Abstract:The purpose of this study was to investigate the differences in bone mineral density (BMD) and hip structure between femoral neck and trochanteric fractures in elderly Chinese individuals using quantitative computed tomography (QCT). A total of 625 Chinese patients (mean age 75.8 years) who sustained low-energy hip fractures (female: 293 femoral neck, 175 trochanteric; male: 82 femoral neck, 75 trochanteric) were recruited. Each patient underwent a hip QCT scan. The areal BMD (aBMD) of the contralateral normal hip was obtained using a computed tomography X-ray absorptiometry module. Using the bone investigation toolkit (BIT) module, the femoral neck was divided into four quadrants: supero-anterior (SA), infero-anterior (IA), infero-posterior (IP), and supero-posterior (SP). Estimated cortical thickness, cortical BMD, and trabecular BMD were measured in each quadrant. Using the hip structure analysis (HSA) function, several parameters were calculated. Stratified by sex, covariance analyses were applied to compare the femoral neck fractures group with trochanteric fractures group after adjustments for age, height, and weight. In women, trochanteric fractures exhibited lower trabecular BMD and estimated cortical thickness at three quadrants of the femoral neck (IA: P = 0.02, P < 0.01; IP: P < 0.01, P = 0.01; SP: P = 0.01, P < 0.01), and lower aBMD at the trochanter area (P < 0.01); femoral neck fractures exhibited lower cortical BMD and estimated cortical thickness at the SA quadrant (P = 0.04, P = 0.01). Differences in HSA parameters were not statistically significant. Among all parameters, the most valuable ones to discrimination of hip fracture type are estimated cortical thickness of the SA quadrant of femoral neck and the aBMD of the trochanter area. In men, only lower cortical BMD at the SP quadrant and aBMD at the trochanter were found in the trochanteric fractures (P = 0.02, P < 0.01). QCT outcomes indicate that spatial differences are helpful to explore the pathogenesis of different type of hip fractures. In women, trochanteric fractures are related to severer osteoporosis, whereas cortical fragility in the SA region of the femoral neck predominates in cases of femoral neck fractures. In men, trochanteric fractures are related to more bone loss of trochanter.

Abstract:BACKGROUND: Few studies have investigated the differences in proximal femoral geometry and risk factors between patients with different types of hip fracture, especially in elderly Chinese. This study aimed to assess the differences in proximal femoral geometry parameters between patients with femoral neck fractures and patients with intertrochanteric fractures to provide guidance for individualized customized prosthesis and accurate reconstruction of proximal femurs in elderly Chinese patients.METHODS: We retrospectively studied the electronic medical records of 198 elderly patients over 65 years of age who were admitted to the orthopedic department with hip fractures between January 2017 and December 2017 in The Third Hospital, Hebei Medical University. Age, fracture site, gender, and proximal femoral geometry parameters (neck shaft angle [NSA], center edge angle [CEA], femoral head diameter [FHD], femoral neck diameter [FND], femoral neck axial length [FNAL], hip axial length [HAL], and femoral shaft diameter [FSD]) were recorded. Student's t-test was used to compare the continuous variables, Chi-square test was used to analyze categorical variables, and multiple logistic stepwise regression analysis was used to evaluate the influencing factors of hip fracture type.RESULTS: Statistically significant differences in NSA (137.63 ± 4.56° vs. 132.07 ± 4.17°, t = 1.598, P < 0.001), CEA (37.62 ± 6.77° vs. 43.11 ± 7.09°, t = 5.597, P < 0.001), FND (35.21 ± 3.25 mm vs. 34.09 ± 3.82 mm, t = 2.233, P = 0.027), and FNAL (99.30 ± 7.91 mm vs. 103.58 ± 8.39 mm, t = 3.715, P < 0.001) were found between the femoral neck fracture group and femoral intertrochanteric fracture group. FHD, FND, FSD, HAL, and FNAL were different between sexes (all P < 0.001). The greater NSA was the risk factor for femoral neck fractures (odds ratio [OR]: 0.70, P < 0.001), greater CEA and longer FNAL were risk factors for femoral intertrochanteric fractures (OR: 1.15, 1.17, all P < 0.001), and greater FND was a protective factor for femoral intertrochanteric fractures (OR: 0.74, P < 0.001).CONCLUSIONS: We demonstrate differences in geometric morphological parameters of the proximal femur in different hip fracture types, as well as an effect of sex. These differences should be considered in the selection of prostheses for fracture internal fixation and hip replacements. These data could help guide the design of individualized customized prostheses and improve the accurate reconstruction of the proximal femur for elderly Chinese hip fracture patients.

