Abstract:Objective:Taking inpatient and outpatient with diabetes in our hospital as research subjects,to investigate the characteristics of TCM syndrome of diabetes by mathematical statistics,and to study the relationship between common syndromes and objective indexes further,such as history of complication,blood fat,MA/CR.Methods:Cross-sectional study on 180 cases of diabetes was carried by questionnaire.Combining with the suggestion of clinic experts,common factor analysis,vari-max procedure and sample cluster were used to determine the TCM syndrome.Then,the dependability of TCM syndromes and objective signs was researched by analysis of variance.Results:Among 137 signs and symptoms,50 were analyzed which had the frequency of occurrence of more than 10% to determine the TCM syndromes of diabetes.Combining with theory of diagnostics of TCM,4 main compound syndromes were acquired:77 cases with defi ciency of both qi and yin and stagnated heat stasis,54 cases with def iciency of kidney qi,39 cases with excessive rising of liver yang and stagnation,10cases with humid heat and stagnation.Further study on dependablity of syndromes and objective indexes showed that syndromes correlated with blood fat,MA/CR and so on at some extent.Conclusion:Def iciency of both qi and yin,stagnated heat stasis were the common syndromes of diabetes.TCM syndromes of diabetes correlated to TC,LDL and MA/CR.

Abstract:Objective The goal of this study was to systematically summarize and categorize the syndrome differentiation, medication rules, and acupoint therapy in the domestic traditional Chinese medicine (TCM) literature on type 2 diabetes mellitus (T2DM), such that guidelines and new insights can be provided for future practitioners and researchers. Methods Taking randomized controlled trials (RCTs) on the treatment of T2DM in TCM as the research theme, we searched for full-text literature in three major clinical databases, including CNKI, Wan Fang, and VIP, published between 1990 and 2020. We then conducted frequency statistics, cluster analysis, association rules extraction, and topic modeling based on a corpus of medical academic words extracted from 3,654 research articles. Results The TCM syndrome types, subjective symptoms, objective indicators, Chinese herbal medicine, acupuncture points, and TCM prescriptions for T2DM were compiled based on invigorating the kidney and Qi , nourishing Yin , and strengthening the spleen. Most TCM syndrome differentiation for T2DM was identified as “ Zhongxiao ” (the lesion in the spleen and stomach) and “ Xiaxiao ” (the lesion in the kidney) deficiency syndromes, and most medications and acupoint therapies were focused on the “Spleen Channel” and “Kidney Channel.” However, stagnation of liver Qi was mentioned less when compared with other syndromes, which did not have symptomatic medicines. Conclusion This study provides an in-depth perspective for the TCM syndrome differentiation, medication rules, and acupoint therapy for T2DM and provides practitioners and researchers with valuable information about the current status and frontier trends of TCM research on T2DM in terms of both diagnosis and treatment.

Efficacy and safety of traditional Chinese medicines combined with conventional Western medicines in the treatment of type 2 diabetes mellitus: a network meta-analysis of randomized controlled trials

Abstract:Aims/hypothesis: Through a comprehensive analysis of the clinical randomized controlled trials of traditional Chinese medicine (TCM) combined with conventional western medicine (CWM) in treating type 2 diabetes(T2DM) in the past ten years, the clinical efficacy and safety of different TCMs combined with CWM were explored. This study aimed to provide specific suggestions for clinical guidance on treating T2DM. Methods: A literature search was conducted in CNKI, WanFang, VIP, CBM, PubMed, Embase, and Web of Science. The search time was limited from 2010 to the present time. The literature type was a controlled clinical trial study of TCM combined with CWM intervention in treating T2DM. The outcome indices of the efficacy evaluation included fasting blood glucose (FBG), 2-hour postprandial blood glucose (2hPG), glycosylated hemoglobin (HbA1c), adverse reactions, and clinical efficacy. Stata 15 and RevMan 5.4 software were used to conduct a network meta-analysis and a traditional meta-analysis. Results: The results showed that shenqi jiangtang granule combined with sulfonylurea, shenqi jiangtang granules combined with metformin and jinlida granules combined with insulin had significant effects on reductions in FBG, 2hPG and clinical efficacy compared with western medicines alone, which included fasting blood glucose [MD=-2.17, 95%CI=(-2.50, -1.85)], blood sugar at 2 hours after a meal [MD=-1.94, 95%CI=(-2.23, -1.65)], and clinical curative effect [OR= 1.73, 95%CI=(0.59, 2.87)]. Conclusions: TCM combined with CWM has a very significant effect on treating T2DM compared with CWM alone. According to the network meta-analysis, the best intervention measures of different TCMs for different outcome indicators were obtained. Systematic review registration: identifier 42022350372.
Efficacy of Pharmacological Monotherapies for Type 2 Diabetes in China: A Systematic Review and Meta-Analysis

