Abstract:Introduction: Immediate inhalation of hydrogen (H 2 ) has been demonstrated to improve post-resuscitation (PR) neurological dysfunction. However, early applying inhaled H 2 is not practical especially in out of hospital cardiac arrest because of the safety issue. In the present study, we investigated the effects of delayed treatments with H 2 on post resuscitation neurological function in a porcine model of prolonged cardiac arrest (CA). Hypothesis: Delayed inhalation of H 2 mitigates neurological dysfunction after resuscitation in a CA porcine model. Methods: Eighteen male domestic pigs weighing 39±2 kg were utilized. Ventricular fibrillation (VF) was induced electrically and CPR was initiated after 10 mins of untreated VF. All the animals were resuscitated successfully and randomized into two groups immediately following resuscitation: delayed inhalation of H 2 (DH group, N=9) or inhalation of room air (C group, N=9). Animals in the DH group were ventilated with 2% H 2 /21% oxygen from 2h post resuscitation (PR) to PR 4h. Serum levels of S100B and neuron-specific enolase (NSE) were measured by ELISA at baseline and PR 360 min. The neurological deficit score (NDS) and survival were evaluated daily for a total of 72 hours. Results: Better survival duration and NDS at 24h, 48h and 72h were observed in the DH group animals compared with those of the C group (Table). The serum levels of S100B and NSE were significantly lower in the DH group when compared with the C group (Figure). Conclusion: Delayed treatment of H 2 mitigates neurological injury and improves neurological dysfunction after resuscitation.

Serum levels of troponin T (Tn T) and N-terminal probrain natriuretic peptide (NTpro-BNP) were measured by ELISA at BL, PR 180 min and PR 360 min. Results: Deteriorations in maximum rate of LVP increase (dp/dtmax), maximum rate of LVP decline (-dp/dtmax) and CO were observed in all the animals after PR 4 h (Figure 1). However, better dp/dtmax, -dp/dtmax and CO were achieved in the DH group after PR 5h when compared with the C group. Although levels of both Tn T and NTpro-BNP in serum increased after resuscitation, they were significantly lower in animals of DH group in comparison with C group (Figure 2). Conclusion: Delayed treatment of H 2 attenuates myocardial injury and improves myocardial dysfunction after resuscitation.

Hypothesis: Delayed inhalation of H 2 reduces proinflammatory cytokines but without effect on oxidative stress after resuscitation in a CA porcine model. Methods: Twenty male domestic pigs weighing 39±2 kg were studied. Ventricular fibrillation (VF) was induced electrically and CPR was initiated after 10 min of untreated VF. Animals were randomized into two groups immediately after successfully resuscitation: delayed inhalation of H 2 (DH group) or continuous inhalation of room air (C group). DH group animals were ventilated with 2% H 2 /21% oxygen from post-resuscitation 120 min (PR120) until PR240 min. Serum levels of oxidative product (8-iso-PGF 2α ) and proinflammatory cytokines (tumor necrosis factor-α [TNF-α], interleukin-6 [IL-6], and high-mobility group box protein 1 [HMGB1]) were measured by ELISA at baseline (BL), PR30, PR180 and PR360. Results: Serum levels of 8-iso-PGF 2α in all the animals were significantly increased at PR30 then decreased to the levels of BL after PR180. The levels of TNF-α, IL-6 and HMGB1 in serum of all animals were elevated after PR180. Although no significant differences in the levels of 8-iso-PGF 2α were observed between both groups, the levels of TNF-α, IL-6 and HMGB1 were significantly lower in DH group animals when compared with those in C group (Figure). Conclusions: Delayed treatment with H2 attenuates proinflammatory cytokines but without effect on oxidative stress conditions after resuscitation.
Inhalation of hydrogen improves myocardial function and survival outcomes in a porcine model of prolonged cardiac arrest

