Abstract:Background:Breast cancer (BC) is the leading cause of death in the female reproductive system, often linked to lymph node involvement, indicating poor prognosis. This study investigated lymph node metastasis incidence and risk factors in M0 stage BC patients who hadn't received preoperative chemoradiotherapy or neoadjuvant therapy. We explored the influence of various factors on lymph node metastasis.Methods:We conducted a retrospective analysis using Surveillance, Epidemiology, and End Results data from BC patients diagnosed between 2010 and 2015. Binary logistic regression and propensity score matching (PSM) assessed significant factors in BC patients without preoperative treatment. We developed predictive nomograms and evaluated model performance using the concordance index, calibration curve, area under the curve, and decision curve analysis.Results:Among 256,504 eligible BC patients, 25.57% had lymph node metastasis. Multivariate logistic regression revealed associations between lymph node metastasis and younger age, African-American ethnicity, central/nipple location, lobular carcinoma, human epidermal growth factor receptor 2 (HER2)-positive status, grade III classification, and T3 stage. PSM confirmed these findings. Interactions were identified between age, race, primary site, histology, breast subtype, grade, and T stage, all influencing lymph node metastasis.Conclusions:This retrospective study identified lymph node metastasis in female BC patients with distinct clinicopathological characteristics who received no preoperative treatment. We constructed valuable nomograms, revealing that: (I) young age (<35 years), African-American race, central/nipple location, infiltrating duct carcinoma, HER2 positivity, high histological grade (grade III), and larger tumor size are risk factors for regional lymph node metastasis; (II) lymph node metastasis may not solely represent the invasive nature of triple-negative BC; (III) patients with different BC subtypes in T1c-T2 stages may benefit from individualized neoadjuvant treatment strategies.

Abstract:Objective To analyze the outcomes and the role of radiotherapy in breast cancer pa-tients with T1-T2 and one to three positive axillary nodes treated with modified radical mastectomy, and to investigate the prognostic factors for loco - regional recurrence in patients without radiotherapy . Methods Three hundred and seventy breast cancer patients with T1-T2 and one to three positive axillary lymph nodes treated with mastectomy and axillary dissection were retrospectively analyzed. Kaplan-Meier method was used to calculate the overall survival (OS) and loco-regional recurrence-free survival (LRFS) rates. The Logrank test was used for the comparison of the survival curves of patients with or without radiotherapy. Univariate analyses of potential prognostic variables for LRFS were performed. Results The 5-year OS and LRFS rates were 85.4% and 91%. Radiotherapy significantly improved the 5-year LRFS rate ( 100% vs. 89.5% ;x2 = 5.17, P=0.023). However, there was no significant difference in overall survival rate between patients with and without radiotherapy. In univariate analyses, T stage, the number of positive axillary nodes, C-erbB-2 and PR status were the significant predictive factors for LRFS. Conclusions For breast cancer pa-tients with T1-T2. and one to three positive axillary nodes, radiotherapy improves the LRFS, but not OS. T stage, the number of positive axillary nodes, C-erbB-2 and PR status are predictive factors for loco-regional recurrence in patients without radiotherapy.
Persistent Lymphopenia after Chemotherapy And/or Radiotherapy Predicted Unfavorable Outcome in Breast Cancer Patients - a Retrospective Cohort Study