Abstract:Background: There are multiple antidiabetic drugs in China, which vary in efficacy and safety. However, no clinical study exists comparing all the alternative drugs simultaneously. This study aims to estimate and compare the efficacy of alternative classes of antidiabetic drugs in the regimen of monotherapy for Chinese patients with type 2 diabetes (T2DM). Methods: Systematic literature review was conducted by retrieving databases including CNKI, WanFang, CQVIP, PubMed, Embase, Web-of-Science, ScienceDirect and Cochrane Library, to identify randomized controlled trials (1990-2016) comparing the efficacy of 'antidiabetic drug monotherapy' vs 'placebo/lifestyle intervention' in Chinese T2DM patients. Three-lines eight-classes of antidiabetic drugs were included based on Chinese guideline: 1st line drug (metformin), 2nd line drug (I±-glycosidase inhibitor, sulfonylurea, glinide, DPP-4 inhibitor, thiazolidinedione), 3rd line drug (insulin, GLP-1 receptor agonist). Meta-analyses were used to estimate the efficacy of antidiabetic drug vs placebo/lifestyle intervention. Findings: 99,072 records were identified through database search, with 46 studies finally included. Overall, compared with placebo/lifestyle intervention, metformin, 2nd line drug and 3rd line drug reduced HbA1c level by 1·04%, 0·72% and 1·22%, BMI level by 1·77kg/m2, 0·24kg/m2 and 1·14kg/m2, TC level by 0·64mmol/l, 0·10mmol/l and 0·58mmol/l, SBP level by 3·13mmHg, 1·37mmHg and 3·00mmHg; while increased HDL-C level by 0·19mmol/l, 0·09mmol/l and 0·15mmol/l, respectively. In 2nd line drugs, the top three treatments for reducing HbA1c level were sulfonylurea (WMD:-1·19;95%CI:-1·56,-0·82), α-glycosidase inhibitor (WMD:-0·90;95%CI:-1·32,-0·47), thiazolidinedione (WMD:-0·80;95%CI:-1·31,-0·29); for reducing BMI level were α-glycosidase inhibitor (WMD:-0·64;95%CI:-1·46,0·17), DPP-4 inhibitor (WMD:-0·27;95%CI:-1·12,0·57), thiazolidinedione (WMD:0·11;95%CI:-0·96,1·17); for reducing TC level were DPP-4 inhibitor (WMD:-0·18;95%CI:-0·35,-0·01), α±-glycosidase inhibitor (WMD:-0·30;95%CI:-1·28,0·68), thiazolidinedione (WMD:0·00;95%CI:-0·30,0·31); for increasing HDL-C level were thiazolidinedione (WMD:0·19;95%CI:0·12,0·26), sulfonylurea (WMD:0·02;95%CI:-0·18,0·22), DPP-4 inhibitor (WMD:0·01;95%CI:-0·05,0·06); for reducing SBP level were α-glycosidase inhibitor (WMD:-3·20;95%CI:-8·39,1·99), DPP-4 inhibitor (WMD:-1·85;95%CI:-4·17,0·47), thiazolidinedione (WMD:1·60;95%CI:-5·22,8·42). In 3rd line drugs, insulin (WMD:-2·37;95%CI:-3·79,-0·95) was superior to GLP-1 receptor agonist (WMD:-1·18;95%CI:-1·45,-0·91) in reducing HbA1c level, while no data for the comparison on other outcomes. Interpretation: Pharmacological monotherapies have better efficacy than placebo/lifestyle intervention in Chinese T2DM patients. Funding: None. Abstract:Traditional Chinese medicine (TCM) has recorded knowledge of diabetes for over 2000 years. Because a considerable number of TCM studies exhibit design defects, such as limited intervention duration, small sample sizes and inconsistent efficacy evaluations, the role of TCM in the treatment of diabetes cannot be fully elucidated. In this review, we evaluate randomized controlled trials of prediabetes, diabetes and diabetic complications published in the past decade. We found that TCM could significantly improve glucose control and clinical indices in patients with diabetes and effectively delay the progression of diabetes. We also summarize potential pharmacological mechanisms underlying the efficacy of TCM medication/herbs and their active ingredients for treating diabetes. More rigorously designed experiments and long-term evaluation of TCM for diabetes will allow for more effective diabetes management.
Efficacy and Safety of Pharmacological Combination Therapies for the Treatment of Type 2 Diabetes in China: A Systematic Review and Meta-Analysis

