Abstract:Timely access to diabetes specialty care is often limited by resources, especially within a safety-net health system serving a large under/un-insured and ethnically diverse patient population. Implementation of a diabetes electronic consultation (e-consult) service offers several potential benefits, including timely access to diabetes specialists’ input, improvement in patients’ glycemic control, decreased need for in-person referral or visit (a potential benefit to patients and providers) , and an opportunity to support primary care providers with targeted education. We performed a retrospective electronic medical record review of completed diabetes e-consults within a 3-year period (2018-2021) at a large safety-net health system in Dallas, TX to determine the effectiveness of e-consult service. A total of 277 e-consult encounters were completed for 266 patients (age 52.8±12 years, 65% female, 46% Hispanic, 56% Caucasian, 39% Black, 91% with type 2 diabetes) . The average specialist response time following an e-consult request was 2 days, with recommendations fully or partially implemented for 78% of the e-consults. For the 145 patients with reliable hemoglobin A1c (HgbA1c) data at baseline and within 6 months, the HgbA1c improved from an average of 10.1±2.1% to 9.3±1.8% (paired t-test, p=<0.05) , with an average HgbA1c change of -0.7±1.8%. Only 51% of these patients were subsequently referred for in-person diabetes clinic visit. Moreover, review of the e-consults questions helped identify common knowledge gaps in diabetes care among the primary care providers, and led to the development of monthly educational webinar series for primary care providers. In conclusion, implementation of diabetes e-consult service improved timely access to diabetes specialty care as well as patients’ glycemic control, and also created an educational opportunity to improve overall diabetes care within a large safety-net health system. Abstract:More than 5 million people in the UK are living with diabetes, of whom 90% have type 2 diabetes mellitus (T2DM).1 Primary care teams play a key role in the diagnosis and management of this condition. How people present, options for lifestyle interventions, therapeutic choices, and the possibility of remission, have substantially changed how primary care teams should be discussing a new diagnosis of T2DM with patients. As members of the Diabetes UK Research Steering group, we are concerned that patients are not receiving the care required at the point of diagnosis; a key moment in the patient's journey and life-long relationship with the health service. This editorial explores the current context of T2DM care and why conversations at the point of diagnosis are crucial for both patients and clinicians. The Health Foundation estimates that health inequalities related to diabetes are widening at an alarming rate, with those in the most deprived areas developing T2DM at a younger age.2 This rise in young adults with T2DM is associated with obesity, deprivation, and diverse ethnic groups.3 Research has shown that this group develop a more aggressive nature of T2DM with subsequent greater risk of diabetes related complications, such as retinopathy, peripheral neuropathy, and renal disease.3 Over 6% of the UK Health budget is spent on diabetes. Impact of diabetes education teams in primary care on processes of care indicators

Abstract:Aims: To evaluate the impact of the integration of onsite diabetes education teams in primary care on processes of care indicators according to practice guidelines. Methods: Teams of nurse and dietitian educators delivered individualized self-management education counseling in 11 Ontario primary care sites. Of the 771 adult patients with HbA1c ≥7% who were recruited in a prospective cohort study, 487 patients attended appointments with the education teams, while the remaining 284 patients did not (usual care group). Baseline demographic, clinical information, and patient care processes (diabetes medical visit, HbA1c test, lipid profile, estimated glomerular filtration rate, and albumin-to-creatinine ratio, measuring blood pressure, performing foot exams, provision of flu vaccine, and referral for dilated retinal exam) were collected from patient charts one year before (pre period) and after (post period) the integration began. A multi-level random effects model was used to analyze the effect of group and period on whether the process indicators were met based on practice guidelines. Results: Compared to the usual care group, patients seen by the education teams had significant improvements on indicators for semi-annual medical visit and annual foot exam. No significant improvements were found for other process of care indicators. Conclusions: Onsite education teams in primary care settings can potentially improve diabetes management as shown in two process of care indicators: medical visits and foot exams. The results support the benefits of having education teams in primary care settings to increase adherence to practice guidelines.

