Abstract:Background: The aim of the present study was to describe the characteristics of dental fear of Chinese adult patients with periodontal disease and provide information for clinical assessment.Methods: A total of 1203 dental patients completed questionnaires that included Corach's Dental Anxiety Scales (DAS), Dental Fear Survey (DFS) and the short-form Dental Anxiety Inventory (S-DAI). Among all the patients, 366 cases were self-reported periodontal disease. The general characteristics were described, such as socio-demographics, dental attendances and oral health behaviors. The statistical analysis was performed by t-test, Mann-Whitney U test and linear regression respectively to evaluate correlations between dental fear and general characteristics according to the three scales.Results: The prevalence of dental fear was 74% among 1203 patients, 23.4% of total with high dental fear, while 27.3% in the patients with periodontal disease. The average score of DAS and DFS for patients with periodontal disease was significantly higher than those without periodontal disease. The regression analysis indicated that gender, age, periodontal status, dental attendances and oral health behaviors were correlated with dental fear. Among 366 patients with periodontal disease, gender, dental attendances and oral health behaviors had correlation with dental fear. The analysis of DFS scale exhibited that 'drilling with handpiece' and 'injecting the anesthetic' were the most important factors to contribute to dental fear.Conclusions: There was high prevalence of dental fear in Chinese adult patients, particularly in patients with periodontal disease, and high level of dental fear may lead to poor periodontal status.

Abstract:OBJECTIVE:To investigate the factors associated with halitosis in Chinese white-collar employees.MATERIALS AND METHODS:Subjects in three randomly selected office buildings in Shanghai, China, were enrolled in this cross-sectional study using cluster random sampling. Oral malodor was assessed by measuring volatile sulfur compounds (VSCs) with a portable sulfide monitor. Subjects' oral health, including dental caries, periodontal status, and tongue coating, was evaluated clinically. A questionnaire was used to obtain information about participants' demographic characteristics, oral hygiene habits, and health behaviors.RESULTS:Of the 805 subjects invited to participate in this study, 720 were enrolled (89.4% response rate). Data from these subjects were used for statistical analyses. The prevalence of halitosis was 33.2%. In the final regression model, halitosis was significantly related to tongue coating thickness, periodontal pocket depth, no food consumption within 2 hours prior to oral examination, and less intake frequency of sweet foods.CONCLUSIONS:In this Chinese white-collar population, tongue coating and periodontal disease were associated with halitosis. Oral hygiene education should be provided at the population level to encourage the maintenance of oral health and fresh breath. Consumption of sweet foods may reduce VSC production, although this finding requires further investigation.

Conclusion In this Shanghai white?collar population, tongue coating and periodontal dis?ease were associated with halitosis. Abstract:Objective The aim of this study is to assess the prevalence of self-reported halitosis and its associated factors.Methods Self-designed questionnaire,with the content of sociodemographic information,medical history,toothbrushing habit,smoking and halitosis,was conducted among 2000 general population in 4 districts/counties in Beijing.Results The prevalence of self-reported halitosis was 26.4%.44.9% of the subjects did not have correct knowledge of halitosis.59.7% reported that they suffered from halitosis for 3 years or above.And 20.9% said that they were affected often by it.27.6% ever tried mouthrinse or gum to eliminate halitosis.However,only 2.5% wanted to solve the problem by visiting dentist.Associated factors with self-reported halitosis were male,smoking,urban residence and system disease.Conclusion The prevalence of self-reported halitosis of general population in our country is higher than other countries.The knowledge about halitosis should be improved.Male,smoking,inadequate oral health habit and system disease contributed to self-reported halitosis.

Abstract:Objective The aim of this study was to assess the relationship between the oral health habit and halitosis. It can provide an advisory basis for nurse giving individualized oral health instruction to patients of halitosis and can improve the result of prevention and treatment for halitosis accordingly. Methods 202 patients with periodontal disease were chosen to participate in this study. The study included the questionnaire of oral health habit and the halitosis diagnosis by organoleptic assessment.Comparisons of difference in the knowledge of oral health habit between the subjects with and without halitosis were conducted. Results Brushing times in subjects without halitosis were significantly more than that in subjects with halitosis. Brushing teeth in evening and interproximal brush usage were much better in subjects without halitosis than that in subjects with halitosis,respectively. Conclusions Good oral health habits could decrease the incidence of halitosis.

