Abstract:Purpose: The objective of this study was to compare and assess the accuracy of computed tomography (CT)-based target delineation with that of magnetic resonance imaging (MRI)-based on high-dose-rate brachytherapy (HDR-BT) for patients with cervical cancer. Material and methods: Data of 20 patients with locally advanced cervical cancer were collected and evaluated. Dimensions, conformity, and dose parameters of high-risk clinical target volume (CTVHR) as well as D-0.(1cc), D-1(cc), and D-2cc of organs at risk (OARs) based on MRI were compared with those based on CT. Results: Average age of 20 patients included was 57.8 years. Width, thickness, and volumes of CT-based CTVHR (CTVHR-CT) were significantly overestimated compared with those of MRI-based CTVHR (CTVHR-MR). Mean values of dice similarity coefficient (DSC), Hausdorff distance (HD), and centroid distance (AV) of CTVHR were 0.82 cm, 0.96 cm, and 0.35 cm, respectively. Dose values of CTVHR-CT were significantly lower compared with those of CTVHR-MR. Concerning OARs, geometrical and dosimetric values on CT were comparable to those on MRI. Conclusions: The delineated ranges of CTVHR were significantly over-estimated on CT compared with MRI. D-98 and D-90 of CTVHR-CT were lower than CTVHR-MR. DSC and AV of CTVHR and CTVIR were similar to each other; however, there was a difference in terms of HD. CT images regarding pre-BT MR images for delineating were not enough and MRI fusion is still required.

Abstract:PURPOSE: The aim of this overview was to assess the accuracy of computed tomography (CT)- based imaging technology and to compare it with magnetic resonance imaging (MRI) for the treatment planning of high dose rate brachytherapy in cervical cancer.METHODS AND MATERIALS: A systematic search in PubMed, Embase, and the Cochrane Central was performed to identify clinical studies involving brachytherapy of cervical cancer and published before February 1, 2016. Outcomes of interest were geometric dimensions, dose parameters, and clinical results.RESULTS: After screening 675 articles, 13 clinical studies involving 465 patients were included for critical appraisal; 10 studies compared CT with MRI and three compared hybrid (CT/MRI based) with MRI only based imaging technologies. The geometric dimensions of the high-risk clinical target volume (HR-CTV), dose parameters, and clinical outcomes were reported in 11, 10, and 1 studies, respectively.CONCLUSIONS: Among those geometric parameters of HR-CTV, width was significantly overestimated on CT compared with MRI. Height might be underestimated, and thickness was comparable. The dose parameters for HR-CTV were lower on CT only-based technique compared with MRI-based one. It is proposed that at least one pre- or at brachytherapy MRI is required to assess the tumor extension. With the help of MRI information, CT contouring will be much more accurate in cervical cancer brachytherapy. (C) 2016 American Brachytherapy Society. Published by Elsevier Inc. All rights reserved.

After breast cancer is diagnosed, surgery is one of the primary treatments. Assessing the extent of breast cancer is vitally important to formulate the best surgical option, obtain clear margins while minimizing the volume of removed tissue, and improve the treatment outcome and patient’s quality of life. Currently, breast surgery relies heavily on Breast MRI (bMRI) for preoperative assessment. However, bMRI may overestimate the tumor sizes. bMRI is contraindicated in patients with claustrophobia, metal implants, and allergy to gadolinium contrast medium. In addition, bMRI is generally expensive and time-consuming. Some patients find bMRI less favorable for cost and time concerns. Cone Beam Breast CT (CBBCT) is a new modality using Cone Beam CT technology and iodinated contrast media for true 3D high-resolution imaging of the breast. Our previous studies have proved the feasibility of CBBCT in the preoperative assessment of breast cancer. A large-scale study has been done to further examine the clinical significance of CBBCT in pre-operative assessment compared with bMRI. Methods: 273 female primary breast cancer patients were included in this study. The maximal diameters of the tumors were measured in CBBCT and bMRI images by an experienced radiologist. The differences between the image-measured size and surgical pathology size (gold standard) were calculated as ΔDCT and ΔDMR. ΔD ˃ 5mm was considered an overestimation and ΔD ˂-5mm was considered an underestimation. The absolute values of ΔDCT and ΔDMR were also calculated as ΔDCT_abs and ΔDMR_abs. A Student t-test was used to compare the statistical differences between ΔDCT_abs and ΔDMR_abs. Among ΔDCT_abs and ΔDMR_abs, a 5mm cut-off value was used to determine the concordance between image and pathology. The concordance rates of CBBCT-Pathology and bMRI-Pathology were tested with the χ2 