Abstract:Background: Maternal exposure to ambient fine particulate matter (PM2.5) is a potential risk factor for pregnancy loss, but the extant findings are inconsistent. One reason for the inconsistency is the difficulty of distinguishing spontaneous from induced pregnancy losses, particularly in countries with planning policies. Objective: To examine the association between maternal PM2.5 exposure and spontaneous incident pregnancy loss in China. Methods: A total of 18,513 women of reproductive age was recruited from Jiangsu Province, China, in 2007. Among them, 2451 women reported 2613 valid records of incident pregnancies from 2007 to 2010. We used Cox regression to link the outcomes (live birth, spontaneous pregnancy loss, or induced abortion) of those incident pregnancies with maternal PM2.5 exposures, assessed using well-developed estimates of historical concentrations at the county level. Results: Among the 2613 incident pregnancies, 69 spontaneous pregnancy losses, 596 induced abortions, and 1948 live births occurred. According to the adjusted model, each 10-mu g/m3 increment in the average PM2.5 concentration during pregnancy was associated with a 43.3% (95% confidence interval, 6.6-92.5%) increased probability of spontaneous pregnancy loss. Advanced maternal age, a potential competing risk factor, weakened the association between PM2.5 and spontaneous pregnancy loss. The association was nonsignificant for unintended pregnancies. Conclusion: Maternal PM2.5 exposure was associated significantly with incident spontaneous pregnancy loss. Our findings provide insight into the harmful effect of air pollution on human reproduction.
Estimation of Stillbirths Attributable to Ambient Fine Particles in 137 Countries.

Abstract:Gestational exposure to ambient fine particles (PM 2.5 ) increases the risk of stillbirth, but the related disease burden is unknown, particularly in low- and middle-income countries (LMICs). We combine state-of-the-art estimates on stillbirths, and multiple exposure–response functions obtained from previous meta-analyses or derived by a self-matched case-control study in 54 LMICs. 13,870 stillbirths and 32,449 livebirths are extracted from 113 geocoded surveys from the Demographic and Health Surveys. Each stillbirth is compared to livebirth(s) of the same mother using a conditional logit regression. We find that 10 - µg/m 3 increase of PM 2.5 is associated with an 11.0% (95% confidence interval [CI] 6.4, 15.7) increase in the risk of stillbirth, and the association is significantly enhanced by maternal age. Based on age-specific nonlinear PM 2.5 –stillbirth curves, we evaluate the PM 2.5 -related stillbirths in 137 countries. In 2015, of 2.09 (95% CI: 1.98, 2.20) million stillbirths, 0.83 (0.54, 1.08) million or 39.7% (26.1, 50.8) are attributable to PM 2.5 exposure exceeding the reference level of 10 μg/m 3 . In LMICs, preventing pregnant women from being exposed to PM 2.5 can improve maternal health.

Abstract:OPS 54: Air pollution and pregnancy outcomes, Room 117, Floor 1, August 28, 2019, 1:30 PM - 3:00 PM Background: There is increasing evidence from epidemiological studies that maternal exposure to ambient fine particles (PM2.5) can trigger pregnancy loss. However, few of these studies were performed in the least developed countries of Africa, which have the highest incidences of pregnancy loss (including miscarriage and stillbirth). Methods: The study collected 42,952 mothers who reported both pregnancy loss and at least one normal delivery in the African Demographic and Health Surveys (1998–2016). Gestational exposure to PM2.5 was evaluated using a state-of-the-art estimator based on both satellite measurements and chemical transport model outputs. Each case of pregnancy loss was compared with the successful delivery control(s) of the same mother. We evaluated the association between gestational exposure to PM2.5 and occurrence of pregnancy loss using a conditional logistical model with multiple adjustments. Results: Each increment of 10 μg/m3 PM2.5 was associated with an excess risk (ER) of 12.2% (95% confidence interval [CI]: 10.7–13.7) for pregnancy loss, including miscarriage (ER = 12.5%, 95% CI: 10.9–14.2) and stillbirth (ER = 9.4%, 95% CI: 5.1–13.8). The association with miscarriage was robust, regardless of differences in adjusted covariates, exposure assessment approaches, control definitions, or subpopulations. For stillbirth, we found that maternal age and low socioeconomic status enhanced the effect of PM2.5. Conclusions: Our findings add to the related epidemiological evidence on the health impacts of poor air quality and support public health interventions to improve maternal health in Africa.
Association Between Ambient Air Pollutants and Preterm Birth in Ningbo, China: a Time-Series Study.