Abstract:Hip dual-energy X-ray absorptiometry (DXA) images were used to calculate hip bone mineral density (BMD) and hip geometry parameters of 18,502 healthy Chinese people (14,435 women and 4,067 men), 254 subjects sustained a femoral neck fracture and 254 age- and sex-matched controls. Our study showed that thinning of the cortical shell and deterioration of the strength index (SI) in femoral neck with aging in both Chinese men and women. SI may be a risk factor for hip fracture in Chinese women.

Abstract:Objective To compare the differences of proximal femoral bone mineral density (BMD) and hip structure parameters between rural and urban elderly people in Beijing.Methods The clinical data of 174 Beijing residents receiving quantitative computed tomography (QCT) of hip from Dec 1 st,2012 to Dec 1 st,2013 was retrospectively analyzed.The participants were divided into the urban group (46 men and 72 women,50-70 years) and the rural group (31 men and 25 women,50-70 years) according to the residence.The differences of femoral neck volumetric bone mineral density (FN vBMD),total hip volumetric bone mineral density (TH vBMD),femoral neck areal bone mineral density (FN aBMD),trochanter areal bone mineral density (TR aBMD),intertrochanter areal bone mineral density (IT aBMD),total hip areal bone mineral density (TH aBMD) and parameters of hip strength analysis (HSA) including neck shaft angle (NSA),femur neck width (FNW),hip axis length (HAL),cross-sectional moment of inertia (CSMI),cross-sectional area (CSA),buckling ratio (BR),averaged cortical thickness (ACT) and section modulus (SM) measured by QCT software between the two groups were statistically analyzed.Results There was no difference on height between rural and urban subjects in both sexes groups,whereas urban elderly males (73.66-± 12.63 kg) were heavier than rural males (67.82±9.58 kg) (P＜0.05).No significant differences were found between rural and urban men in all parameters.Interestingly,HAL of rural women was significantly higher than that of urban subjects (median,109.41 millimeter vs 104.23 millimeter) (P＜0.05),whereas no significant differences were found between rural and urban women in the other parameters.Conclusion HAL of rural elderly women was significantly higher than that of urban women,whereas no significant differences were found in the other QCT BMD and HSA parameters between rural and urban elderly people in Beijing.

Abstract:Structural skeletal differences of the femoral neck of older Beijing-Chinese and Perth-Caucasian women were compared; adjusting for frame size-related differences, Beijing-Chinese have lower periosteal width; however, indices of internal bone distribution suggest that Beijing-Chinese may exhibit increased resistance to fracture that may relate to the reduced hip fracture incidence.

Abstract:Previous studies have suggested that changes in hip geometry increase the risk of hip fracture. The aim of this study was to identify whether body composition were associated with hip geometry or bone mineral density (BMD) in a large sample of Chinese people. A total of 2072 subjects aged 20-79 yr (including 700 males and 1372 females) were selected. The following measurements were taken: lumbar spine (L1-4); proximal femur BMD; lean mass (LM); fat mass (FM); and hip geometric parameters, including hip axis length (HAL), cross-sectional moment of inertia (CSMI), cross-sectional area (CSA), neck-shaft angle, and femur strength index (SI) by dual-energy X-ray absorptiometry. FM and LM were positively correlated with HAL, CSMI, and CSA, and negatively correlated with SI in both men and women. Multiple regression analysis showed that leg LM contributions to HAL, CSMI, and CSA variance were 12.6-37.6%. Compared with FM, LM was generally more strongly related to hip geometry and BMD in young and old men and women. Body composition was a good predictor for hip geometry parameter variation and BMD variation.

