A comparative study of two means of simultaneous treatment for benign prostate hyperplasia and bladder stone

Abstract:Objective To compare the safety and the clinical efficacy of two means of simultaneous treatment for benign prostate hyperplasia (BPH) and bladder stone. Methods Sixty-three cases of BPH and bladder stones were randomly divided into 2 groups: they were treated by combining mini-incisions cystolithotomy (group A) ,transurethral eletroresection with holmium laser (group B). TURP followed after the removal of stone. The removal of stone time, bladder irrigation time, catheterization time, hospital stay , hospital expenses and therapeutic effects were surveyed. Results Compared with group B, the removal of stone time, hospital cost were significantly superiority in group A(t=2.89,17.87, P<0.05).But compared with group A, the bladder irrigation time, catheterization time, hospital stay were significantly superiority in group B(t=2.21,8.04,2.19, P<0.05). The procedures were successful in all cases, a follow up of 3 to 35 months found a good outcome in both groups. All the patients had normal micturation after the urethral catheter was withdrawn and no severe complication. IPSS, Qmax and RUV were significantly improved in both groups. Conclusion The results showed that the two therapies of BPH with bladder stones were safe and effective.
Efficacy Of Combined Amlodipine/Terazosin Therapy In Male Hypertensive Patients With Lower Urinary Tract Symptoms: A Randomized, Double-Blind Clinical Trial

Abstract:OBJECTIVES To investigate the therapeutic efficacy and safety of Amlodipine alone or in combination with terazosin for the presence of lower urinary tract symptoms (LUTS) and hypertension. LUTS and hypertension often coexist in elderly men.METHODS A total of 355 patients with Stage I or 2 hypertension and LUTS (as defined by an International Prostate Symptom Score of >= 10) were randomly assigned to receive 2 mg of terazosin (n = 117), 5 mg of Amlodipine (n = 119), or 5 mg of Amlodipine plus 2 mg of terazosin (n = 119) once daily for a total of 28 days. The primary outcomes were a reduction in the total and subscores of the International Prostate Symptom Score and blood pressure. Analyses were performed by intention to treat. This trial is registered with ClinicalTrials.gov (No. NCT00693199).RESULTS At day 28 of the trial, the Amlodipine Plus terazosin group demonstrated comparable efficacy in lowering the total International Prostate Symptom Score and significant improvement in the presence of overactive bladder compared with the terazosin group (P < .05) and significant improvement in quality of life compared with the Amlodipine group (P < .05). The Amlodipine plus terazosin group also achieved the greatest blood pressure control compared with either the terazosin group (P < .01) or Amlodipine group (P < .05). All 3 treatment regimens were well tolerated by the study patients.CONCLUSIONS The results of this 4-week, double-blind, randomized trial have demonstrated that in Chinese male hypertensive patients with LUTS, low-dose Amlodipine plus terazosin therapy appears to be a safe and effective combination therapy to control both conditions, especially for those with predominant overactive bladder symptoms. Abstract:Introduction Hypertension and lower urinary tract symptoms (LUTS) due to benign prostatic hyperplasia (BPH) are often coexist in elderly men. Men with bothersome LUTS present with various other conditions, including hypertension, heart disease and the metabolic syndrome. Severe LUTS are likely to constitute a risk factor for the development of hypertension. α (1)-blockers initially introduced for the management of hypertension and have become a standard medical therapy for LUTS. This study was conducted to describe the effect of 2 mg terazosin on LUTS in patients with concomitant mild hypertension. Objectives To investigate the therapeutic efficacy of terazosin for mild hypertension patients with LUTS. Methods A total of 60 patients with stage 1 hypertension with LUTS (International Prostate Symptom Score-IPSS >7) at the time of first examination were enrolled in this trial. They were assessed based on IPSS and IPSS-quality of life for LUTS and measurement of blood pressure (BP) for stage 1 hypertension after excluding those with normotensive and stage 2–3 hypertension. They were treated with 2 mg of terazosin once daily for 12 weeks. Changes in blood pressure and IPSS were evaluated before and after 12 weeks of the medication of terazosin. Results After 12 weeks of treatment, terazosin demonstrated efficacy in lowering the score for IPSS and relieving LUTS. Of them, systolic and diastolic blood pressure of 51 patients (81.6%) decreased to normal. No orthostatic hypotension and other blood pressure-related adverse effects occurred in all patients. Conclusion Terazosin therapy appears to be efficacious in relieving LUTS and decreasing blood pressure for mild hypertension patients with LUTS secondary to BPH.
Efficacy and Safety of Combined Therapy with Terazosin and Tolteradine for Patients with Lower Urinary Tract Symptoms Associated with Benign Prostatic Hyperplasia: a Prospective Study

