Abstract:OBJECTIVE:To investigate the personality traits and intensity dependence of auditory evoked potentials (AEPs) in chronic primary insomnia. METHODS:Thirty-seven patients with chronic primary insomnia (insomnia group) and 44 healthy subjects (control group) were enrolled in the study. The AEPs were examined in insomnia and control groups; the personality traits were studied by Zuckerman's Sensation Seeking Scales (SSS) and Zuckerman-Kuhlman's Personality Questionnaire (ZKPQ); and the mood states by Plutchik-van Praag's Depression Inventory (PVP). RESULT:The scores of neuroticism-anxiety and depression in insomnia group were significantly higher than those in control group (P<0.01); and the scores of impulsivity and aggression-hostility were also higher than those in control group (P<0.05); N1-P2 amplitude of AEP increased with stimulus intensity, which were significantly different in 70, 80, 90,100 dB (P<0.01). There were significant correlations between activity and N1 latency at 80 dB, activity and P2 latency at 100 dB (r=0.270, r=0.276, P<0.05); and between total scores of sensation seeking scale and N1-P2 amplitude (r=0.3746, r=0.35329, P<0.01) at 70 and 90 dB stimulus intensity in insomnia group. There were significant correlations among experience seeking and N1-P2 amplitude, experience seeking and slope rate (P<0.01) at 70, 80, 90, 100 dB stimulus intensity in insomnia group (r=0.539, r=0.3439, r=0.439, r=0.3278). There were significant correlations between sensation seeking of boredom susceptibility and slope rate (r=-0.282998, P<0.05) in insomnia group. There were significant correlations between thrill and adventure seeking and N1-P2 amplitude(r=0.2789, P<0.05) at 90 dB stimulus intensity in insomnia group; there were significant correlations between PVP and N1-P2 amplitude (r=-0.3434, r=-0.3158, P<0.05) at 70 dB and N1 latency at 80 dB in insomnia group. CONCLUSION:Chronic primary insomnia sufferers have higher levels of neuroticism-anxiety, depression, aggression-hostility and impulsivity, and some are correlated with stimulus intensity dependence in AEP.
[Probe into Sleep Quality in the Patients with Irritable Bowel Syndrome].

Abstract:OBJECTIVE:To investigate the sleep features in the patients with irritable bowel syndrome (IBS) and compare the sleep quality between those IBS patients who were with and without anxiety and depression. METHODS:Pittsburgh sleep quality index questionnaire (PSQI), self-rating anxiety scale (SAS) and self-rating depression scale (SDS) were measured in the 145 IBS patients and 59 regular physical examination volunteers. IBS patients were also divided into two subgroups--patients with or without anxiety and depression based on cutoff scores of SAS and SDS. Comparisons of sleep quality were made between subgroups, and between IBS patients and volunteer controls. RESULTS:Compared with the controls, the SAS raw score, SDS raw score and SAS positive incidence in IBS patients were shown statistically significant differences (P<0.05), while the SDS positive incidence had no statistically significant difference (P>0.05). PSQI total scores were significantly higher in the IBS patients without anxiety and depression (P<0.05), 3 domains (sleep quality, sleep disturbances and daytime function disorder) were also found statistically significant differences (P<0.05), compared with the controls. The IBS patients with anxiety and depression were statistically significantly different from the controls (P<0.05) in 6 domains (sleep quality, sleep latency, sleep efficiency, sleep disturbances, sleep time and daytime function disorder) and significantly higher PSQI total scores (P<0.05). Statistically significant differences (P<0.05) were also found in all 7 domains and with higher PSQI total scores in IBS patients with anxiety and depression, compared with IBS patients without anxiety and depression. CONCLUSIONS:IBS patients were more likely to have sleep abnormality, mainly in sleep quality, sleep disturbances and daytime function disorder and PSQI total scores. The abnormalities of these factors were independent of emotional disorder. However, emotional disorder worsened the sleep disorder in IBS patients.
Chronotype, sleep quality, impulsivity and aggression in patients with borderline personality disorder and healthy controls