Abstract:Objective To evaluate the bone mineral density ( BMD ) and structure of proximal femur in elderly Chinese women by quantatitive computed tomography ( QCT ) and dual energy X-ray absorptiometry ( DXA), and to further compare the results of these two methods.Methods Sixty-six healthy Chinese women over 65 years old participated in this study.The left hips of all subjects were measured with DXA and the BMD of femoral neck and trochanteric region were calculated.With QCT, the BMD and tissue volume of cortical, trabecular and integral bone were calculated for femoral neck, trochanteric and total femur regions in both hips. Appropriate statistical analyses were performed with SPSS 11.5.Results The BMD and structural parameters in different regions and different compartments of the proximal femur could be precisely assessed with QCT technique.The BMD of cortical bone in femoral neck [ ( 0.52 ±0.04 ) g/cm3 ] , BMD of cortical bone in trochanteric region [ ( 0.49 ±0.03 ) g/cm3 ] and BMD of integral bone in trochanteric region[(0.22 ±0.04)g/cm3] were greater in the right than those in the left [(0.51 ±0.04),(0.48 ±0.03),(0.21 ±0.04)g/cm3].The difference had statistical signification ( P<0.05).However, the difference was less than 3.3%.In the left the parameters of analogue DXA BMD of femoral neck, analogue DXA BMD of trochanteric region, cortical bone volume of femoral neck, BMD of trabecular bone in femoral neck, trabecular bone volume of femoral neck, BMD of integral bone in femoral neck, integral bone volume of femoral neck, cortical bone volume of trochanteric region, BMD of trabecular bone in trochanteric region, trabecular bone volume of trochanteric region and integral bone volume of trochanteric region were (0.52 ±0.10) g/cm2,(0.78 ±0.13) g/cm2,5.80 cm3,(0.06 ±0.03) g/cm3, (5.19 ±1.40 ) cm3 , ( 0.25 ±0.04 ) g/cm3 , 15.66 cm3 , ( 21.74 ±3.43 ) cm3 , ( 0.08 ±0.03 ) g/cm3 , (34.27 ±6.09)cm3 and (76.12 ±11.11)cm3 respectively, in the right the corresponding parameters being (0.52 ±0.10 ) g/cm2 , ( 0.78 ±0.13 ) g/cm2 , 6.01 cm3 , ( 0.06 ±0.02 ) g/cm3 , ( 5.17 ±1.27 ) cm3 , (0.25 ±0.04)g/cm3,15.62 cm3,(22.12 ±3.60) cm3,(0.09 ±0.03) g/cm3,(34.17 ±5.94) cm3 and (76.53 ±10.71)cm3 respectively.There were no significant difference between the left and right parameters above (P>0.05).All QCT parameters of the right hip correlated well with their corresponding ones of left hip with correlation coefficients ranging from 0.656—0.955, P <0.05.QCT-derived simulated DXA femoral neck and trochanteric BMD were correlated well with real DXA measurements ( r 0.685 and 0.855, P<0.05).Conclusion With QCT technique the BMD and structural parameters in different regions and different compartments of the proximal femur can be precisely assessed.QCT is a powerful technique in the researches of osteoporosis.
Sex- and Age-Related Differences in Femoral Neck Cross-Sectional Structural Changes in Mainland Chinese Men and Women Measured Using Dual-Energy X-ray Absorptiometry

