Abstract:Cutis laxa is a rare connective tissue disorder, characterized by a reduced number and abnormal properties of elastic fibers throughout the dermis, creating a clinical appearance of premature aging. It can be subdivided into inherited and acquired, the latter rarer than the former, and skin involvement may be localized or generalized. The etiology of acquired cutis laxa (ACL) remains unknown and there is no definitive treatment. We present the case of a 30-year-old man diagnosed with type I ACL with progressive systemic involvement at the renal, pulmonary, and digestive levels. Histological analysis of the skin revealed reduction and fragmentation of elastic fibers. Immunosuppressive treatment was started with prednisone, cyclophosphamide, and rituximab, with which a complete response to proteinuria was achieved and the progression of lung damage was limited. Autoimmune, infectious, and neoplastic diseases were ruled out.

Abstract:Acquired cutis laxa (ACL) is a rare, nonhereditary cutaneous disorder characterized by saggy inelastic skin. It has been associated with various inflammatory, autoimmune, and neoplastic diseases, in addition to certain infections and medication. This article reviews ACL the demographical, clinical, and histological features of ACL, focusing on all associated disorders. Additionally, this review article provides an in‐depth discussion of all the mechanisms implicated in the pathogenesis of ACL and all therapeutic options available; we also present an algorithm for the workup of patients with ACL. A systematic literature review was performed on PubMed/Medline and EMBASE databases, searching for all available articles on ACL with no limits on participant age, race, sex, nationality, or publication date. Ninety‐eight articles were included. The total number of included patients was 110, with a mean age of 36.4 years at presentation (range 0.25–78) and a M:F sex ratio of 1.24. ACL was most commonly associated with inflammatory disorders (43%) followed by neoplastic disorders (27%). In 73% of the neoplastic‐associated cases, ACL occurred on average 2.4 years before malignancy onset. ACL occurs months to years after an underlying inflammatory disorder. In 10% of the cases, ACL was associated with a particular drug, and in 2%, it was associated with specific infections. Data were derived from case reports, case series, letters to editors, observational studies, and abstracts. Limitations include the accuracy of published data, potential patient selection, and reporting bias. Dermatologists should be alert to these associations to provide adequate screening and management of patients with ACL.

Abstract:Rationale: Calcinosis cutis is a rare skin disease, and idiopathic cases are rarely reported. It is characterized by the deposition of insoluble calcium salts in the skin, subcutaneous tissue, superficial muscles, and tendon sheaths. However, no abnormal changes were found in the bone. In this article, we introduce a case of idiopathic calcinosis cutis of the buttocks with a long course and large lesion area. Patient concerns: A 51-year-old male patient was admitted to the hospital with a chief complaint of 'Due to the discovery of hard nodules with pruritus in the buttocks for 32 years. The patient was a male who was 51 years old. He has been in good health and reported no history of surgery, trauma, infection, metabolic disease, tumor, or other diseases. There was no family history. It is worth noting that the patient has the occupation of driving trucks, which keeps him sedentary. Diagnoses: The accurate diagnosis of calcinosis cutis was confirmed by postoperative histopathological examination with many local calcifications and multinucleated giant cells in subcutaneous tissue. Interventions: The patient underwent skin lesion excision and autologous skin grafting under general anesthesia. A medium-thickness skin graft from the left lateral thigh was transplanted into the hip operation area, and a bolus tie-over pressure dressing was applied. After the operation, the patient received anti-infection treatment and was advised to rest in the prone position to prevent extrusion of the operation area. Outcomes: The postoperative recovery was good, and there was no recurrence after 4 months of follow-up. Lessons: The incidence rate of cutaneous calcinosis is not clear. This patient had a large lesion area, long onset time, an invasion of the fat layer, and the onset site was located in the sacrococcygeal region. It is necessary to choose appropriate treatment methods.

Abstract:Journal of International Medical Research, Volume 52, Issue 10, October 2024. Lipomas are the most prevalent benign soft-tissue tumours, however, pedunculated fibrolipoma in the buttock is rare. Here, the case of a Chinese male patient in his early 70s with a 50-year history of a growing pedunculated fibrolipoma on his right buttock is presented. The attending physician and the registered nurse collaborated on a treatment plan, leading to a successful resection with rhomboid flap transfer. The operation went smoothly, and with continued home care and follow-ups, the patient recovered well, without complications. At 4 months post-surgery, the incision had healed perfectly, boosting the patient's psychological state and allowing him to resume his daily activities unhindered.