Abstract:Abstract Background Lymphopenia was often observed during follow-up for cancer patients, this study aimed to evaluate the prognostic value of prolonged lymphopenia for breast cancer patients. Methods This was a retrospective cohort analysis of 296 patients with clinical stage I-III operable breast cancer, who had received chemotherapy and/or radiotherapy. Absolute lymphocyte count lower than 1.1×10^3/ul was defined as lymphopenia. Lymphopenia around the 6th month after chemo-radiotherapy was defined as cohort L, while normal Absolute lymphocyte count was defined as cohort N. Predictors of lymphopenia were assessed using univariate and multivariate logistic regression analyses. Kaplan–Meier and Cox regression analyses were used to evaluate the relationship between the two cohorts. Results The baseline characteristics analysis: comparing to cohort N, cohort L had more Stage III(47.4% vs. 25.5%) and less Stage I cancer(12.3% vs. 20.1%)(p = 0.005), more patients experienced combined chemotherapy regimen(86.0% vs. 69.5%, p = 0.012), more patients experienced radiotherapy(78.9% vs. 64.9%, p = 0.041), and less patients experienced endocrine therapy(54.4% vs. 68.2%, p = 0.049). Multivariate logistic regression analysis revealed two negative independent factors associated with DFS: pTNM staging (stage II vs. I: HR = 5.346, 95% CI: 2.286–12.504, p < 0.001; stage III vs. II: HR = 3.089, 95% CI:1.739–5.487, p < 0.001), and ALC status 6 months after chemo- and/or radiotherapy (HR = 2.445, 95% CI: 1.577–3.788, p < 0.001). Cohort L had significantly shorter DFS (log rank: p < 0.001), as well as significantly shorter OS (log rank: p < 0.001). Conclusion Persistent lymphopenia after chemotherapy and/or radiotherapy was an independent predictor for breast cancer relapse and unfavorable survival.
The Locoregional Recurrence Risk and Failure Pattern for Breast Cancer Patients with 1 to 3 Positive Nodes and Treated with Mastectomy and Contemporary Systemic Therapy

Abstract:To evaluate risk factors of locoregional recurrence and role of postmastectomy radiation therapy (PMRT) for early stage intermediate risk (T1-2N1M0) breast cancer, and to analyze failure pattern to define the radiation target. A total of 1,348 patients with T1-2 breast cancer and 1 to 3 positive lymph nodes treated with mastectomy without neoadjuvant chemotherapy during 1999-2010 were retrospectively analyzed. 630 (46.7%) patients were T1 and 709 (52.6%) were T2. 667 (49.5%), 417 (30.9%) and 264 (19.6%) patients have 1, 2 and 3 axillary positive lymph node. The molecular subtypes included luminal A (67.5%), luminal B (10.9%), HER2 positive (5.5%) and triple negative (13.6%). 235 (17.4%) patients received PMRT, 1249 (92.7%) received adjuvant chemotherapy, 958 (73.5%) received adjuvant hormonal therapy, and 39 (2.9%) received adjuvant trastuzumab. Overall survival (OS) and locoregional recurrence rate (LRR) rates were calculated by Kaplan-Meier analysis and compared by log-rank test. Cox logistic regression analysis was performed. The median survival time was 79 months. Overall, the 5-year and 10-year LRFS were 97.0% and 94.8%, and cumulative LRFS were 95.7% and 91.6%, respectively. The 5-year LRFS were not statistically different between patients with or without PMRT (95.9% VS 94.5%, P = 0.338). However, multivariate analysis showed that without PMRT was a significant predictor of LRR (P = 0.037, HR = 2.519, 95% CI 1.058 - 6.000). On Multivariate analysis in patients who did not receive PMRT, significant factors associated with an increased risk of LRR were T2 stage (P = 0.014, HR = 2.146, 95% CI 1.170-3.936), PR negative (P = 0.017, HR = 2.651, 95% CI 1.189-5.911) and percentage of positive lymph node (PLN) >10% (P = 0.026, HR = 0.532, 95% CI 0.305-0.928). Molecular subtype were not prognostic factors for LRR, and only Luminal A showed a trend (P = 0.080) for lower LRR. In patients without PMRT who had 3 risk factors, the 5-year and 10-year LRR was both 15.7%. Supraclavicular region and chest wall were the primary failure pattern of recurrence, with similar risk of 48.1% and 50.0%. In patients with T1-2N1 breast cancer with mastectomy, LRR rates are very low with contemporary systemic therapy. Risk factors of LRR included T2 stage, PR negative and PLN >10%. PMRT should be considered for patients with all 3 risk factors, and supraclavicular region and chest wall should be irradiated. The value of molecular subtype for LRR is not clear, further research on this subject is justified.
Risk Stratification Model for Prediction of Locoregional Recurrence in Patients with Pathologic T1-2N0 Breast Cancer after Mastectomy

