Abstract:Objective In order to propose policy suggestion on perfecting the administrative and operational mechanism of the primary prevention and healthcare system,we try to find out the impact on the performance of primary prevention and public hygiene surveillance in Longgang District of Shenzhen when we decentralize the authority to street level.Method This research is based on a questionnaire survey and in-depth interview that was conducted to 12 branch offices of Health Surveillance in Longgang District of Shenzhen,investigating the current status and performance of these branches.Result The quality of the staff working in these offices is quite low and some fundamental equipment is outdated.Moreover,the business income is the major financial support of these offices.Much emphasis has been put on disease prevention but the duty of surveillance is set back.Conclusion The decentralization of authority weakens the capacity of health management and goes against the overall planning of health system.In addition,involving non-professional work force and the increasing health-unrelated tasks lead to the ineffectiveness of performance.We suggest that it is necessary to clarify the administrative and operational mechanism of primary healthcare and prevention,implement the industry-wide management based on the government-led principle,increase the public health investment and enhance the quality of human resource.

Abstract:Lessons for building the health security system of the whole nation learned from public health emergency and corresponding policy measures were discussed in the paper. The SARS crisis has shown the weaknesses of the existing medical security system. It was recommended that we should transform the function of government, strengthen the legislation construction, improve the organization and management regulations so as to set up the system of health security of the whole nation with features of fund-raising in diversified ways, multiple forms of security methods and multi-channels of security level.

Abstract:The government and general public has paid unprecedented attention to the public health after the outbreak of SARS in 2003. Based on the first and the second round of Three-year Action Plan for Public Health System Construction, the public health system in Shanghai has been gradually improved. The city's public health security has been increasingly consolidated and people's main health status and indicators have been constantly improved. Nowadays, the third round of Three-year Action Plan will be started. This paper will review the development of public health system in Shanghai in recent ten years, analyze the possible problems and challenges in the future and put forward the development direction and strategy of the system.

Abstract:Shanghai is facing three underlying problems in the health reform. First is that the relationship between government and market in the health sector is unclear, second is that the environment for the health settings , owners of all kinds to deliver medical care needs to be optimized, and third is that the system and mechanism of public hospitals needs to be improved. China's public health-care system: facing the challenges

Abstract:The severe acute respiratory syndrome (SARS) crisis in China revealed not only the failures of the Chinese health-care system but also some fundamental structural deficiencies. A decentralized and fragmented health system, such as the one found in China, is not well-suited to making a rapid and coordinated response to public health emergencies. The commercial orientation of the health sector on the supply-side and lack of health insurance coverage on the demand-side further exacerbate the problems of the under-provision of public services, such as health surveillance and preventive care. For the past 25 years, the Chinese Government has kept economic development at the top of the policy agenda at the expense of public health, especially in terms of access to health care for the 800 million people living in rural areas. A significant increase in government investment in the public health infrastructure, though long overdue, is not sufficient to solve the problems of the health-care system. China needs to reorganize its public health system by strengthening both the vertical and horizontal connections between its various public health organizations. China's recent policy of establishing a matching-fund financed rural health insurance system presents an exciting opportunity to improve people's access to health care.

Abstract:Objective To study current situation and existed problems in the public health emergency alert and response system so as to formulate more advices to consummate it and help to establish a more reasonable system adapting to our country. Methods A qualitative study was carried out among CDC working staff including two focus group discussions(FGD). Results Although government had put more emphasis on public health,increasing its devotion and strengthening the alert and response capability after SARS outbreak, some problems still existed, such as ① absence of correlative laws and rules; ② lack of government devotion; ③ insufficient utilization of surveillance information; ④ lack of the canonical response guidance. Conclusions With regard to the building up of the public health emergency alert and response system, the following proposals are put forward: increasing payout on public health; consummating the construction of disease surveillance network; affording more education and training to correlative staff engaged in the disease control to improve their abilities.

Abstract:The article discusses key episodes connected with the formation of public health care system in China in the last years of the Qing Empire and the first years of the Chinese Republic. It is shown that this process was part of a more general process of modernization of the Chinese statehood, the reason for which was the interference of foreign countries in the internal affairs of China. In an effort to get rid of the image of the “sick man of Asia” that accompanied the empire in its historical bankruptcy, state dignitaries, intellectuals and the nationalist-minded medical community proclaimed the need to reform the state system. They saw as an essential part of the reform process the creation of an effective health care system, and the condition for this was the use of the achievements of Western medical science and sanitation. Part of the processes under consideration were measures to control the Manchurian plague epidemic, to establish modern medical institutions, to educate the population in sanitary matters and to employ reputable medical specialists in the service of the government.