Abstract:We investigated age-related changes in estimated bone strength and cross-sectional structure of the femoral neck (FN) in mainland Chinese men and women (according to age and sex) using dual-energy X-ray absorptiometry (DXA). A total of 3855 healthy adults (2713 women, 1142 men; ages 25–91years) were analyzed by FN bone mineral density (BMD) assessment and hip structural/strength analysis (HSA), including cross-sectional moment of inertia (CSMI), cross-sectional area (CSA), section modulus (Z), periosteal diameter (PD), endocortical diameter (ED), and cortical thickness (CT) using DXA. HSA differences between age and sex groups were adjusted for body weight, height and FN BMD. Trends according to age were estimated by linear regression analysis. There was no inverse correlation between HSA parameters and age in young adults. Some HSA parameters (CSMI, CSA, Z, CT) decreased significantly with age, whereas PD and ED increased significantly. Older adults had less estimated bone strength and CT and higher PD and ED (p<0.05) than young adults. Men had greater increases in PD and ED than women across all ages. FN strength decreases with age in both sexes, caused by FN cross-sectional structural deterioration. Indirect comparison of our data with those from other populations showed less age-related FN periosteal apposition in Chinese than Caucasian men, but similar amounts in women. This may partly explain different male/female hip fracture rates among ethnic groups. Chinese men have more structural disadvantages regarding FN geometry during aging than Caucasian men, possibly conferring added susceptibility to hip fracture.
[Bone Mineral Density Characteristics at the Femoral Neck and Ward's Triangle for Chinese Women].

Abstract:The incidence of hip fracture is not as high in Chinese women compared to women from Western countries, though they usually have low bone mineral density(BMD) and get osteopenia easily. In this study, reference data(white women) supplied by the manufacturer of Hologic was compared with data obtained from healthy women in Changsha, Hunan, P.R.C. One thousand four hundred and eighty-eight Chinese women aged 15 to 95 years were randomly recruited. Measurements of BMD were taken at the hip by the dual energy x-ray absorptiometry(QDR 4,500A, Hologic Inc., USA). The BMD was somewhat lower than reference curves at all ages and all sites. But at the femoral neck and Ward's triangle, Chinese women reached their peak BMD 5 to 10 years later than the reference group, and had a lower BMD rate of decrease for about 35 years after peak BMD was reached. Whether the differences(i.e., longer time to peak BMD and a lower BMD decrease rate at the neck and Ward's triangle after the peak BMD reached) will result in a protection from hip fractures for Chinese women needs to be studied in the future.
Variations in bone microarchitecture in the osteoporotic hip fracture area

Abstract:Objective To evaluate the changes of trabeculae microstructure in the tensional side of the hip fracture area.Methods Sixteen elderly patients with low-energy femoral neck fractures and undergone arthroplasty were selected in the experimental group.Bone mineral density of the healthy side of the hip was examined before the operation.Five femurs in control group were obtained from 5 young cadavers due to difficult sample collection.Trabecular specimens of 5 mm ×5 mm ×10 mm were obtained from the tensional side of femoral neck trabeculae in both experiment and control group.The difference of microstructure between the 2 groups and the correlation between microstructure parameters and BMD of patients in experiment group were analyzed. Results Microstructure in the experimental gourp:Bone volume/tissue volume ( BV/TV) 0.0645 ±0.0259, trabecular number (Tb.N) (0.8078 ±0.2212) mm-1, trabecular thickness (Tb.Th) (0.0836 ±0.0212) mm, trabecular spacing (Tb.Sp) (1.2197 ±0.4492) mm, connect density (Conn.D) 1.8577 ±1.0217, structure mode index (SMI) 1.778 ±0.5168.Microstructure in the control group :BV/TV 0.1470 ±0.0633,Tb.N (1.2692 ±0.1376) mm-1,Tb.Th (0.1201 ±0.0414) mm,Tb.Sp (0.6810 ± 0.1129) mm,Conn.D 3.3585 ±1.7851,SMI 0.8781 ±0.6665.Comparing to control group, the trabecular of the tensional side in experiment group was obviously destroyed.All the microstructure parameters were altered obviously, and the changes were statistically significant (P<0.05).No correlation was found between microstructure parameters and BMD in experiment group (P>0.05).Conclusion The microstructure of the trabecular in the tensional side of the hip is deteriorated in patients with osteoporosis.This may be one of the important causes of the hip fracture.
Sex-related variations in cortical and trabecular bone of the femoral neck in an elderly Chinese population