Abstract:To identify the risk factors of locoregional recurrence (LR) in patients with pathologic T1-2N0 breast cancer in a real-world and distinguish individuals warrant postmastectomy radiotherapy (PMRT) Patients who had pathologic T1-2N0 breast cancer from 1999 to 2014 were retrospectively reviewed. Locoregional recurrence was defined as reappearance in the ipsilateral chest wall, axilla, infra/supraclavicular fossa, or internal mammary chain. The association of locoregional recurrence with pretreatment factors was evaluated using multi-variable proportional hazards regression. A total of 4841 patients were eligible patients in this study, and all underwent mastectomy plus axillary node dissection without PMRT. 186 LR developed within a median follow-up period for survivors of 56.4 months (range, 1–222 months). Multivariate analysis revealed age < 50yeras old (p<0.001, hazard ratio [HR] =1.795), T2 stage (p=0.008, HR=1.510), negative expressions of estrogen receptor (ER) and progesterone receptor (PR) (p<0.001, HR=1.921), and tumor location in inner quadrant (p<0.001, HR=1.960) were independent predictor of LR. There were 768 patients (15.9%) with no risk factors, 3328 patients (68.7%) with 1-2 risk factors, and 745 patients (15.4%) with 3-4 risk factors. Patients with 0, 1-2, and 3-4 risk factors had 5- and 10-year LR failure of 2.3% and 5.6%, 3.6% and 5.9%, 7.1% and 8.2%, respectively (p < 0.001). 5-year metastasis-free survival and overall survival (OS) were significantly lower for patients with 3-4 factors (89.2% and 93.9%) compared with patients with 1-2 (92.7% and 96.4%) or 0 (97.4% and 99.1%) risk factors. Risk model based on age, T stage, ER/PR and tumor location can stratify patients with pT1-2 N0 breast cancer into subgroups with different LR and outcomes. Patients with 3-4 risk factors may benefit from PMRT.

Abstract:Objective To analyze the role of postmastectomy radiotherapy in different molecular subtypes of breast cancer patients with Stage T1 -T2 and one to three positive axillary nodes. Methods A total of 436 breast cancer patients with T1 -T2 and one to three positive axillary lymph nodes treated with mastectomy and axillary dissection were retrospectively analyzed. Patients were grouped as the following four subtypes:Luminal A, Luminal B, Her2+ and triple-negative. The local recurrence (LR), distant metastasis ( DM ), disease free survival (DFS) and overall survival (OS) rates were compared between paitents with or without radiotherapy in univariate analyses. Multivariate analyses for LR were performed. Results The follow-up rate was 86. 0％. In patients with Luminal A subtype, radiotherapy decreased the 5-year LR rate (4.6％ vs 15.8％ ,x2 =5.74,P=0.017) but had no influences on DM, DFS or OS rates (17.2％ vs 19.7％,x2 =0. 17,P=0.682;77.0％ vs 67. 1％ ,x2 =1.99,P=0. 158 or87.4％:85. 5％ ,x2 =0. 12,P=0. 733 ). Tumor size and radiotherapy were independent prognostic factors for LR rate in multivariate analyses ( x2 =4. 76, P =0. 029 and x2 =8.06, P =0. 005 ). Conclusions For patients with stage T1 -T2 and one to three positive axillary nodes, patients with all molecular subtypes except triple-negative can benefit from postmasteetomy radiotherapy.
Role of radiotherapy and prognostic factors in breast cancer patients at high-risk of recurrence trea-ted with modified radical mastectomy and chemotherapy

