Abstract:Skeletal class III malocclusion with severe skeletal disharmonies and arch discrepancies is usually treated via the conventional orthodontic-surgical approach. However, when associated with tooth impaction and periodontal risks, the treatment is more challenging and complex. The esthetic, occlusal, and periodontal stability of the treatment outcome is more difficult to obtain. The 16-year-old female patient in this case was diagnosed with dental and skeletal Class III malocclusion, bilateral impacted maxillary canines, and scalloped thin gingiva. The multidisciplinary management included a segmental arch technique, extracting two premolars, a subepithelial connective tissue graft surgery, and orthognathic surgery. The esthetic facial profile, pleasant smile, appropriate occlusion, and functional treatment results were obtained and maintained in 8-year follow-up.

Abstract:Congenital absence of permanent second molar is an extremely rare condition, which may appear as part of a syndrome or as a non-syndromic form. Non-syndromic second molar agenesis combined with other types of teeth agenesis has occasionally been described in the literature but isolated cases are rarely observed. It might be caused by a number of complex interactions between genetic, epigenetic and environmental factors during the process of dental development. This report presents an isolated case of hypodontia with absence of bilateral maxillary and mandibular permanent second molars in a healthy 25-year-old male patient and a literature review on the prevalence, etiology, and restoration of the missing teeth.

Abstract:In the present case report, a 13-year-old girl patient has bilateral supplementary maxillary lateral incisors, a rare type of supernumerary teeth. During a regular checkup, it was discovered that the patient had these additional teeth, although she had no notable medical or dental history. The patient had no symptoms, even though they may have led to occlusal problems. The tooth's morphology was identical to that of the nearby natural teeth, and radiographic imaging verified full root growth. The example illustrates the importance of early diagnosis and individualized treatment planning, taking into account the patient's decision to keep the extra teeth while attending to the main issue of widespread dental sensitivity. This study emphasizes the necessity for tailored patient treatment and the variations in the management of extra teeth.

Abstract:Multiple supernumerary teeth are rare in non-syndromic patients<a class="xref-bibr" href="#ref-1">1</a> and are mainly impacted.<a class="xref-bibr" href="#ref-2">2</a> A 26-year-old man presented with four supernumerary teeth around the left maxillary second molar. He was referred to our clinic to remove the left maxillary third molar and four supernumerary teeth because of the bite in the buccal mucosa. He had no significant medical history, except for hay fever, or congenital deformities. Two maxillary central impacted supernumerary teeth were removed at 11 years of age. He had no family history of supernumerary teeth, congenital deformity, or colorectal tumours. However, his younger brother had a congenitally missing tooth. In the intraoral examination, four supernumerary teeth around the left maxillary were noted: one supernumerary tooth on the palate side and three supernumerary teeth on the buccal side (<a class="xref-fig" href="#F1">figure 1A</a>). The panoramic radiograph showed no residual deciduous teeth and congenital loss of the permanent teeth. Cone-beam CT showed four supernumerary teeth around the left maxillary second molar, and these teeth were single-rooted and thinly curved (<a class="xref-fig" href="#F1">figure 1B</a>). All supernumerary teeth and the left maxillary third molar were removed under local anaesthesia. All supernumerary teeth showed microdontia with single roots. Each crown form was different. Conical crowns, incisor-like crowns and crowns with 2 and 4 cusps were noted. The length and width of the supernumerary teeth were 9–13 and 3–5 mm, respectively (<a class="xref-fig" href="#F1">figure 1C</a>).

Abstract:Supernumerary teeth are presented in any region of the dental arches as additional teeth to the standard set of teeth and may present in either the primary or secondary dentition. They can be found as single or multiple teeth on one or both sides of the dental arches, with a preference for the premaxilla. The supernumerary teeth might cause aesthetic and/or functional problems, mainly if situated in the maxillary anterior region. Multiple supernumerary teeth are often related to specific conditions or in syndromic patients, i.e., cleft palate, cleft lip, cleidocranial dysplasia, and Gardner's syndrome. This report presents a case of an eight-year-old female patient with non-syndromic bilateral supernumerary teeth in the primary and secondary dentition. The patient is medically fit, and her family history was non-contributory.