Abstract:BACKGROUND:Exposure to air pollutants has been related to preterm birth, but little evidence can be available for PM2.5, O3 and CO in China. This study aimed to investigate the short-term effect of exposure to air pollutants on risk preterm birth during 2014-2016 in Ningbo, China. METHODS:We conducted a time-series study to evaluate the associations between daily preterm birth and major air pollutants (including PM2.5, PM10, SO2, NO2, O3 and CO) in Ningbo during 2014-2016. A General Additive Model extend Poisson regression was used to evaluate the relationship between preterm birth and air pollution with adjustment for time-trend, meteorological factors and day of the week (DOW). We also conducted a subgroup analysis by season and age. RESULTS:In this study, a total of 37,389 birth occurred between 2014 and 2016 from the Electronic Medical Records System of Ningbo Women and Children's Hospital, of which 5428 were verified as preterm birth. The single pollutant model suggested that lag effect of PM2.5, PM10, NO2 reached a peak at day 3 before delivery and day 6 for SO2, and no relationships were observed for O3 and preterm birth. Excess risks (95% confidence intervals) for an increase of IQR of air pollutant concentrations were 4.84 (95% CI: 1.77, 8.00) for PM2.5, 3.56 (95% CI: 0.07, 7.17) for PM10, 3.65 (95% CI: 0.86, 6.51) for SO2, 6.49 (95% CI: 1.86, 11.34) for NO2, - 0.90 (95% CI: -4.76, 3.11) for O3, and 3.36 (95% CI: 0.50, 6.30) for CO. Sensitivity analyses by exclusion of maternal age < 18 or > 35 years did not materially alter our results. CONCLUSIONS:This study indicates that short-term exposure to air pollutants (including PM2.5, PM10, SO2, NO2) are positively associated with risk of preterm birth in Ningbo, China.
Estimation of pregnancy losses attributable to exposure to ambient fine particles in south Asia: an epidemiological case-control study

Abstract:Background Maternal exposure to ambient particulate matter (PM2.5) is associated with pregnancy loss (ie, stillbirth and miscarriage). South Asia has the highest burden of pregnancy loss globally and is one of the most PM2.5 polluted regions in the world. However, knowledge of the relevant exposure-response function for mothers is insufficient. Methods In this epidemiological case-control study, we collected data from Demographic and Health Surveys from India, Pakistan, and Bangladesh for the period 1998-2016 for women who reported at least one pregnancy loss and one or more livebirths. We assessed ambient exposure during gestation with satellite-based PM2.5; measurements for the period. To derive the exposure-response function, we did a self-compared case-control study in which each case of pregnancy loss was compared with a successful livebirth control or controls by the same mother. Using the estimated exposure-response function, we quantified pregnancy losses attributable to PM2.5 in the region for the period 2000-16 using a standard risk assessment approach. Findings We assessed data for 34 197 mothers, who had at least one pregnancy loss and one or more livebirth, of whom 26 282 (76.9%) were from India, 4228 (12.4%) were from Pakistan, and 3687 (10.8%) were from Bangladesh. The 34 197 cases of pregnancy loss were matched with 76 282 livebirth controls. The mean level of PM2.5; exposure was 56.00 pg/m(3) (SD 30.82) for cases and 54.57 pg/m(3) (31.73) for controls. After adjustment for maternal age, nonlinear terms for temperature and humidity, seasonal variation, and long-term trends, each 10 pg/m 3 increment in PM,., was associated with an odds ratio for pregnancy loss of 1.03 (95% CI 1.02-1.05). According to the estimated non-linear exposure-response function by age and urban residence versus rural residence, for the period 2000-16, an estimated 349 681 (95% CI 152932-489493) pregnancy losses per year were attributed to ambient air exposure of more than 40 pg/m(3) (the annual PM2.5, standard of India), accounting for 7.1% (95% CI 3.3-31.2) of the total annual pregnancy loss burden in south Asia for this period. However, our estimates could be biased because of the limitations of the data (eg, misclassification of induced and spontaneous pregnancy losses). Interpretation Our findings add to epidemiological evidence of the association between pregnancy loss and PM2.5. Suboptimal air quality contributes to a considerable fraction of total pregnancy loss in south Asia. Controlling PM2.5 pollution will promote maternal health in south Asia. The Associations of Particulate Matters with Fetal Growth in Utero and Birth Weight: A Birth Cohort Study in Beijing, China.