Abstract:Background: Chinese guideline recommends using combination therapy of antidiabetic-drug plus metformin for type 2 diabetes (T2DM) patients failed to control glucose on metformin. Treatment effect varies between antidiabetic drugs, but there is no direct evidence on comparing the effect of each alternative drug plus metformin. This study aims to estimate and compare the efficacy and safety of seven classes of antidiabetic-drug added onto metformin for Chinese T2DM patients failed on metformin. Methods: Systematic review was performed by searching databases including CNKI, WanFang, CQVIP, PubMed, EMBASE, Web-of-Science, ScienceDirect, Cochrane-Library, to identify randomized controlled trials(1990-2016) comparing the treatment effect of 'antidiabetic-drug + metformin vs metformin' or 'metformin vs placebo/lifestyle-intervention' in Chinese T2DM patients. Based on Chinese guideline, three-line eight-class of antidiabetic-drugs were included: 1st-line-drug(metformin), 2nd-line-drug/oral-drug(α-glycosidase inhibitor, sulfonylurea, glinide, DPP-4 inhibitor, thiazolidinedione), 3rd-line-drug/injectable-drug(insulin, GLP-1 receptor agonist). Using metformin as common comparator, meta-analyses and indirect-treatment-comparisons were used to estimate the effect of 'antidiabetic-drug + metformin vs placebo/lifestyle-intervention'. Findings: 325 studies were finally included. Overall, compared with placebo/lifestyle-intervention, 'oral-drug + metformin' and 'injectable-drug + metformin' reduced HbA1c level by 1·89% and 2·28%, BMI level by 2·55kg/m2 and 3·10kg/m2, TC level by 1·03mmol/l and 1·50mmol/l, SBP level by 5·59mmHg and 8·51mmHg, while increased HDL-C level by 0·31mmol/l and 0·18mmol/l, respectively. In 'oral-drug + metformin', the top two treatments for reducing HbA1c level were 'glinide + metformin' (WMD:-2·03,95%CI[-2·70,-1·36]) and 'sulfonylurea + metformin' (WMD:-2·00,95%CI[-2·64,-1·36]); for reducing BMI level were 'glinide + metformin' (WMD:-4·25,95%CI[-5·70,-2·80]) and 'DPP-4 inhibitor + metformin' (WMD:-2·81,95%CI[-4·22,-1·40]); for reducing TC level were 'α-glycosidase inhibitor + metformin'(WMD:-1·41,95%CI[-2·15,-0·68]) and 'DPP-4 inhibitor + metformin' (WMD:-1·22,95%CI[-1·71,-0·73]); for increasing HDL-C level were 'α-glycosidase inhibitor + metformin'(WMD:0·51,95%CI[0·27,0·75]) and 'thiazolidinedione + metformin' (WMD:0·35,95%CI[0·10,0·60]). In 'injectable-drug + metformin', 'GLP-1 receptor agonist + metformin' was superior to 'insulin + metformin' in reducing HbA1c level (WMD:-2·42vs-2·04) and BMI level (WMD:-3·27vs-2·05), while inferior to 'insulin + metformin' in reducing TC level (WMD:-1·43vs-1·79). All combination therapies had no significant difference on risks of hypoglycemia or gastrointestinal-reaction vs placebo/lifestyle-intervention. Interpretation: Pharmacological combination therapies had better efficacy than placebo/lifestyle-intervention in Chinese T2DM patients. Funding: No funding was received. Abstract:Objective To investigate and evaluate the therapeutic efficacy by a randomized controlled crossover clinical research on traditional Chinese medical formulas,with herb modification according to syndrome differentiation,in the treatment of 200 type 2 diabetic patients.Methods A total of 200 diabetic patients who met the inclusion criteria,according to the order,were randomly divided into two groups,namely,the Chinese & western medicine combination group(the C&WM group) and the western medicine group(the WM group)(the WM group∶ the C & WM group= 1∶ 3).Then,patients in the C&WM group were further divided into three sub-groups,namely,the QingRunFang group(formula with nature of clearing and moisting),the ZiYiFang group(formula with nature of tonifying and nurishing) and the ShuangTiaoFang group(formula with nature of double adjusting,especially on Yin and Yang),based on the principle of "treatment based on syndrome differentiation" in traditional Chinese medicine(TCM).According to the research protocol,clinical observation was done for 12 weeks,clinical information was collected to create the database,the combination method of subjective indicators(TCM syndrome evaluation) and objective indicators was taken for the evaluation of Chinese medicine efficacy advantages and the establishment of clinical evaluation index system.Results The fasting plasma glucose,postprandial blood glucose and glycosylated hemoglobin at each time point in both groups were under better control.For the subjective indicators,symptoms in both groups were improved than those before treatment,and the C&WM group was better significantly than the WM group.Conclusion The ZiYiFang showed the best therapeutic effects in symptom improvement;the QingRunFang showed the best therapeutic effects in reduction the postprandial blood glucose;and the ShuangTiaoFang showed the best therapeutic effects in reduction in glycosylated hemoglobin.The combination of subjective and objective therapeutic efficacy evaluation method should be widely introduced.