Abstract:Impulsivity, aggression, and suicide are the major clinical symptoms of borderline personality disorder (BPD). Although previous studies indicated poor sleep quality and its relationship with clinical symptoms in patients with BPD, chronotype, an important sleep parameter, was not investigated in these patients. This study aimed to analyze chronotype and its relationship with clinical symptoms in patients with BPD. Participants in this study consisted of 68 BPD patients and 65 healthy controls. Subjective sleep characteristics, impulsivity, aggression, suicide probability, and chronotype were assessed using the Pittsburgh Sleep Quality Index (PSQI), Barratt Impulsivity Scale, Buss-Perry Aggression Scale (BPAQ), Suicide Probability Scale, and Morningness – Eveningness Questionnaire, respectively. PSQI total and subscale scores subjective sleep quality, sleep latency, sleep duration, habitual sleep efficiency, sleep disturbances, and daytime drowsiness were significantly higher in the BPD group ( p < 0.001). There was a positive correlation between the PSQI total score and the BPAQ total score ( r = 0.268, p = 0.027). The rate of evening type was significantly higher in the control group ( p = 0.004). Suicide attempts and the subscale of suicide probability hopelessness, suicidal ideation, and negative self-evaluation scores were significantly higher in evening type BPD patients. ( p = 0.017, p = 0.009, p = 0.001, p = 0.047). Sleep quality is associated with aggression, and the eveningness chronotype is associated with suicide. It may be useful to focus on sleep problems in treating BPD patients.

Abstract:Abstract Introduction Adolescents with bipolar disorder are at ultra-high risk of suicide and poor psychosocial functioning. Sleep is impaired during periods of depression, mania, and euthymia in adults with bipolar disorder and may be a transdiagnostic risk factor for new onset of mood symptoms. Sleep worsens during adolescence, but less is known about the interrelationships between poor sleep and mood symptoms in adolescents with bipolar disorder. Methods Participants included adolescents with bipolar disorder (n=75, 12-18 years, 65 females) who consented to a randomized controlled trial of Dialectical Behavior Therapy (DBT) vs. Standard of Care (SOC; other evidence-based psychotherapy approaches) at a specialty psychiatric clinic. Participants completed seven assessment visits, five during the year-long treatment period (baseline, 3-months, 6-months, 9-months, 12-months) and two during the post-treatment follow-up (18-months, 24-months). During each visit, participants completed the Pittsburgh Sleep Quality Index (PSQI) and a semi-structured interview to obtain weekly clinician-rated mood symptom ratings for depression, hypomania, and suicidal ideation. Results Most participants (75.9%) had a PSQI score >5 at baseline, reflecting poor sleep quality (M=8.86, SD=4.30). Sleep quality improved between baseline and 3-months (Mdiff=-2.01, p=.006) and remained stable thereafter but did not differ between treatment groups. Lag analyses examined whether sleep quality predicted the next month’s average mood symptom ratings. Across treatment group, sleep quality at 6-months predicted next month depression, hypomania, and suicidal ideation severity (ps<.032). For DBT, sleep quality at baseline predicted next month suicidal ideation severity (p=.047), and sleep quality at 9-months predicted next month hypomania severity (p=.003). For SOC, sleep quality at 6-months predicted next month depression severity (p=.027), and sleep quality at 9-months predicted next month suicidal ideation severity (p=.005). Worse sleep quality predicted worse future mood symptoms for all significant lagged effects. Conclusion Sleep quality is poor among adolescents with bipolar disorder. Sleep quality predicted future mood symptoms, suggesting that poor sleep may causally contribute to worse mood symptoms, particularly mid to late treatment. Thus, poor sleep could be a critical therapeutic target in future treatments to further improve mood symptoms in this population. Support (if any) R01 MH100056.

Sleep patterns in patients with unipolar or bipolar depression

Abstract:Objectives To compare the sleep parameters and certain neurocognitive tasks between patients with unipolar and bipolar depression,and to explore the correlation between sleep pattern and certain cognitive function.Methods Fifteen patients with unipolar depression,thirteen patients with bipolar depression,and fifteen health controls were recruited.Sleep parameters were recorded by polysomnography for three groups respectively.The certain cognitive tasks were finished in patients group.Results Compared with control group,the bipolar patients had longer sleep latency (P =0.004),poorer sleep efficiency(P =0.007),while the unipolar patients had poorer sleep efficiency(P =0.038),less slow wave sleep (P =0.002),more numbers of wake (P =0.019),longer duration of wake after sleep onset (P =0.035),increased proportion of S1 (P =0.028) and S2 sleep(P =0.014),decreased proportion of S3 (P =0.002),and longer REM sleep latency (P =0.019).No significant difference of polysomnographically recorded sleep parameters were found between unipolar and bipolar group.There was no significant correlation between sleep parameters and certain cognitive tasks in bipolar group.In unipolar group,the verbal learning field was related with REM sleep duration (r =0.63,P =0.034),proportion of REM (r =0.69,P =0.020),REM sleep latency(r =-0.74,P =0.009),while the visual memory field was related with proportion of S2 (r =-0.65,P =0.029) and REM sleep latency(r =-0.72,P =0.012).Conclusions The sleep disturbance in the bipolar group may be mild,while in the unipolar group,the disturbance is focused on the sleep continuity and architecture.The relationship of the sleep parameters and certain neurocognitive field may be different between unipolar and bipolar depression.
Sleep quality and neuropsychological functioning in bipolar I disorder