Abstract:Health literacy is influential in predicting the future health status of a patient, especially in chronic conditions such as diabetes. Factors influencing health literacy include language barriers. Population data from a general practice where we were on placement demonstrated 31.4% of patients did not speak English as a main language, indicating how language barriers could be affecting their healthcare. A needs analysis survey carried out within the practice showed 26% of patients did not feel confident in managing their diabetes. Our aim was therefore to organise focus group sessions targeting these concerns, with translations and a leaflet being provided. The barriers to effective diabetic management were identified as a lack of understanding of management of the condition, particularly dietary guidance; poor medication adherence; travel and lack of exercise. A leaflet was created in both Arabic and English addressing these areas. Patients with diabetes were recruited to participate in the focus group (n = 20). The intervention was assessed using questionnaire data obtained before and after the focus group. The focus group enabled certain barriers to diabetic management to be overcome with patients showing a improvement in understanding of prescribed medication and patients said they now felt more confident in managing their diabetes. Ten-minute consultations in general practice settings are often insufficient to guage patient understanding of their disease. Focus group sessions are a simple but effective way of educating patients on their illness, encouraging them to be more proactive in managing their condition.
Translating What Works: A New Approach to Improve Diabetes Management

Abstract:Background: The most efficacious strategies to improve diabetes control include case management, health care team changes, patient education, and facilitated transmission of patient data to clinicians ("facilitated relay"), but these strategies have not been translated to permit general use in clinical practice. Methods: A web-based decision support program was developed to include these features, and assessed in patients who had A1c ≥7.0% despite using metformin with/without sulfonylureas or insulin. Staff entered patients' glucose data, obtained management recommendations, reviewed the plan with a clinician, and discussed the new plan with patients. Results: 113 subjects were 96% male and 32% black, with average age 65.6 years and BMI 32.8. During prior primary care, A1c averaged 8.32 ± 0.16% (SEM). In all patients, baseline A1c was 8.18 ± 0.11%, and decreased to 7.54 ± 0.12%, 7.16 ± 0.13%, and 7.54 ± 0.16% at 3, 6, and 12 months, respectively, all P < .001. In 42 subjects who provided glucose data and made requested changes in medications, A1c was 8.12 ± 0.09% at baseline and fell to 7.29 ± 0.11%, 6.98 ± 0.10%, and 7.05 ± 0.10% at 3, 6, and 12 months, respectively, all P < .001. Chart review of 16 subjects followed for 12 months demonstrated that hypoglycemia (symptoms and/or glucose <70 mg/dl) averaged less than 1 episode/patient/month, and there was no severe hypoglycemia. Conclusions: A novel decision support program improved A1c with little hypoglycemia. Use of this approach should allow primary care teams to keep patients well controlled, and reduce the need for specialist referrals.

Abstract:Despite extensive evidence related to the prevention and management of type 2 diabetes (T2D) and its complications, most people at risk for and people who have diabetes do not receive recommended guideline-based care. Clinical implementation of proven care strategies is of the utmost importance because without this, even the most impressive research findings will remain of purely academic interest. In this review, we discuss the promise and challenges of implementing effective approaches to diabetes prevention and care in the real-world setting. We describe successful implementation projects in three critical areas of diabetes care—diabetes prevention, glycemic control, and prevention of diabetes-related complications—which provide a basis for further clinical translation and an impetus to improve the prevention and control of T2D in the community. Advancing the clinical translation of evidence-based care must include recognition of and assessment of existing gaps in care, identification of barriers to the delivery of optimal care, and a locally appropriate plan to address and overcome these barriers. Care models that promote team-based approaches, rather than reliance on patient-provider interactions, will enhance the delivery of contemporary comprehensive diabetes care.

Abstract:Objectives: Diabetes is a leading cause of death and disability in Canada. The co-occurrence of mental illnesses and diabetes is well documented, with diabetes being 2 to 3 times more prevalent in individuals with mental illnesses when compared to the general population. In clinical practices, diabetes management practices continue to be based on the chronic disease model, which conceptualizes diabetes as a physiologic and behavioural deficiency; therefore, clinical and policy efforts are directed toward the enhancement of patient self-management techniques through compliance with pharmaceutical and lifestyle recommendations. Little attention has been given to the exploration of how well the biomedical model aligns with the everyday realities of individuals experiencing mental illness. Methods: This project explored everyday experiences of diabetes self-management by those diagnosed with severe mental illnesses through the lens of critical ethnography. Results: This work demonstrated discordance between biomedical perspectives of diabetes management and the lived experiences of those with severe mental illness, such as schizophrenia spectrum, bipolar disorders and diabetes. Conclusions: This work offers an alternative conceptualization of diabetes management that moves beyond idealized concepts of self-care to introduce the social realities of patients as they attempt to enact and negotiate medical directives. This understanding encourages a shift toward social and contextual understandings of the lived realities of patients. Attention to how social context informs patients' realities may assist in the development of new patient-oriented grounds for public health strategies and clinical practices and may challenge traditional understandings of compliance and noncompliance.
The role of community pharmacists and their position in the delivery of diabetes care: an update for medical professionals