Abstract:IntroductionThere are multiple antidiabetic drugs available in China, which vary in their efficacy and safety. However, no study exists that compares all the classes of antidiabetic drugs simultaneously. This study aimed to estimate and compare the efficacy of alternative classes of antidiabetic drugs for Chinese patients with type 2 diabetes, either in a monotherapy regimen or combined with metformin.MethodsA systematic literature review was conducted by searching various literature databases to identify relevant randomized controlled trials published from 1990 to 2016. A meta-analysis was conducted to compare the efficacy of antidiabetic drug monotherapy and placebo or lifestyle interventions (i.e., diet and exercise), and antidiabetic drug plus metformin versus metformin alone, in Chinese patients with type 2 diabetes. An indirect comparison was used to estimate the efficacy of antidiabetic drug plus metformin versus placebo or lifestyle-intervention using metformin as the common comparator.ResultsThe database search identified 354 relevant studies. Compared with placebo or lifestyle interventions, combination therapies achieved greater reductions in hemoglobin A1c (HbA1c) level (1.9% versus 0.9%), body mass index (BMI) (2.66 versus 0.98 kg/m2), and total cholesterol level (1.07 versus 0.35 mmol/L) than monotherapies. For monotherapies, the top three treatments for reducing HbA1c level were insulin, sulfonylurea, and glucagon-like peptide-1 (GLP-1) receptor agonist. The top three monotherapies for reducing BMI level were metformin, GLP-1 receptor agonist, and α-glycosidase inhibitor. The top three monotherapies for reducing total cholesterol level were metformin, GLP-1 receptor agonist, and dipeptidyl peptidase-4 (DPP-4) inhibitor. For combination therapies, the top three treatments for reducing HbA1c level were GLP-1 receptor agonist plus metformin, insulin plus metformin, and glinide plus metformin. The top three combination therapies for reducing BMI level were glinide plus metformin, GLP-1 receptor agonist plus metformin, and DPP-4 inhibitor plus metformin. The top three combination therapies for reducing total cholesterol level were insulin plus metformin, GLP-1 receptor agonist plus metformin, and α-glycosidase inhibitor plus metformin.ConclusionsPharmacological treatments had better efficacy than placebo or lifestyle interventions, while combination drug therapies were superior to monotherapies.

Abstract:Type 2 diabetes mellitus (T2DM) is a metabolic disease with persistent hyperglycemia primarily caused by insulin resistance (IR). The number of diabetic patients globally has been rising over the past decades. Although significant progress has been made in treating diabetes mellitus (DM), existing clinical drugs for diabetes can no longer fully meet patients when they face complex and huge clinical treatment needs. As a traditional and effective medical system, traditional Chinese medicine (TCM) has a unique understanding of diabetes treatment and has developed many classic and practical prescriptions targeting DM. With modern medicine and pharmacy advancements, researchers have discovered that various bioactive metabolites isolated from TCM show therapeutic on DM. Compared with existing clinical drugs, these bioactive metabolites demonstrate promising prospects for treating DM due to their excellent biocompatibility and fewer adverse reactions. Accordingly, these valuable metabolites have attracted the interest of researchers worldwide. Despite the abundance of research works and specialized-topic reviews published over the past years, there is a lack of updated and systematic reviews concerning this fast-growing field. Therefore, in this review, we summarized the bioactive metabolites derived from TCM with the potential treatment of T2DM by searching several authoritative databases such as PubMed, Web of Science, Wiley Online Library, and Springer Link. For the convenience of readers, the content is divided into four parts according to the structural characteristics of these valuable compounds (flavonoids, terpenoids, alkaloids, and others). Meanwhile, the detailed mechanism and future directions of these promising compounds curing DM are also summarized in the related sections. We hope this review inspires increasingly valuable and significant research focusing on potential bioactive metabolites from TCM to treat DM in the future.
A Retrospective Study on Type 2 Diabetes Mellitus Treated with Integrated Chinese and Western Medicine

