Abstract:Coal chemical enterprises have many risk factors, and the causes of accidents are complex. The traditional risk assessment methods rely on expert experience and previous literature to determine the causes of accidents, which has the problems such as lack of objectivity and low interpretation ability. Analyzing the accident report helps to identify typical accident risk factors and determines the accident evolution rule. However, experts usually judge this work manually, which is subjective and time-consuming. This paper developed an improved approach to identify safety risk factors from a volume of coal chemical accident reports using text mining (TM) technology. Firstly, the accident report was preprocessed, and the Term Frequency Inverse Document Frequency (TF-IDF) was used for feature extraction. Then, the K-means algorithm and apriori algorithm were developed to cluster and for the association rule analysis of the vectorized documents in the TF-IDF matrix, respectively to quickly identify the hidden risk factors and the relationship between risk factors in the accident report and to propose targeted safety management measures. Using the sample data of 505 accidents in a large coal chemical enterprise in Western China in the past seven years, the enterprise accident reports were analyzed by text clustering analysis and association rule analysis methods. Through the analysis, six accident clusters and 13 association rules were obtained, and the main risk factors of each accident cluster were further mined, and the corresponding management suggestions were put forward for the enterprise. This method provides a new idea for coal chemical enterprises to make safety management decisions and helps to prevent safety accidents.

Abstract:This study aimed to provide a greater understanding of the systemic factors involved in coal mine accidents and to examine the relationships between the contributing factors across all levels of the system. Ninety-four extraordinarily major coal mine accidents that occurred in China from 1997 to 2011 were analyzed using the human factors analysis and classification system (HFACS). The empirical results showed that the frequencies of unsafe behaviors, inadequate regulation and failure to correct hidden dangers were the highest among five levels, 14 categories and 48 indicators, respectively. The odds ratio technique was applied to quantitatively examine the relationships between contributing factors. Various statistically significant associations were discovered and should receive greater attention in future attempts to develop accident measures. In addition, several strategies concerning the main contributing factors and routes to failure are proposed to prevent accidents from reoccurring in an organization.

Abstract:The risk factors affecting workers' unsafe acts were comprehensively identified by Human Factors Analysis and Classification System (HFACS) and grounded theory based on interview data and accident reports from deep coal mines. Firstly, we collected accident case and field interview data from deep coal mines issued by authoritative institutions. Then, the data were coded according to grounded theory to obtain relevant concepts and types. The HFACS model was used to classify the concepts and categories. Finally, the relationship between core and secondary categories was sorted out by applying a story plot. The results show that risk factors of unsafe acts of deep coal mine workers include environmental factors, organizational influence, unsafe supervision and unsafe state of miners, and the main manifestations of unsafe acts are errors and violations. Among them, the unsafe state of miners is the intermediate variable, and other factors indirectly affect risky actions of coal miners through unsafe sates. Resource management, organizational processes and failure to correct problems are the top three risk factors that occur more frequently in unsafe acts. The three most common types of unsafe act are unreasonable labor organization, failure to enforce rules, and inadequate technical specifications. By combining grounded theory and the HFACS framework to analyze data, risk factors for deep coal miners can be quickly identified, and more precise and comprehensive conceptual models of risk factors in unsafe acts of deep coal miners can be obtained.

Abstract:Coal mine industry is one of the typical high-risk industries with frequent accidents and numerous incentives. In order to expose the accident-causing factors in the process of coal mine production and explore their internal causal relationship, this paper took 883 coal mine accident reports in China from 2011 to 2020 as the original data. Firstly, driven by data, with the help of text mining technique and Apriori algorithm, a modified Human Factors Analysis and Classification System (HFACS) for coal mines was established and the strong association rules among the contributing factors were extracted. Then, the related hypotheses were put forward according to the frequent patterns within the elements. Finally, under the guidance of the theory-driven approach, hierarchical structure relationships in HFACS-CM framework were identified and analyzed from the perspective of human, machinery, environment and management. The results indicated that machinery and equipment factors, physical environment factors, and unsafe preconditions could directly affect employees' unsafe behaviors, while outside influences, organizational influences and unsafe supervision and could only exert influences on unsafe acts through other intermediary variables. Moreover, the unsafe preconditions had the greatest direct effect on unsafe acts; as for indirect effect and overall effect, the unsafe supervision was the most impactful factor.