Abstract:Pharmacists are the third largest group of healthcare professionals worldwide, but are underused in the delivery of diabetes care. The aim of this narrative was to describe how integration of community pharmacy services into existing healthcare models may improve diabetes care. Relevant literature exploring pharmacy-led interventions for diabetes were identified from a search of Medline, Embase and Cinahl online databases. This review highlights that community pharmacists are accessible, experts in medicine management, trusted by the public and able to achieve financial savings. They are poorly integrated into existing healthcare models, and commissioning arrangements can be poorly perceived by the public and those working in primary care. Community pharmacy interventions in type 2 diabetes have similar, if not greater effects compared to those delivered by other healthcare professionals. It was concluded that community pharmacy interventions in diabetes are feasible, acceptable and deliver improved health outcomes. Future work should build public recognition of pharmacists and improve communication between them and other healthcare professionals.

Integrating point-of-care diabetes detection with lifestyle counselling in community settings: outcomes from Western Sydney, Australia

Abstract:Introduction: Early detection and prevention of type 2 diabetes and its complications are global health priorities. Optimal outcomes depend on individual awareness and proactive self-management of health risks. This study evaluates the effectiveness of a community-based diabetes detection and intervention program in a high-risk area in western Sydney, Australia. Research design and methods: We collaborated with the Workers Lifestyle Group, Tamil Association Arts and Culture Association, and the National Aboriginal and Islanders Day Observance Committee to implement our program. Participants underwent HbA1C testing via point-of-care blood spot testing. They received personalized feedback, education on diabetes management, and were offered opportunities to enrol in lifestyle modification programs. Participants identified with pre-diabetes (HbA1C 5.7-6.4%) or diabetes (HbA1C > 6.4%) were advised to consult their General Practitioners (GPs). A follow-up questionnaire was distributed 3-8 months post-intervention to evaluate the programs usefulness and relevance and lifestyle changes implemented by the participants. Results: Over eight months, 510 individuals participated. Of these, 19% had an HbA1C > 6.4%, and 38% had levels between 5.7 and 6.4%. Among those with diabetes, HbA1C levels ranged as follows: 56% <7%; 20% 7-7.9%; 18% 8-8.9%; and 5% >9%. Post intervention survey indicated that the program was well-received, with 62.5% of responses reporting lifestyle changes and 36.3% seeking further advice from their local healthcare providers. Conclusion: The study demonstrates a significant prevalence of pre-diabetes and diabetes in the community, similar to findings from larger-scale hospital and general practice studies. Point-of-care testing combined with personalized education effectively motivated participants toward healthier lifestyle choices and medical consultations. The paper discusses the scalability of this approach for broader population.

Abstract:Insulin initiation, which was traditionally the province of specialists, is increasingly undertaken by primary care. However, significant barriers to appropriate and timely initiation still exist. Whilst insulin is recognized as providing the most effective treatment in type 2 diabetes, it is also widely considered to be the most challenging and time consuming. This editorial identifies that the organization of existing healthcare services, the challenges faced by patients, and the treatments themselves contribute to suboptimal insulin management. In order to improve future diabetes care, it will be necessary to address all three problem areas: (1) re-think the best use of existing human and financial resources to promote and support patient self-management and adherence to treatment; (2) empower patients to participate more actively in treatment decision making; and (3) improve acceptance, persistence and adherence to therapy by continuing to refine insulin therapy and treatment regimens in terms of safety, simplicity and convenience. The principles discussed are also applicable to the successful management of any chronic medical illness.