Abstract:Background The primary objectives of the treatment for the lower urinary tract symptoms (LUTS) associated with benign prostatic hyperplasia (BPH) are to produce rapid, sustained, and safe improvements in the symptoms that affect the quality of life in the majority of men over 50. In this study, we evaluated the efficacy and safety of the combined therapy with terazosin (alpha 1-adrenergic receptor antagonist) and tolterodine (anticholinergic agent) for LUTS associated with BPH. Methods This combination study included 69 patients diagnosed with LUTS associated with BPH based on the International Prostate Symptom Scores (IPSS), urinary flow rate, prostate volume, urinary residual, and their serum prostate-specific antigen levels. Initially, 191 patients were treated with terazosin 2 mg once daily for one week. Those patients with continued LUTS after the initial treatment were allocated randomly into two groups: terazosin group (n=36) in which patients were treated with terazosin 2 mg once daily for six weeks, and combination group (n=33) in which patients were treated with both terazosin 2 mg once daily and tolterodine 2 mg twice daily for 6 weeks. Results The IPSS were significantly improved in both groups after treatment, and the reduction of IPSS in the combination group was significantly greater than that in the terazosin group (P < 0.01). A decrease in urgency, frequency and nocturia were the main contributory factors causing the reduction of IPSS in the combination group. The differences about the peak urinary flow rate and the residual urine from the baseline values were noted in both groups after treatment, but were not significant between the two groups. The incidence of adverse effects in the combination group was higher than that in the terazosin group. As expected the most common adverse effect was mouth dryness which was associated with anticholinergic drugs such as tolterodine. Conclusions Patients with LUTS associated BPH appear the improved IPSS after combined therapy with terazosin and tolterodine. This study, although short term and limited numbers of patients, provides evidence that the combined therapy with terazosin plus tolterodine is a good approach for meeting the objectives of rapid, sustained, and safe improvements in the LUTS associated with BPH. And the profile of patients in this study might be used as the indication of such combined therapy for LUTS associated with BPH without urodynamic evaluation.
Comparison of α1-blocker monotherapy with α1-blocker and antimuscarinics add-on therapy for the treatment of overactive bladder secondary to BPH

Abstract:Objective Detrusor overactivity (DO) is one known cause of overactive bladder (OAB) symptoms in benign prostatic hyperplasia (BPH).In this study,OAB symptoms suggestive of DO in BPH were treated with α1-blocker monotherapy or α1-blocker and antimuscarinics add-on therapy,and the efficacy and safety were assessed.Methods BPH patients who suffered from OAB symptoms for at least 3 month were enrolled in a prospective self-control study from August 2010 to April 2012.The inclusion criteria are total international prostate symptom score (IPSS) ≥8,OAB Symptom Score (OABSS) ≥3,OABSS urgency score ≥2,Postvoid residual volume (PVR) ＜ 100 ml,and maximum urinary flow rate (Qmax) ≥ 5 ml/s.All the patients who met the inclusion criteria were treated with α1-blocker ( tamsulosin 0.2 mg/day or doxazosin 4 mg/day) for 2 weeks.After 2 weeks,patients with no symptomatic improvement ( OABSS≥3) underwent pressure-flow test,and those whose Pdet≥ 40 cm H2O and DO presented more than one time were added antimuscarinics (solifenacin 5 mg/day or tolterodine 4 mg/day) for an additional 2 weeks.OABSS,IPSS,QOL,Qmax and PVR were re-evaluated every 2 weeks.Results Ninety-four cases of BPH/OAB patients met the inclusion criteria and completed 4 weeks treatment.The baseline of total OABSS was 7.0 ± 1.3,IPSS was 17.0 ± 1.7,QOL was 5.0 ±0.7,Qmax was (8.8 ±2.5) ml/s and PVR was (86.0 ± 16.5) ml.After 2 weeks treatment with α1-blocker alone,OABSS was 5.2 ± 0.8,IPSS was 14.2 ± 1.9,QOLwas4.O±0.8,Qmaxwas (11.4±2.4) ml/s and PVR was (67.9±12.9) ml.After another2 weeks treatment with α1-blocker plus antimuscarinics,OABSS was 3.1 ± 0.8,IPSS was 11.1 ± 1.9,QOL was 3.1 ± 0.7,Qmax was ( 10.8 ± 2.4) ml/s and PVR was (71.8 ± 11.9 ) ml.Compared with baseline values,OABSS,IPSS,QOL,Qmax and PVR significantly improved (P ＜ 0.01 ) in α1-blocker monotherapy group and α1-blocker plus antimuscarinic group.The improvement of OABSS,IPSS,QOL scores of α1-blocker plus antimuscarinic group were greater than α1-blocker monotherapy group (p ＜ 0.05 ),while Qmax and PVR showed no differences between the two groups.No acute urinary retention (AUR) was deteted.Conclusion Both of α1-blocker monotherapy and α1-blocker with antimuscarinics add-on therapy can improve OAB symptoms.
Evolving Role of Silodosin for the Treatment of Urological Disorders – A Narrative Review

