Vacuum erectile device (VED) therapy has become one of the major modalities for penile rehabilitation (PR) because of its low complication rate and relatively high compliance rate, along with being the only modality that prevents PS. However, its clinical benefit in the recovery of erectile function and the mechanism in preventing PS are not clear. This study explored the efficacy and molecular mechanism of VED in PR in the bilateral cavernous nerve crushed (BCNC) rat model. METHODS Adult Sprague-Dawley rats were randomly assigned into three groups: 1, sham surgery; 2, BCNC; 3, BCNC+VED (rat specific VED was applied for 5 minutes twice daily, Monday to Friday from day 10 after BCNC). Penile length and diameter were measured on a weekly basis. After 4 weeks of therapy, intracavernosal pressure (ICP) and mean arterial pressure (MAP) were measured under cavernous nerve stimulation. The penis was then processed for immunohistochemistry or histo-analysis of: HIF-1 alpha, eNOS, TGF-beta 1, & alpha-smooth muscle actin (ASMA), smooth muscle/collagen, and Terminal deoxynucleotidyl transferase dUTP nick end labeling assay for apoptosis. Penile tunica albuginea was processed for transmission electron microscope (TEM) analysis. RESULTS The ICP/MAP ratios were significantly increased by VED therapy in BCNC rats (0.53±0.06 at 5V; 0.65±0.03 at 7.5V) compared to the BCNC control (0.25±0.02 at 5V; 0.32±0.03 at 7.5V) (P<0.01). The VED therapy is effective in preventing PS induced by BCNC (Penile shortening: BCNC group 2.45±0.48 mm; VED group 0.03±0.13 mm; P< 0.01. Penile diameter reduction: BCNC group 0.46±0.04 mm; VED group 0.08±0.03 mm; P<0.01. ). HIF-1 alpha, TGF beta 1, collagen and apoptosis were decreased while eNOS, ASMA were increased by VED therapy in BCNC rats versus BCNC only. TEM showed BCNC induced collagen disorganization in penile tunica albuginea, VED therapy dramatically reversed it. CONCLUSIONS ED and PS after BCNC were protected by VED therapy via anti-hypoxic, anti-apoptotic mechanism.