Abstract:Objectives: To estimate the prevalence of halitosis in the Chinese population and to assess the relationships between halitosis and oral health, social and behavioural factors.Methods: The correlation between the incidence of oral malodor and oral health was surveyed in a sample of 2000 individuals (1000 males and 1000 females) aged 15-64 years residing in urban and rural areas. Malodor was measured with both organoleptic measurements and with a portable sulphide monitor. Assessment of oral health included decayed, missing and filled teeth (DMFT), periodontal status, dental plaque, and tongue coating. Behavioural and social factors related with oral health or halitosis were also investigated.Results: The prevalence of halitosis was 27.5% according to the organoleptic score. The level of volatile sulphur compounds (VSCs) in mouth air was significantly lower in males and in some of the age groups after lunch. Age and location of residence (rural or urban areas) did not influence the VSCs concentration in mouth air. The amount of tongue coating played the most important role in increasing VSCs concentration in mouth air, followed by periodontal status and plaque index values. DMFT, social, and behavioural factors did not contribute to halitosis.Conclusions: Tongue coating score, modified sulcus bleeding index and calculus index were factors significantly related to oral malodor in this study.

Abstract:OBJECTIVES:To describe the characteristics of patients who visited halitosis clinic and to investigate the factors that may associate with halitosis.MATERIALS AND METHODS:Nine hundred and eleven patients, who visited halitosis clinic for consultations, underwent organoleptic tests and volatile sulfur compound (VSC) measurements with a portable sulfide monitor. The oral health of patients, including dental caries, oral hygiene, and gingival inflammation status, was assessed. Questionnaires were used to obtain information on demographics, oral hygiene practices, and dietary habits.RESULTS:Of the 911 patients, more females than males visited the clinic. Patients' age ranged from 18 to 82 years (mean ± s.d.: 40.8 ± 14.1). Almost 30% of patients had complained of halitosis for over 5 years before looking into treatments. Approximately half of the patients self-perceived that they were unable to get close to others. Around 77% of patients had halitosis. Results of multiple logistic regressions for organoleptic scores and VSC values showed that tongue coating and tongue scraping were significantly associated with halitosis (P < 0.05).CONCLUSIONS:Among these Chinese patients, over three-quarters of patients had halitosis, and negative impacts on social communications were observed. Tongue coating was the most important factor responsible for halitosis.
Preliminary study of the psychological condition of halitosis patients

Abstract:Objective To assess the differences of psychological condition between patients with and without halitosis,and to explore the psychological problems of halitosis patients.Methods Patients who were complained of bad breath and were diagnosed as halitosis in the Shanghai Ninth People's Hospital were included into case group,while patients who were diagnosed without halitosis were included into control group.Questionnaires and measurement of the halitosis were used to collect the data.In questionnaires,information on participants' general status,Self-reporting Inventory(SCL-90) were collected.To measure the oral malodor,Oral Chroma and organoleptic test were used.SPSS19.0 was used for data analysis.Results The halitosis group had significantly higher SCL-90 scores than the control group (0.63 vs.0.48,P=0.002).The scores of the factors of interpersonal sensitivity,anxiety,depression,paranoid ideation were found significantly different between two groups.Conclusions Psychological condition in patients with halitosis is significantly worse than patients without halitosis.Four aspects are mainly manifested in the interpersonal sensitivity,anxiety,depression and paranoid ideation.It is suggested that to improve patients' quality of life,we should pay attention to the mental health of patients in the clinical treatment of halitosis patients,strengthen the education and guidance.

Abstract:OBJECTIVE:To evaluate self-reported halitosis in patients who visited a periodontal clinic and assess the possible factors associated with genuine halitosis. METHODS:The subjects included 435 patients who visited periodontal department of Peking University, School and Hospital of Stomatology. All of them were non-smokers. First, the degree of halitosis was estimated by Organoleptic Test. Then, a standardized questionnaire focusing on life style, oral hygiene, medical history, dental condition, self-reported halitosis was completed by all participants. Additionally, tongue coating was evaluated for 312 patients. RESULTS:273 of 435 participants had self-reported halitosis. According to Organoleptic Test, 31.2% patients complaining halitosis had actually no malodor while 68.8% had genuine halitosis. There was statistically significant difference of the proportion of patients who were unwilling to mention about halitosis between genuine halitosis group and pseudo-halitosis group (P=0.003). Proportion of patients who had accepted treatment for halitosis was statistically significant higher in pseudo-halitosis group than in genuine halitosis group (P=0.029). Logistic regression analysis showed bleeding on brushing (OR=2.905, P=0.000), area of tongue coating≥2 (OR=2.395, P=0.019), thickness of tongue coating≥2 (OR=3.419, P=0.000) were factors associated with genuine halitosis. CONCLUSION:Not all self-reported patients actually had malodor, the psychological condition was different between pseudo-halitosis and genuine halitosis patients. Bleeding on brushing, large tongue coating area, thick tongue coating were associated with genuine halitosis.