Abstract:PURPOSE:This study aimed to describe clinical and imaging analysis of supernumerary teeth in the mandibular region of 22 cases. STUDY DESIGN:Retrospective study. METHODS:This study enrolled patients diagnosed with supernumerary teeth who underwent cone beam computed tomography (CBCT) at Stomatology Hospital of Xi'an Jiaotong University from August 2016 to September 2022. Participants included individuals aged 7-29 years of both genders. The following variables were examined: number of supernumerary teeth, location, shape, direction, length, relationship with adjacent teeth and surrounding anatomical structures, and secondary effects. RESULTS:Out of 236 subjects, 22 (6.02%) were found to have supernumerary teeth in the mandibular region, with a total of 62 teeth identified. The male-to-female ratio was 5:6. Most supernumerary teeth were located in the lingual side of the mandibular region, particularly in the 34-35 tooth area (21.66%), followed by the 44-45 tooth region. The vast majority of supernumerary teeth (96.77%) were impacted, and more than half (51.67%) were located near the mental nerve canal. The average length of the supernumerary teeth was 10.5 mm. While no primary complications were observed, some secondary symptoms were noted, such as the ectopic eruption of adjacent teeth and crowding of permanent teeth. CONCLUSION:The supernumerary teeth in the mandibular area have regional characteristics, which can provide for clinical diagnosis and treatment. CBCT can accurately analyze the location of supernumerary teeth and their secondary effects, and provides the treatment plan on that basis.

Abstract:Abstract The first molar has been reported to be the most caries prone tooth in the permanent dentition. Rarely is the extraction of the four first molars the ideal choice in orthodontic treatment, especially the extraction of four molars and four premolars. However, the extraction program presents a treatment alternative in selected cases. In the following case report, the orthodontic treatment is described a 15-year-old male patient who presented with a dentoskeletal bimaxillary protrusion, and a chief complaint of”crooked teeth”. Moreover, the patient had a skeletal Class I malocclusion with crowded dentitions, a hyperdivergent growth pattern, deviated midlines and an extremely deep Curve of Spee. A satisfactory treatment outcome was obtained by the extraction of four premolars and four compromised first molars. The mechanics involved alignment, levelling, detailing the occlusion using a straight arch wire technique followed by retention. The active treatment time was 48 months. The treatment successfully achieved a bilateral Class I relationship of the canines and the right molars, a mild Class III relationship of the left molars, a normal overjet and overbite, aligned dental midlines, optimal intercuspation and a harmonious facial profile. The occlusion remained stable during 5 years of review.

Abstract:Background: The permanent canine usually has a single root and a single root canal. A one-rooted canine with two canals or a canine with two roots and two separate canals may also occur at a lower incidence in the permanent dentition. However, bilateral symmetrical mandibular canines with two roots and two separate canals are less common. Case presentation: This study reported a lower incidence case of bilateral symmetrical mandibular canines with two roots and two separate canals, which was found based on a CBCT examinaton. The patient visited our department and was consulted for orthodontic treatment due to the irregularity of her lower anterior teeth. As the abnormal root morphology of bilateral mandibular canines greatly increased the difficulty of orthodontic treatment, the patient finally gave up orthodontic treatment after communication. Conclusion: This case report provides supplementary data to better understand the complexities of the root canal system of canines.
Imaging analysis of 1 138 supernumerary teeth by using cone-beam computed tomography