Abstract:Diabetes mellitus is a global health concern associated with significant morbidity and mortality. Inadequate control of diabetes leads to chronic complications and higher mortality rates, which emphasizes the importance of achieving glycemic targets. Although glycated hemoglobin (HbA1c) is the gold standard for measuring glycemic control, it has several limitations. Therefore, in recent years, along with the emergence of continuous glucose monitoring (CGM) technology, glycemic control modalities have moved beyond HbA1c. They encompass modern glucometrics, such as glycemic variability (GV) and time-in-range (TIR). The key advantage of these newer metrics over HbA1c is that they allow personalized diabetes management with person-centric glycemic control. Basal insulin analogues, especially second-generation basal insulins with properties such as longer duration of action and low risk of hypoglycemia, have demonstrated clinical benefits by reducing GV and improving TIR. Therefore, for more effective and accurate diabetes management, the development of an integrated approach with second-generation basal insulin and glucometrics involving GV and TIR is the need of the hour. With this objective, a multinational group of endocrinologists and diabetologists reviewed the existing recommendations on TIR, provided their clinical insights into the individualization of TIR targets, and elucidated on the role of the second-generation basal insulin analogues in addressing TIR.

Abstract:Background: Electronic physician consultations (e-consults) with endocrinologists represent a safe and effective alternative to many referrals for diabetes care. E-consults represent an important, but underutilized, modality to address the shortage of endocrinologists. Aim: To implement a scalable diabetes e-consult service for a large network of primary care practices in the United States. Methods: Primary care providers (PCPs) from 40 practices used a designated email to consult endocrinologists based at a diabetes center. Endocrinologists also had remote access to the electronic health records used by consulting PCPs. E-consults provided individualized recommendations for management, evaluation, targeted education or referral to the diabetes center in addition to targeted education. Provider experience surveys were completed online by PCPs and specialists after each consult. Results: Between Feb-Sep 2022, 55 e-consults were completed; 86% within 2 business days. The service was positively reviewed by PCPs; 96% reported the e-consult was helpful, 86% reporting more confidence in diabetes care; and 64% reported the e-consult saved them time. A majority of e-consults (PCP, 79%; endocrinologist, 77%) required <10 mins to complete. Specialists made no recommendations for a patient referral to the diabetes center. Conclusions: A diabetes e-consult service can be rapidly implemented with minimal administrative support. It has the potential to bring greater diabetes expertise to primary care practices and save the PCPs time while avoiding unnecessary referrals to endocrinology practices. Improving access to diabetes care in an inner-city, community-based outpatient health center with a monthly open-access, multistation group visit program

Abstract:Objective: To describe the development and implementation of a multifaceted program in an inner-city healthcare center designed to improve access to care and empower patients to take a more active role in managing diabetes. Procedures: AHC is one of 30 outpatient health centers in the Ambulatory and Community Health Network of the Cook County Bureau of Health Services. AHC serves a predominantly African-American population with four full-time-equivalent primary care providers treating approximately 700 diabetes patients with >450 waitlisted patients, many with diabetes. Budget constraints limit capacity to add providers. In January 2005, open-access, multi-station group visits were implemented to improve access to care and empower patients to take a more active role in managing diabetes. The program is called Diabetic Rewards Issued Via Everyone (DRIVE) Day. Elements include: 1. group visits held monthly; 2. patient-selected activities, including diabetes education, nutrition, exercise, group discussions and Q&A sessions; 3. provider support, including implementation of evidence-based guidelines for glycemic, lipid and hypertension management, retinal screening, foot exams and medication adjustment; and 4. web-based patient registry Findings: Of the clinic's 737 diabetes patients, 294 (40%) have attended > or =1 DRIVE Days, for a total of 775 patient encounters between January 2005 and October 2006. Conclusions: In an environment with limited resources, DRIVE Day has improved access to care, provided an opportunity for diabetes patients to take a more active role in their care and enabled providers to see a higher volume of patients and offer efficient, comprehensive care.
Lifestyle advice, processes of care and glycaemic control amongst patients with type 2 diabetes in a South African primary care facility