method. Factors that may affect the accuracy of tumor size measurement were analyzed. Results: Overall there were no statistically significant differences between ΔDCT_abs and ΔDMR_abs. (0.58±0.68cm vs. 0.65±0.68cm; t=1.653, P=0.100). Using 5mm as the cut-off value of concordance, there was no significant difference in the CBBCT-Pathology and bMRI-Pathology concordance rates (64.8% vs. 60.1%, χ2 =1.320, P=0.251). The overestimation rate of CBBCT was significantly lower than that of bMRI (11.4% vs. 19.4%, χ2=6.810, P=0.009). But there was no significant difference in the underestimation rates (23.8% vs. 20.5%, χ2=0.860, P=0.354). ΔDCT_abs is significantly smaller than ΔDMR_abs among the invasive ductal carcinoma group (P=0.017), the premenopausal group (P=0.004), and the small tumor size (≤2cm) group (P=0.003). ΔDCT_abs and ΔDMR_abs.have no significant differences among the lymph node metastasis group, multi-lesion group, and enhancement type group (P > 0.05). Conclusions: CBBCT is concordant with surgical pathology in breast tumor size measurement and provides more accurate tumor size estimation compared with bMRI. Abstract:INTRODUCTION: To compare the diagnostic performance of cone-beam breast computed tomography (CBBCT) and mammography (MG) in primary breast cancer.METHODS: PubMed, Embase, Web of Science, China National Knowledge Infrastructure, WanFang DATA, and China Science and Technology Journal databases were searched comprehensively from inception to March 2023. Sensitivity and specificity were calculated using bivariate random-effects models, and a summary receiver operating characteristic (SROC) curve was constructed. Bivariate I<sup>2</sup> statistics and meta-regression analyses were also performed. The differences in diagnostic performance between CBBCT and MG were analysed using Z-test statistics. Clinical utility was explored using Fagan's nomogram, and quality assessment was conducted utilising the Quality Assessment of Diagnostic Accuracy Studies-2 checklist.RESULTS: The summary sensitivity and specificity for CBBCT in diagnosing primary breast cancer were 0.92 (95 % CI: 0.87-0.94) and 0.79 (95 % CI: 0.71-0.85), respectively, and the area under the curve (AUC) of the SROC was 0.93 (95 % CI: 0.90-0.95). For MG, the summary sensitivity and specificity were 0.77 (95 % CI: 0.69-0.83) and 0.75 (95 % CI: 0.66-0.82), respectively, with an AUC of 0.83 (95 % CI: 0.80-0.86). The Z-test revealed that the summary sensitivity of CBBCT was significantly higher than that of MG (P < 0.001). Additionally, the summary AUC of CBBCT was significantly higher than that of MG (P < 0.001).CONCLUSION: The diagnostic performance of CBBCT for primary breast cancer was better than that of MG. However, the results of both the CBBCT and MG are based on studies with small sample sizes. Further studies with larger sample sizes and more comprehensive designs are required to address this issue.

Abstract:Objective Breast cancer is the most frequently diagnosed cancer in women. Accurate evaluation of the size and extent of the tumor is crucial in selecting a suitable surgical method for patients with breast cancer. Both overestimation and underestimation have important adverse effects on patient care. This study aimed to evaluate the accuracy of breast magnetic resonance imaging（MRI） and ultrasound（US） examination for measuring the size and extent of early-stage breast neoplasms.Methods The longest diameter of breast tumors in patients with T 1–2 N 0–1 M 0 invasive breast cancer preparing for breast-conserving surgery（BCS） was measured preoperatively by using both MRI and US and their accuracy was compared with that of postoperative pathologic examination. If the diameter difference was within 2 mm, it was considered to be consistent with pathologic examination.Results A total of 36 patients were imaged using both MRI and US. The mean longest diameter of the tumors on MRI, US, and postoperative pathologic examination was 20.86 mm ± 4.09 mm（range: 11–27 mm）, 16.14 mm ± 4.91 mm（range: 6–26 mm）, and 18.36 mm ± 3.88 mm（range: 9–24 mm）. US examination underestimated the size of the tumor compared to that determined using pathologic examination（t = 3.49, P ＜ 0.01）, while MRI overestimated it（t =-6.35, P ＜ 0.01）. The linear correlation coefficients between the image measurements and pathologic tumor size were r = 0.826（P ＜ 0.01） for MRI and r = 0.645（P ＜ 0.01） for US. The rate of consistency of MRI and US compared to that with pathologic examination was 88.89% and 80.65%, respectively, and there was no statistically significant difference between them（χ~2 = 0.80, P ＞ 0.05）.Conclusion MRI and US are both effective methods to assess the size of breast tumors, and they maintain good consistency with pathologic examination. MRI has a better correlation with pathology. However, we should be careful about the risk of inaccurate size estimation.
