Effects of hypoxia-inflammation microenvironment in hepatocellular carcinoma development: hif-1 alpha/IL-1 beta loop enhances epithelial-mesenchymal transition of hepatocellular carcinoma

Abstract:Introductions: Hypoxic environment is a prominent feature of solid tumor including hepatocellular carcinoma (HCC), which is also a prototype of inflammation-related cancer. Our previous studies revealed that hypoxia promoted epithelial-mesenchymal transition (EMT) in HCC cells; however, to what extent the hypoxia-inflammation microenvironment in HCC regulates tumor-associated macrophages (TAMs) function remains unclarified. Here we aim to investigate the potential roles of proinflammatory cytokines in crosstalk between TAMs and tumor cells in hypoxia-inflammation microenvironment. Methods: Both protein level and mRNA level of IL-1β, IL-6 and TNF-α were detected under hypoxia and normoxia conditions in THP-1 cell line and also in serum specimens of HCC patients. Western blot, transwell migration assay, quantitative polymerase chain reaction (qPCR) were used to evaluate the effect of IL-1β on HCC. Results: Compared with healthy donors, the serum IL-1β level is much higher in patients with HCC. In vitro studies, IL-1β expression is markedly elevated under hypoxia and hif-1α is responsible for LPS-induced IL-1β expression in macrophages. In addition, IL-1β selectively induced the expression of hif-1α rather than hif-2α in hepatocellular carcinoma cell lines. Meanwhile, mesenchymal markers are significantly up-regulated and the ability of migration is also enhanced as determined by transwell migration assays. Conclusion: Proinflammatory cytokines (such as IL-1β) are highly secreted by TAMs under hypoxia, which in turn enhance EMT of HCC cells in vitro. Our findings suggest that IL-1β may enhance tumor progression by triggering a positive-feedback loop between TAMs and HCC cells, indicating that IL-1β might be a novel therapeutic target for HCC treatment.