Abstract:Background: Previous studies examined the associations of particulate matters (PM) with fetal growth in utero or birth weight with inconsistent results, and few studies investigated that whether the associations of PM with fetal growth in utero also present at birth. We aimed to investigate the associations of PM with both fetal growth in utero and birth weight. Methods: We established a birth cohort (2014-2017) with 18,863 singleton pregnancies in Tongzhou Maternal and Child Hospital of Beijing, China. Maternal exposure to PMwith aerodynamic diameters <= 2.5 mu m and <= 10 mu m (PM2.5/PM10) during pregnancy was estimated using the inverse distance weighting method. Estimated birth weight (EFW) was assessed by ultrasound measurements and birth weight was measured at birth, which were both standardized as gestational-age- and gender-adjusted Z-score. EFW undergrowth, low birth weight (LBW) and small-for-gestational-age were defined as the categorized outcomes. Generalized estimating equations and generalized linear regression were used to examine the associations of PM with quantitative and categorized outcomes, controlling for temperature, greenspace and individual covariates. Results: A 10 mu g/m(3) increase in PM2.5 and PM10 were associated with lower EFW Z-score [-0.031, 95% confident interval (CI):-0.047,-0.016 and-0.030, 95% CI:-0.043,-0.017]. A 10 mu g/m(3) increase in PM2.5 was associated with lower birthweight Z-score (-0.035, 95% CI:-0.061,-0.010) and higher risk of LBW(OR= 1.240, 95% CI: 1.019, 1.508). These results remained robust in co-pollutant models and sensitivity analyses. We didn't find significant results in other analyses. Conclusions: The study identified an inverse association between PM and fetal growth in utero. The association between PM2.5 and fetal growth persisted from pregnancy to birth. This study supported that further actions towards controlling air pollution are strongly recommended for promoting early-life health. (C) 2019 Elsevier B.V. All rights reserved.
Associating Ambient Exposure to Fine Particles and Human Fertility Rates in China.

Abstract:Background: Adverse effects of ambient fine particles (PM2.5) on sperm quality and oocyte fertilization have been identified by previous research. However, insufficient human studies tested associations between PM2.5 and decreased fertility rates. Methods: We associated long-term exposure to PM2.5 and county-level fertility rates reported by 2010 census across China. Exposure assessments were based on PM2.5 maps (2009-2010) with a spatial resolution of 0.1 degrees derived from satellite remote sensing data from another published study. We used a Poisson regression to examine the relationship between PM2.5 and fertility rates with adjustment of potential confounders including county-level socioeconomic factors (e.g. sex ratio) and a spatially smoothed trend. Results: We found that fertility rates were significantly decreased by 2.0% (95% confidence interval: 1.8%, 2.1%) per 10 mu g/m(3) increment of PM2.5. We also found a geographical variation of the associations. Conclusions: The study add to epidemiological evidences on adverse effects of PM2.5 on fertility rates. (C) 2018 Elsevier Ltd. Associations of adverse pregnancy outcomes with high ambient air pollution exposure: Results from the Project ELEFANT