Abstract:To compare the therapeutic efficacy and cost-efficacy of different treatment methods for treatment of type 2 diabetes mellitus (T2DM).Based on the electronic inpatient clinical information systems, clinical materials of T2DM patients were collected and assigned according to the therapeutic method used, to the groups of Western medicinal (WM) treatment (A), WM combined Chinese drug injection treatment (B) and the WM combined Chinese decoction or patent drugs treatment (C). Depending on the data of symptom scores, blood sugar and blood lipids, etc., the efficacy of different treatments were analyzed in terms of improving symptoms, elevating the quality of life (QOL), controlling blood sugar, reducing dosage of insulin used and diminishing economic expense, with the enumeration data analyzed by chi2 test and the measurement data by ANOVA.Treatment C showed a better efficacy than the other two in improving symptoms, elevating QOL, controlling blood sugar, reducing dose of insulin used and lightening the financial burden (P < 0.05).Selective use of Chinese drugs depending on syndrome differentiation is recommended during combined application of Chinese and Western medicine; and the Chinese medicine injection is not advised as the first scheme for treatment of T2DM.

Compared with placebo/lifestyle-intervention, 1st-line-drug (metformin) could change HbA1c by -1.04%, BMI by -1.77kg/m2, TC by -0.64mmol/l, SBP by -3.13mmHg, HDL-C by 0.19mmol/l, and hypoglycemia by 2%; 2nd-line-drug could change HbA1c by -1.19∼-0.25%, BMI by -0.64∼1.79kg/m2, TC by -0.3∼0.42mmol/l, SBP by -3.2∼1.84mmHg, HDL-C by -0.11∼0.22mmol/l, and hypoglycemia by -3∼2%; 3rd-line-drug could change HbA1c by -2.37∼-1.28%, BMI by -1.11kg/m2, TC by -0.67mmol/l, SBP by -3.00mmHg, HDL-C by 0.18 mmol/l, and hypoglycemia by 0. In the five 2nd-line-drugs, sulfonylurea was optimal for decreasing HbA1c, α-glycosidase-inhibitor was optimal for decreasing BMI and SBP, DPP-4-inhibitor was optimal for decreasing TC, and thiazolidinedione was optimal for increasing HDL-C and decreasing hypoglycemia. In the two 3rd-line-drugs, insulin was optimal for decreasing HbA1c, while no data supported for the comparison in BMI, TC, HDL-C, SBP and hypoglycemia. This study shows that pharmacological monotherapies have better efficacy than placebo/lifestyle-intervention in Chinese T2DM patients.
Qingre Yiqi Method along with Oral Hypoglycemic Drugs in Treating Adults with Type 2 Diabetes Mellitus: A Systematic Review and Meta-Analysis

Abstract:Objective. To evaluate the efficacy of the Qingre Yiqi method in the treatment of type 2 diabetes mellitus (T2DM) with meta-analysis. Method. The randomized controlled trials (RCTs) of the Qingre Yiqi method in the treatment of T2DM in the PubMed, Medline, EMBase, Cochrane Library, Web of Science, Weipu Journal, China Knowledge Network (CNKI), and Wanfang database were conducted. Three reviewers independently conducted the screening, extracted the data, and assessed methodological quality. Data analysis was performed using Rev Man 5.3 software for statistical analysis. Results. A total of 15 RCTs, including 1440 patients, were included. The results showed that compared with oral hypoglycemic drugs alone, the add-on treatment of the Qingre Yiqi method could significantly improve Chinese medicine syndrome (OR (95%CI) = 3.66 [2.47,5.42], P < 0.00001 ) and lower the level of HbA1c (MD (95%CI) = −0.68 [0.91, −0.45], P < 0.00001 ), triglyceride (TG) (MD (95%CI) = −0.38 [−0.58,-0.17], P = 0.0004 ), low-density lipoprotein cholesterol (LDL-C) (MD (95%CI) = −0.25 [−0.37, −0.13], P < 0.0001 ), and total cholesterol(TC) (MD(95%CI) = −0.40[−0.67, −0.13], P = 0.003 ). In terms of fasting blood glucose (FBG) and postprandial blood sugar (PBG), subgroup analysis showed that the baseline of FBG and the number of combined oral hypoglycemic drugs of PBG were the major sources of heterogeneity. Conclusion. Compared with the standard treatment, the Qingre Yiqi method along with oral hypoglycemic drugs showed the more beneficial effects for T2DM on improving TCM syndromes and reducing the blood glucose and partial lipid parameter.

