A16377 Survey on Hypertension Management in the Communities in Shanghai

Abstract:Objectives: To study equipments of hypertension management,drug distribution and physician skills in Shanghai community health centers (CHC) Methods: Used questionnaires,on-the-spot investigations,field surveys to investigate clinic sphygmomanometer,ambulatory blood pressure (ABP) machine,antihypertensive drug distribution;And random extracted the screening and baseline data among 1000 resistant hypertensive patients in 13 primary care hospitals from April 2016 to December 2017. The data included 1737 patients with high blood pressure (BP),3474 BP readings,and 923 antihypertensive prescriptions. Results: 1. Before and after hierarchical management,zero preference of BP decreased 12.2%(37.1% vs. 24.9%) in CHC.2. Before and after hierarchical management,ABP machines increased 141%(17 vs. 24),patients used ambulatory blood pressure monitoring (ABPM) increased 730%(30 vs. 219),and six communities has realized remote ABP transmission. The physicians started to use ABP machines and realized the remote transmission in the function community,and 610 patients used ABPM.3. Antihypertensive drugs distribution was in good condition in 13 CHC. There was an average of 30 species in every hospital,but lack of second-line drugs.4. There were 171 question prescriptions in 923 antihypertensive prescriptions,which accounted for 18.5%.5. Five kinds of antihypertensive agents were used in 923 antihypertensive prescriptions. Used two kinds was 38.1%,and three or more drugs was 61.9%. Antihypertensive drugs used in the top three were class A (805 cases,42.7%),CCB (682 cases,36.1%),DD (510 cases,27.0%), α-blockers used rarely. Conclusion: BP monitoring was not standard in the CHC;The application of ABP was available;Antihypertensive drugs was lack of second-line drugs;The general practitioners’ skills on hypertension management needed to be improved.