Abstract:Objectives: This study aims to analyze and summarize the characteristics of supernumerary teeth by using cone-beam computed tomography (CBCT). Methods: A total of 718 patients with 1 138 supernumerary teeth were retrospectively collected. Age, gender, number, location, morphology, eruption status, and accompanying symptoms of the supernumerary teeth were statistically analyzed. The relationship relative to jaws, gender, and eruption status were analyzed and discussed. Results: The average age of the patients was 9.54±5.32 years, and the male to female ratio was 2.88∶1. About 77.02% of the patients sought medical advice during the mixed dentition period, and 50.70% had one supernumerary tooth. These supernumeraries were most commonly conical in shape, and 85.76% of them were in the incisor region, 92.09% in the upper jaw, 46.75% in inverted position, and 86.20% unerupted. Overall, 65.29% of them had fully developed roots, and 60.63% had an impact on adjacent structures. Significant differences were found in eruption status, morphology, zoning, direction, root development, and impact on adjacent structures between the supernumerary teeth located in the upper and lower jaws (P<0.05). Significant differences were also detected in gender, morphology, zoning, orientation, root development, and impact on adjacent structures between erupted and unerupted teeth (P<0.05). The incidence of supernumerary teeth in the incisor region was higher in males than that in females. Moreover, the root of supernumeraries was more completely developed in males than in females (P<0.05). Conclusions: For supernumerary teeth, CBCT images can provide accurate three-dimensional radiographic data and are valuable for clinical diagnosis and treatment planning.

Abstract:AIM:The aims of this report are to present a case of nonsyndromic multiple supernumerary teeth and a discussion of the value of three-dimensional computerized tomography (3D CT) for precise radiographic imaging of the anomaly. BACKGROUND:Multiple supernumerary teeth without any associated syndromes are very rare. Exact radiographic presentations with an associated diagnosis of nonsyndromic multiple supernumerary teeth have not been well published. REPORT:A young Chinese female patient presented with asymmetrical nonsyndromic multiple supernumerary teeth in the premolar and molar regions of her mouth. A 3D CT was used to evaluate the exact location and orientation of these teeth and to guide the treatment strategy. SUMMARY:The use of 3D CT for the evaluation of supernumerary teeth proved to be very useful in determining their size, location, and state of development, as well as their impact on adjacent teeth and tissues in a 15-year-old patient. CLINICAL SIGNIFICANCE:The 3D CT is a very useful radiological tool to assess nonsyndromic multiple supernumerary teeth.

Abstract:Abstract Hyperdontia involving the premolar teeth can be single or multiple or even related to a syndrome. These are frequently encountered in permanent dentition, but their occurrence has ranged between 0.09 and 0.64% in the general population. These are commonly found between the permanent canines and first premolars but rarely between the first and second premolars. These are usually eumorphic and can be easily identified as they follow the conventional tooth morphology. These are usually asymptomatic and not easily palpable unless they are very close to eruption. A lingual approach is preferred during their removal over the buccal method as it poses less risk of root trauma. In this case, bilateral accessory premolar tooth buds were present between the first and second premolars. Since the patient was undergoing orthodontic treatment, she was advised to remove these accessory teeth. For their removal, a Neumann flap with a buccal approach was used, which led to fracturing the root apex of the mandibular right second premolar root. On follow-up over the next 1 year, no other complications or secondary tooth germs were observed, and following a period of bone formation, the bone in the affected area formed normally again.
Mandibular Molar with Six Canals, a Rare Entity: A Case Report

Abstract:Background: The objective of this report was to highlight the importance of using a dental operating microscope (DOM) to locate supernumerary canals and diagnose variations in root canals using cone-beam computed tomographic (CBCT) images. Case presentation: A 23-year-old male patient had reported with a pain for 1 months and was referred to evaluate symptomatic apical periodontitis for lower right first permanent molar. Root canal therapy was proposed and conducted with the use of DOM and CBCT. Conclusions: Proper diagnosis and careful clinico-radiological examination are necessary, and it is essential to reinforce the knowledge of the rare morphology of root canals for clinicians.
Three-dimensional Evaluations of Supernumerary Teeth Using Cone-Beam Computed Tomography for 487 Cases