Abstract:Background: The influence of processes of diabetes care on glycaemic control is understudied in primary health care (PHC).Aim: To explore the influence of lifestyle advice, drug regimen and other processes of care on glycaemic control.Setting: Johan Heyns Community Health Centre, Vanderbijlpark, South Africa.Methods: In a cross-sectional study involving 200 participants with type-2 diabetes, we collected information on sociodemography, comorbidity, processes of diabetes care, drug regimen and receipt of lifestyle advice. Anthropometric measures and glycosylated haemoglobin (HbA1c) were also determined.Results: Participants' mean age was 57.8 years and most were black people (88%), females (63%), overweight or obese (94.5%), had diabetes for < 10 years (67.9%) and hypertension as comorbidity (98%). Most participants received lifestyle advice on one of diet, exercise and weight control (67%) and had their blood pressure (BP) checked (93%) in the preceding 12 months. However, < 2% had any of HbA1c, weight, waist circumference or body mass index checked. Glycaemic control (HbA1c < 7%) was achieved in only 24.5% of participants. Exclusive insulin or oral drug was prescribed in 5% and 62% of participants, respectively. Compared to insulin monotherapy, participants on combined metformin and insulin or metformin, sulphonylurea and insulin were less likely to have glycaemic control. Comorbid congestive cardiac failure (CCF) significantly increased the likelihood of glycaemic control.Conclusion: There is substantial shortcomings in the implementation of key processes of diabetes care and glycaemic control. Strategies are needed to prompt and compel healthcare providers to implement evidence-based diabetes guidelines during clinic visits in South African PHC.
Designing a regional clinical service for people with early‐onset type 2 diabetes in England

Abstract:Aims To design a regional clinical service for people with early‐onset type 2 diabetes (EOT2D) in Leicester, Leicestershire and Rutland (England). Methods A literature search was undertaken to identify important considerations. A working group of key stakeholders was formed to design a triage system and service pathway. Electronic medical records (EMRs) were searched (15th November 2023) to assess feasibility of the pathway and adapt accordingly. Results A literature search identified important considerations: High risk of complications; large proportion from minority ethnic and socioeconomically deprived backgrounds; significant psychological burden; stigma and other social challenges; and misclassification and miscoding. Novel clinical risk criteria were developed, implementable in EMRs, to match intervention‐intensity to clinical need. Specialist clinics were planned, one for people at the highest‐clinical risk, another for women with adverse perinatal risk factors. A healthcare professional training package was developed to increase awareness of the unmet clinical needs of people with EOT2D and to upskill in provision of holistic care. Subsequent EMR searches supported the need for our service. Due to the large numbers with HbA1c ≥86mmol/mol (10.0%; n=299; 10.8% of total), these people were prioritised for clinic access. We opted for specialist nurse/educator support to practices with clustering of patients and to financially incentivise referrals from primary care into services. Conclusions We showcase a service specifically for people with EOT2D based on the literature, a broad range of stakeholder involvement and utilising a locally‐sourced data‐driven approach. We further discuss areas for development and recommendations based on the challenges we encountered.

Management of type 2 diabetes mellitus in people with severe mental illness: an online cross-sectional survey of healthcare professionals

Abstract:Objectives: To establish healthcare professionals' (HCPs) views about clinical roles, and the barriers and enablers to delivery of diabetes care for people with severe mental illness (SMI). Design: Cross-sectional, postal and online survey. Setting: Trusts within the National Health Service, mental health and diabetes charities, and professional bodies. Participants: HCPs who care for people with type 2 diabetes mellitus (T2DM) and/or SMI in the UK. Primary and secondary outcome measures: The barriers, enablers and experiences of delivering T2DM care for people with SMI, informed by the Theoretical Domains Framework. Results: Respondents were 273 HCPs, primarily mental health nurses (33.7%) and psychiatrists (32.2%). Only 25% of respondents had received training in managing T2DM in people with SMI. Univariate analysis found that mental health professionals felt responsible for significantly fewer recommended diabetes care standards than physical health professionals (P<0.001). For those seeing diabetes care as part of their role, the significant barriers to its delivery in the multiple regression analyses were a lack of knowledge (P=0.003); a need for training in communication and negotiation skills (P=0.04); a lack of optimism about the health of their clients (P=0.04) and their ability to manage T2DM in people with SMI (P=0.003); the threat of being disciplined (P=0.02); fear of working with people with a mental health condition (P=0.01); a lack of service user engagement (P=0.006); and a need for incentives (P=0.04). The significant enablers were an understanding of the need to tailor treatments (P=0.04) and goals (P=0.02) for people with SMI. Conclusions: This survey indicates that despite current guidelines, diabetes care in mental health settings remains peripheral. Even when diabetes care is perceived as part of an HCP's role, various individual and organisational barriers to delivering recommended T2DM care standards to people with SMI are experienced.