Abstract:Background: Individuals with bipolar I disorder (BD-I) experience both poor sleep and neuropsychological dysfunction relative to non-psychiatric populations, which limits functional recovery. Poor sleep adversely affects learning, memory, and executive functioning in healthy individuals; however, little is known about the role of poor sleep in neuropsychological functioning in BD-I. We tested whether sleep disturbance was greater in BD-I than healthy control participants (HC), and compared the effect of sleep quality on learning, memory, and executive functioning between BD-I and HC. Methods: Participants with BD-I (N=250) and HC (N=206) completed the Pittsburgh Sleep Quality Index, neuropsychological testing, and clinician-administered mood measures as part of a naturalistic study of bipolar disorder. We examined effects of both diagnosis and sleep quality on neuropsychological functioning. Results: Relative to HC, BD-I showed poorer sleep quality and neuropsychological functioning in verbal learning, verbal and visual memory, processing speed, psychomotor speed, inhibitory control, and selective attention (7/9 domains). Poor sleep quality was associated with poorer verbal learning, verbal fluency, processing speed, and interference control (4/9). Effects of poor sleep on neuropsychological functioning did not differ between BD-I and HC. Limitations: The assessment of sleep quality using a self-report measure and the effects of medications/sleeping aids (given the naturalistic study design) should be considered when interpreting results. Conclusions: Those with BD-I experiencing poor sleep may also be more vulnerable to verbal learning and executive functioning impairments. The findings of poor sleep in relation to poorer neuropsychological functioning have implications for assessment and treatment of sleep disturbance in BD-I.

Abstract:Introduction: Suicidal ideation is a critical early stage in the progression towards suicidal be havior. Prior research has established links between sleep quality, impulsivity, and suicidal tendencies, yet the interaction among these factors has been less explored. This study aims to explore the mediating role of impulsivity in the relationship between sleep quality and suicidal ideation in adolescents. Methods: Employing a cross-sectional study design, 6,974 questionnaires were distributed,including the Socio-demographic Characteristics Questionnaire, Barratt Impulsiveness Scale, the Positive and Negative Suicide Ideation Inventory,and the Pittsburgh Sleep Quality Index Scale. The participants were high school and middle school students from 33 schools in northeastern Sichuan, China, selected through random cluster sampling. Results: Of these 6,786 questionnaires were analyzed. The participant distribution included 47.2% male and 52.8% female students, with 68.3% from junior schools and 31.7% from senior schools. The prevalence of suicidal ideation was found to be 13.6%. The analysis, which involved correlation analysis and the construction of a structural equation model, revealed that sleep quality had a significant positive effect on impulsivity (β:0.289,p < 0.05), and impulsivity, in turn, had a positive impact on suicidal ideation (β:0.355,p < 0.05).Moreover, sleep quality was directly linked to suicidal ideation (β:0.208,p < 0.05). Thus, sleep quality affects suicidal ideation both directly and indirectly through impulsivity. Discussion: The results of this study suggest that both sleep quality and impulsivity are significant direct influencers of suicidal ideation among adolescents in the region studied, with impulsivity also playing an indirect role in the relationship between sleep quality and suicidal ideation.

[Relationship Between Circadian Rhythm Related Brain Dysfunction and Bipolar Disorder].