Abstract:Abstract Use of α-androgenic receptor blockers remains a mainstay therapeutic approach for the treatment of urological diseases. Silodosin is recommended over other α-blockers for the treatment of lower urinary tract symptoms (LUTS) and benign prostate hyperplasia (BPH), due to its high α1A uroselectivity. Current research data suggest that silodosin is efficacious in the management of various urological diseases. Thus, we herein review the current evidence of silodosin related to its efficacy and tolerability and appraise the available literature that might ultimately aid in management of various urological conditions at routine clinical practice. Literature reveals that silodosin is beneficial in improving nocturia events related to LUTS/BPH. Silodosin exerts effect on relaxing muscles involved in detrusor obstruction, therefore prolonging the need for patients undergoing invasive surgery. Silodosin treatment, either as a monotherapy or combination, significantly improves International Prostate Symptom Score (IPSS) including both storage and voiding symptoms in patients with BPH/LUTS. Patients on other treatment therapies such as phosphodiesterase 5 inhibitors or other α-blockers are well managed with this drug. Steadily, silodosin has proved beneficial in the treatment of other urological disorders such as chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS), overactive bladder/acute urinary retention (AUR), premature ejaculation (PE), and prostate cancer post brachytherapy-induced progression. In patients with distal ureteral stones, silodosin treatment is beneficial in decreasing stone expulsion time without affecting stone expulsion rate or analgesic need. Moreover, there were significant improvements in intravaginal ejaculation latency time, quality of life scores, and decrease in PE profile among patients with PE. Silodosin has also demonstrated promising results in increasing the likelihood of successful trial without catheter in patients with AUR and those taking antihypertensive drugs. Reports from Phase II studies have shown promising role of silodosin in the treatment of CP/CPPS as well as facilitating ureteral stone passage. From the robust data in this review, further silodosin treatment strategies in the management of different urological conditions need to be focused on.
Doxazosin Gastrointestinal Therapeutic System Versus Tamsulosin for the Treatment of Benign Prostatic Hyperplasia: a Study in Chinese Patients.

Abstract:AIM:The efficacy and safety profiles of two alpha1-adrenoceptor antagonists, doxazosin gastrointestinal therapeutic system, a controlled-release formulation of doxazosin, and tamsulosin, were compared in Chinese men with confirmed benign prostatic hyperplasia.METHODS:After a 2-week placebo run-in phase, 117 patients were randomized to daily treatment with doxazosin gastrointestinal therapeutic system (4 mg doxazosin) (n = 60) or 0.2 mg tamsulosin (n = 57) for 6 weeks with no titration of study medications. Efficacy was measured by the International Prostate Symptom Score, maximum urinary flow rate, postvoid residual urine volume, and quality-of-life score from the International Prostate Symptom Score. Adverse events were recorded.RESULTS:Both drugs significantly improved the International Prostate Symptom Score (total, irritative subscore and obstructive subscore) and maximum urinary flow rate. Doxazosin gastrointestinal therapeutic system reduced postvoid residual urine volume significantly more than tamsulosin (-25 +/- 5 mL vs 2 +/- 5 mL, P = 0.041) in patients with residual volume >0 mL at baseline. Other differences between groups were not statistically significant.CONCLUSIONS:The doxazosin gastrointestinal therapeutic system and tamsulosin were effective and well tolerated for the treatment of benign prostatic hyperplasia in Chinese men.
Ejaculatory Dysfunction Caused by Alpha Blockers for the Treatment of Lower Urinary Tract Symptoms in Men with Benign Prostatic Hyperplasia : A Systematic Review and Network Meta-analysis

