A Comparative Study Between Normothermic Continuous and Intermittent Liver Inflow Occlusion Causing Liv-Er Injury and Protection of Injury by Reduced Glutathione

Abstract:Objective To compare normothermic continuous and intermittent live inflow occlusion in inflicting ischemia/reperfusion injury and explore the protective effects of reduced glutathione (GSH) on the injury. Methods Thirty-two SD rats were equally divided into 4 groups according to the occlusion manner and with or without GSH, namely, continuous occlusion with and without GSH (CP and CA) groups, intermittent occlusion with and without GSH (IP and IA) groups. The occlusion continuously lasted for 40 min in CP and CA groups and two cycles of 20 min with an interval of 5 min in IP and IA groups. The time of reperfusion was 60 min. Experimental parameters included malonaldehyde (MDA) and Cu/Zn superoxide dismutase (SOD) contents in liver tissue, ALT and AST activities in the serum and morphological changes of liver tissue. Results In the same group, ALT and AST activities were significantly higher before than after the occlusion (P0. 05). However, there were no significant differences in MDA and Cu/Zn SOD contents before and after the occlusion (P0. 05). The values of all the parameters after the reperfusion were markedly different from those before the occlusion (P0. 05). The values were not remarkably different before the reperfusion (P0. 05) but significantly different after the reperfusion (P0. 05) between CP group and IP group and between CA group and IA group. As for morphology, cellular damage was seen before the occlusion and recovered to some extent after the occlusion in each group. The structure of hepatocytes basically returned to normal in CP and IP groups but a little vacuolar degeneration still remained in CA and IA groups. No irreversible injury presented in every time phase in each group. Conclusions Both normothermic continuous and intermittent occlusions for 40 min can cause reversible liver injury. The two occlusion methods are not significantly different in results of he extent of the liver injury. The continuous occlusion should be regarded as a proper method in hepatectomy in case of this occlusion period. The intraoperative application of GSH has a remarkable effect against the liver injury. Therefore, this might be used as a new protective method.