Abstract:ObjectivesTo compare differences in oral health-related quality of life (OHRQoL) between halitosis and non-halitosis patients and to explore relationships between halitosis and OHRQoL. MethodsPatients who visited the halitosis clinic at the Ninth People's Hospital and were diagnosed with halitosis were included in the halitosis group. The control group consisted of patients without halitosis. All participants were assessed for halitosis (volatile sulphur compound measurements with a portable sulphide monitor, Oral Chroma and organoleptic tests), underwent a clinical oral examination (dental caries status and periodontal condition) and completed a questionnaire. Oral health-related quality of life was measured using the Oral Health Impact Profile (OHIP-14, Chinese version). ResultsA total of 204 patients (102 halitosis and 102 controls) were included. The halitosis group had significantly higher OHIP-14 scores (severity) than the control group (15.7 versus 7.9, P < 0.001). The mean number of items with negative impact (extent) was significantly different between the halitosis and control groups (1.8 versus 0.3, P < 0.001). Around 56% of participants in the halitosis group reported at least one item with fairly often' or very often' (prevalence), while 21% did so in the control group (P < 0.001). In halitosis patients, the most commonly reported negative impacts were within the domains of psychological discomfort' and psychological disability'. ConclusionsOral health-related quality of life status was significantly poorer in halitosis patients than in non-halitosis patients. Halitosis patients may experience some degree of psychological discomfort and disability.

Personality and psychological profile for patients with and without halitosis: A case-control study

Abstract:Objective: To compare the personalities, social avoidance and distress, and anxiety status of Chinese patients with and without objective halitosis, and investigate the association among these psychological disorders. Methods: Patients who complained of bad breath and diagnosed with objective halitosis were enrolled into the halitosis group, while patients without objective halitosis were enrolled into the control group. The questionnaires included the sociodemographic profile of the participants, Eysenck Personality Questionnaire (EPQ), Social Avoidance and Distress Scale (SAD), and Beck Anxiety Inventory (BAI). Results: A total of 280 patients were assigned into objective halitosis group (n = 146) and control group (n = 134). The extraversion subscales (E) score of the EPQ in the halitosis group were significantly lower than that in the control group (p = 0.001). The total SAD score and proportion of patients with anxiety symptoms in the BAI scale in the objective halitosis was significantly higher than that in the control group (p < 0.05). There was a negative correlation between the extraversion subscale and the total SAD score, Social Avoidance and Social Distress subscales (p < 0.001). Conclusion: Patients with objective halitosis have more introverted personality traits and are more likely to have social avoidance and distress than the nonhalitosis population.
Tongue coating microbiome characterization of Chinese preschool children with halitosis

Abstract:Abstract Background: Halitosis may affect individuals of any age. However, epidemiological surveys and etiological research into halitosis among children are limited. To assess the bacterial characteristics and environmental factors associated with intra-oral halitosisamong Chinese preschool children. Methods: An epidemiological study was conducted among 273 preschool children (aged 3 to 4 years) in Shanghai, China,followed by collection of 16 samples from the tongue coating of caries-free healthy and halitosis participants according to their organoleptic score.The characterization of associated microbial communities was performed using 16S rRNA genesequencingon the MiSeq. Results:Halitosis was observed in 13.1% of the preschool children.Logistic regression analysis showed that less frequent intake of sweet snacks and mothers’ poor oral condition were associated with children’s halitosis positively. Further analysis detected the presence of 15 phyla, 24 classes, 35 orders, 53 families, 93 genera, and 226 operational taxonomic units (OTUs) in the microbial communities. In terms of microbial diversity, the tongue coating of control and halitosis groups showed no significant difference (P>0.05). Both groups shared a mass of common OTUs. The dominant bacterial genera observed in both groups included Prevotella_7(15.17% vs. 10.72%), Veillonella (13.52% vs. 10.86%), Streptococcus (13.95% vs. 18.68%),Neisseria (9.44% vs. 21.14%), Actinomyces(7.37% vs. 5.88%), Haemophilus (6.69% vs. 5.69%), andLeptotrichia (5.05% vs. 1.95%). At the genus level, the proportion of Gemellawas statistically larger in the control group (P=0.018). Conclusions: Overall, halitosis is associated with multi-microbial mutual interactions. The oral flora microorganisms may exert potentiating or inhibiting effects on each other.
Analysis of oral malodor-related factors