Abstract:To provide a systematic review and network meta-analysis of randomized clinical trials focus on ejaculatory dysfunction (ED) rate of alpha blockers (ABs) for treatment of lower urinary tract symptoms (LUTS) in men with benign prostatic hyperplasia (BPH), a systematic literature search was performed to identify all available published randomized controlled trial (RCTs) as regards ABs for the treatment in BPH patients. A systematic review and network meta-analysis was performed using ADDIS 1.16.5. 15. RCTs involving 22593 participants were included, comparing 2 selective ABs (silodosin and tamsulosin) and 2 unselective ABs (terazosin and doxazosin) with placebo. For the ejaculatory dysfunction rate, silodosin was higher than placebo (OR=70.31, 95% CI: 17.70 to 538.38), doxazosin (OR=52.82, 95% CI: 5.44 to 833.41) and terazosin (OR=40.14, 95% CI: 2.74 to 1362.98). And so was tamsulosin compared with placebo (OR=8.70, 95% CI: 2.36 to 59.61), doxazosin (OR=6.45, 95% CI: 0.81 to 83.43) and terazosin (OR=4.95, 95% CI: 0.35 to 164.75). No statistical differences were observed among doxazosin, terazosin and placebo (OR≈1). As such, the ranks of ejaculatory dysfunction rate were silodosin > tamsulosin > terazosin ≈ doxazosin ≈ placebo. The selective ABs were associated with higher risk of ejaculatory dysfunction while the unselective ABs have the same risk as placebo. When focusing on better sexual life, the unselective ABs like doxazosin and terazosin are better.

The GRADE approach was used in summarizing conclusions.A total of 15 systematic reviews involving five α(1) blockers met the inclusion criteria. Direct evidence demonstrated that α(1) blockers were superior to placebo in reducing urinary symptom scores and improving peak urinary flow PUF. Doxazosin could significantly reduce urinary symptom scores compared with tamsulosin mean difference (MD -1.60, 95% CI -1.80 to -1.40) and alfuzosin (MD1.7, 95% CI 0.76-1.64). Indirect evidence suggested that the urinary symptom score and PUF at endpoint in men treated with naftopidil were similar to those treated with other α(1) blockers. α(1) Blockers generally lead to more adverse effects compared with placebo, and those caused by terazosin were more frequent than others.α(1) Blockers are more effective than placebo for BPH, doxazosin and tamsulosin seem to be more effective than other α(1) blockers. The adverse effects caused by α(1) blockers are generally mild and well-tolerated.

[Treatments of erectile dysfunction combined with lower urinary tract symptoms associated with benign prostatic hyperplasis].

Abstract:Lower urinary tract symptoms secondary to benign prostatic hyperplasia (BPH/LUTS) and erectile dysfunction (ED) are common conditions seen in aging males. Although the exact mechanism of ED combined BPH/LUTS is unknown, treatment with α-adrenergic receptor resistance agents (α-ARAs) contributes to some improvement in ED, and phosphodiesterase-5 inhibitors (PDE-5I) are also of benefit to BPH/LUTS. The action mechanisms of these medications support these observed benefits, which are being studied in the basic science and clinical settings. Much remains to be investigated, but it is clear that the associations between BPH/LUTS and ED lay the foundation for future therapies and possible preventative strategies. This article has mainly summarized the advancement of epidemiology, basic research and treatment of ED combined with BPH/LUTS with α-receptor blockers and phosphodiesterase-5 inhibitors.