Accuracy of Preoperative Contrast-enhanced Cone Beam Breast CT in Assessment of Residual Tumor after Neoadjuvant Chemotherapy: A Comparative Study with Breast MRI

Abstract:Rationale and Objectives To compare the accuracy of preoperative contrast-enhanced cone beam breast CT (CE-CBBCT) and MRI in assessment of residual tumor after neoadjuvant chemotherapy (NAC). Materials and methods Residual tumor assessments in 91 female patients were performed on preoperative CE-CBBCT and MRI images after NAC. The agreements of tumor size between imaging and pathology were tested by Intraclass Correlation Coefficient (ICC). Subgroup analyses were set according to ductal carcinoma in situ (DCIS), calcifications and molecular subtypes. Correlated-samples Wilcoxon Signed-rank test was used to analyze the difference between imaging and pathology in total and subgroups. AUC, sensitivity, specificity, PPV, and NPV were calculated to compare the performance of CE-CBBCT and MRI in predicting pathological complete response (pCR). Results Comparing with pathology, the agreement on CE-CBBCT was good (ICC = 0.64, 95% CI, 0.35-0.78), whereas on MRI was moderate (ICC = 0.59, 95% CI, 0.36-0.77), and overestimation on CE-CBBCT was less than that on MRI (median (interquartile range, IQR): 0.24 [0.00, 1.31] cm vs. 0.67 [0.00, 1.81] cm; p = 0.000). In subgroup analysis, CE-CBBCT showed superior accuracy than MRI when residual DCIS ( p = 0.000) and calcifications ( p = 0.000) contained, as well as luminal A ( p = 0.043) and luminal B ( p = 0.009) breast cancer. CE-CBBCT and MRI performed comparable in predicting pCR, AUCs were 0.749 and 0.733 respectively ( p > 0.05). Conclusion CE-CBBCT showed superior accuracy in assessment of residual tumor compared with MRI, especially when residual DCIS or calcifications contained and luminal subtype. The performance of preoperative CE-CBBCT in predicting pCR was comparable to MRI. CE-CBBCT could be an alternative method used for preoperative assessment after NAC.

Abstract:Purpose: To compare breast density measurements using CBCT with those based on mammograms. Materials and Methods: 19 mastectomy specimens were imaged with a bench top aSi/CsI flat panel based CBCT system. Scans were performed at 80 kVp with a voxel size of 0.29 mm and an estimated dose level of 24 mGy at the isocenter. The reconstructed images were corrected and segmented to separate glandular tissue from adipose tissue. The voxel numbers for glandular tissue and the entire breast were counted from the segmented images and used to calculate the volume based breast density. The breast density was also calculated based on mammograms using a commercial software package. The number of pixels for dense tissue regions was counted and divided by that for the entire breast to compute the area based breast density. These two breast densities were used for comparison. The volume based breast densities were also converted into BI‐RADS categories for comparison with those from mammographic reading as recorded in the diagnostic report. Results: The volume based breast densities were mostly equal or lower than the mammographic ones (fig. 1 and Table 1) with a correlation (coefficient = 0.73) between the two. Comparison based on the BI‐RADS categories, more breasts with volume based measure were classified as categories 1 and 2 while most breasts with mammographic reading were classified as category 3 (Fig. 2) with a correlation coefficient of 0.55. Conclusion: It was found that breast densities calculated from CBCT images were significantly lower than those from mammograms for the 19 patients studied. Abstract:Abstract Breast cancer (BC) is the most commonly diagnosed non‐skin cancer in women. To achieve early and accurate