Abstract:Background: Diabetes mellitus (DM) is one of the main health problems that perplex people all over the world. The prevalence of DM is still increasing in spite of the great efforts have been made to control DM in recent years. Type 2 diabetes mellitus (T2DM) is the most common type of diabetes, accounting for about 90% of all DM cases. Traditional Chinese medicine has been used on treatment of DM and diabetic complications in China for many years. Baihu Jia Renshen Decoction (BJRD) is one of the oldest classic prescriptions of traditional Chinese medicine that applied in the field of diabetes' treatment. BJRD is proved to be effective after years of clinical practice and basic research. The application of BJRD improves the overall clinical efficacy of T2DM. Therefore, a systematic review is necessary to provide available evidence for BJRD in therapy of T2DM. Methods: Different studies from various databases will be involved in this study. Only randomized controlled trials of T2DM patients diagnosed with World Health Organization 1999 diagnostic criteria will be included. We will search the literature in the databases from China Conference Paper Database, manual searching. Electronic database includes PubMed, Embase, Cochrane Library, Web of Science, CNKI (China National Knowledge Internet), WanFang, VIP (Chongqing VIP), and CBM (China Biomedical Literature CDROM Database). The primary outcomes include 2 hour plasma glucose, fasting plasma glucose, hemoglobin A1c, homeostasis model assessment of insulin resistance, and fasting plasma insulin. The secondary outcomes include clinical efficacy and adverse events. Data will be extracted by 2 researchers independently, risk of bias of the meta-analysis will be evaluated based on the Cochrane Handbook for Systematic Reviews of Interventions. All data analysis will be conducted by data statistics software Review Manager V.5.3. and Stata V.12.0. Results: This study will synthesize and provide high-quality evidence based on the data of the currently published BJRD for the treatment of T2DM, in terms of 2 hour plasma glucose, fasting plasma glucose, hemoglobin A1c, homeostasis model assessment of insulin resistance and fasting plasma insulin, clinical efficacy, and safety. Conclusion: This systematic review aims to evaluate the benefits and harms of BJRD for the treatment of T2DM reported in randomized controlled trials, and provide more options for clinicians and patients to treat T2DM. Abstract:Type 2 diabetes mellitus (T2DM) occurs in 95% of the diabetic populations. Management of T2DM is a challenge. Traditional Chinese medicines (TCM) are usually served as adjuvants used to improve diabetic syndromes in combination of routine antidiabetic drugs. For single-herb prescriptions, Ginseng, Bitter melon, Golden Thread, Fenugreek, Garlic, and Cinnamon might have antidiabetic effects in T2DM patients. Among 30 antidiabetic formulas approved by the State Food and Drugs Administrator of China, top 10 of the most frequently prescribed herbs are Membranous Milkvetch Root, Rehmannia Root, Mongolian Snakegourd Root, Ginseng, Chinese Magnoliavine Fruit, Kudzuvine Root, Dwarf Lilyturf Tuber, Common Anemarrhena Rhizome, Barbary Wolfberry Fruit, and India Bread, which mainly guided by the theory of TCM. Their action mechanisms are related to improve insulin sensitivity, stimulate insulin secretion, protect pancreatic islets, and even inhibit intake of intestinal carbohydrates. However, it is very difficult to determine antihyperglycemic components of TCM. Nevertheless, TCM are becoming popular complementary and alternative medicine in treatment of syndromes of T2DM. In the future, it requires further validation of phytochemical, pharmacological, and clinical natures of TCM in T2DM in the future studies, especially for those herbs with a high prescription frequency.
Systematic Review of Randomized Controlled Study Comparing Traditional Chinese Medicine with Western Medicine in the Treatment of Diabetes