Abstract:The still severe situation of work safety and the increasingly prominent issue of labor shortage are currently the key factors affecting the safety and sustainable development of coal industry. To fundamentally solve these problems, the construction of intelligent coal mines has become an inevitable trend. In the current construction context, intelligent coal mining operations rely more on human-system interaction, bringing new human error risks. The applicable human reliability analysis (HRA) method can effectively identify and evaluate the risks, and further improve the safety level of intelligent coal mines. However, research on HRA for intelligent coal mines is still in early stages, lacking applicable theoretical basis and methodological system. To make up for the limitations, focusing on the research object of intelligent coal mining face (ICMF) system, firstly, a cognitive model suitable for ICMF operations was constructed combining with the system structure and task characteristics, laying a cognitive theoretical foundation for initiating research on human safety of intelligent coal mines. Based on this, the ICMF-HRA variables including general human failure event (HFE), seven main crew functions (MCFs), eighteen crew activity primitives (CAPs), twenty-five crew failure modes (CFMs), and twenty-nine performance influencing factors (PIFs), and three qualitative dependency structures (i.e., HFE-MCF-CAP-CFM, CFM-PIF, and PIF-PIF) and six quantitative probability relationships between them are developed using fuzzy Bayesian network, which constitutes the ICMF-HRA model. Moreover, the application of this model is elaborated combining communication interruption event, confirming the availability and adaptability of this model. The ICMF-HRA model fills the HRA research gap of intelligent coal mines and can be used for predictive and retrospective analysis, providing decision support for risk prevention and disposal. This study is expected to establish theoretical basis and provide practical tool for improving human safety level of intelligent coal mines, further promoting the safety and sustainable development of coal industry.

Abstract:It has been revealed in numerous investigation reports that human and organizational factors (HOFs) are the fundamental causes of coal mine accidents. However, with various kinds of accident-causing factors in coal mines, the lack of systematic analysis of causality within specific HOFs could lead to defective accident precautions. Therefore, this study centered on the data-driven concept and selected 883 coal mine accident reports from 2011 to 2020 as the original data to discover the influencing paths of specific HOFs. First, 55 manifestations with the characteristics of the coal mine accidents were extracted by text segmentation. Second, according to their own attributes, all manifestations were mapped into the Human Factors Analysis and Classification System (HFACS), forming a modified HFACS-CM framework in China’s coal-mining industry with 5 categories, 19 subcategories and 42 unsafe factors. Finally, the Apriori association algorithm was applied to discover the causal association rules among external influences, organizational influences, unsafe supervision, preconditions for unsafe acts and direct unsafe acts layer by layer, exposing four clear accident-causing “trajectories” in HAFCS-CM. This study contributes to the establishment of a systematic causation model for analyzing the causes of coal mine accidents and helps form corresponding risk prevention measures directly and objectively.

Abstract:Accident analysis models play a significant role in accident investigation and analysis. Most traditional models, usually based on a chain of events, have serious limitations when used for complex systems, especially for coal mine accident. HFACS is a widely accepted accident cause model with complex structure and content. Based on HFACS and accident investigation need in China, a new systemic accident analysis method "Six-hierarchy model " is proposed. And the index database of accident causal factors at each hierarchy of coal mine accidents is established. This model provides clear guidance for the prevention of accidents from six levels, which helps to the targeted rectification after accidents and the improvement of safety management level. The source, application scope, advantages and disadvantages of HFACS and six-hierarchy model are compared. The "2.14 " gas explosion accident in Sunjiawan coal mine is analyzed by using HFACS and six-hierarchy model and the safety management suggestions are put forward. The study shows that the Six-hierarchy model provides a clear guide for the prevention of accidents from six levels. And it benefits the targeted rectification after accidents and the continuous improvement of safety management.