Abstract:OBJECTIVE:To investigate the changes of functional connectivity (FC) in the suprachiasmatic nucleus (SCN) of patients with bipolar disorder and perform a cluster analysis of patients with bipolar disorder based on FC.METHODS:The study recruited 138 patients with bipolar disorder (BD) diagnosed according to the 4th edition of Diagnostic and Statistical Manual of Mental Disorders IV (DSM-IV) and 150 healthy control subjects. All the participants underwent resting-state functional magnetic resonance brain scans. DPARSF software was used to generate the FC diagram of the SCN. Based on the FC data, principal components analysis (PCA) and k-means in scikit-learn 0.20.1 were used for cluster analysis of the patients with bipolar disorder.RESULTS:Compared with the healthy controls, the patients showed enhanced functional connections between the SCN and the paraventricular nucleus and between the SCN and the dorsomedial hypothalamus nucleus. Based on these FC values, the optimal cluster of unsupervised k-means machine learning for bipolar disorder was 2, and the Silhouette coefficient was 0.49.CONCLUSIONS:Patients with bipolar disorder have changes in the FC of the SCN, and the FC of the rhythm pathway can divide bipolar disorder into two subtypes, suggesting that biological rhythm is one of the potential biomarkers of bipolar disorder.
A Survey on Sleep Quality in the Common Mental Disorders

Abstract:Objective To investigate the sleep characteristic in the common mental disorders. Methods Subjects came from the epidemiological survey of mental disorders in people aged 18 and older in Hebei Province, and 1874 subjects who were diagnosed one of 13 types of mental disorders according the DSM-Ⅳ criteria were contrasted to 15 117 ones without mental disorders. The Pittsburgh Sleep Quality Index (PSQI), Global Assessment of Functioning Scale (GAF) was used to assess the sleep quality and generally social functioning. Results The prevalence of low sleep quality in people with different mental disorders ranged from 19.30% to 69.92%, and the average prevalence of 48.61% was higher than that in people without mental disorders (5.55%). The prevalence of low sleep quality in major depression disorder and generalized anxiety disorder were reached to 69.92% and 58.27%. The common complaints of sleep were longer sleep latency and shorter sleep duration. The rate of hypnotic druB use was highest in patients with schizophrenia, then in ones with bipolar disorder. The age of ones with mental disorder with sleep disorders was elder than those without sleep disorders. The prevalence of sleep disorder was higher in lowerlever education, female, farmer, retired, loss of his/her spouse, hostess and student. The prevalence of sleep disturbance was not related to depression severity. Conclusions The prevalence of sleep disorder in people with mental disorders is high, which is related to many factors.
Status and correlates of sleep quality in schizophrenia inpatients

Abstract:Objective:To explore the status and correlates of sleep quality in hospitalized patients with schizophrenia.Methods:A total number of 269 schizophrenia inpatients were recruited from 7 hospitals including Peking University Sixth Hospital, Zhumadian Second People′s Hospital and Liaocheng Fourth People′s Hospital from August 2019 to March 2021. The Brief Psychiatric Rating Scale (BPRS), the Pittsburgh Sleep Quality Index (PSQI), the 9-item Patient Health Questionnaire (PHQ-9), the 7-item Generalized Anxiety Disorder Scale (GAD-7) and the suicide module of Mini-International Neuropsychiatric Interview were evaluated. Poor sleep quality was determined by the score of more than 5 according to PSQI, all patients were divided into groups of poor and normal sleep quality. The general demographic data and clinical characteristics of two groups were compared, and the correlates of sleep quality were obtained by Spearman correlations and multiple logistic regression.Results:The prevalence of poor sleep quality in schizophrenia inpatients was up to 44.6% (120/269). Compared with normal sleep quality group, inpatients with poor sleep quality had higher rates of alcohol consumption history, use of benzodiazepines (BZDs) and current suicide risk, and BPRS total score, factor scores (thinking disorders, anxious-depression, hostile-suspiciousness and activation), PHQ-9 and GAD-7 scores were also higher (all P<0.05). Spearman correlation analyses showed that PSQI total score were significantly positively correlated with BPRS total score ( r=0.323), PHQ-9 score ( r=0.553), GAD-7 score ( r=0.456) and current suicide risk level ( r=0.320) (all P<0.001). Multiple logistic regression showed that history of alcohol consumption ( OR=2.897, 95% CI: 1.002-8.372), use of BZDs ( OR=3.181, 95% CI: 1.548-6.534), thinking disorders ( OR=1.563, 95% CI: 1.015-2.406), comorbidity with depression ( OR=4.968, 95% CI: 1.869-13.202), and current suicide risk ( OR=2.496, 95% CI: 1.360-4.581) were independently correlated with poor sleep quality (all P<0.05). Conclusion:Poor sleep quality is common in hospitalized patients with schizophrenia, and history of alcohol consumption, use of BZDs, thinking disorders, comorbidity with depression and current suicide risk are independent correlates of poor sleep quality.
Objective and subjective sleep quality and quality of life in patients with chief complaint of insomnia