Abstract:OBJECTIVE:To evaluate the curative efficacy of Chinese medicine vs. western medicine in the treatment of diabetes. METHODS:The randomized controlled trials(RCTs) comparing western medicine with traditional Chinese medicine alone or combination of Chinese medicine and western medicine in the treatment of diabetes were systematically evaluated. Meta-analysis of the cited trials was performed by evidence-based medicine method with Revman 4.2 software for statistical analysis. RESULTS:A total of 14 RCTs met our inclusion criteria. Meta-analysis showed that there was no significant difference between traditional Chinese medicine and western medicine in clinical efficacy in the treatment of diabetes; however,the diabetic patients treated with integrated Chinese medicine and western medicine showed much better efficacy than those treated with western medicine alone. CONCLUSION:Treatment of diabetes with traditional Chinese medicine plus western medicine has more advantages; however,this conclusion remains to be confirmed further by studies of high quality given the low quality in test methodology.
Chinese Herbal Medicine for Type 2 Diabetes Mellitus with Nonalcoholic Fatty Liver Disease: A Systematic Review and Meta-Analysis

Abstract:Objectives: To evaluate the efficacy and safety of Chinese herbal medicine (CHM) for type 2 diabetes mellitus (T2DM) with nonalcoholic fatty liver disease (NAFLD) with current evidence. Methods: This study was registered in PROSPERO as CRD42021271488. A literature search was conducted in eight electronic databases from inception to December 2021. The primary outcomes were lipid indices and liver functions, including triglyceride (TG), total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), alanine transaminase (ALT), and aspartate transaminase (AST). Review Manager 5.2 and Stata v14.0 were applied for analysis. Results: The research enrolled 18 RCTs with 1,463 participants. Results showed CHM combined with western medicine (WM) was more effective than WM alone in TG (weighted mean differences (WMD) = -0.35.95% confidence interval (CI) [-0.51, -0.19], p < 0.0001), TC (WMD = -0.58.95%CI [-0.80, -0.36], p < 0.00001), LDL-C (WMD = -0.37, 95%CI [-0.47, -0.26], p < 0.00001), HDL-C (WMD = 0.20, 95%CI [0.10, 0.29], p < 0.0001), ALT (WMD = -4.99, 95%CI [-6.64, -3.33], p < 0.00001), AST (WMD = -4.76, 95%CI [-6.35, -3.16], p < 0.00001), homeostatic model assessment of insulin resistance (WMD = -1.01, 95%CI [-1.22, -0.79], p < 0.00001), fasting blood glucose (WMD = -0.87, 95%CI [-1.13, -0.61], p < 0.00001), 2-h postprandial glucose (WMD = -1.45.95%CI [-2.00, -0.91], p < 0.00001), body mass index (WMD = -0.73.95%CI [-1.35, -0.12], p = 0.02), and overall effective rate (risk ratio (RR) = 1.37.95%CI [1.29, 1.46], p < 0.00001). Conclusion: The CHM in combination with WM seems to be more beneficial in T2DM with NAFLD patients in improving lipid and glucose metabolism, liver function, and insulin resistance as well as improving overall efficiency and reducing body weight. Given the poor quality of reports from these studies and uncertain evidence, these findings should be interpreted cautiously. Abstract:Objective: To observe the clinical efficacy of syndrome differentiation and internal Chinese medicine in the treatment of diabetes.Method:There were 54 diabetes patients according to syndrome differentiation typing as follows: qi-yin deficiency,extreme heat due to deficiency of yin,liver-kidney yin deficiency,Immanence moist heat,deficiency of both yin and yang,to link complicating and combine the theory of modern medical science,differentiation of symptoms and signs make up medical prescription Internal chinese medicine,Take a dose once a day,Result: Except 3 patients,there are 42 patients effect and 7 patients in effect,2 invain,all of developing effect are 86.9%.Conclusion: Chinese medicine combine the theory of modern medical science treatment diabetes according to syndrome differentiation is a better method.