Abstract:Purpose. The purpose of this article is to introduce the use of cone-beam computed tomography (CBCT) for evaluation of supernumerary teeth.Methods. The study group comprised 487 patients with a total of 626 supernumerary teeth who were examined by CBCT. Patient characteristics were recorded, and the number, location, shape, and 3-dimensional position of the supernumeraries were analyzed. The ability of CBCT to visualize dental and skeletal structures relative to supernumerary teeth was also evaluated.Results. Males were affected more than females in a ratio of 2.64:1. Seventy-two percent of the patients had I supernumerary tooth, 27.3% had 2, and 0.6% had 3 supernumeraries. Supernumerary teeth were most frequently located in the anterior maxilla (92%), and their sagittal location relative to the neighboring teeth could be used for classification purposes. Supernumeraries were most commonly conical in shape (83.51%). CBCT yielded accurate 3-dimensional pictures of the dental and bony structures. The sagittal positions of the 578 supernumerary teeth in the anterior maxilla were divided into 6 types, of which types I and III were most commonly associated with local malocclusions. Moreover, 43.4% of the premaxillary supernumeraries were inverted and 21.1% were transversely oriented.Conclusions. CBCT imaging yields accurate 3-dimensional pictures of local dental and bony structures, which is helpful for pretreatment evaluation of supernumerary teeth.
Fusion of a maxillary third molar with a supernumerary fourth molar: A case report

Abstract:Key clinical message: Dental fusion should be included in differential diagnosis when panoramic radiograph reveals changes in tooth shape or size. The use of specialized dental photographic techniques can augment the dentists' knowledge and awareness of such conditions. Abstract: Dental fusion of impacted teeth may show up as a change in tooth shape and size on the first radiographic examination. This report presents an impacted maxillary third molar fused with a peg-like distomolar in a 20-year-old male. The patient presented with symptoms of localized periodontitis distal to the second molar, and radiographic examination revealed abnormal dental structure of the third molar. Surgical extraction of the impacted molar was the treatment of choice. The final diagnosis of fusion was based on data from ex vivo CBCT, photographs of the extracted tooth, and extracted tooth's sections using polarizing filters. Two-dimensional radiographs may conceal special dental anatomies. Visualization of such cases using CBCT and dental photography serves to educate dentists and reduce postoperative complications. Knowledge and awareness of possible dental abnormalities are of utmost importance for successful treatment planning.
The unusual anatomy of the maxillary and mandibular molars diagnosed by cone-beam computed tomography

Abstract:Background: Knowledge of variations in dental anatomy and canal morphology plays a key role in the success of endodontic therapy. Case presentation: This report presents a patient who had unusual tooth morphology, including the bilateral existence of 11 maxillary and mandibular first, second and third molars with a single root and a single canal. The patient was diagnosed using cone-beam computed tomography (CBCT). To our knowledge, this is the first report of such case in the literature. Endodontic treatment of the left maxillary second molar was performed. CBCT images confirmed our diagnosis and the unusual morphology of the 11 molars. Conclusions: This report highlights that CBCT has a significant role in confirming the anatomy of teeth, which provided an opportunity to precisely describe the anatomy of 11 maxillary and mandibular molars with single roots and single canals.
Double Mesiodens in the Mixed Dentition of Non-syndromic North-Indian Patients: A Case Series

Abstract:The presence of double mesiodens or mesiodentes, i.e., two supernumerary teeth in the maxillary midline, presents unique challenges in mixed dentition. Common clinical manifestations include delayed eruption, midline diastema, and occlusal disturbances, leading to complications such as root resorption, pathological migration of tooth, crowding, cyst formation, and malocclusion. Mesiodens can be associated with several syndromes, like cleidocranial dysplasia, familial adenomatous polyposis, trichorhinophalangeal syndrome, type I, Rubinstein-Taybi syndrome, and Nance-Horan syndrome, among others. It can also be secondary to trauma, hyperactivity of the dental lamina, and a combination of genetic and environmental factors, but its etiology continues to be idiopathic. Double mesiodens are relatively rare, so this clinical observation aimed to highlight five such cases of double mesiodens in mixed dentition in non-syndromic children and adolescents. Additionally, a literature search reporting cases of double mesiodens in the mixed dentition was done, and the results were tabulated. Clinicians should be able to identify indications of supernumerary teeth, specifically deviations in the eruption pattern. Appropriate investigations and timely intervention are essential to reducing complications that may arise in the developing dentition.