Abstract:Summary Quantitative computed tomography (QCT) was used to investigate sex-related variations in cortical and trabecular bone of the femoral neck. Cortical bone thickness of women in the superior quadrant was thinner than that of men, and the cortex in all four quadrants was negatively associated with age in women. Introduction This cross-sectional study aimed to investigate sex-related similarities and differences in femoral neck structure in an elderly Chinese population by QCT bone investigational toolkit (BIT) analysis. Methods This study included 207 male (67.9 ± 7.7 years; range, 55–87 years) and 400 female subjects (68.0 ± 8.7 years; range, 55–96 years). BIT module was used to measure cortical and trabecular bone in anatomic quadrants of the femoral neck. Measurements of cortical thickness (Ct.Th), cortical vBMD (Ct.vBMD), trabecular vBMD (Tb.vBMD), and integral vBMD (It.vBMD) at the femoral neck were determined in four anatomical sectors. Results The greatest difference between sexes, after adjusting for age, height, and weight, was in Ct.Th of Quadrant Supero-anterior (SA), which was 27.4% lower in women (p<0.001). Ct.Th of Quadrant Supero-posterior (SP) was 15.1% lower in women ( p = 0.027). Ct.Th and Tb.vBMD in all four quadrants appeared to be negatively associated with age in females, whereas no significant relationship was observed in males, except Ct.Th of Quadrant SP. Conclusions The superior femoral neck geometry between males and females was significantly different, even after adjustment for body size and age, and the sub-regional cortical and trabecular bone negatively age-related changes in women indicated that women apparently have a more vulnerable geometrical outcome with age for fractures than men.
Spatial Differences in the Distribution of Bone Between Femoral Neck and Trochanteric Fractures

Abstract:ABSTRACT There is little knowledge about the spatial distribution differences in volumetric bone mineral density and cortical bone structure at the proximal femur between femoral neck fractures and trochanteric fractures. In this case-control study, a total of 93 women with fragility hip fractures, 72 with femoral neck fractures (mean ± SD age: 70.6 ± 12.7 years) and 21 with trochanteric fractures (75.6 ± 9.3 years), and 50 control subjects (63.7 ± 7.0 years) were included for the comparisons. Differences in the spatial distributions of volumetric bone mineral density, cortical bone thickness, cortical volumetric bone mineral density, and volumetric bone mineral density in a layer adjacent to the endosteal surface were investigated using voxel-based morphometry (VBM) and surface-based statistical parametric mapping (SPM). We compared these spatial distributions between controls and both types of fracture, and between the two types of fracture. Using VBM, we found spatially heterogeneous volumetric bone mineral density differences between control subjects and subjects with hip fracture that varied by fracture type. Interestingly, femoral neck fracture subjects, but not subjects with trochanteric fracture, showed significantly lower volumetric bone mineral density in the superior aspect of the femoral neck compared with controls. Using surface-based SPM, we found that compared with controls, both fracture types showed thinner cortices in regions in agreement with the type of fracture. Most outcomes of cortical and endocortical volumetric bone mineral density comparisons were consistent with VBM results. Our results suggest: 1) that the spatial distribution of trabecular volumetric bone mineral density might play a significant role in hip fracture; 2) that focal cortical bone thinning might be more relevant in femoral neck fractures; and 3) that areas of reduced cortical and endocortical volumetric bone mineral density might be more relevant for trochanteric fractures in Chinese women. © 2017 American Society for Bone and Mineral Research.

ANALYSIS OF CORRELATION BETWEEN TRABECULAR MICROSTRUCTURE AND CLINICAL IMAGING PARAMETERS IN FRACTURE REGION OF OSTEOPOROTIC HIP

