blocking I(K1) in control cells with barium mimics the greater depolarization for a given current injection seen in HF. Thus, we present data to support a novel paradigm in which changes in NaCaX and I(K1), and residual beta-AR responsiveness, conspire to greatly increase the propensity for triggered arrhythmias in HF. In addition, NaCaX upregulation appears to be a critical link between contractile dysfunction and arrhythmogenesis.