Abstract:Objective To investigate the beneficial effect of hydrogen(H2)on myocardial dysfunction after cardiopulmonary resuscitation (CPR) in a pig model with a prolonged cardiac arrest. Methods Healthy domestic pigs were subjected to 10 minutes of ventricular fibrillation cardiac arrest induced electrically and followed by CPR. After achieving the return of spontaneous circulation (ROSC), the post-cardiac arrest pigs were randomized into two groups: mechanically ventilated with room air (control group); mechanically ventilated with 2% H2(H2∶O2∶N2=2%∶21%∶77%)for 2 hours after ROSC and turned to ventilate with room air (H2 group). Hemodynamic parameters including HR, MAP, left ventricular pressure derivative (±dp/dtmax) and cardiac index (CI) were recorded for 6 hours continually after successful resuscitation. Arterial blood gas, lactate and serum marker of myocardial injury (cTnT and NT-pro-BNP) were measured at baseline, and at 0.5, 2, 4 and 6 hours after CPR. The survival outcomes of animals were evaluated and recorded until 96 hours after CPR. Results Worse post-resuscitation myocardial function was found in both groups compared to the status prior to CA, but the myocardial function of the H2 group was superior to that of the control group. The indicators of myocardial function in the H2 group especially at 5 hours and 6 hours after CPR [+ dp/dtmax: (3320.1±433.4)mmHg/s and (3294.5±390.4)mmHg/s; -dp/dtmax: (3756.1±498.8)mmHg/s and (3667.4±406.8)mmHg/s; CI: (6.8±1.0) L/min·m2 and(6.7±1.2)L/min·m2] were much better than those of the control group at the same time points [+ dp/dtmax: (2863.5±340.8)mmHg/s and(2925.2±251.3) mmHg/s; -dp/dtmax: (3245.4±259.6)mmHg/s and (3261.9±311)mmHg/s; CI: (5.3±1.5)L/min·m2 and(5.1±1.4)L/min·m2](all P<0.05). At 4 hours and 6 hours after CPR, the serum levels of cTnT and NT-pro-BNP in H2 group [cTnT: (2.7±0.9)ng/mL and (2.7±0.8)ng/mL; NT-pro-BNP: (0.34±0.13)ng/mL and(0.37±0.17)ng/mL] were obviously lower compared to the control group [cTnT: (4.9±1.2)ng/mL and (5.8±1.2)ng/mL; NT-pro-BNP: (0.54±0.12)ng/mL and(0.78±0.21)ng/mL](all P<0.05). In addition, greater survival duration was also observed in H2 group compared to the control group [96(25, 96)hours vs. 16(12, 22)hours, P<0.05]. Conclusion Inhalation of H2 improves post-resuscitation myocardial dysfunction and survival outcomes in a porcine model with prolonged cardiac arrest. Hydrogen Inhalation is Superior to Mild Hypothermia in Improving Cardiac Function and Neurological Outcome in an Asphyxial Cardiac Arrest Model of Rats.

Abstract:BACKGROUND:Non-shockable rhythms represent an increasing proportion of reported cases of out-of-hospital cardiac arrest but with an associated poor prognosis. In the present study, we investigated the effects of hydrogen inhalation on cardiac and neurological function after cardiopulmonary resuscitation and compared the therapeutic benefit with hypothermia in an asphyxial rat model of cardiac arrest.METHODS:Cardiopulmonary resuscitation was initiated after 5 min of untreated asphyxial cardiac arrest. Animals were randomly assigned to three experimental groups immediately after successful resuscitation: ventilation with 2% hydrogen/98% oxygen under normothermia (H2 inhalation), ventilation with 2% nitrogen/98% oxygen under normothermia (Control), and ventilation with 2% nitrogen/98% oxygen under hypothermia (TH). Mixed gas inhalation continued for 1 h while hypothermia continued for 2 h. Animals were observed up to 96 h for assessment of survival and neurologic recovery.RESULTS:No statistical differences in baseline measurements were observed among groups and all the animals were successfully resuscitated. Serum cardiac troponin T and S100B measured during earlier post-resuscitation period were markedly reduced in both H2 inhalation and hypothermic groups. However, significantly better left ventricular ejection fraction, cardiac work, and neurological deficit score were observed in the H2 inhalation group. Ninety-six hours survival rate was significantly higher in the H2 inhalation group (75.0%), either compared with TH (45.8%) or compared with Control (33.3%). But there was no statistical difference between TH and Control.CONCLUSIONS:Small amounts of inhaled hydrogen were superior to mild hypothermia in improving cardiac function and neurological outcome in this asphyxial rat model of cardiac arrest.
Remote Ischemic Postconditioning Alone and Combined with Therapeutic Hypothermia Improve Neurological Outcomes after Cardiopulmonary Resuscitation in A Porcine Model