Abstract:OBJECTIVE:To analyze the correlation between the trabecular microstructure and the clinical imaging parameters in the fracture region of osteoporotic hip so as to provide a simple method to evaluate the trabecular microstructure by a non-invasive way. METHODS:Between June 2012 and January 2013, 16 elderly patients with femoral neck fracture underwent hip arthroplasty were selected as the trial group; 5 young patients with pelvic fracture were selected as the control group. The hip CT examination was done, and cancellous bone volume/marrow cavity volume (CV/MV) was analyzed with Mimics 10.01 software in the control group. The CT scan and bone mineral density (BMD) measurement were performed on normal hips of the trial group, and cuboid specimens were gained from the femoral necks at the place of the tensional trabeculae to evaluate the trabecular microstructure parameters by Micro-CT, including bone volume fraction (BV/TV), trabecular number (Tb.N), trabecular spacing (Tb.Sp), trabecular thickness (Tb.Th), connect density (Conn.D), and structure model index (SMI). The correlation between imaging parameters and microstructure parameters was analyzed. RESULTS:In the trial group, the BMD value was 0.491-0.698 g/cm2 (mean, 0.601 g/cm2); according to World Health Organization (WHO) standard, 10 cases were diagnosed as having osteoporosis, and 6 cases as having osteopenia. The CV/MV of the trial group (0.670 1 +/- 0.102 0) was significantly lower than that of the control group (0.885 0 +/- 0.089 1) (t = -4.567, P = 0.000). In the trial group, CV/MV had correlation with BV/TV, Tb.Th, and SMI (P < 0.05); however, CV/MV had no correlation with Tb.N, Tb.Sp, or Conn.D (P > 0.05). BV/TV had correlation with Tb.Th, Tb.N, Tb.Sp, and SMI (P < 0.05), but it had no correlation with Conn.D (P=0.075). There was no correlation between BMD and microstructure parameters (P > 0.05). CONCLUSION:CV/MV obviously decreases in the osteoporotic hip, and there is a correlation between CV/MV and the microstructure parameters of BV/TV, Tb.Th, and SMI, to some extent, which can reflect the variety of the microstructure of the trabeculae. There is no correlation between BMD of femoral neck and microstructure parameters.
Morphological characteristics of the proximal femur in elderly patients with hip fractures: a case-control study

Abstract:Introduction: Owing to the diverse design, measurement methods and ethnic differences, the influence of the proximal femur geometry on hip fractures is still unclear. Therefore, this study aimed to investigate morphological characteristics of the proximal femur in senile patients with hip fractures on three-dimensional images.Material and methods: One hundred and sixteen women and 38 men with hip fractures were included in the fracture group. The control group included 74 women and 63 men. The geometrical parameters of the proximal femur were measured after three-dimensional reconstruction. The femoral neck width (FNW), femoral neck length (FNL), femoral head height (FHH), femoral head diameter (FHD), neck shaft angle (NSA) and offset were measured and statistically analyzed.Results: The NSA in fracture cases was significantly larger than controls in both men and women (130.18 vs. 126.93, p = 0.001; 131.07 vs. 128.68, p < 0.001, respectively). Moreover, a lower total hip bone mineral density (BMD) in fracture cases was found in both sexes (0.725 vs. 0.812, p = 0.001; 0.743 vs. 0.830, p < 0.001, respectively). In multiple logistic regression analysis, a larger NSA and a lower total hip BMD were independent predictors for hip fractures in both men and women (OR = 1.143 and 1.171, p = 0.010 and 0.016, respectively). However, the FNL was an independent predictor for hip fractures only for women (OR = 1.201, 95% CI: 1.106–1.305, p < 0.001).Conclusions: A larger NSA and a lower BMD were independent predictors for hip fractures of senile patients in both sexes. Moreover, a longer FNL was an independent risk factor for patients with hip fractures in women. As a result, we hypothesized that the geometrical measurement of the proximal femur on three-dimensional images might be appropriate.