Abstract:<h3 class="Heading">Background</h3><p class="Para">Public health system plays a vital role in the development of health sector in China and protects the health of Chinese people. However, there are few comprehensive reviews and studies focusing on its evolution and reform. It is worthwhile to pay attention to the public health development in China, given that the history and structure of public health system have their own characteristics in China.</p><h3 class="Heading">Methods</h3><p class="Para">The study is a retrospective review of the development public health over seven decades in China. It presents the findings from some national or provincial survey data, interviews with key informants, reviews of relevant published papers and policy contents.</p><h3 class="Heading">Results</h3><p class="Para">This study identified four key stages that public health experienced in China: the initial stage centering on prevention, the stage of deviation with more attention to treatment but little to prevention, the recovery stage after SARS(Severe Acute Respiratory Syndromes) Crisis, and the new stage to an equitable and people-centered system. In the latest stage, the National Basic Public Health Service Program (NBPHSP) is implemented to respond the threat of noncommunicable diseases (NCDs) and has achieved some initial results, while there are still many challenges including service quality, poor integration among service items and IT system, lack of quality professionals and insufficient intersectoral endeavor.</p><h3 class="Heading">Discussion</h3><p class="Para">There are unique Chinese wisdom and remarkable achievements as well as twists and turns on the development of China's public health. Prevention-first, flexible structure of the system, multi-agency collaboration and mass mobilization and society participation are the main experience of public health in early stage. Despite twists and turns since 1980s, public health system in China shows substantial resilience which may be from the government's continuous commitment to social development and people's livelihoods and its flexible governance. In 2010s, in order to achieve the well-off society, Chinese government pays unprecedented attention to health sector, which bring a new wave of opportunities to public health such as remaining the NBPHSP for priority. The evolution and reform of China's public health is based on its national condition, accumulates rich experience but also faces many common worldwide challenges. Getting this development and reform right is important to China's social and economic development in future, and China's experience in public health may provide many lessons for other countries.</p><h3 class="Heading">Conclusion</h3><p class="Para">Public health in China needs to focus on prevention, strengthen multi-agency coordination mechanism, improve the quality of public health services in the future.</p>

Abstract:The outbreak of severe acute respiratory syndrome ( SARS) in 2003 brought a catastrophe to mankind and made people to consider the construction of public health system in the society. Abstract:The spread of SARS in China as well as the world has created a global public health crisis with multiple implications. How to control SARS has not only become a test of leadership for Chinese government, it has also created a challenge for China's crisis management system. This paper first analyze the evolution of SARS crisis in light of the crisis management framework, three salient features of SARS crisis were identified. Then, a general framework of crisis management is proposed. Four major components are discussed in details, the role of government, media and public communication, social network, and basic legal framework. Finally, some general lessons from the SARS crisis are discussed.

Abstract:Based on systematic review of the role of clinical treatment, disease control and scientific research, and combining with the problems exposed by the COVID-19 epidemic, suggestions were proposed to reform and improve the prevention and treatment system for major epidemics diseases in China. In clinical treatment, it is necessary to enhance clinical staff’s awareness of law-based reporting, capabilities of syndromic surveillance, the use of infectious diseases reporting systems, and to improve national or local monitoring platforms for emerging infectious diseases detection. In terms of disease control, it is important to provide authorities to disease control departments through laws and regulations, improve the quantity and quality of related human resources, and strengthen the applied research and technical readiness targeted infections disease prevention, control and emergency preparedness. As for scientific research in major epidemic response, it is essential to strengthen research projects guided by national requests, build research institutions that can "make a final decision" , establish national and regional key laboratories, and strengthen strategic technical reserve for emergency disease control and treatment.

Abstract:Public health is a social welfare undertaking, and the government takes the lead in this area given its responsibility to guarantee and provide the people’s interests. The explicit goal of China’s healthcare reform is to establish universal coverage that provides safe, effective, convenient, and affordable basic health services to all urban and rural residents [1]. In our opinion, China’s healthcare reform has the following characteristics: establishing a health system to ensure the social welfare and fairness; emphasis on the foundation (system building), strengthening primary healthcare (human resource development) and establishing mechanisms (health service delivery and health financing); and gradually realizing equitable access to public health services. From the above, it is clear that the government attaches great importance to its leadership role in health. This provides a strong policy environment for the future development of public health in China.