Abstract:Objective: There are differences between subjective and objective assessment of sleep in the insomniacs.In this study,we compared the correlation between the subjective and objective assessment of sleep and quality of life in patients with the chief complaint of insomnia,in order to provide clinical evidence for the treatment of insomnia.Methods: A total of 64 patients with the chief complaint of insomnia were examined successively with polysomnography(PSG),and the PSG paramenters were used to assess objective sleep.The PSG data of 17 heathy people were used as control.The Pittsburgh Sleep Quality Index(PSQI) was used to evaluate the subjective sleep quality,the Short Form 36 Health survery Questionnaire(SF-36) were used to evaluate quality of life,and the Beck Depression Inventory(BDI) and Beck Anxiety Inventory(BAI) were used to evaluate emotional status.Results: Ninety percent of patients had poor subjective sleep quality.Compared with healthy controls,patients demonstrated prolonged sleep latency,increased wakefulness time after sleep onset,lower sleep efficiency and prolonged rapid eye movement sleep latency(Ps<0.05).The PSQI scores had negative correlation with SF-36 physical health scores(r=-0.25,P<0.05),but the significance was reduced when controlled for scores of BDI and BAI.There was no significant correlation between PSG parameters and SF-36 physical health scores.Conclusion: In patients with insomnia,subjective assessment of sleep quality might have stronger correlation with quality of life,in which the insomnia related depressive and anxious symptoms may play a major role.It indicates that we should pay attention to the improvement of subjective sleep quality,also identify and manage the emotional problems promptly in the treatment of insomnia.
Sleep profiles of different psychiatric traits

Abstract:Disturbed sleep comes in many forms. While the key role of sleep in mental health is undisputed, our understanding of the type of sleeping problems that manifest in the early stages of psychiatric disorders is limited. A sample without psychiatric diagnoses ( N = 440, 341 women, 97 men, 2 non-binaries; M age = 32.1, SD = 9.4, range 18–77) underwent a comprehensive assessment, evaluating eight sleep features and 13 questionnaires on common psychiatric complaints. Results revealed that traits of affect disorders, generalized anxiety, and ADHD had the worst sleep profiles, while autism disorder, eating disorder, and impulsivity traits showed milder sleep issues. Mania was the only trait associated with an overall better sleep profile. Across traits, insomnia and fatigue dominated and sleep variability was least prominent. These findings provide support for both transdiagnostic and disorder-specific targets for prevention and treatment.

