METHODS The medical records of 1268 men with PCa treated with radical prostatectomy without neoadjuvant therapy at four medical institutes between 2001 and 2009 were retrospectively reviewed. The association of various risk factors with BCR was statistically analyzed. The Kaplan-Meier method, log-rank test, and Cox's proportional hazards model were used for statistical analysis. A p value of >0.05 was considered statistically significant. RESULTS Of 1268 patients, 222 (17.5%), 664 (52.4%), and 382 (30.1%) patients were classified as having low-, intermediate-, and high-risk PCa according to D'Amico's classification, respectively. In the intermediate-risk group, 96 (14.5%) patients experienced BCR during the median follow-up of 41 months. The rate of BCR was significantly lower in the intermediate-risk group than in the high-risk group (p = 0.001), whereas there was no significant difference in the rate of BCR between the low-risk and intermediate-risk groups (p = 0.122). Preoperative PSA level, prostate volume (PV), and PSA density (PSAD) were independent risk factors for BCR in the intermediate-risk group, whereas other factors including age, primary Gleason score of 4, clinical stage of >T2, percentage positive core were not independent risk factors for BCR. Multivariate analysis demonstrated that low PV and high PSAD were significant risk factors for BCR in the intermediate-risk group. BCR-free survival in the intermediate-risk groups with high PSA (>15 ng/mL), high PSAD (>0.6 ng/ml/cm3), or low PV (<10 ml) was comparable with that in the high-risk group (p = 0.632, p = 0.494, and p = 0.961, respectively). CONCLUSIONS Preoperative PSA, PV, and PSAD are independent risk factors for BCR in D'Amico intermediate-risk PCa patients treated with radical prostatectomy.