Abstract:Even in the context of smart mines, unsafe human acts are still an important cause of coal mine gas explosion accidents, but there are few models to analyze unsafe human acts in coal mine gas explosion accidents. This study tries to solve this problem through a risk assessment method of unsafe acts in coal mine gas explosion accidents based on Human Factor Analysis and Classification system (HFACS-GE) and Bayesian networks (BN). After verifying the reliability of HFACS-GE framework, a BN model of risk factors of unsafe acts was established with the Chi-square test and odds ratios analysis. After reasoning analysis, risk paths and key risk factors of unsafe acts were obtained, and preventive measures were granted. Based on the analysis of 100 coal mine gas explosion cases, the maximum probability of five kinds of unsafe acts of employees is 38%. Among the 22 risk factors, the mental state of employees has the greatest influence on the habitual violation of regulations, and the sensitivity value is 12.7%. This study can provide technical assistance for the risk management of unsafe acts in coal mine gas explosions.

Abstract:Human and organizational factors have been proven to be the prime causes of Chinese hazardous chemical accidents (HCAs). A modified version of the Human Factors Analysis and Classification System (HFACS), namely the HFACS-Hazardous Chemicals (HC), was developed to identify the human factors involved in Chinese HCAs. The '8.12' Tianjin Port fire and explosion, the costliest HCA in recent years, was reanalyzed using this framework, and the results were compared with the official accident inquiry report to determine their differences related to the identification of human and organizational factors. The study revealed that interacting human factors from different levels in Ruihai Company led to this catastrophe, and the inquiry report had limitations in the identification of human factors and the guidance for similar accident prevention. This study showed the applicability of the HFACS-HC in HCA analyses as well as the necessity to recommend this approach for future HCA investigations.

Abstract:Humans are at the core of the social-technical system, and their behavioral errors affect the reliability and safety of the entire system in varying degrees. Occupational accidents and large-scale industrial accidents are often attributed to human errors, accounting for more than 80% of accidents. In view of the complexity of systems and the coupling of elements, a new HFA method is proposed based on a complex network. According to system safety theory, a complex network is regarded as a network composed of humans, matters, environments, and management, and the basic structure of the HFA network is summarized. On this basis, a system safety method of HFA is developed which proposes a universal human error causation model. Moreover, a network analysis method for human errors is also presented, which is a comprehensive analysis of human errors that have occurred. Finally, the above methods are applied to gas explosion accidents that occurred in China. Results show that the two methods proposed are universal to all fields, and their combination improves the effectiveness of human error management and promotes the targeted, proactive, systematic, and dynamic prevention of critical nodes and paths from a holistic perspective.

Abstract:Case study of 300 typical coal mine accident indicates that coal miners’ unsafe behavior is the major internal reason causing coal mine accidents. Factors affecting coal miners’ unsafe behavior were identified because of human’s complexity. We built the relatively entire system of coal miners’ safety behavior and quantitatively analyzed Coal miners’ unsafe behavior system, then explained the path and divergence of impact factors within the system in detail. Management decision can be formulated to advance management level in coal mine industry accordingly.

Abstract:<p>In recent years, several accidents in confined spaces have threatened the safety of staff and property in industries. A statistical analysis of 120 fatal accidents involving confined space operations in China from 2008 to 2018 was conducted in this study. The causes and characteristics of confined space operations accidents (CSOAs) were summarized. Focusing on the impact of human factors on CSOAs, the HFACS-MCS (HFACS-Modified Confined Space Accident) model, which consists of 5 levels and 21 factors, was established based on HFACS (Human Factors Analysis and Classification System). The chi-square (χ2) test and the OR analysis were implemented to analyze the statistically significant correlations between adjacent levels in the model. Finally, three human influence paths in CSOAs were derived. The results show that inadequate safety culture, organizational process vulnerability, inadequate supervision, supervisory violations, decision errors, and operational violations are the principal causes of the accidents. Besides, hurried and ad hoc rescue often caused more casualties. The targeted pre-accident prevention and post-accident emergency rescue countermeasures are proposed to prevent the recurrence of CSOAs and secondary accidents.</p>

Abstract:Intelligent computing is transforming safety inspection methods and response strategies in coal mines. Due to the significant safety hazards associated with mining excavation, this study proposes a multi-source data based predictive model for assessing gas risk and implementing countermeasures. By examining the patterns of gas dispersion at the longwall face, utilizing both temporal and spatial correlation, a predictive model is crafted that incorporates safety thresholds for gas concentrations, four-level early warning method and response strategy are devised by integrating weighted predictive confidence with these correlations. Initially tested using a public dataset from Poland, this method was later verified in coal mine in China. This paper discusses the validity and correlation of multi-source monitoring data in temporal and spatial correlation and proposes a risk warning mechanism based on it, which can be applied not only for safety warning but also for regulatory management.