Abstract:Objective To analyze the outcome and prognostic factors in breast cancer at high-risk of recurrence and evaluate the role of radiotherapy. Methods 381 breast cancer patients treated with mastec-tomy and axillary dissection were retrospectively analyzed. The including criterias were pathologic diagnosis of invasive breast cancer, T_3-T_4 and/or four or more positive axillary nodes. The survival rates was calculat-ed by Kaplan-Meier method, and compared by Logrank test. Cox regression model was used to select poten-tial prognostic variables. Results The median follow up was 48 months. The 5-year overall survival (OS) and locoregional recurrence-free survival (LRFS) rates were 76.8% and 89.7%, respectively. Radiothera-py significantly improved the OS (80.9% vs. 62.3%, χ~2=15.47, P=0.001) and LRFS (93.4% vs. 77.1% χ~2=19.95,P=0.000). The use of ipsilateral chest wall and supraclavicular nodal radiation was associated with increased 5-year chest wall recurrence free survival (96.8% : 86.2%, χ~2= 12.66, P=0.001) and 5-year supraclavicular node recurrence free survival (97.7% : 90.7 %, χ~2= 9.98, P=0.002).However, axillary irradiation had no impact on 5-year axillary recurrence free survival (98.4% : 96.1% ,χ~2=0.74, P=0.389). In multivariate analysis, absence of radiotherapy (χ~2=14.42, P=0.000), 10 or more positive axillary nodes (χ~2=21.60, P=0.000), and T_4 stage (χ~2=10.79, P=0.001) were inde-pendent unfavorable prognostic factors for overall survival. Conclusions Radiotherapy improves the overall survival of breast cancer patients with T_3, T_4 and/or four or more positive axillary nodes. The chest wall and supraclavicular nodal radiation should be given to this group of patients.
Risk Factors of Locoregional Relapse in Locally Advanced Breast Cancer Treated with Neoadjuvant Chemotherapy Following Mastectomy and Radiotherapy

Abstract:We seek to investigate the prognostic factors that could possibly increase the locoregional recurrence of breast cancer patients who do not achieve pathological complete response after neoadjuvant chemotherapy, and to build a prognostic nomogram to predict patients' outcome. The retrospective analysis included 510 patients who had received neoadjuvant chemotherapy followed by surgery and radiotherapy. 62 locoregional events occurred after a median 61 months of follow-up. The five-year cumulative incidence of local recurrence and regional recurrence were 8.63% and 4.31%, respectively. Multivariate analysis revealed that positivity for ≥ 4 lymph nodes and Ki-67 index ≥ 14% were independent factors. According to our prognostic model, the 5-year locoregional free survival rates in the low, intermediate, and high-risk groups were 95.5%, 89.1%, and 67.1%, respectively (p < 0.001). Annual recurrence curves indicated that the relapse peak after mastectomy emerged in the first 1 year. Positivity for ≥ 4 lymph nodes and Ki-67 index ≥ 14% were independent factors for locoregional recurrence. This prognostic model has considerable clinical value in predicting locoregional recurrence, which could help clinicians to design appropriate locoregional treatment specifically and to perform surveillance individually.

Abstract:Background: Adjuvant radiotherapy in T1-T2 invasive breast carcinoma and 1-3 positive lymph nodes is controversial due to discrepancies in the reported locoregional recurrence risks and survival data. Purpose: This work aims to explore the effectiveness of postoperative radiation therapy in patients with 1-3 positive axillary lymph nodes treated by modified radical mastectomy with systemic chemotherapy with or without hormonal treatment on locoregional recurrence and survival rates. Patients and methods: One hundred and thirteen breast cancer patients with T1 or T2 tumors and 1-3 positive axillary lymph nodes, treated by modified radical mastectomy were registered in Clinical Oncology and Nuclear Medicine Department of Mansoura & Tanta University Hospitals in the period between January 1995 and December 1998. Of the 113 patients, 55 patients received postoperative radiotherapy while 58 patients did not receive it. Results: Postoperative radiotherapy reduced the incidence of locoregional recurrence from 18.2% to 5.2% (P=0.03). Distant metastases was 22.4% in the radiotherapy group of patients in comparison to 40% (P= 0.04). Disease free survival and overall survival were prolonged with postoperative radiotherapy received group (P= 0.03, 0.05) respectively. The risk factors including tumor size, grading and number of dissected axillary lymph nodes were of statistically significant value regarding both locoregional recurrence rates and survivals on the univariate analysis. On multivariate analysis they were statistically significant regarding disease free and overall survivals. Conclusion: Postoperative radiotherapy significantly reduces locoregional recurrence and survival in patients with 1 -3positive axillary lymph nodes. Patients with high risk factors including tumors ≥2cm, grade III and inadequate lymph node dissection are more likely to benefit from radiotherapy. Analysis In Early Stage Triple-Negative Breast Cancer Treated With Mastectomy Without Adjuvant Radiotherapy: Patterns Of Failure And Prognostic Factors