Abstract:Abstract Introduction Suicide, a critical global mental health issue, has been linked to sleep difficulties. Previous studies have also found that psychological distress and negative thinking styles could contribute to a higher risk of suicide. While cognitive deficit is one of the most common complaints among individuals with insomnia, its impact on suicidality in the context of insomnia has seldom been investigated. The current study aimed to investigate the relationship between executive functioning and suicidality among youth with insomnia. Methods Adolescents with DSM-5 insomnia disorder were recruited from local schools, universities, and the community in Hong Kong. Eligible participants completed self-reported measures, including Depressive Symptom Index - Suicidality Subscale (DSISS), Hospital Anxiety and Depression Scale (HADS), Insomnia Severity Index (ISI), Ruminative Response Scale (RRS), and were administered Wisconsin Card Sorting Test (WCST). The total scores of HADS were used to indicate psychological distress. Failure to maintain set (FMS) from WCST was used to indicate distractibility, an aspect of executive function, with higher scores suggesting greater distractibility. Results A total of 115 adolescents with insomnia disorder (Age: 18.34 ± 1.56, 14-20, female: 65.22%) were recruited. Multivariate regressions revealed that higher HADS (b=0.06, p=.045), RRS (b=0.05, p<.001), and FMS (b=0.30, p=.035) scores were all significantly associated with DSISS. The results remained significant after controlling for insomnia severity, gender, and age. Conclusion Our findings showed a link between impaired executive functioning, specifically distractibility, and suicidality. Distractibility may be attributed to the reduced suppression of the default mode network, which is associated with depression and rumination, thereby increasing the risk for suicidality. Future neuroimaging research is needed to further explore the underlying mechanism of such associations. Abstract:Introduction Bipolar disorder (BD) is a multifaceted illness encompassing mood, energy, cognitive and biorhythms alterations. Sleep disturbances are common in prodromic, acute and inter-episodic phases of BD. Suicidality presents a known association with sleep disturbances. However, their interplay in BD remains intricate and not fully elucidated. Objectives The aim of the present systematic review (SR) and meta-analysis (MA) is to summarise the available evidence and to provide an estimate of the association between sleep disturbances and suicidality, defined as presence of suicide ideation, behaviour and suicide attempts, in patients with BD. Methods We conducted a comprehensive literature search following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines across PubMed, PsycINFO, and SCOPUS databases. We included all studies reporting an association between sleep problems and suicidal behaviour in BD patients. No language restriction was imposed. Effect sizes were calculated as odds ratio (OR) for dichotomic variables, standard mean difference (SMD) for continuous outcomes, and Spearman's coefficient (r) for correlations. Heterogeneity was assessed using the I2 statistic. Global inconsistency was evaluated using the Q statistics with the corresponding p-value. Results The initial search yielded 911 unique abstracts, of which 62 underwent full-text screening. Fourteen publications were included, comprising twelve cross-sectional and two longitudinal studies. The total sample consisted of 19,601 subjects diagnosed with BD, of which 51.76% were females and 69.52% had a diagnosis of BD type 1. We found that people with BD and sleep disturbances tend to have higher suicidality, both current (SMD=0.79, 95%CI=0.53, 1.05) and lifetime (OR=1.8; 95%CI=1.41, 2.55), when compared with people with BD and no sleep disturbances. Additionally, patients with BD and a history of suicide attempts tend to have more sleep problems (OR=1.37, 95%CI=1.21, 1.55). Moreover, a positive correlation exists between suicidality and poor sleep quality measured by the Pittsburgh Sleep Quality Index (PSQI) (r= 0.24, 95%CI=0.10, 0.36). No heterogeneity was found, except in the subanalysis of correlation (I2=66.67%, Q p-value=0.01). Conclusions Our SRMA outlines a positive relation between sleep disturbances and suicidality in patients with BD. The small number of included studies and the scarcity of longitudinal studies, preventing the inference of a causal relationship, represent the major limitations of this study. Also, studies with objective measures of sleep alterations are currently lacking. The prompt recognition, objective measurement, and treatment of sleep alterations could be crucial in averting or reducing suicidal attempts in BD. Disclosure of Interest C. Abstract:Introduction: Little was known about the relationship between sleep disturbances and depressive and anxiety disorders, as well as the efficacy of treatment regimens. Methods: During 2021 to 2023, a total of 417 participants were screened by Hamilton Depression Rating Scale (HAMD-17) and Hamilton Anxiety Rating Scale (HAMA-14) for psychological status, and Pittsburgh sleep quality index (PSQI) assessment. 409 participants were finally enrolled, of which 188 (45.97%) were suffered from sleep disorders. All participants were received polysomnography (PSG) followed by six-week pharmacological treatment of escitalopram and zopiclone, and finally assessed by HAMD-17 and HAMA-14 for treatment efficacy. Result: PSG monitoring indicated that participants with depression experienced prolonged rapid eye movement sleep latency (REMSL) and increased wakefulness after sleep onset (WASO) (P=0.030 and P=0.002, respectively). Those with anxiety disorders demonstrated a significantly higher percentage of non-rapid eye movement sleep (NREM%) and reduced WASO (P=0.013 and P=0.001, respectively). After six-weeks pharmacological treatment, participants with or without sleep disorders exhibited with similar efficacy outcomes of depression and anxiety disorders (P>0.05). However, every point of PSQI increment at baseline would decrease 0.78 and 0.85 times of probability of effective pharmacological treatment of depression and anxiety disorders. Moreover, participants with both effective outcomes of depression and anxiety disorders were found significant shorter sleep onset latency (SOL) (P<0.001). Discussion: The insights gained underscore the necessity of considering sleep disturbances in enhancing the overall effectiveness of pharmacological treatments for depression and anxiety disorders.

Abstract:Objective Poor sleep quality is common in patients with schizophrenia but estimated prevalence rates in this population have been mixed. This systematic review and meta-analysis examined the prevalence of poor sleep quality in schizophrenia samples and moderators of prevalence from epidemiological studies as well as the risk of poor sleep quality in schizophrenia patients based on case-control studies. Methods Both international (PubMed, Web of Science, PsycINFO, EMBASE) and Chinese databases [Chinese Nation knowledge Infrastructure (CNKI) and WANFANG] were systematically searched. Studies that estimated the prevalence of poor sleep quality in schizophrenia were analyzed using a random effects model. Funnel plots and Egger's tests were used to assess publication bias. Statistical analyses were performed using R software. Results In total, 23 epidemiological studies and nine case-control studies were included. Based on the epidemiological studies, the pooled overall prevalence of poor sleep quality was 63.4 % [95 % confidence interval (CI): 57.0 %–69.9 %]. Additionally, based on the nine case-control studies, schizophrenia patients had a significantly higher risk for poor sleep quality compared to healthy controls [odd ratio (OR) = 4.5; 95%CI: 2.4–8.3; P < 0.0001]. Conclusion Poor sleep quality is common among schizophrenia patients. Considering negative outcomes caused by poor sleep quality, regular screening on poor sleep quality should be conducted and effective interventions should be provided to those in need.