Abstract:BACKGROUND: Investigations on the potential effects of high air pollution exposure before pregnancy on adverse pregnancy outcomes are limited, and it is unknown whether air quality standards looser than that set by World Health Organization (WHO) still can provide sufficient protection pregnant women from adverse pregnancy outcomes.OBJECTIVES: To evaluate the potential effects of high ambient air pollution around pregnancy on preterm birth (PTB) and low birth weight (LBW), and assess the risk of PTB and LBW associated with air pollutants with reference to different air quality standards of WHO and China.METHODS: Our study leveraged 10,960 pregnant women from the Project ELEFANT. Daily average particulate matter with an aerodynamic diameter of ≤2.5 μm (PM2.5) and ≤10 μm (PM10), nitrogen dioxide (NO2), sulfur dioxide (SO2), carbon monoxide (CO) and ozone (O3) concentrations were collected based on Chinese Air Quality Reanalysis datasets. Hazard ratios (HR) of PTB and LBW were estimated for maternal PM2.5, PM10, NO2, SO2, CO and O3 exposures and related proportions of days with daily average air pollution concentrations exceeding air quality standards of WHO and China around pregnancy using Cox proportional hazards regression models with adjustment for potential confounders.RESULTS: Ambient PM2.5, PM10, NO2, SO2 and CO exposure during the before pregnancy and pregnancy period were both significantly and positively associated with increased risk of PTB, PTB subtypes and LBW. A 10% increase in proportion of days with daily average PM2.5 exceeding 25 μg/m3 over the entire pregnancy was most apparently associated with risk of PTB (HR, 12.66; 95% CI, 8.20-19.53) and LBW (HR, 17.42; 95% CI, 6.88-44.10) among all PM2.5 proportion variables based on different air quality standards.CONCLUSION: Air quality standards of WHO are necessary to be implemented to control for risks of adverse pregnancy outcomes associated with ambient air pollution in areas with high air pollution levels.

Abstract:Introduction Recent studies demonstrated that living in areas with high ambient air pollution may have adverse effects on pregnancy outcomes, but few studies have investigated its association with spontaneous abortion. Further investigation is needed to explore the acute effect and lag effect of air pollutants exposure on spontaneous abortion. Objective To investigate the acute effect and lag effect between exposure to ambient air pollutants and spontaneous abortion. Methods Research data of spontaneous abortion were collected from the Chongqing Health Center for Women and Children (CQHCWC) in China. The daily ambient air pollution exposure measurements were estimated for each woman using inverse distance weighting from monitoring stations. A time-stratified, case-crossover design combined with distributed lag linear models was applied to assess the associations between spontaneous pregnancy loss and exposure to each of the air pollutants over lags 0–7 days, adjusted for temperature and relative humidity. Results A total of 1399 women who experienced spontaneous pregnancy loss events from November 1, 2016, to September 30, 2019, were selected for this study. Maternal exposure to particulate matter 2.5 (PM 2.5 ), particle matter 10 (PM 10 ) nitrogen dioxide (NO 2 ), and sulfur dioxide (SO 2 ) exhibited a significant association with spontaneous abortion. For every 20 μg/m 3 increase in PM 2.5 , PM 10 , NO 2 , and SO 2 , the RRs were 1.18 (95% CI : 1.06, 1.34), 1.12 (95% CI , 1.04–1.20), 1.15 (95% CI : 1.02, 1.30), and 1.92 (95% CI : 1.18, 3.11) on lag day 3, lag day 3, lag day 0, and lag day 3, respectively. In two-pollutant model combined with PM 2.5 and PM 10 , a statistically significant increase in spontaneous abortion incidence of 18.0% ( RR = 1.18, 95% CI : 1.06, 1.32) was found for a 20 μg/m 3 increase in PM 2.5 exposure, and 11.2% ( RR = 1.11, 95% CI : 1.03, 1.20) for a 20 μg/m 3 increase in PM 10 exposure on lag day 3, similar to single-pollutant model analysis. Conclusion Maternal exposure to high levels of PM 2.5 , PM 10 , NO 2 , and SO 2 during pregnancy may increase the risk of spontaneous abortion for acute effects and lag effects. Further research to explore sensitive exposure time windows is needed.