Abstract:Introduction: The degree of post-resuscitation cerebral injury is an important determinant on the outcomes for CA victims.We previously demonstrated remote ischemic pre- and post-conditioning equally attenuated post-CPR cerebral dysfunction in a rat model. Here we investigated the effects of remote ischemic post-conditioning (RIpostC) alone and combined with therapeutic hypothermia (TH) on the neurological outcomes after CPR. Hypothesis: RIpostC would improve post-CPR neurological function and cerebral injury, which would be enhanced by its combination with TH. Methods: Twenty-one male domestic pigs weighing 37 ± 2 kg were subjected to 10 mins of untreated VF followed by 5 mins of CPR. The animals were randomized to RIpostC, RIpostC+TH or control. Coincident with the start of CPR, RIpostC was induced by four cycles of 5 mins of limb ischemia followed by 5 mins of reperfusion. At 5 mins after resuscitation, TH was implemented by surface cooling to reach a temperature of 32-34 °C until 4 hrs post-resuscitation, followed by a rewarming rate of 1°C/h for 4 hrs. The resuscitated animals were observed for 72 hrs. Results: Six of the seven animals in each group were successfully resuscitated. Significantly better neurological function was observed in the RIpostC group than in the control group. Surprisingly the combined RIpostC and TH further improved post-CPR neurological function, which was significantly better than that in the RIpostC and control groups. There was less cerebral injury in the animals that received RIpostC alone and combined with TH; which was significantly better at 48 and 72 hrs post-resuscitation when compared to the control group. Furthermore, significantly less cerebral injury was measured at 72 hrs in the combination group than in the RIpostC group (Table). Conclusion: In a porcine model of CA, RIpostC significantly improved post-resuscitation neurological function and cerebral injury. Its combination with TH further enhanced its protective effects

Abstract:Objective: Hydrogen gas has been demonstrated to neutralize free radicals and reduce oxidative stress recently. Our objective was to determine the therapeutic effect of H, inhalation and its antioxidative activity on early brain injury after subarachnoid hemorrhage. Design: Controlled in vivo laboratory study. Setting: Animal research laboratory. Subjects: One hundred thirty-seven adult male Sprague-Dawley rats weighing 280-350 g. Interventions: Subarachnoid hemorrhage was induced by endovascular perforation method in rats. Subarachnoid hemorrhage rats were treated with 2.9% hydrogen gas inhaled for 2 hrs after perforation. At 24 and 72 hrs, mortality, body weight, neurologic deficits, and brain water content were assessed. Blood brain barrier permeability and apoptosis were also measured at 24 hrs. To investigate the antioxidative activity of hydrogen gas, the expression of malondialdehyde, nitrotyrosine, and 8-hydroxyguanosine, which are oxidative markers of lipid, protein, and DNA damage, respectively, were measured at 24 hrs. Measurements and Main Results: Hydrogen gas significantly alleviated brain edema and blood brain barrier disruption, reduced apoptosis, and improved neurologic function at 24 hrs but not 72 hrs after subarachnoid hemorrhage. These effects were associated with the amelioration of oxidative injury of lipid, protein, and DNA. Conclusions: Hydrogen gas could exert its neuroprotective effect against early brain injury after subarachnoid hemorrhage by its antioxidative activity. (Crit Care Med 2012; 40:1291-1296)
Hydrogen-rich saline improves survival and neurological outcome after cardiac arrest and cardiopulmonary resuscitation in rats.