Abstract:Objective: This study aims to analyze the risk factors associated with sleep disorders in patients suffering from depression and investigate the efficacy of psychological interventions in improving these conditions. Methods: A comparative observational study was conducted and a cohort of 162 patients aged 18 to 68, admitted to outpatient or inpatient departments between October 2022 and August 2023, were included in the study. All patients were diagnosed with depression according to the ICD-10 criteria. The patients were divided into two groups: experimental group 1 received cognitive-behavioral therapy (CBT) psychological intervention, while experimental group 2 received conventional psychological treatment. Various parameters, including Hamilton Depression Rating Scale (Ham-D), Hamilton Anxiety Rating Scale (Ham-A), Social Support Rating Scale (SSRS), marital status, and occupation, were assessed and compared between the groups. Multivariate logistic regression was employed to identify risk factors for sleep disorders in depressed patients. The Pittsburgh Sleep Quality Index (PSQI) was utilized to evaluate sleep quality. Results: Logistic regression analysis revealed that depression severity, anxiety levels, subjective social support, and occupational status were significant risk factors for sleep disorders in depressed patients (P < .05). Following intervention, both groups exhibited a significant decrease in PSQI, SAS, and SDS scores, with experimental group 1 demonstrating significantly lower PSQI scores compared to experimental group 2 (P < .05). Moreover, experimental group 1 displayed significantly lower SAS and SDS scores compared to experimental group 2 (P < .05). Conclusions: The severity of depression, anxiety levels, subjective social support, and occupational status contribute to the risk of sleep disorders in depressed patients. Implementing psychological interventions for depressed patients with sleep disorders can effectively improve sleep quality, alleviate anxiety, and enhance overall psychological well-being. These interventions represent a valuable approach to manage depression and comorbid sleep disorders.

Abstract:Introduction Sleep disturbances have been long documented in people with schizophrenia (PwS) and are associated with poor outcomes such as depression and suicidality. Past studies suggest that worse self-reported sleep and poor objective sleep will predict worse depression outcomes among PwS and non-psychiatric participants. Few studies have examined the effects of both subjective and objective sleep measures on depressive symptoms among both PwS and NCs. Understanding these relationships may aid in identifying risk factors and treatment targets for depression among PwS and NCs. In this study, we examined the effects of subjective and objective (assessed by a fitness tracker) sleep measures on depressive symptoms among PwS. We hypothesized that 1) Poor subjective sleep would be associated with depression in both PwS and NCs. 2) Shorter total sleep time, later bed-times/wake-times and more variable bed-times/wake-times would be associated with depression in both PwS and NCs, 3) People with poor subjective sleep quality will also have worse objective sleep measures (shorter total sleep time, later and more variable bed-times and wake-times). Methods The sample included 375 adult participants ages 30 to 70, 44% were PwS and 56% were NCs. Depression was measured by the Calgary Depression Scale for Depression for Schizophrenia. Subjective sleep quality was measured by the Pittsburgh Sleep Quality Index (PSQI), a 17-item self-report inventory on sleep quality as perceived by the respondent over the past month. Poor subjective sleep quality was defined as a global PSQI score >5. Objective sleep variables were assessed using a Fitbit Charge 5 smartwatch worn for 6 days. Objective sleep measures included mean total sleep time, bed-times, wake-times and standard deviation of total sleep time, bed-times, and wake-times. Analyses included T-tests, Chi-square tests and general linear models. Results The sample included 165 PwS and 210 NCs, of which 52% were women and mean age was 53.6 years. We found that 63% of PwS and 53% of NCs had poor subjective sleep quality. Poor subjective sleep quality was associated with depression among PwS and NCs (B=0.253, SE=0.082, t=3.07, p<0.005) for PwS and (B= 0.218, SE = 0.037, t=5.83, p<0.001) for NC's. Older age was associated with depression among NCs only (B= 0.028, SE=0.010, t=2.74, p<0.05). Objective sleep measures were not significantly associated with depression in both groups, though there was a trend for shorter total sleep time being associated with worse depression among NCs (B=-.009, SE =.005, p=.067, partial eta-squared=.104). NCs with poor subjective sleep quality had later bedtimes (t=-2.90, p=.006, d =-.91), compared to NCs with good sleep quality. PwS with poor subjective sleep quality had similar objective sleep measures(mean total sleep time, bed-times, wake-time, standard deviation of total sleep time, bed-times and wake-times) when compared to PwS with good sleep quality. Conclusions Our results support that subjective, but not objective, sleep measures are associated with depression. This suggests that depression is more strongly associated with the perception of sleep than to sleep itself - i.e., depression may affect the experience of sleep or ability to recall sleep. Regardless of diagnostic group, more than half of the sample reported poor sleep quality. Moreover, poor subjective sleep quality was only associated with objective sleep measures among NCs. Optimal objective sleep measures likely vary by individuals. Among PwS, effects of medications like antipsychotics and lifestyle differences may affect the quality of sleep, despite maintaining the duration of sleep. Optimizing sleep for PwS should consider factors such as high rates of disability (affecting daily routines), medication usage, and sleeping environments (shared rooms, etc.). Future directions include investigating the impact of antipsychotic medication, effects of untreated obstructive sleep apnea, and nature of longitudinal relationships between sleep and depression on sleep quality.