Abstract:Objective To evaluate the efficacy and safety of alpha‐1‐blockers in the treatment of female lower urinary tract symptoms (LUTS) .Methods Randomized or controlled clinical trials comparing alpha‐1‐blockers monotherapy with cotherapy of antimuscarinics and alpha‐1‐blockers ,placebo and other types of therapy in the treatment of female LUTS were i‐dentified by electronic and manual retrieval and analysis .The methodological quality of the included trials was assessed by mod‐ified Jadad scoring system and Meta‐analysis was performed with Revman 5.3 software .Results Altogether 10 eligible ran‐domized controlled trials (n=1198) were included in this systematic review ,of which 7 were included for qualitative analysis and 3 for quantitative analysis .Meta‐analysis showed that there were significant improvements on international prostate symp‐tom score [mean difference (MD) = -2 .26 ,95% CI (-2 .60 ,-1 .92) ,P<0 .001) and average flow rate (MD=1 .41 ,95%CI (0 .67 ,2 .14) ,P=0 .000 2] .There was no significant difference regarding maximum flow rate (MD= 0 .41 ,95% CI (-0.96 ,1 .79) ,P=0 .56) .Qualitative analysis indicated combined therapy with antimuscarinics and alpha‐1‐blockers was more ef‐fective ,which could significantly improved both symptoms and subjective urodynamic parameters .Combined with extracorporeal magnetic innervations or uroflowmetry biofeedback ,alpha‐1‐blockers could obtain significant therapeutic effects as well .All 10 articles reported side effects but no serious adverse events were reported .Conclusions Monotherapy of alpha‐1‐blockers may ameliorate lower urinary tract symptoms and improve urodynamic parameters in women .Combined therapy was more effica‐cious than monotherapy .Alpha‐1‐blockers were well tolerated and the overall incidence of adverse events was low .Due to lim‐ited number of the included studies ,the above mentioned conclusions still need to be verified with more data .

Abstract:The aim of this systematic review is to determine the comparative effectiveness and safety of phosphodiesterase 5 inhibitors (PDE5-Is) and α-blockers used alone or combined for the treatment of lower urinary tract symptoms (LUTS) due to benign prostatic hyperplasia (BPH). An electronic search of PubMed, Cochrane Library and Embase up to January 2014 was performed to identify randomized controlled trials comparing the efficacy and safety of PDE5-Is and α-blockers for treatment of lower urinary tract symptoms due to benign prostatic hyperplasia, which assessed IPSS score, maximum flow rate, postvoided residual urine, quality of life and Erectile Function (IIEF) score as outcomes. Data were analyzed by fixed or random effect models using Cochrane Collaboration review manager software. A total of 12 studies were included. Our novel data demonstrated that there was a trend that α-blockers were more efficacious than PDE5-Is on decreasing IPSS score and increasing maximum flow rate. α-blockers were significantly more effective than PDE5-Is on reduction of postvoided residual urine with a mean difference of 3.67 (95% CI 1.56 to 5.77, P = 0.0006) and PDE5-Is showed greater effect than α-blockers on increasing IIEF score with a mean difference of 9.82 (95% CI 3.80 to 15.85, P = 0.001). In conclusion, our novel data demonstrated that PDE5-Is plus ABs ranked the highest on the improvement of LUTS/BPH. PDE5-Is monotherapy was also effective in this kind of disorder except less reduction of PVR than ABs. In addition, both combined- or mono-therapy were safe.
Single drug versus combination therapy with terazosin and amlodipine for middle-aged and old male patients with bladder outlet obstruction associated with benign prostatic hyperplasia and their quality of life