Abstract:Objective:The aim of this paper was to analyse the characteristics of 780 patients who visited halitosis clinic,and to assess the relationships between halitosis and oral health,Systemic disease,eating habits,social and behavioural factors.Method:The patients were explored by means of a standard questionnaire and a clinical examination.Malodor was measured with a portable sulphide monitor(Halimeters).SPSS software package were used for data analysis.Result:The mean patient's age is 39.4±15.2,The detection rate of oral-malodor was 53.7 %,The patients aged from 15～35,with the tongue coating index 3～4 and who had smoking habit had more risk(P <0.05),Tongue brushing decrease the detection rate of halitosis(P= 0.001).Conclusion: The level of VSCs varied with age,tongue coating,smoking habit,and tongue brushing.Systemic disease,eating habits,flossing,mouthrinsing did not influence VSCs levels.
Psychological Characteristics of Chinese Patients with Genuine Halitosis

Abstract:Objective The present case-control study aimed to compare the psychological status of patients with and without halitosis, and investigate the association between psychological disorders and halitosis. Methods Patients who complained about bad breath and diagnosed with genuine halitosis were assigned to the halitosis group, while patients without genuine halitosis were assigned to the control group (n = 106, each group). Information on the demographics and Symptom Checklist-90 (SCL-90) of participants was collected. The organoleptic score and Halimeter measurement were used to measure halitosis. Results The mean SCL-90 score of participants in the halitosis group (0.63) was significantly greater than that in the control group (0.48) (p = .002). The scores of the domains of interpersonal sensitivity, anxiety, depression, and paranoid ideation were found to be significantly different between the two groups (p < .05). However, there were no significant differences in SCL-90 scores between the mild and moderate-severe halitosis groups (p = .479). Conclusion The psychological status of genuine halitosis patients was significantly worse than normal patients without halitosis complaint and also without halitosis. The main problems were in the aspects of interpersonal sensitivity, anxiety, depression, and paranoia. However, the negative impact was not related to the severity of halitosis.

Abstract:Objectives: This study aims to review halitosis research, discuss its various causes, and propose effective interventions based on the underlying etiologies and mechanisms. The main research question is to identify the primary factors contributing to halitosis and appropriate strategies to address them. Materials and methods: A comprehensive literature review was conducted on halitosis and its associated causes, including oral pathological factors, oral microbial influences, microbial metabolic pathways, gastrointestinal diseases, and gut microbiota dysbiosis. Results: Unhealthy eating habits and an imbalance of microorganisms in the oral cavity and gastrointestinal tract were identified as primary causes of halitosis. Dental caries, periodontal disease, xerostomia, and digestive disorders like gastritis and irritable bowel syndrome were also found to be related to the development of halitosis. Due to poor oral hygiene or antibiotic use, disruption of microbial communities can result in dysbiosis, inflammation, and halitosis. Conclusions: Halitosis is a multifactorial condition with various underlying causes, including oral and systemic diseases. Effective interventions should be tailored based on the specific etiologies and mechanisms involved. Clinical relevance: Understanding the factors contributing to halitosis is crucial for developing appropriate treatment strategies. Enhancing oral hygiene habits, using antimicrobial drugs, or administering probiotics may help regulate oral or intestinal flora, thereby improving halitosis and overall oral health.
Comparison of the Psychological Condition of Chinese Patients with or Without Halitosis Complaints

Abstract:Objective: To investigate the psychological condition of Chinese patients with or without a primary complaint of halitosis. Methods: The psychological condition of 196 Chinese patients who visited the Department of Periodontics was evaluated using the Cornell Medical Index (CMI) health questionnaire. The breath malodour of all patients was scored using the organoleptic test (OLT). Results: More than half of patients (53.1%) complaining of halitosis actually demonstrated pseudo-halitosis. The overall scores and scores for emotional symptoms of CMI were significantly higher in patients with a chief complaint of halitosis than in those without such a complaint (P < 0.05). Females with a chief complaint of halitosis exhibited significantly higher scores for all parameters relative to females without a complaint (P < 0.05), while males with a chief complaint of halitosis exhibited enhanced emotional symptoms, particularly inadequacy, anxiety, and tension (P < 0.05) when compared with males without a halitosis complaint. Among patients complaining of halitosis, CMI scores for all parameters showed no difference between genuine halitosis female patients and pseudo-halitosis female patients, while male patients with pseudo-halitosis showed higher scores in the CMI overall score, scores for emotional symptoms and sensitivity, than males with genuine halitosis. Conclusion: There was a marked inconsistency between the complaints of patients and their actual odour status. Patients with a chief complaint of halitosis exhibited a greater level of inadequacy, depression, anxiety, sensitivity, anger, and stress. The psychological status of the patients varied with gender.
Microbiome Variations in Preschool Children with Halitosis.