Abstract:Evidence for effects of PM2.5 on systemic oxidative stress in pregnant women is limited, especially in early pregnancy. To estimate the associations between ambient PM2.5 exposures and biomarkers of lipid peroxidation and total antioxidant capacity (T-AOC) in women with normal early pregnancy (NEP) and women with clinically recognized early pregnancy loss (CREPL), 206 early pregnant women who had measurements of serum malondialdehyde (MDA) and T-AOC were recruited from a larger case-control study in Tianjin, China from December 2017 to July 2018. Ambient PM2.5 concentrations of eight single-day lags exposure time windows before blood collection at the women's residential addresses were estimated using temporally-adjusted land use regression models. Effects of PM2.5 exposures on percentage change in the biomarkers were estimated using multivariable linear regression models adjusted for month, temperature, relative humidity, gestational age and other covariates. Unconstrained distributed lag models were used to estimate net cumulative effects. Increased serum MDA and T-AOC were significantly associated with increases in PM2.5 at several lag exposure time windows in both groups. The net effects of each interquartile range increase in PM2.5 over the preceding 8 days on MDA were significantly higher (p < 0.001) in CREPL [52% (95% CI: 41%, 62%)] than NEP [22% (95% CI: 9%, 36%)] women. Net effects of each interquartile range increase in PM2.5 over the preceding 5 days on T-AOC were significantly lower (p = 0.010) in CREPL [14% (95% CI: 9%, 19%)] than NEP [24% (95% CI: 18%, 29%)] women. Exposure to ambient PM2.5 may induce systemic lipid peroxidation and antioxidant response in early pregnant women. More severe lipid peroxidation and insufficient antioxidant capacity associated with PM2.5 was found in CREPL women than NEP women. Future studies should focus on mechanisms of individual susceptibility and interventions to reduce PM2.5-related oxidative stress in the first trimester.

Abstract:Objective To quantitatively analyze and evaluate the health impacts of air pollution on the adverse pregnancy outcomes,such as preterm delivery,low birth weight,birth defects around Beijing.Methods The personal data of all delivery cases of Haidian Maternal and Child Health Hospital in July,2007-July,2009 were collected,the concentrations of the main three air pollutants: sulphur dioxide(SO2),nitrogen dioxide(NO2) and particulate matter(PM10),as well as the corresponding meteorological data.Logistic regression analysis was used to explore the health impacts of ambient air pollution on the adverse pregnancy outcomes of the exposed women.Results After adjustment for the influence of maternal parity,age,fetal gender, meteorological factors,with a 100 μg/m3 increase in level of SO2 during the first month after pregnancy,the odd ratio of the occurrence of low birth weight was 1.50(95%CI: 1.09-2.05);With a 100 μg/m3 increase in level of SO2 during the first trimester after pregnancy and the last month before delivery,the odd ratio of the occurrence of preterm delivery were 1.50(95%CI: 1.042.18),1.79(95%CI: 1.30-2.46) respectively;With a 100 μg/m3 increase in level of PM10 during the second month before delivery,the odd ratio of the occurrence of preterm delivery was 1.45(95%CI: 1.15-1.84);With a 100 μg/m3 increase in level of NO2 at the 3rd-8th week of pregnancy,the odd ratio of the occurrence of birth defect was 2.85(95%CI: 1.08-7.50).Conclusion SO2 exposure during the early stage of pregnancy and PM10 exposure during the late stage of pregnancy are associated with preterm delivery,NO2 exposure during the 3rd-8th week of pregnancy are associated with birth defect statistically.