Abstract:To ensure the safety of coal mine drilling operation and reduce losses caused by accidents, this study proposes a fuzzy-reasoning-based early prediction method for in-hole accidents during underground coal mine drilling processes. First, the mechanism of in-hole accidents during underground drilling in coal mines was analyzed, and the changes in different accident-related parameters were summarized. Second, based on the suddenness of different accidents, they were distinguished into sudden-change and slow-change accidents, and the corresponding features were extracted from short- and long-timescale information, respectively. Subsequently, a rule base was constructed based on the analysis of field data and manual experience, and different accident occurrence probabilities were determined using fuzzy reasoning to realize the early prediction of in-hole accidents in the coal mine drilling process. Finally, WinCC was used to design the upper computer interface for in-hole accident prediction in underground drilling processes in coal mines. It displays the results of three types of in-hole accident prediction: drill bit failure, stuck pipe, and bit drop.
An Interactive Model among Potential Human Risk Factors: 331 Cases of Coal Mine Roof Accidents in China

Abstract:In order to explore optimal strategies for managing potential human risk factors, this paper developed an interactive model among potential human risk factors based on the development processes of accidents. This model was divided into four stages, i.e., risk latency stage, risk accumulation stage, risk explosion stage and risk residue stage. Based on this model, this paper analyzed risk management procedures and relevant personal’s responsibility in each stage, and then probed into the interactive mechanism among human risk factors in three aspects, i.e., knowledge, information and communication. The validity and feasibility of the model was validated by analyzing a coal mine roof accident in China. In addition, the contribution of different functional levels’ personnel in risk evolution was discussed. It showed that this model can effectively reveal the interactive mechanism of potential human risk factors, and can thus give significant insights into the development of risk management theories and practices. It also proves that the contribution of different functional levels’ personnel in the model is different. This can further help practitioners design enhanced Behavioral-Based Safety (BBS) intervention approaches which can have a more sustainable and persistent impact on corporate personnel’s safety behavior. Specific recommendations and suggestions are provided fundamentally for future BBS practices in the coal mine industry.
Contributory factors interactions model: A new systems-based accident model

Abstract:Understanding the interactions between contributory factors associated with accidents is important. However, the widely applied Swiss cheese model (SCM) and the human factors analysis and classification system (HFACS) oversimplify the causes of accidents and ignore the interactions among the various components of an entire system, which is the essence of systems thinking. A new systems-based accident model that reflects the nature of the accidents and can be explained by the underlying accident model must therefore be developed. This study aims to create a new accident model that represents the interactions between the contributory factors, which can offer a systems approach to conducting a comprehensive examination of accidents. We use data obtained from the official accident reports on 94 extraordinary major coal mining catastrophes that took place from 1997 to 2011 in China. The results reveal that all latent conditions, including external factors, organizational influences, unsafe supervision, and preconditions for unsafe behaviours can influence active failures, that is, unsafe behaviours. However, only the preconditions for unsafe behaviours can influence unsafe behaviours within the SCM and HFACS frameworks. Furthermore, latent conditions can affect each other within the contributory factors interactions model. This study provides a new systems-based accident model that allows us to examine and explain the relationships and linkages within and between complex sociotechnical systems and identify the common factors and interactions that cross human, external, and organizational factors.
Analysis of Factors Influencing Miners’ Unsafe Behaviors in Intelligent Mines Using a Novel Hybrid MCDM Model