Abstract:Federally qualified health centers (FQHCs) serve vulnerable DM patients in primary care in underserved settings. The state of DM (glycemic control, complications) may differ and utilization of technologies (e.g. continuous glucose monitoring [CGM]) may be essential. But the current state of DM and CGM is poorly characterized. Using EHR data from a national FQHC network (2015-2021) we examined baseline demographic characteristics, CGM prescriptions, and their association. We included 47,579 with DM: n=1630 with T1D [3%], n= 45,949 T2D [97%], 38% Hispanic, 34% Black (Table 1). Notably, T1D pts [mean age 43 SD 16], had poor glycemic control (HbA1c 9.4 SD 2.4) with elevated DM complications severity index scores [mean DSCI 2.3 SD 4.7]. Overall CGM prescriptions were extremely low; 9% in T1D and only 1% in T2D. Among T1D, those who were Hispanic [OR 0.39, (CI 0.23, 0.66), p < 0.001] or Black [OR 0.52, (CI 0.33, 0.81), p = 0.003] were less likely to have CGM prescribed than White adults, after adjustment. Among T2D, those who were Hispanic [OR 0.42, CI 0.33, 0.55, p < 0.001] or Black [OR 0.68, CI 0.54, 0.85, p = 0.001] were similarly less likely to have CGM. Non-private insurance was also a significant factor. DM severity (poor control, complications) is notable in FQHCs, yet CGM is exceptionally low. These findings underscore the major translation gap in standard of care for FQHCs and the critical need to understand inequities. Disclosure A. Abstract:The Voluntary, Community and Social Enterprise (VCSE) sector offers services and leadership within the health and care system in England and has a specialist role in working with underserved, deprived communities. This evaluation aims to identify best practices in self-management support for those living with type 2 diabetes mellitus (T2DM) and to develop a theory of change (TofC) through understanding the impact of VCSE organisations on diabetes management. An appreciative inquiry (AI) was carried out and co-delivered using qualitative interviews and an embedded analysis with VCSE partners. A voluntary service coordinated seven VCSE organisations who assisted with recruiting their service users and undertook interviews to identify the impact of existing activities and programmes. People living with T2DM were interviewed about services. Themes were as follows: (a) individual and group activities; (b) trusted services and relationships across the community; (c) long-term engagement; (d) sociocultural context of diet and nutritional choices; (e) experience of adaptation; and (f) culturally appropriate advice and independent VCSE organisations. The structured educational approach (DESMOND) for T2DM was accessed variably, despite these services being recommended by NICE guidelines as a standard intervention. The VCSE offered continuity and culturally appropriate services to more marginalised groups. This evaluation highlights the importance of targeted engagement with underserved communities, particularly where primary care services are more limited. The TofC is a unique insight into the impact of VCSE services, offering bespoke support to manage T2DM, suggesting areas for improvements in capacity and offering the capability to sustain the VCSE sector as an essential element of the T2DM care pathway in England.

Abstract:INTRODUCTION: Diabetes is a chronic disease amenable to management in the community and outpatient setting. The increasing incidence of diabetes places outpatient endocrinology services under pressure to provide a quality service in a timely manner. Our aim was to apply lean thinking to the diabetes clinic in a tertiary referral centre in the West of Ireland to improve flow, as reflected in reduced patient journey times.METHODS: The project lasted 6 months, from January to June 2011. An introductory seminar on lean thinking was arranged to inform and motivate the Diabetes Day Centre staff. Two 'rapid improvement events' took place. Value stream mapping (VSM) was the predominant lean tool employed. Patient journeys were mapped and quantified (minutes) using timesheets allocated to each step in the process at baseline, and following intervention. Data were analysed using Minitab V.16.0.RESULTS: VSM allowed the value-adding and problem-causing steps in the patient journey through the diabetes clinic process to be identified and addressed. Total patient journey time through the clinic was significantly reduced from 118 (± 38.02) min to 58 (± 18.30) min (p<0.001).CONCLUSIONS: This project reflects the successful application of VSM as a lean tool in a pilot study at our institution as evidenced by improved patient flow and a significant reduction in patient journey time through the clinic. Through the incorporation of Lean into the ethos of the hospital, we have the potential to deliver excellent care in a safe environment and in an efficient manner, while benefiting the patient, employees and tax-payer.
