Abstract:Objectives: The association between arsenic exposure and prevalence of hypertension is controversial. This study aims to evaluate the association between chronic exposure to arsenic and the prevalence of hypertension. Methods: We evaluated the association of arsenic exposure in relation to the prevalence of hypertension in 2,151 participants enrolled in the Hunan resident health and environmental factors study (HEAVY). Demographic and behavioral characteristics, such as diet, exercise and mental condition assessment, were obtained via questionnaire. Blood pressure was measured three times with OMRONTM electronic sphygmomanometer HEM7200. Baseline Arsenic level was measured from both urine and hair samples. Mixed-effect models were used to estimate the association of baseline arsenic level and blood pressure. Results: In the HEAVY cohort participants, the prevalence of hypertension in the arsenic exposure area (41.3%) was obviously higher than that in the control area (23.2%). Individuals in the highest quartile of urinary creatinine-adjusted arsenic level had a greater HR with hypertension compared to those in the lowest quartile group (1.91, 95%CI: 1.34, 2.72). Hair arsenic level was positively correlated with systolic blood pressure and diastolic blood pressure in the elderly (>65 years old). Moreover, a 1 μg/kg increase in hair arsenic concentration was associated with 1.67 mmHg higher systolic blood pressure (95% CI: 0.02, 3.33; p < 0.05). Conclusion: Our findings suggest that a high arsenic level in vivo was an independent risk factor for hypertension in Chinese rural population. More cohort evidence will facilitate the evaluation of the role of arsenic exposure in the development of hypertension.