Abstract:Coal mine accidents seriously affect people’s safety and social development, and intelligent mines have improved the production safety environment. However, safety management and miners’ work in intelligent mines face new changes and higher requirements, and the safety situation remains challenging. Therefore, exploring the key influencing factors of miners’ unsafe behaviors in intelligent mines is important. Our work focuses on (1) investigating the relationship and hierarchy of 20 factors, (2) using fuzzy theory to improve the decision-making trial and evaluation laboratory (DEMATEL) method and introducing the maximum mean de-entropy (MMDE) method to determine the unique threshold scientifically, and (3) developing a novel multi-criteria decision-making (MCDM) model to provide theoretical basis and methods for managers. The main conclusions are as follows: (1) the influence degree of government regulation, leadership attention, safety input level, safety system standardization, and dynamic supervision intensity exert the most significant influence on the others; (2) the causality of government regulation, which is the deep factor, is the highest, and self-efficacy displays the smallest causality, and it is the most sensitive compared to various other factors; (3) knowledge accumulation ability, man–machine compatibility, emergency management capability, and organizational safety culture has the highest centrality among the individual factors, device factors, management factors, and environmental factors, respectively. Thus, corresponding management measures are proposed to improve coal mine safety and miners’ occupational health.

Abstract:China-Tianjin Port fire and explosion on August 12, 2015, was a major accident that involved hazardous chemicals and resulted in 165 fatalities and 798 injuries. Three-system-based accident models, human factor analysis and classification system (HFACS), AcciMap, and system theoretic accident modeling and process (STAMP), were applied to identify contributory factors and relationships in the accident. The analysis outputs and usage of the three techniques were compared. The three-system accident models show several differences in terms of the emphasis on the models, system structure, classification of contributory factors, and interactions between system components. An important advantage of HFACS is the taxonomic nature, which can be easily applied in practical application. AcciMap provides a clear graphic representation of the causal flow of accidents, which is suitable for academic research. STAMP is suitable for both academic research and practical applications.
C-HFAMF: A New Way to Accident Analysis Considering Human Factor

Abstract:Reason's (1990) “Swiss cheese” model of human error integrates the human error perspectives into a single unified framework. The original description of his modal was geared toward academicians rather than practitioners. The Human Factors Analysis and Classification System, HFACS (Douglas A. Wiegmann and Scott A. Shappell, 2003) was specifically developed to define the latent and active failure implicated in Reason's “Swiss cheese” modal so it could be used as an accident investigation and analysis tool. But the hardware failure and human-machine interaction failure are not considered in HFACS. However, in complex systems, the human-machine interaction failure is the main reason for the accident, in this paper, based on the HFACS and the energy-based accident causation theory, Complex Human Factor Analysis and Classification Framework (C-HFACF) is proposed. It contains not only the human error, but also the failure to consider the human-machine interaction. The framework provides an effective way for accident analysis, which will help to effectively find out the key nodes and core events in the accident. Exploring human factors of major chemical accidents in China: Evidence from 160 accidents during 2011–2022

Abstract:Major chemical accidents (MCAs) usually cause multiple casualties, property losses, and environmental damage. MCAs in hazardous chemical production enterprises (HCPEs) in China have been frequent in recent years. Understanding past accidents is essential for accident prevention and risk mitigation. However, no accurate statistical analysis and systematic human cause investigation of MCAs in China is available. Hence, this study systematically identified the main characteristics and human factors of MCAs in China during 2011–2022 using official accident investigation reports. First, the 160 MCAs were investigated by mathematical and statistical methods involving eight main characteristics: evolution trend, spatial locations, accident types, domino effects involved, occurrence links, time volatility, hazardous chemicals involved, and enterprise scales. Second, based on the characteristics of MCAs and existing research on chemical accidents, the Human Factors Analysis and Classification System (HFACS) for Chemical Accidents (HFACS-CA), which consists of 26 cause categories and is suitable for the analysis of chemical accidents, was proposed on the basis of HFACS and other extended HFACS. Then, the selected 146 representative MCAs were analyzed using the HFACS-CA framework; the human factors (comprising human and organizational factors (HOFs) and outside factors) of MCAs in China were systematically explored. Finally, according to the main characteristics and human factors of MCAs, comprehensive recommendations for the prevention of MCAs involving enterprises, governments, and interested parties were proposed. This work can provide a useful human factor analysis tool for investigating chemical accidents and valuable information for preventing MCAs in China and other countries.