Abstract:BACKGROUND:Sudden cardiac arrest is a leading cause of death worldwide. Three-fourths of cardiac arrest patients die before hospital discharge or experience significant neurological damage. Hydrogen-rich saline, a portable, easily administered, and safe means of delivering hydrogen gas, can exert organ-protective effects through regulating oxidative stress, inflammation, and apoptosis. We designed this study to investigate whether hydrogen-rich saline treatment could improve survival and neurological outcome after cardiac arrest and cardiopulmonary resuscitation, and the mechanism responsible for this effect. METHODS:Sprague-Dawley rats were subjected to 8 minutes of cardiac arrest by asphyxia. Different doses of hydrogen-rich saline or normal saline were administered IV at 1 minute before cardiopulmonary resuscitation, followed by injections at 6 and 12 hours after restoration of spontaneous circulation, respectively. We assessed survival, neurological outcome, oxidative stress, inflammation biomarkers, and apoptosis. RESULTS:Hydrogen-rich saline treatment dose dependently improved survival and neurological function after cardiac arrest/resuscitation. Moreover, hydrogen-rich saline treatment dose dependently ameliorated brain injury after cardiac arrest/resuscitation, which was characterized by the increase of survival neurons in hippocampus CA1, reduction of brain edema in cortex and hippocampus, preservation of blood-brain barrier integrity, as well as the decrease of serum S100β and neuron-specific enolase. Furthermore, we found that the beneficial effects of hydrogen-rich saline treatment were associated with decreased levels of oxidative products (8-iso-prostaglandin F2α and malondialdehyde) and inflammatory cytokines (tumor necrosis factor-α, interleukin-1β, and high-mobility group box protein 1), as well as the increased activity of antioxidant enzymes (superoxide dismutase and catalase) in serum and brain tissues. In addition, hydrogen-rich saline treatment reduced caspase-3 activity in cortex and hippocampus after cardiac arrest/resuscitation. CONCLUSIONS:Hydrogen-rich saline treatment improved survival and neurological outcome after cardiac arrest/resuscitation in rats, which was partially mediated by reducing oxidative stress, inflammation, and apoptosis.

Abstract:BACKGROUND:Heme oxygenase 1 (HO-1) has been shown to attenuate neuronal injury. Therefore, the authors examined whether HO-1 would reduce the brain damage caused by cardiac arrest.METHODS:Rats anesthetized with halothane were subjected to 8 min of cardiac arrest by asphyxia without any pretreatment (control group) or pretreated with an inducer of HO-1 (hemin group) or with an inhibitor of HO-1 (tin protoporphyrin group). Then, the animals were resuscitated with a standardized method. Brain water content (1 and 6 h after resuscitation), neurologic deficit score, viable neuronal counts, and caspase-3 immunostaining in the hippocampus (4 and 14 days) were evaluated.RESULTS:Water content in the hemin group was significantly reduced (mean +/- SD: 83.1 +/- 1.9% for 1 h after resuscitation; P = 0.03) and significantly greater in the tin protoporphyrin group (91.1 +/- 2.0% for 1 h after resuscitation; P = 0.035) when compared with the control group (88.2 +/- 2.4%). Water content of the cortex was nearly the same as that of the hippocampus. Neurologic deficit scores and neuronal survival were significantly better in the hemin group than in the control group on the 4th and 14th days. In rats that survived for 4 days, the amount of caspase 3-positive neurons was 27 +/- 7 in the control group, whereas the value was 14 +/- 6 in the hemin group (P < 0.05).CONCLUSIONS:In rats resuscitated from cardiac arrest, induction of HO-1 by hemin reduced brain edema, improved neurologic outcome, and protected neurons against apoptosis.
Hydrogen inhalation ameliorated mast cell-mediated brain injury after intracerebral hemorrhage in mice