Abstract:Introduction: Post-liposuction fibrosis is a relatively common complication which can be repaired. Case report: We report a case of cutaneous atrophy and diffuse irregularity of the abdominal region with achromia post-liposuction. She was treated with subcision, PMMA filler and fractioned CO2 laser. Results: Cutaneous colour and irregularities get improved.

Abstract:Background: The application of autologous fat transplantation in facial lesions of patients with localized scleroderma (LoS) has been reported in recent years. Objective: The authors report a case of worsening of active localized scleroderma after autologous fat transplantation. Methods: A man presented with neck and facial skin atrophy and pigmentation with a history of LoS. Appearing 1.5 years ago, the lesion had progressively grown in size and shape. Consent was obtained after the patient was informed of the possible surgical risks during the active phase of the disease. He underwent autologous fat grafting into the right cheek with about 30 ml Coleman fat graft. Results: Skin dyspigmentation and atrophy progressively deteriorated 1 month into therapy, with slightly increased erythema and enlargement of the lesion. Six months after the therapy, the localized scleroderma-related score worsened. Limitations: There are different factors, such as that systemic medications could affect the treatment of localized scleroderma by autologous fat transplantation. Meanwhile, considering the limitation of the 6-month follow-up period, obtaining long-term follow-up data is necessary to evaluate sustained outcomes and potential complications. Conclusion: More clinical research is needed to determine the time interval between disease inactivity and the application of any surgical procedures to avoid reactivation.

Abstract:The patient, a 54-year-old female, presented with papular nodules on the waist, buttocks, and bilateral thighs accompanied by intense itching for over 6 months. Her medical history included 30 years of hypertension, 20 years of coronary heart disease, 4 years of diabetes, uterine fibroid removal, and allergies to penicillin and phosphorus drugs. Cutaneous examination revealed scattered papules and nodules on the waist, buttocks, and bilateral thighs with central umbilication necrosis or keratin plug, along with scratch marks and ulcers, accompanied by Koebner phenomenon (Fig. 1A).

Abstract:Naevus lipomatosus cutaneous superficialis (NLCS) is a rare idiopathic benign hamartomatous skin tumour characterized by mature ectopic dermal lipocytes with no associated systemic symptoms. It was first described by Hoffman and Zurhelle in 1921 [1] and is now classified into 2 clinical subtypes: classical (multiple) and solitary. The classical form usually presents in adolescence and consists of grouped, fleshy, skin-coloured to yellow papules and nodules that coalesce into plaques and are often located on the lower trunk, buttocks, or thighs in a segmental distribution. The second variant presents in adulthood as a solitary papule or nodule anywhere on the skin, including unusual sites such as the sole [2], nose [3], and vulva [4]. Here, we report a rare case of classical giant NLCS on the right buttock with successful treatment by surgical excision. To date, reported cases of NLCS with positive treatment outcomes, as in the case presented herein, are rare. A 19-year-old girl presented with a giant (17 × 25 cm in size), flesh-coloured, soft, nontender, rubbery, sessile, and cerebriform mass on the right buttock, which was attached with multiple comedones on the surface (Figure 1 A). The mass appeared 10 years prior and gradually increased in size. When she visited our hospital, the mass was so large that her appearance and movement had been obviously influenced. There were no symptoms due to the lesions. The patient was otherwise healthy, and there was no relevant family history. No therapies had been attempted previously. Serum triglyceride and cholesterol levels were normal. The lesions were completely removed by excisional surgery and the treatment site was covered with local advanced flaps. Histological examination of a diagnostic biopsy found normal epidermis and mature fat cell deposits in the superficial dermis (Figure 1 B). Based on the clinical and histopathological features, the diagnosis of naevus lipomatosus cutaneous superficialis (NLCS) was made.

Abstract:Introduction: Lichen sclerosus is a chronic inflammatory dermatological condition of unknown etiology, primarily impacting the genital epidermis in individuals of all genders, with a higher prevalence observed among postmenopausal women and prepubescent girls. Additionally, extragenital manifestations occur in approximately 20% of the patients diagnosed with genital lichen sclerosus. Notably, folliculocentric extragenital lichen sclerosus is rare and unusual, with only limited instances documented in existing literature. Case description: We report a 33 years old lady presented with multiple asymptomatic lesions on the dorsal feet for 1 year and similar lesions on the left hand for 4 months. On examination: folliculocentric, shiny, atrophic papules coalescing into reticulated plaques over the dorsum of both feet and few shiny, flat-topped, pink papules over the dorsum of the left hand. A skin biopsy was performed and confirmed the diagnosis of extragenital lichen sclerosus. Conclusion: Acral folliculocentric extragenital lichen sclerosus is an unusual and rare clinical variant. Clinicopathologic correlation is necessary to establish the correct diagnosis. Contribution to the literature: Herein, we present an unusual presentation of extragenital lichen sclerosus, and we highlight the importance of considering it in the differential diagnosis of guttate acral skin lesions. We also review and summarize relevant cases from the literature in hope to aid physicians, especially dermatologists, to consider and swiftly reach the diagnosis and offer appropriate management. We also hope to bring about new insights and broaden future research efforts regarding lichen sclerosus especially and atrophic skin disease in general.