Abstract:Exposure to fine particulate matter (PM2.5) during pregnancy has been associated with preterm birth (PTB). However, the existing evidence is inconsistent, and the roles of specific PM2.5 chemical constituents remain unclear. Based on the China Labor and Delivery Survey, we included birth data from 89 hospitals in 25 provinces in mainland China, and conducted a national multicenter cohort study to examine the associations of PM2.5 and its chemical constituents with PTB risk in China. We applied satellite-based models to predict prenatal PM2.5 mass and six main component exposure. Multilevel logistic regression analysis was used to examine the associations, controlling for sociodemographic characteristics, seasonality, and spatial variation. We observe an increased PTB risk with an increase in PM2.5 mass and the most significant association is found during the third trimester when the adjusted odds ratio (OR) per interquartile range increases in PM2.5 total mass is 1.12 (95% confidence Interval, CI: 1.05–1.20). Infants conceived by assisted reproductive technology (ART) show greater PTB risk associated with PM2.5 exposure (OR = 1.33, 95% CI: 1.05–1.69) than those conceived naturally (OR = 1.11, 95% CI: 1.03–1.19). We also find black carbon, sulfate, ammonium and nitrate, often linked to fossil combustion, have comparable or larger estimates of the effect (OR = 1.07–1.14) than PM2.5. Our findings provide evidence that components mainly from fossil fuel combustion may have a perceptible influence on increased PTB risk associated with PM2.5 exposure in China. Abstract:Objectives Studies related to air pollutants and spontaneous abortion in urban northwestern China are scarce, and the main exposure windows of pollutants acting on pregnant women are unclear. Study design Case-control study. Methods Data were collected from pregnant women in Tongchuan City from 2018 to 2019. A total of 289 cases of spontaneous abortion and 1156 cases of full-term labor were included and analyzed using a case–control study. Logistic regression models were developed to explore the relationship between air pollutants and spontaneous abortion after Chi square analysis and Air pollutant description. Results O 3 (odds ratio [OR] = 1.028) is a risk factor for spontaneous abortion throughout pregnancy. PM 2.5 (OR = 1.015), PM 10 (OR = 1.010), SO 2 (OR = 1.026), and NO 2 (OR = 1.028) are risk factors for spontaneous abortion in the 30 days before the last menstrual period. PM 2.5 (OR = 1.015), PM 10 (OR = 1.013), SO 2 (OR = 1.036), and NO 2 (OR = 1.033) are risk factors for spontaneous abortion in the 30–60 days before the last menstrual period. PM 2.5 (OR = 1.028), PM 10 (OR = 1.013), SO 2 (OR = 1.035), and NO 2 (OR = 1.059) are risk factors for spontaneous abortion in the 60–90 days before the last menstrual period. Conclusion Exposure to high levels of air pollutants may be a cause of increased risk of spontaneous abortion, especially in the first trimester of the last menstrual period.

Abstract:The adverse effects of air pollution during pregnancy have been studied intensively, but mainly utilizing linear and logistic models, which generally yield little information about how air pollution may change the distribution of birth weight in populations. We aimed to examine the effects of fine particulate matter (PM2.5) on quantiles of birth weight, and if effects were heterogeneous in different populations and regions. We used a prospective cohort study of 196 283 singleton term live births from 16 counties across China during 2014-2018. PM2.5 exposure for full gestation, each trimester and last gestational month were assessed by Inverse Distance Weighting interpolation. Linear and quantile regression were conducted to estimate associations between PM2.5 exposure and mean birth weight, as well as birth weight distribution, with birthweight z-score as the main outcome. Stratified analyses and Cochran Q tests were conducted by maternal and geographical characteristics. Each 10 mu g m(-3) increase in average PM2.5 over the entire pregnancy was associated with reduced birthweight z-score (-0.010, 95% CI: -0.015, -0.005) and birth weight (-3.21 g, 95% CI: -5.27, -1.15). In quantile regression, more pronounced effects were observed in lower and intermediate quantiles, with a decrease of 0.021 (95% CI: 0.033, 0.009) and 0.009 (95% CI: 0.015, 0.002) in the 5th and 50th quantiles of birthweight z-score, respectively. Additionally, we observed stronger associations among well-educated, migrant and primiparous mothers as well as in coastal areas. Maternal exposure to PM2.5 was associated with reduction in birth weight, especially for those with very low birth weight. Well-educated, migrant and primiparous mothers, as well as births in coastal areas may be more sensitive to PM2.5 in our study population. The results may be relevant to targeted public health interventions to reduce maternal exposure to air pollution.

