Increased Bronchoalveolar Lavage Fluid Peroxiredoxin 6 Levels in Acute Respiratory Distress Syndrome

Abstract:Objective: Peroxiredoxins (Prdxs), the antioxidant enzymes, have been recently recognized as the damage-associated molecule pattern molecules (DAMPs) when they were released to extracellular compartments. Methods: Bronchoalveolar lavage fluid (BALF) of clinical diagnosed ARDS patients and controls were collected at the 1st, 3rd, 7th and 14th day during their treatment. Results: Twelve ARDS and 8 control patients were enrolled in our study. Five patients died. The number of patients in mild(200< PaO2/FiO2≤300mmHg), moderate(200< PaO2/FiO2≤300mmHg) and severe group (PaO2/FiO2≤100mmHg) were 3, 5 and 4, respectively. The BALF Prdx6 levels in ARDS patients(9.455±2.6ng/ml) were significantly higher than those in controls(2.65±0.21ng/ml). On the 1st day of ARDS, patients already presented significantly higher BALF Prdx6 levels (12.47±3.96 ng/ml) as compared to that of control. The BALF Prdx6 levels continuously increased to peak on 3rd day of ARDS(16.18±9.51 ng/ml) and then markedly decreased to similar level of controls on the 7th day (2.25±0.04 ng/ml) and the 14th day of ARDS(2.163±0.0087 ng/ml). The BALF Prdx6 levels in severe ARDS patients were higher than those in mild and moderate ARDS patients but not significantly. Although the correlation between BALF Prdx6 levels and PaO2/FiO2 ratios was not significant, an obvious negative correlation trend was presented. The BALF Prdx6 levels was positively correlated to the percentage of blood lymphocytes and BALF high mobility group box-1 (HMGB1) levels. Conclusion: The BALF Prdx6 levels are significantly higher in ARDS patients. The extracellular Prdx6 may play a pivotal rolein the pathogenic inflammation process of ARDS.