Abstract:Background Subjective reports of insomnia and hypersomnia are common in bipolar disorder (BD). It is unclear to what extent these relate to underlying circadian rhythm disturbance (CRD). In this study we aimed to objectively assess sleep and circadian rhythm in a cohort of patients with BD compared to matched controls. Method Forty-six patients with BD and 42 controls had comprehensive sleep/circadian rhythm assessment with respiratory sleep studies, prolonged accelerometry over 3 weeks, sleep questionnaires and diaries, melatonin levels, alongside mood, psychosocial functioning and quality of life (QoL) questionnaires. Results Twenty-three (50%) patients with BD had abnormal sleep, of whom 12 (52%) had CRD and 29% had obstructive sleep apnoea. Patients with abnormal sleep had lower 24-h melatonin secretion compared to controls and patients with normal sleep. Abnormal sleep/CRD in BD was associated with impaired functioning and worse QoL. Conclusions BD is associated with high rates of abnormal sleep and CRD. The association between these disorders, mood and functioning, and the direction of causality, warrants further investigation.

Abstract:Abstract Introduction Autonomic nervous system dysfunction and reduced heart rate variability (HRV) have been reported in individuals with mood disorders, a phenomenon likely to be influenced by sleep disturbances. Several studies have previously assessed HRV in individuals with major depression or bipolar disorder across the entire sleep period. This study investigated whether distinct heart rate (HR) and HRV profiles across wake, rapid eye movement (REM) sleep, and non-REM (NREM) sleep are linked to unipolar versus bipolar mood disorders in individuals with sleep complaints. Methods Polysomnographic data was retrospectively collated for 120 adult patients with sleep complaints and depressive symptoms referred to a specialized sleep clinic for sleep assessment [60 diagnosed with bipolar disorder (70% female, mean age= 43.4±11.6 years) and 60 age-matched cases diagnosed with a unipolar depressive disorder (68.3% female, mean age= 43.2±11.6 years)], and 60 age-matched healthy controls (68.3% female, mean age= 43.4±12.6 years). HR and time-based HRV parameters were computed on 30-second segments and averaged across the night for wake and sleep stages. Results Significant group by sleep stage interactions showed that the unipolar and bipolar groups had lower standard deviation of normal-to-normal intervals (SDNN) and vagal tone root mean square of successive R-R interval differences (RMSSD) compared to controls during NREM sleep ( p≤.001) and REM sleep (p≤.003), but not during wake (p>.050). The unipolar group had significantly higher heart rate than controls regardless of sleep stages (all, p≤ .042), while the bipolar group had higher heart rate than controls only during NREM 2 (p=.012) and NREM 3 (p=.009) sleep. These interactions persisted after excluding individuals taking antipsychotic, lithium, anticonvulsant, and cardiovascular medications. Conclusion While additional research is required to account for manic and euthymic states, as well as the impact of psychotropic and cardiac medications, and potential confounders like variations in body mass index, the present findings suggest that the sleep-based autonomic signature of depressive states differs across different types of mood disorders and could potentially inform the development of biomarkers and therapeutic targets.