diagnosis, three‐dimensional (3D) cone‐beam breast computed tomography (CBBCT) and digital breast tomosynthesis (DBT) modalities are used. Importantly, the comparison of reconstruction accuracy of both CBBCT and DBT has rarely been investigated, thus constituting a research gap. This study systematically investigated the performances of the CBBCT and DBT for different reconstruction algorithms using both BR3D breast phantom and breast images. We acquired clinical breast images and scanned the BR3D phantom for additional breast images. These acquired images were used to simulate projection data using predefined CBBCT and DBT geometries. The simulated projections were reconstructed using five different reconstruction algorithms. To evaluate the reconstruction accuracy, we calculated average image quality assessment (IQA) indices, including peak signal‐to‐noise ratio (PSNR), structural similarity index (SSIM), root mean square error (RMSE), and others, across different algorithms and modalities. The pooled PSNR, SSIM, and RMSE for DBT and CBBCT images are (31.6265 ± 0.8725), (0.9353 ± 0.0077), and (0.0270 ± 0.0025) and (29.7007 ± 0.9249), (0.9136 ± 0.0130), and (0.0342 ± 0.0040), which implies that the overall IQA indices of DBT are superior to CBBCT; therefore, DBT tends to reveal more BC detectability as the diagnosis outcome would largely depend on good quality images. The results show that DBT gives an improved result for all algorithms compared to CBBCT, although further experimental trials may be needed to establish the findings fully. The findings suggest that using DBT may enhance the accuracy of BC diagnosis compared to CBBCT due to its superior image quality in clinical practice, emphasizing the importance of selecting optimal reconstruction algorithms for improved diagnostic outcomes.

Abstract:Purpose: The purpose of this study was to evaluate magnetic resonance imaging (MRI)-pathology concordance of tumour size in patients with invasive breast carcinoma, with an emphasis on Breast Imaging Reporting and Data System (BI-RADS) descriptors of dynamic contrast-enhanced MRI (DCE-MRI). Material and methods: Of patients who had preoperative DCE-MRI, 94 were enrolled. Concordance between MRI and the pathological findings was defined as a difference in tumour size of 5 mm or less. The greatest dimension was measured by two radiologists, and BI-RADS descriptives were described in accordance. The gold standard was chosen as the pathologic assessment. Results: Tumour measurements determined by MRI and the pathological reports were not statistically different (2.64 ± 1.16 cm, Wilcaxon Z = -1.853, p = 0.064). Tumour sizes were concordant in 72/94 patients (76.6%). The mean difference between the pathological and MRI tumour sizes was -0.1 cm. MRI overestimated the size of 17/94 tumours (18.1%) and underestimated the size of 5/94 tumours (5.3%). Discordance was associated with larger tumour size. Histologic and molecular type of tumours, patient age, histologic grade, lymphovascular invasion or perineural invasion positivity, fibroglandular volume, background parenchymal enhancement, and being mass or non-mass were not associated with concordance. Irregular margin and heterogenous enhancement in DCE-MRI were associated with discordance in logistic regression analysis (p = 0.035, OR: 4.24; p = 0.021, OR: 4.96). Conclusions: Two BI-RADS descriptors of irregular contour and heterogeneous contrast uptake were found to be associated with tumour size discrepancy. This might be attributed to the dynamic and morphologic specialities of tumours primarily rather than tumour biology.
Diagnostic Efficacy of Contrast-Enhanced Ultrasound for Breast Lesions of Different Sizes: a Comparative Study with Magnetic Resonance Imaging.