Abstract:OBJECTIVE To assess the bacterial characteristics associated with intra-oral halitosis among Chinese preschool children. METHODS An epidemiological study was conducted among 273 preschool children (aged 3 to 4 years) in Shanghai, China, followed by collection of 16 samples from the tongue coating of caries-free healthy and halitosis participants. The characterization of associated microbial communities was performed using 16S rRNA gene sequencing on the MiSeq. RESULTS Halitosis was observed in 13.2% of the preschool children. No significant difference in terms of microbial diversity (P>0.05) were detected in control and halitosis groups. The dominant bacterial genera observed in both groups included Prevotella, Veillonella, Streptococcus, Neisseria, Actinomyces, Haemophilus, and Leptotrichia. The relative proportions of thirteen species (including Leptotrichia sp. HMT_417, Prevotella Pallens and Alloprevotella rava) were significantly higher in the halitosis group compared to the control group. Whereas the distribution of seventeen species from genera Streptococcus, Gemella and Kingella showed a higher abundance in the control group. Further investigation of interactions among species demonstrated obvious differences in two groups, indicating that various microorganisms interact to produce halitosis in preschool children. CONCLUSIONS Overall, halitosis is associated with multi-microbial mutual interactions. The oral flora microorganisms may exert potentiating or inhibiting effects on each other.
Relationship between halitosis and helicobacter pylori infection:a case-control study

Abstract:Objective To investigate the relationship between halitosis and helicobacter pylori infection in stomach. Methods 226 subjects without periodontal diseases or systematic disease ( except helicobacter pylori infection) were included. Nasal measurement method was used to divide the participants into the halitosis group ( case group) and the non?halitosis group ( control group) . Basic in?formation such as gender and age and oral health situation was matched between groups with no statistical difference. Halitosis was diag?nosed by OS≥2 and VSC≥110 μg/L respectively. Infection of helicobacter pylori was diagnosed by biopsy. Clinical examination ( in?cluding malodour test and oral examination) and questionnaire survey were conducted before gastroscopy. SPSS19.0 software package was used to analyze the data. The logistic regression model was used to analyze the relationship between halitosis and helicobacter pylori infection. Results All the subjects were periodontally healthy according to the periodontal index, with 117 in the case group and 109 in the control group. The prevalence of helicobacter pylori infection in halitosis subjects diagnosed by OS≥2 was significantly higher than that in the normal subjects (60.7% vs 22.9%, P<0.01). Logistic regression analysis showed helicobacter pylori infection was the significant variable in the equation, and the adjusted OR was 4.6 ( P<0.05) . When diagnosed by VSC≥110 μg/L, adjusted OR was 9.3. Conclusion Helicobacter pylori infection in stomach may be involved in the presence of halitosis in periodontally healthy subjects. Further large scale sample prospective studies should be done.
Whether Chinese Medicine Have Effect On Halitosis: A Systematic Review And Meta-Analysis

Abstract:Object. Halitosis has great adverse impact on personal and social life. There is no strong evidence for the effect of Chinese medicine (CM) and combined Chinese and western medicine (CWM) on halitosis. The aim of the present study is to evaluate the effective rate of CM and CWM on halitosis. Materials and Methods. Literature search in English and Chinese was conducted in PubMed, Embase, CNKI, CBM, and Wanfang database. Study selection and data collection were conducted. Risks of bias were assessed by the Cochrane tool. Synthesis of results was done by RevMan 5.3. p< 0.05 was considered significant difference. Subgroup analysis by classification of halitosis and sensitivity analysis were also conducted. Results. Seventeen studies were included. The follow-up length ranged from five days to eight weeks. CM had significantly better effect than WMon intraoral halitosis (I-2 = 24%; RR= 1.21 (95% CI, 1.04, 1.40), P=0.01) and extraoral halitosis (I-2 = 0; RR= 1.39 (95% CI, 1.19, 1.63), P<0.0001). CWM had significantly better effect than WM on intraoral halitosis (I 2 = 0; RR= 1.25 (95% CI, 1.16, 1.35), P<0.00001) and extraoral halitosis (I-2 = 0; RR= 1.19 (95% CI, 1.08, 1.31), P=0.0004). Subgroup analysis and sensitivity analysis showed insignificant results. Conclusion. With the limitation of our study, both CM and CWM have significantly better effect on halitosis than WM. More effort should be made to explore long-term effect of CM and CWM on halitosis. This study was registered with the PROSPERO (ID: CRD42018107229).