Abstract:A case of acral persistent papular mucinosis is reported. A 64-year-old male presented with generalized papules on the surfaces of the forearms. Histopathological test showed that a collection of Alcian blue positive stainning mucin in the upper and middle dermis with edemas of superficial dermal, increase of fibroblasts and deposits of masses of mucins.

Abstract:Objective To report a case of granulomatous slack skin which is an extremely uncommon variant of mycosis fungoides. Methods The clinical, histopathologic, immunologic, ultrastructural and molecular biologic changes of the disease were evaluated. Results Conclusion To report a case of granulomatous slack skin which is an extremely uncommon variant of mycosis fungoides.Methods The clinical, histopathologic, immunologic, ultrastructural and molecular biologic changes of the disease were evaluated.Results A massive dermal infiltration composed of Iymphocytes intermingled with numerous histiocytes and multinucleated giant cells was found.A few Iymphocytes showed mild epidermotropism.There was almost complete loss of elastic fibers.The lymphocytes were of T-lineage (CD3+, CD45RO+).The rearrangement of the T-cell-receptor beta chain was identified hy molecular hiologic study.Ultrastructurally, there were macrophages with numerous surface villous processes and lysosomes.Conclusion This is the first case repori of granulomatous slack skin in China and the defined raised border of skin lesions has been emphasized. Abstract:Familial partial lipodystrophy (FPLD) is a rare disease, presenting as localized areas of lipodystrophy in the body and associated with defined genes mutations. Labia majora hypertrophy is rarely reported in the literature, and it has been described only once associated with the FLPD syndrome. We present a rare case of labia majora lipodystrophy successfully corrected with a pubo-pexy and a labial reduction plasty.

Abstract:A case of porokeratosis presented as eruptive pruritic papular lesions is reported. The patient was a 50-year-old man. He complained of pruritic and hyperkeratotic papules distributed over his buttocks for 7 years. The lesions aggravated 1 year ago, and the lower limbs were involved,with severe pruritus especially when sweating. Histopathological examination revealed the characteristic comoid lamellae. He was treated with topical keratolytics and oral acitretin, but the lesions showed no evident improvement.
A Case of Eruptive Collagenoma of Skin

Abstract:A case of eruptive collagenoma is reported. A 38-year-old man, who had first noticed asymptomatic nodules and plaques from a few mm to cm in diameter on his back and shoulders lasting for 8 years. These lesions were scattered and multiple, some forming confluent plaques. Histologically collagen fibers in the dermis were coarse and increased while elastic fibers decreased. Based on these findings the patient was diagnosed as eruptive collagenoma of skin.

Abstract:Porokeratosis, a keratinizing disorder of unknown etiology, exhibits an autosomal dominant inheritance pattern or manifests as an isolated acquired dermatosis. This condition can occur at any site on the skin; however, scrotal lesions are extremely rare. Only 18 cases of scrotal lesions were identified through a comprehensive review of the relevant literature. Herein, we present a case of a 19-year-old patient with porokeratosis of the scrotum. Additionally, we provide a summary of the etiologies, clinical manifestations, and histopathology of scrotal porokeratosis, and present differential diagnoses by reviewing the related literature.

Abstract:Aplasia cutis congenita (ACC) is a congenital disorder that can be classified into nine types, with Type I ACC being the most common. Type V ACC associated with fetus papyraceus is a rare subtype of ACC. We report the case of a Type V ACC in a male newborn with extensive abdominal skin defects. The patient received conservative treatment using hydrogel foam and silicone foam dressings. Approximately five weeks later, the patient was discharged when more than 60% of the skin had completed epithelialization. After discharge from West China Second University Hospital, Chengdu , the patient continued to be followed up regularly at the Burns and Plastic Surgery Clinic at local hospital in Gansu. We followed up the child by telephone. After 4 months of follow-up, scar tissue formation was observed in the trunk area. The infant is 2 years and 5 months old now, physical examination did not reveal any organ problems.