Abstract:Objective: Hydrogen inhalation was neuroprotective in several brain injury models. Its mechanisms are believed to be related to antioxidative stress. We investigated the potential neurovascular protective effect of hydrogen inhalation especially effect on mast cell activation in a mouse model of intracerebral hemorrhage. Design: Controlled in vivo laboratory study. Setting: Animal research laboratory. Subjects: One hundred seventy-one 8-week-old male CD-1 mice were used. Interventions: Collagenase-induced intracerebral hemorrhage model in 8-week-old male CD-1 mice was used. Hydrogen was administrated via spontaneous inhalation. The blood-brain barrier permeability and neurologic deficits were investigated at 24 and 72 hours after intracerebral hemorrhage. Mast cell activation was evaluated by Western blot and immuno-staining. The effects of hydrogen inhalation on mast cell activation were confirmed in an autologous blood injection model intracerebral hemorrhage. Measurement and main results: At 24 and 72 hours post intracerebral hemorrhage, animals showed blood-brain barrier disruption, brain edema, and neurologic deficits, accompanied with phosphorylation of Lyn kinase and release of tryptase, indicating mast cell activation. Hydrogen treatment diminished phosphorylation of Lyn kinase and release of tryptase, decreased accumulation and degranulation of mast cells, attenuated blood-brain barrier disruption, and improved neurobehavioral function. Conclusion: Activation of mast cells following intracerebral hemorrhage contributed to increase of blood-brain barrier permeability and brain edema. Hydrogen inhalation preserved blood-brain barrier disruption by prevention of mast cell activation after intracerebral hemorrhage.