Abstract:Objective: This study aimed to compare the diagnostic performance of contrast-enhanced ultrasound (CEUS), MRI, and the combined use of the two modalities for differentiating breast lesions of different sizes. Methods: A total of 406 patients with 406 solid breast masses detected by conventional ultrasound underwent both CEUS and MRI scans. Histological results were used as reference standards. The lesions were categorized into three groups according to size (Group 1, <= 20 mm; Group 2, > 20 mm, Group 3: total lesions). Sensitivity, specificity, accuracy, and receiver operating characteristic (ROC) curve analysis were used to assess the diagnostic performance of these imaging methods for breast lesions. Results: There were 194 benign and 212 malignant breast lesions according to the histological diagnosis. Compared with MRI, CEUS demonstrated similar sensitivity in detecting breast cancer (p = 1.0000 for all) in all the three groups. With regard to specificity, accuracy, and the area under the ROC curve (Az) values, MRI showed a better performance than that shown by CEUS (p < 0.05 for all), and the combination of the two modalities improved the diagnostic performance of CEUS alone significantly (p < 0.05 for all) in all the three groups. However, the diagnostic specificity and accuracy of the combined method was not superior to that of MRI alone except for Group 2. Conclusion: CEUS demonstrated good sensitivity in detecting breast cancer, and the combined use with MRI can optimize the diagnostic specificity and accuracy in breast cancer prediction. Advances in knowledge: Few studies have compared the diagnostic efficacy of CEUS and MRI, and this study is the first attempt to seek out the diagnostic values for breast lesions of variable sizes (lesions with <= 20mm and >20 mm).

Abstract:Objective To explore the diagnostic accuracy of MRI in diagnosing the small breast cancer.Methods From June 2008 to June 2009,a total of 62 cases suspected by the surgeons as small breast cancer(≤2 cm),based on the information offered by clinical physical examination,mammography(MG),or/and ultrasound,were performed with breast MRI.Of them,31 had a recent mammography study.All of the imaging features of these 62 cases were retrospectively reviewed and categorized as 3,4,and 5 by using the Breast Imaging Reporting and Data System(BI-RADS) for analysis.Every case was confirmed by surgical pathology.The detection rate,sensitivity,and positive predictive value were compared between the MRI and MG.Results Fifty cases were pathologically confirmed as small breast cancer.Among the fifty cases,21 had a recent mammography study.The detection rate,sensitivity,and positive predictive value of MRI and mammography for the small breast cancer were 100%,94.0%,81.0% vs 85.7%,61.9%,65.0%,respectively(P0.05).Conclusions The detection rate and sensitivity of the MRI in diagnosing the small breast cancer are much higher than those of the MG.

Abstract:ObjectivesTo compare the background parenchymal enhancement (BPE) levels on contrast-enhanced cone-beam breast CT (CE-CBBCT) and MRI, evaluate inter-reader reliability, and analyze the relationship between clinical factors and BPE level on CE-CBBCT.MethodsIn this retrospective study, patients who underwent both CE-CBBCT and MRI were analyzed. BPE levels on CE-CBBCT and MRI were assessed by five specialists independently in random fashion, with a wash-out period of 4 weeks. Weighted kappa was used to analyze the agreement between CE-CBBCT and MRI, and intraclass correlation coefficient (ICC) was used to evaluate the inter-reader reliability for each modality. The association between BPE level on CE-CBBCT and clinical factors was evaluated by univariate and multivariate logistic regression.ResultsA total of 221 patients from January 2017 to April 2021 were enrolled. CE-CBBCT showed substantial agreement (weighted kappa = 0.690) with MRI for BPE evaluation, with good degree of inter-reader reliability on both CE-CBBCT (ICC = 0.712) and MRI (ICC = 0.757). Based on majority reports, BPE levels on CE-CBBCT were lower than MRI (p < 0.001). BPE level on CE-CBBCT was significantly associated with menstrual status (odds ratio, OR = 0.125), breast density (OR = 2.308), and previously treated breast cancer (OR = 0.052) (all p < 0.05). BPE level for premenopausal patients was associated with menstrual cycle, with lower BPE level for the 2nd week of menstrual cycle (OR = 0.246).ConclusionsCE-CBBCT showed substantial agreement and comparable inter-reader reliability with MRI for BPE evaluation, indicating that the corresponding BI-RADS lexicons could be used to describe BPE level on CE-CBBCT. The 2nd week of menstrual cycle timing is suggested as the optimal examination period for CE-CBBCT.Key Points• CE-CBBCT showed substantial agreement and comparable inter-reader reliability with MRI for BPE evaluation.• Menstrual status, breast density, and previously treated breast cancer were associated with the BPE level on CE-CBBCT images.• The 2ndweek of the menstrual cycle is suggested as the optimal examination period for CE-CBBCT.