Abstract:[Objective] By retrieval of the traditional Chinese herbs appled to treat type 2 diabetes mellitus(T2DM) in real world , with"detecting syndrome by therapeutic herbs (formulas)"method and"interpreting theory by therapeutic herbs(formulas)"philosophical idea in"herb-syndrome"theory and method, to reversely probe into TCM syndrome differentiation or classification on T2DM in real world. [Methods] Inpatient medical record information of T2DM collected by the TCM individualized diagnosis and treatment platform (clinical information collection system of traditional Chinese medicine), with the"disease-symptom-differentiation syndrome-treatment-herb-effect"model, was constructed into a data warehouse, by using scale-free network model, the taste and nature of these therapeutic herbs were classified, and from the classifications, the main treatment principles were find out, as on the one hand to provide reference for TCM berb choice in treating diabetes, on the other hand, to derive the TCM syndrome differentiations from these classifications. [Results] Medical record information of 3247 cases of T2DM inpatients were included in the study, and three network structures of characteristic distribution emerged, where 60 commonly used herbs in each network were shown. The centers of these network structures respectively were as follows:1) herbs of"eliminating heat and cooling blood, nourishing Yin and producing body fluid"in nature, such as ShengDiHuang (Radix Rehmanniae), MaiDong (Radix Ophiopogonis), XuanShen (Radix Scrophulariae), ChiShao (Radix Paeoniae Rubra), ZhiMu (Rhizoma Anemarrhenae), etc., mainly herbs of cold in nature; 2) herbs of"the supplementing qi and nourishing Yin"in nature, such as ShengDiHuang (Radix Rehmanniae), TaiZiShen (Radix Pseudostellariae), BaiZhu (Rhizoma Atractylodis Macrocephalae), FuLing (Poria cocos), BaiShao (Radix Paeoniae Alba), MaiDong(Radix Ophiopogonis), etc.;3) herbs of"invigorate the circulation of blood and Qi, expelling retained fluid and dampness, warming Yang to produce Qi"in nature. Three big network structures suggested that the syndrome types of T2DM in three stages were respectively excess heat and Yin deficiency; both Qi and Yin deficiency;and Yin and Yang deficiency, often accompanied with syndromes of blood stasis, phlegm, dampness, and retained fluid, etc. [Conclusion] In real world, the clinical essence of TCM Syndromes of T2DM are three major syndromes, i.e., excess heat and Yin deficiency, both qi and Yin deficiency, and Yin and Yang deficiency, which may be accompanied with blood stasis and phlegm and/or dampness pathogens, as can provide important clinical evidence for the "three-typed"syndrome differentiation theory system currently most extensively applied in domestic. Besides, the herbs found in this real-world based study may provide useful reference for TCM berb choice in treating diabetes.

Abstract:Both type 2 diabetes mellitus (T2DM) and nonalcoholic fatty liver (NAFLD) are metabolic diseases, which often affect each other and occur simultaneously. Insulin resistance is the common pathogenesis of both. According to the pathogenesis and clinical manifestations of T2DM with NAFLD, modern doctors classify it as "diabetes disease" combined with "accumulation" or "hypochondriac pain". It is believed that the disease is mainly related to congenital deficiency, improper diet, poor mood and excessive work and rest. Disease location involves liver, spleen, kidney three viscera, disease is mostly the deficiency of the deficiency of liver, spleen and kidney, qi stagnation, blood stasis, phlegm-dampness obstruction as the standard. Give full play to the unique advantages of the whole concept of TCM and the thought of syndrome differentiation and treatment, so as to reduce symptoms, reduce recurrence and regulate the metabolism of the body. In recent years, new progress has been made in the study of T2DM complicated with NAFLD by traditional Chinese medicine. Now, the etiology and pathogenesis of T2DM complicated with NAFLD are systematically reviewed, which provides new ideas for the clinical diagnosis and treatment of T2DM complicated with NAFLD.
The Effects of Tai Chi Exercise for Patients with Type 2 Diabetes Mellitus: An Overview of Systematic Reviews and Meta-Analyses

Abstract:Objectives. Tai chi (TC) is a potential complementary treatment for type 2 diabetes mellitus (T2DM). This overview systematically summarizes and evaluates the existing evidence of TC in the treatment of T2DM. Methods. Systematic reviews (SRs)/meta-analyses (MAs) on TC interventions for T2DM were comprehensively searched in seven databases. Methodological quality, risk of bias, reporting quality, and quality of evidence were assessed using the Assessment of Multiple Systematic Reviews 2 (AMSTAR-2), the Risk of Bias in Systematic (ROBIS) scale, the list of Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA), and the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) system. Results. Eight published SRs/MAs were included in our study. Based on the methodology and quality of evidence assessment, all SRs/MAs are considered to be of very low quality, and only 1 SR/MA has been assessed as low risk of bias, and none of the SR/MA has been fully reported on the checklist. A total of 65 outcome indicators extracted from the included SRs/MAs were evaluated, and only 1 item was assessed as high quality. Conclusions. TC may be an effective and safe complementary treatment for T2DM. However, this conclusion must be treated with caution because the quality of the evidence provided by the included SRs/MAs is generally low.