Abstract:BACKGROUND The objective of this study was to evaluate and identify patterns of failure and prognostic factors for locoregional recurrence (LRR) that could justify postmastectomy radiotherapy after modified radical mastectomy in patients with early stage triple-negative breast cancer. METHODS Between January 2000 and July 2007, the authors retrospectively analyzed 390 patients who had triple-negative breast cancer with T1/T2 tumors and from zero to 3 positive lymph nodes (pathologic T1-T2N0-N1) who underwent modified radical mastectomy without postmastectomy radiotherapy at the author's institution. The 5-year cumulative incidence for events was calculated using Kaplan-Meier analysis, and subgroups were compared using the log-rank test. Multivariate analysis was performed using a Cox proportional hazards model. RESULTS Overall, 86.4% of patients received chemotherapy. At a median follow-up of 60.5 months, the 5-year cumulative rates of local recurrence, regional recurrence, LRR, and distant metastasis were 5.4%, 4.7%, 8%, and 13.4%, respectively. On multivariate analysis, age <50 years, the presence of lymphovascular invasion, grade 3 tumor, and 3 involved lymph nodes were associated significantly with an increased risk of LRR. The 5-year LRR rate for patients who had 0 or 1 risk factor, 2 risk factors, and 3 or 4 risk factors was 4.2%, 25.2%, and 81% (P<.0001), respectively. The presence of lymphovascular invasion and having 3 involved lymph nodes were statistically significant predictors of regional recurrence, and the patients who had regional recurrence had a significantly greater risk of distant metastases compared with patients who had local recurrence (59.1% vs 20.9%; P < .0001). CONCLUSIONS Several risk factors were identified in this study that correlated independently with a greater incidence of LRR in patients who had early stage triple-negative breast cancer. The current results indicated that postmastectomy radiotherapy should be considered for those patients who have 2 or more of these factors. Cancer 2013;119:2366-2374. Abstract:Purpose: The role of adjuvant postmastectomy radiotherapy (PMRT) remains controversial for patients with pT3N0M0 breast cancer, especially when patients are treated with the updated adjuvant chemotherapy. Our study aimed to compare locoregional recurrence-free survival (LRFS), disease-free survival (DFS), and breast cancer-specific survival (BCSS) in pT3N0M0 patients with and without postmastectomy radiotherapy. Patients and methods: Between October 2000 and 8 September 2016, the database of the Breast Cancer Center of Shanghai yielded 114 patients with node-negative non-metastatic breast cancer larger than 5 cm. Univariate and multivariate analyses were performed to assess the risk factors for survivals. Differences between the two groups were compared using the Log rank test. Results: Fifty-nine (51.8%) of the patients received adjuvant PMRT. The median follow-up was 62.3 months. Five-year LRFS was 100% in the PMRT group vs 98.1% in the non-PMRT group (P=0.17); 5-year DFS was 97.1% for the entire cohort, 98.0% for the PMRT group vs 96.2% for the non-PMRT group (P=0.18). Univariate analysis identified that family history of malignant tumors, lymphovascular invasion (LVI), or triple-negative breast cancer (TNBC) molecular subtype were associated with higher locoregional recurrence (LRR) (P<0.05). No PMRT was the only risk factor independently associated with poorer DFS (P=0.048) on multivariate analysis. No difference in BCSS was observed between the two groups. Conclusion: The present study demonstrated a low LRR rate and good survival for node-negative breast cancer >5 cm. Patients with family history of malignant tumors, TNBC subtype, LVI positivity, or grade 3 disease are at high risk for LRR and might benefit from PMRT.