Abstract:BACKGROUND:Gestational exposure to fine particulate matter (PM2.5) has been reported to be associated with an increased risk of fetal death in recent studies, but earlier studies in the past century have usually reported a non-significant association. As such, it remains unknown whether this adverse effect of PM2.5 exposure varies with time.METHODS:Nearly 49.2 million eligible birth and fetal death records from 1989 to 2004 were selected from the United States (US) birth and fetal death certificate datasets. For each record, the level of prenatal exposure to PM2.5 was taken as the average concentration in the mother's residential county during the entire gestational period, according to well-established estimates of monthly levels across the contiguous US. We first stratified the dataset by the month of the last menstrual period (LMP) and then independently evaluated the nationwide association between PM2.5 exposure and fetal death within each stratum using five typical logit models: unadjusted, covariate-adjusted, propensity-score, double robust, and diagnostic-score models. Finally, we conducted a meta-analysis to pool estimated LMP-specific associations and explored how the overall association varied by LMP month.RESULTS:Different models showed temporal heterogeneity in the estimated association between PM2.5 exposure and fetal death. According to the meta-analysis, double robust model estimates were more homogeneous than the rest, and thus the model outcome was recognized as the main result. For each 1-µg/m3 increase in prenatal exposure to PM2.5, the pooled odds ratio (OR) of fetal death was estimated to be 1.08 [95% confidence interval (CI): 1.05, 1.10]. The LMP-specific ORs exhibited a slightly increasing trend and a significant seasonal pattern. Compared with the pooled OR among samples with the LMP in spring, the estimates for summer, fall and winter were higher by 11.1% (95% CI: 6.2%, 16.3%), 27.8% (95% CI: 22.1%, 33.8%) and 28.8% (95% CI: 23.7%, 34.1%), respectively. We also found that temporal patterns in the association between PM2.5 exposure and fetal death could be explained by several population-level indicators or modifiers (i.e. ethnicity, maternal age, gestational weight gain, previous pregnancy of abnormal termination and diabetes).CONCLUSIONS:Prenatal exposure to PM2.5 can increase the risk of fetal death. The effects of PM2.5 exposure may be modified by complex factors, which leads to a time-varying association.

Abstract:Background The adverse effects of short-term exposure to PM 2.5 and its components on hospital admissions for threatened and spontaneous abortions (TSAB) are still controversial. Methods Data on daily hospitalizations for TSAB and PM 2.5 and its components, including sulfate (SO 4 2− ), nitrate (NO 3 − ), ammonium salt (NH 4 + ), organic matter (OM), and black carbon (BC), were collected from January 2015 to December 2021 (total 2,557 days) in five cities in China. Case-crossover analyses were conducted to investigate the short-term associations between PM 2.5 and its components and TSAB. Additionally, the modification effects by age (<35 and ≥35 years), season (cold and warm seasons), and the "Three-Year Action Plan to Win the Blue Sky Defense War" (before and after implementation) on the above associations were further conducted. Results For each 10 μg/m 3 (1 μg/m 3 for BC) increase, the strongest relative risks (95% confidence intervals) of hospitalization for TSAB were 1.011 (1.001–1.021) for PM 2.5 in lag02, 1.060 (1.003–1.120) for SO 4 2− in lag02, 1.035 (1.000–1.070) for NO 3 − in lag02, 1.065 (1.009–1.124) for NH 4 + in lag02, 1.047 (1.008–1.088) for OM in lag01 and 1.029 (1.005–1.054) for BC in lag02 (all P <0.05). Furthermore, significant modifying effects of age and the Action Plan were found. The effects of NO 3 − (lag2), NH 4 + (lag2), and BC (lag2) were more pronounced in mothers aged ≥35 years and the effects of PM 2.5 (lag4), NO 3 − (lag4), NH 4 + (lag4), OM (lag4), and BC (lag4) was more pronounced in the period before the Action Plan was implemented (all P modification <0.05). Conclusion Short-term exposure to ambient PM 2.5 and its components (SO 4 2− , NO 3 − , NH 4 + , OM, and BC) was related to increased risks of hospitalization for TSAB. The effects were more pronounced in mothers aged ≥35 years and the period before the Action Plan.