Abstract:Abstract Background Fragility fracture is associated with bone mineral density (BMD), and most databases used in related researches are instrument-matched. Little is known about the relationship between BMD and fragility fracture risk of native Chinese, especially using local databases as reference databases. Objective To investigate relationship between BMD and risk of fragility fracture in native China. Methods 3,324 cases, including 2,423 women (67.7 ± 8.9 years) and 901 men (68.4 ± 11.6 years) having radiological fragility fractures and 3,324 age- and gender-matched controls participated in the study. We measured BMD at posteroanterior spine and hip using dual-energy X-ray absorptiometry (DXA), calculated BMD measurement parameters based on our own BMD reference database. Results BMDs and mean T-scores were lower in case group (with clinical fragility) than in control group (without clinical fragility). In patients with fragility fractures, prevalence of lumbar osteoporosis, low bone mass, and normal BMD were 78.9 %, 19.3 %, and 1.8 %, respectively, in women, and 49.5, 44.8 %, and 5.7 %, respectively, in men. In hip, these prevalence rates were 67.2 %, 28.4 %, and 4.4 % in females, and 43.2 %, 45.9 %, and 10.9 % in males, respectively, showing differences between females and males. Multivariate Cox regression analysis showed that after adjusting age, height, weight, and body mass index, fracture hazard ratio (HR) increased by 2.7–2.8 times (95 % CI 2.5–3.1) and 3.6–4.1 times (95 %CI 3.0–5.1) for women and men respectively with decreasing BMD parameters. In both sexes, risk of fragility fracture increased approximately 1.6–1.7 times (95 % CI 1.5–1.8) for every 1 T-score reduction in BMD. Conclusions Risk of clinical fragility fracture increases with decreasing BMD measurement parameters and anthropometric indicators in native China, and fracture HR varies from gender and site.
Gender Differences in a Reference Database of Age-Related Femoral Neck Geometric Parameters for Chinese Population and Their Association with Femoral Neck Fractures

Abstract:Femoral neck geometric parameters (FNGPs) are closely related to the strength of the femoral neck and the risk of fragility fractures. No reference database is available for FNGPs for Chinese population, and gender-related differences in FNGPs as well as their association with the risk of femoral neck fractures are unknown. This investigation aimed to set up reference databases for FNGPs, understand gender-related differences in FNGPs, and examine the association between FNGPs and the risk of osteoporotic fractures of the femoral neck. This study included 5268 females and 2156 males (aged 15–91years) from Chinese population. A total of 384 patients (282 females and 102 males) had sustained femoral neck fractures; 384 age- and sex-matched individuals without any fractures served as controls. Femoral neck DXA images were used to measure bone mineral density (BMD) and eight FNGPs. Our results showed that the age-related trends of FNGPs were fitted with the best goodness-of-fit by applying the cubic regression model. The trends shown by FNGPs were significantly different between male and female subjects, and the fitting curves were significantly higher in male subjects. After adjustments were made for age, height, weight, and body mass index, Cox regression analysis showed that changes in all FNGPs were related to increased hazard ratios (HRs) of femoral neck fractures. After further adjustment was made for BMD of the femoral neck, the HRs related to a cortical thickness (CT) decrease and buckling ratio (BR) increase in females went up by 3.35-folds (95% CI: 2.75–4.07) and 1.86-folds (95% CI: 1.33–2.60), respectively. In males, the HRs related to the decrease in CT and cross-sectional area (CSA) increased by 3.21-folds (95% CI: 2.32–4.45) and 1.88-folds (95% CI: 1.03–3.44), respectively. In conclusions, the reference databases of FNGPs established in this study will assist in the evaluation and prediction of femoral neck fracture risk in the clinic. The decrease in CT and increase in BR of the femoral neck were independent risk factors for osteoporotic fractures of the femoral neck in females from mainland China, while a decrease in CT and CSA were risk factors in male.