Abstract:Objective: Generation of negative intrathoracic pressure during the decompression phase of cardiopulmonary resuscitation enhances the refilling of the heart. We tested the hypothesis that when compared with closed-chest manual compressions at 80 chest compressions per min, treatment with active compression-decompression cardiopulmonary resuscitation at 80 chest compressions/min combined with augmentation of negative intrathoracic pressure would lower intracranial pressure and increase cerebral perfusion, thereby improving neurologically intact survival rates following prolonged untreated cardiac arrest. Design: Prospective, randomized animal study. Setting: Animal laboratory facilities. Subjects: A total of 26 female farm pigs in two different protocols (n = 17 and n = 9). Interventions, Measurements, and Main Results: Seventeen pigs were subjected to 8.5 mins of untreated ventricular fibrillation and prospectively randomized to cardiopulmonary resuscitation at 80 chest compressions/min or active compression-decompression cardiopulmonary resuscitation at 80 chest compressions/min plus an impedance threshold device. Coronary perfusion pressures (29.5 +/- 2.7 mm Hg vs. 22.4 +/- 1.6 mm Hg, p = .03), carotid blood flow (44.0 +/- 12.2 vs. 30.9 +/- 10.4, p = .03), and 24-hr neurological survival (88% vs. 22%, p = .015) were higher with active compression-decompression cardiopulmonary resuscitation + an impedance threshold device. Cerebral perfusion pressures, measured in nine additional pigs, were improved with active compression-decompression cardiopulmonary resuscitation + an impedance threshold device (21.9 +/- 1.2mm Hg vs. 8.9 +/- 0.8 mm Hg, p < .0001). With active compression-decompression cardiopulmonary resuscitation + impedance threshold device, mean diastolic intracranial pressure during decompression was lower (12.2 +/- 0.2 mm Hg vs. 16.6 +/- 1.2 mm Hg, p = .02) and the downward slope of the decompression phase intracranial pressure curve was steeper (-60.3 +/- 12.9 mm Hg vs. -46.7 +/- 11.1 mm Hg/sec, p < .001). Conclusions: Active compression-decompression cardiopulmonary resuscitation + an impedance threshold device increased cerebral perfusion pressures and lowered diastolic intracranial pressure and intracranial pressure rate during the decompression phase. These mechanisms may underlie the observed increase in cerebral perfusion pressure, carotid blood flow, and survival rates with favorable neurologic outcomes in this pig model of cardiac arrest. Abstract:BACKGROUND/AIMS:Hydrogen selectively neutralizes reactive oxygen species (ROS) and ameliorates various ROS-induced injuries. Spinal cord injury (SCI) is a serious injury to the central nervous system, and secondary SCI is closely related to excessive ROS generation. We hypothesized that hydrogen inhalation ameliorates SCI, and the mechanism of action may be related to the protective effects of hydrogen against oxidative stress, apoptosis, and mitochondrial damage.METHODS:Mechanically injured spinal cord neurons were incubated with different concentrations of hydrogen in vitro. Immunofluorescence staining and transmission electron microscopy were used to confirm the protective effects of hydrogen. ROS and related proteins were detected with dihydroethidium fluorescence staining, enzyme-linked immunosorbent assays, and western blotting. Terminal deoxynucleotidyl transferase dUTP nick end labeling assays, flow cytometry, and western blotting were used to detect neuronal apoptosis. ATP concentrations, Janus Green B staining, and mitochondrial permeability transition pore (mPTP) status were assessed to investigate mitochondrial damage. RNA sequencing was performed to screen potential target genes of hydrogen application. Hydrogen was administered to mice after spinal cord contusion injury was established for 42 days. The Basso Mouse Scale (BMS) and footprint analyses were used to assess locomotor functions, and immunofluorescence staining of the injured spinal cord segments was performed to detect oxidative stress status.RESULTS:Spinal cord neurons were preserved by hydrogen administration after mechanical injury in a dose-dependent manner. ROS generation, oxidative stress injury-related markers, and the number of apoptotic neurons were significantly reduced after hydrogen treatment. The ATP production and mPTP function in injured neurons were preserved by hydrogen incubation. The expression levels of Cox8b, Cox6a2, Cox7a1, Hspb7, and Atp2a1 were inhibited by hydrogen treatment. BMS scores and the footprint assessment of mice with SCI were improved by hydrogen inhalation.CONCLUSIONS:Hydrogen inhalation (75%) ameliorated SCI in vivo and attenuated neuronal mechanical injuries in vitro, and its protective effect on spinal cord neurons was exerted in a dose-dependent manner. The underlying mechanisms included reducing ROS generation and oxidative stress, inhibiting neuronal apoptosis, and restoring mitochondrial construction and function. Cox8b, Cox6a2, Cox7a1, Hspb7, and Atp2a1 were identified as potential target genes of hydrogen treatment.

Abstract:Abstract Molecular hydrogen (H 2 ) protect neurons against reactive oxygen species and ameliorates early brain injury (EBI) after subarachnoid hemorrhage (SAH). This study investigated the effect of H 2 on delayed brain injury (DBI) using the rat SAH + unilateral common carotid artery occlusion (UCCAO) model with the endovascular perforation method. 1.3% H 2 gas (1.3% hydrogen premixed with 30% oxygen and balanced nitrogen) inhalation was performed on days 0 and 1, starting from anesthesia induction and continuing for 2 h on day 0, and starting from anesthesia induction and continuing for 30 min on day 1. EBI was assessed on the basis of brain edema, expression of S100 calcium-binding protein B (S100B), and phosphorylation of C-Jun N-terminal kinase on day 2, and neurological deficits on day 3. Reactive astrogliosis and severity of cerebral vasospasm (CV) were assessed on days 3 and 7. DBI was assessed on the basis of neurological deficits and neuronal cell death on day 7. EBI, reactive astrogliosis, and DBI were ameliorated in the H 2 group compared with the control group. CV showed no significant improvement between the control and H 2 groups. This study demonstrated that H 2 gas inhalation ameliorated DBI by reducing EBI without improving CV in the rat SAH + UCCAO model.