Abstract:BackgroundExposure to particulate matter with a diameter of less than 2·5 μm (PM2·5) during pregnancy has been linked to preterm delivery; however, the health effects of PM2·5 constituents remain unclear. We estimated the associations of prenatal exposure to PM2·5 and its constituents with preterm delivery in China.MethodsIn this national multicentre cohort study, we used China Labor and Delivery Survey data collected from 89 hospitals in 25 provinces of mainland China. Gestational and trimester-specific exposures to PM2·5 and six key constituents (organic carbon, black carbon, sulphate [SO42–], nitrate [NO3−], ammonium [NH4+], and soil dust), predicted by satellite-based models, were matched with the hospital addresses of pregnant women. Associations with preterm birth were examined by logistic regression models with province-level random effects.Findings62 895 singleton livebirth records from March 1, 2015, to Dec 31, 2016, were included, of which 5681 (9·0%) were preterm. Median PM2·5 concentration throughout pregnancy was 53 μg/m3 (IQR 39–68). In the fully adjusted model, the odds ratio (OR) for preterm birth was 1·09 (95% CI 1·01–1·18) for every IQR increase in PM2·5 exposure during the third trimester only, whereas PM2·5 exposures in the first and second trimesters and over the entire gestation period showed no significant associations. Stratified analyses revealed significantly increased risk from third-trimester PM2·5 exposure for fetuses conceived by assisted reproductive technology (OR 1·31 [95% CI 1·00–1·72]) compared with those conceived naturally. Ammonium (adjusted OR 1·12 [1·00–1·25] for third trimester, 1·53 [1·11–2·10] for entire gestation period), nitrate (1·16 [1·02–1·32] for first trimester, 1·17 [1·04–1·31] for third trimester, 1·43 [1·07–1·90] for entire gestation period), and soil dust (1·11 [1·03–1·19] for first trimester) exposures showed significant associations with preterm birth in at least one trimester after adjusting for PM2·5; whereas black carbon and sulphate exposures (during the third trimester only) were associated with preterm birth only in unadjusted analyses.InterpretationPM2·5 exposure increases risk of preterm birth in China, especially among fetuses conceived by assisted reproductive technology. Efforts to reduce PM2·5 exposure, particularly through targeting fossil fuel emissions, could help to prevent a substantial proportion of preterm births in China.FundingNone.

Abstract:Air pollution has been documented with a series of adverse pregnancy outcomes, yet their reproductive and developmental toxicity on human beings has not been fully elucidated. Here, we analyzed the geographic distribution of Jinan and examined its contribution to air pollution. After adjusting demographic variables and environmental co-pollutants, we built statistical models based on 424 couples and checked different air pollutants on their pregnancy outcomes. We find that Jinan is tightly surrounded by mountains from 3 of 4 sides, geographically resulting in a typical basin texture that hinders the diffusion of ambient pollutants. Of 424 pregnant women enrolled in this study, 17 subjects were diagnosed with preterm birth. Using air quality index (AQI) as an integrated indicator of PM10, PM2.5, SO2, NO2, CO, and O3, we found that each interquartile range (IQR) increase in AQI was associated with 11% increased odds of preterm birth. Also, elevating PM2.5, PM10, SO2, and O3 led to different increased risk levels of preterm birth. By running the generalized additive model analyses, the association of AQI and preterm birth was further confirmed. In conclusion, based on samples in Jinan, east China, prenatal exposure to multiple ambient pollutants is associated with reduced gestational age and increased risk of preterm birth.
Increment of Ambient Exposure to Fine Particles and the Reduced Human Fertility Rate in China, 2000-2010

Abstract:Epidemiological and toxicological studies suggest that exposure to ambient fine particles (PM2.5) can reduce human reproductive capacity. We previously reported, based on spatial epidemiology, that higher levels of PM2.5 exposure were associated with a lower fertility rate (FR) in China. However, that study was limited by a lack of temporal variation. Using first-difference regression, we linked temporal changes in FR and PM2.5 with adjustment for ecological covariates across 2806 counties in China during 2000-2010. Next, we performed a sensitivity analysis of the variation in the PM2.5 -FR association according to (1) geographic region, (2) indicators of the level of development, and (3) PM2.5 concentrations. Also, we quantified the reduction in the FR attributable to ambient PM2.5 in China for the first time. The FR decreased by 3.3% (1.2%, 5.3%) for each 10 mu g/m(3) increment in PM2.5 . The association varied significantly among the geographic regions, but not with the level of development. Nonlinearity analysis suggested a linear exposure-response function with an effect threshold of -8 mu g/m(3). We also found that comparing to the 2000 scenario, increment of PM2.5 in 2010 might result in a reduction of 2.50 (2.44, 2.60) infants per 1000 women aged 15-44 years per year in China. Our results confirm the statistical association between ambient particles and FR and suggest that poor air quality may contribute to childlessness in China. (C) 2018 Elsevier B.V.