Abstract:Objective To investigate the correlation between hip geometric structure and body composition in-cluding bone mineral density (BMD), lean mass (LM) and fat mass (FM) in health women in Shanghai.Methods A total of 679 healthy women in Shanghai which were divided into three groups: young (2140y), middle-aged (41y-menopause ) , and postmenopausal .Women were evaluated by assessing BMD of lumber spine and total hip , total lean mass, total fat mass, and hip geometric parameters such as cross-sectional area (CSA), average cortical thickness (ACT), and bucking ratio (BR) at narrow neck (NN), intertrochanteric (IT), and shaft (FS).Correlation analysis was performed by Pearson ( normal distribution ) or Spearman ( abnormal distribution ) test, and attribution analysis was performed by stepwise test of multiple regressions .Results The peak BMD values of the spine and total hip were ob-served at age 31 to 40 years.The peak LM and FM were observed at age 31 to 40 years.The hip geometric parameters in-cluding CSA and ACT at NN , IT and FS both reached their peaks at 31 to 40 years, and valley BR at three regions above mentioned was observed at 31 to 40 years as well .The correlation between hip BMD and hip geometric structure were the strongest at all three regions .LM showed significantly positive correlations with CSA and negative correlation with BR .FM effect on hip geometric parameters at the three regions were inconsistent .The positive contribution produced by FM to hip geometric structure became stronger gradually with aging .The contribution produced by trunk LM was the largest positive contribution to CSA and ACT in all group .Conclusion Among all body composition parameters , hip BMD showed the best correlation with hip geometric structure .LM produced the second greatest contribution .The impact of FM and LM on hip geometry changed with aging and different distributions .

Abstract:Purpose: This study evaluated the prevalence of vertebral fractures (VF) in middle-aged and elderly Chinese men and women and explored the differences in lumbar spine volumetric bone mineral density (vBMD) derived from quantitative CT (QCT) between those with a grade 1 vertebral fracture and non-fractured individuals. Materials and methods: 3,457 participants were enrolled in the China Action on Spine and Hip Status (CASH) study and had upper abdominal CT examinations. Vertebral fractures were identified by Genant's semi-quantitative method from lateral CT scout views or CT sagittal views. L1-3 vBMD was measured by Mindways QCT Pro v5.0 software. The characteristics of different fracture severity groups were compared using one-way ANOVA, independent-samples t-tests, and Kruskal-Wallis H-tests. Results: 1267 males (aged 62.77 ± 9.20 years) and 2170 females (aged 61.41 ± 9.01 years) were included in the analysis. In men, the prevalence of VF increased from 14.7% at age<50 years to 23.2% at age ≥70 years, and in women from 5.1% at age<50 years to 33.0% at age ≥70 years. Differences in mean age and vBMD were found between the different fracture grade groups. After age stratification, vBMD differences in men aged < 50 years old disappeared (p = 0.162) but remained in the older age bands. There was no significant difference in mean vBMD between those with multiple mild fractures and those with a single mild fracture. Conclusion: In women, the prevalence of VF increased rapidly after age 50, while it grew more slowly in men. In general, with the exception of men <50 years old, participants with a grade 1 VF had lower vBMD than non-fractured individuals. The majority of women younger than 50 with a grade 1 VF had normal bone mass. We recommend that a vertebral height reduction ratio of <25% be diagnosed as a deformity rather than a fracture in people under the age of 50. The presence of multiple mild fractured vertebrae does not imply lower BMD.

Abstract:To enhance our understanding of the relationship between bone mineral density (BMD) and sex, age, body mass index (BMI), weight, and height in elderly Chinese, we studied 258 males aged 50–80 years (mean ± SD, 62.9 ± 6.2 years) and 193 females aged 46–75 years (59.0 ± 6.2 years). We measured BMD at the lumbar spine (L1–L4), hip (femoral neck, trochanter, and intertrochanter), and Ward’s triangle. A significant difference of age-adjusted BMD among male-female groups (P ≪ 0.0001) was observed. After adjustment for weight, the magnitude of the sex difference in BMD was reduced at all studied skeletal sites; for example, the difference declined from 18.3% to 5.5% at the spine. There were significant differences in BMD among age-stratified groups at all the sites in both sexes (P ≪ 0.01), except for spine BMD in males (P = 0.928). Regression analysis suggested that, with aging, greater differences of BMD distribution exist in elderly females than in males. Weight accounted for the greatest proportion of age-adjusted BMD variation (e.g., at femoral neck, R2 = 0.17 in males) among four variables: weight, height, BMI, and a principal component formed from weight and height. These results suggested that weight decreased the sex difference in BMD in elderly Chinese. Patterns of age-related BMD distribution and BMD change among different age groups differed between the sexes and between the studied sites. Weight accounted for most of the effect of two correlated variables (weight and height) on BMD in our sample.
