Mo1491 – Novel Preoperative Nomograms Predict Microvascular Invasion and Lymph Node Metastasis in Hepatocellular Carcinoma

Abstract:atresia.However, utility of HIDA scan in preterm neonates is not well-established.Herein, we aim to (1) review the etiologies of neonatal cholestasis among term and preterm neonates with cholestasis, (2) identify clinical variables associated with a diagnosis of BA and non-BA and (3) investigate the utility of HIDA in predicting BA among preterm neonates.Methods: Retrospective cohort study of neonates born and investigated for neonatal cholestasis at 2 co-located neonatal and children facilities from January 2013 to December 2017.Demographic, clinical, laboratory variables and ultrasound features were collected to identify for variables associated with diagnosis of BA and non-BA via univariable and multivariable analysis.Sensitivity of specificity of HIDA in predicting BA were calculated for term and preterm neonates.Results: 139 neonates with cholestasis were identified (40 term; 99 preterm), of which 28 term and 24 preterm underwent HIDA investigation.Among the term neonates, the etiologies of cholestasis were BA (18%), intestinal failure related liver disease (IFALD) (13%), cytomegalovirus (1%), idiopathic neonatal hepatitis (1%).Among the preterm neonates, these proportions were IFALD (66%), CMV/sepsis related (5%), BA (2%), cystic fibrosis (2%).Incidence of BA among neonatal cholestasis was higher in term(1:6) than preterm(1:50) cohorts.(OR 10.29; 95% CI 2.06-49.97,P=0.0024) Variables significantly associated with BA on univariable regression analysis were: higher birth weight(OR P=0.002), presence of acholic stool(P=0.002),absent gallbladder(P=0.003)or abnormal gallbladder morphology(P=0.024).Variables significantly associated with non-BA on multivariable regression analysis were gestational age ≤32 weeks(P=0.046),total parenteral nutrition ≥14 days(P=0.009).In predicting BA, non-draining HIDA demonstrated a lower specificity (73% vs 90%) and lower PPV (25% vs 78%) in preterm compared to term neonates.Conclusions: Etiologies of cholestasis among preterm neonates differ from those in term neonates and neonatal cholestasis in preterm may not warrant extensive evaluations for exclusion for BA, given the lower prevalence of BA and lower utility of HIDA among preterm neonates.Preexisting diagnostic approach to neonatal cholestasis should be modified for preterm cohort, taking into accounts of the prevalence for each etiology, potential predictors and cost-efficiency.