Abstract:Background Earlier studies have examined the association between the diameter of primary tumors measured by magnetic resonance imaging (MRI) and histopathology in breast cancer patients. However, the diameter does not completely describe the dimensions of the breast tumor or its volumetric proportion relative to the whole breast. The association between breast tumor volume/breast volume ratios measured by these two techniques has not been reported.Methods Seventy-three patients were recruited from female patients with primary breast tumors admitted to our center between January and December 2010. They were divided into two groups. Group A (n=46) underwent modified radical mastectomy (MRM), and Group B (n=27) underwent preoperative neoadjuvant chemotherapy before MRM. They were examined by dynamic-contrast enhanced MRI (DCE-MRI) to measure breast volumes (BVs), tumor volumes (TVs), and tumor volume/breast volume ratios (TV/BV). These measurements were compared with histopathology results after MRM, and the associations between MRI and pathology were analyzed by linear regression and Bland-Altman analysis.Results For Group A, the correlation coefficients for BVs, TVs, and TV/BV ratios measured by the two techniques were 0.938, 0.921, and 0.897 (all P<0.001), respectively. For Group B, the correlation coefficients for BVs, TVs, and TV/BV ratios were 0.936, 0.902, and 0.869 (all P<0.01), respectively. The results suggest statistically significant correlations between these parameters measured by the two techniques for both groups.Conclusion For these patients, BVs, TVs, and TV/BV ratios measured by DCE-MRI significantly correlated with those determined by histopathology. Abstract:Objective To assess the value of contrast enhanced mammography (CESM) in the detection of breast cancer. Methods A total of 145 patients who were suspected of breast abnormalities by clinical examination or ultrasound were prospectively collected. All patients underwent bilateral breast CESM and MRI examinations. Breast CESM and MRI examination were completed in the same week. The pathological specimens were analyzed and the maximum diameters of pathological lesions were measured. The lesions were observed on low energy(LE), CESM and MRI images, and then the maximum diameters of lesions on the above images were measured. Using pathology results as the gold standard, the diagnostic efficacy of LE, CESM and MRI were analyzed by ROC curve, and Z test was used to compare the areas under the ROC curves among different imaging methods. Bland-Altman method was used to analyze the consistency of the maximum diameters of the lesions obtained with different imaging methods. Results One hundred and fifty three lesions were found in 145 patients, in which 36 were benign and 117 were malignant. The LE, CESM and MRI showed 140, 151 and 149 lesions respectively, and the qualitative diagnostic errors were 25, 8 and 11, respectively. The areas under ROC curves of LE, CESM and MRI were 0.87, 0.96 and 0.97 respectively. There was significant difference of the area under ROC curve between CESM and LE, so did the MRI and LE (P<0.05), but there was not significant difference between CESM and MRI (P=0.51). The sensitivity, specificity and accuracy in the diagnosis of malignant breast lesions were 88.98%, 65.71% and 83.66% for LE, while they were 95.80%, 91.18% and 94.77% for CESM, 94.17%, 87.88%and 92.81%for MRI. The average difference of diameters between LE, CESM, MRI and pathologic size was-1.7, 1.1 and 0.3 mm, respectively, with 95%consistency interval range of-18.6 to 15.1,-9.8 to 12.1,-10.6 to 11.2 mm, respectively. There was best consistency between the pathological size and the size on MRI. Conclusion CESM can significantly improve the diagnostic efficacy of breast lesions, which is comparable with MRI.