Abstract:AIM: To evaluate the efficacy and safety of single drug versus combination therapy with terazosin and amlodipine in the treatment of middle-aged and old patients with bladder outlet obstruction (BOO) associated with benign prostatic hyperplasia (BPH), and observe the improvement in their quality of life (QOL). METHODS: A total of 637 middle-aged and old patients with primary hypertension, who were treated with the combined therapy with terazosin and amlodipine, were adopted from Anqing and the neighboring area from July 2005 to February 2006. Informed consents were obtained from all the patients. Finally 569 patients completed 4-week follow-up visits, and 272 patients aged 50-75 years were selected for observations, in accordance with International Prostatic Symptom Score (IPSS) ≥ 8 points and prostate gland ≥ π/6×2 mL×3 mL×4 mL by abdominal B ultrasound. Patients were randomly divided into five groups and taken amlodipine 5 mg (n =100), terazosin 1 mg (n =29), terazosin 2 mg (n =30), terazosin 1 mg plus amlodipine 5 mg (n =55), and terazosin 2 mg plus amlodipine 5 mg (n =58), respectively. The administrations were given once per day (7:30-8:00) for 4 weeks. The IPSS, maximal urinary flow rate (Qmax), average urinary flow rate (Qave) and QOL were determined at baseline and 4 weeks of follow-ups as efficacy parameters. RESULTS: There were no significant differences in the larithmics feature and curative effect among the single groups and the combined groups.①The improvements of IPSS and Qave in the combined group were significantly greater than that in the terazosin or amlodipine group (P < 0.05).②The improvement about the Qmax and QOL score were better in the combined group, but the differences were not significant compared with the single groups (P > 0.05).③No statistical differences were found in three group for adverse events (P > 0.05). CONCLUSION: The combination of terazosin and amlodipine is superior to the use of terazosin or amlodipine individually in the treatment of BOO associated with BPH in the middle-aged and old patients, who in the combination group are well tolerated without obvious adverse events.
A multicenter prospective clinical study on the effectiveness and safety of terazosin in the treatment of Chinese benign prostatic hyperplasia patients

Abstract:Objective To evaluate the effectiveness and safety of terazosin in the treatment of Chinese benign prostatic hyperplasia (BPH) patients. Methods A multicenter prospective postmarketing observational study was conducted from June 2007 to March 2008 in 32 urologic centers.Patients were given terazosin for 4 weeks according to the routine medical care procedures following instructions. Effectiveness evaluation included the primary endpoint focusing on the changes in IPSS total score at the end of 2nd and 4th week compared with the baseline. The secondary endpoints were the changes in Qmax and QOL at the end of 4th week, diastolic and systolic blood pressures at the end of 2nd and 4th week compared with the baseline and the discontinuation rate of terazosin within the four weeks. Safety was assessed by adverse events. Results There were 1006 patients included in this study (FAS) and 992 patients (PP) completed the study. Among them, there were 344 patients having hypertension. The total IPSS score reduced from 22.32±6. 13 at baseline to 16. 98±5.92 at the end of the 2nd week and to 14.00±5. 52 at the end of the 4th week in FAS population (P＜0. 01).The total IPSS score changed from 22.32±6.15 at baseline to 16. 96±5.93 at the end of the 2nd week and to 13. 95±5.52 at the end of the 4th week in the PP population (P＜0.01). The efficacy rate was 26.54% at the 2-week treatment and 60.64% at the 4-week treatment, which was defined as obtaining improvement by 30% compared with the baseline. Patient's IPSS in different age groups with different prostatic hyperplasia levels and patients combined with or without 5-α reductase inhibitors were all decreased significantly(P＜0.01). With 4-week treatment of terazosin, Qmax and QOL were improved significantly by 32% and 45% (P＜0.01). Terazosin decreased BPH patient blood pressure with untreated or uncontrolled hypertension (P＜0.05), but had little influence on normal blood pressure of those under control. The incidence of adverse reactions was low. The most common adverse event was dizziness (3.68%). At the end of the study, 960 subjects (95%) were taking drug continuously.Conclasions Terazosin can significantly improve the symptoms and quality of life in Chinese BPH patients with good safety and compliance.

