Safety of Sputum Induction in Patients with Severe Persistent Asthma

Abstract:Objective To evaluate the safety of sputum induction by hypertonic saline inhalation in(severe) persistent asthmatic patients.Methods 37 patients with persistent asthma were included in the study,of whom 21 were severe and 16 were mild to moderate asthmatics.Sputum induction was performed with ultrasonic inhalation of 4% hypertonic saline according to standard protocol,but no pretreatment with β_2 agonist was administrated.Forced expiratory volume in one second(FEV_1) and SaO_2 were monitored before and after the procedure.Adverse events during the procedure were recorded.Results In patients with severe asthma,the percent predicted of FEV_1(FEV_1%pred) were(60.8±15.8)% and(53.6±15.9)% before and after sputum induction,respectively,which were different significantly(P0.01).SaO_2 were(96.0±1.2)% and(93.5±1.9)%,respectively before and after sputum induction with no significant difference.In patients with mild to moderate(asthma),the FEV_1%pred were(83.3±12.1)% and(72.5±15.0)%,respectively before and after sputum induction(P0.01).SaO_2 were (98.0±1.0)% and(96.3±2.5)%,respectively before and after sputum induction(P0.05).The percent changes of FEV_1%pred and SaO_2 were not significantly different between severe and mild-moderate asthmatics(P0.05).Notable dyspnea was observed in 5 cases(23.8%)of severe asthmatics,all with a decrease of FEV_1 more than 15% but the symptom was relieved by inhalation of a β_2 agonist.Mild chest tightness,which resolved spontaneously,was present in 6 patients(37.5%)with mildmoderate asthma after sputum induction.Mild nausea,mouth dryness and sore throat were common but well tolerant.All of the patients completed the procedure and adequate samples were obtained from 34 patients(91.9%).Conclusion Sputum induction can be safely and successfully performed in patients with severe persistent asthma.