Abstract:Mitochondrial dysfunction plays a critical role in brain injury after cardiac arrest and cardiopulmonary resuscitation (CPR). Recent studies demonstrated that hydrogen sulfide (H2S) donor compounds preserve mitochondrial morphology and function during ischemia-reperfusion injury. In this study, we sought to explore the effects of sodium hydrosulfide (NaHS) on brain mitochondria 24h after cardiac arrest and resuscitation. Male Sprague-Dawley rats were subjected to 6min cardiac arrest and then resuscitated successfully. Rats received NaHS (0.5mg/kg) or vehicle (0.9% NaCl, 1.67ml/kg) 1min before the start of CPR intravenously, followed by a continuous infusion of NaHS (1.5mg/kg/h) or vehicle (5ml/kg/h) for 3h. Neurological deficit was evaluated 24h after resuscitation and then cortex was collected for assessments. As a result, we found that rats treated with NaHS revealed an improved neurological outcome and cortex mitochondrial morphology 24h after resuscitation. We also observed that NaHS therapy reduced intracellular reactive oxygen species generation and calcium overload, inhibited mitochondrial permeability transition pores, preserved mitochondrial membrane potential, elevated ATP level and ameliorated the cytochrome c abnormal distribution. Further studies indicated that NaHS administration increased mitochondrial biogenesis in cortex at the same time. Our findings suggested that administration of NaHS 1min prior CPR and followed by a continuous infusion ameliorated neurological dysfunction 24h after resuscitation, possibly through mitochondria preservation as well as by promoting mitochondrial biogenesis.

Abstract:Deep hypothermic circulatory arrest (DHCA) has been widely used in the operations involving the aortic arch and brain aneurysm since 1950s; but prolonged DHCA contributes significantly to neurological deficit which remains a major cause of postoperative morbidity and mortality. It has been reported that hydrogen exerts a therapeutic antioxidant activity by selectively reducing hydroxyl radical. In this study, DHCA treated rats developed a significant oxidative stress, inflammatory reaction and apoptosis. The administration of HRS resulted in a significant decrease in the brain injury, together with lower production of IL-1β, TNF-α, 8-OHdG and MDA as well as decreased activity of NOS while increased activity of SOD. The apoptotic index as well as the expressions of caspase-3 in brain tissue was significantly decreased after treatment. HRS administration significantly attenuated the severity of DHCA induced brain injury by mechanisms involving amelioration of oxidative stress, down-regulation of inflammatory factors and reduction of apoptosis.
Hydrogen Inhalation Improves Mouse Neurological Outcomes after Cerebral Ischemia/reperfusion Independent of Anti-Necroptosis

Abstract:This study aimed to investigate the role of necroptosis in the neuroprotection of hydrogen in a mouse model of cerebral ischemia/reperfusion (I/R) injury. C57BL mice were randomly divided into sham group, I/R group, hydrogen/oxygen group (HO), nitrogen/oxygen group (NO). Middle cerebral artery occlusion (MCAO) for 1 hour followed by reperfusion was introduced to animals which were allowed to inhale 66.7% hydrogen/33.3% oxygen for 90 minutes since the beginning of reperfusion. Mice in NO group inhaled 66.7% nitrogen/33.3% oxygen. 24 hours after MCAO, brain infarction, brain water content and neurological function were evaluated. The protein expression of mixed lineage kinase domain like protein (MLKL) was detected at 3, 6, 12, 24 and 72 hours after reperfusion in HO group and the protein and mRNA expression of MLKL at 24 hours after MCAO in four groups. Hydrogen inhalation significantly reduced infarct volume, attenuated brain edema and improved neurobehavioral deficit in MCAO mice. The MLKL expression increased after MCAO and peaked at 6–24 hours after reperfusion. However, hydrogen inhalation had no significant effect on the MLKL expression at transcriptional and translational levels after MCAO. This study indicates high concentration hydrogen improves mouse neurological outcome after cerebral I/R injury independent of anti-necroptosis.
Combination of hyperoxygenation and targeted temperature management improves functional outcomes of post cardiac arrest syndrome irrespective of causes of arrest in rats

