Hemodialysis with Plasma-Based Dialysate -- a New Blood Purification Modality for Hyperbilirubinemia

Abstract:Objective:We proposed a new blood purification modality——hemodialysis with plasma based dialysate(HD PBD) for patients with hyperbilirubinemia, and evaluated the effect of this treatment in the removal of serum bilirubin. Methodology:Eleven patients with hyperbilirubinemia were involved in this study. Four of them were treated with high volume hemofiltration (HVHF) for 12 hours, 3 patients were treated with hemoperfusion(HP) using macroporous resin sorbents, the other four were treated with a this new modality——PHD for 6 hours. During a session of PHD, 4 000 ml frozen plasma was circulated in the dialysate route at flow rate of 6 000 ml/hr, and a BLS816G filter was used as the dialyzer. Serum total bilirubin(TB), direct bilirubin(DB) and indirect bilirubin(IB) were monitored before and after treatment in all the patients. TB, DB and IB concentrations were determined in the ultrafiltrates of the patients treated with HVHF, and in the wasted plasma dialysate in the patient treated with PHD. Results:After a 12 hour's treatment of HVHF, the mean reduction of serum TB, DB, IB is (10 4±4 2)%,(10 0±6 3)% and (10 1±7 5)%,respectively, and the average concentration of TB, DB and IB in the ultrafiltrates is (3 33±0 96),(1 98±0 81) and (1 35±0 26) μmol/L, respectively. After a session of HP treatment, the mean reduction of serum TB, DB and IB is (14 4±3 2)%,(14 4±4 0)% and (14 8±3 2)%, respectively. After a 6 hour's treatment with PHD, the mean reduction of serum TB, DB and IB is (21 5±5 3)%, (20 5±2 9)% and (24 1±14 0)% respectively. Conclusion:PHD, a newly proposed modality for patients with hyperbilirubinemia, is more effective in the removement of serum bilirubins than other modalities as HVHF or as HP.
