Surgical Reduction and Bone Implant Repair of the Posttraumatic Orbitozygomatic Deformities.

Abstract:To Determine the clinical characteristics, treatment modalities and repair material of the posttraumatic orbitozygomatic deformities. Methods:Thirteen patients with orbitozygomatic complex(OZC) fractures that had 3 months to 2 years follow-up were studied.Demographic information and methods of treatment were obtained from the medical record.Quality of reduction and cosmatic outcome were assessed by clinical assessment and postoperative images. Results:Of the thirteen patients,twelve were male.Incidence of orbitozygomatic asymmetry,enophthalmos,diplopia were 92.3% ,46.2%,38.5% respectively.All patients required surgical intervention and bone grafting repair. The most frequently used approach was the coronal incision in combination with the maxillary vestibular incision and sometimes the lower eyelid incision or medial canthus insision was used simultaneously.The rescontructive techniques included osteotomy of zygoma and orbital bone,orbitozygomatic and nasal bone grafting,reposition of the canthal ligaments. The most frequently used grafting material was demineralized bone( n =8).Other material were autogenous bone such as ilium( n =3) ,calvarium( n =2) and rib( n =1).One patients still showed slightly post surgical enophthalmos and diplopia while another showed enophthalmos only.three patients regained improved facial appearance but not very satisfactory. Conclusions:Surgical reduction combined with bone grafting is a satisfactory method for the correction of delayed orbitozygomatic deformities.Autogenous bone are still the ideal repair materials and demineralized bone implants is a safe grafting material too.