Abstract:Objective To retrospectively analyze the diagnostic value of dedicated breast Magnetic Resonance Imaging(DBMRI) in diagnosing breast cancer by comparing with Mammography(MG) and ultrasound(US).Methods A total of 612 patients underwent preoperative DBMRI,MG,and US examination were collected and 36 cases received neoadjuvant chemotherapy before the examination were excluded.The remaining 576 cases were included to be analyzed using pathological consequences as the golden criteria.Statistical analysis was performed using SPSS 13.0 for Windows.P value less than 0.05 indicated statistical significance.Results For detecting breast cancinoma,DBMRI had higher sensitivity(97.1%) than MG(77.6%)(χ2=53.904,P=0.000) and US(91.0%)(χ2=10.370,P=0.001).MG had higher specificity(88.8%)than DBMRI(82.8%)(χ2=3.925,P=0.048) and US(82.5%)(χ2=4.383,P=0.036).For detecting DCIS,DBMRI had higher sensitivity(97.9%) than US(77.8%)(χ2=29.445,P=0.000) and MG(71.1%)(χ2=9.680,P=0.002).For detecting breast carcinoma in dense breast patients,the sensitivity of DBMRI(96.0%) and US(92.1%) was higher than MG(66.7%).For detecting breast carcinoma in non dense breast patients,the sensitivity of DBMRI(97.9%) was higher than MG(85%)(χ2=19.684,P=0.000) and US(90.4%)(χ2=19.684,P=0.002).The sensitivity of DBMRI,MG,and US in detecting breast cancer with calcifications was 98.3%,94.0%,and 94.8%.Conclusion Compared with MG and US,DBMRI has a higher sensitivity and lower specificity in detecting breast cancer.It has more valuable than MG and US in diagnosing DCIS.DBMRI and US have a higher sensitivity in detecting breast cancer in dense breasts than MG.

Abstract:Purpose: To investigates the effects of detector properties and imaging conditions on the imaging properties of cone-beam breast CT with both computer simulations and imaging experiments. Method and Materials: Cone beam breast CT was simulated with the breast analytically modeled as cylinder embedded spherical shape soft tissue masses and calcifications. X-ray spectrum, breast attenuation, geometric magnification, focal spot blurring, x-ray detection, detector blurring, image pixelization and digitization were all incorporated in computing the projection images. Quantum noise, system noise, detector blurring were also simulated and incorporated in the model. Image filtering and reconstruction were then performed using the Feldkamp algorithm. Simulation was performed for two flat-panel detectors, one CsI based and the other a-Se based. Images of phantoms and breast specimens were also obtained to demonstrate the ability of our experimental cone beam breast CT system to image the 3-D structures of the breast with embedded cancers and calcifications. Results: Our simulation results shows that the a-Se detector performs slightly better at 30 and 40 kVp's while the CsI detector performs better at 50 or higher kVp's. Image SNRs are optimized at 50 and 60 kVp for the s-Se and CsI detector, respectively. Phantom images obtained with our experimental system show that with higher dose and smaller pixel size, calcifications as small as could be resolved. Images of breast specimens show excellent separation between glandular and adipose tissues. The speculated nature of the tumor masses can be clearly seen in selected projection while ambiguous in other projections or in regular mammograms. It was also found that inclusion of surgical clips (used to indicate tumor location) had caused detrimental reconstruction artifacts. Abstract:Objective:To compare the performance of mammography,magnetic resonance image and positron emission tomography/single photon emission computed tomography(PET/SPECT) in the diagnosis of breast cancer by Meta analysis.Methods: Based on the assessment standard of evidencebased diagnostic experiment,we searched PubMed,OVID,and Medline database(1986-2006) for the articles on the diagnosis of breast cancer with MG,MRI,PET and/or SPECT.The data and parameters of the 3 diagnostic methods were subjected to Meta analysis.Using software Excel 2003,SPSS13.0 and RevMan 4.2,we calculated the summary sensitivity and specificity,the pooled odds ratio(OR) and likelihood(LR).Summary receiver operating characteristic curve(SROC) was drawn to evaluate accuracy of the 3 methods.Results: We obtained 30 articles,which included 41 data sets: 14 on MG(2 941 lesions),10 on MRI(1 428 lesions),and 17 on PET/SPECT(2 247 lesions).The pooled estimates for sensitivity of MG,MRI and PET/SPECT were 82%(95%CI:76%-86%),86%(95%CI:83%-88%),and 87%(95%CI:83%-90%),respectively;for specificity were 69%(95%CI:62%75%),65%(95%CI:62%-69%),and 82%(95%CI:76%-86%),respectively.Their weighted AUC(area under curve) were 0.84,0.89 and 0.90,respectively;Q* values were 0.77,0.81 and 0.88,respectively.The sensitivities of the 3 methods were not significantly different.The specificity of PET/SPECT was higher than those of MG and MRI(P0.05).The overall diagnostic efficacies of PET/SPECT and MRI were better than that of MG.Conclusion: MG remains to be a satisfactory method for initial diagnosis of breast cancer;MRI examination should first be considered for younger patients.PET/SPECT should be used when a diagnosis of breast cancer can not be confirmed.
