Abstract:Penicillin allergy labels in critical illness are associated with poor prescribing and patient outcomes. Many penicillin allergy labels are low-risk. While clinical decision rules (e.g., PEN-FAST) to safely guide direct oral challenge (DOC) have been validated outside of critical illness, their use in intensive care (ICU) is uncertain. We sought to examine the feasibility, safety, and efficacy of DOC for low-risk penicillin allergy in critical illness.