Abstract:Purpose: The aim of this study is to evaluate the role of regional nodal irradiation (RNI) in patients with T1-2N1M0 breast cancer and to identify the subgroup that could benefit from RNI. Methods and materials: A total of 4,243 women with pT1-2N1M0 breast cancer treated at two institutions in China were retrospectively reviewed. Survival rates were calculated by the Kaplan–Meier method and compared by the log-rank test. The association of risk factors with survival outcomes was evaluated using multivariable proportional hazards regression. Results: A total of 932 patients (22.0%) received RNI. At a median follow-up of 5.9 years, the 5-year locoregional recurrence (LRR), distant metastasis (DM), disease-free survival (DFS), and overall survival (OS) rates were 4.0% and 7.2% ( P = 0.001), 13.2% and 10.6% ( P = 0.465), 85.0% and 84.7% ( P = 0.131), and 93.9% and 92.8% ( P = 0.004) in the RNI and non-RNI groups, respectively. Multivariate analysis revealed that RNI was an independent prognostic factor for lower LRR ( P = 0.001) and longer DFS ( P = 0.013). Patients were stratified into low-, intermediate-, and high-risk groups based on the eight non-therapeutic risk factors. RNI significantly decreased the 5-year LRR (2.2% vs. 7.0%, P = 0.001) and improved the 5-year DFS (88.8% vs. 84.9%, P = 0.015) and OS (95.8% vs. 93.9%, P = 0.010) in the intermediate-risk group. However, neither the low-risk group nor the high-risk group had survival benefit from RNI. Conclusion: T1-2N1M0 breast cancer is a heterogeneous disease. We found that RNI only improved survival in the intermediate-risk group. It might be omitted in low-risk patients, and the role of RNI in high-risk patients needs further study.

Abstract:Purpose Postmastectomy radiation therapy (PMRT) in T1–T2 tumors with 1–3 positive axillary lymph nodes (ALNs) is controversial. This study was to identify prognostic factors of locoregional control (LRC) following mastectomy with or without PMRT for patients with T1-2N1 breast cancer and to discuss the selection of patients who might omit PMRT. Materials and methods Between January 2006 and December 2012, the data of 1474 postmastectomy patients staged pT1-2N1 were analyzed. PMRT was applied in 663 patients. LRC and disease-free survival (DFS) were calculated using the Kaplan–Meier method. Cox regression model was applied in the univariate and multivariate analyses to recognize the recurrence risk factors. Results With the median follow-up duration of 93 months (range, 5–168 months), 78 patients (5.3%) failed to secure LRC and 220 patients (14.9%) experienced any recurrence. The 7.7-year LRC and DFS was 94.9% and 85.4% respectively in the entire cohort. PMRT significantly improved 7.7-year LRC from 93.4% to 96.6% ( p = 0.005), but not the DFS ( p = 0.335). Multivariate analysis revealed that PMRT was an independent prognostic factor of LRC ( p < 0.001), meanwhile, age ≤ 40 years ( p = 0.012), histological grade 3 ( p = 0.004), 2–3 positive nodes ( p < 0.001) and tumor size of 3–5 cm ( p = 0.045) were significantly associated with decreased LRC. The 7.7-year LRC for patients with 0, 1, and 2–4 risk factors was 97.7% / 98.9% ( p = 0.233), 95.3% / 98.0% ( p = 0.092), and 80.3% / 94.8% ( p < 0.001) in the non-PMRT and PMRT group, respectively. Conclusions In patients with T1-2N1 breast cancer, clinical-pathological factors including young age, histological grade 3, 2–3 positive nodes, and tumor size of 3–5 cm were identified to be predictors of a poorer LRC following mastectomy. Patients with 0–1 risk factor might consider the omission of PMRT.
Postmastectomy Radiotherapy Improves Disease-Free Survival of High Risk of Locoregional Recurrence Breast Cancer Patients with T1-2 and 1 to 3 Positive Nodes