CBCT scan coupled with 3-dimentional reconstruction was used to characterize the detailed location, morphology, orientation of ST, and their relationship with adjacent teeth and neighboring structures. All relevant information regarding age and gender of patients, morphology, and 3-dimentional topography of ST as well as ST-associated complications were recorded and statistically analyzed.RESULTS:A total number of 1149 ST was identified in 921 eligible patients screened from 60,104 subjects with the prevalence of 1.5%. Male patients outnumbered females with a gender ratio of 1.76:1. The majority of ST was single, located in the maxilla, especially the maxillary central incisor region. Most ST were conical shape, inverted orientation, and impacted. ST-associated complications including impaction or root resorption of adjacent teeth, and cystic/tumor-like lesions were totally found in 13% ST and significantly associated with location, orientation, and morphology of ST.CONCLUSIONS:Most ST in Chinese adults were conical, inverted, impacted, and located in the maxillary central incisor region, and associated with various complications. Our findings offer valuable information concerning the prevalence, clinical, and radiographic characteristics of ST in non-syndromic Chinese adults.CLINICAL RELEVANCE:These findings are beneficial for clinicians to comprehensively understand the incidence, pathogenesis, and clinical management of ST.

Abstract:Supernumerary teeth can have normal or abnormal morphologic structure and characteristics, and their impacted form is diagnosed usually during X-ray examinations. In this case report, the presented patient is a 16-year-old female with anterior and right posterior open bite and bilateral posterior crossbite, upper right supernumerary paramolar, and impacted second and third molars. The paramolar development was the reason for the asymmetric growth of the alveolar bone in the upper jaw. The development of the bone is connected with the development of the teeth, and one additional tooth leads to extensive development in the maxilla. There is a risk of gingival recession occurrence when leveling the lower incisors due to the thin gingival biotype. A combined surgical-orthodontic treatment was done according to the following plan: extraction of supernumerary paramolar, germectomy of the upper right third molar (18) and at the same time periodontal graft in the lower anterior segment. A digitally three-dimensional (3D) printed appliance for rapid maxillary expansion was used for the transverse insufficiency of the upper jaw. The upper dental arch expander was designed with distal extension in the area of the upper right second molar (17). The extension was used as an anchorage during the orthodontic traction of the second molar. The treatment continued with a fixed orthodontic appliance-braces in the upper and lower jaw. With the extraction of the impacted and supernumerary teeth in the right maxillary segment, the eruption of 17 was stimulated and a change in the height of the alveolar bone was achieved. This favored the vertical changes and normalization of the occlusion. The maxillary expansion was also a significant factor in normalizing the occlusion. Observations on paramolar behavior showed that more often they develop in the bone and do not erupt. Each clinical case is highly individual, and patients seek orthodontic treatment at different stages of dentition development and corresponding development of the supernumerary teeth.
Post-surgical complications of supernumerary teeth in the mandibular premolar area: A systematic review

Abstract:In the mandibular premolar area, supernumerary teeth are a rare disorder that are rarely documented in the literature. The purpose of this study was to evaluate the post-surgical problems associated with the buccal and lingual techniques used to treat the extra teeth in this area. Using PubMed, PubMed Central, Science Direct, Cochrane, Wiley, and manual search, a thorough search was done to locate and identify cases and case series with extra teeth in the mandibular premolar region. For every study that was included, data on the surgical technique and postoperative problems were documented. Seven studies involving 90 patients were included. Supernumerary teeth in the mandibular premolar region were predominantly found in male patients (65.55 %), with most cases occurring in the right mandibular region (77.16 %). Postoperative complications after removing supernumerary teeth using buccal and lingual approaches were similar, with only one complication reported among the 90 cases reviewed. Although the findings suggest that surgical management of supernumerary teeth in the mandibular premolar region is relatively safe, caution is advised due to the limited sample size. Detailed pre-operative evaluation, a comprehensive treatment plan, and regular follow-up can minimize postoperative complications.