Abstract:This study sought to investigate the clinical efficacy and safety of combined oral therapy with sildenafil and doxazosin GITS compared to sildenafil monotherapy in treating Chinese patients with erectile dysfunction (ED) and lower urinary tract symptoms secondary to benign prostatic hyperplasia (BPH/LUTS). The trial was conducted in hospitals in Beijing, Shanghai, Changsha, Wuhan and Guangzhou, five major cities in China. A total of 250 patients diagnosed with ED and BPH/LUTS aged 50-75 years, and who had International Index of Erection Function-5 (IIEF-5) scores <= 21 and International Prostate Symptom Score (IPSS) >= 10 points, were enrolled and randomly divided into Group A (168 cases; doxazosin GITS 4 mg once daily plus sildenafil 25-100 mg on demand) and Group B (82 cases; sildenafil 25-100 mg on demand). Efficacies were evaluated by IIEF-5 and IPSS scores and a quality of life (QoL) questionnaire, and adverse effects were evaluated during the treatment period. There were no statistically significant differences in mean age, and IIEF-5, IPSS and QoL scores pre-treatment between the two groups. After treatment, IIEF-5, IPSS and QoL scores were significantly improved in Group A, while only IIEF-5 scores were significantly improved in Group B compared with pre-treatment. There were no significant differences in side effects between the two groups. The results indicated that combined therapy with sildenafil and doxazosin GITS for the treatment of ED and BPH/LUTS is safe and effective compared to sildenafil monotherapy. Abstract:Alpha1-adrenoceptor antagonists (α1-blockers) are first-line drugs for the treatment of lower urinary tract symptoms associated with benign prostate hyperplasia (BPH). Doxazosin gastrointestinal therapeutic system (GITS) and tamsulosin belong to the 2 most frequently prescribed α1-blockers. This systematic review and meta-analysis was performed to compare the efficacy and tolerability of these 2 α1-blockers.A systematic review of published randomized controlled trials in English or Chinese language was performed using the PubMed, EMBASE, Cochrane Library, CNKI, Wanfang, and Vip databases. After data extraction and quality assessment, the meta-analysis was performed to compare clinical parameters (International Prostate Symptom Score [IPSS] total [IPSS-T], storage [IPSS-S], voiding [IPSS-V], maximum urine flow [Qmax], and postvoid residual) and adverse events (AEs) that changed after first drug intake.After the screening, 8 eligible randomized controlled trials with 1316 patients were identified. Doxazosin-GITS showed a significantly higher efficacy compared with tamsulosin (IPSS-T P < .001, IPSS-S P < .001, and IPSS-V P < .001). There were no significant differences between the 2 drugs for changes in Qmax (P = .477) or postvoid residual (P = .739). The overall AEs were significantly lower in the doxazosin-GITS group (risk ratio: 0.77; 95% CI: 0.54-1.08; P = .036). However, dizziness (P = .387), headache (P = .745), asthenia (P = .693), postural hypotension (P = .114), and retrograde ejaculation (P = .187) were similar between the 2 groups.This meta-analysis indicates that doxazosin-GITS has significantly higher efficacy and lower AEs than tamsulosin in patients with lower urinary tract symptoms/benign prostate hyperplasia.

Abstract:OBJECTIVES: Benign prostatic hyperplasia (BPH) is a common and progressive disease affecting elderly males, often associated with lower urinary tract symptoms (LUTS). α1-blockers are the mainstay in symptomatic therapy of BPH. Because of their greater uroselectivity and minimal hemodynamic effects, alfuzosin, tamsulosin, and silodosin are generally preferred. The aim of this study was to compare the efficacy and tolerability of alfuzosin, tamsulosin, and silodosin in patients with BPH and LUTS.METHODS: Ninety subjects with BPH and LUTS were randomized into three groups of thirty in each, to receive alfuzosin sustained release (SR) 10 mg, tamsulosin 0.4 mg, or silodosin 8 mg for 12 weeks. The primary outcome measure was a change in the International Prostate Symptom Score (IPSS), and the secondary outcome measures were changes in individual subjective symptom scores, quality of life score (QLS), and peak flow rate (Qmax) from baseline. The treatment response was monitored at 2, 4, 8, and 12 weeks.RESULTS: IPSS improved by 88.18%, 72.12%, and 82.23% in alfuzosin SR, tamsulosin and silodosin groups (P < 0.001) at 12 weeks. Improvement in QLS was >75% in all the three groups (P < 0.001). A significant improvement in Qmax was seen with alfuzosin and tamsulosin (P = 0.025 and P < 0.001) but not with silodosin (P = 0.153). However, the intergroup differences in IPSS, QLS, and Qmax were not significant. Ejaculatory dysfunction was more common with silodosin and corrected QT (QTc) prolongation occurred only with alfuzosin (two subjects) and tamsulosin (three subjects).CONCLUSION: Alfuzosin, tamsulosin, and silodosin showed similar efficacy in improvement of LUTS secondary to BPH, with good tolerability, acceptability, and minimum hemodynamic adverse effects. Alfuzosin, tamsulosin, and silodosin are comparable in efficacy in symptomatic management of BPH. The occurrence of QTc prolongation in three subjects with tamsulosin in the present study is an unexpected adverse event as there are no reports of QTc prolongation with tamsulosin in any of the previous studies.