Abstract:Objectives The indications for post-mastectomy radiotherapy (PMRT) with T1-2 breast cancer and 1-3 positive axillary lymph nodes is still controversial. The purpose of this study was to investigate the role of PMRT in T1-2 breast cancer with 1-3 positive axillary lymph node. Methods We retrospectively reviewed the file records of 79 patients receiving PMRT and not receiving PMRT (618 patients). Results The median follow-up was 65 months. Multivariate analysis showed that PMRT was an independent prognostic factor of locoregional recurrence-free survival (LRFS) (P = 0.010). Subgroup analysis of patients who did not undergo PMRT showed that pT stage, number of positive axillary lymph nodes, and molecular subtype were independent prognostic factors of LRFS. PMRT improved LRFS in the entire group (P = 0.005), but did not affect distant metastasis-free survival (DMFS) (P = 0.494), disease-free survival (DFS) (P = 0.215), and overall survival (OS) (P = 0.645). For patients without PMRT, the 5-year LRFS of low-risk patients (0–1 risk factor for locoregional recurrence) of 94.5% was significantly higher than that of high-risk patients (2-3 risk factors for locoregional recurrence) (80.9%, P < 0.001). PMRT improved LRFS (P = 0.001) and DFS (P = 0.027) in high-risk patients, but did not improve LRFS, DMFS, DFS, and OS in low-risk patients. Conclusions PMRT is beneficial in patients with high risk of locoregional recurrence breast cancer patients with T1-2 and 1 to 3 positive nodes.

Abstract:Patients with isolated RR after mastectomy have an optimistic OS but a high risk of sLRR after RR in the contemporary era. Comprehensive locoregional treatment including surgery and RT combined with systemic therapy is recommended. The CW, axillary and IM regions should not be routinely included in the radiation target volume.

Abstract:To establish a risk scoring system for predicting locoregional recurrence (LRR) and explore the potential value of radiotherapy in T1 to T2 node-negative breast cancer patients treated with mastectomy. From January 2001 to February 2008, a total of 353 node-negative T1 to T2 breast cancer cases treated with mastectomy without adjuvant radiotherapy were retrospectively analyzed. Preliminary screening of the prognostic factors was accomplished by Kaplan-Meier univariate analysis, and survival curves between different groups were compared by log-rank test. Risk factors were determined using Cox proportional hazards model. A categorical risk scoring system was generated according to the Cox model, weighing the relative importance of each risk variable. Median follow-up was 115.7 months (range, 1.2-238.4 months). The overall 5-year locoregional recurrence-free survival (LRFS) was 89.8% (95% confidence interval [CI]=86.7%-92.9%). Chest wall (53.8%) was found to be the most common site of LRR, followed by supraclavicular nodes (48.7%). Age 40 years, primary tumor size 4.5cm and number of nodes resected 10 were found to be independent factors for poor prognosis of LRR. Two risk stratifications based on the scoring system were subsequently obtained. The 5-year LRFS was 91.6% (95% CI=88.5%-94.7%) with low risk (score <2) and 75.7% (95% CI=61.8%-89.6%) with high risk (score 2), respectively ((2)=7.544, P=.006). In addition, significant differences in overall survival (P=.045) and disease-free survival (P=.019) were presented between them. Patients with T1-2N0M0 breast cancer achieved favorable prognosis in general. Those with risk factors, including age 40 years, primary tumor size 4.5cm and number of nodes resected 10, were at higher risk of LRR. The established scoring system could help to distinguish the subgroups that might potentially benefit from postoperative radiotherapy.