Abstract:Abstract Background The high mortality rates of patients who are resuscitated from cardiac arrest (CA) are attributed to post cardiac arrest syndrome (PCAS). This study evaluated the effect of hyperoxygenation and targeted temperature management (TTM) on PCAS in rats with different causes of CA. Methods and Results One hundred and sixty-eight Sprague-Dawley rats were equally divided into asphyxial and dysrhythmic groups. Animals were further randomized into four subgroups immediately after resuscitation: 1) Normoxia-normothermia (NO-NT): ventilated with 21% oxygen under normothermia; 2) Hyperoxia-normothermia (HO-NT): ventilated with 100% oxygen for 3 h under normothermia; 3) Normoxia-hypothermia (NO-HT): ventilated with 21% oxygen for 3 h under hypothermia; 4) Hyperoxia-hypothermia (HO-HT): ventilated with 100% oxygen for 3 h under hypothermia. Post resuscitation cardiac dysfunction, neurological recovery, and pathological analysis were assessed. For asphyxial CA, HO-NT and HO-HT (68.8% and 75.0%) had significantly higher survival than NO-NT and NO-HT (31.3% and 31.3%). For dysrhythmic CA, NO-HT and HO-HT (81.3% and 87.5%) had significantly higher survival than NO-NT and HO-NT (44.0% and 50.0%). When all of the rats were considered, the survival rate was much higher in HO-HT (81.3%). Compared with NO-NT (57.7 ± 14.9% and 40.3 ± 7.8%), the collagen volume fraction and the proportion of fluoro-jade B-positive area in HO-HT (14.0 ± 5.7% and 28.0 ± 13.3%) were significantly reduced. Conclusions The beneficial effects of hyperoxygenation and TTM are dependent on the cause of arrest: hyperoxygenation benefits asphyxial whereas TTM benefits dysrhythmic CA. The combination of hyperoxygenation and TTM could effectively improve the functional outcome of PCAS regardless of the cause of CA.

Abstract:Objective To evaluate the effects of hydrogen-rich liquid on the expression of NOD-like receptor thermoprotein domain-associated protein 3（NLRP3） inflammsome in the hippocampus of rats with cerebral injury after cardiac arrest/cardiopulmonary resuscitation（CPR）.Methods A total of 80 healthy male SD rats weighing 280-350 g and aged 8-10 weeks were selected. According to the random number table method, they were divided into three groups: a sham operation（Sham） group（n=20）, a cardiopulmonary resuscitation（I/R） group（n=30） and a hydrogen-rich liquid（H） group（n=30）. A cardiopulmonary resuscitation model of cardiac arrest was established through transesophageal electrical stimulation.Group S was subjected to endotracheal intubation and arteriovenous puncture alone, followed by intraperitoneal injection of 5 ml/kg hydrogen-rich solution immediately after recovery of spontaneous circulation（ROSC） and 6 h after ROSC. The other two groups were intraperitoneally injected with an equal volume of normal saline. Their neurological function score（NDS） was evaluated 48 h after ROSC. The CA1 area and pyramidal cell count were observed by H-E staining. The expression of NLRP3, pro-caspase-1 and apoptosis-associated speck-like protein containinga caspase recruitment domain（ASC） in the hippocampus were detected by Western blot. Results Compared with the Sham group, the I/R and H groups showed decreases in the neurological function score and pyramidal cell count 48 h after ROSC（P＜0.05）, with up-regulated expression of NLRP3, pro-caspase-1 and ASC in the hippocampus（P＜0.05）. Compared with the I/R group, the H group presented increases in neurological function score（P＜0.05） and pyramidal cells count（P＜0.05）, as well as down-regulated expression of NLRP3, pro-caspase-1 and ASC（P＜0.05）. Conclusions Hydrogen-rich fluid can alleviate brain injury in rats with cardiac arrest/cardiopulmonary resuscitation, which may be related to inhibiting the activation of NLRP3 inflammsomeand alleviating inflammatory response.