This study was to identify prognostic clinical-pathological factors of locoregional control (LRC) following mastectomy and axillary dissection with or without adjuvant RT for patients with T1-2N1 breast cancer, and to discuss the selection of patients who might not benefit from PMRT. Between January 2006 and December 2010, the data of 839 post-mastectomy patients staged pT1-2N1M0 were analyzed. Radiotherapy was applied in 483 patients. LRC was assessed using the Kaplan-Meier method and log-rank test. At a median follow-up of 87 months (range, 1-167months), 47 (5.6%) patients developed locoregional recurrence (LRR). The 7-year LRC was 94.2% and 94.3% in the non-RT group and RT group respectively (P > .05). Multivariate analysis revealed that RT was an independent prognostic factor of LRC (HR, 0.46; 95% CI, 0.24-0.89; P = .02), meanwhile, age < 40 years, histological grade 3 tumor, and 3 involved lymph nodes were associated significantly with decreased LRC. The 7-year LRC for patients with 0, 1, and 2 or more risk factors was 97.3% / 95.7% (P = .97), 92.7% / 96.9% (P = .04), and 72.9% / 87.2% (P = .18) in the non-RT and RT group respectively. In patients with T1-2N1 breast cancer, clinical-pathological factors such as adjuvant RT, old age, grade 1-2, and 1-2 involved nodes were identified to be prognostic of decreased risk of LRR following mastectomy, and might be helpful in selecting candidates for whom adjuvant RT could be omitted.

Abstract:Purpose. Several studies have demonstrated poor locoregional control in patients with triple-negative breast cancer (TNBC), compared with other molecular subtypes of breast cancer. We sought to evaluate whether or not postmastectomy radiotherapy (PMRT) improves locoregional recurrence-free survival (LRFS) and disease-free survival (DFS) outcomes in TNBC patients.Methods and Materials. Between January 2000 and July 2007, 553 TNBC patients treated with modified radical mastectomy from a single institution were analyzed retrospectively. Patients were categorized into three groups: low risk (stage T1-T2N0), intermediate risk (stage T1-T2N1), and high risk (stage T3-T4 and/or N2-N3). Cox proportional hazards models were used to evaluate the association between PMRT and LRFS and DFS times after adjusting for other clinicopathologic covariates.Results. With a median follow-up of 65 months (range, 1-140 months), 51 patients (9.2%) developed locoregional recurrence and 135 patients (24.4%) experienced disease recurrence. On multivariate analysis, PMRT was associated with significantly longer LRFS and DFS times in the entire cohort. In the intermediate-risk group, PMRT was associated with a longer DFS time but not with the LRFS interval. In the high-risk group, PMRT was associated with significantly longer LRFS and DFS times.Conclusion. PMRT is associated with longer LRFS and DFS times in high-risk TNBC patients and a longer DFS time in intermediate-risk TNBC patients. Abstract:The purpose of this study was to investigate the value of post‐operative radiotherapy in the treatment of pT3N0M0 breast cancer after mastectomy. We analyzed the clinical data of 1390 patients with pT1‐3N0M0 breast cancer who were admitted and treated from 1998 to 2007 at the Sun Yat‐sen University Cancer Center. All patients underwent mastectomy and did not receive radiotherapy. The locoregional recurrence‐free survival, distant metastasis‐free survival and overall survival of different T stages of breast cancer were compared. The median follow‐up duration was 72 months. The 10‐year locoregional recurrence‐free survival patients with pT1N0, pT2N0 and pT3N0 breast cancers were 95.3, 91.9 and 93.6%, respectively (χ2 = 2.550, P = 0.279). The 10‐year distant metastasis‐free survival rates of patients with pT1N0, pT2N0 and pT3N0 breast cancers were 88.1%, 81.0% and 78.4%, respectively (χ2 = 8.254, P = 0.016). The 10‐year overall survival rates of patients with pT1N0, pT2N0 and pT3N0 breast cancers were 91.9%, 83.5% and 73.0%, respectively (χ2 = 12.403, P = 0.002). Univariate analyses failed to identify any prognostic factors for locoregional recurrence in pT3N0 patients. Multivariate analysis showed that the T stage had no effect on locoregional recurrence. The locoregional recurrence rate in patients with pT3N0M0 breast cancer who underwent mastectomy and did not receive postoperative radiotherapy was not higher than that in patients with pT1‐2N0M0 breast cancer who received the same treatment, suggesting that routine adjuvant post‐operative radiotherapy should not be recommended in this patient population.
